"The current paper presents the results of a study designed to explore the types of practices that are fundamentally important to developing and maintaining comprehensive risk-based health and safety management systems (HSMS) in mining organizations. Eighteen practicing health and safety (H&S) executives, managers, and professionals from a variety of mining commodities were asked to assess the fundamental importance of 20 distinct elements and 133 different practices assembled from a review of several HSMS consensus standards. The research approach assumes that developing and maintaining a robust HSMS requires unwavering attention to activities designed to either: 1) directly contribute to a risk management system’s plan-do-check-act cycle; or 2) build an organization’s internal infrastructure without which the cycle cannot be sustained. The results of this study suggest that the following areas require consistent focus and attention: Leadership Development; Accountability; Knowledge, Skills, and Abilities Development; System Coordination; Culture Enhancement; Behavior Optimization; and Risk Management Studies. Each element and its corresponding top-ranked and top-rated practices are discussed.INTRODUCTION Since the time of the Industrial Revolution, occupational health and safety (H&S) management has been a topic of practitioner thought and academic inquiry (1). In the U.S., decades of regulation and research have resulted in a surplus of prescriptive information (techniques, tools, approaches, standards, programs, practices, policies, etc.). With an abundance of such information, a simple H&S solution to address a problem in the U.S. mining industry may seem straightforward. Counter to this assertion, however, some evidence suggests that the abundance of information may create complexity and cause confusion in the minds of organizational leaders and H&S managers, ultimately discouraging the voluntary pursuit of a sound approach (2). In line with this perspective, in 2006 the U.S. National Mining Association (NMA) formed an independent commission to examine H&S management issues within the mining industry. The findings of the commission called for the mining industry to adopt a risk-based management paradigm. In the presentation of its findings, the commission stated that “…every mine should employ a sound risk-analysis process, should conduct a risk analysis, and should develop a management plan to address significant hazards identified by the analysis” (3). Grounded in the well-known plan-do-check-act cycle, the recommended risk management system allows for mines to ground strategic management choices in their mine-specific hazards and risk profiles"

As it becomes more and more apparent that our nation must place a great deal of reliance on coal as an energy source in the coming decades, prospecting for new reserves Un Missouri and other states in the Western Interior Region is increasing. As more and more of the known reserve becomes committed to existing or planned projects operators will be forced to look beyond the fringes of existing coal fields for additional reserves. Perplexing geologic problems invariably confront all prospectors and are particularly bothersome to those new to a particular area. It is the purpose of this paper to alert prospective operators to geologic problems likely to be encountered and to offer guidance in the recognition and solution of these problems. Particular emphasis is placed upon geologic structure and washouts. Other geologic problems have been discussed in a previous paper.

Introduction People, systems, methods, and organization make up maintenance productivity. Methods promote efficiency in operations, systems ensure consistency and continuity, and organization maintains direction and control. The common denominator in these techniques, though, is people. Management systems can be technically correct, but fail to work because management systems do not work by themselves - people make them work. Therefore, the foundation for mine maintenance productivity must begin with the people involved. Traditionally, the emphasis placed on mine maintenance, as opposed to production, is not proportional to the impact that maintenance has on a mining operation. The performance and productivity of mine maintenance substantially affects the operating costs, revenues, and profits of the mine. The mining industry has recently experienced a period of economic depression. While it has created financial problems in all segments of the industry, it has caused the industry to focus on the need for improved productivity. Mining companies that have made investments in productivity during this period will have a competitive edge during the recovery, and mine maintenance may be the best place to put that investment. Future lost production and maintenance cost problems can be avoided if mine management has the foresight to seize the opportunities that now exist. Background Technological improvements over the last 30 years have been dramatic in the industry. Before these changes, mining equipment was not very sophisticated. So maintenance needs were not very sophisticated. Maintenance people were not required to have any formal training in mechanical or electrical skills. At many mines there was no formal maintenance organization. The mechanic was a part of the section crew and received direction from the face boss. Mine maintenance was not a function, it was a task. Technological change in mining came about in response to an expanding economy and greater demand for mined products. The past 20 years brought about a change in the kind of individual needed to mine ore and coal, and an even greater change in the person needed to maintain equipment. Equipment went from mechanical and pneumatic to complicated electrical and hydraulic. Maintenance progressed from being a task to being a function. Types of Mechanics Mine maintenance workers can be grouped into three categories based on experience: mechanics with more than 20 years, five- to 10-years, and less then five years. These groupings have influenced mine maintenance productivity. The 20-plus mechanic has been the foundation of the maintenance force. He had no formal training and his skills were acquired on the job. As new equipment was introduced, he learned to repair it by trial and error. Not all of what he learned was correct. Personal initiative and conscientiousness are characteristics of this group. They have experienced the bad times and the good, and appreciate the opportunity to work. The five- to 10-year group worked with the older, more experienced mechanics to acquire maintenance skills. These mechanics came in during industry boom times, and many progressed quickly by being in the right place at the right time. Many in this group have difficulty with diagnostic trouble-shooting, and depend on changing out parts to identify problems. This group is generally more mobile and less apt to have a strong work ethic. Mechanics with about five years experience have vocational training, some formal education or basic skills training. Like the five- to 10-year group, these miners are mobile and tend to move readily to satisfy their changing needs. These two younger groups are less apt to feel any loyalty to the employer or obligation to be productive. Their attitudes and abilities reflect the influence of their general age groups, and has a definite impact on mining productivity. Past Requirements In the 1960s, the basic requirements for a maintenance supervisor was that he be a top mechanic, get along well with the men, and be a hustler and improviser. He learned the details of new equipment as repairs were needed. Oftentimes, this individual would be the only person on the property with the ability to read prints, use diagnostic testing equipment, and troubleshoot using an analytical approach. This approach to maintenance is sufficient when the equipment design is reasonably uncomplicated. Management Changes As technology progressed, organized labor gained strength, government regulation increased, and the job of supervising and managing people had to change. A general model which represents not only batch but also continuous flotation in multi-cell machines is presented. This model considers both the size distribution and the distribution of flotabilities in a size class for valuable mineral particles in the pulp. The general expression is shown to reduce to several previously reported models. In addition, a number of new models are found to arise for different specific assumptions concerning the flotability distribution and/or the size distribution of the particles undergoing flotation. Several of these new models are tested experimentally using laboratory-scale batch and pilot-scale continuous data. Model parameters are estimated from this data and a comparison of calculated recoveries and those experimentally determined are presented. Finally, the influence of the feed rate, aeration rate and valuable mineral content of the feed on the recovery is examined.

Introduction The geological processes that form coal can also concentrate trace elements in the coal. For example, the average concentration of arsenic in bituminous coal (20 ppm) is ten times the average concentration found in all the other rocks that make up the earth's crust (2 ppm). Similarly, other elements, such as antimony, cadmium, mercury and selenium, are more concentrated in coal than in the earth's crust. When coal is burned, trace elements can be further concentrated. Although no new constraints on trace clement emissions were placed on the power generation industry under the 1990 Clean Air Act Amendments, the act does mandate a three-year study of air toxics. Any new regulations aimed at electric utilities as a result of this Federally-mandated study will almost assuredly be very costly-an estimated $1 billion per year. Many trace elements in coal are associated with mineral matter. For example, arsenic is commonly associated with pyrite, cadmium with sphalerite, chromium with clay minerals, mercury with pyrite and cinnabar, nickel with millerite, pyrite and other sulfides, and selenium with lead selenide, pyrite and other sulfides (Finkelman, 1980). There are also cases in which some of these elements are organically bound. Just as both organic and pyritic sulfur can be found in the same coal, the same trace element may be both organically bound and present as part of a mineral in the sane coal. Physical coal cleaning techniques are effective in removing mineral matter from coal and can potentially remove at least some of the trace elements associated with specific minerals, thereby reducing the release of these elements into the atmosphere. Conventional coal cleaning to remove trace elements As part of a project funded by the Electric Power Research Institute, CQ Inc., a wholly-owned EPRI subsidiary located in western Pennsylvania, has demonstrated that large reductions in the concentration of many trace elements are possible if conventional coal cleaning techniques are properly applied. Four examples are given in Tables 1 to 4. In each example, the results shown were generated by cleaning the coal at CQ Inc.'s commercial-scale cleaning test facility. Cleaning results for Upper Freeport Seam coal from Northern Appalachia are provided in Table 1. Data are presented in the table in two ways: as a weight-based concentration (parts per million) and as a concentration per heat unit (grams per billion Btu). Grams per billion Btu is analogous to pounds per million Btu, but avoids the use of numbers with many decimal places. The heat-based concentration provides a better measure of boiler impacts, because the increased heating value obtained through coal cleaning reduces the number of tons that must be burned to produce a given thermal output. Reducing the quantity of coal burned reduces the quantity of trace elements entering the boiler. This raw coal is relatively high in several trace elements of environmental concern, including arsenic, cadmium and chromium. Cleaning provided large reductions in the quantity of arsenic, barium, cadmium, chromium, fluorine, lead, mercury, nickel, silver and zinc. The results for tests with a Powder River Basin coal, Rosebud/McKay, are presented in Table 2. Large reductions in arsenic, barium, cadmium, fluorine, mercury, nickel, selenium and zinc were observed with cleaning. The concentration of chromium increased with cleaning, while lead concentration increased in one test and decreased in another. Table 3 presents test results for Croweburg Seam coal from Oklahoma. Large reductions in arsenic, barium, cadmium, chromium and zinc were obtained with cleaning. Smaller reductions were obtained with lead and nickel, while chromium, fluorine and mercury increased in at least one of the tests. Table 4 presents cleaning test data for Kentucky No.11 Seam coal. In this case, large reductions were obtained with all elements measured. In general, these data indicate that physical coal cleaning is effective in reducing the concentration of many trace elements, especially if they are present in the coal at relatively high concentrations. The degree of reduction achieved is coal-specific, relating in part to the degree of mineral association of the specific trace element and the degree of liberation of the trace element-bearing mineral. Mineral exploration consists of the acquisition of data and their interpretation to ascertain the presence or absence of uncertain events. In general, the amount of uncertainty decreases during exploration when targets become increasingly well defined. Decisions on further exploration and whether or not mining should be commenced are based on new information using both objective facts and subjective reasoning. Computers play an important role during all stages of exploration. Three recent books (References 1-3) on decision-making in exploration and mineral-resources evaluation provide additional background supplementing the papers in this volume and articles in scientific journals or other conference proceedings (e.g., References 4 and 5) dealing with the subject. Other new developments to be considered are that microcomputers are becoming increasingly important in the earth sciences (Reference 6), and so are applications of artificial intelligence (Reference 7). Microcomputer- based technology allows rapid communication between field parties and main office. Expert systems are becoming increasingly sophisticated aids for mineral exploration an can now contain objective rules based on statistical theory (Reference 8).

High speed stirred milling is a relatively new method of grinding in the metalliferous process industries. However it has been it has been in wide spread use in the industrial mineral field for over 30 years for various minerals. Many existing metallurgical operations are looking to maximize economic return from existing deposits, requiring new thinking and technologies. The operating experience of two existing copper operations will be covered to show the effect of the introduction of the Metso Stirred Media Detritor (SMD) a new type of high speed stirred mill. Both operations have derived significant metallurgical benefits from the introduction of this new technology. The benefits and process difficulties are covered and explained.

Judging by the inquiries that come to Battelle, a number of companies have run into problems in determining the size and character of markets for mineral fillers in general and specialty fillers in particular. Usually these problems arise during a project to develop a new mineral filler when management suddenly realizes that a considerable sum of money will have to be spent before any profits are realized from sales of the new filler. What management needs in this case is the odds for or against the success of the development. And these odds frequently are controlled by the markets for the new filler. Basically, the problems involved in gaging markets can be simplified to how much of this new filler can be used and by whom? But there are some potential booby traps in these simple questions that should be exposed. This paper will discuss a few of these problems such as: 1. Describing the filler so that the prospective customer recognizes its possible applicability. 2. Finding; a prospective customer who can verify the filler's applicability. 3. Expanding successful test results to total industry markets. 4. Appraising competitive products, and 6. Discovering all the possible uses for the new filler.

INTRODUCTION Frontier-Kemper Constructors, Inc. is an Evansville, Indiana based heavy construction contractor engaged in projects for both the public and private sectors of the underground construction industry. Construction activities range from conventional shaft, slope, tunnel and chamber excavation through state-of-the-art mechanical excavation, ground freezing, standard support and structural concrete operations. Conventional excavation and concreting equipment, as well as raise boring machines, road headers, tunnel boring machines, and freeze plants are owned, operated, maintained and modified "in-house" to suit the needs of individual projects. The balance of this paper wilt present a general overview of the Frontier-Kemper planning and construction operations on the recently completed Phase II of the U.S. Army Corps of Engineers, John Day Dam, Juvenile Fish Bypass Project. General Project History The U.S. Army Corps of Engineers is responsible for designing, constructing and maintaining various navigation and impoundment structures on the nation's inland waterways. Maintenance involves both the work required to maintain the structure in fully-operable condition and the ongoing work required to maintain a positive impact on the environment. John Day Dam is located on the Columbia River approximately 95 km (60 miles) upstream of The Dalles, Oregon. It was constructed to: provide electric power to the Pacific Northwest; provide Locking for up-and-downstream bound commercial and private watercraft; act as a water impoundment structure to control flooding conditions; and to provide a source of irrigation water

The Activity Distribution Diagrams (ADD) developed by the authors are computed for Cu-S-H2O and Mo-S-H2O systems. The distributions of metal and sulfur between all species are estimated against pH at different potential levels. Both M-H2O and S-H2O distributions are compared to the M-S-H2O distributions to estimate the changes occurring due to the presence of sulfur. The method of computation follows the Activity Term Criterion, taking into consideration the multi-equilibrium interrelations and conservation of mass. Results are analyzed in terms of dividing a distribution curve into three basic segments. These segments are formation, stability, and dissolution.

On July 20, 1990, a Spontaneous Combustion Control Plan was approved by MSHA for Jim Walter Resources, Inc. No. 5 Mine. The mine operated under the plan for twenty-nine shifts before MSHA revoked the plan. A total of fifty-five seals were constructed during the process of implementing the plan. The major difficulty in applying the control concept resulted from problems controlling leakage at the numerous seals. The number of seals was a direct consequence of attempting to merge the traditional bleeder system, used in all the Blue Creek Mines, with the bleederless system Jim Walter Resources proposed as a means of controlling the spontaneous combustion hazard. This paper discusses Jim Walter Resources experience with spontaneous combustion control and the new safety standard which applies to underground coal mines that demonstrate a history of spontaneous combustion and how this standard would apply to our No. 5 Mine. The new Ventilation Standards were published in the Federal Register on May 15, 1992, and scheduled to go into effect August 16, 1992.

"A booster fan is an underground mechanical ventilation equipment installed in series with a main surface fan that is used to boost the air pressure provided by the surface main fan passing through it. As mining continues to expand and go deeper, the need for improved and efficient ventilation increases. This has led to the use of booster fans and other auxiliary ventilation devices in underground mines. This paper details experiments and results carried out at Missouri S&T Experimental Mine that investigated air leakage and recirculation caused by the use of booster fans. The results were compared with the situations where booster fans are used and not used.The variables considered were the booster fan blade angle and the fan combinations. Air quantity measurements were taken at each individual stopping under a number of scenarios and across four blade angle settings. The first set of situations examined were; the main surface Joy fan only running, the main Joy, and the west booster fan running, and the main Joy fan and east booster fan running. Observations were made which lead to the conclusion from the experimental analysis that booster fans affect the behavior of leakage and recirculation. The locations of the booster fan and the blade angle setting have the most effect on leakage and recirculation. To limit the potential for system leakage and recirculation, the location and size of a booster fan in a ventilation system should be thoroughly evaluated.INTRODUCTION Missouri S&T Experimental Mine ventilation system The Missouri S&T Experimental Mine has two shafts. One currently serves as the main intake shaft. Air is forced through the main shaft by a Joy Manufacturing Vane Axial fan and follows the direction shown on the map on Figure 1. The air is currently naturally exhausted through the Wheeler portal. As indicated on the map there are eight stoppings in the mine, with stoppings number 2 and 8 fitted with man doors. The mine also consists of three raises which are either fully or partially blocked, two portals with one acting as an exhaust and the other generally closed by a hydraulic Kennedy door."

"Chicago’s Tunnel and Reservoir Plan (TARP) is a nearly $4.0 billion and over 30 years long program and arguably the largest and longest combined sewer tunnel and reservoir system in the world. Recently, a massive tunnel construction is underway for connection of the McCook Reservoir to the existing TARP tunnel system. Once completed in two stages in 2017 and 2019, the reservoir will hold 38 billion-liters (10 billion-gallons) of combined sewer overflows (CSO) and flood waters from the city of Chicago and over 20 communities in Cook County, Illinois. McCook Main Tunnel will connect Chicago TARP’s Mainstream Tunnel to the McCook Reservoir. Connecting two 10-meter (33-ft) diameter tunnels at 92 meters (300 feet) underground has not been a simple task for the McCook Tunnel Project participants, the project owner U.S. Army Corps of Engineers (USACE), local sponsor Metropolitan Water Reclamation District (MWRD) of Greater Chicago, designer Black & Veatch, and the contractor, Kiewit Infrastructure Company (Kiewit). Yet alone excavating and lining massive size tunnels, the team is also challenged with live tunnel conditions in TARP Mainstream Tunnel that carries CSO and flood flows from city of Chicago which runs full during rain events and must remain in service at all times. The project is a noteworthy feat for engineering and construction with the upcoming installation of world’s largest underground wheel gates for flow control within the tunnel. In this paper, we describe one of its kind elements of this major tunnel system along with key design and construction considerations, specifically for the live tunnel connection which is a rare occurrence in the tunneling industry. INTRODUCTION The McCook Main Tunnel system consists of a 10 m (33 ft) finished diameter and 490 m (1,600 ft) long hard rock tunnel constructed from a 27.5 m (90 ft) diameter and 92 m (300 ft) deep main gate and tunnel construction access shaft. The gate shaft houses six high head 4.4 m by 9 m (14.5 ft by 29.5 ft) wheel gates to be installed in the bifurcated and steel lined section of the tunnel. The tunnel also includes portal and energy dissipation structures as it daylights into the reservoir."

The use of service variables in geostatistics (grade-thickness) is common in industry when developing block grade models and geologic resource estimates. This is particularly true in the case of sedimentary and strata-bound-type deposits. The approach has proven merits when assessing ore reserves. However, there are cases where its application for preliminary data analysis produces biased estimates. One such case is encountered in uranium disequilibrium studies. This paper demonstrates the inadequacy of service variables when performing uranium disequilibrium studies. Disequilibrium analysis is a necessary pre-processing step when radiation counts are used as raw data to calculate uranium reserves. Typically, a probe is used to obtain the gamma counts from open drill holes. These counts are then regressed (and corrected) against chemical assays obtained from core samples. During this process, the grade-thickness variable produces an optimistic image of the relationship between probe grades and chemical (true) grades, causing overestimation of the final geologic resource. The problem is analyzed through a case study.

The productivity of the underground driller has increased 500-600% during the past 20 years. This increase is not due so much to improved percussion drills as it is to improved drill support equipment and modernization of mining methods, Efforts to mechanize drilling in mining operations were made as early as the late 1800's; and, patents on rock drill manipulating machinery date back to 1906. However, the great rush to mechanization began in the 1950's when the development of trackless mining methods gave mining engineers and equipment designers their first opportunity to create improved operating conditions for the equipment and to match the equipment with these conditions. Accordingly, this paper will summarize the developments of the past 20 years and will attempt a forecast of developments that can be expected during the next few years.

As a consequence of the oil crisis of the 70?s, the commercial value of coal increased very sharply and an upward trend in coal prices although now less pronounced, continues today. The graph shown in figure 1 illustrates the trend in the F.O.B. price of U.S. metallurgical coal. This trend occurs during a time when mechanized mining is firmly established as a means of production throughout the coal mining world. With modem methods of mining, mechanical cleaning is a necessity and quality control is no longer entirely dependant upon coal face practice. The efficiency of fine coal recovery is of vital economic importance to present day operations and good practice in this field contributes significantly to improving profitability. Numerous processes are available for the duty and this paper discusses one such system, namely the practice of froth flotation.

INTRODUCTION In the past decade, the international coal trade has been undergoing a period of gradual change. This change will continue through the remainder of this century until by the year 2000 the international coal industry will look very different from the industry which existed in the early 1970's. The industry which will emerge from the thirty year period of change (1970-2000) will be different in many ways. -The volumes of coal transported will be much larger. - The primary type of coal traded will be steam coal rather than metallurgical coal. - A much broader group of countries will be participating in the trade both as suppliers and as consumers. - The trade will be dominated by a relatively small number of large producing and trading companies. - The coal mined for international shipment will be primarily surface- mined. – The United States will play a decreasingly important role in international coal trade. In the course of this paper I propose to address three questions which are crucial to the formulation of an understanding of the changes outlined above : (1) Which supplier nations will be most successful in the international metallurgical and steam coal trade in the next decade and a half? (2) What emerging trends will shape the international coal trade in the 1980's and 19901s? (3) Where are U.S. producers likely to fit in this scheme of things? As a background to my remarks, I should note that what follows is based on five years of work in a consulting capacity on coal supply and demand issues worldwide. Although in the course of this work I have had occasion to develop mining and transportation cost figures for all of the countries discussed below--both my own and those obtained from coal producers--it is apparent that success in the marketplace can be determined in some instances by considerations other than costs. Nevertheless, my comments today about international competitiveness will be dominated by cost considerations; there are, after all, not many markets that reward the high-cost producer with success. Let us begin by establishing some criteria for success in the international coal market and then compare some of the producer nations to some of these criteria. The criteria are: - Low delivered cost, - Good quality coal, - The producer's partnership in a joint mining venture with an importer or a consumer of coal, - The producer's international business orientation and commitment to penetrating the market, - The producer's reliability as a supplier, and - Political pressure exerted by the producing country on consumers.

The advantages of producing a fine mill feed have been recognized for many years. The extent to which fine crushing can be carried out will vary and depends on the ore characteristics, plant and crusher design. Unfortunately, the ore characteristics are generally beyond our control, so the concentration of effort lies primarily on the plant arrangement, equipment design and operation Where fine crushing should stop and grinding begin is controversial and is influenced greatly by personal opinions and experiences. Often, this break point will be different if consideration is being given to a new plant in comparison to a modest expansion. Most new plant designs employ the largest and most efficient equipment. The modification of existing plants becomes more complex because of the space requirements and the related equipment already in operation. Fine crushing is, of course, a relative term. Obtaining a final crusher product of minus 3/4" is commonplace today and can occasionally be obtained in an open circuit final crushing stage.

Historically, research organizations have made significant contributions to the development and commercialization of new chemical products for the mining industry and examples of commercial results are cited With the recent demise of research organizations within the mining industry and dynamic changes in the industry, the supporting role of R&D groups has become limited or non-existent. Cat- reduction requirements and new raw-material sources offer future opportunities for R&D organizations to continue their effective participation in the commerical development of new chemical products. Potential areas of interest for future process applications are reviewed.
