INTRODUCTION Measurement of certain data associated with nuclear testing necessitates the reception of waves at the instrument location which are unaffected by reflection off internal structures or tunnel surfaces. Therefore, in order to provide necessary facilities for the fielding of experiments in support of the underground nuclear testing program, the Defense Nuclear Agency (DNA) commissioned a cost and feasibility evaluation pertaining to the design and construction of large underground cavities in Rainier Mesa. The effort included research into past experience pertaining to the excavation and support of large underground caverns, assessment of the geological conditions which exist in Rainier Mesa, preliminary design of the chambers and their rock support systems based upon the geological setting, development of mining plans for the excavation of the caverns, and estimation of the cost and manpower schedule to complete construction. This paper summarizes the results of the feasibility evaluation. The investigation was limited to modified hemispherical configurations ranging in size from 24.4 to 91.4 m (80 to 300 ft) in diameter. Two cavity support systems were considered. The first employs internally installed rockbolts as the means of primary rock support while the second involves the use of annular tendon galleries located above the chambers from which primary support members are introduced prior to general excavation. The internal support configuration was considered for cavities with spans of 24.4, 36.6, 48.8, and 54.9 m (80, 120, 160, and 180 ft) while the external support configuration was considered for cavities with spans of 54.9, 73.2, and 91.4 m (180, 240 and 300 ft).

The trapezoidal tapered ring method has made significant progress in recent years for shielded-TBM projects, excavating tunnels with high overburden, strict water tightness criteria, and multiple curve alignments. The selection of compatible tapered rings is critical for successful advancement of the tunnel, assuring efficient installation of rings, as well as prevention of ring damage and excessive leakage. Using experience from the Arrowhead Tunnels Project, this paper describes guidelines for ring selection in tunnel curves, geometric analyses that comply with criteria principals, and provides techniques developed to determine the appropriate relative key position.

This paper reviews difficult ground conditions encountered along the alignment of large-diameter sewage tunnels for the City of Los Angeles. It offers a detailed one-of-a-kind solution to a unique problem for shallow urban underground tunnels in a seismic zone, using state-of-the-art materials, technology, and equipment. On Nov. 13, 1985, Colombia's Ruiz volcano erupted, killing an estimated 25,000 people and burying several villages. Damages were estimated to be about $US300 million. The following is an account by SME member Antonio Rivera of the volcanic activity that led to the eruption. His account also includes a description of the geography and some geology of the affected area. Mining operations in the area were mostly unaffected, although some operations were forced to close until the roads leading to them could be repaired. Introduction The Ruiz volcano is located on the Central Cordillera of Colombia about 28 km (17 miles) southeast of Manizales, the capital of the state of Caldas. Ruiz is one of nine volcanoes that form a complex volcanic chain within the middle portion of the Central Cordillera. The Cordillera has 30 other volcanoes in the southern part of Colombia. Ruiz volcano Ruiz's elevation is 5200 m (17,060 ft). It is covered by an ice cap estimated to be 20 m (66 ft) thick at its summit, an area that covers 17 km2 (6.5 sq miles). Drainage is composed of several rivers that flow into the Magdalena River on the east and Cauca River on the west. The rivers flowing to the east are Guali, Azufrado, Lagunillas, and Recio. The rivers that flow to the west are Claro, Molinos, and Chinchina. There is a road from Manizales to El Refugio, a ski area located on the northeast flank of the volcano at 4800 m (15,750 ft) in elevation. The Ruiz volcanic complex was built in two stages. The first is a shield type of volcano (mainly lavas) formed about a million years ago. The second is a younger stratovolcano overlaying the ancient one. It consists of lavas and pyroclastics and was built over the past 500,000 years. Twenty-four eruptions of Ruiz are known to have occurred during the last 14,000 years. Its products mainly consist of lava flows, pyroclastic flows, mudflows (lahars), and directed lateral explosions. Historic eruptive record 1595 - Debris avalanche, blast, pyroclastic flows, mudflows, ash flows, and ash fall. 1828-1829 - Ashes (phreatic). 1831-1833 - Gas emissions and ashes. 1845 - Mudflows and pyroclastic flows. 1985 - Sept. 11- Fumarolic activity, phreatic eruption, ash fall, and small mudflows. 1985 - Nov. 13 - Ash and pyroclastic fall, major destructive mudflows (magmato-phreatic eruption). From 1845 through 1984, Ruiz volcano always had a slight fumarolic activity. It was restricted to the crater area. In November and December 1984, the fumarolic activity increased and the first ash emission and vapor-sulfur formation started. Small local earthquakes accompanied these volcanic activities. The volcano had a small erup¬tion on Sept. 11, 1985. It was manifested by phreatic explosions that ejected fragments of the vent and formed ash deposits 25 mm (1 in.) thick around an area 2.5 km (1.5 miles) from the crater. Fine material was blown by the wind and reached Manizales and neighboring areas, producing a small layer of ash up to 10 mm (0.4 in.) thick.

Introduction Guar gum is a natural nonionic polysaccharide. Guaran, the functional polysaccharide in guar gum is a chain of (1?4)-linked ß-D-mannopyranose units with a-D-galactopyranose units connected to the mannose backbone through (1?6) glycosidic linkages. The poly-mannose chain is randomly substituted with galactose units at a mannose-to-galactose ratio of 1.8-1.0 [1]. Cheng et al. reported that the molecular weight of natural guar gum is 1.935 • 106 [2]. Robinson et al. found that the molecular weights of five grades of commercial guar gums ranged from 4.4 • 105 to 16.5 • 105 [3]. Guar gum is frequently used as a depressant of naturally hydrophobic gangue minerals (e.g. talc) in the flotation of PGM-bearing ores, and according to Harris et al. a wide variety of modified guar gums are available for this application [4]. The Canadian potash industry utilizes natural guar gum as a blinder of water-insoluble slimes in the flotation of potash (sylvinite) ores, a process that is carried out in a saturated KCl/NaCl brine. The role of the polysaccharide is to prevent a primary amine collector from adsorbing onto these unwanted fine minerals. Hydrophobic interactions between basal cleavage planes and the polymer appear to be involved in guar gum adsorption on talc [5,6]. Liu and Laskowski stressed the importance of metal impurities on mineral surfaces that can serve as the active sites for polysaccharide adsorption [7]. This postulate was consistent with the later observation that, in addition to hydrogen bonding, the adsorption of guar gum onto talc was strongly dependent on the surface concentration of magnesium sites [8,9]. In this case the adsorption process was visualized as metal-polysaccharide complexation facilitated by the cis-configuration of the hydroxyl groups of mannose monomers. Hydrogen bonding and chemical interactions with metal sites were also suggested by Rath and Subramanian in their studies on guar gum adsorption on biotite mica [10]. In a recent study, Wang et al. demonstrated that hydrogen bonding is the main driving force behind guar gum adsorption onto talc [11]. Their data also show that the adsorption density of guar gum on alumina is about five times lower than the adsorption density on talc. Pawlik and Laskowski reported that the amount of guar gum adsorbed on illite was independent of ionic strength over a wide range of salt concentrations [12]. The adsorption density of the polysaccharide on illite did not measurably change with ionic strength from that of distilled water to about 50%-saturated KCl/NaCl potash brine (~3N KCl/NaCl). However, guar gum adsorption on dolomite dramatically decreased over the same range of ionic strengths, and no reliable explanation of that phenomenon was proposed [12]. The water structure-breaking or structure-making capabilities of background ions are usually ignored when discussing adsorption mechanisms of polymers. The concept of “structure breakers” and “structure makers” was first introduced by Gurney, and Frank and Wen who postulated that each ion is surrounded by three distinct regions of water structure [13,14]. In the first layer, water molecules are tightly bound to the ion. The second region extends farther away from the ion and is referred to as the region of structure breaking. Only at larger distances, where the ionic field is weak, water molecules form the “normal” ice-like structure. It is also now recognized that the first layer can be further divided into the primary and secondary hydration shells. Small, strongly hydrated ions reinforce the “normal” structure of water and the region of structure breaking disappears. In contrast, large less-hydrated ions disturb the ice-like structure and generate an extensive region of structure breaking. Experimentally, these two effects can readily be distinguished through viscosity measurements. Increasing concentrations of structure-breaking ions result in a decrease of the viscosity of aqueous solutions. On the other hand, increasing amounts of structure-makers significantly reduce the viscosity of water. Alkali metal cations form a sequence known as the Hofmeister series. Conventional barrier pillars in underground coal mines are intended to protect mains and sub-main entries for relatively long periods of time. Inter-panel barrier pillars that may be used in especially deep coal mines, say, below 800 m (2,400 ft), are intended to protect longwall panel faces and adjacent panel entries for the life of nearby panels. Barrier pillar failures have serious consequences, especially when failure is sudden and catastrophic. Design of barrier pillars is thus of considerable importance to safety and efficiency of operations. Locally, failure occurs at the limit to elasticity when stress tends to exceed strength. Strength of rock is stress dependent, so an analysis of stress is central to rational design of barrier pillars. There are several approaches to stress analysis for coal mine pillar design that range from the simplified but popular tributary area method including variations that use empirical definitions of the tributary area (volume) to more sophisticated numerical methods such as the boundary element method and the finite element method. Despite ease of use, empirical methods are limited and always suspect when applied to conditions beyond their scope of definition, while boundary element methods do not generally include pillars in the solution domain. The finite element method is not so restricted and thus is a natural choice for conventional and inter-panel barrier pillar safety analyses. Caving of roof strata certainly affects the distribution of stress about longwall faces and nearby barrier pillars. Conventional wisdom that considers roof strata to respond as cantilever beams indicates the longer the span the greater are loads on longwall faces and face supports. Failure of roof beams, caving, shortens spans and relieves face and support loads. The same view is applicable to barrier pillars in that long roof spans outside of barrier pillars leads to higher pillar loads than short spans. Hence, caving should be beneficial to barrier pillar safety. However, the benefit may be small depending on the extent of caving and only in the form of a slight reduction in overburden weight transmitted to the barrier pillar, especially in case of inter-panel barrier pillars. FINITE ELEMENT MODEL Finite element models for stress analysis in rock mechanics are formulated in consideration of physical laws, kinematics, and material laws. These models meet the essential requirements for equilibrium of stress, satisfy strain displacements relations, and in a general sense, relate stress to strain through material laws, often referred to as constitutive relations. The last may be complex and replete with nonlinearities. A de facto engineering material law is the elastic model in the form of Hooke?s law for linear, homogeneous materials that may be anisotropic. A limit to elasticity is imposed by strength in the form of a failure criterion or yield function that accommodates triaxial stress states. Examples used in rock mechanics are the well-known Mohr-Coulomb (MC)criterion, the popular Hoek-Brown (HB) criterion, and the Drucker-Prager (DP) criterion. The first is linear in stress, while the second is nonlinear. Both do not include any effect of the intermediate principal stress. The last is also linear in stress but includes the intermediate principal stress. All show an increase in the major principal stress at failure with confining pressure. In the UT2/3 finite element programs, the failure criterion Y is a nonlinear, anisotropic form and is also the plastic potential [1]: [YJI=+ 11NYJI=+- (1) ] where N is an exponent, usually 2, and J2 and I1 are nonlinear anisotropic forms of the second invariant of deviatoric stress and the first invariant of principal stress. The form (1) reduces to DP when N=1and isotropy prevails. When Y = 1, the limit to elasticity is reached and plastic deformation is possible. The material constants that enter (1) are all computed from unconfined compressive, tensile, and, in case of anisotropy, shear strengths. These latter strengths may be estimated from conventional laboratory test data. Of course, joints in field-scale rock masses induce different values. How different is often a matter of conjecture. For comparative studies, laboratory rock properties seem adequate. Inelastic, that is, plastic behavior occurs with strain beyond the elastic limit. Strain hardening, ideal plasticity, and strain softening are possibilities. Ideally brittle behavior with complete loss of strength is an extreme form of strain softening [2]. Caving may also be considered as strain softening with a less drastic reduction of material properties. As intact roof strata fail, fall, and form a rubble pile below, a new material is formed with greatly reduced elastic moduli and strengths, say, 1/100th of the original intact moduli and strengths. From another viewpoint, caving is simply a natural cut and fill process. Input for stress analysis by the finite element method includes: 1. Preexcavation stress state 2. Geology (stratigraphic column) 3. Structure (major faults, joint sets) 4. Elastic moduli 5. Strengths, and specific gravities of each rock type 6. Excavation sequence Output from an analysis includes distributions of: 1. Stress 2. Strain 3. Displacement 4. Local factor of safety (element fs) The last is defined as the ratio of strength to stress and in view of the failure criterion (1), is the ratio [fsJstrengthJstress=] where the numerator is the maximum possible value and the denominator is the actual value of the square root of the second invariant of deviatoric stress. This invariant is a measure of shear stress intensity but also reduces to uniaxial tensile and compressive strengths as the case may be. DOUG HALBE: Second in our quick run-through on "Operating Ideas That Have Saved Me Some Bucks," was Debbie Laney of Rayrock Mines. Debbie apologizes. She can't be here. But she said, "Doug, don't worry. I've asked Mary Korpi to present the paper. We're both short, blond, and wear glasses. Nobody will know." [laughter] Mary, I caught it right away. You ladies can't fool us. Debbie, who wrote the paper, has spent almost her entire career in operations. She is currently metallurgical manager of Rayrock Mines in Valmy, Nevada. Mary Korpi is also a real operator (that doesn't sound right). She has been responsible for a number of operating areas at Newmont's Nevada operations, and she is currently taking a break from operations as manager of public relations/employee communications for Newmont Gold in Carlin. MARY KORPI: They told us this year at Newmont that we were going to have to work twice as hard. I had no idea it meant I would have to work for another company, too. Over the years, and many times in association with work for mid- to small-size mining companies, there have been two areas that seem to have been the focus of cost savings when looking at new facilities or new processes.

Soil erosion is strongly related to soil hydraulic properties. Understanding how surface coal mining affects these properties is therefore important in developing effective management practices to control erosion during reclamation. To determine the impact of mining activities on soil hydraulic properties, soils from undisturbed areas, areas of roughly graded mine spoil, areas with replaced topsoil before seeding, and revegetated areas at the Rosebud Mine in Eastern Montana were evaluated. Field and laboratory studies were conducted to determine the soil hydraulic properties of the four representative areas, including particle-size distribution, organic matter content and saturated hydraulic conductivity. Using this data, Water Erosion Prediction Project (WEPP) simulations and risk analyses were performed to investigate mining’s effects on changes in soil erosion potential. Significant differences were seen in the hydraulic properties of the soils in the representative areas due to the mining and reclamation practices at the mine. The WEPP simulations show that the potential for soil erosion increases as a result of mining activities disturbing the soil. The simulations also indicate that the erosion potential may return to pre-mining levels over time with effective revegetation practices.

Led by a surge in coal production and prices in recent years, Colorado’s mining industry is taking part in the same revival as others states and the world. Not too many years ago, the state’s industry was languishing in the same doldrums as the rest of the mining world. Ironic for a state whose history was based on mining. Now, however, Colorado’s mining can boast increased production in all commodities produced, along with prices that allowed companies to expand, increase work forces and post improved shareholder value. So there was a lot of optimism at the Colorado Mining Association’s (CMA) 107th National Western Mining Conference in February. The conference, held in Denver, attracted more than 500 attendees and 39 exhibitors. The theme was Colorado Mines the World.

The management of mine tailings poses a complex range of potential risks, including physical risks, such as the risk of catastrophic failure of a tailings facility and chemical risks such as the risk of long-term leaching of contaminants into the downstream environment. If these risks materialize, they can lead to a wide range of impacts on people and the environment, including human fatalities and severe impacts on downstream ecosystems, and the costs of recovery borne by industry, communities, governments, and society as a whole can be immense. Tailings facility failures in Canada (2014), Brazil (2015 and 2019), South Africa (2022), and in other locations around the world underscore how high the stakes are when mines produce and manage tailings (WISE Uranium Project 2019). Understandably, many people do not trust the mining industry to manage tailings responsibly. As an industry, it is our responsibility to manage tailings in a manner that is responsible and strives to minimize harm associated with tailings management, including effectively managing both the physical and chemical risks associated with tailings. Standards have an important role to play in responsible tailings management by setting expectations for standards of care and providing a means to measure and report on performance. In jurisdictions with strong legal requirements for tailings management, standards play a complementary role, pushing performance improvements in areas that may be difficult or impossible to address in legal requirements. In jurisdictions where legal requirements are lagging, standards may represent the de facto leading practice and may be the primary tool to help ensure responsible tailings management. Implementation of standards can help companies demonstrate that tailings are being responsibly managed and help build trust and credibility. Most importantly though, standards are important tools to help ensure that the risks associated with tailings are reduced, and that remaining risks are effectively managed. However, we must recognize that conformance with standards can have a down-side – the proverbial two-edged sword. Having standards is vital but there is a risk that implementing standards becomes a “box checking” exercise, with ongoing conformance leading to complacency. We must not allow ourselves to fall into the trap of believing that, just because we are complying with applicable standards and laws, we are therefore doing our best, and that “it can’t happen to us.” In addition, if conformance becomes the goal and the standard does not have mechanisms to drive continual improvement, then once the standard is met, there may be no incentive to improve. The point of complying with standards is not to pass the test. The point is to manage tailings responsibly, prevent catastrophic failures, and minimize harm. The point is to do better, because we must do better. To do that, we need to strive not for conformance, but for excellence in tailings management, driven by continual improvement and a quest to do better.

Rhenish lignite will continue to play an indispensible role in the energy mix of the German Federal Republic for a long time to come. The use of lignite for electricity generation, upgrading and, if necessary, as a raw material is geared to the long term. The holistic management of opencast mines underpinned by approval law in the long term and a stronger focus on man, the environment and nature contribute to the acceptance of the mines by the local population. The same is true of state-of-the-art lignite uses: as regards their operational flexibility, lignite-fired power plants are ideal partners for the expansion of renewables. It is along these lines that RWE is continuing its power plant renewal programme as planned. With a view to that which will be conceivable the day after tomorrow, RWE is researching future technologies today in order to meet objectives such as security of supply, profitability and environmental compatibility to the same extent in the long term. Another important contribution to acceptance is public transparency and communications as means to support approval procedures and the implementation of measures in all areas related to lignite mining and utilization.

When considering all the aspects of a viable mine plan, it is important to estimate the accurate cost of mine ventilation, which varies as a function of airway dimensions, fan selection and ventilation network. Optimization of the overall elements of ventilation design can lead to very significant cost savings when capital and operating cost savings are considered in combination. This paper will describe a methodology, whereby ventilation costs can be minimized by optimizing the ventilation aspects of mine design and fan selection, by providing a brief description on ventilation theory and the factors which affect ventilation operating and capital costs.

Mineral extraction activities in the deep sea will expose the local fauna to potentially toxic compounds. However, very few data are currently available on the complex ecotoxicological risks to deep-sea fauna from the potential release of heavy metals, trace metals and Rare Earth Elements (REEs). Nonetheless, this is key information essential to support the regulation of industrial activities in the deep sea. One of the fundamental issues that the MIDAS project will address consists in exploring the concept of whether the existing toxicity test protocols and limits established in shallow-water species are applicable to deep-sea mining. We will detail the specific challenges to recommending exposure limits within the deep sea and show preliminary results of comparative toxicity tests in the shallow-water and in the deep-sea environment.

An assessment of potential groundwater movement beneath the tailing impoundment at the Red Dog Mine was performed, and has indicated a unique aquifer situation within sub-permafrost groundwater at the site. The aquifer was shown to be isolated from both vertical recharge and a real discharge, and is characterized as a virtually no-flow system. The permafrost resulted in a deep aquifer with high tidal efficiency, meaning that variable surface loads are easily transmitted to the fluid. The tidal efficiency has been characterized through analysis of water level fluctuations that respond to earth tides, barometric pressure loads, and loading from the pond.

"The District of Columbia Water and Sewer Authority (DC Water) is implementing the $2.2-billion DC Clean Rivers Project (DCCR) to control combined sewer overflows (CSO) to the Anacostia and Potomac rivers and Rock Creek, and to provide flood relief to the Bloomingdale and LeDroit Park neighborhoods in the District of Columbia. The long-term control plan complies with the requirements of a Federal Consent Decree entered into by DC Water, the District of Columbia and the United States, as represented by the U.S. Environmental Protection Agency and the Department of Justice. The Northeast Boundary Tunnel, a design-build project, will store combined sewage and deliver it to a system of downstream tunnels and ultimately to the Blue Plains Advanced Wastewater Treatment Plant (Blue Plains), where it will be treated. In addition, the Northeast Boundary Tunnel (NEBT) is a major project associated with the mayor’s task force to alleviate extreme flood events in Bloomingdale and LeDroit Park. The Northeast Boundary Tunnel Utility Relocations (NEBTUR) design-bid-build (DBB) project is an accelerated component of the Anacostia River System, as shown in Fig. 1. During dry weather conditions, the entirety of the sewage in the combined system is conveyed to Blue Plains.During storm events, when sewer pipe capacity is exceeded, the flow, which is a mixture of sewage and stormwater runoff, overflows into receiving waters through outfalls. There are a total of 47 active CSO outfalls along district waterways.The influx of storm runoff into the combined sewer has an additional impact on the communities of Bloomingdale and LeDroit Park. The sewer system is outdated and undersized. In these neighborhoods, when the capacity of the sewer is exceeded, localized flooding occurs and backups into adjacent residences are prevalent. In any given year, there is a 50-percent chance that flooding will occur in these neighborhoods."

Research has shown that excessive soil compaction is detrimental to the establishment of trees in reclaimed soils. Dry bulk density and penetration resistance are two soil parameters that can be used to indicate the level of compaction in reclaimed soils. It is believed that reclamation practices which minimize soil compaction will produce better results when forestland is the intended post-mining use. The University of Kentucky has developed reforestation test cells on reclaimed soils at a surface mine in eastern Kentucky to test this hypothesis. Each test cell has been planted with various species of trees, and since 1997, data have been collected at these test cells for soil average dry bulk density, average penetration resistance, average depth to refusal, and average survival rate of trees. These data have been correlated to determine the relationship between soil compaction and tree survival rate. It is believed that the two soil compaction parameters may ultimately be used to predict tree survival rates on reclaimed surface mined lands.

This paper describes the overall delivery of the tunneling for the Mass Rapid Transit Authority of Thailand’s Chaloem Ratchamongkhon Line in Bangkok and the constraints provided by the alignment. Eight earth pressure balance tunnel boring machines were employed under two major civil works contracts to drive approximately33 kilometers of tunnel in soft saturated ground beneath the crowded streets of Bangkok. Responsibility within the contract for building damage provided an inherent control for ground movement arising form tunneling when the right of way was narrower than the influence zone. These limits affect the earth pressure balance requirements and permissible ground loss. The methods adopted to limit ground movements and deal with the various obstacles to tunneling influenced the design and operation of the machines. The challenges provided by the need to tunnel through numerous lines of piled foundations of canal bridges and overpasses crossing the alignment are described.

This paper presents the results of a review of historic blast design practices at the Dragon Products Company, LLC (Dragon) Quarry Operation, along with the results of two different blast design vibration monitoring programs. A blast pattern design modification was initiated to determine if production volume per shot could be increased and highwall instability could be decreased. A vibration monitoring program was undertaken to investigate peak particle velocity (PPV) and relate the data to the blastability of the jointed onsite rock mass, utilizing the relationship between PPV and dynamic stress and strain. The Dragon Quarry Operation has historically used a 6-in. diameter, 14-ft by 14-ft production blast pattern, with blast holes loaded manually using cartridge style explosive products. In 2011, Quarry Operations introduced a 6-in diameter, 16-ft by 16-ft production blasting pattern, using pre-mixed, truck-loaded, bulk emulsion/ammonium nitrate/fuel oil (ANFO) blends. This paper summarizes the results of the blast design modifications, the results of the vibration monitoring data, and describes how production blasting and excavation at the Dragon Quarry has benefited from such modifications.

From 1999 through 2008, groundfall events resulted in 75 fatalities, 5,941 injuries and 13,774 noninjury roof falls in U.S. underground coal mines, according to the U.S. Mine Safety and Health Administration (MSHA). A comprehensive analysis of these events was conducted to better understand where and why these incidents occurred. The first segment of the study used the MSHA database to evaluate groundfall injury rate trends related to mining method, year, locality, seam thickness, mine size, coalbeds and seasonal effects. In the past decade, roof fall injury rates have dropped 50%, but rib fall injury rates have remained constant. Room-and-pillar mines had a roof fall injury rate that was nearly 2.5 times the longwall rate. High groundfall rates occurred in the Illinois Basin, a finding that correlates with other studies that have shown that roof rock in the Illinois Basin is weaker than that in the central Appalachian coalfields. Finally, noninjury roof fall rates in the eastern and central United States were found to be 50% higher from July through September, possibly because of the changes in humidity levels that may cause shale mine roofs to deteriorate. The second segment of the study evaluated the root causes of failure by reviewing all MSHA groundfall fatality reports for 1999-2008. Pillar recovery was the leading cause of groundfall fatalities (19%), followed by rib falls, roof skin falls and traveling under unsupported roof (16% each). Defining prominent ground control incident trends and hazards will identify areas where additional study is needed and where innovative solutions need to be developed to reduce these severe occupational hazards.

INTRODUCTION Bratislava is located at the intersection of important traffic routes, including motor-ways, roads, railways, air traffic, trading, and inland waterway with an important harbour. The construction of the D2 Motorway Bratislava, Lamacska cesta Stare grunty was the last missing section of D2 Motorway and its completion linked the Czech Republic through Bratislava to the Hungarian Republic. Traffic was obliged until its completion to use the Lamacska Road and Mlynska Dolina, which were the most heavily trafficked roads in the city. The section of D2 Motorway to be completed in 2006 was 3,300m in length, of which the SITINA Tunnel was approximately half the length. After putting into operation the transit traffic(motorway) and urban traffic was separated. Transit traffic by passing the town was eased. The junctions D2 with Lamacska Road (Harmincova) and Mlynska Dolina also allowed better use of the motorway for the urban traffic The tunnel construction was specified by the features with the highly weathered and thermally altered granitic rock formation with the overburden of Quaternary sediment of 2?4 m thick, very thin overburden, severely controlled deformation limit and environmental protection. The tunnel had to be excavated beneath and nearby the academic complex area and the zoo, where sensitive facilities exist and animals live in. The tunnel consisted of two tunnels (Tunnel A and B) with the length of 1,415 and 1,440 m, respectively. The overburden thickness was in the range from 10?30 m. The standard cross-section and the one with invert arch were 79.5?81.5 m2 and 92.1?93.3 m2. The excavation took 21 months from September 2003 to May 2005. The construction was based on the concept of NATM. The SITINA Tunnel was the third application of NATM in Slovakia, following the BRANKISKO Tunnel (commenced in July 2003) as the first and the HORELICA Tunnel (in October 2004) as the second NATM practice. To protect the surrounding structures and citizens from tunneling operations, seismic monitoring was performed in association with blasting. The allowable limit of blasting vibration was fixed to be under the particle velocity of 15 mm/sec. The tunnel excavation cleared this limit as expected before construction. In driving through the difficult geological situation the excavation work hit 49 faults, and some of them occasionally caused serious face instability. In principle, the rock formation has been affected by the high hydrothermal effect and the resultant clay filing along faults and joints. The auxiliary reinforcement with the 4?8 m long pipe roofing was needed to protect the vicinity of mining face area. This operation extraordinarily required additional works and time which had not been expected before construction.
