Why discuss techniques of accident prod lot on? Would it not be more useful to discuss means to reduce accidents? This discussion will answer these questions by suggesting ways in which effective accident prediction techniques improve the ability to foresee and prevent accidents. Accident Prediction Process The secret to accident prediction is to utilize the tendency for history to repeat itself. The same circumstances and the same human behavior that produced accidents in the past tend to produce accidents in the present and the future. The ability to predict accident occurence in the future by projecting from the past is a statistical process. In a bookmaker's jargon this means "playing the odds" or "the percentages". A horse or a sports team may be beaten on any given day, but by skillfully assessing the factors of past performance and assigning odds, many clever bettors make money in the long run.

Although manganese is a metallic element and is widely dispersed in nature, it never occurs except as a compound in combi¬nation with other elements. Use of such compounds in the production of glass is known to have occurred in early Egypt. The dioxide of manganese was considered a compound of iron until 1774 when C.W. Schule first recognized it as an element. In the same year, a Swedish mining engineer, J.G. Gahn, became the first to isolate manganese. In 1856 development of the Bessemer process of steelmaking gave economic importance to manganese, and later, in 1882 Robert Hadfield discovered the benefits of high manganese steels. GEOLOGY Physical Properties Elemental manganese is a silver-gray metal, resembling iron but harder and more brittle and used primarily in alloys (both ferrous and non-ferrous) and a wide variety of chemical compounds. Some of the physical properties of manganese include: melting point¬1 245°C; boiling point-2 150°C; density at 20°C-7.43 g/cm3; specific heat at 25.2°C-O.115 cal/g; latent heat of fusion-63.7 cal/g; hardness on Mohs scale-5.0; linear coefficient of thermal expansion from 0 to 100°C-22 x 10-6. Mineralogy There are over one hundred minerals that contain manganese. These minerals vary from those with compositions that are pre¬dominantly manganese to those having only minor percentages of manganese. Distribution of Deposits Manganese ore deposits are found worldwide and were formed in various geological environments, but only a rather limited num¬ber of deposits have high grade manganese ore in sufficient quan¬tities to be mined and utilized economically on an industrial scale. It is worth noting the fact that almost all of the significant deposits can be classified into two types of deposits: marine chemical sed¬imentary deposits, and residual (secondary) enrichment deposits. There are, however, a much larger number of geological types but these are not of commercial significance at this time. Sedimentary deposits are the most common and are usually stratiform or lenticular. Manganese minerals were formed by a chemical process during the deposition of marine sediments. They usually contain manganese oxides and carbonate minerals, some¬times interbedded together or with other sedimentary rocks such as limestone or shale. Examples of this type of deposit are the Russian ore bodies of Nikopol and Tchiatoura, as well as the Kalahari deposits in South Africa and deposits of Groote Elyandt in Aus¬tralia. Residual deposits were formed in a different way: by alteration of existing manganese deposits or by concentration of the manga¬nese minerals when other minerals were washed away by weath¬ering or ground water processes. The Nsuta deposit in Ghana, the Amapa deposit in Brazil, the Moanda deposit in Gabon, and nodules in the residual clays of the US Southern Appalachians are examples of this type of geological process. In the case of the Ghana and the Amapa deposits, this is only true for the outer layers of the deposit containing oxide minerals, the inner part being comprised of car¬bonate minerals including manganese carbonate, probably from marine origin. Some sedimentary and residual-type deposits have been metamorphosed, giving rise to small high grade ore bodies. These deposits are regionally metamorphosed, occurring in mar¬bles, slates, quartzites, schists, and gneisses. Some of these deposits, such as the Franklin, NJ, deposit are rich enough to be commercial without secondary enrichment; however, most of the exploitable deposits have been secondarily enriched. Due to the diversity and complexity of manganese deposits, both with respect to deposition and chemistry, a wide range of impurities are almost invariably present in the ores. Table 1 shows that the reserves (i.e., a measured resource that can be economically and legally extracted) are estimated at 814 x 106 tons of contained Mn, which equates to more than a 100 year supply at the current level of production. The reserve base is made up of the marginally economic reserves and sub-economic re¬sources and are some 4.5 times greater than the proven reserves. Also apparent from study of Table I is the fact that some 75% of manganese reserves are found in two countries, the USSR and South Africa. On the other hand, North America (i.e., the United States and Canada) have few significant deposits. The San Manuel Oxide Pit is located within the subsidence boundaries of Magma Copper Company's block caving operation. Ground conditions offer a constant blasting challenge and vary from hard, competent crystalline rock to widely spaced fracturing in a sedimentary conglomerate. A successful drilling and blasting program has undergone several changes since mining commenced in 1985 and continues to be developed. This paper discusses the site specific progress made with the drilling and blasting program from selection of feasibility parameters to current practice. Researchers from the National Institute for Occupational Safety and Health (NIOSH) evaluated the impact of potential shale gas well breaches and subsequent gas inflow on selected mine ventilation systems of operating longwall panels using a 1:30 scaled Longwall Instrumented Aerodynamic Model (LIAM) and Sulfur hexafluoride (SF6) as a tracer gas. A series of tests were performed at 340, 400, and 500 cfm inflow levels utilizing different mine ventilation scenarios. Results suggest that the breached gas can be diluted to meet statutory levels when the longwall panel is adequately ventilated. The results enhance the understanding of the gas inflow and mine ventilation system interaction and provide critical information to the industry and regulatory agencies for improving miners’ safety.

"INTRODUCTION The vision for a “Mine Escape Vehicle” (MEV) is to provide capabilities to better enable underground travel to allow miners to escape to safety in the event of a major mine emergency such as a fire or explosion. Enhanced transportation and survival capability such as a MEV may have minimized or eliminated the loss of life associated with several of the mine disasters that occurred in 2006, which together resulted in the deaths of 19 miners. Although the most desirable response in an emergency is for the miners exit the mine, if escape proved to be impossible, the miners could use the vehicle to proceed to an underground point of refuge to await rescue. Under a NIOSH contract issued in April 2008, Raytheon developed a conceptual vehicle design including (1) the capacity to provide life support functions for a number of miners, (2) operate in a reduced oxygen atmosphere with little or no visibility, and (3) travel at speeds faster than miners could walk out of the mine. The proposed MEV concept envisioned modification of a personnel carrier (a.k.a. a “mantrip”) that would be used every day during normal mine operations, not a specialized vehicle reserved only for or dedicated solely to escape. The MEV concept also included escape aids that are detachable from the vehicle in the event that the vehicle must be abandoned on the way out of the mine and the dismounted miners had to proceed on foot. To advance MEV development, NIOSH awarded a second contract in 2010 to Raytheon to build, test, and evaluate the performance of a proof-of-concept MEV that culminated in a 7-month performance evaluation of vehicle in an Eastern Kentucky coal mine (Figure 1). User feedback and lessons learned from the proof-of-concept vehicle evaluation provided support for the general MEV approach, guidance related to the practicality of MEV capabilities, and indication of specific technology shortcomings for this application."

Risk is a major factor in all mining activities, arising from many internal and external variables. The authors? research has resulted in a systematic method for the quantification of risk in mining projects (Park, 2012). This is the second part of a three-part series. The first part was published in the October 2013 issue of Minining Engineering magazine and can be found here at www.me.smenet.org. This paper discusses methods of assessing the overall risk associated with those variables, and presents a statistical model based on a survey of 31 experts. The third paper, to be publised at www.me.smenet.org on Oct. 14, presents an evaluation of the statistical model, using data from previous mining investment decisions in which the authors have been involved.

Today many small diameter tunnels are being constructed in Metropolitan areas and towns as transmission holes for utilities such as water, electricity, natural gas and sewage, to name a few. These tunnels function as utilities beneath the streets, highways, railways, and other structures where open-cut techniques are impractical for any one of a number of reasons. Many such bores range in diameter from 15 to 72 inches and in lengths from 100 to 500 feet. Auger drills with auger type heads have long been used to bore these "road crossings" as they are usually termed; however, many such crossings must be bored in rock with rock-boring heads. Rock with a compressive strength of 15,000 p.s.i. and higher seems to present the greatest challenge for the contractor. These rocks include most igneous and metamorphic rocks and some sedimentary rocks. In general, this grasps most granites, basalts, gneisses, slates, schists, quartzites, limestones, and cherts. In the past ten years boring machines have been developed and used to excavate road crossings through hard rock. Many crossings have been successfully excavated using these machines, and the degree of success has increased substantially in the last few years. A variety of machines have been built and manufactured to bore most rock in a range of diameters up to 72 inches. The machines vary in capabilities, size ranges, and designs but all perform the same basic task and function of boring horizontally. Incidently, here the term horizontal implies horizontal or within five degrees of horizontal. "Iron ore is the source of primary iron for the world’s iron and steel industries. Its production can be reported as crude ore, usable ore or iron content of ore. Historically, the U.S. Geological Survey (USGS) used reported crude ore production from China in tabulations of world iron ore production while other countries have typically reported their production in terms of usable iron ore. When China’s crude ore production was tabulated with usable ore production from other countries, world iron ore production totals have been overestimated by 10 percent to a high of content and may include products such as concentrates, direct-shipping ore, pellets, and sinter.• Iron content, also referred to as metal content or grade, denotes the amount of elemental iron in the crude or usable ore.BackgroundIron ores consumed in the steel industry typically require an iron content greater than 58 percent before being considered commercially usable. Iron ores that are not commercially usable must undergo beneficiation to raise the iron content. Approximately 98 percent of the world’s usable iron ore production is consumed in blast furnaces to make pig iron, which is further processed to make steel.High-grade iron ore mines can supply usable iron ore, known as direct-shipping ore, to blast furnaces, with minimal physical processing and no beneficiation. With direct-shipping ore, the reported production for crude ore and usable ore are the same. Ore from low-grade deposits must undergo beneficiation to increase the iron content before it can be used by the steel industry. In this instance, the mined material is called crude ore and post-beneficiation concentrates are called usable ore; the amount produced will be less than the crude ore amount. With the notable exceptions of countries such as Australia and Brazil, most of the world’s iron ore deposits are low-grade and require beneficiation."

In the fairly recent past, with a history of cheap and abundant energy, it has been possible to predict with reasonable confidence that world energy demand would continue to double about every 15 years. Those who attempted to forecast the role of nuclear energy in future electricity generation could also start with the fairly safe assumption that electricity demand would increase even faster : with a doubling in demand about every 10 years. To estimate the likely future world demand for uranium it was then necessary to introduce a number of other assumptions including the rate of penetration of nuclear energy, the timing and the specific fuel requirements. This series of assumptions naturally compounded the uncertainty, but, remarkably, usually led to future uranium demands being overestimated. Such a bias which has almost always been observed in predictions made over the past 20 years has increasingly made uranium producers sceptical when expected to match the forecast demand with increased production capacity and supporting reserves. This scepticism has, of course, been reinforced by the persistence so far of a buyers' market in uranium throughout the entire period of emergence of commercial nuclear electricity generation.

At 6,898 ft below surface, the historic No. 9 shaft at Resolution Copper near Superior, AZ is now the second deepest single lift mine shaft in the United States and connects to the deepest single lift mine shaft in the U.S., Resolution Copper’s No. 10 Shaft (6,989 feet). The shafts serve as ventilation and access for future underground development of what could one day become one of the most significant copper mines in the United States. It is from the base of No. 9 and No. 10 shafts that hundreds of employees of Resolution Copper, a joint venture of Rio Tinto and BHP, hope will soon be given the go ahead to begin driving lateral tunnels to reach a copper deposit that is estimated to contain 28 Mt or 40 billion pounds of copper. Copper was recently added to the U.S. Geological Survey’s (UGGS) critical minerals list, in large part because of increasing demand. A study by S&P Global Energy & Market Intelligence “Copper in the age of AI: Challenges of electrification” released on Jan. 8, 2025, “identifies a transformative trajectory for copper demand, projecting a surge from 28 Mt (31 million st) in 2025 to 42 Mt (46.3 million st) by 2040 – a 50 percent increase that underscores the metal's pivotal role in multiple technological and economic domains.” The surge in demand is driven in large part by the global energy transition, electrification of vehicles, growth of artificial intelligence and data centers, and most recently robotics. Meanwhile, geopolitical tensions have increased the focus on the need for the United States to secure more of its own mineral supply chain. S&P Global’s report is one of many warnings that while copper demand has accelerated rapidly, future supply is being held back. Resolution Copper stands out as one example of a known mineral deposit remaining locked in the United States’ complex and lengthy permitting slog.

The Schaffer linear kimberlite complex and Aultman kimberlite diatremes comprise a major part of the State Line kimberlite district of Colorado and Wyoming, which encompasses approximately 200 sq mi (500 km2). The district is located along the eastern flank of the northern Front Range and is underlain prin- cipally by Precambrian granites (approx. 1.4 b.y.) which have been intruded into an older (approx. 1.75 b.y.) metamorphic complex. The kimberlite occurrences are Devonian in age and are related to older Precambrian linear structures. The Schaffer kimberlite complex is a dike or fissure system with enlargements or blows developed at the intersections of cross faults; the two Aultman occurrences are diatremes. Kimberlite within the Schaffer complex is predominately grey to green phlogopite-calcite kimberlite breccia and tuffisitic breccia. Carbonate is abundant in some phases. Inclusions of host granite are common and locally abundant. Upper mantle peridotite, pyroxenite, and eclogite and lower crustal granulite xenoliths are present at many localities. Four phases of tuffisitic kimberlite breccia occur in the Aultman diatremes. These breccias are characterized by high carbonate and serpentine contents. Xenoliths of upper crustal granite, lower crustal granulite, and upper mantle assemblages are abundant locally. Lower Paleozoic sedimentary rock xenoliths are also abundant in the Aultman diatremes but are absent in kimberlite of the Schaffer complex. From 1979 through September 1982, Cominco American, Inc., conducted geologic mapping, petrographic studies, geophysical surveys, geochemical surveys, and auger drilling in the Schaffer-Aultman project area as part of an extensive evaluation program of kimberlite occurrences. Forty bulk samples (approx. 8,000 metric tons) were excavated from the occurrences and then were processed for diamonds at Cominco American's heavy media separation and diamond recovery test plant at Fort Collins, Colorado. Indicated grades from the Schaffer and Aultman kimberlites range from approximately 0.5-1.0 carat per 100 metric tons and as such, the occurrences do not appear to approach economic viability at this time.

INTRODUCTION At the end of 1959, high levels of radioactivity attributed to radon and its daughter products were discovered in the fluorspar mines at St. Lawrence, Newfoundland. These levels were presumed to be the cause of an unusually high incidence of lung cancer among the fluorspar miners (deVilliers & Windish, 1964) (Parsons et al. 1964). The mining of fluorspar (calcium fluoride) began in 1933 as open pit operations but converted to standard underground mining procedures in 1936. During the second world war, production was greatly expanded as a result of increased demand for fluorspar used in the production of steel. Wet drilling was first introduced into general use in 1942. Ventilation was mainly by natural draft occasionally supplemented by small blowers. The amount of ventilation varied greatly between mines as well as over time. For example, one large mine, the Iron Springs mine, had only a single small raise to the surface some 600' from the central shaft. Other mines, such as the Director mine, had a number of raises to the surface and, as a result, had far better ventilation. Mines also varied by the amount of ground-water which seeped into them. In the early 1950's, an unusually large number of lung cancer cases were diagnosed among the fluorspar miners. As a result, in 1956 and 1957, J.P. Windish of Canada's Department of Health and Welfare tested for possible causative agents in the mines. Unfortunately, radon measurements were not conducted until 1959 and 1960 when Windish tested Director mine as did the A.D. Little company in 1960. As a result of the high radon levels found, mechanical ventilation was introduced and the concentration of radon dauthers fell, on the average to well below 1 WL. During this period, lung cancer cases continued to be diagnosed with 29 lung cancer deaths recorded by 1964 and 71 by 1971. As of July 1981, 105 lung cancer cases had been identified (Hollywood, 1981). Previous reports concerning the fluorspar miners have dealt in detail with the factors in the occupational environment and discussed occupational mortality patterns. The purpose of this paper is to review further the mortality experience with particular reference to lung cancer in relation to cumulated radiation exposure and to describe briefly our ongoing study of this group. METHODS Occupational histories were prepared for men who had been employed by the mining companies at St. Lawrence during the period 1933 to 1977. The histories were compiled from company records except for the period 1933 to 1936, records for which were lost in a fire; however, the occupational histories for this period were completed by searching census records and interviewing company officials, ex-employees and others. In addition, occupational and smoking histories were also obtained for some miners during a survey conducted in 1978. Occupational records included name and date of birth as well as the type, place and hours of work by year. For each year prior to 1960, hours of work were converted to working months (1 WM = 167 hours) and were multiplied by the estimated average radon daughter concentration in working levels (WL) to yield the annual radiation exposure in working level months (WLM). Pre-1960 radiation levels were estimated on the basis of the history of mining methods employed, ventilation history of the mine, type and place of work and conditions under which the first radiation measurements were made in 1959 and 1960 (deVilliers and Windish, 1964). During the period from 1960 to 1967, the average exposure was about 0.5 WL. Beginning in 1968, radiation levels were measured more frequently, and, beginning in 1969, daily exposures for each worker were recorded based on radiation levels in the place worked on a given day. Mortality data were obtained from medical certificates of death. In a small number of cases, medically certified death certificates were unavailable. In these cases, probable cause of death were obtained from forms completed by the local clergyman (returns of death), parish records, information obtained from interviews with family members of the deceased and/or hospital information, before assigning a cause of death. Data obtained from these sources were found in Tables 1, 2 and 4, cover the time period 1933 to 1971. Data in Table 3 as well as in Figures 1 through 3 cover deaths from 1933 to 1977, and includes only those miners for whom medical certificates of death were available. Two medically certified causes of death were changed from other causes to lung cancer on the basis of pathology reports.

A bewildering number of continuous thickeners and clarifiers are used in mineral industries and other heavy industries. General types are: 1. Conventional Thickeners (Fig. 1). 2. Thickeners with Flocculating Feedwells (Fig. 2). 3. Solids Contactors (Fig. 2). 4. Tray Thickeners (Fig. 3). 5. Lamellas (Fig. 4). 6. Conventional Thickeners with Lamellas 7. High Flow, Fluid Bed Thickeners (Fig. 5). These units must be adapted to varied applications. They must provide operating flexibility to meet process needs. Such factors multiply the decisions which must be made prior to the selection of a thickener. The seven types of thickeners listed above, with their variations, share design concepts which limit their application and which can be used to evaluate their performance. Some of these are listed: 1. Feed velocities must be limited and/or controlled. 2. Natural or reagent flocculation is necessary. 3. Horizontal settling area is the basis for capacity. 4. Overflow velocities must be limited and/or controlled. 5. A controlled underflow is necessary. Applying these needs to the basic process concepts is thickener selection. The basic function of a thickener in the minerals industry is the separation of a thickened sludge containing fine solids from water. It is widely accepted that particles stabilize flotation froths and that stable froths result in improved flotation performance. Predicting the effect of particle addition on froth stability is, however, challenging. Furthermore, there is currently no technique that allows the prediction of the effect of different flotation reagents on froth stability, without carrying out froth stability experiments.

A theoretical and experimental study was made of the surface phenomena occurring in a silicate - sulfonate system and a silicate -amine system, Experiments were conducted varying the collector concentration to examine its effect both on the total surface sorption and the rate of sorption. The results were compared to a theoretical model based on a Langmuir-type isotherm, Finally, the flotation behavior of selected silicate minerals was predicted from the sorption results and an explanation for the flotation of silicates postulated on the basis of both the theoretical predictions and experimental data.

Problems encountered in barite flotation from various waste materials such as spent drilling muds are likely to be closely connected with the effect of chemical additives and metal ions present either added or present as contaminants, on the flotation. Fuerstenau and Palmer (1976) have studied in detail the adsorption of multivalent cations onto oxide and silicate minerals. It was observed that the adsorption of a multivalent metal species exhibits a sharp pH dependence which is related to the formation of hydroxy complexes of the metal ions in solution. Investigations on the recovery of barite from tailings ponds and mining wastes (Sullivan and Lamot, 1981) have shown that such barite materials can contain large quantities of carbonate and silicate minerals (including clay minerals). Also iron oxide slimes can be present. From these substances cations such as Ca2+, Mg2+, Fe2+ and A13+ can potentially dissolve and be present in the aqueous phase accompanying the various minerals. These ions and their hydrolysis products can have a substantial effect on any subsequent flotation scheme. The present study investigates the effect of the noted ions on both the stability and electrokinetic behavior of barite suspensions.

Controlling dust generation and keeping it below permissible limits to meet federal dust standards at the working face of a room-and-pillar coal mine is a challenge for a mine operator. With the recent changes in federal dust regulations requiring lower worker exposure, maintaining compliance has become increasingly difficult. The current most effective practice of dust control at a continuous miner face in an underground mine is the use of a flooded-bed scrubber. A study carried out by the U.S National Institute for Occupational Safety and Health (NIOSH) indicated that a flooded-bed scrubber could achieve cleaning efficiencies between 58 and 90 percent. But the operation of such a system is maintenance intensive. The flooded-bed scrubber screen becomes clogged with dust particles and requires frequent cleaning to maintain performance. However, the dust control issue is not solely a mining-industry problem. Other industries face similar issues. The University of Kentucky collaborated with Toyota Motor Manufacturing on the development of a novel wet scrubber, called the vortecone scrubber, for capturing over-sprayed paint particles in automotive paint booths. The vortecone scrubber achieved a cleaning efficiency of 99 percent and required minimal maintenance. This study aims to assess the ability of this vortecone scrubber to capture respirable dust in an underground coal mine. The paper presents the results of computational fluid dynamics (CFD) modeling of this vortecone scrubber. It discusses the effects of air quantity and dust particle size on the performance of the scrubber.

The Stillwater Mining Co. (SMC) is a partnership of Pittsburg & Midway Coal Mining Co. and Manville Corp. Since the summer of 1990, SMC has operated a precious metals smelter at Columbus, MT for further upgrading of concentrates from its platinum-palladium mine. The facility's gas-cleaning system utilizes "oxidation/dilute-mode," dual-alkali scrubbing technology to capture sulfur oxide gases from the process gas streams. This paper describes the rationale for selecting the dilute-mode, dual-alkali system and summarizes the design parameters. Also discussed are the startup problems encountered and their solutions.

This paper presents an overview of some of the basic issues related to tunnel construction in the presence of flammable and toxic strata gases. Gas properties and possible sources are outlined for the various gases that are commonly encountered underground. Assessment of potential gas hazards prior to construction is discussed. Measures for mitigating the problem during construction are summarized. This RETC Session on Gassy Ground has been organized by the Underground Technology Research Council (UTRC), Technical Committee on Gassy Tunnels.
