This surveillance report summarizes data on work-related fatal and nonfatal injuries, illnesses, and hazardous exposures in the mining industry for the 10-year period 1986-1995. The term ?surveillance,? as used in public health, may be new to many readers of this report. With regard to occupational safety and health, the goal of surveillance is to describe the occurrence of work-related injuries, illnesses, and known hazardous exposures; to identify new hazards that may occur due to the introduction of new technology or other factors; and to assess the effects of preventive measures designed to improve worker safety and health. Surveillance provides an overall picture, which can be used to focus resources on areas most in need of prevention programs or further research. Surveillance generally requires the use of multiple sources of data, all of which have their own strengths and limitations. Surveillance in the U.S. mining industry is complicated by the fact that U.S. national data systems that provide information on mining are based on two different definitions of the mining industry. One is established by the Mine Safety and Health Administration (MSHA), the other by the Standard Industrial Classification (SIC) System [Office of Management and Budget 1987] and the 1980 U.S. Bureau of the Census (BOC) Classification System [U.S. Bureau of the Census 1982].

Introduction This paper is a descriptive analysis of injuries and fatalities at small surface bituminous coalmining companies in Pennsylvania. The purpose of analyzing a specific industry sector and location is to identify issues that might be missed in a wider analysis. Other studies of small mine safety (e.g., Peters and Fotta, 1994, Randolph and Boldt, 1997) have shown that the relationship between safety and mine size is not a simple one. Some injury types occur at higher rates at small mines, while others occur at lower rates. Consequently, an analysis limited to a specific type of mining can help identify hazards and issues specific to that sector. The results of this analysis will be useful for directing training, intervention, and research efforts towards the most prevalent causal factors and incident types. Procedures and Definitions Data sources The primary sources of data for this analysis were files provided by MSHA containing information collected under Title 30 Code of Federal Regulations, Part 50. These files, commonly referred to as “Part 50 data”, are provided though MSHA’s Program Evaluation and Information Resources directorate. Two primary types of injuries were selected from the Part 50data: Fatalities (MSHA “degree of injury” code 1) and lost-time injuries, otherwise known as nonfatal with days lost (MSHA “degree of injury” codes 2 through 5). Lost-time injuries involve either actual days away from work or days of restricted activity or both. Fatality and lost-time injury incidence rates were calculated using the standard MSHA formula: Number of injuries per 200,000 employee-hours. Additional information on company sizes and mine ownership was provided by the Pennsylvania State University College of Earth and Mineral Sciences. Time span A five-year time span was selected for this study. More than a single year of safety experience is usually needed to capture a broad range of incident types. Many types of injuries, especially fatal injuries, can be extremely rare, and may be missed if too short a time span is selected. However, a time span that extends too far back may begin to suffer historical threats to validity. That is, incidents that occurred several years ago may be caused by hazards that have since been remedied or may be related to mining practices and conditions that no longer exist. A five-year span offers a practical compromise between these two concerns. Consequently, the MSHA Part 50 data files from 1993 through 1997 were used. The 1997 file was in “preliminary” form at the time of the study, so small variations from the final 1997 “close out” file can be expected. Based on past experience, we do not expect the close-out variations to have a significant impact on the findings.

Injury experience in the mineral mining and processing industries during 1968 was slightly improved in the nonfatal-injury record but worse in the fatality record. The resulting frequency rate of all injuries was slightly better, whereas the injury-severity rate retrogressed appreciably from the figure for 1967. Overall operating activity, as measured by man-hours of worktime, was slightly higher in 1968. However, there were several major segments of the mineral industries in which activity was lower owing to reduced demand for mineral products and to several major work stoppages. These statistics comprise the injury and work experience of all personnel engaged in production, exploration, development, maintenance, repair, and force-account construction work, including supervisory and technical personnel, and working partners and owners at mineral-producing and mineral-processing establishments in the United States. Data concerning officeworkers are excluded except for the oil and gas industry for which such information is not separable. All injury rates and totals are calculated from data before rounding. The data for 1968 are preliminary except for the anthracite, coke, petroleum and natural gas, native asphalt, peat, and slag industries, which are final. The figures represent full coverage for all industries except oil and gas for which coverage is not complete, particularly with respect to small companies.

THE injury data and experience in this bulletin are presented in three general sections. The first section contains general and detailed data relating to 1948 injury experience in bituminous-coal and Pennsylvania anthracite mines, separately and combined into totals for all coal mines. The related employment, worktime, production, and productivity data are given in each breakdown of the statistical data. Tables 1 through 22 constitute a subsection and present statistics on number of injuries, frequency and severity rates, average severity, detailed and major causes of injuries, injury records of the coal-producing States, distribution of injuries by part of body, and distribution of injuries by days of disability. Tables 23 through 45, the second subsection, present injury experience in bituminous-coal mines by various selected factors of physical or operating conditions that ma contribute to the occurrence of injuries. The degree of mechanical loading, productivity, size of mine by number of men working and by tonnage mined, thickness of bed, length of shift, number of shifts, type of cap lamps, rating of mine as gassy or nongassy, type of ventilation, and combinations of these factors influencing mine safety are presented in this group of tables. Similar data on Pennsylvania anthracite mines are listed in tables 46 through 63, which comprise the third subsection. The second section of the report contains the generalized annual injury records of bituminous-coal, Pennsylvania anthracite, and all coal mines from the earliest available data through 1948. Tables 64 through 97 list the number of fatal and nonfatal injuries, injury rates, and causes of injuries on an industry basis, the long-term injury records of each coal-producing State, and the related employment, worktime, and production data.

REPORTS received from operating companies by the Bureau of Mines, United States Department of the Interior, indicated marked progress in the reduction of injuries to employees in the coking industry in 1951 as compared with 1950. The fatality rate (0.14) per million man-hours of exposure declined 33 percent and was the second best performance since statistics on the industry were first collected in 1916. The nonfatal-injury rate (10.94), the best since 1940, declined 8 percent and was bettered only in 8 of the past 36 years. The nonfatal rate is based on all injuries resulting in loss of time for more than the remainder of the day on which the injury occurred. The exposure to hazard, as represented by the number of man-hours of work time during the year, was 7 percent higher than in 1950 and, except for the year 1943, was the highest since 1920. The fatality rate for men employed at beehive-coke ovens decreased 16 percent and the nonfatal-injury rate 23 percent, while the man-hours of work time or exposure increased slightly over 16 percent. The fatality and nonfatal-injury rates at byproduct ovens declined 33 and 2 ½ percent, respectively, while the number of man-hours worked increased 6 percent.

STATISTICS of injuries in the quarrying and related industries in the United States first became available when the Bureau of Mines collected reports from the operating companies for the calendar year 1911. Since then reports have been collected annually. The present bulletin, covering the calendar year 1952, is the 33d publication containing statistical data on injuries at quarries and related plants, and it is the first to be published since 1944. The last publication was Bulletin 458, Quarry Accidents in the United States during the Calendar Year 1942. The unpublished statistics for 1942-51 may be found in the Appendix of this bulletin. The Bureau of Mines is authorized to collect data on injuries at quarries, but there is no Federal law that compels operators to supply such data, and the reports received from operators are voluntary responses to the Bureau's requests for information. Although the figures presented herein may not be complete for the entire industry, every effort has been exerted to make them so, and the figures given are believed to be thoroughly representative of the hazards to which quarry workers are exposed. Moreover, the figures are com¬parable as between States -which is significant in view of the lack of uniformity among the States as regards classes of plants covered by State laws, classes of injuries covered by State reports, and other factors that tend to make it impracticable or impossible to compare the injury experience of one State with that of another or to comprehend the relative importance of the various causes of injuries in the industry as a whole.

Introduction The use of diesel engines as an alternate power source in US underground coal mines has been a controversial step towards more efficent mining practices. Three areas of potential benefit are foreseen. First, diesel-powered mining equipment is believed to be more efficient, resulting in less occupational risk for any given amount of production. Second, diesels are believed to pose fewer intrinsic hazards, especially those related to fires and ignition of combustibles. Finally, several other health and safety advantages are foreseen for diesel units. Each of these issues will be discussed separately. Diesel Efficiency Proponents of the use of underground diesel-powered mining equipment point to increased production efficiency due to greater maneuverability, speed, power, and payload (Bradbury, 1973; Jorgensen, 1982; Ogden, 1978; Wenberg, 1979; Olsen, 1980). Units most suitable for diesel power include LHD vehicles, shuttle cars, personnel carriers, and maintenance vehicles. Estimates of increased efficiency vary from 60%-300% and more (Wenberg, 1979). Increased diesel mining efficiency is usually viewed as making it practical to mine lower grade seams. Diesel units can also operate over a wider range of floor pitches and distances than electrically-powered units, thus allowing mining of seams with special mining problems (Olsen, 1980). Diesel application is not a universal substitute for electrical power. For example, continuous miners are unlikely diesel candidates. Reduced Intrinsic Hazards Proponents of diesel use have pointed to the increased safety potential due to a reduction in electrocutions, arcs, burns, underground cable fires, and methane ignition (Ogden, 1978). Diesel units are frequently employed in maintenance activities for lifting and moving, and for coal cleanup. Thus the case is made that diesel use, by making powered operations more conveniently available, reduces the incidence of lower back injuries and related causes for days lost due to disability (Olsen, 1980). Several other issues relating to diesel safety have also been suggested. Diesel units can continue operation even during power outages allowing, for example, personnel evacuation. The maneuverability of diesel units make them particularly attractive in difficult physical conditions, such as long haulage from the face to the belt and in areas where water is deep. The case for diesel use focuses heavily on the potential for a "per worker" reduction in accidents, or a reduction in the severity of accidents. Since accidents are a major part of the health impact of mining, a reduction in accidents and resulting injuries would make a major contribution to the health of coal miners. Opposition to Diesel Use Opposition to the use of underground diesel mining equipment in the US seems to have started with the United Mine Workers of America. Some opposition to US diesel use has been based on a widely publicized 1976 letter from NIOSH to Mine Enforcement and Safety Administration administrator Robert Barrett (NIOSH, 1982). In essence, this letter warned that while insufficient evidence existed as to diesel effects on underground coal miners, NIOSH had concerns based on prior knowledge of separate diesel exhaust constituents and the potential for interaction between these constituents and coal mine dust. Current opposition to underground diesel use has focused on respiratory health effects related to diesel emissions, particularly nitrogen dioxide (NO,), carbon monoxide (CO), and particulate content (Barrett, 1982). Today, with the growth of the team concept in industry, a mutual-caring (interdependent) approach to workplace safety is gaining wide acceptance. AXTAT is a computerised system developed by the Western Australian Department of Minerals and Energy in conjunction with the local mining industry for recording and retrieving information about fatalities and work related injuries. Since its inception, the system has gained considerable recognition by consistently demonstrating its usefulness as a tool, not only to the Department, but for the entire industry nationwide. AXTAT has played an important role in the continuous improvement process in management of occupational health and safety in the Western Australian mining industry with gains in both human and financial terms, by regularly providing timely and accurate statistical feedback on fatalities and lost time injuries to industry.

The first metal-mine accident-prevention course was prepared and published in 1942-45 as a series of seven miners' circulars (Nos. 51-57). The course has been broadened in scope, revised, and brought up to date and is being published as a similar series of seven miners' circulars (Nos. 51-57, revised) on accident prevention in metal and non-metallic mines. These circulars are as follows: Injury Statistics (Miners' Circular 51), dealing with general statistics on injuries from accidents at metal and nonmetallic mines, including causes, costs, and the use of investigations and reports of all injuries.

In mid 1978 Inland Cement Industries Ltd. brought on-line their new one million megagram (Mg) per year cement plant at Tilbury Island, Delta, British Columbia. The Plant--Its Philosophy of Design The design of this plant began in 1974, at the beginning of the rapid rise in petroleum costs later referred to as the "energy crisis". The trend in fuel costs was clear, so one of the basic criteria for the selection of process equipment was high thermal efficiency. Throughout the process of selection of materials handling and process equipment, energy consumption was a factor in decisions. A second factor which influenced the design of the plant was its location on navigable water. The plant is designed to make maximum use of this means of transport to minimize transportation costs. All raw materials (and some fuel) are received by water, and a major portion of the cement and clinker is sent out by barge. Extensive modern dock and material handling facilities were provided to accomplish the shipping and receiving functions. A plant meeting stringent environmental regulations was the third major factor which formed the basis for design. To meet this requirement, all storage facilities had to be covered, 53 bag type dust collectors were applied throughout the plant, and materials handling equipment was designed to minimize dust and spillage.

Introduction The ore mining industry has been successful because of technological progress and huge sums of capital investment. But new technology and plants have not come cheap. The problem now is companies will run out of money before they run out of ideas. Survival Strategies Industry survival strategies demand restructuring and new plans. Inland's prescription for the future includes: • Continued emphasis on technological improvements to upgrade pellet quality and lower costs; • Effective use of human resources resulting in employee satisfaction and improved planning and productivity; • Deliberate, well planned, and documented cost-control programs for the short- and long-term; and • Capital investments to support the first three initiatives. Technology Technological changes are not only required by the need for operational improvements, but by product quality improvement dictated by the user - usually the blast furnace operator. Established ambient measurements of pellet quality, such as tumble index, compression strength, porosity, size distribution, and chemical composition, are no longer considered meaningful in predicting pellet performance in a blast furnace. New specifications, called low temperature and intermediate temperature properties, have been recently introduced. Low temperature specifications are characterized by disintegration of pellets in the 500°C (930°F) range in the upper portion of the granular zone during the hematite-to-magnetite transformation in blast furnace stack. Intermediate temperature properties (measured by 1000°C or 1800°F) are characterized by swelling, reducibility, and compression strength after reduction, reflecting conditions in the lower portion of the granular zone of the furnace. These new parameters are being used for pellet quality evaluation at Inland, and will be introduced at its Minorca mine for more routine analysis and correlation with the pellet furnace operation. With increased emphasis on quality, effective communication between the blast furnace operator, research, and the pellet plant operator is critical. At Inland, this is accomplished through technical meetings where the opportunity is provided to solve problems, establish a common understanding on the value of quality, and develop future quality standards and programs. With the degree of sophistication in determining pellet quality as it affects blast furnace operation, the future pellet plant operator will actually be producing a custom pellet. Human Resources Technological change has been a strong point at Inland, and will continue to be. However, human resource management is equally important. Is management giving sufficient attention to employee motivation and involvement? It is a way to improve productivity, reduce costs, and enhance employee job satisfaction. The industry is beginning to understand the principles and the potential for tapping employee brainpower. The payoff can be handsome. Management has to be willing to share both information and power if participative management is to live up to its potential. It has to become more flexible and be willing to adapt to change on the human side. Management should embrace rather than fear employee involvement. Given time, joint problem solving will replace adversarial attitudes and pay dividends for both sides. Productivity and product quality will improve and new dimensions of employee job satisfaction will be achieved. But, it will not happen overnight. Attitudinal change must permeate the organization from top to bottom. This change is needed to support the working relationships and give everyone a sense of direct and meaningful involvement.
