This paper presents the results of a unique study conducted by the National Institute for Occupational Safety and Health (NIOSH) from 2016 to 2019 to evaluate the effects of longwall-induced subsurface deformations on shale gas well casing integrity and underground miner safety and health. At both deep and shallow cover instrumentation sites, surface subsidence measurements, subsurface in-place inclinometer measurements, and underground pillar pressure measurements were conducted as longwall panels were mined. Comparisons of the deep and shallow cover test site results with those from a similar study under medium cover reveal an interesting longwall-induced response scenario. Under shallow and medium covers, measured horizontal displacements within the abutment pillar are one order of magnitude higher than those measured under deep cover. Conversely, measured vertical compressions under deep cover are one order of magnitude higher than those under shallow and medium covers. However, FLAC3D simulations of the casings indicate that, in all three cases, the P-110 production casings remain intact under longwall-induced deformations and compressions, which has serious implications for future mine design in areas where shale gas wells have been drilled ahead of mining.

Faced with rapid growth that strained their current water supply capabilities, the Somerset Water Service in Somerset, KY decided to construct a new intake structure on the shore of Lake Cumberland. With an existing intake structure within 200', very hard limestone geology with calcite geodes, and lake level fluctuations as high as 50 ' ,this project posed many challenges. The following paper discusses how mechanical excavation techniques were used to overcome these obstacles.

In 1991, LTV Steel Mining Company decided to close an open pit taconite mine in northeastern Minnesota, using a passive treatment approach consisting of limiting infiltration into the stockpiles and wetland treatment to remove metals. Over 50 million metric tons of sulfide-containing waste had been stockpiled adjacent to the mine during its 30 years of operation. Drainage from the stockpiles contained elevated levels of copper, nickel, cobalt and zinc. Nickel is the major trace metal in the drainages, and prior to closure annual median concentrations ranged from 1.5-50 mg/L. Copper, cobalt and zinc are also present but are generally less than 5% of the nickel values. Median pH ranges from 5.0 to 7.5, but most of the stockpile drainages have pH greater than 6.5. Based on the chemical composition of each stockpile, a cover material was selected. The higher the potential a stockpile had to produce acid drainage, the lower the permeability of the capping material required. Covers ranged from overburden soil removed at the mine to a flexible plastic liner. Predictions of the reduction in infiltration ranged from 40% for the native soil to over 90% for the plastic liner. Five constructed wetlands have been installed since 1992, and have removed 60-90% of the nickel in the drainages. Total capital costs for all the infiltration reduction and wetlands exceeded $6.5 million, but maintenance costs are less than 1% of those for an active treatment plant. Since mine drainage problems can continue for over a hundred years, the lower annual operating costs should pay for the construction of the wetland treatment systems within seven years.

This paper will discuss one of the largest construction projects the City of Portland has undertaken. This phase of the Willamette River Combined Sewer Overflow (CSO)project consists of approximately 18,500 ft of tunnel in soft ground, requiring the construction of six deep shafts and jet grout ground improvement. The purpose of the project is to reduce volume of combined sewerage overflows from the drainage areas on the west side of the Willamette River. Bencor-Petrifond-Pacchiosi Joint Venture was awarded a contract to design and construct six deep circular shafts, jet grout ground improvement for three bridges and jet grout ground improvement break-in/outs for some shafts. Of the six shafts the Pumping Station Shaft was the deepest and largest, requiring a permanent ground support system with a deep jet grout ground water cut-off to 335-ft deep. The joint venture used state of the art methods and equipment to achieve its objectives and build the structures through some very difficult alluvial soils to great depths next to the Willamette River. The paper will provide a general discussion of the shaft construction and jet grout ground improvement installed for this phase of the project. It will also discuss some of the technical challenges encountered while effectively managing risk and costs, resulting in a successful project completed in a timely manner.

Brightwater West (BT4) represents the state of the art in utility tunneling, namely a long relatively small diameter, soft ground tunnel, with no intermediate shafts, under significant groundwater pressures, requiring very precise survey control in order to hit a small exit window. The tunnel is over 6.4 km (4 miles) in length, is expected to encounter active earth pressures of over 5 bars in glacial geology, and a planned hole through into a shaft eye constructed at 45.7 meter (150 feet) below the water table. Despite the fact that all of these challenges have been previously overcome in larger diameter tunnels, the technical solutions are much more challenging in a smaller diameter due to the lack of space available for implementing the equipment and techniques required. In addition to the technical challenges, the project is faced with many of the constraints designed to minimize the impact of these types of projects on the neighboring community and environment. The work shaft for BT4 is within 15.2 m (50 feet) of Puget Sound and is on a site that was previously occupied by petroleum storage tanks, and all of the tunnel muck has to be removed from the site by barge as trucking is not allowed by contract. The paper addresses the work performed during the preparation for tunneling stage of this project both in assembling the TBM and in preparing the site for its arrival.

Tunnel boring machines continue to develop to take on not only larger and larger diameters but also more difficult ground conditions. Ground conditions consisting of unstable faces with water pressures up to 15 bar has required innovative design approaches. A systems approach has been required to produce tunnelling machines that can not only safely excavate the tunnel but do so in an efficient, cost effective manner that stresses production and labor saving approaches. This paper will review the current design developments and present recent projects where these most recent principles have been applied.

Several recent tunnel jobs have required special adaptations of hard rock TBMs. Some of the special job conditions are ?no blast? launch requirements, small shafts or launch pits, proximity to existing underground structures, and short tunnels where the TBM must be retrieved through the primary lining. This paper describes a few of these projects, and the machine adaptations that have made TBM excavation feasible.

This paper describes the use of post- tensioned rock anchors to reduce the risk of future damage to a residential structure as a result of coal mine subsidence. The project described in this paper was performed by the Office of Surface Mining Reclamation and Enforcement (OSMRE) of the U. S. Department of the Interior. The OSMRE was created to administer Public Law 95-87, the Surface Mining Control and Reclamation Act of 1977. The responsibilities of OSMRE include "reclamation and restoration of land and water resources, adversely affected by past coal mining including prevention, abatement, control of coal mine subsidence. ?('l) Consequently, OSMRE has been addressing mine subsidence problems since its initial subsidence stabilization project in 1979. Stabilization activities have primarily consisted of supporting the overburden material by injecting materials through drilled bore holes into the underground mine voids. Injection of various materials into mine voids, including fly ash mixtures, coal refuse, cement grout, and gravel has proven to be a successful method of reducing mine subsidence-related surface movements, and has been extensively utilized in mitigating mine subsidence problems. However, the cost of drilling for injection can be extremely high in areas where the depth to the coal seam is great.

The Egnatia Odos (Odos meaning road) is one of Greece’s largest ever highway construction projects and one of Europe’s most important. The Highway will assist in the connection of the Greek Ionian/Adriatic Coast with Turkey and the Black Sea reviving an historic Roman alignment “Via Egnatia” connecting Rome to Constantinople (now Istanbul). There are 74 twin tunnels (60 bored and 14 cut-and-cover) with lengths ranging from 160 m (175 yd) to 4,600 m (5,031 yd) and 100 m (109 yd) to 270 m (295 yd),respectively. The complex geology and resulting physical geography have had a major influence on both the design and construction of these tunnels. This paper describes the management, design and construction challenges of these numerous tunnels.

Oxide ores can consume enormous amounts of expensive sulfuric acid. The problem is amplified with increasing heap heights. One solution is to provide a thin liner and drainage system between each lift of ore, thereby intercepting the pregnant leach leach solution (PLS) after it exits the active lift. However, these liner systems require special attention to hydraulics, slope stability and geomembrane design criteria. Inadequate attention to the first two can result in reduced recoveries, increased acid consumption andsystem failures. The geomembrane selection criteria are different from the base liner, and optimizing its selection can have significant effects on the operation’s bottom line.

INTRODUCTION Countries that are currently developing their mineral potential have unique information systems requirements to support these activities. The mining laws usually grant the State considerable authority over the process. In many instances, the mining laws give a government agency the responsibility for granting and controlling exploration permits and development concessions. This is the case in Costa Rica. The Direcion of Geologia, Minas, y Hidrocarburos (DGMH) of the Ministerio de Industria, Energia y Minas (MIEM) is responsible for the management and operation of the National Mining Register. This paper describes the results of two studies of the information system requirements to support the National Mining Registry as well as those supporting some of the other function of the DGMH. The first of the studies reviewed the situation before the installation of computer equipment. The second study evaluated the installed systems. BACKGROUND Historical Perspective Mining in Costa Rica is at present a relatively minor part of the economy. It consists primarily of industrial minerals and gold production. Gold production is largely by small-scale miners and may be more substantial than reported. There is considerable interest on the part of the Government in both having a better understanding of the existing mining industry and promoting future development. Mining law was first provided by the Ordinanza de Mineral (of 1830). This was replaced by the first Mining Code of 1953. Apparently, one of the first problems caused by the 1953 law was the granting of concessions which were not worked. A 1968 agreement with ALCOA also resulted in political conflict. The Government worked on a new Mining Code for many years and in 1982 the current Mining Code was enacted. Draft implementing regulations were prepared in April 1984. Mining Code of 1982 The Mining Code of 1982 addresses most aspects of mining in Costa Rica including the declaration that all mineral resources are to be the property of the State. The State may grant exploration permits and exploitation permits. The exploitation of hydrocarbons, radioactive minerals and certain water resources was reserved to the state. The DGMH was given authority to settle placer claim disputes, maintain a registry of exploration permits, and exploitation concessions. and generally supervise mining activity. Towards this, the Mining Code of 1982 includes rather detailed requirements for the processing of application for permits and concessions. Mining Code Application Requirements The following briefly describes the requirements to give the reader a feeling for the complexity of the process. Currently, over 1,000 active applications, exploration permits and exploitation concessions are being monitored under the process. The number remains relatively consistent in that the number of new applications is usually about the same as the applications that are put aside. The time and date of the filing must be certified by the DGMH. The application must include all the information specified in the Mining Code. If incomplete, DGMH must notify the applicant of the deficiencies or omissions and direct that the applicant correct the deficiencies or omissions within 8 business days (a single 30 business-day extension is allowable). If the applicant does not respond as directed, the application is deemed as not having been filed and the pertinent information is placed in the files. The DGMH must reject the application outright and direct it be placed in the files in the following cases: (a). If the new application covers the area of a previous permit or concession, either being processed or already granted.

Engineering properties of rock are essential in most phases of mining from site reconnaissance through mineral processing. Review of the literature indicates, however, that property data is scarce for most coal measure rocks in the United States. The Bureau of Mines, U.S. Department of the Interior, has undertaken a wide-ranging testing program to provide more complete property data for coal strata in the U.S. coal basins. The property data are expected to have application in geophysical exploration, the development of pre- mining and hazard detection probes and instrumentation, mine design, engineering and rock mass classification, subsidence engineering, and ground control. This paper describes the comprehensive program encompassing in situ geological, geophysical, and geotechnical properties and laboratory determinations of acoustic, electromagnetic, physical, mechanical, and index properties and discusses results for 500 ft of coal measures at a minesite in the Northern Appalachian, Eastern Coal Province.

Whether drilling through tough clay or breaking rock, or facing the most severe conditions including mixed face ground, perched water and running and flowing sands, working under-ground certainly comes with no shortage of challenges. In an environment where the unknown is faced on a daily basis, tunneling contractors must be able to rely on their equipment to get the job done properly. Not only that, their equipment must comply with two of the highest priorities: safety and efficiency. In addition to being the number one concern, a proven safety record provides additional benefits to a company. A safe work environment serves as a key component to company growth, including the ability to remain competitive in bidding and maintaining higher-level, more challenging projects.

LOCATION AND HISTORY The Lakeshore Copper property is located on the Papago Indian Reservation approximately 30 miles South of Casa Grande, Arizona. This major porphyry type deposit was discovered by El Paso Natural Gas Company in 1966. Following an extensive diamond drilling program, El Paso invited several mining companies, including Hecla, to examine the property with a view towards participation. Hecla's examination, and the attendant feasibility studies, led to agreements, which were finalized in September 1969, whereby Hecla would acquire a 50% interest in the property, and would act as operator. The Lakeshore deposit covers an area approximately 3,000 feet in width by 6,000 feet in length. It consists of an upper zone of disseminated copper oxides, averaging approximately 100' in thickness overlying a zone of disseminated copper sulfides up to 1,000 feet thick. Near the base of the disseminated ore is a blanket like zone of high grade copper sulfides, referred to as the tactite ore body. The mine is being developed to produce approximately 6,500 tons per day of oxide ore and 9,000 tons per day of sulfide ore. This represents a total daily production rate of 15,500 tons per day, which will be achieved by underground mining methods using rubbertired dieselpowered mining equipment. DEVELOPMENT LAYOUT The mine is being developed by two parallel 15° decline tunnels. The declines were collared 5,000 feet southeast of the ore body, and are being driven in a northwestery direction, to pass beneath the tactite ore zone. Novel dewatering aids were tested on various coal samples from the U.S. and Canada in laboratory and pilot-scale filtration experiments. The reagents substantially improved the dewatering kinetics and reduced the cake moistures. The cake moistures were typically reduced from 20-30% by weight to 8-14% range in vacuum filtration. When the reagents were used in conjunction with a pressure filter, higher degrees of moisture reductions were achieved. In the present work, the novel dewatering aids were tested using different types of pilot-scale vacuum filters. The results were consistent with those obtained in laboratory experiments.

Introduction Rock bolting is a general term that encompasses pinning, roof bolting, rib bolting, and cable bolting. Rock bolting contributes the highest nonproductive cost and is the most significant safety risk in mining today. Total installed bolt costs are now more than $20 per bolt. Rebolting or repair work doubles this. And bolting accidents account for about 30% of all fatal and nonfatal lost time accidents. It is only logical, then, for the mine operator to install the highest quality anchors possible. Every bolt on the market - tensioned, untensioned, full contact, or point anchor - has designed capabilities in axial, shear, and fluctuating ground conditions. Bolts will provide a designed support if material specifications and installation procedures fall within the manufacturer's recommendations. This also assumes that there are no unexpected geological or operational changes. Proper material specifications and installation procedures should guarantee the mine operator a quality anchor. To help assure this, the Mine Safety and Health Administration (MSHA), the US Bureau of Mines (USBM), and the AMC Manufacturers group developed stringent ASTM materials standards. These govern mechanical point anchor bolts and bolts using threaded and nonthreaded rebar. There are also minimum standards that suppliers must comply with in the manufacture of frictional bolts. However, most frictional bolt standards are based on the manufacturer's own specifications. It would be impossible to set uniform regulations on frictional bolts. Each bolt provides support in a slightly different manner and thereby requires different material needs. Quality Control In any case, material quality can generally be assured through tight order specifications and periodic quality control checks. The problem with installing quality anchors does not solely lie with the material. It includes all the events that occur from the time a bolt leaves a manufacturer to the time it is installed. The anchoring ability of any major type bolt can be affected by transportation, storage, mechanical performance of installation equipment, installation demands of the operator, proper drilling accessories, and local geology. Table 1 and Fig. 1 document the negative events that can affect each bolt and the degree to which it can be affected. Drilling a hole of oversized diameter, for example, is an operator controlled factor. It will adversely affect most bolts. An over-sized diameter reduces, or even eliminates, the anchorage capability of a mechanical shell. It will reduce the support capacity of a split set to nearly nothing. And it will cause a partial or incomplete grouting around the resin bolt. Frequently, due to bit wear, holes are drilled undersized in diameter and may not be properly cleaned out. In this case, it will be difficult for the operator to push the mechanical shell into the hole, thereby causing him to drill another hole. In cases where the bolt is obviously not providing anchorage, operators will probably drill another hole and place another bolt. Rock Bolt Regulations It is the situations where a bolt is improperly installed in the hole, but gives the appearance of a good bolt, that are frightening. In fact, some safety officials and engineers believe that only 50% to 60% of installed bolts are working. These bolts cause irregular ground load transfers in other bolts and overload them. This causes hazardous situations and overstressing. It is these bolts that fail prematurely 200 to 255 mm (8 to 10 in.) behind the face and possibly contribute to the high accident rate. Political risk is an integral part of the mining business in Northern Andean countries. Peru, Ecuador, Colombia, and Venezuela all have a higher than average political risk compared with other South American countries. Successful operations in such contexts require: 1. Good political risk information; 2. A thoroughgoing political risk management plan; and 3. A strategy to deal with political risk perception on the part of the investing public. Obtaining quality political risk information, developing a political risk management plan, and addressing political risk perception are the foci of this work.

INTRODUCTION Project Description The Silver Line is a Bus Rapid Transit (BRT) Line and is Boston?s newest transit service. Silver Line Phase I extends along Washington Street entirely on the surface in priority bus lanes from Dudley Station in Roxbury through the South End and Chinatown neighborhoods to downtown Boston. The Phase I opened in July 2002. Silver Line Phase II consists of an underground transit tunnel from South Station to the South Boston Waterfront with underground stations at Courthouse and World Trade Center to Silver Line Way east of D Street. Phase II continues through the Ted Williams Tunnel to all the terminals at Logan Airport, and on the surface to the Boston Marine Industrial Park and City Point. Phase II opened in December 2004. Silver Line Phase III is a one mile tunnel connection to the existing Phase II tunnel in the vicinity of South Station and will eventually portal on Tremont Street just north of Marginal Road. After leaving the portal, the buses will use dedicated bus only lanes on the surface roadway network to connect to the Phase I service. Figure 1 shows the alignment of the Silver Line upon completion of the Phase III project.

The classical approach to control rock fall and ravel in open pit mining has been to use catch benches. The width and volume of benches is most dependent on the bench height, size of loading equipment, blast damage to the face and mining regulations. This was compatible with open pit slopes of less than about 45°. As our understanding of rock mechanics and blasting control has improved over recent years, much steeper rock slopes can be developed at many open pit mines. However, narrower benches result with less rock fall capacity. The transportation industry has been very successful in using catch fences to protect highways throughout North America and Europe for years. The authors recommend that cable and ring net fences have a potential major application in open pit mining to allow steeper rock slopes with significant economic benefit. This paper outlines the application, design and operation of catch fences in surface mining for specific conditions. Typical case examples are presented.

More than 111,350 mining and construction industry professionals from throughout the world gathered in Las Vegas, NV, March 19-23 for ConExpo-Con/Agg 2002 and IFPE 2002. The triennial event is the Western Hemisphere’s largest equipment show for the quarrying, aggregates, construction and ready-mixed concrete industries. Despite the difficulties in the world economy, ConExpo-Con/Agg 2002 attracted more than 2,450 exhibitors from throughout the world. This was an increase from the 1,882 exhibitors that were on hand for the 1999show. And this year’s show occupied more than 180,225m2 (1.94 million sq ft) of exhibit space at the Las Vegas Convention Center. This was an increase from the160,900 m2 (1.73-million sq ft) occupied at the 1999event.
