Fatigue is it the newest buzz word or is fatigue a real phenomenon that is creating safety and organisational risk for you? There is no debate that fatigue is natural, and it affects us all, just like gravity; however, there has been much debate on the severity of its impact in the workplace. For many years the impact was not measurable, hidden out of sight and, quite frankly, written off as the infamous ‘operator error’. Over the last several years, advancements in technology have provided safety and health professionals, sleep scientists, fatigue experts and organisations with the ability to monitor and detect fatigue events in real time, as well as predict and monitor increased risk for fatigue events. Caterpillar Safety Services has assembled an extensive and exceptional set of technologies that will provide the best methods and data to measure the impact of fatigue. There have been several high-profile accidents in the South African mining industry where fatigue was identified as either causal or contributory. The risk of fatigue is inherent in any work-time arrangement involving shift work and work that is physically or mentally demanding, repetitive, or requires high vigilance. The management of fatigue is not simply a matter of correct shift scheduling. Amore comprehensive approach is needed. In order to be successful, a fatigue management programme should address the unique needs of the operation in which it will be implemented and should be integrated into normal operations. Fatigue management is a shared responsibility between the organization and its employees, and all stakeholders should participate in order to provide the safest and healthiest working environment possible.

Selectivity and recovery of many industrial minerals can be directly affected by the carbon length, degree of unsaturation, and melt point of fatty acids used as collectors. Examples of several modifications are provided based on actual experiences of commercial applications. A review of recent history indicates that fatty acid collectors are applicable from C9'C18 in carbon length, however, not for all minerals. The mesh size, density, and surface characteristics of the particular mineral, as well as other related operating conditions, are important in selecting the correct fatty acid configuration. A review of some past programs are submitted as a guideline in the selection of fatty acids and supported by isolated, successful examples demonstrating the effect of carbon length and unsaturation.

Recently, research efforts have been made to lower the magnesium content of phosphate rock concentrate to be a suitable feed for phosphoric acid production and other related industries. Among these efforts is the bioprocessing technique. However, this study is devoted to study of using an enzyme, extracted from coffee waste, as a surface modifier during the fatty acid flotation of calcareous phosphate rock. A mixture of oleic acid and kerosene was used as collector. Factors, which affect the enzyme activity such as enzyme dosage, solution pH, temperature and conditioning time, were studied. Under the appropriate conditions, a concentrate product containing less than 0.60% MgO was achieved from the Abu Tartur feed (1.80% MgO) with a P2O5 recovery of more than 94%.

The fatty acid flotation process has been used to produce phosphate for decades, yet the chemical mechanism involved has never been thoroughly investigated. A basic knowledge of the process is lacking, and as such, industry has had to develop their own guidelines for the design and operation of conditioning and flotation equipment. These guidelines are the result of many trial and error attempts, and are not by any means universally endorsed. There are almost as many opinions about how to operate a plant as there are practitioners of the art. This paper covers the currently recommended rules for design as viewed by one practicing phosphate metallurgist.

The University of Alabama Mineral Resources Institute (MRI) has developed a unique process for selective fatty acid flotation of carbonate gangue from sedimentary apatites (francolite) in the pH range of 4-6 without collector conditioning of the pulp. The process, which does not require an apatite depressant, was modified to benepciate a high- MgO siliceous phosphate matrix from south Florida. The modified process includes two flotation stages, one for car- bonate removal and the other for phosphate recovery from the siliceous gangue. The selective flotation separation of the carbonate and phosphate from siliceous constituents of the matrix at various pH levels and collector dosages are discussed. In the carbonate flotation stage, about 70% of the MgO was rejected in the froth. The phosphate flotation stage produced concentrates analyzing about 31% P2O5 0.7% ? 5 MgO, and 4% insol. The overall P2O5 recovery In the two stages was about 85%

This paper discusses the geotechnical condition of bedding shears and small- and large-scale faults and shears in Archean banded iron formation (BIF) bedrock, based on observations of core and televiewer images. Information that can be collected includes the orientation of defects, infill thickness, strength and material type, defect shape and surface roughness.The spacing and continuity of bedding shears may be critical to slope design where performance is structurally controlled with bedding dipping out of a wall. The intensity of development of the bedding shear is a useful indicator of the amount of shearing a bedding surface has been subjected to. This can allow for engineering judgement to be used in adopting peak or residual shear strengths along defect surfaces when developing the design.The spacing and continuity of bedding shears may also contribute to the blockiness and degradation of a rock mass where the structural orientation is otherwise favourable. Additionally the condition, thickness and strength of infill of major fault structures can affect slope design and performance, depending on orientation.Comparison of the condition of structures observed in core with the corresponding in situ televiewer image has been made to assess what characteristics are most accurately captured by each technique and whether the additional information gathered from using the televiewer technique is justified by the cost. This paper describes the development and validation of a strategy for vibration-based online detection of faults in a particular class of machinery commonly found in industry. This machinery is defined by two basic characteristics that preclude it from the application of standard online condition monitoring approaches. The first characteristic is that the machine is in a constant state of transient operation, thus making it difficult to differentiate between changes in transducer measurements that are due to variable duty and those that are due to machine malfunction or fault. The second characteristic is the fact that the machinery involves multiple sub systems that are subject to identical and simultaneous loading, operating environments and duty cycles. An example of such machinery is a system of multiple hydraulic pumps driving a common load. The monitoring strategy presented here employs an anomaly detection scheme together with methods of signal processing and feature extraction that exploit the parallel duty arrangements of this equipment. Preliminary data is collected from a pair or motor-gearbox assemblies used in a material haulage system in the mining industry. Testing and validation was performed on a mechatronic test bench with both healthy and faulted components. Experiments were carried out to investigate how various configurations of the system were able to detect different types of faults. The results indicate that this approach is effective and merits further investigation.

Fault detection and identification are major challenges in process engineering and manufacturing and the key component of abnormal event management systems. Timely detection, diagnosis and rectification of abnormal or faulty process conditions can lead to savings of billions of dollars in equipment damage and lost productivity, not to mention the prevention of injury and loss of human life associated with industrial accidents. A major contributing factor to current losses in industry is the reliance on human operators to interpret high-frequency samples from hundreds or thousands of variables simultaneously. As a result, the automation of fault detection and diagnosis is seen as crucial to the successful implementation of abnormal event management, the need for which is becoming all the more urgent given the increased complexity associated with modem industrial plants. In this paper, a methodology for process monitoring that uses support vector methods to extract nonlinear features from data is discussed and applied in the diagnostic monitoring of an industrial liquid-liquid extraction column.

Fault diagnosis systems have attracted growing interest in a number of engineering fields. The number of applications has increased and successful results have been widely reported. The fault diagnosis system presented in this paper is integrated, and it monitors the states of the process and performs fault detection and diagnosis on the process equipment. The monitoring of process states is carried out by means of Kohonen Self-Organising Maps that is a powerful tool in visualisation and analysis of high dimensional data in engineering applications. This paper presents the results of SOM analysis carried out on the Outokumpu flash smelting process. The subsystems analyzed in more detail are the condition of the concentrate burner, the condition of the gas line on the basis of the pressure distribution in the process and the cooling water circulation of the waste heat boiler, and the condition of the furnace and furnace equipment on the basis of the temperature distribution in the furnace and gas line. Geological structure is one of the main controls on ground conditions within mining. It has a significant impact on the mining conditions and productivity of coal mines within the Sydney - Bowen Basin. The three main types of faults common to this region, normal, reverse and strike-slip systems, each present distinct challenges to mining. These systems effect a wide range of geotechnical and mining conditions within the mines. These include coal quality, strata stability, goat and caving behaviour, productivity and maintenance schedules of mining equipment, mine design and mine-site stress fields. A better understanding of the regional, local and mine-scale structural setting, timing relationships of structural events and the mechanisms of fault formation will greatly aid industry's push for greater productivity and competitiveness.

Keyboard patterns of hundreds of minor faults with vertical displacements of 3 mm to 2 m occur within the Greta coal seam and cause serious mining problems in collieries. Many of the faults are concentrated in linear zones termed swarms. One swarm is 600 m wide and more than 800 m long and others are of comparable size.The minor faults between the swarms are normal and are relatively widely spaced and show dip-slip movements, but the faults within a swarm are clustered and show strike-slip and oblique-slip movements. Fault swarms ofthis size and frequency have not previously been described from the Sydney Basin or on a comparable scale elsewhere but appear to be a widespread phenomenon in Australian coalfields. The faults are assigned to classes onthe basis of orientation which are then assigned to sequential phases I to IV on the basis of offsetting relations between representatives of each class. Phase I faults are minor thrusts. Phase IT, Ill, and IV faults are concentratedin the swarms and show sinistral strike-slip and normal dip-slip movements. The interference of slickenlines indicates that multiple movements have occurred within a single phase; their order is (i) strike slip (ii) dip slip and (iii) oblique slip. All faults truncate and post-date both cleat and sedimentary "stone" dykes.Phase I minor thrusts trend at a high angle to the Hunter Thrust and are considered to predate the Late Permian movements on the Hunter Thrust. Phase II and III minor faults are younger and may be any age between the latePermian and Tertiary.

The issue of miner safety in case of an underground mine fire has been extensively examined by both government and industry. These efforts have intensified in recent years following a number of fatal fires at underground coal mines. As a result, there is an increasing interest in the reliability of coal mine escapeways during a mine fire. This paper discusses the use of fault tree analysis for evaluating escapeway reliability. A fault tree is developed using standard logic formulations. It is later evaluated using commercially available software. The computer model uses Boolean reduction to obtain the minimal cut sets and minimal path sets of the tree. A brief analysis is also conducted to determine the occurrence probability of the TOP event. This study shows that fault tree analysis is a viable means of analyzing escapeway reliability. The cut and path sets reveal those event sequences leading to fatalities. Such information is useful in safety training and accident prevention. The probability of the TOP event shows the frequency of occurrence for fatalities in an underground mine fire. The usefulness of this value, however, is limited by the accuracy of the data for the initiating events. For mining events, the data is difficult to estimate precisely.

A fatal accident may result when a dozer is driven over a hidden void in a coal surge pile and the dozer falls into the void, burying and suffocating the dozer operator. This type of accident was analyzed by researchers from the Spokane Research Laboratory of the National Institute for Occupational Safely and Health using inexpensive fault-tree programs that had been developed, for use on a personal computer. Fault-tree analysis is a systematic safety-analysis tool that proceeds deductively from the occurrence of an accident event to the identification of the root causes of that event. The analysis identified basic and intermediate events that led to the burial of the dozer and graphically depicted the interrelationship between the basic events as well as the intermediate events leading up to the accident. A sensitivity analysis on assigned probabilities showed which events had the greatest influence on dozer burial in a coal surge pile. Operational guidelines and engineering controls are suggested to decrease the probability of a burial event.

THE Wright-Hargreaves mine is one of a group of seven mines long active in the Kirkland Lake district, Ontario. Detailed geological reports and maps covering the area have been published by the Ontario Department of Mines (1,3)t. Though all these mines work several productive veins there is one vein which is common to all and is known as the North Vein of the camp?not that it is the most northerly vein mined, but it is the more northerly of the two major veins of the camp. At the Wright-Hargreaves mine there are in all ten important producing veins, and most of them have a N.60°-65°E. strike and in general a steep southerly dip. It is a common belief that most of these veins lie along faults but it will be shown that whilst this is true of some of them it is not true of all, and also that a vein may lie in a fault along part of its course and elsewhere fill a fracture in line with the fault, this being a consequence of rotational movements that took place during faulting. The paper describes the history of faulting at this mine in particular, makes observations on the principal pre-ore fault, and describes the method used in solving the movements on the post-ore faults. Other details of geology are not discussed except to mention here that our main veins lie almost entirely in the syenite porphyry stock except on lower levels close to the east boundary of the property, where the North vein leaves the porphyry to enter the greywacke.

For a long time there has been a known relationship between the northern Heavy Mineral (HM) deposits of the Murray Basin to the extensive north east trending ridges in the area. These ridges have been a targeting tool for exploration and have uncovered some significant discoveries, some of which have turned into mining projects. Bemax’s Ginkgo and Snapper deposits are good examples of how the faulting helped create an economic deposit but also result in significant planning issues in the exploitation of these deposits. Material balance, induration and pyrite issues have been made easier to manage through good geological understanding of the deposits.

This paper records the observations of faulting in the Black Star mine workings. By classifying the faults into two groups, i.e., strike faults and transverse faults, and by pointing out the differences between the groups and their effects on the silver-lead-zinc orebodies, a time relationship between the two groups of faults and the sulphide mineralisation is suggested.GENERAL GEOLOGY AND MINERALISATIONFolding and faulting are closely related to the mineralisation in the shales by silver-lead-zinc and iron sulphides.Commonly the various sulphides are fine-grained and closely associated with one another. However, exceptions do occur locally, where massive coarse galena, sphalerite, pyrite, and to a lesser extent chalcopyrite, have been observed singly or closely associated one with the other.In the main, the sulphide mineralisation forming the orebodies occurs as replacement of, and infilling along, the sheared bedding planes of the finely bedded shale host rock.The shales in general strike north-south with an average dip to 60° to the west; strong shearing along the bedding planes is evidenced by much slickensiding and slaty cleavage.Sequences of finely bedded shales of varying stratigraphic thicknesses alternate with very. massive beds, and it is in the more highly sheared thinner beds that the greatest concentration of sulphides occurs. The unmineraIised massive beds of shale, 11 in. up to 12 in. in thickness, occur singly or in groups between, and to a lesser extent. within, the orebodies. The massive beds resisted sulphide replacement and impart to the orebodies a most prominent banded structure.

FaulIing west of the Mount Isa mine is described and is shown to be related to three groups of folds that have been recognized in the area. The first period of faulting postdates the first generation folding; these faulIs are regional faults (tectonic discontinuities) which are folded by second generation folds. The faulting accompanying the second generation folding divides the area west of the Mount Isa mine into four structurally and metamorphically identifiable blocks. The most prominent of these second generation faults is the Mount Isa Fault, which is characterized by a number of distinct zones of intense second generation folding and the presence of localized layers of mylonite. Only limited evidence for brittle deformation has been observed and the major movement appears to be associated with the plastic deformation of the rocks. Confined to the areas of second generation folding in the Mount Isa Fault are numerous third generation kink folds. A detailed description of the Mount Isa and other faulIs is presented together with evidence for their existence. The displacement accompanying this faulting cannot be established because of the complexity of the structure.Postdating these faults and all the folding of the area are minor cross faults.
