Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

The largest surface coal mine in North America requires complex materials handling infrastructure to fulfil the highly variable quality specifications of its contracts. These diverse demands, reflected in the arrival of numerous customer trains every day, necessitate the integration of information in every stage of production. To meet this goal, quality measurement, train line-up and mine planning processes are being integrated into local and remote control rooms. The effectiveness of such a system can only be measured by comparing decisions that are made with, and without, each of the individual integrated control systems. Traditionally, mine controllers use their experience, intuition or a variety of trial-and-error spreadsheets to determine the ideal materials handling flows needed to achieve the correct quality. These balanced decisions are complex since many factors must be considered such as the mine plan, fleet costs, quality variability and others; consequently mine controllers usually have widely varying success between crews.An evaluation model was developed to assess the impact of a decision support system (DSS) integrated in a control room for controlling the coal blending process. To this end, an ARENA-based simulator representing the whole production process can be used to pull performance and current materials handling settings directly either from the real-time systems in the mine, or from historical data. In this way, the same shift (breakdowns, cycle rates, etc) can be simulated and performance compared. Different settings of the DSS, in terms of what information is displayed and the methods for doing so, can be tested for effectiveness. The historical data are extracted directly from a corporate data warehouse, without altering the everyday working databases. At the same time, the simulator writes its results back into the same data warehouse structure, allowing the same KPI reports to be used to measure simulated shifts.

Productivity and fuel consumption of the blast furnace is driven by the quality of the iron ore sinter, which in turn is related to the sinter mineralogy. The sintering conditions including temperature and gas composition have a significant influence on the formation of melt and finally the mineralogy of the sinter. In this work, analogue sinter tablets were produced from iron ore fines in a laboratory furnace with tightly controlled temperature profile and oxygen partial pressure in the gas. The mineralogy of the sinters was analysed using reflected light microscopy. The results showed that increasing thermal enclosed area (area enclosed by heating and cooling curve above a reference temperature) reduces primary hematite and platy silico-ferrite of calcium and aluminium (SFCA) contents. This is attributed to increasing melt formation, leading to greater assimilation of the iron ore particles. Lower magnetite was found for tablets with greater median pore size, which is thought to be due to greater oxidation of magnetite to hematite during cooling in air. It was also found that higher alumina content was found to result in more SFCA formation under the same heating/cooling conditions, whereas tablets with higher goethite had higher assimilation and produced more secondary hematite on cooling. Finally, it is necessary to relate the enclosed area to the temperature at which the melt formation becomes significant which strongly depends on chemical composition of the ore studied.

Electronic detonators bring a fundamental and radical change to the potential of rockbreaking operations. The immense technical challenge of safely merging electronic systems with explosives has been largely overcome, but a surprisingly difficult obstacle has been to convert users to the much more capable electronic systems, in routine as opposed to exotic applications. Unit cost is probably the main point of resistance. For years papers have confirmed significant improvements in blasting results, feeding through to enhanced bottom line value for the whole operation: much reduced environmental impact, and improved control over fragmentation, muckpile shape and overbreak. In Africa, a number of operations have been employing the technology for sufficient time to see enduring improvements in throughput, control and cost structure, and these improvements have fed through to the planning process: both the cost and the benefits are part of the plan. This has essentially rendered very unattractive the option of reverting to pyrotechnic systems and incurring the penalties previously unseen but now understood to be avoidable. Examples given are of improved throughput at opencast and underground mines, and greatly extended life at an urban quarry. The general experience of management in implementing the technology is that it over-delivers: adopted to solve one particular problem, significant other issues are also addressed. These extra benefits were quite logical, but not at all obvious prior to implementation. Exploring this reality is essential for harvesting the benefits of the technology sooner rather than later.

Excavation engineering involves complex processes for primary and secondary extraction of materials. The shovel excavators bear varying mechanical energy input and stress loading across the working bench during excavation. These varying mechanical energy input and stress loading effects cause significant wear and tear and fatigue failure in equipment components. These effects further result in unplanned downtimes, reduced efficiency and high production costs. In addition, the mechanical and electrical control systems must be operated in efficient profiles to generate maximum outputs and efficiency from these excavators. Efficient technologies are therefore required to solve these problems in order to reduce production costs and increase efficiency. In this paper, the authors develop a rigorous simulator for a hydraulic shovel in a surface mining environment. The finite element model is used to capture the stress profile within the shovel components during excavation, such as, the bucket, stick and boom assemblies. The profiles are classified into normal, critical and extreme ranges, with appropriate corrective measures to avoid extreme loading conditions. The loading capability of shovel excavators can be examined by assessing its structural strength against all possible maximum loads working on the equipment. The dynamic model is used to define efficient profiles of the critical structural components of hydraulic shovels during digging operation. This assessment is used to locate the maximum possible loads and the critical sections of the structures. The method presented in this paper forms the basis for developing comprehensive simulator models for efficient shovel operations in constrained mining environments.

A comprehensive understanding of the rocks associated with coal seams, and also of the coal itself, is essential in assessing the likely behaviour of the materials before, during and after the mining process. The nature of the rock materials involved in coal mining can give rise to risks from adverse geomechanical properties, create problems in coal quality control and produce difficulties with environmental impacts, all of which may affect the profitability, impact and safety of the operation. Examples of issues associated with rock composition include slaking and degradation leading to roof, floor, pit wall or spoil pile instability, ignition of methane in mine atmospheres by frictional processes, liberation of quartz-rich dusts into ventilation systems, production of coal with unfavourable preparation or marketing characteristics, and escape of suspended solids or potentially toxic leachates from spoil piles and other types of emplacements. Methods for evaluating rock composition include microscopic examination, chemical analysis and X-ray diffraction, with the latter being especially useful for fine-grained rocks or for clay-bearing materials. Other techniques, such as geomechanical testing and down-hole geophysical logging, are also useful, but give less direct information about the actual composition of the materials involved. Indeed, understanding of rock composition, through microscope or quantitative X-ray diffraction studies, provides a basis for checking information from other sources, explaining different aspects of rock behaviour, and an essential link in relating geophysical or geotechnical data to broader-based geological models as part of the risk assessment process.

The Hydrogeological Survay of the bauxite mines in Hungary was establisned 30 years ago. The goal of the HS has been to collect the all available date needed to solve the hydrogeological problems of bauxite mines. The results of activity of the HS have been summarized in the annual reports-. This report is going to present a few methods to evaluate the hydrogeological properties of fissured, karstic rock ie. the evaluation of drilling mud losses, the statistics of core samples evaluated by a special computer program for calculation the porosity and permeability of the rocks. An experimental injection test is presented together with the development of a new portable instrument to get signes of the water table change during the injection test in every two second. It can be proved that the flow around the well may be a transient one if the yield of the injection exceeds a definite one (Figure 2.). It was observed that the permeability calculated from the pumping test date may be 1.2-1.6 time higher than it coming from the injection test. A practical example is presented also how to use the results of the HS for designing the dewatering of a new mine together with an analogous computer model to forecast the yield expectable for making the drift tunnel. A suggestion based on the hydrogeological feature has been done for the dewatering without using electric energy (see Figure 3.-6.).

The Q-slope method for rock slope engineering was developed to enable engineering geologists and rock engineers to rapidly assess the stability of excavated rock slopes in the field and make optimal adjustments to slope angles as rock mass conditions become apparent during construction of road cuts or benches. An empirical relationship between Q-slope and long-term stable slope angle is now supported through over 500 cases studies from Asia, Australia, the Americas and Europe. This paper presents about the stability analysis by first-ever applying Q-slope method on abandoned quarry wall in Singapore. The 30 m high rock slope which is composed of monzogranite rock was severely damaged by blasting operation. As the rock slope is intended for public view the objective was to retain the natural outlook and without any artificial support application thus manual scaling was carried out to clear the loosed rock blocks. In the end stable slope angle of clean rock face is determined once again by Q-slope method.

Gold was discovered at Waihi in 1878. In 1987 the modern Martha Mine opened in the middle of the town of Waihi. Newmont Waihi Gold as the current licence holder has been operating the mine since 2000. The current open pit was scheduled to stop production in mid 2006. An underground mine is currently being developed to the east of the town.   In 2001 serious subsidence issues from historical underground mine workings outside the mining licence and the forecast closure of the open pit acted as a catalyst for the development of new communication initiatives.   In this context Newmont Waihi Gold and the Hauraki District Council initiated a community consultation process with the people of Waihi. The Waihi Community Consultation Committee (WCCC) was established in May 2003. After an initial brief to act as a forum for community participation in closure and rehabilitation of the Martha Mine, the group quickly took on a much more proactive role working towards the long-term social, environmental, cultural, economic and spiritual sustainability of Waihi.   In the course of over three years of meetings all groups represented on the committee have presented their views, developed ideas and established an organisational structure to bring those ideas to fruition. The challenge now is to work together to consolidate likely options, seek sources of funding, produce business proposals and implement the plan for the future of Waihi.   This paper backgrounds the experience to date and describes the outcomes of the process. For more detail on process considerations this paper should be read in conjunction with Part C by process facilitators, T Clark and S Stewart. For more detail on progress towards the groupÆs goals and future possibilities, this paper should be read in conjunction with Part B by Vision Waihi Trust CEO, E Morrow.

Management of environmental issues is an integral part of the mine decommissioning process. In Queensland, miners must comply with environmental aspects of the Mineral Resources Act and Contaminated Land legislation. Successful decommissioning involves the integration of a final Plan of Operations, decommissioning report addressing the mineÆs commitments included in its Environmental Management Overview Strategy (EMOS), and Site Contamination Report for relinquishment of leases under the Contaminated Land Act (1991). Niugini MiningÆs Red Dome Mine is proceeding through the decommissioning process. This paper discusses the important environmental management issues including sampling programs, management of culturally significant areas, assessment of background concentrations of target analytes, the use of historical reviews and risk assessment techniques, the timing of works, and the remediation/management of large areas of impacted land. The importance of a clear communications plan between key stakeholders is also discussed.

At the East Sullivan site, wood wastes covering the abandoned mine tailings impoundment prevent sulfide oxidation by creating an anoxic environment. The addition of coarse ligneous wastes and their high permeability favour infiltration, resulting in a water table rise that maintains most tailings under water saturation and thus provides an additional protection against sulfide oxidation. Moreover, high infiltration allows a more rapid flushing of acid prone groundwater generated prior to the cover placement, which could cause acid mine drainage over a long period of time. Finally, the pore-waters under the cover are characterised by a strong reducing potential, high alkalinity, and near neutral pHs. These conditions favour sulfate reduction and base metal precipitation as sulfides and as carbonates. Pore-water composition monitored from 1992 to 1998 showed a significant Fe2+ decrease, following cover placement. Thereafter, the restoration strategy capitalised on the alkaline and reductive properties of the waters underlying the wood-waste cover. An original treatment of acid effluents, based on the recirculation of water discharging around the impoundment through the organic cover, was implemented. In 2002, the recirculated water from May to October had a mean pumping rate of 240 m3/h, for a total volume of 800 000 m3. Data gathered near the dispersal zone from 2000 to 2002 show that despite the recirculation of acid water (pH ÿ3), the pH decreases by only one unit from 7 to 6, during the recirculation period, ie from May to November. However, alkalinity decreases from 800 to 100 mg/L-CaCO3. Despite this decrease, the alkalinity is back up to 800 mg/L-CaCO3 the following May, thanks to sulfate reduction. From 2000 to 2002, Fe2+ concentration in the pore waters near the dispersal zone is maintained below 2 mg/L. Evolution of surface water quality suggests that the contaminated groundwater flush is completed in the north and north-west area of the impoundment; the east sector is under recovery, while the south sector remains the most problematic. A wood-waste cover, besides limiting sulfide oxidation, can fill the role of alkaline reducing barrier for the treatment of these acidogenic waters, until a balance between acidity and alkalinity in the effluent is reached.

Magmatic arcs are prime targets for porphyry- and epithermal-style mineral deposits, although settings within continental arcs and/or tectonically-thickened crust generally tend to be more prospective than intra-oceanic arcs in extensional settings. In this paper, we report new geochemical and geochronological data on igneous rocks of the Western Belt of the Vanuatu intra-oceanic arc. Dating of igneous hornblende (Ar-Ar) and zircon (U-Pb) from the igneous rocks place their formation in the late Eocene to Miocene. In most respects, these are typical arc igneous rocks. However, they contain inherited zircon grains with significant age populations at ca 2.8–2.5 Ga, 2.0–1.8 Ga, 1.75–1.5 Ga, 850–700 Ma, 530–430 Ma and 330–220 Ma. This inheritance signature is unlike anything recognised from the oceanic realm of the south-west Pacific, but, in general, does match the age of the major crustal blocks of the Australian continent. An exception is the significant proportion (~20 per cent) of zircons of Rodinia breakup age (~800 Ma), which previously has not been found in eastern Australia or the south-west Pacific.We propose that part of the Vanuatu arc basement comprises a ribbon of continental material rifted from northern Australia prior to, or during, the Cretaceous. The presence of ancient continental material within the Vanuatu island arc may help reconcile the relatively large thickness and low density of the crust of Vanuatu, and may enhance the prospects for significant porphyry and epithermal ore deposits in the region.CITATION:Spandler, C, Buys, J, Holm, R J and Richards, S W, 2015. The production of synthetic rutile from ilmenite using hydrochloric acid leaching is investigated. Previously published data is used to comment on the optimum process conditions. It is shown that the most important aspect to consider is the maximisation of impurity dissolution and the minimisation of titanium losses.

Silico-ferrite of calcium and aluminium (SFCA) is a bonding phase in the iron ore sinter. The phase transition upon melting of SFCA was investigated with Differential Scanning Calorimetry and Confocal Laser Scanning Microscope. The initial melting of SFCA occurred at 1183.2°C and a massive melting was found at around 1351.1–1358.2°C. A phase transition-melting was investigated at 1476.5–1478.7°C. The liquid phase was composed of immiscible phases. The present results would be utilised to design a new iron ore sinter for better processing performance in a hydrogenenriched ironmaking process.

Reaction products between high-temperature hydrothermal solution/uppermost mantle peridotite or lowermost crustal gabbro have been reported along Wadi Fizh of the northern Oman ophiolite; they are named as ‘mantle diopsidite’ and ‘crustal diopsidite’ respectively (Figure 1).The mantle diopsidite contains Cr-rich minerals like chromite, Cr-rich diopside (<2.5 wt per cent Cr2O3) and Cr-bearing grossular (<7.0 wt per cent Cr2O3). The chromites are varied in shape in the mantle diopsidite, euhedral to anhedral chromite grains contain globular inclusions of grossular and chlorite, and film-like chromite veinlets branch off from partially dissolved coarse chromite grains. In the crustal diopsidite, fine chromite grains are included in oscillatory-zoned skeletal uvarovite, especially in its high-Cr zones. The uvarovites are rich in rare earth elements (REEs). Direct laser ablation – inductively coupled plasma – mass spectrometer (LA-ICP-MS) analysis of the primary fluid inclusions detected Na, but not K.Our detailed studies of Wadi Fizh diopsidites clarify that the high-temperature hydrothermal solution dissolves chromite grains at the uppermost mantle section and precipitates hydrothermal chromite at the lowermost crust section. In addition, REEs were mobilised in the hydrothermal solution together with Cr. The hydrothermal solution involved in the diopsidite formation is assumed to have been rich in NaCl, and could mobilise appreciable amounts of Cr and REE.CITATION:Akizawa, N, Arai, S and Tamura, A, 2015. THIS excellent volumetric method commends itself as a means of estimating the fineness of silver firstly, owing to the large number of possible impurities which do not affect its accuracy (these impurities include Cu-up to 70 % Au, Ph, Zn, Bi, Cd, Fe, Mn, Sb, and As); secondly, the method combines accuracy with speed. The above impurities are the only ones likely to be present in the B. H. A. S. Pty. Ltd. refined silver, and, with the exception of Cu and Au, the others, if present at all, would be well under 0.1 parts per 1000.Briefly stated, the method consists in the solution of the refined silver in HN03 , addition of ferric alum (indicator), and titration with an alkaline (either ammonium or potassium) thiocyanate solution until the brown ferric thiocyanate is formed, and gives a definite tint to the solution. The method is used at the Port Pirie smelters, and the outstanding feature in the method as there applied is the type of burette employed. This burette enables the operator to read 0.01 of a cc. with ease, whereas, with the ordinary burette fitted with a float, 0.05 of a cc., when a number of readings have to be taken, becomes very straining to the eyes. Moreover, waiting for the float to rise, and dislodging air bubbles from it, hinders rapid working.A description of the burette and fittings is appended with fig. 2. Hindustan Zinc Limited (HZL) is one of the leading base metal producers in India. It operates a two-smelter Complex at Visakhapatnam, Andhra Pradesh. At this zinc-lead smelter zinc is manufactured by the conventional hydrometallurgical route and lead is produced by Blast furnace technology generating residues or wastes which contain substantial quantities of metal values. Disposal of such wastes is a problem in any non-ferrous industry. Both the stringent environmental regulations and the changing industrial scenario emphasise the need for industrial research to find economically viable and environmentally friendly technologies to recover the metal values from these waste materials. The in-house R&D Centre at Vizag Zinc Smelter (VZS) thinking globally re-engineered its activities and major thrust was given for converting waste to valuable metals and metal compounds. Basic analytical chemistry with the hydrometallurgical concepts has been adopted in developing these innovative technologies. In this paper attempts are made to provide a clear history of the different waste raw material, the process developed for each raw material and their superiority of over other parallel work. Most of the processes described here are pilot plant tested and under patenting. The process flowsheet, results obtained and advantages of each process are presented. This paper provides a consolidated approach through which the varied smelter waste materials can be processed with cost effective and eco-friendly innovative technologies. In-house R&D efforts at VZS with its contribution and achievement in converting æwastes into wealthÆ technologies are presented in this paper on the following the major thrust areas:recovery of zinc from liquid effluent; recovery of zinc from effluent treatment plant (ETP) gypsum sludge; production of highly pure electrolytic zinc metal powder from zinc bearing gypsum residue/sludge generated at the effluent treatment plant; hydrometallurgical recovery of copper as copper sulphate solution which can be used for CuSO4.5H2O, copper cathode/copper metal powder from copper-cadmium cement; recovery of manganese, lead and silver from anode mud generated at Zinc Cell House and 4-metal extraction/recovery of manganese, cobalt, copper and nickel from Indian ocean bed manganese nodules; recovery of zinc, lead, silver and sulphur values from silver concentrate residue generated at the froth flotation process; recovery of silver and lead as lead-silver sponge/lead-silver sulphide concentrate from lead bearing residues; recovery of lead, silver and copper from copper matte; recovery of antimony as antimony metal or lead-antimony master alloy from antimony bearing dross; and recovery of nickel and dimethylglyoxime from Ni-DMG complex.
