Computer-related accidents have caused injuries and fatalities in mining as well as other industries. Normal accident theory (NAT) explains that some accidents are inevitable because of system complexity. NAT is a classic argument in organizational sociology although it has been criticized as having imprecise definitions and lacking criteria for quantifying complexity. These limitations are addressed by a unique approach that recasts this organizational theory into an engineering-based methodology to quantify NAT complexities of computer-based systems. In this approach complexity is categorized as external or internal. External complexity is defined by the external behavior of a system, and is quantified by these dependent variables: system predictability, observability, and usability. Dependent variable data contain the perceptions of 32 subjects running simulations of a system. The system?s internal complexity is characterized by modeling system-level requirements with the software cost reduction (SCR) formal method. Model attributes are quantified using 15 graph-theoretical metrics?the independent variables. Five of 15 metrics are correlated with the dependent variables as evidenced by structure correlations exceeding 0.25, with standard errors <0.10 and a 95% confidence interval. The results also show that the system predictability, observability, and usability decreased as NAT complexities increased. This research takes a step forward in operationalizing NAT for computerized systems. The research benefits mining and other industries as well.

Bull Lake is one of several saline lakes situated a few miles north of Rawlins, Carbon County, Wyo. (figs. and 2).. The Bureau of Mines conducted preliminary investigations of sodium sulfate deposits in many of the lakes and performed detailed work on Bull Lake. This investigation consisted of drilling 11 vertical holes through salt beds using diamond core bits and Iwan-type auger bits. Bull Lake lies in a shallow, closed depression in loosely consolidated sand. The salt beds, ranging in thickness from 2.5 to 26 feet, occur above a thick bed of shale of upper Cretaceous age. The salts appear to have been deposited in the lake from surface and subsurface drainage. The source of salt is undoubtedly the enclosing sedimentary rock&apos;s exposed to the west of the basin. Sodium sulfate in the deposits probably is present as mirabilite or Glauber salt (Na2SO4-10H2O). Chief impurities in the deposits arc mud and sand. Other soluble salts present average approximately 1 percent combined. Cores from the 11 holes were analyzed in the Bureau of Mines laboratories at Salt Lake City. Drilling totaled 512 feet. Results of analyses are given in the appendix.

A practical method for removing the phosphorus would be valuable to the iron and steel industry. Gravity concentration has been of little value as far as the removal it of phosphorous is concerned, because the compounds of this element are, as a rule, disseminated very finely. Removal of the phosphorous by leaching is not a new idea and has been the subject of some investigation,3,4/ but few data have been published. W. R. "INTRODUCTION Compressed air for operating coal-mining equipment is seldom considered in conjunction with modern methods and appliances. But the Jamison Coal & Coke Co. has proved the economy and efficacy of compressed air for this purpose at its Mine 8 near Farmington, 8 miles west of Fairmont, Marion County, W.. Va., and the mining industry as a whole concedes its superior safety in gassy or dusty coal mines.The uses for compressed air in coal mining are now generally associated with such equipment as coal punchers, chain-breast machines, and to a lesser extent the ""radialaxe"" type of mining machines.Electricity has rapidly superseded compressed air for power in coal mines during the past 20 years; and in the last few years the general, and often unwise, use of electricity at the face region has taken on added impetus through the widespread use of mechanical loading equipment.Some of the unfavorable results of this increased use of electricity are shown in the growing number of mine explosions and fires of electrical origin. In the past four years 59 explosions of this type took the lives of 672 men, or 74.8 per cent of the total number of deaths from explosions were due to explosions of electrical origin.3In 1926, an explosion in Mine 8 of the Jamison Co. killed 19 men. The exact source of ignition was not determined, although it was definitely decided that it was of electrical origin and probably was at an electric coal-cutting machine.Prior to this disaster the company had done some experimenting with compressed-air-driven mining machines, and its Mine 9 had been completely equipped with air-driven chain-breast machines; a few of these machines were also in use in Mine 8. The explosion, however, gave impetus to the adoption by this company of compressed air for operating face equipment. The chain-breast machines were as satisfactory as electric machines of the same type; but the trend in the industry was toward faster cutting machines, which required less physical exertion on the part of the operating crews. A track-mounted machine of the arcwall type was found to be suitable for the conditions and system of mining, so that one of these operated by compressed air was developed by the Jeffrey Manufacturing Co. working with the officials of the Jamison Co.The first of these machines was installed in the latter part of 1926 and additional machines were delivered from time to time, until at present there are nine of them in the mine. Necessary improvements have been made, with the result that a fast-cutting, easily handled, efficient, and safe mining machine has been developed for cutting the hard Pitts¬burgh bed coal of the Fairmont field."

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. Approximately half of all the fuel energy used in the United states is now pro-vided by oil and natural gas. A persistent and rapid growth in energy demand during the last 3 decades has been absorbed largely by the liquid and gaseous rather than solid fuels. In 1948 the energy provided by coal was substantially the same as in 1920. Meanwhile, the use of oil and natural gas each had increased fivefold. On a per capita basis, the change is more significant. Coal consumption has dropped from about 6 tons per person to 4-1/2 tons, while oil has increased from 4-1/3 barrels to 14-1/3, and natural gas from 7,500 cubic feet to about 35,000. These facts would have only academic interest if the United States had unlimited supplies of all fuels. However, while the Nation&apos;s reserves of coal are adequate to meet our needs for centuries, those of oil and natural gas appear limited. It is obvious, therefore, that the security of our liquid-fuel position would be greatly enhanced if the supply could be based in an increasing degree on solid fuels, such as coal and oil shale, which make up more than 95 percent of our fuel reserves, instead of natural oil and gas. In addition, this would create new employment and help to stabilize the market for coal, which may be facing comparatively bleak prospects in the near future unless its position with respect to other fuels is improved.

This Bureau of Mines publication summarizes the status of mine electrical equipment reliability research. Failure rate data were estimated from theoretical, laboratory, and field studies. Tables are presented listing reliability characteristics of molded-case circuit breakers, undervoltage relays, ground check monitors, cables, motors, and control circuitry. Application of failure rate data is shown in tables listing maintenance scheduling intervals of components and components that are most sensitive to the probability of hazardous voltages on power distribution systems in the mines. Complete, accurate, and appropriate failure rates on mine equipment are of the utmost importance for reliability evaluations.

The Greenburg-Smith impinger was developed at the Bureau of Mines in cooperation with the United States Public Health Service in 1922 in the course of an investigation of dust-sampling instruments available at that time. Although it is not used extensively outside of the United States, it is now soused in this country to collect samples for the determination of any dustiness, and most of the information correlating degree of dustiness with physiological effect has been obtained by the impinger method. The chief disadvantages in the use of the regular impinger, especially in mines and other places accessible with difficulty, are its bulk, weight, and power requirements. It requires a source either of electricity or of compressed air, which necessitates connections and extension of existing supply lines to each sampling point; the only alternative is the use of a relatively large pump hand-operated with considerable effort.

Bahrain&apos;s oil revenue increased from $337 million2 in 1916 to $378 million in 1977. Despite the increase in value, petroleum receipts made up only 584 of all Government income in 1877, compared with 74% in 1976 and 83% in 1975. Bahrain became an oil producer in 1932 with the discovery of the Awali oilfield, and since that time, the Awali oilfield has produced over 600 million barrels of crude oil. Awali production in 1977 was 21.2 million barrels. Bahrain shares the revenue from the Abu Safah oilfield, located in the Persian Gulf north of Bahrain, equally with Saudi Arabia. Revenue from the Abu Safah oilfield was approximately $179 million or 47% of oil revenues in 1977. Output from the Abu Safah oilfield was 48 million barrels, an increase of nearly 30% over 1976 output. The Bahrain Petroleum Company (BAPCO). the nation&apos;s only oil-producing company, was owned 40% by California Texas Oil Corp. (CALTEX and 60% by the Govern¬ment. The Government established Bahrain National Oil Co. (BANOCO) in 1976 to administer the Government&apos;s share of BAPCO, oversee exploration concessions, and manage domestic marketing of petroleum products. The Government announced its intention to acquire 100% control of crude oil and natural gas production. CALTEX was to retain complete control of the 250,000-barrel-per-day oil refinery near Sitrah.

The petroleum industry continued to be the most important mineral activity in Bahrain during 1978-79. Revenues generated by the petroleum sector increased from $378 million&apos; in 1977 to an estimated $419 million in 1978 and approximately $522.5 million in 1979, despite decreasing production over the 2 year period. There were two oilfields that produced revenues for Bahrain, the Awali oilfield, the nation&apos;s only onshore producer, and the Abu Sa&apos;fah oilfield, located in the Persian Gulf, north of Bahrain, The Await oilfield has been decreasing output from a high of 27,973,000 barrels in 1970 to 19,000,000 barrels in 1979. Bahrain shared the revenues from the Abu Sa&apos;fah oilfield with Saudi Arabia, as settlement of an ownership dispute. For the first time, in 1978, revenues from the Abu Sa&apos;fah oilfield represented over 50% of the total receipts from the petroleum sector. The years 1978-79 saw the gradual takeover of the Bahrain Petroleum Co. (Bapco), formerly a subsidiary of the California Texas Petroleum Co. (Caltex), by the Government-owned Bahrain National Oil Co. (Banoco). By yearend 1978, Banoco owned 60% of Bapco, and Caltex the remaining 40%. The Government-owned company controlled exploration concessions, managed domestic marketing of petroleum products, and controlled natural gas production and marketing. On December 15, 1979, the Bahrain Government completed full takeover of the Bapco operations. Bapco will continue to own and operate the 250,000-barrel-per-day oil refinery located near Sitrah, as well as handle foreign marketing of petroleum products.

In connection with the cooperative metal-mine dust and ventilation investigations of the U. S. Bureau of Mines and the U. S. Public Health Service , some underground data have been obtained with intent to ascertain the adaptability of the katathermometer for determining the comfort of working places in mines . This preliminary study indicates that the instrument will probably be useful for making routine determinations of comfort conditions in our mines , and that it may also prove an important accessory for investigative work on problems in ventilation and kindred subjects ; in both coal and metal mines , although the instrument should probably be altered somewhat for maximum utility under the varied conditions in the mines of the United States . The katathermometer is an instrument contrived by Dr. Leonard Hill , an eminent English physiologist , and its various features have been fully described by him in his book entitled " The Science of Ventilation and Open Air Treatment . " He made exhaustive experiments and concluded that the rate of cooling of the human body was the controlling factor in determining comfort or discomfort , and then set out to make an instrument that would measure this rate of cooling . The resulting instrument the katathermometer measures its own rate of cooling when its temperature approximates that of the human body and thus serves as an index of the rate of cooling of the body itself . - The instrument consists of an alcohol thermometer having a cylindrical bulb 1 inches long and 3/4 inches in diameter, and a stem about 8 inches long graduated from 95 to 100° F. This temperature range gives a mean temperature of 97.5° F. ( 36.5° C. ) , which is approximately that of the clothed body surface . The stem terminates in a small reservoir which serves the double purpose of acting as a safety overflow in case of overheating the bulb , and of insuring a regular rate of cooling by the time the meniscus has fallen to the 1000 point .

The Bureau of Mines analyzed the structure of a high-grade metallurgical coke, carbonized at different temperatures in the range 500° to 1,000° C, through X-ray diffraction studies. Growth in the crystallite size with increasing coking temperature was calculated from profile analysis and compared with values obtained from the radial distribution function. The carbonization temperature appeared to have little influence upon the stack height, whereas the layer diameters more than doubled in size between 500° and 1,000° C. These results are comparable to those obtained from other studies of poorer quality cokes.

This section of the Minerals Yearbook reviews the minerals industries of 27 countries: the 12 nations of the European Community (Belgium, Dermark/Greenland, France, Germany, Greece, Ireland, Italy, Luxembourg, the Netherlands, Portugal, Spain, and the United Kingdom); 6 of the 7 nations of the European Free Trade Association (Austria, Finland, Iceland, Norway, Switzerland, and Sweden); Malta; the 8 Eastern European economies in transition (Albania, Bulgaria, Czechoslovakia, Hungary, Poland, Romania, and Yugoslavia); and the U.S.S.R. Western Europe Western Europe [loosely defined to include the 12 nations of the European Community (EC) and the 7 nations of the European Free Trade Association (EFTA)] is the single largest trading area and consumer of raw materials in the world. If Western Europe was at one time an important mining sector, it has now increasingly relegated the role of supplying minerals for its minerals processing industry to the more minerally endowed countries in North and South America, Africa, and Australia. In this regard, Western Europe is the most important determinant of raw materials consumption (and thus, indirectly, raw materials production). Western Europe has significant reserves of industrial minerals but has limited availability of metalliferous raw materials. It, therefore, imports significant quantities of the latter and ranks along with the United States and the U.S.S.R. in the production of refined metals. Nonetheless, there is an inherent tendency to underestimate the importance of Western Europe in the world of minerals, both as a minerals processor and raw materials buyer. The reason for this is that Western Europe is generally thought of on the basis of individual nations rather than as a whole. Viewed in this limited context, the United States, the U.S.S.R., and Japan appear to dominate the world economy. Viewed as one regional area, however, Western Europe includes the fourth (FRG), fifth (France), sixth (United Kingdom) and seventh (Italy) largest economies of the world, all bordering on one another. With the remaining 15 EC and EFTA countries, Western Europe has a land area approximately 40 % of that of the United States, exceeds the U.S. population by 50 % and has a gross domestic product about the same as that of the United States.

"The National Safety Competition conducted by the Bureau of Mines, United States Department of the Interior for the promotion of safety in mines and quarries, has just been closed for seventeenth consecutive year. Enrollment covered more plants than in any previous contest. Moreover, a larger volume of employment was represented than in the contests of previous years, and several new plants were enrolled for the first time. Six safety trophies are awarded annually in these competitions one to the leading plant in each of six groups. An anthracite, mine in Pennsylvania, a bituminous-coal mine in West Virginia, a metal mine in Tennessee, a nonmetal mine in Virginia an open-cut mine in Minnesota, and a quarry in Alabama won the 1941 trophies.To be eligible to win a trophy, a mine or quarry must have had at least least 25 men working under ground or in the pit for not loss than 100 days. The number of man-hours worked must total 30,000 or more. Mines or quarries having no accidents during the year are rated according to the number of man-hours worked. If there are no plants with accident-free records, the mines and quarries in each of the six groups are rated according to their accident-severity rates; the plant having, the lowest rate is declared the winner.The Sentinls of safety trophies presented to the winning mines and quarries in the six groups were donated by the Explosives Engineer magazine to promote safety in the mineral industry. They are held by the winning companies for 1 year or until won by other mines and quarries."

A real-time Diesel Particulate Matter (DPM) monitor has been developed on the basis of the successful National Institute of Occupational Health and Safety (NIOSH) designed Personal Dust Monitor (PDM) unit. The objectives of a recently completed Australian Coal Association Research Program (ACARP) study were to modify the PDM to measure the submicrometre fraction of the aerosol in a real-time monitoring underground instrument. Mine testing focused on use of the monitor in engineering evaluations to determine how conditions can be improved. Studies, including a selection during Longwall (LW) moves as described in this paper, demonstrated how DPM concentrations from vehicles fluctuate under varying ventilation and operational conditions. Correlation between the current SKC DPM measurement system and real-time DPM monitors were conducted and results from six mines show a correlation between elemental carbon (EC) and the new monitor DPM mass ranging from .51 to .81 with R2 >.90. These differences are suspected to be due to variations from mine to mine in aspects such as mine atmospheric contamination, vehicle fleet variations, fuel type, engine maintenance, engine combustion efficiency, engine behaviour or interference from other submicrometre aerosol. Real-time monitoring readily reflects the movement of individual diesel vehicles and allows pin-pointing of high exposure zones such as those encountered where various vehicles work in areas of constrained or difficult ventilation. DPM monitoring approaches that have been available for some time based on shift average monitoring do not readily allow successful engineering evaluation exercises to determine acceptability of pollution levels. Identification of high DPM concentration zones allows efficient modification of mine ventilation, operator positioning and other work practices t o reduce miners? exposures without waiting for laboratory analysis results.

The Bureau of Mines conducted electric furnace smelting tests on sintered flotation concentrate and siliceous log-washed are. The flotation concentrate was derived from the Maggie Canyon deposit of the Artillery Mountains area, Mohave County, Ariz., and contained, before sintering, 35.6 percent Mil and 19.4 percent Si02 + A1203. The log-washed ore was from the McGregor deposit of the Artillery Mountains area, Mohave County, Ariz., and contained 31.2 to 33.6 percent Mil, 19.5 to 27.6 percent Si02 + A1203, and 2.5 percent Pb. This work was undertaken to determine the most effective smelting technique for producing standard products from such nonspecification ore and concentrates. The objective was to collect data, using a small furnace, which would be reasonably accurate and permit extrapolation to large-scale practice for comparing with results on specification-grade are.

The National Institute for Occupational Safety and Health (NIOSH) has conducted research to improve the ventilation of large-opening mines. The research has demonstrated that the ventilation of large-opening mines improves significantly by including ventilation requirements in the mine planning process and implementing somepractical techniques during mining. Planning the location of long pillars to reduce the number of crosscuts between intake airways and return airways, positioning auxiliary fans in their proper locations, and establishing ventilation stoppings with minimal leakage all significantly improve the ventilation of large-opening mines. Mine operators that adopt these techniques in the mine ventilation plan will reduce mineworkers’ exposure to airborne contaminants.
