"How Wall Street Darling Molycorp Undermined the U.S. Economy, Degraded our National Security and Distorted the Global Economic Balance and How These Falsehoods Continue to be Promoted Even Today INTRODUCTION & SUMMARY In 2007 the Molycorp Mountain Pass rare earth mine was resurrected from its desert-grave in California by its then-owner Chevron Mining. Responding to elevated rare earth prices Chevron began processing on-site rare earth concentrates left over from past mining operations that were terminated in 1998i. Chinese demand had sparked what was being described as a commodities super-cycleii. Commodity prices were on the move. Wall Street smelled opportunity. In 2008 Goldman Sachs, Pegasus Partners, Resource Capital Funds, Traxys North America LLC and Carint Group LLC acquired the mining assets from Chevron. In an environment of ever-declining ethical standards and regulatory consequences the private capital group and their investment bankers would bring Molycorp to market. Molycorp, with its high powered investment banking team of Morgan Stanley and J.P. Morganiii, easily dominated the contest. Molycorp promoted itself, largely based on its past operating history and proven reserves, as the best company to challenge China’s monopoly control over the production of rare earths: a group of elements critical to most advanced technologies and U.S. defense systemsiv. Most of Wall Street’s hottest investment sectors were dependent on these materials: alternative energy, green technologies, computing, personal communications, advanced electronics and defense systems. By late 2010 there were dozens of prospective rare earth start-up ‘mining companies’ seeking capital. Eventually the number of prospective rare earth mining companies seeking capital exceeded 400, despite the reality that supply requirements of this market were disproportionally small when compared to other natural resource sectorsv. In the world of finance ‘smart money’ picks the winners. Consequently just twovi projects attracted nearly all of the capital (Molycorp and Lynas). Wall street picked Molycorpvii. This paper will primarily focus on Molycorp. By early 2010 the words “Molycorp” and “Rare Earths” electrified many people in the mining industry, most non-Chinese manufacturing technology companies, U.S. & allied defense contractors, technocrats in the U.S. national security community and eager investors across most of the world’s stock markets. It was the hottest story of the yearviii. The financial, technology and defense press coverage verged on euphoric: heaping fuel on fuel until the highly combustible Molycorp-pyre cast a long shadow over all of its rivalsix. When the IPO match was lit the spectacular flaming pyre sucked all of the oxygen out of the rare earth spacex. Molycorp was ranked the most lucrative IPO of 2010, resulting in record breaking profits to its private investors and massive returns for its early stock holding investors."

This publication covers the work of the Division of Coal Mine Inspection, Bureau of Mines, as performed under provisions of the Federal Coal Mine Safety Act. It presents background information and data since 1952 when the Act became effective and includes up-to-date statistics for the calendar year 1962. During that year, approximately 9,704 coal mines were active in the United States, and the Bureau made 12,810 regular inspections. BACKGROUND The Federal coal-mine inspection program was established to carry out the provisions in Public Law 49, 77th Congress, (H. R. 2082) approved May 7, 1941. This act has since been amended as Title I, Advisory Powers Relating to Health and Safety Conditions in Mines, under Public Law 552, 82d Congress, approved July 16, 1952, which also included Title 11, Prevention of Major Disasters in Mines. Public Law 552 is known as the Federal Coal Mine Safety Act.

Organized efforts toward the prevention of injuries and occupational diseases have been of comparatively recent origin in the mining industry, dating generally from the beginning of this century. Mining-and particularly underground mining- is more hazardous in many respects than most other major industries, and continual and increasing attention has been given to its health and safety aspects. The importance of the physical well-being of those engaged in mining was recognized by Agricola, who, after describing the hazards encountered in 16th century mining operations, stated that "Some of these evils, as well as certain other things, are the reason why pits are occasionally abandoned." In the past 50 yr particularly, mine managements and labor organizations, separately or collectively, have mounted productive efforts toward the prevention of accidents and industry-associated diseases and illnesses. A statistical study of this period indicates that increasing awareness coupled with the endeavors of all concerned agencies has resulted in decreasing frequency of occurrences of injuries and illnesses. In recent years particularly the mining industry has used the services of safety engineers, industrial hygienists, inspectors, ventilation engineers and other specializing in locating and correcting deficiencies affecting the welfare of employees. Concurrently, the labor movement has incorporated into its organization individuals working in the fields of health and safety, many devoting full time to these responsibilities. Others serve in behalf of their fellow workmen as on-the-job safety representatives. Most authorities agree that accidents are caused by (1) unsafe acts, (2) unsafe conditions and, exceptionally, (3) Acts of God. The "unsafe act" often is cited as the prime cause. but many accidents, and attendant injuries, are the result of a blend of "unsafe acts" and "unsafe conditions," with each chargeable in varying proportions. It therefore is necessary to consider both the physical elements of the mine plant and the training, mental attributes and actions of all personnel involved. Analysis of an accident, if based on hasty reconnaissance and unduly hurried questioning, seldom will disclose more than surface causes and consequently will result in inadequate measures to prevent repetition. Mining involves unavoidable and calculated risks, as in all facets of life. It is most important then that men employed in mining be schooled in the hazards and develop fully the ability to evaluate and minimize risks. The environment, of underground mining often is harsh and strange compared to the more natural surface conditions man has learned to cope with as a part of established life patterns. The miner. therefore, must adapt to unusual situations and conditions and must develop a mental attitude permitting acceptance of change. 3.1-INDUSTRY-AFFILIATED SAFETY AGENCIES L. S. "Roof bolting has long been used in underground mines across the world to provide ground support. Modern roof bolts are cheap and easy to install with the use of specialized machines as a part of the production cycle. Lhoist North America’s Crab Orchard Mine is an underground room and pillar limestone mine that uses mechanically anchored roof bolts for ground support. The mine currently employs two different roof bolting patterns: a standard 5 × 5 foot pattern, and a 2.5 × 2.5 foot pattern for use in areas with particularly hazardous roof conditions. The purpose of this study is to evaluate the relative effectiveness of each bolting pattern.A series of numerical models were created using RocScience’s RS2. The models were based on a symmetrical section of the mine at its deepest point, and were modeled using Generalized Hoek-Brown failure criterion along with a discrete fracture network. A series of sensitivity analyses were performed on the models by varying parameters such as joint friction angle, crack persistence, joint randomization, and tensile strength of the limestone. Based on the results of the original models and sensitivity analyses, it appears that the standard bolting pattern provides sufficient roof support capacity under almost all the expected conditions at the mine, since factors of safety below the design value of 1.5 were only found for individual bolts in a few of the worst test cases considered. These results can help improve the Mine’s productivity and reduce operating costs without compromising safety.INTRODUCTION AND CASE STUDY DESCRIPTIONLhoist North America’s Crab Orchard Mine is an underground limestone mine located in Crab Orchard, Tennessee. The mine is located under an 800-foot-high mountain, and outcrops on three sides. The mine is accessed through three separate portals and has two main levels, along with a partially developed third level. Extraction is accomplished by the room and pillar method using 50 × 50 feet pillars and 50 × 50 feet rooms. The mining height is limited to 50 feet to keep the pillar width to height ratio around 1.0. Each heading is excavated in a two-stage process that consists of a 30-foot-tall top heading and a 20-foot-tall bench. A distance of at least 30 feet is maintained between the two levels at all times (Crab Orchard Mine Technical Staff, 2017)."

To determine the availability of manganese for metallurgical purposes from selected world resources, the Bureau of Mines evaluated the potential production of contained manganese in concentrates based on the demonstrated resources of 23 foreign and 8 domestic mines and deposits in 9 market economy countries. All but 2 of the 23 foreign deposits are operating mines; none of the domestic deposits are in operation. The demonstrated resources of the mines and deposits used in this study represent an in situ ore resource of approximately 2.2 billion tons," containing 780 million tons of manganese with a total recoverable production potential of 514 million tons of contained manganese in concentrates. The analyses of mines and deposits in this study indicate that about 213 million tons of the contained manganese (41 pct) is potentially available at a January 1981 constant dollar long-run total cost of up to $1.50 per long ton unit (22.4 lb, or 10.15 kg contained Mn), approximately 428 million tons (83 pet) is potentially available at a cost of about $1.75 or less, and 491 million tons (95 pet) is available at less than $3.30. The remaining 23 million tons, which is potentially available at much higher costs (about $8 to $35 per unit), represents the U.S. deposits. On an annual basis, with a cost range of up to $1.75 per long ton unit, the availability from currently operating mines would last at least 80 years based on 1980 world production data. Total manganese production costs are estimated to comprise mining-8 pet, beneficiation-3 pet, transportation-26 pet, and smelting-63 pet.

"Recent project failures have called into question the use and relevance of benchmarking and independent reviews, forcing a rethink of traditional processes and procedures.With cost estimates and schedules dramatically varying from lows seen around the global financial crisis to peaks experienced in the recent mining boom, benchmarking has ranged between essential and misleading.Profound shifts are now underway in future project delivery strategies as the market moves away from cost-plus engineering, procurement and construction management. The current trends in project implementation are to utilise more lump-sum design and construct or fixed-price supply and construct contracts. This trend will remove transparency in detailed cost information by reducing the sources and clarity of cost data.Independent peer reviews (IPRs) of studies and investment recommendations performed over the last decade have both failed to prevent flawed or deficient projects from proceeding to approval and funding, and been successful in ensuring that owners did not proceed with doomed investments.In recent times, it has been recognised by some owners, contractors and engineers that traditional benchmarking checks or IPRs, or a combination of both, will not necessarily produce reliable assessments of study conclusions and of the forecasts of project outcomes. To be of value, the benchmarking process now also needs to recognise the highly volatile responses from the engineering and construction industry to the current and future markets for their services.Similarly, reviewers who draw upon benchmarks of past project experiences on an understanding of the things that shaped and influenced their outcomes need to be more aware of how a future dynamic market may impact the outcome of the projects and raise warning flags accordingly.With the commodity cycle currently approaching what is hoped is its bottom, ensuring that benchmarking and independent reviews evolve into reliable, transparent and worthwhile processes becomes even more important. The serious shortage of domestically produced asbestos during the war prompted the Bureau of Mines to investigate reported asbestos deposits in Alaska. The most promising' appeared to be these in the Cosmos Hills and Jade Hills areas along the Kobuk River in northwestern Alaska. The deposits along the west side of Shungnak River, a tributary of the Kobuk River, were the first to be investigated by the Bureau, in the summer of 1944. Four bulldozer trenches were excavated, from Which a 2-ton sample was taken. This sample was shipped to Rolla, Mo., for testing in the Bureau of Mines laboratory. In the summer of 1945 an investigation of asbestos deposits on Cosmos Creek, also a tributary of the Kobuk River, was begun; the work was completed during the field season of 1946.

In order to safely store radioactive waste long-term, HAW repositories in saliferous formations have to be backfilled. The main purpose of those backfill operations is the restoration of the geological barrier function of the containment area. For best possible results the used material should be of a comparable composition or characteristic for the host rock formation. Additionally, the material should show a high density after completion of the backfilling process as well as a low permeability. A major aspect to consider in these processes is the minimization of the gap of the backfill body to the roof of the drift. To fulfil those requirements, a micro-structure stabilized crushed salt backfill material has been developed as part of the GESAV-I-project. Due to the material composition of this so-called GESAV-material, salt mineralcrystallization processes start after emplacing it, a process that leads to the polyhalite formation. With the crystallization of polyhalit, the halite grains are bridged, which in return stabilizes the matrix of the backfill. The GESAV-II-project continued successfully what GESAV-I started and determined an ideal backfill method in order to build the backfill bodies composed of GESAV-material. Due to the compaction of the material during emplacement by vibration, high in-situ densities, low permeabilities and a backfill factor close to 1 was able to be reached. In the following SAVER project this journey of practical test underground will be continued.

Introduction It is only fitting on this occasion of the fiftieth anniversary of the incorporation of the Institute that we should indulge in a bit of retrospection and review the progress that has been made within this period in the various branches of the mining industry- progress that is so closely associated with the members of this Institute. Such a moment of retrospection will enable us better to appreciate the present status of the industry and to perceive more clearly the trends that should guide us in making sound plans for the future. The past half century has been a period of amazing industrial progress in Canada. In this period, the economy of our country has changed from one based chiefly on agriculture and the export of raw materials, to an economy based largely on a well integrated manufacturing industry. In all' o( these major developments, industrial minerals have played an important part and will play an increasingly, important part in the future. By ?industrial minerals? we mean the various non-metallic minerals (exclusive of fuels) as well as the rocks, sands, and clays used for industrial purposes. They are now used so widely and in such large quantities that modern industry could not function without them, and their continued availability at low cost is vital to the industrial future of Canada. The key position occupied by industrial minerals in modern Canadian industry was well illustrated two years ago when a strike occurred in the only Canadian plant producing soda-ash and calcium chloride at a time when those materials were already in short supply and unavailable from other sources. Within a short time of the shutdown of this plant, in which the two principal raw materials are limestone and salt, the great aluminium reduction plants on the Saguenay river were forced to curtail operations because of the lack of soda-ash, essential to the process of extracting aluminium from its ore. Automobile production, already harassed by a shortage of steel, was further hampered when glass for windshields and windows could not be supplied because the glass industry was slowed down almost to a standstill for want of soda-ash. Soap became almost unobtainable both to the housewife and to industrial users, because soda-ash is required in its manufacture. As a consequence of the soap shortage, the production o synthetic rubber at Sarnia was threatened because soap in large quantities is essential to the polymerization process. Motorists on gravelled highways all over Eastern Canada drove through clouds of dust because calcium chloride, used for dust laying, was not to be had. There were other cases of dislocation of industry related to the shutdown of this particular plant, but the above illustrates the wide ramifications of industrial minerals in modern industry. However, it is extremely doubtful if many Canadian citizens realized that these apparently unrelated happenings were connected in any way with limestone and salt.

The transportation of solids by pipeline is not new. In 1891 a United States patent broadly covering a method of pumping coal with water was granted to W. C. Andrews. His pilot plant was constructed on what is now the site of the New York Public Library in Manhattan. His process was applied to pumping coal mine waste back into the worked out portion of coal mines. On October 26, 1967, the world's first long distance iron ore slurry line was put into service by Savage River Mines. The commissioning of this 5.0 million dollar installation climaxed three years of development, testing, and construction and proved the Company's faith in pioneering this unique transportation method for iron ore. This paper will discuss some of the more pertinent items in connection with that three- year period. Also to be discussed will be the operation of this line during the past eleven months.

LEAD POISONING as a result of eating lead pellets deposited in marsh areas is a cause of high mortality among ducks, geese, and other waterfowl ingested lead shot become trapped in the gizzard with gravel and are slowly ground away by the grit. Absorption of lead as it passes through the intestine in a finely divided state produces severe lead poisoning which is usually fatal. Ducks and geese with lead poisoning develop first a typical green diarrhea. Later, paralysis of the wing muscles prevents flight. As the poisoning continues with further grinding of the shot trapped in the gizzard, increasing paralysis reduces the bird to helpless-

ABSTRACT Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage organizations to work together, executing such risk assessments comes with challenges. This paper focuses on one trona mine’s experience in the development and implementation of a field level risk assessment program to help its entire organization understand and manage risk to an acceptable level. Through a transformational process of ongoing support and communication, Solvay Green River fostered a culture of trust and safety grounded in risk assessment, safety interactions, and hazard correction. The application of consistent risk assessment (RA) tools was critical to create a participatory workforce that not only talks about safety, but actively identifies energies that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s process of RA implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks. As discussed, consistent efforts to establish a participatory risk management program are worth the initial struggle to ultimately reduce incidents. INTRODUCTION Work-related health and safety (H&S) incidents often account for lost days on the job, contributing to organizational/financial and personal/social burdens [1-2]. Accompanying research demonstrates that risk and ambiguity around risk contribute to almost every decision that individuals make throughout the day [3-4]. In response, understanding individual attitudes toward risk has been unquestionably linked to predicting H&S behavior [5]. Although an obvious need exists to identify more comprehensive methods to assess and mitigate potential hazards, some argue that risk management is not given adequate attention in occupational H&S [6]. Additionally, research suggests that a current lack of knowledge, skills, and motivation are primary barriers to mitigating workplace risks [3-6]. Therefore, enhancing knowledge and awareness around risk-based decisions, including individuals’ abilities to understand, measure, and assign levels of risk to determine an appropriate response, is increasingly important in hazardous environments to predict and prevent incidents. This paper focuses on tailored approaches to field level risk assessment (FLRA), measurement, and common barriers to participating in such activities. The authors use a trona mine in Green River, Wyoming, as a case study to illustrate how the organization overcame widespread and site-specific challenges to implement a proactive risk management program. By discussing the mine’s tailored FLRA program, this case study contributes to the literature by first, providing examples of critical components that need to be in place to ensure successful risk management and second, context for continuous quality improvement that can foster a higher level of trust and empowerment on site."

This report covers the calendar year 1953 and has been prepared and submitted in accordance with the provisions of Sec. 106(a) and Sec. 212(c) of the Federal Coal Mine Safety Act (66 Stat. 692; 30 U.S.C. Secs. 451-483). During this first full calendar year of operations under the Federal Coal Mine Safety Act, the coal-mining industry of the United States attained the best injury records in its history - improving the former best records (made in 1952) by 91 fatal and 10,020 nonfatal injuries. Whereas, the mandatory provisions of the act were intended and designed to prevent major mine disasters, apparently the effects have reached beyond the scope thereof in reducing mine accidents generally.

The following report is made through the Bureau of Mines as a result of the work under the cooperative agreement with the State geological survey and the engineering experiment station of the University of Illinois. It deals with the use of permissible explosives in the coal mines of Illinois, and is intended primarily to present to mining men the possibility of assuring safer conditions in mining through the use of these explosives. Twenty coal mines in Illinois were using these relatively safe explosives at the time this report was prepared. Of these, seventeen were in Franklin County, two were in Saline County, and one in Montgomery County. In these various fields experience had shown that although black blasting powder was a useful agent in breaking down the coal at the working face, yet it had given such conclusive evidence of its danger to life and property in causing explosions and mine fires that a substitute was sought and has been found in permissible explosives. Each coal mine in Illinois in which permissible explosives are used was visited and a study made of the problems involved. The report deals briefly with the dangers of black blasting powder now in such common use in coal mining. It discusses the development of permissible explosives and their introduction into Illinois coal mines and points out the characteristic differences between permissible explosives and black blasting powder. The present system of blasting and the most approved methods of using permissible explosives are described. Data are given to show that not only has safety and efficiency in these mines been greatly improved, but that the cost of explosives to the miner per ton of coal has been reduced, and that sizes of coal produced from permissible explosives compare favorably with those previously obtained at the same mines with black blasting powder. The successful use of permissible explosives in these Illinois mines as well as in many other mines throughout the country should aid in their introduction into a larger number of mines and bring about that degree of freedom from explosions and fires which should be attained in mining.

CHINA can he roughly divided into three metallogenetic province: North China, the Yangtze Valley, and South China. In North China the old Pre-Cambrian schists and gneisses are represented by the abundance of quartz mines in which sporadic occurrences of copper, lead, zinc, gold, and silver are sometimes found in subordinate quantities. Along the Yangtze Valley and in Central China, extending eastward as far as Shantung, are found here and there intrusions of dioritic rocks, in which occur the famous contact deposits of iron ore, besides which minor quantities of copper. lead, and zinc are also found. In South China the igneous intrunsions are of granitoid type in which, or in the vicinity of which, occur in certain regions immense deposits of antimony, tungsten, tin, and mercury. Broadly speaking. most of the famous coal mines are now situated in regions north of the Yangtze River, in which occur also the sedimentary deposits of iron in contradistinction to the contact deposits of iron along the Yangtze Valley.

Protective canopies for conventional electric face equipment, including shuttle cars, operating in low coal (less than 48 inches) were designed, fabricated, installed and evaluated on in-mine machines engaged in regular coal mining operations. This work was performed under contract to the U.S. Bureau of Mines, Pittsburgh Mining and Safety Research Center. The canopies will support a vertical load of 18,000 pounds and provide protection against both roof falls and contact with the mine rib. Canopy configurations were established based on human engineering studies, full scale canopy mockups, and in-mine observation and consultation with mine and equipment manufacturing personnel. Overall height of each canopy is hydraulically adjustable between a minimum of 32 inches and a maximum of 40 inches. Canopy tops are open framework, welded structures, covered with expanded metal mesh. Structural members are 1 3/4-inch or 2-inch square tubing of AISI 413ON and ASTM A500 material, respectively. The canopy tops are typically supported by an understructure and upright post arrangement that is either integrated into the current operator platform or attached to the machine frame. The four upright corner posts house the hydraulic actuators. Canopy tops are elevated horizontally. Canopies are welded or bolted to the existing machines. Installation is accomplished with a minimal amount of interface modifications. No major machine design changes are required. Canopies were trial installed and load tested at the manufacturer's facility prior to installation on the in-mine machines. Canopies were installed on a Joy 14BU10 Loader, 16RB-3AH Cutter, and two 21SC Shuttle Cars, and a Galis 300 Roof Drill and 460-S Face Drill. Canopy installations were evaluated in the Jewell Ridge Coal Company No. 12 and Big Creek Tiller Mines. Operational time accumulated on the installations during evaluation ranged from 1 week to 14 weeks of two-shift operation. All five canopy designs, manufacture, and test were successfully accomplished. The five canopies met all safety requirements. Three of the five canopies were deemed to be completely successful from an operator's point of view. That is, mine management as well as the machine operator found that the canopies did not impede the operation of the machines to function with the same efficiency as before the installation of the canopies. The loader canopy, in fact, appeared to have provided a mole efficient operation of the loader by providing added safety to the operator from rib contact thus allowing him to concentrate more fully on the loading function. Operational problems were encountered with the shuttle car and the roof drill. The shuttle car canopy obstructed visibility to an objectionable amount and the roof drill canopy impeded maneuverability of the machine, more than doubling the roof bolting time.

During these tests, certain apparatus variables are measured during rest periods. Because of concerns that the present tests may not accurately simulate the stresses to which an apparatus is subjected in the field, the Bureau of Mines proposed new human subject tests. It was also felt that different measurement techniques would supply more reliable data on the apparatus response to these stresses. The Los Alamos National Laboratory was asked by the Bureau to evaluate the proposed tests and monitoring techniques. The major conclusions derived during this study are these: (1) The proposed tests are not too physically demanding. (2) As long as human test subjects are used, a minimum subject weight should be required for certification tests. (3) Important apparatus variables are the breathing resistance, temperature, and carbon dioxide and oxygen concentrations of the inhaled gas. (4) Continuous monitoring should be used, rather than monitoring only during rest periods. (5) New pass/fail criteria based on realistic estimates of the user's physiological requirements, as tempered by present and future engineering limitations, are needed. (6) When available, breathing machines, rather than human subjects, should be used for most certification testing. (7) A more realistic ergonomics test than that proposed is needed. (8) A gas leak test for the system and a facepiece leakage test are also needed.
