Replacement of an aging twin span river crossing in Vermont required a temporary bridge to accommodate traffic during construction. Soils beneath the temporary approach embankment consisted of very soft organic clayey silt underlain by interbedded, loose alluvial fine sand and silt/clayey silt. Reinforcement was required for the southern temporary approach embankment to limit settlement and to increase global stability factors of safety. A ground improvement design was developed using Rammed Aggregate Pier® reinforcement and was included in the contract documents as an FHWA Category II Experimental Feature. Instrumentation intended to document the performance of the supported embankment was installed. Piers were then constructed and monitoring commenced as embankment and bridge construction occurred. Weekly instrumentation data reports indicated continuing excess pore water pressure, settlement, and lateral deformation of the subgrade soils as the embankment and temporary bridge abutment was constructed. In response to the unanticipated monitoring results, further investigation indicated that significant deviations from the conditions and basis upon which the ground support design was developed were made during construction. This paper discusses these deviations and their specific impacts to embankment support performance, how these deviations occurred, the role that the ground support system played in maintaining global stability, measures taken to limit further detrimental impacts to the embankment, and provides recommendations for contract document preparation and construction oversight to prevent similar situations from occurring on future projects.

In 1963, the full-scale engineering study for the development of an iron, steel, and vanadium plant was initiated. This was the birth of Evraz Highveld and its process flow as it is known today. Highveld started out as a steelworks that produced steel and vanadium-bearing slag from a titaniferous magnetite iron ore mined from its own mine site near Steelpoort. The process utilized co-current rotary kilns for prereduction, and submerged-arc furnaces for the production of pig iron with approximately 1.15% vanadium. Because of various technical challenges that the submerged-arc furnaces experienced over the years, a decision was taken in 2004 to convert the furnaces to open slag bath (OSB) operation, in order to enable the maximum production of iron and vanadium while improving the control over operational parameters. The conversion proved to be highly successful and, since 2005, three furnaces have been converted to OSB operation. The fourth furnace will be converted in the first half of 2011. The conversion to OSB furnaces not only yields higher vanadium recovery to the metal at the iron plant, but also improves the overall throughput of the steel plant. This is the result of the higher carbon content of the liquid iron from the OSB furnaces, eliminating the need for anthracite addition to the shaking ladle process. This reduces the blowing time by approximately 60%. The conversion of the furnaces to OSB configuration is seen as an enabling technology change and a prerequisite to the implementation of other plant improvements that are envisaged. The long-term vision of EHSV is to convert five furnaces to OSB mode, and permanently decommission the two remaining submerged-arc furnaces. This will allow EHSV to maintain current steel and vanadium production but at a significantly improved overall efficiency.

Helical piles and ground anchors were used to meet critical design and constructability requirements for an urban forest Canopy Walk at the Atlanta Botanical Garden, Atlanta, GA. The 12-foot wide pedestrian walkway path was to be nestled among the treetops of a forest, extending to 600 ft. in length, and soaring 40 ft. high above the forest floor. Of particular concern during construction was that the foundation system for the temporary steel shoring system had to be non-invasive to the subsoil, could not damage the forest floor, trees, root systems, or foliage, and upon completion of the concrete walkway pour, could be removed, again without disturbing the subsoil. Accubrace, Inc. designed and installed all the helical foundation types, as well as provided steel braces, typically used in Tilt-Up building construction, for the walkway project. The braces, some extending to 43 ft. in length, were used to provide lateral support against wind loads for the walkway form. In addition, the braces acted as secondary support for lateral loads during the placement of wet concrete along the 600 ft. length of the walkway. Helical foundation products installed included vertical compression piles, ground anchors for tension rods, and ground anchors for the braces. The installation of the piles and anchors was particularly challenging since the forest floor elevation varied as much as 50 ft. due to the presence of a ravine. Information is provided on the subsoil conditions, required loads, design, installation, and acceptance criteria for each helical product type installed for the project. The results of vertical and lateral load tests on the helical piles in the pre-construction phase, conducted in conformance to project specifications are presented. Photographs to illustrate the nature and complexity of the project are included. The concrete pour for the walkway was successfully completed on December 12, 2009, and the walkway was opened for visitors in May, 2010.

Lonmin, earlier than any other primary platinum producer, had to deal with the concentrating and smelting of UG2-rich ores and concentrates respectively. Smelting was performed at a fairly modest scale compared to the company's industry peers in the platinum group metals (PGMs) industry, and was focused on smelting concentrates obtained through low mass pulls while still maintaining high recoveries at its concentrators. Lonmin gradually smelted larger quantities of UG2 concentrates. Initially two 2.3 MW Infurnco furnaces were commissioned in 1982, followed by three 5 MW circular 3-electrode Pyromet furnaces in 1991. Deep electrode immersions and moderately high hearth power densities were used in all the designs. Lonmin decided to continue with circular furnace technology when it planned its new high-intensity No. 1 Furnace. Neither Lonmin nor the technology supplier and EPCM company foresaw the challenges that scale-up would bring when applied to the smelting of UG2-rich concentrate blends. Superimposed on the high chromite content, was the low base-metal loading, mineralogical difficulties, and fine particle size that resulted from milling of all concentrates (especially UG2 and recycle materials within the smelter). Through a process of fundamental diagnoses of furnace run-outs and wear patterns observed during repairs, the main challenges operating this high-intensity furnace were identified as sulphur vapour corrosion of the copper coolers, uneven and unpredicted movements of refractory bricks with associated copper cooler lift and matte tapblock movement, the formation of three-phase 'mushy' zones, high refractory and taphole wear rates, uncertainty in matte level and associated insufficient matte buffer height, and high furnace operating temperatures. These factors seldom worked in isolation and will be explored in more detail in the paper. The variability in furnace feed characteristics led Lonmin to redesign Furnace No. 1, to invest in backup furnace capacity, and to invest in improved monitoring and control. These improvements consisted of online and high-frequency off-line monitoring of feed chemistry and mineralogy, online pressure monitoring of water-cooled circuits, an alternative matte liquid level measurement in the furnace, and electrode immersion estimation. Improvements were also made at the converters by installing and utilizing optical spectral analysis of the converter flame to characterize converter behaviour and achieve the desired iron end-point for white matte. This paper also briefly discusses the current and future expansion plans, as well as ancillary operations at the smelter such as flue-gas handling and materials handling.

A pilot plant paste thickening campaign was conducted at the Anglo Platinum Limited Mogalakwena South Concentrator plant in South Africa in order to determine the water saving capability of P&TT technology. In the process a simple water consumption model was developed for estimating the overall water consumption of the mine. The model indicates that for the Mogalakwena tailings, significant water savings are achieved by discharging thickened tailing directly to the TSF but only at densities where free water release at the TSF is close to zero. In July 1982 the Committee on Underground Coal Mine Safety completed its study of safety in U.S. underground coal mines; the results of the study were published in the National Research Council report entitled Toward Safer Underground Coal Mines. One of the findings of this study was: THERE IS A STRONG CORRELATION BETWEEN MINE SIZE AND FATAL INJURIES. THE FATALITY RATE IN MINES WITH 50 OR FEWER EMPLOYEES IS NEARLY THREE TIMES THAT OF LARGE MINES (WITH OVER 250 EMPLOYEES), AND ABOUT TWICE THAT OF INTERMEDIATE-SIZED MINES (WITH 51-250 EMPLOYEES). Although the Committee explored possible reasons for the higher fatality rate in the smaller mines, it was not able to explain this phenomenon fully. Therefore the Bureau of mines requested that the Committee undertake a supplementary study of fatalities in small underground coal mines. The Committee met in the fall of 1982, discussed the problem, developed an approach to it, approved a program of statistical analyses, and authorized a panel of four committee members to explore the problem in greater depth. The panel, in turn, reviewed the results of the statistical analyses and met with the president of the National Independent Coal Operators Association and with several owners of small underground coal mines for a detailed discussion of the unique problems of small mines. Panel members examined 66 individual fatality reports prepared by Mine Safety and Health Administration (MSHA) inspectors who investigated fatal accidents that occurred in small underground coal mines in 1979 and 1980. This report is the result of those studies, analyses, and deliberations. The first draft was prepared by the panel, but the report has been reviewed and approved by the entire Committee.

Transportation accounts for some 26% of all mine accidents in South Africa. Although this figure may not be as prolific as fall of grounds accidents or accidents caused by seismic events, the enormity of transportation accidents demands attention, especially as many of these accidents are avoidable. This paper investigates vertical (i.e. shaft), horizontal (i.e. haulages), and in-stope transportation systems and their function as it applies to safety. Recent transportation accidents are discussed looking at current trends within the three transportation areas. In the shaft area, discussions will focus on handling of material cars, loading and off-loading of the workforce and management of shaft orepass systems. The horizontal component will focus on personnel transportation, safety around trains and hoppers, and handling of material in the cross-cut. The discussion on in-stope transportation will focus on the general congestion of the stope environment and how it affects the overall movement of personnel, material, and rock. Of concern are the large numbers of material handling and scraper winch accidents that occur and which will be discussed in some detail. The paper will conclude with a section on proposed guidelines within the three above-mentioned systems for mine engineers to consider in mine design and life of mine planning and scheduling. The ISASMELT? top submerged lance (TSL) bath smelting process was developed in Mount Isa, Australia by Mount Isa Mines Limited (now a subsidiary of Xstrata plc) during the 1980s. By the end of 2011, the total installed capacity of the ISASMELT? technology will exceed 8 000 000 metric tons per year of feed materials in copper and lead smelters around the world. Commercial plants, operating in Belgium and Germany, are also batch converting copper materials in ISASMELT? furnaces. This TSL technology is equally effective for continuous converting processes, in which role it is called ISACONVERT?. This paper presents the recently patented ISACONVERT? process for the continuous converting of nickel and platinum group metal (PGM) containing mattes using the calcium ferrite slag system. The paper focuses on the potential application of the ISACONVERT? technology to existing nickel and PGM smelting complexes.

Mining engineers frequently encounter complex design problems for which the critical components of the design are difficult to quantify or compare. Teams of people typically work on these designs and human perceptions and judgments play a strong role. To assist with this process, the analytical hierarchical process (AHP) as a decisionmaking tool is described in this paper. Although the technique is used in many other disciplines, it is currently not widely used in mining engineering in South Africa. To use the AHP process, the problem should be treated as a hierarchy that defines the goal, the alternatives to reach it, and the criteria for evaluation of these alternatives. Pair wise comparisons are conducted on the criteria of the hierarchy to establish priorities. The value of the technique is that it is simple to test for consistency amongst the pair wise comparisons to ensure that the answer obtained is better than that provided by a random selection. To illustrate the technique, the use of a backfill support system in a platinum mining project was investigated. Ten design parameters, which will be impacted by the use of backfill, were identified and weighted according to their relative importance. The result of the AHP evaluation was that the use of a backfill support system should be preferred to a conventional support system at the mine.

The interaction between rock and drill bit during drilling has been modeled for many years, but a complete understanding of the phenomena occurring has yet to materialize. Successful models will allow the prediction of rate of penetration in a given environment and optimal selection of drill bit and drilling parameters, thus minimizing exploration costs. In most rock-drilling models the value of the unconfined compressive strength of the rock (UCS) is used in the predictive equations, within the concept of specific energy, and the value of UCS is the percentage of the value of the stress applied on the drilling bit in order for the bit to advance. While the exact percentage depends on the model used and it is not known with certainty, good knowledge of UCS is nevertheless required before any decent prediction can be made on rate of penetration. Determination of UCS, normally done via destructive testing, requires not only the availability of sound rock core samples but also expensive testing and significant time for the test, which frequently are not available for routine drillability predictions. Hence, a multitude of methods and techniques has been proposed for estimating UCS from various indirect and/or non-destructive measurements, or combination of measurements with neural networks, such as point load index, block punch index, unit weight, and apparent porosity, water absorption by weight, sonic velocity, and Schmidt hardness. The many proposed approaches are critically reviewed and the results are compared, and what becomes apparent is that after many years, not only in mining but also in oil-well drilling, accurate indirect determination of UCS is still an elusive goal. An equation to predict UCS from sonic velocity data is suggested based on several data sets reported in the literature. Use of the specific energy equation with UCS or sonic data and utilization of drilling data allows an estimation of the efficiency of energy transfer from the bit to the rock and of the friction coefficient. Analysis of data reported in the literature, both from laboratory and field studies, has shown that this approach is sound and enables the determination of energy transfer efficiencies and friction coefficients, which for the cases studied range between 15 and 30% and 0.15 and 0.30 respectively. Thus, the suggested data analysis approach allows drillers to focus on inefficiencies and optimize drilling practices in future campaigns. Large quantities of combustible materials are often found in underground metal and nonmetal mines, with support timber most often the predominant combustible material. Oils, lubricants, paper, cartons, and rags are additional combustible materials and are commonly found in maintenance shop areas. Welding and cuting, shorted or overloaded electrical cables, spontaneous combustion, and careless smoking are some of the common fire ignition sources. Inexpensive, reliable fire detection and suppression systems are available to provide early warning of such fires and to isolate and extinguish fires that originate in underground maintenance shop areas. Technology developed and demonstrated under Bureau of Mines Contract H024216, Mine Shaft Fire and Smoke Protection System, may be applied to protect underground maintenance shop areas. The mine area described in this report is a representative ex-ample only; the fire protection system described considers costs, benefits, equipment performance and includes the concepts of: ? Early warning and alarm ? Isolation ? Extinguishment ? Control. Each fire protection system application requires analysis of numerous variables including total mine profile (depth, personnel, escape routes, hazards, etc.).

The stripping of iron-laden solvent with hydrochloric acid in the iron-removal step of the clay-hydrochloric acid process for the extraction of alumina from calcined kaolinitic clay results in a loaded ferric chloride stripping liquor. Treatment of this stripping liquor at boiling with calcined kaolin to form insoluble Fe203(s) and a dilute A1C13 solution was undertaken by the Bureau of Mines to characterize process problems and to aid in sizing of solids-liquid separation equipment. Variables identified as important in the chemical reaction were calcined kaolin particle size, ratio of calcined kaolin to FeC13, and reaction time. These parameters were tested for significance and their interactions investigated. Fine calcined kaolin (-65 mesh) at a greater than 1:1 mole ratio of contained acid-soluble A1203 to Fe203 equivalent of the FeC13 in the stripping liquor required less reaction time than coarser (-16 mesh) material. Solids-liquid separation of the reacted slurries by both settling and filtration requires flocculation.

In the Summer of 2011, emergency repairs to the Jefferson Memorial North Plaza and Seawall were completed. These repairs were necessitated by settlement and lateral movement of the seawall and North Plaza which prompted the National Park Service (NPS) to initiate an investigation as to the possible causes of the movement, and commission a design for its mitigation. Information gathered from extensive instrumentation on site led to the design of a movement mitigation scheme that included demolition of the Ashlar Seawall and reconstruction of a new seawall. The new seawall foundation is an A-Wall consisting of vertical caissons and battered pipe piles connected to the seawall itself. The scheme provides resistance to future vertical and lateral movement of the North Plaza and the new seawall. Repair to the seawall and plaza began in December 2009, and was completed in August 2011. This paper discusses the mechanisms believed to be responsible for settlement and lateral movement, summarizes the procedure followed for design of the mitigation, and describes the construction methods and some challenges faced during the repair.

This case study describes aspects of the design of bridge foundations using 320mm diameter steel tube piles in the vicinity of Montréal, Canada. In this area, these piles are a very commonly used and cost effective solution for deep foundations. Typically they are driven through thick Champlain Sea Clay to bear onto stiff, strong rock. However, in some locations there is a layer of glacial till between the base of the clay and the rock which cannot always be fully penetrated by the piles. There is currently little published information, about the bearing capacity of these piles in the local till, and this paper presents and discusses some pile-test data from a recent project. The nature of the glacial deposits in the study area is presented using information gained from published geological data, the project ground investigations and site observations. The design approach and assumptions are discussed, together with the aims and outcomes of preliminary field driving trials and pile tests. The paper then discusses the results of dynamic tests undertaken on production piles. Bearing capacity factors are back analysed from the dynamic test results and compared against values assumed within the design. Observations are made that designers of future foundations in similar conditions may find informative.

The purpose of the paper is to give an introduction to the concept of fatigue and its causes in the mining industry. By knowing the fundamental role it plays, together with its dynamics, can positively contribute to a mine?s safety management system. The difference between physical and psychological fatigue will be addressed with a possible range of causes that could trigger fatigue. There are two main sources of fatigue: firstly, work-related fatigue which is associated with activities at the workplace; and secondly non-work-related fatigue. The shared responsibility between the employer and employee is also discussed as it involves factors that occur both in and outside the workplace. Employers have the responsibility to manage fatigue through using a risk management approach. Employees have the responsibility to ensure they get enough sleep, take sufficient and regular nutrition, are healthy and physically fit and come to work fresh and alert. The impact of the implementation of a fatigue management plan and procedures has the potential to eliminate employee fatigue or its causes, reduce the likelihood of fatigue occurring in the workplace, and counteract the effects of fatigue when it occurs. Factors considered when implementing a fatigue management system include: extended hours of work, shift work, time of day and work design. Fatigue is one of the major role players (either causal or contributory) when it comes to causes of fatalities in the mining industry. Our main challenge in identifying whether fatigue played a role is the fact that it cannot be tested in post-mortems like drug or alcohol abuse. It is an aspect worth understanding and getting an organization sensitive to managing it properly to benefit both organizational as well as employee needs. The Leeville underground mine has been in operation since 2006 and currently produces 3,500 tons of ore per day with an average gold grade of 0.34 opt. The mine has a large horizontal extent of 2,500 feet by 500 feet and a vertical extent of 20 to 200 feet. Based on the characteristics of the ore bodies, a long-hole stope and cut-and-fill mining methods are used at the deposits which are divided into several mining zones. Cemented rock fill (CRF) is used as the backfill in the open stope. CRF design includes the aggregate design, the ratio of cement and fly ash design, and strengths for the long-hole stope and cut-and-fill. In the aggregate design, the particle size distribution is one of the most influential aggregate characteristics in determining how it will perform as a basic backfill material. Gradation helps determine durability, porosity, cement and water requirements, strength, and shrinkage. To cut the backfill cost, fly ash usually is added into the binder to replace some part of cement. However, adding fly ash can affect the strength of the backfill. The reasonable percentage of fly ash is also important for making a good and low cost backfill. Based on the different mining method, the different backfill with different strength is designed. A systematic quality control procedure for aggregate and backfill is followed. After four yeas application, CRF system in Leeville achieved very good results.

This case history describes the foundation design and construction for the New Mississippi River Bridge, presently under construction in St. Louis, Missouri. The 1,500 ft (457.2 m) main span of the cable-stayed bridge is supported by two delta tower pylons in the river, with anchor piers on each bank. The final foundation design was completed as a part of an Alternate Technical Concept (ATC) proposal by the winning construction team, and utilized drilled shafts up to 11.5-ft (3.5-m) diameter that were socketed into very hard limestone. As part of the ATC foundation design, a full-scale, bi-directional static load test was performed to verify the design and allow use of higher resistance values. This paper presents the results of the site exploration, construction of the load test shaft, details of the load testing, and a comparison of the test results with various design parameters.

The U.S. Bureau of Mines began a five-year study of the 7.0 million acre Ketchikan Mining District during 1990. This study is part of the Bureau's ongoing statewide mining district evaluation program. The study area was subdivided and then prioritized by mineral potential into work units. Bureau field work in 1990 focused on the southern portion of Prince of Wales Island and vicinity. Extensive mineral resources have been mined historically in the area and are presently being evaluated by private industry. Information in this report is based on an extensive literature search and Bureau field work. Over 150 mines, prospects, and mineral occurrences were visited in 1990; an aggregate of 1,300 rock, placer, and limestone samples were taken; and nearly 3 miles of underground workings were mapped. Many prospects, some untouched for more than 80 years, were mapped and sampled. High metal values at several prospects were confirmed by Bureau sampling. Reconnaissance sampling along logging roads on Southern Prince of Wales Island revealed mineralized areas not previously recorded. Follow-up work at many locations is planned for the 1991 season.

The use of helical piles as a deep foundation option has considerably increased in the recent years to support a variety of loads, from small loads for several applications such as residential housing, solar farms, utilities and retrofit projects, to large loads for many applications such as commercial, power transmission lines, oil facilities and industrial applications. Therefore, it is necessary to qualify and quantify their axial capacities and performance characteristics. This paper presents the first full-scale axial compression and tension (uplift) testing program executed on large capacity helical piles installed in cohesionless soils. A total of eleven pile load tests using either single or double-helix piles with shaft diameters that varied between 324 mm to 508 mm (12¾ in to 20 in) were carried out, including seven axial compression tests and four tension (uplift) tests. The results of the axial compressive and tensile pile load tests as well as field monitoring data of helical piles installed in dense sand are presented in this paper. Based on the results of this study it was found that helical piles have developed significant resistance to axial compressive loads up to about 2920 kN (656 kips) and tensile loads up to 2900 kN (652 kips).
