This paper updates an earlier report by the authors that studied electrical injuries from 1992 to 1998. The previous information is expanded and supplemented with fatal and nonfatal injury rates and trends through 2002. Injury numbers and rates were used to compare and trend electrical injury experience for various groups and categories. This information allowed identification of at-risk groups that could most benefit from effective electrical safety interventions. The data presented in this paper are derived from the U.S. Labor Department’s Bureau of Labor Statistics’ Census of Fatal Occupational Injuries (CFOI), Survey of Occupational Illnesses and Injuries (SOII) and Current Population Survey (CPS). Between 1992 and 2002, 3,378 workers died from on-the-job electrical injuries. Electricity remained the sixth leading cause of injury-related occupational death. From 1999 through 2002, 4.7% of all occupational deaths were caused by electricity, down from 5.2% in the 1992 to 1998 time period. The cause of death was listed as electrocution in 99.1% of fatal cases. Contact with overhead power lines was involved in 42% of all on-the-job electrical deaths. The construction industry accounted for 47% of all electrical deaths between 1992 and 2002, but showed overall improvement from 1995 through 2002 by reducing its electrical fatality rate from 2.2 to 1.5 per 100,000 workers. An additional 46,598 workers were nonfatally injured by electricity. Contact with electric current of machine, tool, appliance, or light fixture and contact with wiring, transformers, or other electrical components accounted for 36% and 34% of nonfatal electrical injuries, respectively. Contact with underground, buried power lines was involved with 1% of fatal injuries and 2% of nonfatal injuries. NIOSH research aimed at evaluating commercially available overhead power line proximity warning alarms is described. This research is expected to be the initial step for eventual development of a performance standard for such systems.

The paper describes solutions given to foundation problems during the reconstruction of a power plant set on fire, located adjacent to several still Working Units. After a detailed site and laboratory investigation, it was decided that it could be possible to include within the new foundations the preexisting cast in situ driven piles with enlarged base 50 cm in diameter and let them work with new 41 cm driven, cast in situ piles without enlarged base. Demolition of existing caps was carried out with expansive mortar in order to avoid any possible damage to piles, whose actual conditions were checked by several load tests. New piles were designed to support service load by shaft resistance, in order to limit total and differential settlement. When piles had to be driven adjacent to nearby structures, measurements of peak soil particles velocity were carried out while driving piles at decreasing distance from critical locations. By preboring most of the upper clay layer it was possible to reduce within safe limits for structures the induced vibrations.

Masbate Gold Mines, dormant since WWI I, are now back in operation. The ore treatment plant incorporates carbon-in-pulp processing, utilized because of the ready availability of coconut shell charcoal in the Philippines and the high clay content of the ore. A detailed description of the plant, including the surprisingly fast and efficient alcohol stripping facility is presented. Operating data for the plant is provided together with an interesting performance evaluation of mechanical versus air- lift agitated leach tanks.

An important phase in the Bureau of Mines investigation of electrical machines designed for operation in gassy coal mines is testing enclosures of electrical components for explosion-proof qualities. In brief, this testing consists of filling and surrounding the enclosure with an explosive atmosphere of methane and air and then igniting the atmosphere within the enclosure to determine (1) Whether the resulting explosion is confined without discharge of flame; (2) whether the enclosure walls are permanently distorted; and (3) whether excessive pressures are developed within the enclosure. Ordinarily, the pressures encountered in routine testing do not exceed 100 pounds per square inch gage. Hence, the designer of the machine does not have too much difficulty in taking corrective measures to prevent flame discharge and distortion of the enclosure. Occasionally, however, the explosion pressures greatly exceed 100 pounds. Then the goblet of the designer may not be solved so readily, From experience over the year 0, the testing personnel often can predict that certain construction will result in high. Pressures, but, only in a very general way, has it been possible to advise designers what modifications might be made to reduce the explosion pressures to normal values. For example, if high pressures were developed in a motor starter having a partition with an opening through it, some enlargement of the opening may appear to be the logical procedure, but it has not been- possible to give information concerning the exact amount the opening should be enlarged to prevent development of high pressures .

A thermal decomposition study of the kerogen in three Colorado oil shales was made at different temperatures by heating the shales in the absence of oxygen at atmospheric pressure and measuring the decomposition products (gas, oil, and bitumen) formed. From these data the specific reaction rates, "k", for the thermal decomposition of kerogen were determined for each temperature studied, making it possible to calculate the time required to accomplish a given amount of decomposition within the temperature range. The influence of temperature on reaction rates was presented by plotting the data according to the Arrhenius equation, thus providing a means of estimating the values of "k" for any temperature within the range of this study. It was found that the specific reaction rates for the thermal decomposition of the kerogen in the three sampler were comparatively equal at corresponding temperatures.

PT Nusa Halmahera Minerals (PT NHM) is an Indonesian joint venture company owned by Newcrest Mining Limited (75 per cent) and PT Aneka Tambang (Persero) (25 per cent). PT NHM operates two underground gold mines, Kencana and Toguraci at the Gosowong mine site in North Halmahera District, North Maluku Province, Indonesia.Due to several major stope overbreak events at Toguraci, the mining method and sequence was reviewed. The method was changed from a bottom up Avoca stoping approach using waste rock as fill material and leaving sill pillars, to a top down open stope method using paste fill without sill pillars.The revised mine plan originally had stoping progressing from the current level to the base of reserves followed by extraction of the sill pillars in the upper levels at the end of mine life. Geotechnical modelling was conducted on the revised mine plan and it was recommended that the sill pillars be taken early due to the risk of them overstressing and being difficult to recover if left until the end of mine life. The backfill material above the sill pillars is unconsolidated run-of-mine development waste rock or unconsolidated rock fill (URF).Due to earlier attempts to extract sill pillars under URF not being successful in the upper areas of the mine, it was stipulated that the URF above the five remaining pillars must be consolidated prior to stoping. Cost benefit analysis showed this to be justifiable due to the high-grade nature of the ore locked up in the pillars.Grout was initially planned to be injected in one pass through self-drilling anchors (SDA) in a 2.5 m pattern. Initial attempts to do this failed due to the SDA aperture and grout consistency. A modified method was employed using poly pipe installed into open holes that were drilled through the sill pillar and into the URF from below. Repeated application at each location ensured full grout consolidation. Drill holes were used to confirm grout thickness as well as add more grout. Design thickness was based on stress analysis.Despite different URF types being present due to varying quantities of fines, the process was similar for each area. The presence of paste in the URF of one target affected the quantity of grout it received.With a design thickness of 4–5 m of consolidation in the URF, depending on stope width and dip, each consolidated pillar (to date) maintained integrity during the stope operation. This report contains a description of the contract activities performed by ENSCO, Inc. under contract H0292025. The purpose of this project was to prove the feasibility of detecting hazards in a coal seam using a synthetic pulse radar. This work was conducted in three phases. The first was an analysis of the types of errors which might cause problems. The second was to build a breadboard radar and the third phase was to conduct a field test with the system. The work from each phase is discussed and the results of the field tests are presented. These results show propagation of the radar signal through 200 feet of coal along with reflections from 50 feet away. The final section contains recommendations for future development.

Real output expanded at a 3.0% rate worldwide in 1987, a slightly lower pace than the 3.2% rate in 1986. The decline was accounted for largely by slower growth in the U.S.S.R. and the East European countries, as well as in the developing countries (table 1). According to the International Monetary Fund, output of industrial countries output grew 3.1% and rose most strongly in Japan, the United Kingdom, and Canada (table 2). U.S. economic growth remained at 2.9%, the same as in 1986. Consumer and government spending declined in industrial countries, while gross fixed investment: rent increased. This increase in fixed investment expenditures could be attributed partly to lagged results of some 1986 developments: the fall in oil prices, declining interest rates, and exchange rate changes. The U.S. budget deficit, as well as the large external deficits and surpluses among major industrial countries, hindered further growth .2 A report from the Organization for Economic Cooperation and Development (OECD) indicates that the impact of the October stock market crash was short-lived because it hit economies that were growing strongly, anyway.3 The growth of output in the developing countries slowed in 1987; however, output declined in the fuel-exporting developing countries. These countries have been adversely affected by weakness in oil prices and the depreciation of the U.S. dollar. The weighted average of inflation rates in industrial countries--as measured by the [ ] GNP deflator, was 2.9% in 1987, down from 3.4% in 1986. Prices rose 3.0% in the United States, up from 2.6% the previous year. Inflation for 1988 is expected to remain slightly higher in the United States than in Japan and the Federal Republic of Germany because of recent exchange rates changes and the elimination of slack productive capacity and labor in the U.S. economy. The Bureau of Mines conducted a flammability study of plastic ventilation ducts. Six rigid 16-in-diam fiberglass-reinforced plastic ducts from five manufacturers were examined using laboratory-scale, large-scale, and full-scale fire tests. The laboratory-scale tests included the American Society for Testing and Materials E162 radiant panel test, the E2863 oxygen index test, and the standardized small-scale flammability test (S3FT) developed by the Mine Safety and Health Administration (MSHA), U.S. Department of Labor, and replicated by the Bureau of Mines. Ten-foot lengths of the same ducts were also subjected to a large-scale vertical fire test, and 20-to 30-ft lengths were tested in a full-scale in-mine fire test at the Bureau's Lake Lynn Laboratory mine. The data from the laboratory-scale radiant panel test and oxygen index test indicated that the ducts were fire-resistant, However, five of the six ducts failed the S3FT and vertical fire tests, and four of the ducts failed the full-scale in-mine tests with flame propagation rates that ranged from 3 to 10 ft/min. The results indicate the need for improved fire-resistant plastic ducts. The good agreement between the S3FT re-sults and the vertical and full-scale tests show that the S3FT provides a good indication of the flammability of fiberglass-reinforced plastic ducting under realistic fire conditions.

The Paracatu Expansion Project III in Brazil was implemented between 2006 and 2008 to increase the existing capacity from 20 to 61 Million tons per year of gold ore treatment. This expansion led the Brazilian subsidiary of the Canadian Kinross Gold Corporation to become the single largest gold producer in Brazil and the largest single gold concentrator in the world, with a nominal capacity to treat around 167,000 tons per day of ore. This paper describes the Project Development areas for the Paracatu Expansion, using the approach proposed by the Project Management Institute (PMI) and the methodology included in the PMBOK (Project Management Book of Knowledge). The author has more than 20 years of experience in the gold mining industry, having worked as a plant metallurgist, mill superintendent and development manager for RPM until he took the position as the Project Manager of the Paracatu Expansion III in 2005. Luis Tondo has a BSc in Mining Engineering from UFRGS-Brazil, an MScEng from the University of Queensland - Australia and an MBA degree from FDC-Brazil. Luis is currently based in Santiago-Chile, acting as the Project Director for developing projects in Latin America for the Kinross Gold Corporation.

An extensive design phase testing program in which 10 piles and 2 drilled shafts were instrumented and statically load tested to failure, was undertaken to establish relevant foundation design parameters for two bridges in Charleston, South Carolina, USA. The bearing strata at both test sites was a calcareous, overconsolidated, firm to very stiff, slightly sandy, clayey silt (locally known as Cooper Marl). Confirmation dynamic and static loading tests on production piles indicated only 50 to 60% of anticipated pile capacities; including consideration of time effects. Supplemental subsurface explorations did not reveal any significant variations in marl properties. Ultimately, 22 static and 75 dynamic tests were performed on production piles for a 2 mile section of the project. Pore water pressures in the marl were also monitored during and after pile driving. A study of unit skin friction in the marl as a function of time revealed two distinct trends: piles driven in areas with a sand overlaying the marl gained more capacity significantly faster than those driven through a clay or silt. Although the insitu marl properties are apparently similar across the site, the material immediately above the marl evidently has a large effect on pile capacity. This effect of overburden on pile capacity was verified by testing piles over a period of two years.

Este trabalho objetivou comparar, por meio de experimentos desenvolvidos em uma fundição, através da adição de três inoculantes diferentes (IM-22 com FeSi-Ba/Zr, G 20 com FeSi-Ba) e IMSR 75 com FeSi-Sr), para observar as características da vida util de ferramentas na usinabilidade das peças, microestrutura e propriedade mecânica. A metodologia da pesquisa se baseou em analisar principalmente pelo processo de fresamento o ferro fundido cinzento GG-25 inoculado diferentemente, denominado de acordo com a Norma DIN EN 1561. As formas de avaliação dos resultados terão como base uma análise quanto ao desgaste da ferramenta, acabamento superficial, análise microestrutural, composição química e propriedades mecânicas do material da peça. Os resultados mostraram que dentre os inoculantes aplicados o com Sulfeto de Estrôncio (SrS) apresentou-se mais estável termodinamicamente que as amostras inoculadas com os outros dois inoculantes, pois ele levou a uma variação da energia livre de Gibbs mais negativa e portanto mais favorável a formação de núcleos com raio crítico (rc) maiores, o que permitirá uma solidificação com nucleação heterogênea. Todavia o respectivo inoculante também foi mais eficiente na formação de uma microestrutura mais estável, com maiores quantidades de células eutéticas e vida útil do inserto mais longa quando usinado.

Forty one whole rock samples from the Gold Cross low-sulfidation epithermal Au-Ag deposit have d18O values that range from 4.4 to 9.3 per mil, with an average value of 7.0 per mil. Unaltered and weakly altered rocks have d18O values greater than eight per mil, and the orebody is surrounded by samples that are depleted in 18O. A strongly silicified sample adjacent to the Empire Vein System has a d18O value of 9.0 per mil, similar to previously reported analyses of vein quartz (7.0 to 11.7ë, average 9.4ë). This suggests that, in detail, Golden Cross may have a zone of 18O-enriched wall rocks in the core of the deposit, adjacent to the main underground veins. Although some workers have suggested that stable isotope geochemistry may provide useful information for epithermal mineral deposit exploration, at Golden Cross this is not the case. Alteration minerals, major elements and trace elements all define larger, less ambiguous halos than the zone of 18O-depleted wall rocks.

Results of the mineral resource study of the Upper Priest Lake RARE II study area indicate a low potential for the discovery and economic recovery of silver and lead in the Prichard Formation near contacts with the Kaniksu Batholith in the vicinity of the Mountain Chief Mine (fig. 1). Silver and lead minerals locally occur disseminated and in irregular quartz fissure-fillings in the Prichard Formation. Between 18^6 and 1967, 105 lode raining claims were located in the study area. The area was designated for "further planning" in the Final Environmental statement but was later changed to "nonwilderness" by President Carter after work was scheduled for the area.

"Statistics show3 that in 1927 the United States consumed 60 per cent of the world's production of chromite and produced none. The uses of chromium are rapidly expanding, and a dependable supply is essential to our industries. Since the political and commercial control of our supply of chromite is now in foreign hands it is prudent to determine how this nation can best prepare to supply its need for chromium if the imports were cut off or restricted and whether it is now possible and desirable to stimulate production of chromium from domestic deposits. We have only small amounts of chrome ore of high enough grade to meet present trade requirements. Therefore the Bureau of Mines is making a study of methods for beneficiating our low-grade ores.Chromite (Cr203.Fe0), is the only commercial source of chromium. For refractories, a minimum of 40 per cent Cr203 and a low silica content (5 to 8 per cent) is usually specified. In most instances bricks or lumps having a physical structure that will not crack when heated are required. The only way an inferior ore could be adapted to this use world be to concentrate it and develop a satisfactory binder to make bricks out of the concentrates.For making ferrochrome the requirements are a minimum of 45 per cent Cr203 and an iron content not exceeding one-third of the chromic oxide. On some low-grade ores gravity concentration will give a product meeting these requirements; other ores yield concentrates containing too much iron or other impurities to be saleable."

"This paper describes the development of a mathematical model for determining the liberation of an ore at various grinding sizes, based on the combined results of microscopic counting, grinding tests, and chemical analysis.The method is illustrated with an application to the magnetite ore of Sherman Mine in Temagomi, Ontario. For this ore, a degree of liberation of 98.7% was determined at a grinding size of 90% passing 44 microns, which closely corresponds to the final grinding size employed at the Sherman Mine Concentrator.Introduction Liberation characteristic of ores have direct and significant bearing on overall economics of concentration processes.Target grades and recoveries can be achieved only it minerals are adequately liberated prior to separation. Degree of liberation depends on the extent of size reduction. However, due to the high costs associated with size reduction processes, it is important to keep size reductions within limits of satisfactory liberation. Prevention of excessive size reduction is also necessary because of inefficient mineral separations in very fine sizes. Hence, a study of reduction size — liberation relationship is essential for optimum profit from milling operations.A number of procedures have been developed for liberation studies (1-12), but there is lack of method for universal adoption.A new approach is presented in this paper that might provide a realistic estimate of liberation and give a practical guidance as to the degree of size reduction required for satisfactory liberation."

This report describes the design and testing of an improved quiet hard rock percussion drill. The improved drill incorporates a tappetless design with independent rotation in a hand held drill weighing 50 lbm. Laboratory and field testing in three mines were conducted. The testing showed improved drilling performance with no increase in measured noise levels over the fist design.
