INJURY experience and employment data are presented in this chapter for metal and nonmetal mines, stone quarries, sand and gravel pits, iron blast-furnace slag plants, and metallurgical plants (including ore-dressing and nonferrous reduction plants and refineries) in the United States. Employment and injury experience data for all mineral industries are given in volume II. The 1962 figures are preliminary summations which are subject to revision. They were compiled from all reports from producers received by the Bureau of Mines through April 15, 1963. The data for metal, nonmetal (except well and brine operations), stone, and sand and gravel operations were collected by a mandatory reporting system under the provisions of the act of September 26, 1961, Public Law 87-300 (75 Stat. 649). The act authorized the Secretary of the Interior to study the various aspects of safety in metal and nonmetal mines (excluding coal mines) and report the findings 2 years sub-sequent to the enactment of the law. Data on nonmetal well and brine plants and nonferrous reduction plants and refineries were reported voluntarily by the operators. METAL MINES Preliminary data for the metal mining industry comprise reports on 1,871 mines and pits. The combined fatal and nonfatal injury-frequency rate was 31.95 per million manhours in 1962, a decrease of 8 percent from the similar rate of 34.54 in 1961. Metal operations reported a total of 61 fatal and 3,201 nonfatal injuries.

"INTRODUCTION The U. S. Bureau of Mines in 1930 and 1931 made a study of the properties and characteristics of liquid-oxygen explosives (L.O.X.) at the request of one of the largest users, in order to determine and eliminate the hazards attending its use.This report deals with the examination of absorbents for L.O.X. to (1) determine their most desirable and most undesirable characteristics, (2) to find ways and means of desensitizing absorbents to mechanical shock, and (3) to determine the effect that such desensitization of absorbents has on the explosive Characteristics of L.O.X.ABSORBENT MATERIALS The absorbent used in driving the Simplon Tunnel, the earliest project in which L.O.X. was used, was composed of crude petroleum, soot, and kieselguhr. During the World War, powdered cork, soot, sawdust, powdered straw, or other similar powdered materials were mentioned in the German military instructions for use of ""liquid air"" explosives. At that time the common types of absorbent, materials were compounded ones containing the above ingredients as well as kieselguhr, paraffin, gas oils, charcoal, bituminous coal, etc.Kuryla and Clevenger4 investigated absorbents of the above type and made the following statement concerning them:"

"INTRODUCTION The Warrior View mine of the Seaboard Coal Corporation is located in Tuscaloosa County on the Warrior River about 14 miles northeast of Tuscaloosa, Ala. The Brookwood bed at this point yields a coal with fair coking qualities, and the product from neighboring mines on this bed has been used for the manufacture of blast furnace coke. The output of the Warrior View mine, however, is used exclusively for domestic and steam-raising purposes.This paper continues the series° of reports on the washability of Alabama coals which are based upon the study of coal preparation in Alabama now being conducted by the U. S. Bureau of Mines and the University of Alabama with the cooperation of the different coal mining companies. This paper contains screen-sizing and float-and-sink tests of a representative sample of run-of-mine coal, and flakiness and crushing tests on certain sizes.Published information on the Brookwood bed is very meager. Fieldner6 gives the proximate and ultimate analyses for a number of seam samples taken from representative mines working this bed. Gandrud and Britton7 show sections of this bed taken at Searles, Ala. They also give the specific gravities of the coal, the bone, and the shale partings.Table 1 give the proximate and ultimate analyses for the 3/4 to 3/16 inch size of raw coal. Table 2 gives the sulphur content and the ash fusibility temperatures of the float-and-sink fractions for this same size. The character of the coal is very uniform throughout the range of sizes produced and, as it was not convenient to have these analyses on all sizes, this size was chosen as being representative of the coal. From the information available it is evident that the coal preparation problem is restricted to one of ash reduction.SAMPLING AT THE MINEAt the Warrior View mine the run-of-mine coal is dumped from the mine cars upon a 3-inch grizzly. The oversize is crushed in rolls set at 24 inches and joins the undersize before being elevated to the raw-coal bin ahead of the jig. The sample was caught at the point where elevates discharges into the raw-coal bin, by taking a proportional amount of the coal at 10-minute intervals over the period of a day's run, The total sample amounting to 2.7 tons was shipped to the laboratory at Tuscaloosa for the washability studies."

"Ventilation is one of the meet important factors in mining, although until recent years less thought has probably been given to effective ventilation than to any other one phase of mining. In the past, very little, if any, attention was given air courses until they became so constricted by falls that the volume of air was greatly reduced; and, in addition, the stoppings and doors were so poorly constructed that only a very small percentage of air reached the face workings.As a matter of fact, mines have been visited where the volume of air being circulated in certain sections was much larger than in others, and where the management was striving to increase the flow of air in those sections with the least air current, believing the ventilation to be deficient. After sampling and analyzing the return air from the various sections in such mines it has been found that the quality of the air in certain places with the largest flow was more impure than in some of those with the least flow.In past years, practically all mine fans were operated by steam-driven engines supplied with steam from the mire boiler plant. No check was made on the steam consumption or the power cost of operating the fan, therefore the management had very little if any definite information as to cost of ventilating the mine.With the increased use of electrically-driven equipment in mines electricity is being used more and more to drive mine ventilating fans, and since in many instances the current is purchased from central power plants operators have come to realize that in many cases the cost of operating the fans is very high. Some engineers estimate that it ranges from 20 to 50 per cent of the total power cost of the mine. This information, together with the fact that mines are now being developed on a larger scale, requiring larger quantities of air for the men to dilute the gases liberated, has caused the large progressive companies to make, a more comprehensive study of mine ventilation, resulting in a decided improvement in the quality and quantity of air at the face workings, as well as reducing materially the ventilating costs."

Mine seals can he remotely constructed in underground coal mines through vertical boreholes when direct access to a fire is impossible or considered to he too dangerous. This method has great merit because the boreholes can be drilled to specific mine areas, and the seals can be positioned close to a fire zone. The technology however can be largely ineffective if the constructed seals do not provide of effective barriers to airflow or if they cannot he used to impound water and other inert materials. Unfortunately, no viable alternatives exists to sealing the entire mine to the ground surface. Full-scale remote mine seal construction research is being conducted at the National Institute for Occupational Safety and Health (NIOSH) Lake Lynn Laboratory (LLL). Under this effort. Howard Concrete Pumping Company and GAI Consultants Inc. have joined forces to evaluate a potentially significant improvement to the current state of the art. This paper covers the development o f novel technology or rewrote mine seal construction, the evaluation of rite materials used, the construction practice and the Fellow-up testing.

The behavioral accident simulator (BAS) is a computer program that can be used to simulate accident situations. It demonstrates how the potential to have an accident grows or diminishes as a result of changes in various environmental, organizational, and behavioral factors. It calculates the number of fatalities that may result from various conditions and graphically displays these data. The BAS is useful for studying systems of accident causes, for demonstrating the various ways to diminish fatalities, and for training employees. This Bureau of Mines report provides the detailed instructions needed to use the BAS.

"INTRODUCTION Zinc must be of high grade to satisfy the present demand. Even when the amount of premium paid is small, the purer metal as the buyers' preference. The following specifications for spelter were taken from the American Society for Testing Materials for 1927, Part I, pane 524. (The specifications for 1930, p. 582, are tile same.)Another specification for zinc of a much higher grade than any of the above has been established lately by the same society. The committee on zinc die-casting (Proc. Am. Soc. Test. Mat., vol. 31, 1931, p. 263) reached the following, conclusions: I. ""That the purest of zinc is necessary to produce stable zinc-base die castings, and as a result requested specifications for zinc spelter .with total impurities not over 0.02 percent."" Their tests showed that the die castings made of the purest zinc had the least elongation and the greatest tensile strength, and exhibited the least changes with temperature.Still more far-reaching claims are made for the properties of zinc containing over 99.99 percent metal. Zinc foil, for instance, may someday be made as a common commercial product. It would seem likely that the demand for zinc of high grades rill continue and that even purer varieties may be desired. This demand fits in with the modern trend in manufacturing plants of reducing the number of variables to a minimum by using pure materials, and of controlling the processing of metals during their various stages of fabrication by mechanical means instead of by expert laborers."

"INTRODUCTION The Connellsville mine of the Yolande Coal & Coke Co. is in the southwestern part of Jefferson County, Ala., about 30 miles southwest of Birmingham. The mine is on the Blue Creek bed, which is about 6 feet thick and dips at a angle of 18º. The bed is worked by the room-and-pillar method and is mined by both machine and hand methods. The Blue Creek bed at this point yields a high ash, low-volatile coal with excellent coking properties. Because of its exceptionally low volatile content and high coking strength, this coal is excellent for blending with Alabama coals of higher volatile content and weaker coking properties to form blends suitable for production of metallurgical coke.A sample of washed coal taken during this investigation gave the following analysis on an as received"" basis: Moisture, 8.3 per cent; volatile matter-, 20.2 per cent; ash, 8.0 per cent; fixed carbon, 63.5 per cent. The high ash content of this coal limits the quantity which can be utilized when low-ash coke is required. In the Birmingham district it is usually difficult to sell as coking coal a product that contains more than 8.0 per cent ash. To produce a. coal with this ash content at Connellsville presents a somewhat difficult washing problem. This paper gives the results of an investigation that was conducted by the Southern Experiment Station of the U. S. Bureau of Mines in cooperation with the University of Alabama and the Yolane Coal & Coke Co. for the purpose of determining the washing characteristics of the coal and the possibility of reducing its ash content on coal-washing tables.The practice at the Connellsville washery at the time of beginning this investigation was to crush the run-of-mine coal to pass a 1 1/2-inch round-hole screen and to treat the satire output on two primary, 3-compartment jigs. A. middling product was recovered from the primary jigs which was re-treated on a single-compartment rougher jig. This system yields a product of acceptable ash content, but the yield is low and the loss of good coal in the refuse is high."

"INTRODUCTION The first paper of this series contained data concerning the behavior of polyhalite (K2S04.MgS04.2CaS04.2H20) upon calcination and subsequent leaching with water at 100° C. It was found that upon heating polyhalite to temperatures between 298 and 500° C. the water of crystallization is driven off, leaving, an amorphous mixture of K2S04, MgS04, and CaS04. Two transition points were located; one at 298° C. which coincides with the vigorous evolution of steam; and one at 551° C. which results in the formation of a hard mass of crystals. The latter probably consist of a solid solution If the compounds langbeinite (K2504, 2MgS04), potassium dicalcium sulphate (K2304. 2CaS04), and anhydrite (CaS04). The heat of decomposition of polyhalite at 298° C. was measured and found to be 66,000 ± 5,000 calories per mol. The optimum temperature of calcination with regard to subsequent extrac¬tion with two parts of water (at "" 0° C.) is 430 to 450° C. for 65 to 100 mesh material, if one half hour at the calcination temperature is allowed. The maximum concentration of K2S04 obtainable in the extraction of calcined polyhalite is 11.4 grams per 100 grams of water. This concentration of K2304 is considerably above the equilibrium value (3.6 g. per 100 L. of H20 at 83° C.) with respect to the solid phases polyhalite, potassium calcium pentasulphate, ¬and anhydrite. The figure 11.4 K. of K2804 per 100 g. of H20 is also somewhat than the equilibrium value (9,65 g. per 100 g, H20) with respect to the solid phases potassium calcium pentasulphate and syngenite."

"Refiners are generally considered solely as fuel producers, but actually they also comprise a group one of the largest consumers of fuel. For example, during the year 1929 the refineries-burned 51,544,000 barrels of oil as fuel, or the equivalent of over 5 per cent of the crude oil processed in that year. In addition enough other fuels - coal, natural gas, refinery or still gas, and petroleum coke - were utilized to raise the total fuel consumption based on B. t. u. equivalent to more than 10 per cent of the heat value in the total crude oil run to stills. Refineries utilize fuel chiefly as the heat-forming element distillation, but also under-boilers in the formation of steam used in refining, for the heating of buildings, and in the generation of electricity.In the early days of refining, the problem of fuel utilization and fuel economy was of scant importance to the refiners. Margins of profit were comparatively high and the major portion of the operatros1 attention was directed toward increasing their throughput. However, the keen competition that has prevailed in the refining industry over the past five years or more has taught the refiners that their expenses, one of the largest items of which is the cost of fuel, must be curtailed. Thus in 1925 the refiners used an average of 829,000 B. t. u. to refine a barrel of crude oil, wheras by 1929 this average had been reduced to 639,000 B. t. u., a decline of 23 per cent (see fig. 1)Since 1925 oil has ranked first as a refinery fuel (see fig. 2). The use of natural gas increased during 1926 and 1927 but declined in 1928 and 1929. On the other hand, the use of refinery gas has grown rapidly in importance. Coal has shown a steady decline in use at refineries since 1925. Petroleum coke has never supplied more than a very small fraction of the total fuel requirements, although there has been a steady increase in its use."

"The pure mineral chromite, FeO.Cr203, consists of ferrous and chromic oxides, chemically united in equal molecular proportions. Unfortunately, pure chromite as defined above appears to be a rare exception. In most chrome ores the mineral may be considered a combination of spinel, MgO.Al203, and chromite, with the latter predominating. The ratios between the four common constituents Fe0, Mg0, Cr203 and A203 vary greatly, and other minor constituents such as TiO2 and. Fe203 may also be present.In the mechanical concentration of chromite ores the maximum grade of concentrates that can be produced is thus limited by the composition of the chrome minerals present. Pure chromite contains 68 per cent Cr203, but the Cr203 content of the chrome minerals in commercial ores varies between 45 and 60 per cent and the average is not far from 55 per cent.Chromite is usually associated with serpentine. Other silicates, quartz, dolomite, magnetite, ilmenite and other iron Minerals are frequently present.The amenability of a chrome ore to concentration, depends upon the kind and amount of the various gangue minerals present and the amount .of crushing necessary to separate the chromite from them.A relatively small amount of chrome ore is concentrated by milling. In most cases the mine product is simply hand-sorted. Such chrome deposits as are known in the United. States can not be worked in this way on account of one or both of the following conditions: High cost of labor and finely disseminated impurities. There are, however, several deposits in the United States, Canada, and Alaska which may be able to compete with imported or by efficiently utilizing modern ore-dressing and milling methods."

"The problem of recovering potash from the mineral polyhalite (K2SO4.MgSO4.2CaSO4.2H2O), occurring in large quantities in the salt beds of Texas and New Mexico, has been under investigation at the Nonmetallic Minerals Experiment Station of the U. S. Bureau of Mines continuously since 1928. Most of the results previously obtained have been published in technical journals and in a series of Bureau of Mines Reports of Investigations.The experiments described in the present report were performed to ascertain whether a continuous countercurrent extraction of calcined polyhalite by hot water would yield satisfactory recoveries and concentrations of sium sulphate in the top liquor. Previous investigation by Storch 4 had shown that the rate of solution of uncalcined polyhalite was too slow to be of value in an industrial process, and Storch and Clarke 5 had demonstrated that calcination of the polyhalite greatly, accelerated the rate of solution in hot water, allowing most of the potassium sulphate to be readily extracted."

The National Institute for Occupational Safety and Health (NIOSH) is conducting a program of research to evaluate and improve remote mine seal construction technology for mine fire control and suppression. The main focus of this work is to develop reliable technology that will remotely seal a mine opening from floor-to-roof and rib-to-rib. NIOSH, in partnership with Strata Mine Services Inc.1, is evaluating the capability of a two-component phenolic foam-based product, called Rocsil®1, that is intended for cavity filling, air and gas sealing, and consolidation of highly fractured strata. Rocsil® can be rapidly applied, has a quick cure time, a high expansion rate, good compressive resistance and excellent anti-static properties. When the resin and catalyst are mixed, an immediate foaming reaction occurs followed by rapid expansion up to 30 times the original volume. By virtue of its fire-resistance properties, Rocsil® foam has application for sealing underground mine areas undergoing heating from spontaneous combustion and it could be useful for remote mine seal construction to aid in mine fire control and suppression. This paper presents an overview of remote mine sealing technology, introduces the use of Rocsil® foam for remote mine seal installations, offers a case study of the use of this rigid foam material to remotely construct a temporary in-mine seal and discusses technology improvements. Although known since 1902 , the Capitan iron deposits have not been mined mainly because of the low grade of the ore . A small amount of exploratory work carried on at an early date was reported . C. E. Johnson , district engineer of the Bureau of lines for New, Mexico , visited this propert , and as a result of preliminary sampling and mapping by the Bureau3/ and the Federal Géological Survey , an exploratory project was conducted during March , April , and liay 1944. Further mapping and logging of the vagon -drill samples was done by V. C. Kelly , geologist , of the Survey .

"Observation of the use of trailing cables in ""mines shows that the chief limiting factor in the life of the cables and their safe use is the smashing or crushing of the cables in service, together with the effects of consequent splicing.Trailing cables are crushed when run over by cars, mining machines; and locomotives or when hit by falls of roof. Crushing damages the insulation and may cause failure at once or in later use. Failure may be either by short circuit between the conductors or by grounding to the track. The failure of a live cable causes electric arcs capable of igniting explosive mixtures of methane and air. Every cable failure in a gassy or dusty mine is, therefore, a real or potential hazard.Some mine operators do not allow the splicing of trailing cables for use in gassy mines, bet require that a damaged cable be immediately replaced and if repaired that it be used only in nongassy mines. Other operators allow the making of temporary splices to permit completion of the shift but require that such splices be remade and vulcanized before the cable is used again.Still other operators allow the continued use of temporary splices, which in general means that a cable is used until the splices give trouble in reeling, or overheat, or pull apart. Then the spliced sections are either cut out or the whole cable is replaced.Splices often determine the useful life of a cable. A splice poorly made may pull apart ueder mechanical strain and cause arcing that burn* the surrounding cable or ignites as or coal-dust. If it has lower conductivity a splice is the first part of the cable to heat; it may become so hot under load that it will burn apart and damage other parts of the cable, or, if it be a soldered splice, the solder may melt and allow parting of the cable. A poorly made splice often has insufficient insulation which may alloy short-circuiting between conductors or permit external contacts capable of giving an electric shock to one handling the cable. The splice is sometimes bulky from too much insulation and takes up too much room on the reel, causing uneven reeling; also by catching on the spooling device the oversized splice may receive expressive pulls and wear. Splicing is therefore a very important factor in the use of trailing cables."

Objective The main purpose of this video is to teach mine rescue personnel certain aspects of rescue work that are not traditionally taught. These include the strain caused by working continually under adverse conditions; the "survivor guilt" that occurs from being spared the fate of one's deceased cohorts; and the posttraumatic stress that may be experienced during the sometimes lengthy task of recovering the victims' bodies. Background On November 20, 1968, an explosion rocked the Farmington No. 9 Mine in northern West Virginia (figure 1). The explosion killed 78 of 99 miners who were working in the mine, including all but one member of the mine's rescue team. Some 30 years after the event, researchers from the NIOSH Pittsburgh Research Laboratory (PRL) conducted a video interview with Danny Kuhn (figure 2), the only Farmington No. 9 mine rescue team member who was not working that day and who did not die in the explosion. According to the Mine Safety and Health Administration (MSHA), there are about 80 underground coal mine rescue teams in the United States, and according to the National Mining Association, there are more than 850 underground coal mines. This represents a significant decrease in both over the past decade. The low number of rescue teams is a concern because serious incidents, though fewer, still happen, and if a disaster occurs, there will be fewer rescue teams to respond. This would result in an increased need to use available human resources very efficiently. Much training to prepare these rescue teams still takes the form of what educators call "teaching to the test." In other words, teams practice in order to win mine rescue contests. This test-oriented approach has traditionally been augmented by interactions with others who have responded to real situations or by participating in actual incidents. Given few incidents and fewer rescue teams from whom to derive enrichment, the preparation of coal mine rescue personnel becomes a problem. Recently, more emphasis has been placed on active learning, such as working out problems in smoke. These sessions more nearly simulate the actual conditions that today's team members might rarely encounter. To properly prepare and retain the current coal mine rescue personnel who are left in the workforce, it is necessary not only to offer such hands-on experience, but also to replace the learning narratives that once were passed from team to team. The present video is intended to contribute in that capacity. Approach NIOSH researchers first learned of Mr. Kuhn in the fall of1998. Mr. Kuhn was contacted and agreed to an interview. An oral history interview was conducted with Mr. Kuhn in early November1998. After reviewing the videotape of the interview, researchers determined the content to be so rich as to warrant a second interview in a more formal setting. The second interview was done in early December 1998. During this interview, Mr. Kuhn spent about 2 hours describing the Farmington disaster, including the workplace climate leading up to the explosion, the events that took place in the 10 days after the explosion, and the nearly 10-year effort to safely and successfully recover most of the victims' bodies. The videotape of the second oral history interview with Danny Kuhn was reviewed and edited by NIOSH researchers for content. Careful attention was used to ensure that valuable information was presented, but within a reasonable timeframe. A detailed map of the Farmington No. 9 Mine was edited into the final video and used to highlight critical features as they were being described by Mr. Kuhn. Results and Accomplishments A 60-minute videotape and instructor's guide for use in safety training or other settings was completed. The target audience consists of all those who work in underground coal mines. This training video will help safety instructors better prepare mine rescue team members for the situations that they will encounter when engaging in extended rescue and recovery work. To determine the critical air velocity for preventing smoke rollback, diesel-fuel fire experiments were conducted in the National Institute for Occupational Safety and Health’s (NIOSH) Pittsburgh Research Laboratory’s Safety Research Coal Mine. Such information is necessary for preplanning and implementation of ventilation changes during mine fire fighting and rescue operations. The fire intensity varied from 50 to 300 kW depending on the fuel tray area. Airflow in the 2-m- (6.6-ft-) high, 2.9-m- (9.5-ft-) wide coal mine entry was regulated during the course of each experiment. The airflow was measured upwind from the fire as an average over the entry cross-section with an ultrasonic airflow sensor and was recorded dynamically with a mine monitoring system. The extent of smoke reversal was monitored with light-obscuration monitors, ionization smoke sensors and visual observations. Experimental results for the critical air velocity for smoke reversal as a function of fire intensity compared very well with model predictions based upon a computational fluid dynamics fire dynamics simulator.

"INTRODUCTION The character of mine drainage from areas of low pyritic coal3 and from average pyritic coal4 has been investigated, and to complete the study it seemed desirable to select a number of mines situated in high pyritic are as to determine whether or not this variation determines the relative acidity of drainage from mines operating in these characteristic coal fields. Advantage has been taken of some worked on in 1918 by Henry Leighton 5, then Professor of Economic Geology, University of Pittsburgh. He visited and has reported certain areas of high pyritic coal which we also visited during this study. Many of the mines were found to be idle in 1930 and a few were caved shut, apparently for several years. However, samples of the outflow from all mines visited were collected if possible and analyzed, and some general information was gathered for each mine. The principal objective is to provide theoretical insight to the operation of electromagnetic sensing of the internal properties of wire ropes that are used extensively in mine hoists. An overall review of past work is also included.

"DEFINITION OF VISCOSITYViscosity has been defined as the resistance experienced by one portion of a liquid moving over another portion, such as one molecular plane moving over an adjacent plane. A measure of viscosity is the force required to keep one plane in motion at Unit velocity, the other plane being considered stationary. Figure 1 illustrates the general idea that the surface friction between two adjacent areas will be greater the larger the molecules. Therefore it may be assumed that viscosity will be increased by molecular association or agglomeration, and likewise dissociation rill decrease viscosity. According to Maxwell (1)6 the unit of viscosity, the poise, is the tangential force per square centimeter of area of either of two horizontal planes 1 centimeter apart, one of which is fixed while the other moves with unit velocity, the space between being filled with viscous liquid of unit viscosity.The need for a satisfactory apparatus and experimental procedure for the determination of viscosity of high-melting materials at temperatures corresponding to those in pig-iron and steel making has been recognized during the past few decades. The condition of the slag during the smelting and refining periods in the iron- and steel-making processes exerts an important influence on the metal product obtained, this slag condition really meaning the state of viscosity or, vice versa, fluidity, which the slag attains under certain conditions."
