The Australian mining industry is a significant contributor to the nation’s greenhouse gas (GHG) emissions, and like any industry, it must make efforts to reduce these. Medium- and long-term strategies will include the replacement of diesel engines with alternative energy sources, but what can be done in the short-term to reduce diesel consumption and hence CO2 emissions? Haul trucks are identified as the major contributors of CO2 emissions in the mining fleet and this paper focuses on firstly identifying what drives haul truck emissions and secondly, discussing what is the role of the Fleet Management System (FMS) in reducing these. A key enabler of this is having a robust fuel consumption model within the FMS, and I describe how this may be done. Besides novel approaches to incorporating fuel consumption (CO2) management into truck assignment algorithms, this paper also describes how the FMS can be used to implement and manage procedures and policies to reduce haul truck emissions.

The record for autonomous haulage systems in underground mining over the past 15 years is patchy, marked by many trials and attempts, with few examples of sustained operation until relatively recently. The drivers for the introduction of this technology are variously listed as the occupational health and safety of people working in underground mines, reductions in direct operating costs and increases in productivity.This paper reviews the trends for operation of semi-autonomous and autonomous haulage achieved by underground mines and the defining features of the technology currently available. The inclusion of autonomous haulage as part of mining studies and the fundamental conditions for its successful application is discussed in the paper. The components required for an autonomous underground haulage system are outlined and the key aspects that need to support its effective implementation are examined together with their associated opportunities and risks. Special note is made of the critical role people, including executive leadership, have in ensuring the successful introduction of this technology.The paper draws on published case study material, equipment supplier information and research findings. The Century zinc-lead-silver deposit has a geological resource of 118 million tonnes averaging 10.2% zinc, 1.5% lead and 36 g/t silver. It lies approximately 250 krn NNW of Mt Isa in NW Queensland, Australia, and is hosted by middle Proterozoic age siltstones and shales of the Lawn Hill Formation. Within the orebody, most mineralisation occurs selectively within black shale units as fine grained sphalerite and galena lamellae with minor pyrite and siderite. The black shale units are separated by weakly mineralised sideritic siltstone horizons. The deposit is unmetamorphosed and only weakly deformed, displaying excellent lateral stratigraphic and grade continuity. The deposit is mostly overlain by younger Proterozoic sediments and later Cambrian age limestone cover and only a small proportion of the mineralisation crops out. Mineralisation has a subtle surface expression without a classic gossan profile, probably due to the low iron sulphide content of the ore. These factors contributed materially to the length of time between the first recognition of lead-zinc-silver mineralisation in the area by prospectors in the 1880s, to the discovery of the orebody by CRA Exploration Pty Ltd (CRAE) in April 1990.   Small scale prospecting and mining continued in the area from the date of the first mining lease in 1887. Post 1950, the deposit area was also included within nine separate regional exploration titles prior to CRAE's title acquisition in 1987. CRAE's original exploration strategy was partly pragmatically and partly conceptually inspired, but the eventual discovery only partly conformed to the original conceptual model. The deposit was found higher in the stratigraphy than the original target position, with unexpectedly low total sulphide content relative to other known deposits.   The discovery soil anomaly was located by the CRAE's first regional orientation traverse line in the area, but was not drill tested for three years. The discovery sequence at Century has strong elements of persistence, technical competence and some luck. It demonstrates that even in well explored terrains (by conventional standards) the exploration story can never be said to be over. The foreshadowed development of an economic resource at Century will hopefully inspire continued commitment to 'brownfields' exploration by mining companies, and demonstrate to external regulators the importance of genuine multiple land use strategies and the need for continued access to land for exploration and mining purposes.

Outotec has performed numerous research and development projects and various surveys in the study of reverse iron ore flotation. Over the past four years their TankCell-70 and TankCell-160 flotation cells at the Germano concentrator of Samarco in Brazil were extensively surveyed. These surveys have revealed some extreme operating conditions in reverse iron ore flotation and are the subject of this paper. Some of the findings include large variations in the content of solids in the various flotation stages. At the bottom of the cleaner cells, the content of solids by weight reached over 60 per cent, whereas at the bottom of the scavenger cell this was less than 50 per cent. Froth carry rates were also significantly different in the various flotation stages. In the case of coarse material, the scavenger cells operated at a froth carry rate of over 40 t/h.m2. With fine material, however, froth carry rates of less than 10 t/h.m2 were measured. High froth carry rates caused significant changes in the residence times at Samarco. In the rougher cells, the fifth cell had more than double the residence time of the first cell. This was even more pronounced with the scavenger cells, where the second cell had more than triple the residence time of the first cell. High froth carry rates also caused losses in iron recovery. As a consequence of such findings, it appears that different rules apply in the sizing of reverse flotation circuits compared to direct flotation circuits. Different operating parameters (mixing intensities and air flow rates) must also be used in the rougher, cleaner and scavenger cells in order to optimise the metallurgical performance. The transition from exploration to feasibility is a stage in the development of mineral resource projects that is frequently not handled well. The transition marks a point in the project development process where resource evaluation priorities change significantly from the discovery of mineralisation and the rapid assessment of the potential tenor of the mineralised system, to the detailed assessment of the deposit.   Information used in the exploration process tends to be of a qualitative or theoretical nature initially, but gradually becomes more quantitative in character as geological knowledge relating to individual prospects grows.   The transition from discovery and rapid exploration of a new deposit occurs at a relatively early stage in project development, coinciding with the first attempt to prepare a resource estimate for the deposit. Fundamental assumptions regarding both structural and grade continuity and uncertainty inherent in existing information need to be made so that they themselves may be addressed during the collection of additional data.   The effective commencement of feasibility studies involve the introduction of new accountabilities either as a result of appointment of a project manager, and through needing to be able to quantify: ò     the quality of data being collected for resource evaluation purposes; ò     confidence associated with resource estimates; and ò     the need to develop an audit trail for the project, required to make the resource evaluation decision-making process as transparent as possible.   Quality assurance is a concept to which exploration geologists have frequently had little exposure. Being able to demonstrate the quality of data used in feasibility studies is, however, a crucial component of the overall risk management and reduction.   Development of a transparent audit trail for data assists in demonstrating the quality of data which is frequently perceived to translate directly to confidence inherent in resource estimates.   Establishing procedures for geological data collection and management also provides a more reliable framework for making decisions that impact on project execution, again resulting in improved confidence in resource estimates and associated geoscientific data.   Regular, independent technical audits, in principle similar to audits conducted for the ongoing certification of standards compliant quality assurance systems, are a major, positive factor in effective project management and execution.   Information used in project evaluation, feasibility and development must be based on facts. Facts are effectively observations that are ideally made systematically, recorded in a systematic manner, and by being collected effectively are not subject to change.   Effectively managing the transition from exploration to resource evaluation, feasibility and project development involves recognising the nature of changes in the nature, detail and scope of geoscientific data required at each stage of project development. In doing so it is essential to: ò     recognise when the change occurs (as early as preparation of the initial resource estimate for a project); ò     accept that the change may not be gradual; and, ò     ensure that the project evaluation team is adequately resourced.   These objectives require recognition of the value of previous experience and the need to instill a true team approach amongst both staff and contractors associated with the project. Regular formal and informal communications between staff, contractors and other stakeholders in the project are an essential tool in ensuring personnel are aware of factors affecting their role in the project.

The Isa Mine of Mount Isa Mines Limited produces 2.6 million tonnes of silver-lead-zinc ore annually from sub level open stopes of which approximately 40% comes from the narrow Racecourse orebodies below 15 level.Geologically the stoping environment is complicated by highly fissile shale hangingwalls and below 17 level by a high stress regime.The realization that major hangingwall failures and generally poor ground conditions could lead to serious dilution problems, and ultimately production shortfalls, prompted investigations into the use of low strength explosives.

The Eleonore property (James Bay district of Northern QuTbec), owned by Goldcorp, hosts gold within stockworks of quartz-tourmaline-arsenopyrite veins and veinlets contained within microcline (potassic alteration) and brown tourmaline replacement zones. The preliminary investigation to recover gold from drill core samples showed that flotation followed by ultra-fine grinding and cyanidation is the preferred option. Ultra-fine grinding is used to liberate gold finely disseminated in sulfides. The flotation concentrate used in the investigation had a P80 of 11 ¦ and contained 75.5 g/t Au, 5.0 g/t Ag, 10.3 per cent S, 0.07 per cent Cu and traces of antimony (0.03 per cent). The mineralogical characterisation showed the presence of 65 per cent gangue minerals, 23 per cent pyrrhotite (hexagonal), 9.6 per cent arsenopyrite, 2.2 per cent pyrite and 0.08 per cent chalcopyrite with trace amounts of galena (0.05 per cent) and sphalerite (0.03 per cent). Gold was in the form of native gold and electrum. A straight cyanidation conducted at pH 11.0, DO of 3 - 5 ppm, 2000 ppm NaCN, 20¦C and for 72 hours extracted 97.0 per cent Au and consumed 31.5 kg/t NaCN. The equilibrium was reached within 24 hours. Increasing the dissolved oxygen to 16 ppm reduced the consumption of cyanide to 22.0 kg/t with 96.7 per cent Au extraction. A 16-hour pretreatment with 2.0 kg/t lead nitrate, followed by a 24-hour cyanidation reduced the cyanide consumption to 6.0 kg/t with a gold extraction of 97.5 per cent. Increasing the lead nitrate addition to 6.0 kg/t reduced the cyanide consumption to 1.2 kg/t by passivating the surface of pyrrhotite. The gold extraction remained at 97.0 per cent. It was possible to decrease the concentration of cyanide to 800 ppm NaCN without compromising gold extraction. The associated cyanide consumption was 1.2 kg/t NaCN. The passivation of gold grains was an issue when the cyanide concentration was decreased to 550 ppm NaCN. The gold extraction decreased to 92.2 per cent. The average lime addition for all the tests was 7.4 kg/t and the average silver extraction was 80 per cent. Efficient cyanidation of an ultra-fine sulfide concentrate with only 800 ppm NaCN represents an advance in leaching of ultra-fine sulfide concentrates. The high addition of lead nitrate would certainly make cyanide destruction easier; consequently reducing the effluent treatment costs and making possible to meet the discharge regulations criteria.CITATION:DeschOnes, G and Fulton, M, 2013. The Rosedale Outfall Project utilised an earth pressure balance (EPB) tunnel boring machine (TBM) for only the second time in Auckland. Although the tunnel was excavated fully through East Coast Bays Formation (ECBF), the downhill gradient along with some unexpected areas of extreme water posed some significant challenges during construction.The Rosedale Waste Water Treatment Plant Outfall Tunnel project was commissioned in response to the growing needs of North Shore City, New Zealand. The outfall tunnel comprises a 3 km long segmentally lined tunnel and a 2.1 km buried marine pipeline, and discharges 2.7 km offshore. The capacity of the new outfall is 6 m3/s compared to the existing outfall capacity of 1 m3/s.Following previous project experience, and with the tunnel located along its full length in the bedrock weak sandstone/siltstone bedrock of the East Coast Bays Formation, tunnelling conditions were expected to be benign. However during construction six high inflow events were encountered, including a full face (3.4 m diameter) of Albany Conglomerate over a 48 m length of tunnel. This material comprised cobbles in an uncemented sand matrix, and inflows were recorded at over 25 L/s.This paper describes the design challenges on the project including the design of a steel fibre reinforced segmental lining for internal pressures. The specification of the machine is discussed including the modifications required to enhance the risk mitigation measures already in place. The ground conditions encountered are discussed and have improved the knowledge of the range of conditions that can be expected in Auckland.Despite the challenges presented during construction, excellent progress rates were achieved and the tunnelling was successfully completed ahead of program.

The Fimiston Open Pit (commonly referred to as the æSuper PitÆ) is a part of the Kalgoorlie Consolidated Gold Mines Pty Ltd (KCGM) operation in Kalgoorlie-Boulder. KCGM manages the Fimiston Open Pit, the Mt Charlotte underground mine, the Fimiston Mill and the Gidji Roaster for its Joint Venture Owners, Newmont and Barrick Australia. KCGM produces up to 850 000 ounces of gold per year. The KCGM operations are located on the historical Golden Mile on the outskirts of the City of Kalgoorlie-Boulder, about 600 kilometres east of Perth Western Australia. This traditionally underground mining centre has been worked since 1893, yielding in excess of 50 Moz of gold from ore with an average head grade of 10 g/t. This represents approximately ten per cent of the gold produced in Australia and one per cent of the gold produced in the world. The Fimiston Open Pit is essentially an aggregation of what were known as the Judd, South, Paringa, Croesus/Eclipse, Central, Brownhill, Drysdale, Morrison, North and Horseshoe pits into one continuous large-scale operation. The old Fimiston Underground Operations (included the Main, Croesus, Chaffers, Lake View, and Perseverance shafts) were also located within the area of the Fimiston Open Pit. The underground operations closed in 1992 due to high mining costs.

The Gladstone Hill area hosts epithermal veins similar to those at Martha Hill, which is less than one kilometre away. There are several vein types in the Gladstone Hill area including (1) pyrite-marcasite, (2) quartz, (3) base metal sulfide, and (4) calcite. Pyrite veinlets formed first, whereas the quartz and carbonate veins formed later. The base metal sulfide vein occurs at depth and is predominantly comprised of quartz and chlorite intimately intergrown with pyrite, sphalerite, galena, and rare chalcopyrite. Platy calcite and massive calcite veinlets were the last vein types to form. Several vein textures occur in the Gladstone Hill area, including colloform and crustiform banding, lattice-bladed, zonal, comb, massive and feathery textures. Crustiform banded veins are the dominant texture and contain variable gold-silver grades. Colloform banded veins consist of ubiquitous quartz and minor chalcedony with gold grades as high as 60 g/t. The lattice-bladed texture forms from the deposition of calcite, which has been replaced in many samples by quartz, indicating that boiling and later cooling has taken place. Unreplaced platy calcite only occurs at deeper levels (>300 m). Zonal and comb textures are preserved in quartz and late-stage amethyst veins. Feathery textures within quartz are commonly associated with breccias and some colloform banding, and with significant Au-Ag grades.   The homogenisation temperatures (Th) of primary and secondary inclusions in quartz, amethyst and platy calcite range from 166 to 274¦C, with salinities <1.3 wt% NaCI equiv. The presence of vapor-dominated inclusions and primary fluid inclusions in platy calcite suggests that boiling occurred intermittently throughout deposition. The veins formed under a relatively flat lying paleowater table that occurred 280 - 300 m above sea level. The fluids involved in vein formation were alkali chloride in composition and low in CO2.

Over recent years there has been a significant increase in the number of miners in the Pilbara region of Western Australia targeting channel iron deposits (CID) located in valleys, intersected by ephemeral creeks and below the groundwater table. The falling iron ore price has triggered a drive to reduce cost and maximise yield. Creek diversions provide an opportunity to maximise the utilisation of orebodies and extend the life-of-mine.Creek diversions that are required during mining operations to access ore and provide flood protection have historically been constructed as uniform engineered channels designed primarily for flow conveyance while minimising earthworks costs; however, the design of diversions becomes more complex when they are required to function in a similar way to the existing creek system during operations and remain stable during more extreme events than would typically occur following mine closure, in accordance with current regulatory guidance. The design specifications for closure are often more rigorous than the operational phase, and could incur significant additional cost to implement retrospectively if not planned and designed upfront.The complexity of diversion design depends on site specific factors such as: the size of the creek to be diverted; hydrology; corridor restrictions; geomorphology; hydrogeology; geology; environmental and heritage values; potential impacts and the risk to operations and environment. The complex interactions of these factors must be well understood and hydraulic modelling undertaken to characterise the existing creek system, demonstrate the diversions, provide adequate flood protection during operations, and can function in a similar way to the existing system following mine closure. The flow of information, and consequently the decision-making along the chain of mining from exploration to beneficiation, typically occurs in a discontinuous fashion over long timespans. In addition, due to the uncertain nature of the knowledge about deposits and the inherent spatial distribution of material characteristics, actual production performance often deviates from expectations. Reconciliation exercises to adjust mineral resource and reserve models and planning assumptions are performed with timely lags of weeks, months or even years.The key concept of real-time mining promotes a change in paradigm from discontinuous intermittent process monitoring to a continuous process and quality management system in the resource extraction process. The framework includes a real-time feedback control loop that rapidly links online data acquired during extraction at the mining face, material handling and processing with a sequentially updatable resource model. This will allow near real-time optimisation of decisions related to long-term planning, short-term sequencing and production control.The proposed framework integrates the following building blocks in a holistic manner: automated sensor-based material characterisationonline machine performance measurementsunderground navigation and positioningunderground mining system simulation and optimisation of planning decisionsupdating techniques for resource/reserve models.This paper introduces the real-time mining concept and the building blocks and provides a review of the state-of-the-art. Technological readiness of necessary building blocks is assessed, and critical gaps for further technology development are identified. Although mining companies have made broad corporate-level commitments to adopt sustainability, there continue to be problems when attempts are made to adopt higher-level sustainability aspirations and translate them into appropriate targets and methods at the more-specific operational level. The integration of sustainability principles into minerals processing operational decision-making processes has unique challenges that are not addressed adequately by current tools and methodologies, yet impacts from operational decisions can have longer-term effects realised at closure and rehabilitation.This paper describes a proposed methodology to achieve the integration of sustainability at the operational level through a systematic and rigorous process of identifying and qualifying sustainability issues and opportunities in an operation. The methodology, starting from a sustainability point of view rather than an operational one, guides the identification of process issues and opportunities by examining the operation, unit-by-unit, similar to a Hazard and Operability (HAZOP) process. Each issue or opportunity is qualified according to its measurability, scopes of impact and consequences so that it may be understood fully and the correct engineering problems are formed. Through improved decision-making to account for broader sustainability factors, operations can better address environmental and social expectations and pressures (both corporate and community, as well as immediate and in the longer-term), whilst still delivering on operational and business goals.The methodology has been tested with two case studies at minerals processing operations. At one site, the methodology identified a dust issue which had considerable business risks (loss of valuable product) compared to conventional engineering analysis processes. The other site also benefited from using the methodology as an opportunity to improve the operation of the milling and flotation circuits was identified. The opportunity not only showed a potential to improve operational efficiency, but also potential improvements in water and energy efficiency and a resulting increased resilience against water and energy supply risks. It was important to utilise the holistic approach of the methodology in the identification of opportunities at this site; the goals of the operation showed a strong coupling between water and energy issues which would be difficult to overcome using the operationÆs current organisational model, which deals with such issues separately of each other.CITATION:Tuazon, D, Corder, G D, Powell, M and Ziemski, M, 2012. Australian companies and consultants are becoming ever more involved in projects in African, South American, East European and Middle Eastern countries. However, significant differences in culture and approach to projects sometimes make these projects challenging for reasons that are not purely technical. Often the challenges are rather related to how the project owner and local authorities perceive the reality. These perceptions and sometimes unrealistic expectations regarding project costs and potential outcomes need to be addressed by the æWesternÆ project developer and consultant.This paper will discuss the specific experience of Snowden in studies commissioned by companies from such countries for the development of very large, low-grade gold deposits in some of the most inhospitable areas in the world. The paper will further review similar projects undertaken in other ænew worldÆ countries and compare these to the initial Snowden experience.As the process operating costs of operations in many of these countries are extremely competitive when compared to the typical Australian operating costs, the paper will review the operating costs and the factors which may be pertinent to a large project of this nature, both in the Australian and the ænew worldÆ context. Movement of the fragmented rock mass is an inevitable consequence of open cut blasting. Uncontrolled, unaccounted movement from blasting commonly causes ore loss and dilution from waste mixing. Therefore, the prediction of the post-blast location of both ore and waste is vital in grade control and of direct interest to mining geologists. Direct measurement techniques have matured significantly over the past decade, but their use remains expensive and labour intensive. Access and safety issues are further disincentives to regular movement monitoring. What is needed is a combination of direct measurement to establish baselines and reliable predictive modelling.The W H Bryan Mining and Geology Centre of the University of Queensland has pioneered 3D blast movement modelling using ‘physics engines’ to handle in-flight collisions and predict the final resting place of blasted material. Open cut metalliferous mines are a demanding environment for this style of modelling as they can involve complex geometries and variable buffering, which leads to inconsistent confinement. A version of the model designed to deal with these complexities is under development. A well-controlled field trial was needed to validate this new version.A validation trial was completed at a gold mine in South-East Asia. The aim was to increase throughput by increasing the blast powder factor without losing grade through dilution and mixing with waste. Four blocks were blasted to identify the effect of bench height and face confinement on ore movement, fragmentation and process throughput. The powder factor and blast timing were kept constant. Movement of the broken rock was measured by placing blast movement monitors at known positions in the block and then locating them after the blast. The majority of sensors were recovered, providing a unique insight into blast movement dynamics and generating a substantial data set for model validation.The model (called ‘MetaMove ‘) was calibrated for one blast and applied to investigate the extent of grade loss and possible mitigation strategies. Conventional mining techniques that involved firing across the strike of the orebody, and no post-blast boundary adjustment of the ore/waste boundaries prior to digging, were found to cause up to 44 per cent loss of high-grade ore, depending on the grade distributions. Post-blast adjustment of the ore/waste boundaries can negate this loss. The model indicates that altering timing and ensuring that the block boundaries are well confined can further constrain the movement of ore/waste boundaries. Thus, the model can be successfully applied to minimise grade dilution and limit misclassification of ore as waste whilst using higher powder factors to increase mill throughput. Construction work on one of the world's largest landslide stabilisation projects is drawing to a close in Central Otago. Begun in mid 1990, major stabilisation work has been carried out on seven large rockslides which will be partially inundated when the Clyde dam reservoir (Lake Dunstan) is filled. The work has required the construction of 14.5 kilometres of tunnel, 60 kilometres of surface drilling, 78 kilometres of drilled drainage holes and five million cubic metres of earthworks. The stabilisation programme has been carried out by the Electricity Corporation of New Zealand as part of the Clyde Power Project, a hydroelectric development on the Clutha River.

The history of mining is rightly proud of its achievements in improving productivity and cost improvements over the years. No matter what measure is used, tonnes per manhour, cost per tonne, etc vast improvements have been made over the last hundred years. In fact over the last ten years in Australian coal mines raw coal output per employee has increased by 60 per cent. When one examines why these improvements have taken place the principal reasons have been technological advancement in mining equipment, excavator and dump truck capacity increases, longwall and continuous miners continually improving in reliability and size. Of course all these improvements are correlated to an increase in capital cost. Even with the above-mentioned advances the pressure for increased performance is as strong as ever. The international mining and mineral-processing sector is currently operating in a recessive market with some traditional commodities, eg gold trading at 20 year lows. During recessive times senior management will demand greater returns on capital employed: the obvious choices being reducing costs or increasing throughput. In addition, head office environments will often place a capital freeze on operational management so the opportunity to take advantage of technological change is not available. The challenge is, therefore, to increase throughput and cut costs without the need for capital. The answer is to optimise the existing labour and plant. If done systematically a result in productivity improvement, in our experience, as high as 45 per cent can be achieved with the average across South East Asia and Australasia being 21 - 23 per cent. The intention of this paper is to present a systematic approach to labour and machine utilisation thereby gaining the greatest possible return on the existing capital base.

Utilisation of lower grade (Jigging technology has progressed to a point where SishenÆs 50 to 60 per cent Fe run of mine (ROM) material, once considered as waste, can be beneficiated to produce a saleable 64 per cent Fe product. This has been confirmed through appropriate beneficiation testwork. Consequently, Sishen can now produce saleable product from material not currently included in the resource definition, significantly increasing the resource base of the mine and ensuring higher resource utilisation. Mine planning utilised the geological models to produce detailed planning schedules, reflecting the current and SEP plants separately, optimised to comply with product specifications of both the current and SEP plants and to confirm the viability of the low-grade resources identified as SEP feed material. The SEP product specifications have been derived by optimally matching iron ore market requirements with the revised resource base. A 64.0 per cent Fe lump product and a 63.5 per cent Fe fine product are the optimal SEP products. The SEP plant will be able to produce 10 Mtpa at an average yield of 60 per cent and a lump to fine ratio of 62:38. The plant consists of five major sections: namely crushing, material preparation, beneficiation, final product handling and quality control services. Maximum integration with the current Sishen plant was considered. Kumba currently has an agreement with Transnet to rail up to 23.5 Mtpa through the Sishen Saldanha iron ore export channel (IOEC). Transnet and Sishen Iron Ore Company have signed a Definitive Agreement that deals with the expansion of IOECÆs capacity, which increases KumbaÆs entitlement from 23.5 Mtpa to 35.0 Mtpa. The detailed enviro-legal impact assessments for the SEP indicated that the project has low enviro-legal risk as a result of it being a brownfields project incorporated into the current Sishen operations.

The approach to risk management during the procurement of major projects often has a significant outcome on the ultimate delivery of a project. The NZ$1.4 B Waterview Connection project is the largest and most complex project yet undertaken by the New Zealand Transport Agency (NZTA) and the procurement process for an alliance contract was refined to provide compliance with the International Tunnel Insurance Group (ITIG) Code of Practice for Risk Management of Tunnel Works (TCoP). The principle of shared risk within an alliance incentivises a common understanding of project risks prior to finalising the alliance agreement and therefore enables superior risk management through design, construction and beyond. Risk management practices need to be instigated well in advance of commercial phases, so that commercial competition does not lead to a significant elevation of project risk. In particular the practice of gaining commercial advantage by taking on a potentially unmanageable level of risk is one of the outcomes to be avoided by the TCoP.During the procurement phase the NZTA placed high value on design and construction mitigation measures proposed in order to address ground risks, as is appropriate for a major tunnel project. In order to achieve alignment on the level of geotechnical risks an Alliance Geotechnical Baseline Report (AGBR) process was developed.This paper describes the measures taken during the procurement process to achieve certainty with respect to outcomes and minimise project risks whilst also achieving value for money. During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015.
