Metropolitan Washington Airport Authority is currently undertaking the first Phase of Metro?s Extension to Northern Virginia. A wide range of tunnel options was considered for the Tysons Corner segment that involves about 20,000 feet of track and four stations. After consideration of many options that involved EPBM single track tunnels, NATM tunnels and large bore tunnel concepts for the entire Tysons alignment, the design now being progressed into construction involves two 520 meter (1,700 feet) long soft ground NATM tunnels with several hundred feet of cut-and-cover construction. The paper discusses the process that led to selection of the short NATM tunnels as the most feasible of the options considered for Tysons.

The purpose of this project is to convey sanitary sewage from an existing pump station to a new waste water treatment plant. This paper will discuss the design and construction aspects of the project. The scope includes construction of three main shafts ranging from 34 to 100 feet in diameter at depths of 200 to 230 feet and 21,000feet of tunnel. Shaft construction consisted of slurry wall excavations keyed into claystone and limestone at depths ranging from 114 to 130 feet, followed by rock excavation utilizing drill and blast methods. The tunnel is being excavated in the Warsaw and Salem shale and limestone formations utilizing a 12'-6" diameter Wirth hard rock TBM.

This paper reviews the recovery of potassium from different rocks and mineral sources. Global demand for potassium is rising steadily due to the growth of agricultural production. Fertilizer manufacture consumes the majority of the world’s potassium production, with other uses including those in the pharmaceutical, glass, ceramic, food and detergent industries. The availability of soluble potash (K2O) minerals, such as sylvite, kainite and carnalite, in countries like Canada, the United States, Israel and Russia makes them major potash producers, whereas agriculture-based countries like Liberia, Somalia, Central African Republic, Thailand, Indonesia, Malaysia and India meet their potassium requirements through imports only. In these countries, the huge availability of potassium-bearing rocks/minerals like nepheline syenite, feldspar, mica and glauconitic sandstone, containing around 4 to 17 percent K2O, brings up the prospect of commercial production of potassium. Potassium recovery from minerals/rocks is complicated due to the uniform distribution of potassium throughout the crystal structure. Different physico-chemical separation methods like bioleaching, chemical leaching, flotation and roast-leaching have been discussed for the successful recovery of potash values from these rocks/minerals. Although seawater is vastly available, the recovery of potassium from seawater using chemical precipitation, solvent extraction, membrane separation and ion-exchange methods is not cost-effective due to low concentration of potassium.

Hematite floatability from two low grade banded iron ores of India, namely banded hematite quartzite (BHQ) and banded hematite jasper (BHJ) using fatty acids and amine as the collectors is investigated under identical physico-chemical conditions. The results indicated that hematite present in BHQ ore is easy to float using both the collectors, compared to BHJ ore. The difference of flotability between the two banded iron ores is that quartz grains are coarser and well liberated below 100 micron size in case of BHQ compared to BHJ ore. In case of BHJ ore, fine hematite and quartz grains are intimately inter-grown and are dispersed in thick jasper bands leading to liberation problems. Besides that, dusty jasper particles coat over hematite surface in case of BHJ ore during the process of grinding, which hinders specific adsorption of collector on hematite or quartz surfaces. This is substantiated by scanning electron microscopy (SEM). The flotation studies have indicated that it is possible to achieve ~64% Fe with ~67% recovery from a BHQ ore using both anionic and cationic collectors from a feed containing around 38% Fe. In contrast, the hematite recovery in BHJ ore was poor. Differences in the ore characteristics that affect the flotability of hematite present in both the ores are compared. "As the bulk handling sector continues its transformation to Industry 4.0, the mining industry, and its harsh working and environmental conditions, provides a perfect spot to study the feasibility of remote analysis and maintenance of the components used for bulk handling. The most important and expensive tools have been adapted to the digital era in recent years, from remotely controlled haul trucks to monitored conveyor belts.However, the big items of the installations rely on smaller parts that have been leftaside, mainly due to the difficultyof adapting them to the Internet ofThings (IoT).The paradigmatic example is the lack of monitored rollers, while all the other components of the conveyorbelt installations (the belt itself, motors, drums, etc.) were monitored some time ago. The main problem is that a failure of a single roller can cause the stoppage of the entire conveyor, due to possible damage to the belt. Thus, the implementation of an integral monitoring system of all the components of the conveyor is a must.The research and development department of ULMA Conveyor Components has been working since early 2012 on the development of monitored rollers, testing all the problems until their final installation in different demanding conditions, from the extreme heat in Australia to the freezing cold of Norway. All the tests have been done under actual working conditions, in real installations and have provided valuable knowledge in the identification of problems, their solutions and the final advantages of a roller monitoring system.Background and current stateThe malfunction of a single roller in certain conveyor belts can force the standstill of the entire installation until its replacement, due to the risk of greater and more expensive failures, such as a rip in the belt. In the case of a bulk handling installation, where a continuous supply of material is necessary, an unexpected stop of one hour could result in huge economic losses."

INTRODUCTION A mine's power system represents the driving force behind all of the extraction and auxiliary operations, because the production and transportation of mined material and the operation of equipment, such as fans and pumps, are all dependent upon a power source. Once, compressed air was the primary power source in mining; today, however, electrical power dominates. This chapter reviews the basic theories of com- pressed-air and electrical power and provides several examples of applications in mine power systems. COMPRESSED-AIR POWER General Compressed air has been, and should continue to be, an important source of power in mining operations. It has been used as a means of blasting coal in conventional mining and is widely used to operate stopers, mucking machines, and other air tools in both coal and hard-rock mines. Such applications demonstrate the suitability of compressed air for applications requiring linear motion, but it is also utilized for its reliability and safety. A compressed-air system is composed of a compressor, a receiver, a distribution network, and the air-operated machines. A compressor takes in air at normal atmospheric pressure (free air) and compresses it to a higher discharge pressure. The discharge pressure must be high enough to overcome the friction in the distribution system of pipes and hoses and deliver the compressed air to the machines at the pressure recommended by the manufacturer. The most common type of compressor used in mines is the reciprocating compressor in which the air is compressed by a piston in a cylinder. An air receiver is a container or storage tank that is located in the distribution system between the compressor and the machines. It stores compressed air, when the full capacity of the compressor is not being used, and gives a more steady flow of air to the machines. The distribution network consists of pipes, Oalves, elbows, tees, and hoses that transmit the compressed air from the receiver to the machines in the mine. It is essential that the distribution system be designed with the proper sizes and lengths of pipe, hose, and other components to keep pressure losses well within allowable limits. The air-operated machines consist primarily of drifters, stopers, pluggers (sinkers), slushers, and several other pieces of equipment. Compressor Operation Every compressor is made up of one or more basic elements; a single element, or a group of elements in parallel, comprises a single-stage compressor. Many compression problems involve conditions beyond the practical capability of a single compression stage. Too great a compression ratio (absolute discharge pressure divided by absolute intake pressure) may cause excessive discharge temperatures or other design problems. It, therefore, may become necessary to combine elements or groups of elements in series to form a multi- stage unit, in which there will be two or more steps of compression. The number of stages commonly used in reciprocating compressors is as follows: Pressure Number of stages 0- 150 pig 1 80- 500 psig 2 500-2500 psig 3 2500-5000 psig 4 When pressure is referred to as pounds per square inch gage (psig), it means the pressure (in pounds per square inch) above barometric pressure as measured by a gage. The sum of barometric pressure and gage pressure is the absolute pressure (in pounds per square inch). The gas is frequently cooled between stages to reduce the temperature and volume entering the subsequent stages, thereby reducing the work required for compression. The basic reciprocating compression element is a single cylinder compressing on only one side of the piston (single-acting). A unit compressing on both sides of the piston (double-acting) consists of two basic single-acting elements operating in parallel in one casing. The reciprocating compressor uses automatic spring-loaded valves that open only when the proper differential pressure exists across the valves. Intake valves open when the pressure in the cylinder is slightly below the intake pressure. Discharge valves open when the pressure in the cylinder is slightly above the discharge pressure. The primary advantage of using oxalic acid is in the formation during leaching of soluble complexes that are both photo-chemically and microbiologically degradable. In this study, chemical leaching and bioleaching methods were used for quartz sand. Using the ANOVA-Yates test technique, the effects of solids rate, temperature and oxalic acid rate were investigated using chemical leaching experiments. Solids rate and temperature were effective parameters on the chemical leaching. After the tests, the removal of Fe2O3 increased to 97.02%. Finally, the iron content decreased from 0.315% Fe2O3 to 0.0094% Fe2O3 in the leached quartz sand. The effect of pulp density was investigated on the bioleaching experiments. The bioleaching of iron oxides by Aspergillus niger resulted in removal of 47.70% of the total iron in the quartz sand, and suggested that this strain has not a good ability to remove iron impurities from quartz sand in comparison with chemical leaching. The iron content decreased from 0.315% Fe2O3 to 0.164% Fe2O3 in the bioleached quartz sand.

Introduction Tanco History Tanco is sometimes known as the Bernic Lake Mine. It is located about 180 km east-northeast of Winnipeg close to the Manitoba-Ontario boundary. Tanco operates an underground mine employing ramp access, room and pillar mining and shaft hoisting for the production of tantalum, lithium and cesium minerals. Tantalum recovery employs conventional gravity and centrifugal concentration methods along with a flotation process which recovers ultra-fine tantalum. All tantalum concentrates are shipped to Cabot Supermetals, Boyertown, Pa., for the production of tantalum chemicals and metals. Tanco also produces lithium products from spodumene ore. The lithium concentrates from Tanco are shipped worldwide for use in ceramics, specialty glass and mold powders. Cesium processing involves the dry grinding of pollucite as feed stock to the on-site chemical plant dedicated to the production of cesium formate, a high density drilling fluid. Tantalum milling operations commenced in 1969. In the early years of the operation, feed grades were three to four times as high as they are today. Coarse-grained tantalum mineralization with complete tantalum liberation at sizes of 300 µm allowed for easy recovery with conventional equipment such as spirals and shaking tables. Over the years, the process and feed material to the plant have drastically changed. Due to drastically reduced liberation sizes, efforts have been made to try to recover as much of the liberated particles as possible in order to maintain acceptable recoveries.

Measurements of the quantity of entrained air within processes have challenged instrumentation developers for years. This is particularly true in the unique and demanding operating environment and applications seen in minerals processing. In minerals processing applications, entrained air plays a role in errors seen within nuclear density gauge measurements and in pumping efficiency. It is also the major component of flotation processes. The measurement of the quantity of entrained air, known as the gas void fraction, can now be performed from the outside of a process pipe through CiDRA?s patented passive sonar array-based technology. Within column cells, the need for a robust, no maintenance, accurate means of measuring the entrained air, known as gas holdup, has resulted in a submersible version of this technology. CiDRA?s meter technology calculates the gas void fraction by first measuring the speed at which naturally occurring sound in the process slurry propagates past an axial array of sensors. These sensors are incorporated in a band that is wrapped around the outside of the pipe, resulting in no process downtimes for installation and unprecedented reliability. A known relationship between this speed of sound and the entrained air captured in the form of bubbles within the slurry is then applied in CiDRA?s meter to calculate the gas void fraction. This paper will present the measurement technology, its operation, its calibration and its application in a variety of gas void fraction and gas holdup measurement situations such as pumping efficiencies, hydrocyclone operation, external sparging, and column cells.

PRINCIPLES OF MINE PLANNING During the initial planning stages, all mining operations are analyzed and evaluated in a similar fashion. This similarity holds true because the mine-planning procedure can be reduced to a network of interrelated systems that are tied together by a common philosophy of mine planning: namely, that the resource is to be extracted in a manner which is safe, efficient, and profitable. The success of the mining operation cannot be guaranteed unless each of these three requirements is met. Fig. 1 shows a flowchart for the initial planning of any mining operation. Notice that there is a preplanning stage that leads to the planning of four interrelated systems. The preplanning stage includes property exploration, estimation of production and manpower requirements, and the establishment of construction timetables. The four systems, evaluated after the pre-planning stage, are broken down into the following: (1) Excavation and handling system, (2) Strata control system, (3) Operating support system, and (4) Service support system. The Excavation and handling system includes the selection of all the equipment for the removal of the resource from its in-place deposit (e.g., draglines, continuous miners, rock drills, etc.) and for the transportation of the mined material from the face (e.g., belt conveyors, trucks, etc.). The Strata control system incorporates all those considerations which maintain the integrity of the roof, ribs, floor, and overburden both during and after the extraction phase; typical considerations in this area include pillar sizing, artificial support of the immediate roof in mine entries, and subsidence protection. The Operating support system contains the "necessary evils" of mine planning; the system consumes a large percentage of the mining engineer's planning time but does not contribute to the actual extraction of the resource. However, if proper attention is not paid to the various components of the system, such as ventilation, drainage, and power, then the mining operation will not perform to its capabilities. Finally, the Service support system is composed of those operations which keep the mine running efficiently; typical examples of these include maintenance and sup- ply. One important concept that Fig. 1 is designed to emphasize is that the four systems are interrelated; typically, a slight alteration of one can affect the performance of another. If these interrelationships are ignored, the effects on the system can be disastrous. The mine preplanning stage and the four subsequent system evaluations constitute the Analysis portion of the mine planning flowchart. After this portion of the study is completed, the overall projected operation can be evaluated by techniques such as financial analysis. This last step in the study is referred to as the Synthesis of the previously calculated parameters into an overall plan. Much has been written about the overall evaluation of mining properties, but, over the past thirty years, very little has been written about the analysis of mine- planning systems in a coordinated manner. Further, those texts which attempted this topic have failed to give sufficient examples for successful implementation, thus supporting the operating engineers' lament that it is easy to discuss theories but difficult to show applications. This text is intended to bridge this gap. Examples of the various stages of the analysis portion of the mine planning flowchart shown in Fig. 1 will be covered in this text, with the exception of the Service support system, since this system is oriented more toward day-to-day planning than long-term mine planning. INTERNATIONAL STANDARDIZATION The benefits that can be derived from an international standardization of the units used in mining engineering calculations are numerous. The interchange- ability of equipment parts and the ability to analyze and compare performance and design specifications on an equal basis are just two of the goals desired by both manufacturers and mine operators. Unfortunately, conversion to an international standard has not been rapidly achieved by engineers in the United States, due to their training under the English system. The Society of Mining Engineers of AIME has made the commitment for the adoption of the Systeme International d'Unites (SI) in all mining engineering calculations. Since many practicing mining engineers and students in undergraduate programs still find difficulty in relating to SI units, the calculations in this textbook will use the English system, with Table 1 available for conversion into SI units.

Surface and underground mining operations require equipment with high power output ratings. Haul trucks, loaders, dozers, and other equipment need increasing horse-power to meet production demands. To get more horsepower out of the engines driving the equipment, most manufacturers use turbocharging. Detroit Diesel puts the number at about 90% turbocharged, while Caterpillar estimates 75% to 80% of all its equipment is turbocharged. Turbocharging increases the power output and overall engine efficiency. But it presents some maintenance and operational problems that were not factors when only naturally aspirated engines (nonturbocharged) dominated the scene. A turbocharger is an exhaust-driven air compressor. Using the waste energy in the engine exhaust, a turbine wheel is driven. This wheel is connected by a common shaft to a compressor wheel. The shaft is supported by two plain bearings, or bushings. As the turbine wheel is rotated, the compressor wheel continually compresses the intake air to the cylinders. Compressing the intake air means that more air can be put into the cylinder to burn more fuel. The more fuel the engine can completely burn or combust, the more horsepower the engine can produce. Even though the turbocharger is a relatively simple device, it is a sensitive engine component. It requires special consideration at engine start-up and shutdown. Being freewheeling and exhaust driven, turbochargers can experience temperatures of 540°C 1000°F) and rotational speeds of more than 100,000 rpm. Due to those temperatures and speeds, lubrication is critical to the operation and life of a turbocharger. The turbo is located high on the engine, with the oil supply running through an external line from the sump. The recommended practice is to start the engine and allow it to idle for a few minutes before any revving or loading. This allows the oil to reach the turbo and properly lubricate the bearings before high rotational speeds are seen. Turbo oil supply lines can be as long as 1.5 m (5 ft). Depending on ambient temperatures and oil viscosity, it can take as long as 30 seconds for oil to reach the spinning turbo. However, as little as five seconds without oil can damage the turbo's bearings, reducing its life and overall efficiency. Lubrication problems seen at shut-down can also significantly affect turbo life and efficiency. Manufacturers recommend an idling period of five to seven minutes before engine shutdown. This allows the turbo to slow down and cool off somewhat to help prevent oil coking and bearing damage. When the engine stops, so does the oil supply. There is very little oil left on the bearing surfaces and in the turbo oil supply passages. The immediate heat transfer from the turbine wheel to shaft, bearings, and turbo body literally cooks what is left. This high heat without oil flow can cause bearing deformation by actually melting some of the bearing material. The rapid oil oxidation and coking will restrict oil passages, damage bearings and shaft. Besides heat soaking, there is another problem. If the turbo is spinning at high speeds when the engine is shutdown, the turbo must spin-down to a stop without oil flow. With the large size of turbos used on mining equipment, this spindown time without oil can be for as long as 10 to 20 seconds. Fortunately, lack of lubrication at start-up and shutdown and the subsequent bearing damage can be prevented by oil injection devices. These devices hold a reservoir of engine oil. The oil is injected to the turbocharger bearings immediately on start-up and for several minutes after shutdown. Oil injection devices are spring loaded, hydraulic accumulators. They are actuated at start-up by a solenoid wired to the starter motor. Engine oil pressure charges the accumulator during normal operation. Upon shut-down and loss of oil pressure, the device provides oil to the turbo for about two or three minutes. With an oil injection device, the engine can be started and immediately driven without fear of turbo damage. Also, an abrupt shutdown will not damage the turbocharger since the device will continue to lubricate for two or three minutes. A word of caution. Be sure the oil injection device provides positive oil displacement (pressurized oil flow). Do not use a gravity-flow device. Unless it is a true accumulator, the device could starve the turbo for oil on start-up, Ask for start-up and shut-down pressures and delivery times before considering installation. Keep the oil clean, and follow the manufacturer's recommended oil change intervals. These intervals are usually shorter than with naturally aspirated engines. Use oil with a CD rating designed for diesel applications. A multiviscosity oil is preferable. Use a synthetic engine oil, such as Mobil's Delvac 1, Gulf's Super Duty II, or Chevron's Sub-zero Fluid. Synthetics have much better flow properties at low temperatures. They also have greater oxidation stability (they can take the high heat of turbo-bearing lubrication. And they produce fewer deposits because they are more thermally stable. But synthetic oils do cost more - two or three times more. One additional point - maintain the air intake system. Since all mining is usually in a dusty environment, air filters should be changed regularly and intake systems should be checked for leaks. All intake air passes through the turbo. Any dirt or abrasives entering the turbo will damage the unit and shorten its life. While mining is a heavy duty business with severe operating conditions, you can get the life from your turbocharger that you expect by following a few simple steps: • equip the turbo with an oil-injection device to provide lubrication on start-up and shutdown; • change the oil and filters regularly; and • use the proper oil-synthetic, if possible.

This paper will discuss the construction of 15.9 km (9.88 mi) of tunnels on a project in Chicago, Illinois. The tunnels were driven utilizing two rock and four soft ground TBMs ranging in sizes from 7.39 m (24.25 ft) in rock down to 1.95 m (77 in) in soft ground. In addition to the mining, there are 63 upper level structures and 16 dropshafts averaging 61.5 m (202 ft) deep included on this project. Many diverse mining, excavation, support and lining techniques were employed on this project.

The Getchell Mine, near Winnemucca, NR, was discovered in the 1930s and was initially mined to recover gold from oxide ores. During the 1940s and 1950s, gold was recovered from the highly refractory arsenic sulfide ores using horizontal kilns to roast the ores. During the 1960s, fluid bed roasters were used to oxidize the ore and recover the gold. The mine was shut down from 1968 through 1985. In 1985, heap leaching was started on the low-grade oxide ores that were available. In 1989, a pressure oxidation circuit was started to recover gold from the higher grade sulfide ores. Flowsheet The basic plant flowsheet of the Getchell Mine are shown in Figs. 1 and 2. The plant was designed to process a nominal 2.7 kt/d (3000 stpd) of ore with about an 85% onstream availability and a gold production of 15.5 kg/d (500 oz/ day). Run-of-mine (ROM) ore is fed directly to a wet semiautogenuous grinding (SAG) mill after the +457-µm (+18in.) rock is removed on a stationary grizzly. From the SAG mill, the ore goes to a ball mill where it is ground to 80% -75 µm (-200 mesh) and is then sent to a thickener. The SAG and ball mill operate in closed circuit using cyclones.

An OCS Expert control system was implemented by Svedala Cisa at the Minera Maricunga Refugio Crush-Leach Operation near Copiapo, Chile. The purpose of the Expert system was to maximize production through the primary and secondary crushing circuits and help balance crusher usage between each stage to maximize total plant production. Interesting aspects of the control strategies are discussed. Benefits of the control were verified through a randomized On/Off test program. Tonnage increases ranged from 5% to approximately 20% at good confidence intervals. Benefits analysis results are discussed. The crushing plant at Refugio is very remote and is located at approximately 15,500 ft. This poses many challenges. The effect of altitude and remote location on plant operation are discussed.

Continuous developments in tunneling require knowledge transfer between designers and suppliers to optimize the application of support systems. This collaboration allows the designer access to the latest research results, technical innovations, and support specific measurement techniques as well as the supplier?s experiences on recent projects. Project specific requirements determine the information that can be used to increase support system efficiency, advance rate, degree of safety, or decrease costs. This cooperative effort results in not only an unambiguous tender document, but also an optimized design with reduced risk that is coordinated with safe and high performance construction techniques. This paper will address the benefits of an early collaboration to both the design and the specifications in the tender document by using the A3 Hindhead project (UK) as an example.

Tunnel excavation using tunnel boring machines (TBMs), has become increasingly common in recent years, despite the fact that precise evaluation of certain risks have not kept pace with the use of these machines. One of the risks easily overlooked by Engineer and Contractor alike are the effects of abrasive ground on the costs and schedule of a given project. The impacts of worn and damaged TBM cutter heads have been observed on hundreds of tunnel projects around the world. It would appear that a reliable prognosis of the abrasiveness of soils on a project would be of great importance for designers, clients and contractors alike. Several well acknowledged test and prognosis methods already exist for rock, however there is only very limited knowledge available to describe the abrasiveness of soil and its impact on soft ground TBMs. This paper will examine approaches to this problem and suggest a new approach based on a current project undergoing design.

From 1993 through 1999 Cyprus Amax Minerals Company successfully completed eight major construction projects around the world. Six greenfield mines were brought into production, an ongoing operation was completely modernized, and an environmental issue was appropriately remediated. The projects stretched over three continents, from arctic Russia to the Atacama Desert, from 16,000feet to sea level, from city suburbs to remote wilderness, from a low of $130 million to over $1 billion. Open pit and underground; mill and leach; gold, copper, molybdenum, and coal were all encompassed. The breadth of diversity experience from these projects inspired Cyprus Amax to create a “best practices” project management protocol, to steer corporate expansion into the next millenium. This paper summarizes those best practices, while high-lighting project management pitfalls to avoid.

The four top barite-producing countries in the world are China, India, Morocco and the United States. China is currently the largest exporter of barite followed by India. More than 95 percent of the barite mined today is used by the petroleum industry as driller’s mud. The rest is used in the industrial market and the medical field. Most of the barite mined in the United States is in Nevada. This year, mining activity has increased due to an increase in oil exploration/development. This was caused by an increase in the crude oil prices.

Dimension stone is a natural stone (granite, marble, limestone, sandstone and slate) that can be quarried, cut and shaped to conform to acceptable industry standards for size, consistency and durability. There are numerous outlets for the finished product in exterior and interior architectural and design applications. A continued growth market is with residential usage— particularly in new, upscale construction. This has become a competitive market between domestic producers and lower cost imported products. These imported products have become available at many of the building supply retail outlets and have increased the use of stone in residential construction and renovation. The traditional architectural application of dimension stone is tied to commercial and institutional building construction trends and market. There has been a noticeable increased usage of stone in the Sun Belt. This stone is usually supplied by local fabricating shops. It is used in the exterior and interior of shopping malls. This market generally uses local stone sources. It has increased the demand for sandstone, limestone and quartzite in this region of the country.

Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.
