Contemporary approaches to safety management appear to be failing short of meeting its mark in improving mine safety. This is evidenced by the high workers compensation, high incidence rates and fatalities. Evidence from high-risk and complex organisations points towards safety culture as being important in improving site safety. In more recent years resilience engineering has been touted as a new and innovative way of managing safety. This paper reviews and synthesises previous literature on safety culture and resilience engineering. It then highlights methods that can be used to measure safety culture and resilience engineering, and explores similarities and differences between complex organisations and gold mining to identify opportunities for more innovative approaches to improving safety in gold mining operations through safety culture and resilience engineering.

Resue or split face firing has the potential to separate ore and waste rock into separate muck piles within a stope. Thus split face firing has the potential to reduce dilution of the ore stream to be transported to the surface for subsequent processing. It also potentially reduces the total amount of broken rock needing to be transported out of the mine as the waste material is physically separated from the ore as part of the blasting process. Waste can therefore be used directly from the stope to backfi ll other mining areas. This paper reports on the results of split face firing trials at the Hillgrove mine in NSW and analyses the results of these blasts.

Uranium exploration expenditure in Australia reached a record level of A$220.5 million in 2008, which has subsequently declined to A$185.2 million for the year ended 30 June 2009. Recent discoveries include: Ranger 3 Deeps east of Ranger open pit (Northern Territory), Pepegoona and Yadglin sandstone hosted deposits (South Australia), and Thunderball project (Northern Territory). Australia?s Reasonably Assured Resources recoverable at costs of <US$80/kg U at June 2009 are estimated to be 1,210,000 t U, 4% higher than the estimate for December 2008. The main additions were in the south eastern portion of the Olympic Dam orebody. The Olympic Dam Expansion is based on a large open pit and will increase output to an annual capacity of 19,000 t U3O8; 750,000 t copper; 800,000 ounces gold. The Four Mile in situ recovery project was approved in July 2009. An ion exchange facility is to be constructed at Four Mile and uranium-bearing resins will be transported to the Beverley plant to recover the uranium. Recent developments at Ranger mine, and Honeymoon, Oban and Yeelirrie projects are discussed.

Horizontal directional drilling from existing underground workings will greatly reduce, if not replace, surface exploration drilling. Diminishing ore reserves along with increasing difficulty in acquiring access and permitting of drilling surface holes requires a change in procedure. The length and direction of the required long holes is site specific, but once determined could be quickly mobilized. Data would be available for a much more timely evaluation without any surface disturbance and required reclamation issues. Preplanned directional drilling from underground will also provide an evaluation of already inconsistent uranium deposits located under surface terrain that is inaccessible to surface drilling.

Uranium Refining is the process in which undesirable neutron absorbing elements such as Boron (B), Gadolinium (Gd), Cadmium (Cd), Europium (Eu), Samarium (Sm) and other heavy metals (Fe, Ni, Co, Ca, Mg etc.) are removed to make it nuclear pure. At present, Uranium refining is carried out by solvent extraction process by using organic based solvent e, g. TBP diluted with Kerosene. The above process generates lot of nitrate bearing liquid waste and degraded organic. In order to eliminate use of hazardous organic and ammonia for Uranium refining altogether, a new process has been developed. In the new process, Yellow Cake is dissolved in nitric acid and uranium peroxide hydrate is precipitated selectively from the solution leaving all the impurities in the solution by using commercial grade hydrogen peroxide at a pre-defined pH, U conc., temperature and stoichiometric quantity of hydrogen peroxide under mild agitation. The precipitate is filtered and washed with DM water to obtain nuclear pure product meeting purity standard as per ASTM C ? 753-99.The product is spray dried and calcined to obtain uranium oxide. The process eliminates use of organic, flammable, toxic chemicals, consumes less quantity of nitric acid and uses environfriendly chemical hydrogen peroxide. The process is simple and can be integrated in the mill. A patent application is entering national phase in several countries including CANADA and abstract/full paper is available in WIPO publication no: WO/2009/013759.

Mining operations in the Obuasi mine of AngloGold Ashanti Limited began in 1897 using mainly gravity concentration and amalgamation to recover the gold from predominantly high-grade quartz ores. The mine is primarily an underground operation, although some surface mining still takes place. Ore is currently processed by two main treatment plants: the Sulfide Treatment Plant (STP) for underground ore and the tailings treatment plant for tailings reclamation operations. STP was commissioned in February 1994 to treat 180 000 t/m of double refractory gold ore and it hosted the then world&apos;s largest commercial scale bacterial oxidation plant which uses Gencor&apos;s BIOX« technology. Gold is recovered by gravity and flotation, with the regrinding of the sulfide concentrate before bio-oxidation to liberate locked gold in sulfi de minerals. The BIOX« product is leached using the carbon in leach (CIL) process. Gravity gold recovery is a major component of STP operations and its contribution to overall plant recovery is about ten to 13 per cent. Gravity recovery has undergone series of technological upgrading, since the very first circuit which consisted of a Knelson concentrator and a Gemeni shaking table which was incorporated into the milling circuit at the start of the plant. Presently, each of the gravity circuits which consists of Knelson concentrator(s), cyclones and Gekko InLine Reactors (ILR), are located at the Milling and BIOX« circuits. More recently a new gravity recovery facility comprising of four 48 inch Knelson concentrators and cluster of ten inch cyclones have been incorporated at the tail of the flotation circuit. An additional ILR will be commissioned in 2010 to treat the concentrate generated from the new Semi Autogenous Mill #2 (SAG 2) Knelson: concentrator tower under construction. Since the inception of STP, significant gravity recoverable gold has been recovered via Knelson concentrators and the employed circuits have seen a number of modifications and upgrades, motivated by ore mineralogical changes and recovery concerns. The pioneering adoption of the ILR technology in 2001 and 2003 was a critical factor in enhancing the improved recovery benefit of the gravity circuits. Therefore the objective of this paper is to highlight the mine&apos;s experience in the gravity recoverable gold process with emphasis on plant modifications and gravity equipment upgrade. Operational challenges and the contribution of gravity gold to overall plant recovery are also discussed.

The nucleation of d hydride in Zircaloy-4 has been studied using electron backscatter diffraction. Hydrides prefer to nucleate at intergranular rather than intragranular sites. (0001) a -Zr//(111) d -ZrHx is the most common crystallographic relationship. The hydride platelets are generally interlinked each other and form a chain-like configuration. Based on thermodynamic model, the formation mechanisms of the hydrides at different sites and the interlinked structure were analyzed. We found that the misorientation of two adjacent grains has a crucial effect on hydride nucleation sites, and the tensile stress near the tips of hydride platelets is a key factor affecting the appearance of interlinked structure.

Application of teleoperated and automated (or ?autonomous?) mobile equipment in underground metal mines represents a promising avenue for overcoming some of the challenges facing the industry. This paper addresses the overall effectiveness of autonomous loading and haulage equipment compared with conventional human-operated machines. Factors affecting availability, utilization, and production rate are explored, and an emphasis is placed on some problems and risks not present in conventional human-operated systems, as well as on the character and criticality of autonomous equipment. The need for a more in-depth analysis of factors affecting overall autonomous equipment effectiveness is stressed; these factors should be taken into consideration at the stage of feasibility studies. The paper concludes with recommendations and directions for future research.

Security and safety due to environmental impact as well as the radiological consequences during the uranium mining operations, tailings management, uranium processing, disposal of waste in conventional landfill sites are matters of great importance. The Uranium National Company (CNU) from Romania is responsible for all aspects belonging to the management of the entire Romanian uranium industry- the uranium ore extraction, transport and processing of uranium ore, concentrates processing, as well as disposal of sterile ore and waste. The paper will presents specific safety problems related to the identification and evaluation of potential environmental risks, as well as possible radiological consequences associated with the disposal of sterile and the uranium processing waste resulting from processing of the uranium ore. Computer codes will be used, such as IAEA INTERTRAN II and RADTRAN 5. It is also to be noted that this paper considers the main results of the scientific research carried out by the main author as CSI (Chief Scientific Investigator) for Romanian Research Contract on NORM concluded with IAEA Vienna.

The implementation of robust, reliable and accurate sampling systems is the key to the successful management of any mineral processing plant. The representative sampling of a mineral processing plant is the only way to determine what is coming into the plant, what is going out as fi nal product and what is going to tails. In plants with high gravity gold components, where high recovery effort gravity circuits are installed, the use of a gravity recoverable gold (GRG) sampling system can assist in the online monitoring of the performance of the circuit. The typical installation location would be in CIP or fl otation feed (ie cyclone overfl ow), downstream of the gravity circuit. A GRG sampling system generally consists of the following equipment: true proportional, continuous drive, linear cross cut sampler, secondary vezin sampler operated on a timer, a batch centrifugal concentrator, and sample dewatering module. The operating philosophy of the GRG sampling system is that the primary sampler operates on a continuous basis, generating a constant stream of truly representative sample. Consep&apos;s H&amp;S drive system is unique in its ability to be able to operate in a continuous mode, therefore enabling a continuous stream of representative sample to be generated. The primary sample stream reports to the secondary sampler. The secondary sampler operates on an intermittent basis (nominally ten minute interval), resulting in a relatively small sample volume per shift. Normally the sample volume would be ten to 15 L per shift. The secondary sample rejects stream reports back to the batch centrifugal concentrator. The sample generated from the secondary sampler can be dewatered using a Consep H&amp;S dewatering module. This device produces a final sample that consists of separate filter cake and filtrate. These are suitable for taking directly to the laboratory. The continuous stream of truly representative secondary sampler rejects is passed through an appropriately sized Knelson Concentrator, typically a KCMD7, KCCD10 or KCCD12 sized unit. This unit recovers any available GRG from the stream. The Knelson Concentrator is also purged of its gravity concentrate at the end of the shift. This sample, which is nominally around 1 - 2.5 kg of concentrate, is also taken to the laboratory. At the end of the shift the client will have a measure of the gravity recoverable gold content of the target slurry stream, as well as a true representative sample of the complete stream. Through the addition of a Knelson Concentrator to an otherwise typical sample station, it is possible to generate a true measure of the performance of your gravity circuit. This will take the guess work out of the daily variation in gravity circuit gold recovery, as the site knows whether the changes are due to gravity circuit equipment performance, or due to the natural variation of ore GRG contents. Maintenance planning can also be designed around the system, as the site will have a composite sample to measure the performance of the circuit, therefore indicating where performance has dropped and maintenance is needed. Uranium is conventionally extracted using a process that employs strong acid (H2SO4) and an oxidant (MnO2) as leaching agents. This method often creates environmental problems, requires large amounts of energy, and involves a complex operational plant. It is not, however, economically attractive to extract uranium from low-grade ores by chemical leaching. Because the content of U3O8 in the Indian ore is usually below 0.1% by weight, it is necessary to develop an alternative process to enable the efficient and economic recovery of uranium. Uranium ore from the Narwapahar mines, UCIL is a type of vein deposit found in India, which accounts for ~54% in India. The ore (0.047% U3O8), though of Singhbhum area, possess exceptionally different mineralogy due to presence of some refractory minerals and richness of apatite (5%) resulting in a maximum 78% recovery through conventional processing at UCIL, with a very high consumption of sulfuric acid and pyrolusite, besides possible loss of uranium as uranium phosphate. To avoid usage of environmentally non-benign oxidants, remediation of influence of phosphates, and improve the overall process output of uranium; an alternate extraction technology using microbial isolate(s) is worthwhile exploring and the same is presented in the paper. The microbial isolate enriched with Acidithiobacillus ferrooxidans grown from source mine water was employed for bio-leaching of uranium while varying pH, pulp density (PD), particle size, etc. On varying pH at 35oC using 20%(w/v) PD and <75µm particle size, 83.5% and 78% uranium bio-recovery were observed at 1.7 and 2.0 pH in 40 days against maximum recovery of 46% and 41% metal in control experiments respectively. Whereas, it was observed that grinding of ore to finer size actually helped dissolution of uranium. Finer size particles (<45µm) gave a maximum biorecovery of 93% in 40days at 10%(w/v) PD, 1.7 pH and 35oC. The bio-recovery of uranium fell down from 93% to 82.73% and 60% on increasing the pulp density from 10% to 20% and 30% respectively for particles of <45µm size. The higher uranium dissolution during bioleaching at 1.7pH can be correlated with increase in redox potential from 433 mV to 664mV in 40 days.

The consolidation of soil material is central to many types of engineering analysis. Problems such as the building of highway structures, landfills, or buildings over saturated soft clays require application of such theory. It has been found in industry that the application of consolidation theory has lagged significantly behind the theory which has been developed some time ago. The application of small-strain saturated consolidation theory was first proposed in a 1D form by Terzaghi. Although this theory has been available for some time it?s implementation into software packages in 2D and 3D forms has been slow due to inherent complexities in the coupling equations. The flow and stress equations are also inherently somewhat unstable and this further complicates software implementation. Of particular interest in the mining industry is the consolidation of uranium tailings. One such disposal method that has been utilized is the subaqueous deposition of uranium tailings in large pit structures. When performing such deposition the calculation of consolidation rates is central to determining final pit capacity and expected life. Consolidation software can be successfully used in such cases in order to determine pit filling rates. The primary complexity, however, is that tailings are typically deposited in slurry form and therefore the potential deformations are large. Large-strain consolidation theory is therefore required in order to properly solve the deformations over time. This paper presents the use of a large-strain consolidation analysis in order to predict long-term filling rates in a uranium tailings pit.

Cameco ? McArthur River Operation is a high-grade uranium mine in northern Saskatchewan. Average grades mined are nearly 20%, more than 20 times higher than other operating uranium mines in the world. The significant risks involved with mining this grade has been effectively mitigated using innovative and unique controls, implemented from development and production mining through to ore processing. The types and sources of radiation are presented and protection measures are discussed. McArthur River?s strong radiation culture is evidenced by continually low radiation exposures while transitioning to new mining zones.

Sampling errors result from heterogeneities within the lot under study. Within a rock mass in situ heterogeneity results in the geological nugget effect, which accounts for most of the natural variability. In broken rock, constitution and distribution heterogeneities are important. A major result of the constitution heterogeneity is the Fundamental Sampling Error (FSE) which is the smallest residual error that can be achieved. It cannot be eliminated, but can be minimised and refl ects the fact that the broken rock sampling process is never completely free of error. Together with other sampling errors, FSE contributes to the total nugget effect. The FSE for a sampling protocol can be evaluated using the so-called Gy equation. Its practical applications to low-grade gold ores have often been unsuccessful due to problems linking the FSE and minimum sampling mass and the defi nition of key parameters such as the liberation diameter. Well-designed sample protocols minimise the total sampling error and hence the nugget effect. Reduction of the nugget effect is a key requirement for all sampling programs whether related to exploration and resource evaluation, metallurgical test sampling, grade control or metallurgical plant sampling. This contribution reviews the relationship between heterogeneity, FSE and nugget effect in gold deposits; and emphasises the need for FSE management throughout the mine value chain.

Sampling protocols and metallurgical flow sheets should be designed to suit the style of mineralisation in question. In support, the a priori need for evaluation programs is the characterisation of the mineralisation type(s) and domain definition. The proportion and size of gold particles has a direct impact on the effectiveness of sampling protocols and metallurgical processes employed. Within the coarse- to-fi ne gold spectrum, the samplability of a deposit ranges from relatively simple for fine-grained disseminated particles, through to difficult for coarse gold mineralisation. Conversely, metallurgical processing may be relatively simple for coarse gold ore and more difficult for very fi ne gold ores. Most deposits contain a proportion of both particle size types. The nature of mineralisation, mineralogy and associated gold particle sizing can only be determined by preliminary sampling to facilitate testing. Mineralogical studies, focussing on ore mineralogy and gold particle deportment, size and distribution are required as part of wider paragenetic and geometallurgical investigations. This paper discusses key issues and presents a methodology for the determination of gold particle size in coarse gravity-gold ores to support sampling optimisation and which also contributes to metallurgical characterisation.

Breakwater Resources Ltd is a Canadian base metals producer with Cu-Pb-Zn operations in Canada, Honduras, and Chile. Two of its operations contain economical gold levels which are recovered in both gravity and flotation concentrates. The gold payable terms in zinc concentrate are poor when compared to gold concentrates or copper concentrates and various flow sheets have been evaluated by Breakwater staff to maximise gold reporting in higher value products. The Myra Falls mine in British Columbia recovers coarse gold in the grinding circuit, part of which is upgraded and sold as a gravity-gold concentrate with the balance being directed to the copper concentrate. A sampling survey showed that there was potential gravity-recoverable gold in the copper regrind circuit. Installing a gravity concentrator in the copper regrind circuit increased the gold grade and recovery in the higher revenue copper concentrate and reduced the quantity reporting to the less well paid zinc concentrate. The El Toqui Mine in southern Chile has distinct high-and low-gold orebodies which are campaigned separately through the flotation plant. The high gold zone yields both a gravity concentrate and an unusual gold flotation concentrate. The gravity concentrate undergoes intensive cyanidation and electrowinning on-site to produce dore, while the fl otation concentrate is sold to gold producers. The zinc concentrate contains signifi cant gold values. The paper describes efforts over the past few years to improve gold returns at Breakwater&apos;s operations and highlights the different approaches dictated by the mineralogy of the gold occurrences at these two operations.

Strategically located within the Uravan Mineral Belt District of western Colorado, the Piñon Ridge Project is the first new uranium mill being proposed for construction in the United States in over 25 years. In 1992, dramatic changes to the US regulatory environment for uranium ore processing occurred making regulatory compliance an increased challenge for a new facility. Proposed as a 6.3 million tonne uranium milling operation, the mill will receive ore from a number of mines in the region. The project includes design and permitting of a uranium/vanadium processing facility, tailings cells, evaporation ponds, and ore stockpile pads. The focus of this paper is to provide an overview of the US regulations and how these regulations were applied to design of the mill facilities, as well as provide a status update on the project as the mill license application was submitted to the Colorado Department of Public Health and Environment (CDPHE) in November 2009.

The Rabbit Lake In-Pit Tailings Management Facility contains frozen layers of tailings due to subaerial deposition during the coldest periods of winter. Thawing is required to ensure full consolidation of the tailings and to regain disposal space presently occupied by ice. Electric resistance heating has been used to heat soil for bitumen extraction and remediation of volatile contaminants. A preliminary evaluation of the application of electric resistance heating to thaw about 0.3 cubic metres of frozen tailings was conducted using tailings frozen at about -2 oC. Visible thawing of the tailings mass was observed after 25 hours, demonstrating the feasibility of this approach.

Best practice in the design and construction of soil cover systems for mine waste management has advanced extensively over the past two decades. While significant progress has been achieved, we still have little experience with respect to proven performance for cover systems that must last hundreds of years. Some cover designs appear to be functioning well, while at the same time the performance of many more are now being called into question. This presentation evaluates the integrity of cover systems from both a global and local perspective in relation to the uranium industry within Canada.

Northern Saskatchewan comprises an area of about 350 000 km2 and boasts some of the world&apos;s largest known high-grade uranium deposits. The successful mining of these deposits, however, can not be accomplished without overcoming technical challenges. Not only is the high grade of the uranium ore challenging, but mining methods need to be developed to deal with groundwater at very high pressures, and ground conditions that vary substantially from excellent to wholly unconsolidated clays and sand. This paper discusses the various mechanisms of inflows at the three of the Cameco Corporation Mines in Northern Saskatchewan. The risk of inflows is quantified in terms of unique challenges from hydrogeologic conditions, rockmass integrity, and uncertainty in geologic conditions. Mitigation strategies in case of an inflow are also briefly described. The paper concludes by addressing the impact of institutional rules (corporate standards) on minimizing, monitoring, and controlling the probability and/or impact of unexpected inflows.
