The United States Bureau of Mines has been conducting investigations of electrical equipment curing a period of more than 25 years in an endeavor to encourage the development of safer electrical equipment for coal mines. In order to conduct these investigations in an orderly manner they are carried out under a published schedule of tests known as Schedule 20, which specifies detailed requirements for motors, controllers, etc., and fees covering the various inspections and tests made as part of a routine investigation.

DURING the year just closed the bituminous industry has been marked by a continuation of the period of low prices and a steady deflation, accompanied by the closing of mines and the consolidation of small units into larger ones. This pro- gram has marked almost every field in the country, and it bids fair to continue for some time. Some figures recently published for Kentucky show that in 1924 there were 660 mines producing 45,147,000 tons; in 1928 the number had decreased to 537, while production had increased to 61,860,000 tons, and a similar movement is occurring all over the country. Prices throughout the early part of the year were very low, but during the fall and winter the prices received are more satisfactory than they were earlier in the year. One of the leading developments in the marketing of coal during the year was the trade practice movement, which began in the Harlan field, and has now been adopted in the Harlan, Hazard, Appalachian, Logan. Williamson and Kanawha fields. The purpose of this movement is to prevent the shipment of coal except on order, and by the publication of price lists to prevent much senseless price quotation. Where it has been practised the longest this movement has resulted in a considerable degree of stabilization, and, undoubtedly, it will help existing conditions very materially if it becomes universal.

Didipio gold-copper mine operation is situated in the island of Luzon, approximately 270 km north of Manila in the Philippines, in an area of high seasonal rainfall. The hydrogeological water flow model implied that there would be high connectivity between regional structures and underground operations. This model predicted that underground mine groundwater inflows were expected to quickly rise as the decline progressed to a peak of 510 L/s. Designing a capital pump station to deal with 510 L/s has its own challenges, however developing the decline down to the required level, always dealing with 300 L/s of water and constructing the pump station is extremely challenging and required a robust water management plan and mine design. When dealing with expected high volumes of water, underground mine design and scheduling plays a key role in protecting electrical infrastructure, equipment, secondary pumps, and the decline from flooding. A good drainage system and controlled dewatering plan is essential for safety and progress. This paper details the dewatering techniques and mine designs required to accomplish decline development with high water inflows as used in the Didipio project. It will describe the learnings and outcomes that are achievable with a flexible design, as new structures are mapped, and water intersections are increased. It also highlights the potential for adaptability in mine planning to provide evolving improvements or to evade problems that are expected from the constant reconciliation of data recorded during development.

"Process Safety Management for METTOP means avoiding risks as far as possible - but also dealing with the residual risk by using all existing management tools or external assistance, if required. The presented paper has two parts. The first one introduces a new and very safe cooling technology for metallurgical furnaces. The second part shows how to deal with the residual risk as well as minimize it by using the tapping area of an electric arc furnace as an example.Today more and more intense furnace operations require effective cooling in order to achieve low refractory wear and good furnace lifetime, making cooling technology an important aspect of furnace operation. However, the use of water - today’s standard cooling medium - has some disadvantages, as it can cause problems both during furnace start up and operation, namely hydration problems, corrosion, and explosion.To overcome the disadvantages of water, a new cooling technology using an alternative cooling medium, namely ISIS B (a special kind of ionic liquid) was developed. The main advantage of ISIS B is the fact that no explosion occurs when this liquid comes in contact with molten metal. The working temperature range of ISIS B of 50 to 200 °C results in other advantages as there are no corrosion problems and no risks of hydration.The fact that no explosion occurs when ISIS B contacts molten metal leads to a revolutionary solution for the metallurgical industry in terms of safety. Especially in critical areas - like tap-holes - new safety standards can be achieved.Together with the TÜV Austrian Holding AG a risk analysis was done for a water cooled tap-hole as well as for a tap-hole cooled with ionic liquids. It is shown that using the new technology the risk can be decreased dramatically and the importance of a best available technology is described."

The silver deposits of Rockvale near Armidale, N.S.W., occur in fissure veins in epi-Permian granite. The ore is fine grained, mineralogically complex, and contains twenty-seven different minerals including eleven silver minerals. The ore is characterized by a unique multiple unmixing from a sphalerite solvent. The following minerals have exsolved from solid solution in sphalerite: pyrrhotite, chalcopyrite, cubanite I, cubanite II (chalcopyrrhotite), valleriite, "brown" stannite, "olive" stannite, argentian tetrahedrite, pyrargyrite, and an unknown grey-white mineral. Several of these minerals derive from a second-stage unmixing, the immediate solvents of which are the products of earlier exsolutions.INTRODUCTIONSmall silver-rich base metal deposits occur at several localities near Armidale in the New England district of New South Wales. Chief among these are the Tulloch Mine and the Rockvale Mine at Rockvale, 27 miles SE. of Armidale, and the Tait's Gulley deposits some 15 miles NE. of Armidale.At Rockvale (Kenny, 1928), irregular lenses of lode material, principally quartz and calcite, occur along a zone of shearing in biotite granite and altered sediments. The orebodies had a maximum width of 6 ft but averaged 2 ft 6 in. The largest lens was 150 ft long and 50 ft high. The Rockvale deposits were worked for both arsenic and silver; 2,897 tons of arsenic were produced but no figures are available forsilver.Little is known of the Tait's Gulley deposits which occur in folded arenites containing small but rich veins of pyrargyrite with base metal sulphides.The sediments in these localities are possibly Silurian.

"Horizontal grinding mills have been used in the mining industry for more than a century, and due to their operational and maintenance simplicity they remain a widely used technology for various stages of comminution. The relative ‘efficiency’ of horizontal grinding mills compared to other comminution equipment is often debated, and efforts to improve the energy efficiency of these mills is an on-going area of attention for process engineers.The discharge arrangement design affects the material transport characteristics within a horizontal grinding mill. Material transport plays a significant role on efficiency of horizontal grinding mills. The magnitude of benefits that can be achieved with efficient material transport in AG/SAG mills has been demonstrated in a Greenfield milling operation at Mirabela Mineração’s SantaRita Nickel operation in Brazil. This grinding circuit was commissioned in October 2009, which was designed as a typical SABC circuit to grind 4.6 mtpa (575tph) to produce a P80 of 125um with 10-12% ball charge in SAG mill. The nameplate design capacity has been achieved in few days in ABC circuit without grinding media in SAG mill, and has been operating in ABC mode. The through has gradually increased to an average of 800 tph and recently 7% ball charge has been introduced to increase the throughput to +900tph.This paper investigates improved material transport using Outotec’s TPL™ (Turbo Pulp Lifter) technology and the opportunities this technology presents when designing horizontal grinding mills. Operational data, with particular reference to energy and throughput optimization, is presented."

"The paper is introduced by an examination of the factors which have led to the present world-wide interest in charging preheated coal to coke ovens. Various possible methods of preheating and charging the coal are considered and reasons are given for the adoption of the methods chosen. The design of the pilot preheating plant for the British Coke Research Association is discussed and its influence on the design of the commercial units for the South African Iron and Steel Corporation's plant at Pretoria and the British Steel Corporation's plant at Beighton are explained. Operating procedures on the pilot plant are described, particularly with regard to the charging of the preheated coal. Design features of the commercial installations, engineered by Simon-Carves Ltd., are explained with emphasis on the built-in safety of the control system of the preheating plant, and the particular problems associated with the erection of such plants on working coke oven batteries. Commissioning and operating procedures are described and the problems associated with the integration of the preheating plant into the normal pushing schedule of the plant are discussed. Preliminary operating results with regard to the effect of preheating on coal throughput on the commercial plants and on the properties of the coke produced are given."

This paper summarises the lessons learned during a recent carbon reactivation kiln upgrade project at the Sukari Gold Mine in Egypt. The kiln is being replaced to ensure that the plant achieves full value from the current throughput expansion from 5 to 10 Mt/a.The suitability of the original engineering design criteria for kiln selection and the adequacy of the original kiln are reviewed in the light of the carbon chemistry, operating experience and a site inspection of the kiln. Particular aspects of the reactivation facility are discussed including: predrying, preheating, feeding, gas seals, kiln heat transfer, steam injection, counter-current sweep gas flow, carbon cooling, fines minimisation and removal.Design criteria for a carbon predryer, reactivation kiln and cooler combination for carbon heavily fouled by high molecular weight frothers and collectors are discussed and the abridged Ausenco datasheet for the combination is presented.Responses to a request for a budget price sent to most of the worldÆs suppliers of carbon regeneration kilns are summarised.CITATION:Claflin, J K, La Brooy, S R J and Preedy, D, 2013. "The Canadian Copper Company, formed in 1885, began mining the nickel-copper ores of Sudbury in 1887 and producing nickel-copper matte from its smelter in 1888(1). Although it was quickly appreciated that the separation of nickel and copper (i.e. refining) was critical to the production of a marketable nickel product, refined nickel was not produced in Canada until about 1918. From the beginning of the industry, there was considerable political agitation to refine nickel in the Province of Ontario(2). The companies, however, claimed that the existing processes, viz. the sodium sulphide ""tops and bottoms"" process operated by the Orford Copper Company of New Jersey, U.S.A.(1,3,4) and the carbonyl process(5,6) developed by the Mond Company at Clydach, Wales, were too costly to operate in Canada. Nickel was first refined in Ontario about 1918 when the International Nickel Company opened a nickel refinery at Port Colborne, Ontario(6). Initially, however, the refinery at Port Colborne employed the Orford sodium sulphide process, and the first electrorefined nickel in Canada was produced in 1920 by the British America Nickel Corporation whose mine and smelter were located in Sudbury, Ontario, but whose electrorefinery was located at Deschenes, Quebec, near Ottawa, Ontario. The purpose of this article is to outline the early history of nickel electrorefining and to trace the significant developments which led to the first successful commercial electrolytic nickel refinery in Canada. The development of an electrolytic process for refining nickel is due largely to the efforts of four individuals, viz. Carl Hoepfner, Hans A. Frasch, D.H. Browne and ultimately N.V. Hybinette, and the contribution of each is discussed separately below."

A paper identifying anticipated challenges in the extraction of the N3500 Orebody at Glencore Xstrata Copper’s Mount Isa Copper Operations was written in 2007. The paper identified the depth of the deposit, the anticipated high stresses, talc alteration and issues with ground support as factors that may affect production. A number of strategies were planned to cope with these issues. After six years of extraction those strategies are evaluated and additional measures taken are outlined. Processos de fundição de ligas de alumínio com altos teores de Mg são um desafio da indústria, uma vez que diferentes taxas de resfriamento ocorrem em geometrias diversas. Este trabalho tem como objetivo caracterizar a liga de alumínio AA5052 com variações em teores de Mg por meio de experimentos de solidificação direcional transitória, e compreender seus efeitos com relação a evolução de parâmetros térmicos, microestruturais, e propriedade mecânica. As leis experimentais de evolução do espaçamento dendrítico primário (?1) com relação a taxa de resfriamento resultam em: ?_1=640 ?T ?_L?^(-1/4) para a liga 5052 com 2,4%Mg e ?_1=300 ?T ?_L?^(-1/4) para a liga com 3,2%Mg. As leis de crescimento de espaçamento dendrítico secundário (?2) com relação a velocidade de solidificação resultaram em: ?_2=27 ?V_L?^(-1,1) para a liga 5052 com 2,4%Mg e ?_2=15 ?V_L?^(-1,1) para as ligas com 2,6%Mg e 3,2%Mg. Maiores teores de Mg na liga AA5052 proporcionaram uma expressiva redução no valor de ?2 para uma determinada VL. A evolução da dureza mostrou-se diretamente relacionada ao tamanho e a distribuição dos intermetálicos presentes, sendo ?2 o fator predominante neste controle.

This paper presents a case study where Cenozoic age detritals and an ancient buried landslide have been modelled in the final walls of an iron ore pit. Orebody 25 is located in Western Australia’s Pilbara region and is an operating pit where a redesign of the final wall cutback was required. The existing slope design utilised a simplistic geological model of mixed conglomeratic, calcretic and clay –dominated detritals overlying deformed bedrock sequences. This simplicity resulted in a conservative design that was not reflective of the slopes performance to that point. Further field investigation was carried out with the results assessed in conjunction with historic drilling, mapping and laboratory testing. A detailed geotechnical model was developed including three conglomerate and two clay dominated units and laterstage calcrete overprinting. The clay units were of significant importance to the stability assessment and resulting slope design due to the thickness and poor strength conditions indicated by the laboratory and field testing. However, the drilling investigation shed only limited understanding as to the origin of the clays and thereby how they should be treated as a component in the stability analysis. A comparison of the ratio of various geochemical elements measured by assay was made between the clays and 1) shale bedrock and 2) clays of known detrital origin from other nearby sites. This indicated the clays had a strong affinity with weathered bedrock. This and other textural features resulted in the interpretation that the clay units were relic debris preserved following ancient landslides in bedrock shale. Using laboratory testing data and calibration of results for a landslide origin, stability analyses were run, resulting in a steeper slope design being implemented. Excavation of the final cutback is now in progress with the model confirmed as appropriate.

This research focuses on the strata control issues in a Bord and Pillar depillaring working under the influence of dead load condition of voluminous and fragmented softcover of overburden dump material. It reports the modeling of a mine working through the FLAC 2D software under the Indian geo-mining conditions. The modeling results of an actual mine working under softcover were compared with that under the intact overburden condition to assess the influence of the soft overburden over the depillaring working. The modeling work involved progressive caving of the strata and the cyclic goaf filling following the modelsimulated occurrence of main fall and periodic caving. Mohr-Coulomb Failure criteria along with a simplified strain-softening (MCSS) model was used to study the failure and caving mechanism. Salamon’s compaction model was used to simulate compaction and resultant stress recovery in the goaf material. Comparison of the model findings was done in terms of load on supports, abutment stress, and convergence, apart from the stress redistribution and failure mechanism. The load transfer mechanism of the soft overburden was also studied for an overall understanding of the strata behavior in such workings.

"On January 9, 2014, a large chemical spill occurred on the bank of the Elk River near Charleston, WV. Within hours, the potable water supply for more than 300,000 people was contaminated, and the incident captured national headlines for weeks. Although the primary chemical, crude 4-methylcyclohexanemethanol, or MCHM, is deemed nonhazardous, the impacts of the spill are significant – with all stakeholders, including the public, members of industry, and relevant government agencies experiencing losses. This case highlights the need for a more proactive approach to environmental management, shifting from crisis control and remediation to crisis prevention and risk management. INTRODUCTION On the morning of January 9, 2014, approximately 10,000 gallons of crude 4-methylcyclohexanemethanol (MCHM) leaked from a 48,000-gallon storage tank sitting on the bank of the Elk River near Charleston, West Virginia (WVDEP, 2014; CSB, 2014). MCHM is a chemical commonly used in preparation of fine coal, and is deemed nonhazardous in terms of transportation and storage regulations and also toxicity (CDC, 2014a). Freedom Industries, Inc., a producer and vendor of specialty chemical products, had acquired the storage facility where the spill occurred from Etowah River Terminal, LLC just a few weeks prior in December 2013 (WVDEP, 2014; Freedom, 2005). The direct cause of the spill was ultimately determined to be two small holes in the base of the 76-year-old tank, which were created due to corrosion damage (Bauerlein & McWhirter, 2014). The spill site was located just over a mile upstream from the intake of a municipal water treatment facility operated by West Virginia American Water (WVAW), which serves a population of 300,000 across nine counties in central West Virginia (CDC, 2014a). In a matter of hours, the leaked MCHM made its way through the treatment facility and into the water of nearby homes and businesses, where its strong smell was noticed almost immediately. On the evening of the spill, WVAW concluded that its conventional treatment processes, which include carbon filters to remove a variety of contaminants from water, were not capable of fully removing the MCHM and a “Do Not Use” order was issued for all customers (WVAW, 2014a). With little information readily available regarding the potential human health effects of MCHM, the US Centers for Disease Control and Prevention (CDC) quickly issued an advisory ruling that set a maximum screening level of 1 ppm for safe drinking water (CDC, 2014a). Based on this recommendation, the “Do Not Use” order remained in effect for a total of eight days, until January 17, when the WVAW distribution water was confirmed to have MCHM concentrations below this level (WVAW, 2014a). However, due to the pungent odor of the chemical, its presence was still recognizable in the WVAW facility and in consumers’ tap water after the order was lifted (Manuel, 2014). Recent studies have shown that the chemical can be detected in water at concentrations as low as 0.55 ppb by individuals experienced in odor identification (Rosen et al., 2014); and even individuals with no such training can detect pure MCHM at just 7 ppb (WVTAP, 2014). The fact that this chemical can be sensed at such low levels – indeed far below those considered safe – undoubtedly added to growing public concern. In response to numerous complaints and demands, WVAW continued flushing the treatment facility and pipes until February 25 when MCHM levels were measured below 2 ppb (WVAWb, 2014), and eventually replaced the affected filter media (WVAWc, 2014)."

During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. The Sago Mine Disaster, on January 2, 2006, placed renewed focus on several different aspects of safety in the underground coal mining industry. One area which received increased attention since Sago and other underground coal mining events is the concept of Safety Culture. This report describes the results of a pilot Safety Culture Assessment implemented at a small Appalachian underground coal mine, hereafter referred to as the Mine. The purposes of this pilot assessment were to (a) evaluate the feasibility and usefulness of a methodology for assessing safety culture that had previously been developed for use within other industries, (b) to initiate an investigation into the understanding and defining of the existing safety culture of the underground coal mining industry, and (c) provide meaningful and useful recommendations for the participating mine for their consideration in their ongoing quest to enhance their existing safety culture. This pilot assessment was conducted in July 2007. The report provides information regarding the presence or absence of safety culture characteristics within the Mine. Observations regarding the characteristics of the Mine?s safety culture that should be sustained are presented. Areas in need of attention and management focus to improve the Mine?s safety culture are presented as well as recommendations for moving forward. Safety culture characteristics that are important for the existence of a positive safety culture have been identified to include: - Safety is a clearly recognized value in the organization. - Accountability for safety in the organization is clear. - Safety is integrated into all activities in the organization. - A safety leadership process exists in the organization. - Safety culture is learning driven in the organization. - A process for establishing a strong and effective Safety Conscious Work Environment (SCWE) is in place. Performance objectives are associated with each of the safety culture characteristics and particular behaviors and attitudes have been identified that can be measured to evaluate these objectives. Using a methodology originally developed with the support of the U.S. Nuclear Regulatory Commission, an assessment of selected organizational behaviors and attitudes was conducted to evaluate the Mine in terms of these safety culture characteristics and their associated performance objectives. The methodology involves obtaining a variety of quantitative and qualitative information, using multiple data-gathering methods. The information collected is largely based upon the perceptions of the individuals in the organization. The evaluation is a ?point in time? snapshot of the Mine, but cultural beliefs and assumptions do not change quickly. The feasibility and usefulness of the methodology was successfully demonstrated during this pilot assessment. During the last decade, many exhaustive studies have been made of process energy requirements for the production of various metals and materials. Some excellent technical papers and text books have resulted, and some of these are included in the bibliography of this paper. These studies were, of course, prompted by the economic pressures which resulted from the beginning of the "Energy Crisis" in 1973. Our intensified consciousness of the cost of energy led, then, to a greatly improved understanding of the energy consumption attributable to individual mining, processing, and refining steps. It is worth noting, however, that an awareness of the cost of energy is not new and that efforts to minimize this cost were an important part of sound mine and plant management practice throughout the earlier days of our industry. One only has to read a few accounts of mining operations around the beginning of this century to develop an appreciation for the cost of supplying wood, coal, fuel oil, or electric power to remote locations.

THE heavy shrinkage in the production of bituminous coal has reflected adversely in the matter of tonnage produced by stripping arid mechanical loading machinery. The purchase of stripping and underground loading equipment has likewise been circumscribed. How- .ever a growing percentage of bituminous coal is being loaded by stripping and mechanical loading machinery. This situation is due to the fact that numerous strip pits were able to operate in Illinois and Indiana when under- cover mines were subjected to labor interference, a further advantage gained by their ability to produce and market coal at a price below that of the undercover mines. The mines equipped with mechanical loading. apparatus have enjoyed more continued operation for the double reason that the companies using coal-loading machinery are, generally speaking, more substantial and in better financial condition than many of the smaller properties who maintain hand-loading methods, and the market advantage due to lower production costs also reflects to the advantage of the undercover mines using mechanical loading equipment. Where a number of mines are con- trolled by one company, those not equipped with loading machinery and suffering from higher producing costs have often been closed, with the available tonnage concentrated in the more modernly equipped mines.

Fatigue is it the newest buzz word or is fatigue a real phenomenon that is creating safety and organisational risk for you? There is no debate that fatigue is natural, and it affects us all, just like gravity; however, there has been much debate on the severity of its impact in the workplace. Over the last several years, advancements in technology have provided safety and health professionals, sleep scientists, fatigue experts and organisations with the ability to monitor and detect fatigue events in real time, as well as predict and monitor increased risk for fatigue events. Caterpillar Safety Services has assembled an extensive and exceptional set of technologies that will provide the best methods and data to measure the impact of fatigue. Monitoring for signs of elevated spontaneous combustion in the active goaf behind the longwall face is crucial for early intervention of any potential heating incidents. Typically, trigger action response plans (TARPs) are established at the start of longwall mining and often are only reviewed when triggers are activated frequently and become ‘nuisance alarms’, or in the worst case scenario after a heating event. Narrabri Mine has adopted a proactive approach of reviewing the longwall active goaf spontaneous combustion monitoring TARP at the commencement of each longwall block and prior to reaching a known ‘mylonite zone’ of crushed coal, which is associated with increased coal oxidation. The review is based on information recorded from the monitoring data of the previous longwall blocks as well as interpretation of relevant spontaneous combustion propensity results for the coal. This has resulted in the mine implementing a zoned TARP approach for each longwall block covering both the longwall retreat through normal undisturbed coal and the zone where the goaf is influenced by the increased coal oxidation associated with the mylonite coal. The consequence of this approach is that it has removed the issue of having nuisance triggers being activated during mining through a known increased oxidation zone, while maintaining a high awareness of indicators of spontaneous combustion.CITATION:Baxter, H, Claassen, C, Salisbury, O, Brady, M and Beamish, B, 2015.
