Although researchers have struggled to replicate Heinrich’s safety triangle findings (Heinrich 1931) for various reasons (e.g., no access to his original database and it is unclear if the database was of a single establishment/operation or for an entire industry), the occupational safety community has continued to adopt and expand the applications of this theory (e.g., McSween 2003). Within the mining industry, the potential exists for operations and companies to use concepts from the safety triangle as a way to inform or challenge the practices employed within their health and safety management systems (HSMS) to prevent incidents (Backlund 2016). This paper considers data obtained from the Mine Safety and Health Administration’s databases and demonstrates the validity of applying the safety triangle theory at the mine level. The results support the use of this theory and demonstrate that lower severity incidents can predict fatalities in a subsequent year where significant nuances and caveats to applying the theory are identified and discussed.

During the major expansion of the Inspiration smelter, which took place in 1973, the bulk material handling system underwent a matching expansion in order to move the required materials. Inspiration is the first smelter worldwide to employ electric smelting in conjunction with the use of Hoboken converters (1), (2). The installation includes a 51 -MVA electric furnace, five 4.25m (14-foot) diameter Hoboken converters, rotary dryer, bedding plant, acid plant, heat recovery steam plant, and gas and dust handling system. See Figure 1, Plot Plan. As of today, the 51-MVA electric furnace is the world's largest in the smelting of copper. Engineering design for the installation was per- formed by Treadwell Corporation of New York. The task of designing the conveyor system was complex because the system had to be installed while the smelter was operating and with minimum plant shutdowns, and it had to serve both the existing and the new smelter. Existing conveyors were redesigned where necessary and incorporated into the new conveyor system. See Figure 2, Flow Sheet. The system encompasses the handling of such diverse materials as wet and dry copper concentrates, cement copper, silica flux, cold dope, and concentrate and flue dust.

Underground heat issues are mainly focused on the detrimental effects to a workers health. However, working in hot, humid conditions has other negative effects. Studies have shown that safety, productivity, workforce morale and operating cost are also worsened in these conditions. The human body has several mechanisms for rejecting heat and remaining cool. However, once the air temperature exceeds about 34°C, the only effective mechanism is by evaporation of sweat from the skin. This is assisted by acclimatisation which is the human body?s enhanced ability to combat extended exposure to heat. The body has to remain well hydrated when working in heat as even small degrees of dehydration cause significant decreases in the ability to work in heat. There are two ways in which heat load can be reduced: Through use of refrigeration/vent/cooling to reduce thermal stress on workers; Reduce the contribution that heat sources have to underground heat load (e.g. reduce idle time, fleet size, etc). Some of the sources of underground heat have the potential to be more manageable than others. Geothermal heat and heat from autocompression will be found in all mining situations in varying degrees depending on geothermal gradient and surface temperature, however, the use of equipment and blasting may be streamlined to reduce the underground heat load. It is through a combination of the available options that most mines will successfully control their heat related problems. Mt Isa, which has been a focus for heat related study in Australia, has successfully implemented protocols for working in heat. Through combining these with the installation of a surface bulk air cooling plant, Mt Isa has consistently reduced the incidence of heat illness. This is likely to be the most cost effective method of reducing heat load for deep mines in Australia. Shallower mines which may only occasionally encounter hot conditions should consider more portable cooling options. This study will review the heat issues in underground Australian metalliferous mines.

Adaptive management can be described as a structured, iterative process of robust decision making in the face of uncertainty, with a goal of reducing uncertainty over time through system monitoring. Adaptive management is a tool which can be applied to not only change a system, but learn about the system (Holling, 1978). Adaptive management has been used in ecosystem management for years and guidance is provided in a variety of handbooks and publications. Adaptive management can be applied to industrial facilities and mines through the use of an adaptive environmental management plan (AEMP) and associated practices. The goal of this paper is to provide an explanation of how the plans and practices support regulatory agency approvals and permitting, span the life-cycle of mines, manage uncertainty, and assist in addressing mining impact and legacy issues. AEMPs and associated practices are useful tools to address the uncertainties in identifying, evaluating, and managing impacts of proposed new and modified mining facilities undergoing environmental review. Once a proposed project is approved, the AEMP continues as a living document, being informed, revised, and adapted by the operations. The AEMP can serve as an ongoing management tool for describing and evaluating environmental impacts, continuing in operations into post reclamation. If initiated during planning and environmental review of a project (and in some locations such as the United States [U.S.], the preparation of an Environmental Impact Statement (EIS)), the AEMP becomes a basis to identify, monitor, and manage potential project environmental impacts. Development of an AEMP for an operating mine is also beneficial since there are generally developing and ongoing environmental issues at those facilities as well. Over time, the AEMP is adapted to changing requirements and conditions including those anticipated and unanticipated. Ever increasing scrutiny of mining projects and the complexities of the EIS process highlight the uncertainties inherent in impact predictions. In making the momentous decision of initial project approval, these uncertainties can discourage the regulatory agencies? actions and create stakeholder anxiety and controversy about a project. A well-designed AEMP can support the decision to approve an EIS and enable a project to move forward. Additionally, an AEMP can inform financial assurance estimates and revisions over the life of a mine providing all parties including the citizens of the surrounding communities with greater confidence that potential environmental impacts and risks will be well managed.

A major process control system is in some ways like a complex organism. The extremities, (that is, field instruments) are exposed to many hazards and suffer malfunctions of relatively minor and repetitive nature. Here I equate a stuck valve or dead transmitter with a skin abrasion or cut. The maintenance and repair of the extremities can be done by technicians with basic but thorough knowledge of relatively few problems and their solutions. Here I equate with administering Band-Aids and aspirin. On the other extreme, when the master control device (such as a computer) fails, one does not attempt home remedies. A "brain" specialist is called in. It is, however, crucial that the patient be able to clearly describe his symptoms. Getting down to the case at hand, a complex system consisting of field instruments, instrument racks, interface equipment, and perhaps a computer, may require maintenance at all levels. At the field equipment end where physical damage, corrosive atmospheres, reconfiguration, and routine calibration checks are common, the customer has the opportunity to develop a competent staff of maintenance people. The relatively high maintenance activity assures competence through repetition.

"As human beings, we have managed risk since the dawn of the ages and have been adept at doing so. In order to survive, we hunted or gathered. If we did not take enough risk, we died. If we took much risk, we died (Fig. 1). As individuals and tribes, risk had to be optimized to maximize our chances to thrive, as individuals and as tribes.In today’s world, the risks are vastly different, but in order to thrive, these must be managed. And no industry knows this as much as mining. In fact, the International Council of Mining & Metals (ICMM) has proposed the Critical Risk Model (Fig. 2) as a means of identifying critical risk, critical controls and associated performance standards. The ICMM focuses on sustainable development performance in the mining and metals industry. It has 23 members, representing some of the largest mining companies in the world.This approach is going to help the mining industry. But it must be applied effectively, with the acknowledgment that the system alone cannot be effective. We can say this confidently, as even companies known for the strength of their risk and safety management systems continue to experience significant incidents, and, in some cases, catastrophes. Why does that happen when all safety metrics, including injury rate, are pointing in the right direction? In the view of the author, it is only when leadership, culture and systems work in complete harmony that the resilience of an organization to manage its significant risk is at an optimal level. It remains an ongoing leadership challenge to maintain that balance and the optimal mindset in the organization. So how do you maintain that special balance?Let’s start with a basic premise. Leadership, supported by simple effective systems, creates culture which creates outcomes. Everyone going home safely is an outcome, as is cost effective operations, and to achieve those outcomes efficiently, the organization must focus on the inputs. And those inputs include effective leadership, effective systems (for the purposes of this article, risk and health, safety and environment (HSE) systems), good planning, stable operations and competent workers. This article will focus on gaining maximum performance from your risk management system."

The surge of activity in the solar energy market, spurred by government incentives, has created new and more efficient solar technologies, but issues surrounding land- and water-use have backlogged the full-scale development of solar farms. This paper explores siting solar power plants on degraded or polluted lands, such as mine tailings impoundments. Historically, mining and has been integral to Arizona?s economy, but has dwindled in relevance due to urban pressures and environmental regulations. Currently, it is legal to site solar power plants on degraded or polluted lands, but the question remains whether the economics of this degraded lands solar development is more favorable than developing pristine land. Therefore, we explore economics of developing mine tailings impoundments at both the institutional and financial level. Methods of research for this paper were interviews with government regulators, environmental advocacy organizations, farmers, and mining company executives. Financial models using extant tax incentives show a positive net present value for the installation of solar power plants, even when accounting for the costs of land remediation.

Accurate prediction of the partition coefficients in coal gravity separation is key to improving the preparation efficiency of coal and promoting its clean utilization. In China, the approximate formula method (AFM) has been applied to the calculation of coal gravity separation partition coefficients for decades, but its accuracy is limited. To improve the accuracy of the predicted partition coefficients, and thus the product yield of coal preparation plants, the present study proposes a modified hyperbolic tangent model to derive direct calculation formulas for the heavy medium and water medium separation partition coefficients. Four groups of coal samples were used to verify the accuracy of the direct formulas, and their predictions were compared with those of the AFM. The results showed that the partition coefficients and partition curves predicted by the direct formulas were closer to the actual values than those predicted by the AFM. Further, when the cleaned coal ash content was basically the same, the cleaned coal yield predicted by the direct formula was lower than that predicted by the AFM, and was also closer to the actual value. These results demonstrate that the direct formula produces more accurate results compared to the conventional AFM. As this formula has a reasonable theoretical basis, is simple to apply, and produces accurate results, it is expected to help improve the design and management level of coal preparation plants.

Monitoring slope displacement in open-pit mines can help predict collapses and prevent serious damage, injuries, and fatalities. Early detection of movement may indicate the need to modify slope geometry, initiate dewatering if the slope is wet, or consider artificial support measures such as cable bolting. Slope monitoring at the Berkeley pit was carried out with varying degrees of intensity and continuity for about 15 years. Some of the old mine records are still intact and accessible. Slope monitoring data can therefore be collected and published for the benefit of other mine operators. Because prediction of slope failures is empirical in nature, publication of several well documented case studies will be a valuable contribution to the industry. Background - Berkeley pit Open-pit mining of copper from the Berkeley pit, located on the east side of Butte, MT, began in 1955 and ceased in 1982. During that time, about 1 Gt (1.2 billion st) of material were removed to form a pit about 1.8 km wide by 2.1 km long (6000 ft by 7000 ft) at the rim, and 550 m (1800 ft) deep as measured from the high hill on the northwest side. From the south side, the pit only measures 365 m (1200 ft) deep. The Berkeley pit was first owned and operated by Anaconda Minerals Co., and subsequently by Arco. Following its closure in April 1982, the mine began to fill with water. Washington Construction purchased the property in 1986, and Montana Resources Inc. began mining to the east of the Berkeley pit in July 1986. Numerous instances of slope failure were recorded during the course of mining and several intensive geotechnical investigations were conducted in an effort to optimize the mine design. One of the last and most comprehensive studies was initiated in 1977. For purposes of geotechnical analysis, the mine was divided into nine design sectors on the basis of geologic conditions, geographic orientation, and economic importance (Fig. 1). Only two of these sectors - the concentrator and the southeast corner - are considered here. Material properties were obtained from several studies of samples collected from the mine and adjacent areas. The status and outlook for mining, in your view. Minerals remain a major part of the world’s economic foundation. The fortunes of the mining industry rise and fall with general economic trends, but mining is no less important today on a global basis than it has been historically. Certainly, portions of the industry are critically depressed — domestic copper to name one area —but other sectors are prospering. Mineral consumption, both worldwide and in the United States, has increased steadily since the Great Depression, hitting all-time highs in 2000, the last year for which I have seen data. The highly developed economy in the United States tends to obscure the continuing importance of minerals here. But in less developed nations the critical nature of minerals is obvious and well accepted. In short, although there is no shortage of problems or challenges facing mining — there never has been — mining remains an essential business with great global opportunity.

In February of 1989 the Mission Complex of ASARCO Incorporated completed an expansion of its concentrator, raising the base mining rate from 25,900 TPO (28,500 STPD) to 37,200 TPO (41,000 STPD). The expansion was accomplished by extending the twelve existing Mission ball mills in length, by installing the grinding circuit from Asarco's idled Sacaton Concentrator along with various auxiliary equipment, and by modifications to the flotation circuit and water systems. This paper will give a brief history of the consolidation of the ore body which made the expansion possible and explain the various parts of the expansion.

This geometallurgical study examines the gangue mineral variability at the Bingham Canyon porphyry copper mine in Utah. Understanding an orebody?s mineralogy is important for mine and mill planning because it provides information on the size, texture, and distribution of ore minerals, the relationship between ore and gangue minerals, and the extent and type of alteration. Quantification of the mineralogical variability and properties of an orebody provides valuable input to the ore model and may have a significant effect on recovery optimization. This study was designed to be carried out on an initially detailed scale in order to develop a robust methodology for measuring and quantifying specific parameters, such as the distribution of mineral phases, alteration assemblages and fractures from key ore domains at the Bingham Canyon mine (Figure 1). Details of the methodology are reported below. This technique can be applied to testing the impact of mineralogy on a number of processing parameters. [ ] Figure 1. The samples for this study were collected from the northwest part of the deposit circled on the map. PROJECT BACKGROUND Geology The Bingham Canyon deposit is a large porphyry Cu-Mo-Au deposit with associated skarn and base-metal replacement deposits mined in an open-pit mine measuring about 2.5 miles across and 4,000 feet deep. The deposit is hosted by the Bingham Stock, a Late Eocene to Mid-Oligocene intrusive complex. The skarn is hosted by the Late Carboniferous-age Bingham Mine Formation, which is comprised of quartzites and basal limestones (Babcock Jr. et al., 1998). The Bingham Stock is a composite intrusive complex that was emplaced over a period of about two million years. The oldest phase is described as a fine-grained equigranular monzonite (MZ), which intruded the country rocks at 39.8 ± 0.4 Ma (U-Pb zircon TIMS age) (Parry et al., 2001). Subsequently, the quartz monzonite porphyry (QMP) was emplaced, followed by intrusion of a latite porphyry (LP). The final intrusive phase, which forms narrow dikes that crosscut the Bingham Stock, is represented by a quartz latite porphyry (QLP), which is dated to 37.5 ± 0.4 Ma by 40Ar/39Ar dating of sanidine (Deino and Keith, 1998). Also present, are a minette dike in contact with a QLP dike (Waite et al., 1998; Deino and Keith, 1998), and a biotite porphyry (BP), a quartz latite porphyry breccia (QLPbx), and a pebble dike (Redmond, 2002). The copper orebody is centered above the Bingham Stock and forms an inverted cup shape with three high-grade copper zones extending to depth. Molybdenite mineralization occurs within and extends below the copper orebody, but is not correlated with copper mineralization. The main sulfide assemblage is chalcopyrite, bornite, molybdenite, and pyrite. Gold is locally intergrown with chalcopyrite and bornite (Babcock Jr. et al., 1998; Phillips et al., 1998). The surrounding country rocks host skarn deposits with three main ore types - garnet skarns, massive sulfides, and iron oxides. The garnet skarns contain chalcopyrite and pyrite with magnetite veins. In the massive sulfides chalcopyrite and pyrite occur either in banded sulfides or in unconsolidated microbreccias. The iron oxides contain disseminated chalcopyrite with minor chalcocite and bornite (Harrison and Reid, 1998). The deposit is overprinted by several stages of alteration. Potassic alteration is concentrated at the center of the deposit. It is surrounded and partially overlapped by a propylitic alteration zone. Both zones are overprinted by sericitic alteration. In the northern part of the deposit evidence for argillic alteration can be found (Bowman et al., 1987). Similarly, the skarn deposit was also overprinted by several stages of alteration: wollastonite, idocrase, and garnet in cherty limestones, and quartz and diopside forming in calcareous quartzites. Distally, diopside was replaced by talc, tremolite, dolomite, secondary calcite, and pyrite, and quartz and sulfide veinlets with biotite and actinolite selvages are present. Andradite garnet, diopside, quartz, magnetite, hematite, and copper sulfides were overprinted by wollastonite (Babcock Jr. et al., 1998). Because the monzonite, quartz monzonite porphyry, latite porphyry, and quartz latite porphyry host the bulk of the mineralization, our research is focused on these lithologic units. Geometallurgy On the basis of geological (lithology, alteration assemblages, fracture density, copper mineralization) and metallurgical properties, the deposit has been divided into 33 ore domains by mine geologists and metallurgists (Figure 1). In map view, these domains cross geologically defined boundaries (MZ, QMP etc.). These domains reflect regions of the deposit that have particular, mapable attributes or properties. They reflect a combination of geological and mineralogical characteristics that combine to impact processing. Alteration, for example, as shown by talc mineralization, is one such key feature. This paper presents the optimum auxiliary fan(s) location to control air recirculation in dead-end workings where dieselpowered vehicles operate. Investigations were conducted with various secondary fan locations from the dead-end crosscut with varying the intake air quantities using a 30 m3/ s capacity twin 75 kW auxiliary fan and 45 m3/ s capacity twin 110 kW auxiliary fan to control air recirculation and DPM. The results showed that if the drive intake airflow rate matches the fan capacity, air recirculation will occur even when the fan is located 10 m away from the crosscut entry. Results also showed that if the intake drive air quantity was greater than or equal to 150% of fan capacity, no recirculation was observed when the twin 75 kW fan location was at least 5 m and the twin 110 kW fan location was at least 10 m away from the dead-end crosscut access.

Talc is a layered, hydrous magnesium silicate mineral. It has a soft, soapy feel and typically a smooth texture. Talc is also known for its insulation, heat resistance, chemical stability, oil absorption and strong covering quality. Talc, Mg3Si4O10 (OH)2, has a theoretical chemical composition of MgO at 31.7 percent, SiO2 at 63.5 percent, and H2O at 4.8 percent. However, talc’s chemical and mineralogical composition can vary depending on its geological history/parent rock association. These mineral associations and variable levels are usually chlorite and carbonates (magnesite, calcite and dolomite). Two key elemental substitutions that can occur in the talc crystal structure are iron for magnesium and fluorine for hydroxyl. These compositional differences may limit or enhance the talc’s usage in specific market niches. The United States remains self-sufficient in most grades of talc.

Experience at Energy West Mines in the Wasatch Plateau Coal Field, Utah, has shown that longwall face bursts occur mostly near the tailgate when extracting the third adjacent longwall in a panel section where depths exceed 610 m (2000 ft). It is believed that this is due to a combination of cover depth and side abutment stresses created by the widening of a pressure arch formed by interlocking and bridging of large blocks of" very competent strata. The 60-m-thick (200 ft) Castlegate Sandstone probably plays a key role in pressure arching. This paper presents a back-analysis of the location and magnitude of the stresses associated with face bursts that occurred when mining the 7th Right Longwall at the Deer Creek Mine. Improvement of these conditions may be achieved by widening the longwalls to attain critical subsidence width sooner while mining the second long-wall, and to reduce stresses at the face. Wider longwall faces, however, increase the chance of sandstone channel interception often associated with bursts. Satisfactory mitigation of face bursts beyond cover depths of 610 to 670 m (2000 to 2200 ft) may be difficult without using destressing techniques. The hydraulic fracturing/liquid infusion technique seems to offer the best possibilities for application in longwall mining.

A requirement for geological controls during orebody modeling results from the recognition of heterogeneous geological characteristics and/or abrupt variations in orebody geometries. While purely statistical methods are often sufficient for modeling uniform homogeneous deposits, and the only recourse for many that are extremely complex, geological interpretations can be extremely valuable as controls during the modeling process. The advent of relatively inexpensive, sophisticated graphical computer hardware and software has focused considerable attention on the three dimensional visualization capabilities now available to the mining industry. Orebody geometries can be easily imported into many systems as wireframe or solid models, rendered with shading and color, rotated in space, exploded into composite parts, zoomed into and otherwise displayed to awe any audience. The requirements for an initially meaningful, accurate, geological interpretation are sometimes lost when the focus is on an apparently complex, highly resolved end product. Details of the initial data and sensitivities of modeling assumptions are not always sufficiently scrutinized. Software capabilities have extended the geopoetic license into three dimensions. The new tools, while greatly facilitating model construction and rendering, have done little to expedite basic data preparation and analysis. This paper attempts to briefly summarize the purpose and method of the construction of geological controls. The examples selected are from wire­framing and block modeling; application software processes commonly used in the mining industry today.

Newmont Mining’s Ahafo Mine is located in Ghana approximately 300 km (190 miles) northwest of the capital city of Accra in the Brong-Ahafo region. The nearest city is Sunyani, located 45 km (28 miles) to the north. The Ahafo Mine will process ore from seven major pits that extend along a 40-km (25-mile) strike. Four pits are in the south and three pits are in the north. The resource is currently stated at 373 t (12 million oz). But it still has the potential for growth both from new discoveries and the development of underground operations.

In Part I of this paper, it was established that the width of longwall faces in highly gassy mines is primarily limited by the volume of air reaching the tail-end of the face. The ventilation air reaching the tail-end of the face should be not only enough to keep methane concentrations below the statutory limits (generally 1 percent) but also enough to prevent any gas layering. The air volume leaking from the longwall face into the gob should also be able to push the explosive methane-air mixture away from the gob area immediately behind the longwall face where active roof-fall is taking place (typically 30-45 m). The second group of variables that also controls the width of the longwall panel consists of (a) the rate of advance, (b) bleeder air quantities, (c) specific methane emissions for the gob area and, (d) efficiency and cost of gob gas capture. This paper will discuss the second group of variables and its impact on the optimum width of longwall faces in highly gassy coal mines.

Chemical and mineralogical data on 25 apatitic phosphate ores of diverse origins from commercial deposits around the world were widely variable. From 14 to 18 chemical constituents of each sample were determined, and each sample contained 4 to 10 mineral phases. Altogether there were 34 different mineral phases, many of which occurred in only one sample. The samples contained varieties of francolites, fluorapatites, and mixed hydroxyl-fluorapatites. The apatite compositions calculated from the chemical data showed that certain types of substitutions were characteristic of different apatite varieties, but every variety had some degree of substitution of (CO3)2 for (PO4)3 and the P2O5 contents of the apatites ranged from 35.1% for a North Carolina apatite to 41.9% for a Missouri apatite. In a concentrate with a grade of 32% P2O5, the North Carolina sample should contain about 8.7% gangue and the Missouri sample about 23.7% gangue. To optimize the digestion process of a phosphate are, it is necessary to consider the dissolution behavior of the different mineral phases during acidulation and the effects they may have on the chemical properties of a fertilizer intermediate. In the production of wet-process acids or superphosphates, the acidulation of certain minerals may cause foaming and formation of sludge, filtration, corrosion, and handling problems.

The health and safety of miners is critical in the industry. Prolonged exposure to dust-laden air results in irreversible ailments like pneumoconiosis that have a severe impact on their lives. Operations use various personal protective devices (PPEs) to alleviate their exposure to respirable dust. Powered Air Purifying Respirator (PAPR) is one such device that uses high- efficiency filters and positive pressure masks to shield miners from airborne contaminants. Used in the medical and other industries, these respirators efficiently protected healthcare workers during the COVID-19 pandemic. Recently, the mining industry has shown a growing interest in adopting this PPE to ensure safe working conditions for the miners.
