The West Australian Government hold an annual ‘Mines Safety Roadshow’ around the State’s regions and the capital city, Perth. Each year presenters provide an update on legislation and safetyrelated themes. A recurring theme is the mining industry fatality rate. Professor Michael Quinlan’s ‘Ten Pathways to Death and Disaster’ (Quinlan, 2014) has been referenced regularly to assist in structuring workshops and conversations concerning fatality prevention in the mining industry. The ‘Ten Pathways’ are the outcome of an analysis of fatal injuries in the mining industry, which identified a series of common themes to the fatalities: Pathway 1: Design, engineering, technical and maintenance flaws. Pathway 2: Prior warnings or causes for alarm ignored. Pathway 3: Failures in risk assessment. Pathway 4: Failures in management systems and hazard management plans. Pathway 5: Failures in auditing. Pathway 6: Economic pressures compromising safety. Pathway 7: Failures in regulatory oversight and inspection. Pathway 8: Worker and others expressing concern prior to the incident. Pathway 9: Poor management – worker communication and trust. Pathway 10: Emergency and rescue resources and procedures. To determine a fatality risk profile we asked participants at the Mines Safety Roadshows to indicate their perception of their employer’s performance in each of the Ten Pathways. We administered the questionnaire at the Mines Safety Roadshows in 2017, 2018, 2019 and 2023. In this paper, we report on the results of the questionnaires, and suggest how organisations can improve their fatality risk score.

This paper presents the results of studies on the development of an efficient non-cyanide technology for processing massive, finely disseminated, copper-zinc ores of the Urals deposits. Copper is mainly represented by chalcopyrite and zinc û by sphalerite. The main ore mineral is pyrite, with its content in ores varying from 50 to 80 per cent. At the present time, separation of copper concentrate at the Urals concentrating plants is achieved in a lime medium by non-cyanide technology using sodium sulfide and zinc sulfite as depressors of zinc and iron sulfides and butyl xanthate as the collector. Copper content in copper concentrates amounts to 15 - 17 per cent; considerable losses of zinc are associated with copper concentrate and final tailings. Various collectors were tested during the studies: xanthates and aerofloats with different lengths of hydrocarbon radicals, thionocarbamates, Aerofine 3418A of Cytec Industries, reagent S-703G of Mineral Reagents Int, and modifiers û sulfur dioxide, sodium sulfide and zinc sulfite, with a pH of the medium varying from 5 to 12. Optimum results of copper concentrate separation were achieved in a weakly acidic medium using S-703G as the collector and sulfur dioxide and zinc sulfite as modifiers. The copper content in the copper concentrate was increased by 4.8 per cent. Total copper recovery into the copper concentrate and zinc recovery into the zinc concentrate exceeded five per cent.

A summary of the more important work by previous workers is followed by brief descriptions of the rock formations and regional structure. Type examples of the various forms of mineralization are described and the conclusion is drawn that there has been one main period of ore formation, namely, during the "Viti Revolution" in the mid or late Miocene.INTRODUCTIONThe Crown Colony of Fiji is comprised of a group of some 250 islands situated between latitude 160 sand 21 0 S and longitudes 1770 E and 1780 w: By far the greater, part of the total area, 7,083 sq. miles, is contained within the two main islands, Viti Levu 4,053 sq. miles, and Vanua Levu 2,128 sq. miles, and it is to these that most geologists have directed their studies. The most detailed work has been in the Tavua Basin, north coast of Viti Levu, centering about the mining town of Vatukoula and the geology of this area forms the subject of a separate paper by Blatchford (this volume p. 13).The present contribution summarizes geological knowledge of Fiji, at the time of writing (Feb. 1952), with particular reference to the known mineralIzation, but excluding the Tavua Basin. The search for economic mineral deposits is becoming more difficult as exploration goes deeper in the established mining camps and spread out to the surrounding covered areas, which may have an `overburden' some hundreds of metres thick. Geophysics is finding an ever increasing role in such exploration. When the target is zinc, the task often becomes even more difficult since sphalerite, the most commonly mined mineral, is non-magnetic, resistive, and relatively light compared to other sulphides. Fortunately, sphalerite commonly occurs in association with other sulphides, which are magnetic and/or conductive and/or heavy, thus most zinc-rich orebodies have at least one contrasting physical property to aid exploration. This paper presents the physical properties of sphalerite and other potentially important zinc minerals. Results of petrophysical measurements of samples from various zinc-rich ores are presented together with examples of geophysical surveys over a number of zinc deposits. The results show that the physical properties are not merely dependent upon the properties of the orebodies' constituent minerals. For example, the small (0.4 Mt) and low-grade (five per cent Zn + three per cent Pb + two per cent other sulphides) Flying Doctor deposit at Broken Hill, New South Wales (NSW) is much more conductive than the large (3+ Mt) and high grade (14 per cent Zn + five per cent Pb) Cadjebut deposit in Western Australia (WA). Shizuyuan deposit is a compound type of greisen and skarn associated' with W,mo,Sn,and BI mineralization. Tungsten-bearing skarns are located at the contacts between carbonate rocks and granitic stock. Ore-bearing greisens occur in the top of granite or superimpose on the skarn rocks. Although mineralization and alteration are heterogeneous,there are clear patterns in the preferred associations of minerals on all scales, and an orderly sequence can be distinguished. Skarn formation is divided into an early sk.arnoid stage and a late metasomatic stage. The skarnoid stage formed grandite garnet and diopside pyroxene in the limestone. The iron content of garnets decreases significantly away from the granite, whereas pyroxenes maintain a relatively constant diopsitic composition. The metasomatic skarn stage is charac- terized by hydrous silicates. The mineral assemblages of. greisens are regularly distributed in space. The feldspar-quartz-fluoride was usually formed in-the deeper part, upwards giving way to muscovite-quartz, and topaz usually occurs in the uppermost part. The distribution, and miner- alogical relationships of alteration facies suggest that they formed as a result of the progressive reaction of an ascending hydrothermal solution with aluminum-nearing limestones an-d granitic rocks respectively. The ore elements were mobilized and deposited in association. with hydrosilicate alteration of skarns and greisens.

A short overview of the research program of SveBeFo, Swedish Rock Engineering Research, which was founded in 1993 is first given. Then four recent and ongoing projects in rock blasting and explosives engineering are presented in greater detail: 1. Burn Equations for Commercial Explosives. 2. Blast Damage in Contour Blasting. 3. The Role of Gas Pressure in Pit Wall Damage. 4. Fragmentation of Aggregate Rock. The goal of the bum equation project is to be able to control the energy release from explosive charges to the desired part of the expansion curve of the reaction products under various rock conditions. SveBeFo's work has reached the stage where a burn equation for a lab emulsion has been established which can model the influence of charge diameter on the VOD and detonation front curvature in a cylindrical charge quite well. This work now continues with other emulsion recipes. In the blast damage project the goal is to develop techniques and recommendations on how to minimise the blast damage in construction blasting. The project has gained extensive practical knowledge from the blasting of more than 200 holes in a granite quarry under controlled conditions. The cracks in the rock surrounding the contour holes have been cut open and measured. The influence of factors like charge concentration, decoupling ratio, hole and charge size, VOD and time delay in initiation have been investigated. A simultaneous initiation of a group of contour holes with EDD detonators is quite superior to single hole blasts, even when they are timed as closely as 1 ms.

An account of milling "methods and costs at Mount Isa Mines Limited up to the year 1938 has been published. The present paper outlines the methods since adopted in the milling of lead-zinc ores and presents a description of the equipment now being provided for the milling of copper ore.The programme approved for the mining, milling and smelting of both lead-zinc and copper ores involves the milling 9f 50,000 tons of lead-:-zinc ore and 35,000 tons of copper ore per month.The crushed ores will be delivered to the surface from alternate levels through No. 2 Ore shaft and conveyed to their respective crude ore bins at the crushing plant. The ores will be crushed and milled in separate circuits in common buildings and the tailings will be stored on a common tailings dump. The water services to the two circuits will be separate.LEAD ZINC PLANTCrushing PlantThe crushing plant described in the previous paper was located adjacent to the Urquhart shaft. The crushing plant had grown considerably over the years by the addition of a 4ft. BY-mons shorthead crusher and a set of Traylor 72 in. x 20 in. crushing rolls and, whilst operating satisfactorily, was difficult to maintain and cramped in many places. Scale build-up in titanium palladium alloy spent electrolyte heat exchangers in the residue treatment plant commissioned at Risdon, Tasmania in 1971 was shown to be a mixture of manganese dioxide and anhydrite (CaSO4). The manganese dioxide was deposited as a result of thermal decomposition of permanganate ions present in the spent electrolyte. It was shown that deposition of manganese dioxide and associated anhydrite scale could be prevented by prior reduction of permanganate ions to the manganous (Mn++) state by the addition of ferrous ions. In the plant this was conveniently accomplished by injecting into the inlet line to the heat exchangers sufficient zinc plant solution containing ferrous sulfate to decolourise the spent electrolyte. This action prevented deposition of such scale for another 21 years, until the heaters were finally decommissioned in March 1992. Measurements indicated that addition of the reducing solution lowered the anodic potential of the titanium alloy by 200 - 300 mV (depending on the solution resistance), but that it was still +300 to 400 mV with respect to a standard calomel electrode. It was shown that, if necessary, a small impressed current could be applied to provide anodic protection. Before scale deposition was overcome by the addition of ferrous sulfate solution, badly scaled heat exchangers were cleaned by recirculating a solution of hydrogen peroxide in spent electrolyte or dilute aqueous sulfuric acid, after laboratory testing of a variety of possible solvents for the scale. Corrosion tests indicated that attack on the titanium alloy by the acidic peroxide cleaning solution was insignificant.

The removal of sulfur from SO2 containing flue gas is improved by adsorbing SO2 into solution with manganese ions as catalyser, however, burnt lime slurry has to be added to modify solution pH and the CaSO3 by-product is of low economic value. In this paper a novel one step process for simultaneous manganese leaching-desulfurisation of flue gas is presented. An investigation of the effect of process parameters included flow rate and SO2 concentration in flue gas, adsorbing time, solution to solid mass ratio, leaching time, and their impact on the oxidation and removal of SO2 from flue gas. The catalysing mechanism of manganese ions on the oxidation and SO2 removal was demonstrated by calculation of the species originating from manganese ions and SO2 in the solution according to solution chemistry. SO2 removal in the adsorbing solution took place at pH 5 - 6, where manganese ions dominantly occurred as Mn2+, and the chain reactions for oxidation of SO2 starting from HSO3- were improved by the presence of Mn2+. SO2 bearing flue gas was effectively desulfurised by leaching rhodochrosite (MnCO3), with desulfurisation of the flue gas and modifying the solution pH being achieved simultaneously, and MnSO4 of high economic value recovered from the leachate. 85 per cent manganese leaching and over 70 per cent desulfurisation of the SO2-bearing flue gas were achieved with only one adsorption stage under optimum conditions.

FOR some time it has been recognised by the Metallurgical Staff that the presence of pyrite in the ore was adversely affecting the grades of the flotation concentrates.Following experimental work, it was decided to change over from the acid circuit in the mill to an alkaline circuit. The following description sets out the procedure adopted, -and the results obtained when such change-over was made.The Zinc Corporation ore consists mainly of galena, marmatite, calcite, and quartz, with smaller quantities of pyrite, fluorspar, and rhodonite.A clean specimen of marmatite picked from the ore assayed 53.4% zinc, and 10% iron, with no lead or insoluble. However, as zinc concentrates have been, at times, produced in the plant assaying over 53% zinc, and with only 8.7% iron, apparently the picked specimen of marmatite did not represent an average composition.The average would probltbly be between 8 and 9% of iron, and from 53.5 to 54.5% of zinc. It is obvious, therefore, that from this ore it is not possible to produce zinc concentrate of a higher grade than this average figure.In the Colorado School of Mines Quarterly for April, 1928, Vol. 23, No. 2, on page 47, we find the following statement:As the zinc in the ore exists as marmatite, which contains approximately 54.5% zinc and 10% iron, 49% zinc in the concentrate may be considered high.This statement refers to the ore treated in the Sullivan concentrator, owned by the Consolidated Mining and Smelting Company of Canada Limited.

A statistical investigation of folding at Mount Isa disclosed similar patterns in both the shales and the "silica dolomite" zones. Analysis of these patterns yields 3 groups of folds fully developed north plunging folds, fully developed south plunging folds, and flexuresrepresenting stages of development of folds. Interpretation of results indicates that a regional force from a southerly direction was responsible for the folding.The general pattern of shearing is allied to that of folding, with the exception of bedding-plane shearing. Many shears resulted from continued application of force on folds which ruptured after overthrusting. Other shears resulted from rupture of the more incompetent shale members, without associated folding. The shear pattern is consistent with the theory of a regional force from a southerly direction.INTRODUCTIONThe notes on folding and shearing presented in this paper are part of a current study of the structural geology of Mount Isa.FOLDING 1Measurement of FoldsInvestigation of folding underground involved the measurement of plunge, strike and dip of the axial plane, and the order of magnitude of each fold, together with its location and the rock type. The total number of folds measured was 701, of which 411 occurred in shale areas and 290 within the "silica dolomite" zones. All folds measured are located on main levels, from 7 level to 13 level inclusive. The sections of the mine involved are the Black Star, Black Rock and Racecourse. The physical properties of monazite and the minerals with which it is associated in east coast beach sands are studied and discussed. Possible means for improving recovery are examined. In particular, it is suggested that the practice of splitting the heavy mineral concentrate into two parts as a preliminary to treatment in separate "rutile" and "zircon" mills can be modified in a simple fashion by the addition of an extra magnetic separation stage so as to aid monazite recovery.INTRODUCTIONBeach sand deposits on the east coast of Australia are a major source of rutile and zircon, and supply 80 per cent or more of world requirements. The deposits also contain a small amount of monazite, averaging about 0.5 per cent of the total heavy minerals. Yet despite a steady price in recent years of £150-160 per ton, monazite production has remained relatively small.On the basis of rutiIe production, potential monazite production on the east coast was about 1,500 tons in 1956, but only 102 tons of high-grade concentrate was produced. The main deterrent to increased production has probably been the capital cost of the necessary equipment. Separate electrostatic and magnetic equipment and separate table concentration and drying constitute a considerable outlay for a by-product contributing 4 to 5 per cent of the revenue from rutiIe.It is the purpose of this paper to discuss monazite concentration generally and to suggest modifications to flowsheets, which may facilitate monazite production while maintaining the main objective, simple and cheap means for maximum rutile production.

The intent of this address is to look forward into the 21st Century and present certain thoughts as to how underground coal mining might be conducted. The views in this talk are based principally on the author's experience with mining practices in the United States. It may be difficult to accept, but the 21st Century is a mere 12 years away. What new techniques will be available? To develop an understanding of the way in which technology has been and could be changing, a two- stage analysis is made of mining technology for U.S. Coal. The first phase covers the period 1975 to the present, while the second includes 1960 to 1975. This historical perspective includes a view of underground production by mining methods, considerations of labour productivity, and a review of accident and fatality rates. Several topics are important features of this historical analysis. Among the most significant are: (1) the introduction of Ion-wall mining technology into U.S. underground coal mines, (2) the use of computers to assist in mine planning and design activities, (3) major efforts to improve health and safety performance, with emphasis on underground dust control, and (4) improvements in mining equipment with an eye towards increased reliability and up-time performance. Based on that historical perspective, projections are made in three increments. The first being immediately at the turn of the century, the second being a dozen or so years beyond that, and the third being at the quarter point of the 21st Century.

The traditional method of scale-up uses the population-balance model. In particular, the selection function values for a material are determined in a laboratory mill and a variety of correlations are used to scale these values to plant scale mills. For example, the factor D2-5L (D-mill diameter, L-mill length) appears in many power correlations. The number of collisions is proportional to the number of balls (aD2L), the frequency of mill rotation ((x 1/4D-) and effectiveness of impact ((XD), which gives rise to the 'D2.5L' expression. However, what is lacking in this expression is that it does not throw light on the intensities of collisions. In other words, the collision intensities in both laboratory and plant scale mills are implied to be identical. This fact has been very limiting. Alternatively, the collision frequencies and intensity of impacts can be computed with the discrete element code. This code computes the motion and collision of each individual ball in the ball charge. Therefore the number of collisions and the fraction of collisions in the energy range Et, E2.òòE. (collision energy density) are calculated for a given mill size and operating conditions. Then, using the breakage function determined in drop-weight experiments, the evolution of size distribution is computed for a lab mill. The collision frequency and energy density for larger mill sizes are shown. The fundamental theory outlined for the calculation of size distribution is equally applicable for the large mill, which is the basis for scale-up.

Over a 4 hr period of steady plant operation samples were taken at regular intervals from points within the grinding and flotation circuits. Size fractions of each composite sample were analysed for lead and zinc values and a comparison made of the behaviour of the lead, zinc, and gangue minerals, size by size throughout the whole circuit.Analysis of the breakage behaviour using dynamic mathematical models gave the rates of breakage of each size fraction of each main component. Simulated grinding experiments showed that the main grinding circuit had some reserve capacity.In the lead flotation circuit all lead values between 150 mesh (Tyler) and 5 microns had excellent flotation characteristics, and a major proportion had a high rate of flotation which was practically the same for all sizes. Increase in overall lead recovery would require improvement of the flotation behaviour of ultra-fine lead minerals. These were slow floating and were responsible for approximately half the lead losses in the final zinc concentrate and one-third of the lead losses in the final tails.All sizes of zinc values between 100 mesh and 7 microns floated well and the performance of the zinc flotation circuit was mainly limited by the relatively poor flotation of the very fine zinc values. Approximately 60 per cent of the zinc lost to the final tails lay within the minus 7 micron fraction.The overall flotation conditions were conducive to very good selective flotation of the major proportion of the values and limitations in metallurgical performance were mainly due to the properties of the ultra-fines.

THE South Mine. owned by the Broken Hill South Limited, occupies mineral leases 7 and 8 on the Broken Hill lode. The ore reserves are 4,250,000 tons, and the ore hauled for the year ended June, 1930, was 387,000 tons, the total to date being 7,840,000.The mine, down to the 1070-ft. level, is naturally divided into two sections, the northern and southern ends. Ore extraction is practically completed in the northern end to this horizon, while the southern end is developed ready for a concentrated attack.Fig. 1 shows the state of the 8215-ft. level, after connection to the new shaft.The levels above and below to the 1070-ft. level are in a similar condition, while below the 1070-ft. level the stoping has been confined to the north end, but not completed. The south end on these levels is also partially developed.The ore at present is hauled up No. 1 shaft, which is centrally situated to the present workings. This shaft was put into use in 1909, and in 20 years has had approximately 5,000,000 tons of ore pulled through it.The company, having proved the existence of large ore-bodies in the southern end of the mine, decided to sink No. 7 shaft. This is a six-compartment frame-set shaft (see Fig. 2) equipped with two cages and two 5-ton skips, the remaining compartments being utilised for electric cables, pipes, and ladders. The surface arrangements will include a steel head frame, electric winders, and breaker station.

Semi-massive to massive sulfides were drilled in a shallow-water (630 m) hydrothermal system in an island arc volcanic setting at the Palinuro volcanic complex in the Tyrrhenian Sea,Italy. Overall 12.7 m of sulfide minerlisation were drilled in a sediment-filled depression in a water depth of 630 - 650 m using the lander-type Rockdrill 1 drill rig of the British Geological Survey. The longest drill core recovered consists of 4.9 m of continues semi-massive to massive sulfides + sulfates with abundant late native sulfur overprint. Metal enrichment at the top of the deposit is evident in some cores with polymetallic (Zn, Pb, Sb, As, Ag) sulfides overlying more massive and dense pyritic ore. The massive sulfide mineralisation at the Palinuro volcanic complex contains a number of unusual minerals, including enargitefamatinite, tennantite-tetrahedrite, stibnite, bismuthinite and Ag-sulfosalts, that are not commonly encountered in mid-ocean ridge massive sulfides. In analogy to epithermal deposits forming on land, the occurrence of these minerals and the abundance of native sulfur suggest an intermediate to high sulfidation state for the hydrothermal fluids during deposition, implying that the mineralising fluids were acidic and oxidised rather than near-neutral and reduced as those forming typical base metal-rich massive sulfides along mid-ocean ridges. Oxidised conditions during sulfide deposition are likely related to the presence of magmatic volatiles in the mineralising fluids that were derived from a degassing magma chamber below Palinuro volcanic complex. The Archean Gossan Hill Group at Golden Grove, Western Australia, hosts two potentially economic volcanogenic massive sulphide deposits. The volcanic-sedimentary succession hosting this mineralisation is composed of a facies association which includes the products of rhyodacitic to,andesitic effusive volcanism, mass-flow sedimentation and concentrated hydrothermal fluid discharge, in a deep subaqueous setting. The Tertiary Kuroko deposits of Japan are widely compared to Archean volcanogenic massive sulphide deposits and a comparison of the facies associations hosting this style of mineralisation, in these two successions separated widely in time, allows a refinement of facies models suitable for exploration in ancient successions. The required deep subaqueous environment for the formation of volcanogenic massive sulphide deposits places constraints on the physical processes that can operate during accumulation of the host stratigraphy. Limitations on the operation of physical processes is reflected by the resultant facies association which can be used to characterize suitable volcanic-sedimentary successions in which volcanogenic massive sulphide mineralisation may be present. The facies association includes: (1) proximal effusive volcanic products - lavas and associated autoclastic breccias (2) distal volcanic products - mass-flow resedimented pyroclastic debris (3) background sedimentation - reworked volcanogenic and terrigenous mass-flow sediments and (4) chemical sediments - exhalites including cherts, sulphides and oxides, with perhaps various carbonates and sulphates. Primary pyroclastic deposits are not expected to be present, other than distal water-lain air-fall deposits, derived from shallow to emergent basin margin explosive volcanic centres, which also provide a source for the resedimented pyroclastic debris.

Modern petroleum refining offers a complexity of environments, each requiring different metals for the satisfactory performance of-the various sections of the refinery, and this paper first outlines the varied enva.x¦on-ments thus met.It follows that the metal types in use will also vary to suit each particular environment, and the paper' consequently covers the use of ferrous-metals, ranging from plan carbon steels to the high alloy types and the non-ferrous metals, particularly copper and its alloys.The failures of some,of these metals and alloys are then illustrated by experiences it Kwinana. These failures are duo to (a) corrosion, (b) erosion, (c) faulty welding, and (d) mechanical defects,' of which the first is by far the most prevalent. The protective measures taken to prevent subsequent failures are then given under one or other of the following headings :(1) alterations in process conditions, (2) the use of alternative ,alloys,(3) suitable heat treatment, (4) redesign of the failed section, (5) cathodic protection, and (6) use of corrosion inhibitors.Reference is then made to the use of specialised metals and alloys.Metals, generally in the form of oxides or salts, are also used as catalysts in refining processes. Activated alumina is used, combined with silica .forScracking' of heavy oils, and as a support -for platinum, cobalt, molybdenum, nickel and similar metals, which catalyse hydro-genation and dehydrogenation-processes. .In the final formulation of petroleum products metallic compounds may be added. Reference is made to the large quantities of lead used by the petroleum industry and to the use of metallic soaps and detergents in the formulation of lubricating oils and greases.

Solid Energy New Zealand Limited (æSolid EnergyÆ) is a state-owned coal mining company, producing 4.0 Mtpa from a number of opencast and underground mines in the Huntly, West Coast and Southland regions in New Zealand. The company currently exports 2.1 Mtpa of coking coal. Exports are expected to grow strongly over the next five year as the company expands production into new semi-soft and thermal coal reserves.
