The section has been driven about 4,000 feet from 4 West Mains. One of the shuttle car operators took a call from the fireboss saying that there was smoke from an unknown source coming into your section. You attempt to contact someone to find out where the smoke is coming from but get no response. You assemble the crew and start riding out of the section on the mantrip when you encounter heavy smoke. At this point, you decide to take your crew and travel on foot in the belt entry that is on a neutral split of air. After traveling about 6 crosscuts in the belt entry, you encounter heavy smoke at which time you and your crew don your person-wearable SCSRs (PWSCSR). After donning the apparatus, you and your crew continue traveling outby in the belt entry. Near the mouth of the section, one miner in the group starts to have trouble breathing from his SCSR. Because breathing resistance is increasing with the length of time the apparatus is worn, the miner is "outbreathing" the device since his oxygen requirements are greater that what the PWSCSR can supply. It is several days after the accident and the mine superintendent assigns you and the safety supervisor to thoroughly investigate the accident to determine all the contributing factors. Coal pillar design has been based on generalized formulas of the strength of the coal in a pillar and experience in localized situations. Stress measurements above and in coal pillars indicate that the actual strength and deformation of pillars vary much more than predicted by formulas. This variation is due to failure of strata surrounding coal. The pillar strength and deformation of the adjacent roadways is a function of failure in the coal and the strata about the coal. When the pillar is viewed as a system in which failure also occurs inthe strata rather than the coal only, the wide range of pillar strength characteristics found in the United Kingdom, United States, Republic of South Africa, Australia, People's Republic of China, Japan, and other countries are simply variations due to different strata-coal combinations, not different coal strengths. This paper presents the measured range of pillar strength characteristics and explains the reasons. Methods to design pillar layouts with regard to the potential strength variations due to the strata strength characteristics surrounding the seam are also presented.

Empirical methods involve the scientific interpretation of real-world experience. Many problems in ground control lend themselves to an empirical approach because the mines provide us with plenty of experience with full-scale rock structures. During the past 10 years, powerful design techniques have emerged from statistical analyses of large databases of real-world pillar successes and failures. These include the Analysis of Retreat Mining Pillar Stability (ARMPS), the Analysis of Longwall Pillar Stability (ALPS), the Mark-Bieniawski rectangular pillar strength formula, and guidelines for preventing massive pillar collapses. In the process, our practical understanding of pillar behavior has been greatly enriched.

Pillar design is the first line of defense against rock falls—the greatest single safety hazard faced by underground coal miners in the United States and abroad. To help advance the state of the art in this fundamental mining science, the National Institute for Occupational Safety and Health organized the Second International Workshop on Coal Pillar Mechanics and Design. The workshop was held in Vail, CO, on June 6,1999, in association with the 37th U.S. Rock Mechanics Symposium. The proceedings include 15 papers from leading ground control specialists in the United States, Canada, Australia, the United Kingdom, and the Republic of South Africa. The papers address the entire range of issues associated with coal pillars and have a decidedly practical flavor. Topics include numerical modeling, empirical design formulas based on case histories, field measurements, and post failure mechanics.

Late Cretaceous Blue Spur Conglomerate preserved as isolated remnants at Gabriels Gully, Weatherstones, Forsyth, Waitahuna Gully and Adams Flat was deposited during the intiation and early stages of extensional movement on the Tuapeka Fault zone at each locality. The deposits represent successive courses of a single fluvial system flowing across the Tuapeka Fault zone from the up-thrown (northeast) block. Each course, beginning with the channel at Gabriels Gully, was abandoned as a result of uplift-induced defeated antecedence in favour of a new course farther to the southeast around the southeast-propagating tip of the Tuapeka Fault zone. Schist and quartz in the conglomerate was derived ultimately from schistose rocks northeast of the Tuapeka Fault zone. Caples Terrane greywacke detritus in the middle and upper beds of the conglomerate was probably recycled from pre-existing sedimentary cover on the up-thrown block, following initiation of uplift on the Tuapeka Fault zone at each locality. Most of the gold is derived from vein sources in Otago Schist rocks to the north, and/or associated placers. Hydrothermal veins at Waipori, Hindon, Barewood, and Macraes are the most likely primary sources. There is no evidence to support previous interpretations of deposition by river systems flowing from the southwest, or axially along the Tuapeka Fault zone from northwest to southeast. High gold grades within Blue Spur Conglomerate should have a linear trend more or less similar to the orientation of the host channels, but are likely to drop off rapidly to the southwest. Extensions of the conglomerate-filled channels are unlikely to be preserved, but there is a strong probability that younger placers recycled from the former channels exist.

Recent advances in X-ray diffraction (XRD) technology have made it possible to conduct powder XRD measurements on samples at high temperatures during reaction. State-of-the-art detectors are now capable of rapidly acquiring diffraction patterns across a wide angular range (typically 120"20) enabling phase transformation rates to be measured directly during sample reaction. In this study, the phase transformations during the reduction of hematite to metallic iron by five per cent hydro ºen in nitrogen gas mixtures, were measured in situ between 500 - 700 C. Complete X-ray diffraction patterns were acquired within 15 seconds every one minute, enabling direct measurement of the transient phase abundances during the reaction. The data were quantitatively analysed by the Rietveld method to yield the abundances of hematite, magnetite, wustite and metallic iron, as functions of time during the reduction process. A first-order reactions-in-series model was proposed, and the reaction rate constants and activation energy were determined from the time-dependent phase abundance data.

The eighth annual RMCMI Mine Rescue Evaluation was held during the 1999 Institute convention at the Crested Butte Marriott Resort, Colorado. [ ] Elliott Tsosie, Mary Rowell, Janna Aune, Guy Horton, Art Saenz and Bill Balaz were pleased with the 1999 Mine Rescue Program, The Mine Rescue committee is but one of 17 volunteer committees; this one co-chaired by Bill Balaz, Elliott Tsosie and Janna Aune. There were three evaluators, Art Saenz, Western Environmental; Mary Rowell, ProCare and Guy Horton, Horton Rope Rescue. Nine teams participated and brought 89 participants, 26 spouses, nine children and five teens. The teams were BHP LaPlata, BHP Navajo, BHP San Juan, all from Farmington, NM; Bridger Coal Co., Rock Springs, WY; The Coteau Properties Co., Beulah, ND; Northwestern Resources, Jewett, TX; Powder River Coal Co., Gillette, WY; Solvay Minerals, Green River, WY and Western Energy Co., Colstrip, MT. To observe were members from Centralia Mining Co., Centralia, WA. [ ] Evaluators: Mary Rowell, Guy Horton &Art Saenz The three professional evaluators provided challenging scenarios for the Mine Rescue participants on the grounds of the resort property. Some attending convention were alarmed to see stretchers with "victims" and phoned the hotel switchboard to report the catastrophe. Team members had no prior knowledge of what the scenarios might be and derails were provided only as they arrived at the scenario location, with one exception. Seven to ten or more articipants make up each team. Some teams also ring an alternate and team trainer. For identification purposes, the "victims" wore white shirts emblazoned both front and back with a red VICTIM designation. There was a staged demonstration by three response teams who had not previously worked together. This was enacted on the convention floor following a Technical Session meeting attended by about 200 members. Three response teams from BHP's Navajo and San Juan Mines together with Coteau Properties' Freedom Mine provided a demonstration of their response skills. For never having worked together before, they presented a very professional enactment. This is described at length in the Welcome Session copy. Training sessions were held Saturday and Sunday to enable the teams to prepare for the scenarios. A swap meet was Saturday evening for teams to swap hats, shirts, belt buckles, stickers etc. Each participant received a royal blue golf shirt and ivory cap with royal blue bill. Culmination of the Mine Rescue program was a Tuesday morning Awards breakfast when teams were presented with jade acrylic plaques. [ ] Teams participating in the Mine Rescue Evaluation received trophies on Tuesday morning. The captains above represent BHP-La Plata, BHP-Navajo, BHP¬San Juan, Bridger Coal Co., The Coteau Properties Co., Powder River Coal Co., Solvay Minerals, and Western Energy Co. Disasters are often tragic outcomes of high-risk technologies such as mines. In Australia, more mines are being developed every day, and the risk of disasters is ever increasing. No matter how effective our conventional safety devices are, there is a form of accident that is inevitable. These relate to accidents that result from æinteracting failuresÆ in a way that could not be foreseen by the designers. In so-called ætightly coupled production systemsÆ (processes that happen very fast, such as on a high producing mine) the risk is even higher and our risk controls mostly introduce some sort of a technological fix. While we are at the one hand attempting to control the risk, we are also introducing another level of complexity. Are we really controlling the risk? This is one of the fundamental questions that will be addressed in this paper: Do we have the ability within the mining industry and other industries to effectively prevent these catastrophes, or are we, on the contrary, faced with an increasing risk as a result of increasing complexities of our technology, management systems and practices. This paper will contend that perceived improvement in risk control is an illusion of activity, and that the likelihood of mining catastrophes may be exponentially increasing. Although the paper focuses on the mining sector many aspects may be applicable across a range of industries. This paper will further contend that we are applying the right solutions to the wrong problems. Our focus is technological and procedural, while our problem is one of production cultures ripe for error and failure.

All mining and quarrying operations that employ blasting to fragment and move broken ground have a price to pay for æby-productsÆ such as vibration and noise. These æby-productsÆ cannot be eliminated but they can be controlled and modern blasting techniques have improved to the point that excessive environmental restrictions can be adhered to whilst maintaining acceptable production rates and quality. Accurate measurement is the key to control, and as the environmental limits are becoming tighter, the proper way to measure vibrations from blasting operations becomes an integral part of the mining or quarrying operations. The basic vibration monitoring system consists of a primary sensor connected to a data logger and computer software for the analysis of the signal received and recorded. Currently, as with all fields of measurement, much improvement has occurred over the past decade, and electronic devices are much more accurate and reliable. Data analysis has also improved considerably with the computing power that is available to field operatives today. Mounting the primary sensor has been often overlooked in supposed æstandardsÆ with procedures such as double sided tape, resting on a flat surface provided the vibration level is less than a given amount. But if it is worth going to the expense of the monitoring equipment and manpower, surely it is worth a little extra effort to ensure the primary sensor is coupled to the ground effectively. The primary sensor must move with the ground and measure this movement; so if the primary sensor is allowed to move without respect to the ground (ie poor coupling to the ground) then a false record of the physical event will be measured. This scenario can easily be a practical reality if the primary sensor coupling method to the ground is not effective. In this study a number of mounting methods were fabricated and connected to similar data monitors and a series of ænormalÆ production blasts were used as the vibration source. These mounting devices were measured against an in-house procedure (the æstandardÆ) and the relative merits compared. A laboratory investigation was also carried out to look at the physical parameters of the æsoil and their effect on vibration attenuation of the in-house standard method.

For Fe-Mn alloy production, two reactions are responsible for this process: the MnO reduction in the slag and the carbon dissolution in the bath. The smelting-reduction process has been studied as alternative route to be used to produce alloy so as steel. This technical paper presents some initial studies to be used to determine the main ways that one can have for the mechanisms for this reaction. The limited factors like mass transport or chemical reaction were analysed. The Mn content in the bath were varied and it was concluded that by increasing the former, dissolution of Mn into the bath was increased, so as a reduction of MnO in the slag. For carbon dissolution, it was used a experiment with a stationary bar, this one was added into the bath, after ten minutes probes were catched, by temperature 1600¦C, for the dissolution of MnO, one graphite crucible was employed, after 45 min the slag and alloy were analysed. After the experimental part, some curves were plotted against time, based on these graphics, and kinetic analysis was made.

The Kennecott Ridgeway Mine in South Carolina is scheduled to complete mining and ore processing in the last quarter of 1999. Tailings are disposed in a 300 acre, lined tailings impoundment. To facilitate closure and reclamation, modification of the tailings impoundment operation has been adopted. Mining involved the sequenced development of two open pits. In 1996 mining of the South Pit was completed and backfilling of the pit with waste rock from the North Pit was initiated and will continue until mining ceases. Both pits will be reclaimed as fresh water lakes. This paper presents the physical, geochemical and hydrogeochemical evaluation of the operational and post operational management of the project. Comparison of the operational monitoring data against predicted performance is reviewed.

The seismic vulnerability of the existing outlet tower at Lake Mathews reservoir, located in Riverside County, California, necessitated the design of a new outlet facility to assure serviceability of the facility following a major earthquake. The components of the proposed facility include: a new outlet tower; a shaft; a new outlet tunnel; an approach channel that will be excavated underwater; and two short tunnels that will connect the new outlet tunnel to the existing outlet tunnel via the shaft. As the new facilities will be constructed while the reservoir is in operation, a cofferdam will be constructed around the site of the new outlet tower. The design for the underground facilities used a combination of empirical methods; limit equilibrium analyses, including displacement-based pseudo-static analyses for dynamic loading conditions; and numerical analyses. This paper outlines the design methodology used to develop an innovative design for the facilities which can be constructed within the time frame required by the operational schedule of the reservoir and existing outlet facility.

This paper presents an evaluation of tailings dam incidents from the 1994 USCOLD Tailings Dam Committee report. Comparisons are also made with water storage dam incidents from USCOLD reports and recent tailings dam incidents. The evaluation results show that tailings dam incidents are due to a number of factors including overtopping, static and seismic instability, seepage and internal erosion, and foundation conditions, and that no particular type of tailings dam or operational condition is more susceptible to incidents. In addition, the results confirm that tailings dams should be located, designed, operated, and reclaimed with a thorough knowledge of site conditions, mill operation, site monitoring, and tailings characteristics.

In March, the Prospectors and Developers Association of Canada (PDAC) held its annual conference in Toronto, Ontario. This is the third PDAC meeting the author has attended. The following are some impressions of the meeting. And the author looks forward to the joint meeting of the PDAC and the Canadian Institute of Mining, Metallurgy and Petroleum to be held March 5-10, 2000, entitled Mining Millennium 2000. One of the features of next year’s gathering is a proposed forum of world mining ministers, the first ever to be held anywhere.

Subway tunnels proposed for the Los Angeles County Metropolitan Transportation Authority (LAMTA) East Side Extension are to be excavated below the groundwater through an area of Los Angeles, California with dissolved hydrogen sulfide concentrations in excess of 200 mg/L. To address unique construction and worker safety issues, bench scale and field pilot test studies were conducted to evaluate various mitigation alternatives including in-situ oxidation by chemical injection, slurry pH adjustment to suppress H2S evolution, and chemical treatment to remove the H2S from the slurry and muck material. The results provided the basis for detailed hydrogen sulfide control specifications for the contractor.

Basic solvent extraction theory indicates that pH initiates the basis of an oxime's metal complex formation and therefore its selectivity toward certain metal species. The recommended optimum pH for some of the commercial copper reagents is in a range that should maximise copper loading of the oxime. However, this pH range also tends to maximise iron(III) loading under certain operational conditions resulting in low selectivity values. Recent operational parameters at Girilambone Copper Company have highlighted some of these conditions that have a major influence on the oxime's selectivity for copper over iron(III) in the leach solutions. The result is that the pH for maximum copper extraction is no longer within the optimum range to maximise oxime selectivity. Therefore, specific copper and iron(III) pH isotherms have been determined on-site for the changing leach solutions that are fed to the solvent extraction plant in order to optimise oxime selectivity. The influences of solvent extraction plant configurations are also discussed as a controlling parameter.

"Sylvinite ore from the Saskatoon area typically contains about 5% insolubles, occurring as clays and carbonates. These insoluble slimes must be removed sufficiently well to permit an efficient subsequent potassium chloride flotation, due to their high adsorptive capacity for amines used as potash collectors. The insolubles are liberated from sylvinite by high attrition scrubbing in a slurry. The liberated slimes are separated by multistage cycloning or by a selective flocculation-flotation process.An outline of the potash processing operations at Agrium Vanscoy Potash Operations is given, emphasizing the insoluble slimes froth flotation separation process. This process was originally implemented at the Agrium plant in the mid- 1970's and replaced a hydrocyclone system.IntroductionAgrium Vanscoy Potash Operations is located 40 Km southwest of Saskatoon, almost midway between the communities of Vanscoy and Delisle. Processing operations started in 1969, and the annual production capability was subsequently increased to the current 1.8 MM tonnes per year of muriate of potash.The mining operation at Agrium Potash which was described in a paper by Jones and Pruggerl , is done underground using four large continuous double rotor boring machines for the main production with development done by heliminers. Annual ore production is 5.5 MM tonnedannum. Ore is hoisted 1,100 m to surface for processing."

The Hurtières Tunnel was built in 1995 with drill and blast method, in metamorphic schist. It is located between Lyon and Torino on the A 43 motorway in the French Alps. During the construction, a geological hazard was encountered, which caused a massive ground collapse. The tunnel was filled up with 3,000 m3of glacial materials. Geological investigations realized after the accident showed that a narrow glacial valley was present at this location the water table was 40 m above the tunnel level. The paper will describe the collapse and the remedial methods which were implemented to proceed through the failure area. These included drainage works,2,000 m3 grouting and 6,200 ml jet grouting columns.
