This publication describes an investigation conducted by the Federal Bureau of Mines regarding the cause of an explosion of a dephlegmator or fractionating column at the Cities Service Oil Company Refinery, Ponca City, Okla., January 22, 1959. The column was that used with the refinery's No.1 Dubbs Coker. The explosion caused the death of an employee of the refinery and resulted in extensive damage to the refinery and to nearby property. The Bureau's study of the explosion began January 26.

Assessment of workplace exposures is an important tool for helping to minimize dust-related occupational illness and diseases. Real-time particulate monitoring can effectively locate areas where dust controls are needed and determine how well they are working. An affordable, person-wearable, real-time dust monitor is needed. The SKC Inc. Dust Detective (SKCDD) fulfills those requirements (Disclaimer: Mention of any company or product does not imply endorsement by NIOSH). The SKCDD consists of a disposable sampling tube (DST) connected to a small hand-held sampling pump. The SKCDD was developed through a Cooperative Research and Development Agreement (CRADA) between the National Institute of Occupational Safety and Health (NIOSH) and SKC Inc. The relationship of dust concentrations measured by personal gravimetric samplers to those measured by the SKCDD was determined in a laboratory aerosol chamber. A comparison of the means and the relative standard deviation of triplicate measurements of each type of sampling device demonstrated the SKCDD to be a viable alternative means for measuring dust. While it provides accurate measurements for specific coal types, it requires correction factors (those correction factors still need to be calculated) for other coal types.

Bureau of Mines laboratory experiments with reduced scale cutter and cutter forces were performed to (1) define the fundamental relationships governing disk cutter performance and (2) to develop a method of predicting disk cutter performance in soft, medium, and hard rocks. The rock cutting experiments were performed in four igneous and metamorphic rocks that ranged in compressive strength from 38,000 to 67,000 psi. A special testing machine called a linear cutter apparatus (LCA) was used to load and traverse a free rolling disk cutter across a smooth rock surface. Each pass of the disk produced a single crater, and runs were spaced to avoid breakage between craters. During each run the normal and tangential cutter forces were measured and recorded. The soft and medium rock data were obtained from previous Bureau experiments with disk cutters. Crater depth, tangential cutter force, crater volume, specific energy versus normal cutter force, and crater width versus depth are defined within this study. With the use of standard physical properties, performance prediction equations were developed using stepwise linear regression analysis techniques. A method of using these prediction equations to calculate the boring rate, the cutter spacing, and the torque and energy requirements of a full-scale boring machine was also developed.

Certain roof shales in coal mines deteriorate upon exposure to the atmosphere, causing difficulties in the mining operations and adding to the cost. This report presents the results of both field investigations, in a West Virginia mine, and laboratory studies on the effects of atmospheric moisture on roof shales. Based on these observations, a conceptual procedure is presented on how shale deterioration might be predicted during exploration, and guidelines are given as to how air tempering or conditioning chambers may be designed to reduce moisture effects. Recommendations for further studies are included. An annotated bibliography of the literature constitutes the second volume.

Preliminary safe distances for blasting wiring from typical electromagnetic sources are presented. The distances are for underground coal mines and electromagnetic sources in the frequency range below 10 MHz. The distances are based on measurements performed at 3 locations in each of 3 coal mines. The measurement apparatus, measurement procedure, measurement results and the rationale used in the calculation of the safe distances are all described. The safe distances presented are dependent on the measurement results and their interpretation. More measurements are clearly necessary to establish the validity of the suggested distances.

Blast-induced ground vibrations from surface coal mine, quarry, and construction blasting were measured and analyzed for frequency content and duration characteristics. Eighteen commercially available ground vibration measurement systems were evaluated in the field and laboratory for linearity, accuracy, and crosstalk. Buried, surface, and sandbagged transducer placement methods were compared, along with peak and vector-sum measurements. The recommended minimum frequency ranges for ground vibration instrumentation are 2 to 150 Hz for coal mine and quarry blasting, and 5 to 200 Hz for construction and excavation blasting. When higher or lower frequency vibrations are possible, as in construction blasting or for a quarry shot close to a residence, care should be taken to insure proper choices of instrumentation and vibration criteria. Several instruments operating in these ranges are available, and all but one of the seismographs tested fell within ±3-dB accuracy limits (+41 pct, -29 pct). Waveform recordings of all three ground vibration components are recommended as the peak amplitude and frequency may vary among the three. Peak or vector-sum readings are adequate if only amplitude levels are desired. The soil density matched box should be anchored or buried, particularly for high-frequency, high-amplitude construction blasting.

It is of upmost importance to keep the size of the program to a minimum for the computer being used. One way is to reduce the size of the data bank by adjusting the large common array A in the main program (page, line 32) and the statement following the assignment of A. The current value of A is 35001 and is optimized for the UNIVAC computer with 5,000 words of useable storage. Another way to reduce the required storage is to overlay the program. That is, to have in the computer only the subroutines which are being used. In large finite element codes, overlays or segmentation of the program is a necessity. The code presented here has three levels of segments and are presented graphically in the tree in Figure D-1.

These citations were gathered by U.S. Bureau of Mines personnel over the period of 1978-1983. The citations concern diesel engines as powerplants for underground mining use; their design, emission characteristics and control, maintenance, usage; and health and safety aspects. All citations have been abstracted and arranged 1n a subject classification system with extensive cross-referencing.

Frequency rates of injuries per million man-hours of exposure were improved during 1965 slightly to moderately over those of 1964 at metal mines, nonferrous reduction and refining plants, nonmetal mills, stone quarries and mills, sand and gravel operations, and slag operations. However, at metal mills and nonmetal mines the rates of occurrence of work injuries were less favorable. The severity rates of injuries during 1965 were better than in 1964 at metal mines and mills, stone quarries and mills, and slag operations. Conversely, severity rates were worse at nonferrous reduction and refining plants, nonmetal mines and mills, and sand and gravel operations.

Abstract- Emergency shutdown systems (ESDs) for mining machinery provide critical functions to safeguard miners. Traditionally, ESDs were realized with simple hardwired circuits; today, there is a growing trend to use programmable electronic technology such as programmable logic controllers (PLCs). This paper describes an analytical study to quantify the safety integrity of a PLC-based ESD and a hardwired ESD. The safety integrity level of each design approach was determined by quantifying the average probability of failure on demand (PFDavg) as described by the recommendations for programmable electronic mining systems published by NIOSH and the IEC 61508 international standard. The safety analyses addressed system architecture, hardware failure probability, proof test interval, diagnostic coverage, and human error probability. The results indicated that a same level of safety, safety integrity level 3 (SIL 3), could be attained when evaluating random hardware failures. Neither approach could attain SIL 3 if manual activation was used. Human error was the limiting factor where, using human reliability analysis, PFDavg < 1 x 10-1; thus, the ESD does not meet SIL 1. It is apparent that automatic verses human¬activation of the ESD is a very important safety consideration. Manually actuated ESDs can only achieve SIL 1 regardless of the technology; therefore, additional independent safety layers of protection are needed to exceed SIL 1. Secondly, it is apparent that the technology choice is very important. The PLC-based ESD was much simpler to design and to validate safety1.

Instrumented handles or dynamometers are widely used to measure hand forces and/or the biodynamic response of hand-arm system. To study hand-transmitted vibration exposure, six generations of instrument handles were constructed or initially developed by researchers in ECTB/HELD/NIOSH. This presentation provided a summary of these handles. Their basic characteristics, limitations, and usefulness are described, which may help their appropriate applications and further improvements. Six Designs of Instrumented Handles Handle 1: The conceptual design is recommended in ISO 10819 (1996)1 for glove test. The grip force is measured by detecting bending strains on a measuring beam in the handle. A special handle fixture was designed to connect the handle to a shaker. Except the screws, the handle and fixture were made from aluminum. Handle 2: The design is based on the principle of shear strain measurement.2-3 Both grip and push forces can be measured simultaneously using this handle. This handle was directly designed for a simulated vibrating tool. Handle 3: This design is basically composed of a handle base, a measuring cap, and two charge-based sensors (Kistler 9212) sandwiched between the base and cap. The handle was also made from aluminum. The fixture for Handle 1 was also used with this handle. This generation of handle has three different handle diameters (30, 40, and 50mm).4 Handle 4: This design is an improvement from Handle 3. The handle fixture was totally redesigned and it was much stiffer than the previous one. The aluminum measuring cap was replaced with a magnesium cap. Handle 5: The basic structure of this handle is the same as that for Handle 3. However, the piezoelectric sensors were replaced with two strain gage based sensors (Interface SML-50). Handle 6: This handle includes two measuring caps, four piezoelectric sensors, and a handle centre base. The handle fixture was the same as that with Handle 4. Methods for Handle Examinations The static and dynamic characterizations were performed using the methods reported by Dong et al.5-6 Results and Discussion Handle 1: The static force measurement depended on the hand grip location on the handle. Its natural frequency was less than 200 Hz.5,6 Because the transmissibility of gloves may not vary significantly with the applied grip force, this handle may be acceptable for glove test. However, the force measurements with this handle may not be reliable. Handle 2: The static force measured with this handle was insensitive to the hand acting location. However, when the handle was vibrating, the force signals could be totally distorted. For this reason, it was not used for vibration studies.

An experimental hydraulic jetting device intended to operate in a 16-in-diam borehole was designed, fabricated, and used to cut coal and simulated coal samples. This device consisted of two 0.107-in-diam nozzles that rotated in a plane perpendicuIar to a 50-inch length of high-pressure tubing. Experi¬ments were also conducted with stationary jets issuing from a single 0.142-in-diam nozzle. Numerous experiments were performed using systematic variations of jet parameters coupled with assessments of cutting rates. These Bureau of Mines tests showed that when operated at 4,000 psi pressure and a 40-gpm flow rate (1) the device can fragment coal up to a distance of 8 feet from the nozzle, (2) 4,000-psi jets cut more effectively than lower pressure jets with the same nozzle diameter, (3) stationary jets do not cut as effectively as moving jets, (4) interkerf breaking occurs when the kerf spacing is less than 2 inches, and (5) if the kerfs are spaced <1/2 inch, the jet is deflected and thereby reduces cutting effectiveness.

This report presents a study of the Cooper sand of the Fork Run pool of Warren and McKean Counties, Pa., and a prediction of the secondary oil recovery by gasflooding the reservoir. A computer program employing a modification of the Craig, Geffen, and Morse secondary recovery calculation was used to make the gas flood performance forecast and is presented in the appendix. The Cooper sand reservoir in the Fork Run pool lies at a depth of about 1,900 feet and covers an area 1/2 mile wide by 2-1/4 miles long. Original oil in place is estimated to have been 8,266,000 STB. The primary recovery mechanism is solution gas drive, and ultimate primary recovery may reach 20 percent over a well life of 90 years. Two computer predictions were made for gas flooding the Cooper reservoir. The first was for a 25-acre pilot five-spot utilizing existing wells, but project economics were poor since only 25 percent of injection gas is utilized for pilot oil recovery. Recovery was 71,000 STB, or 22 percent of oil in place at flood initiation, but required 25,700 scf gas injected per STB of oil recovered. In the second prediction, pilot area performance was projected for the entire Fork Run pool, and economics became good. Recovery was 1,753,000 STB from 25 five-spots after the injection of only 6,400 scf per STB of oil recovered.

"INTRODUCTION During the recent war attention was directed to prevent the destruction of American industries by sabotage. By Executive order of Lay 19, 1942, the Bureau of Mines was required to conduct investigations and make recommendations that would prevent loss of production of essential war minerals. The Health and Safety Branch of the Bureau of Lines was able to extend its activities with minimum loss of time to include security surveys of the important mineral facilities in the United States.A Mineral Production Security Division was established in the Health and Safety Branch with funds appropriated for that purpose by Congress in April 1942. From July 1942 to July 1945 engineers of this division, under the direction of D. Harrington, Chief, Health and Safety Branch, and J. J. Forbes, Chief, Mineral Production and Security Division, conducted investigations of 1,560 metallic and nonmetallic mines and associated facilities in accordance with the requirements of the Executive order.PURPOSEAn analysis of losses in the production of minerals shows that failure or destruction of equipment either from deliberate or accidental causes produces essentially the same result. Maximum production is insured by the prevention of subversive activities, fires, and industrial accidents."

[An economic trading region made up of 22 eastern Washington, northern Idaho, and western Montana counties, with Spokane, Washington as the central place, was defined for this study. modeling system with the, 1985 county database was to create an Input/Output based model of the regional economy. The leading ice, with respect to income, employment, value of exports, and total industry output are shown for each county and the region. The value of total industry output from natural resource based industries is also shown for each county and the region. Interindustry transactions, final demand, and value added components for the region are shown in the Regional Gross Flows Table. The total gross output for the region is valued at $36.7 billion. Total industry output for the region is $18.1 billion. The region provides $5.4 billion in income to a work force of 286,000. Spokane County, Washington accounts for about 50 percent of the regional total industry output, income, and employment. Both Spokane County and then as a whole produce about 80 percent of the total industry output for final demand with remaining 20 percent going to intermediate inputs for further production. Spokane County exports about 43 percent of its total output while 35 of the regional output is exported. Exports account for more 50 percent of the total in 18 of the 22 counties within the region. "Methods and costs of rock-dusting bituminous coal mines have been discussed in various publications by the United States Bureau of Mines and others.3 This paper discusses the present methods and cost of dusting at the Swastika mine of the St. Louis, Rocky Mountain and Pacific Co., Raton, N. M.In the Swastika mine the recommendations of the United States Bureau of Mines with regard to rock-dusting are followed at least as well as in any mine in the States of Wyoming, Colorado, and New Mexico. This mine is located at Swastika, N. M., approximately 4 miles west of the town of Raton and is in the Raton coal seam. The mine is rated as gassy and produces approximately 1,000 tons of coal per day.A mining system known as the ""room-and-pillar with cut-offs"" is used which results in recovery of nearly 100 per cent of the coal. The mine employs approximately 250 men.Ventilation is maintained by a Sirocco fan so arranged that the air current may be reversed and exhausting approximately 130,000 cubic feet of air per minute at 3 inches water gauge. This fan is supplemented by two auxiliary fans for emergency use. Three main entries penetrate the hill in a straight line for approximately 8,000 feet. The haulage is through the center entry, which is also used as the intake in conjunction with a two-compartment concrete-lined shaft located at the end of the entries. The main return for 3,000 feet inby the fan is 12 feet wide and 7 feet high. For this distance the ribs have been reinforced by a wall made of coke-breeze blocks plastered over with cement. Solid concrete blocks are used for the top row of blocks of the wall. Steel 10-inch I-beams spaced 40 inches apart center to center are supported on these solid blocks and thus support the roof. The next 1,000 feet inby has the same area, but the cement blocks extend to the roof, as no crosspieces are needed. The airways are kept clean and well rock-dusted.A rather unique system is used to handle gas feeders along the intake and haulage entries. There these feeders occur in the roof or where gas is apt to collect in high points along the haulage, tin funnels as large as 3 feet in diameter are placed near the roof. From these a 2-inch or other suitable-sized pipe is run into the return through the wall of the nearest break-through. A valve on this pipe line controls the amount of air pulled. These devices keep the high places clear of any gas, even when strong feeders are encountered.Fifteen-ton electric trolley locomotives are used on the main haulage and approximately 30 roller-bearing oars, each having a capacity of approximately 3,000 pounds of coal, compose a trip. All loaded tripe carry steel drags on the last car, and automatic derails are located at advantageous points throughout the mine.All loaded trips are thoroughly sprinkled on the sectional partings and at the main parting. The track is sprinkled every idle day, or if needed, every day; from a large tank mounted on a truck hauled by a motor. Neither nozzle nor spray is used, as these were unsatisfactory. The water leaves the tank. through a 11/2-inch valve and is permitted to flow on the road through a horizontal 11/2-inch T. Water is obtained from filling stations located throughout the mine. It is stated by the officials that the use of this method of sprinkling permits the entire haulage roadbed to be wetted down in two hours."

Under Bureau of Mines Contract H0122007, Telcom, Inc., with the assistance of Fenix & Scisson, Inc., conducted a 17-month research and development project aimed at provid¬ing a field-test model of a guidance system for use with the Bureau of Mines Probe Drill. The Probe Drill is a part of the Bureau&apos;s Mine Rescue System developed under Contract H0101262. Volume I of this report is subdivided into seven sections. Volume II consists of a number of supporting appendices. In addition, there were delivered under the Contract, a number of other documents which are referenced in the text. Section 1 deals with essential contract information, Program Background, and a general system description. Section 2 covers the project chronology in brief. Section 3 is a digest of design considerations and implementation related to the requirements of the Contract. Section 4 is a description of the System, including illustrations of the hardware, and software. Section 5 is devoted to Operation and Maintenance. Para. 5.1 describes step-by-step field assembly and disassembly of the downhole unit. Para. 5.2 covers Operation. Para. 5.3 covers various aspects of maintenance from the lowest field echelon possible. Higher level maintenance must be accomplished in the laboratory, for the present field-test model. Para. 5.4 covers system calibration. Para. 5.5 covers detailed step-by-step operational sequences. Section 6 covers the results of the field testing, post-test evaluation, and recommendations. Essentially, we conclude that the present hardware (field-test model) has proven every aspect of system performance, but failed to provide reliable assurance of an operationally ready system. Telcom, therefore, recommends a three-phase continuing effort devised to provide the operationally-ready model of the system.

Development of automatic fire protection systems for large, mobile mine haulage equipment was initiated under Department of Interior&apos;s Bureau of Mines-FMC Corp. contract H0122053, "Improved Sensors and Fire Control Systems for Mining Equipment June 1972 to May 1973." After successfully designing and mock-up testing an effective economical automatic fire protection system, and after having demonstrated that the system could extinguish controlled fires on 100-ton capacity haulage trucks, USBM-FMC contract 110232059 was issued in June, 1973, to conduct further development and validation testing. This report documents technical progress and activity under the latter contract. Figure 1 shows areas of the two haulage trucks that were protected by the automatic fire protection systems tested during this program. Figures 2 and 3 are composites showing hardware and schematic diagrams of the systems tested.

The U.S. Bureau of Mines and the U.S. Geological Survey are responsible for the evaluation of the mineral resources and potential of further planning areas included in the U.S. Forest Service Roadless Area Review and Evaluation (RARE II) program. The Bureau of Mines primarily evaluates mineral deposits and prospects within the areas. The Geological Survey naps the area&apos;s geology and conducts geochemical sampling. Evaluations are in conformance with the Forest and Rangeland Renewable Resources Planning Act of 1974 as amended by the National Forest Management Act of 1976. The Hauser RARE II area, which consists of 7,600 acres (3,100 ha) within the Cleveland National Forest, is in southern San Diego County, California. The area, which forms the north slope of Hauser Canyon, ranges between 1,600 and 3,919 ft (490 m) in elevation. It is sparsely to densely covered with desert shrubs. The area is underlain by granitic rocks of the Peninsular Ranges batholith.

The problems associated with the presence of organic materials and of water in explosion-proof enclosures used in high voltage, high capacity applications were examined through a literature search and theoretical reasoning. Information on volatilities is presented. The conclusion is reached that explosive pressures are significantly enhanced by inclusion of these materials.
