This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. A research program to aid development of better and more efficient methods for producing petroleum and increasing its ultimate recovery from underground formations is an integral part of the work of the Bureau of Mines on conservation of the Nation's natural resources. Economic, efficient production of recoverable oil and gas from a reservoir depends to a great extent upon the best and most accurate information that can be obtained from a study of the reservoir rock and its fluid contents. Information on oil and water saturation is pertinent and essential in wildcat, extension, and pool wells for good reservoir operations and prediction of performance. This report presents results of an investigation by the Bureau of Mines of core recovery and degree of drilling-fluid contamination occurring during cable-tool coring operations using oil-base mud. Representative core samples were obtained from 6 major oil-producing sands in 8 fields of West Virginia, Pennsylvania, and New York. Field-mixed, oil-base drilling mud was used for the first coring test, and lightweight, premixed oil-base mud, with an organic chloride tracer added, was used in coring eight other wells. Tracer analyses were made to determine contamination of core by drilling-fluid filtrate. Core analyses, including oil and water content and minimum interstitial water-saturation tests, were made on selected core samples to compare and evaluate water saturations determined by the two methods.

Porous silica was obtained by acid leaching of chrysotile. The porous silica had been characterized by chemical analysis, powder X-ray diffraction (XRD), Fourier transform infrared spectros-copy (FT-IR), and N2 adsorption-desorption techniques. It is shown in this study that the porous silica retains the nanofibrous morphology of the original chrysotile, and produces a high specific surface area (SBET) of 391. 8 m2/g and total pore volume of 1. 05 mL/g. The increase in SBET and the total pore volume of the porous silica reflect the formation of micropores of 1.2~1.5nm and meso-pores of 3.2~3.8nm in diameter inside the porous structure. The hysteresis loop of N2 adsorption-desorption curve indicates mainly slit-shaped pores. The porous silica is amorphous, but its structure is regular to some extent. The structural unit of silica from chrysotile is a SiO4 tetrahedron and six-member silicon-oxygen rings with the tetrahedral positioned alternately up and down in the six-member rings.

Blasters face a long list of tasks to execute daily—from vehicle inspections to blast design and layout, hole loading and tie-in, securing the site, and finally, safely executing the blast. Good practices and, in most cases, regulations and requirements necessitate documenting their work, specifically their blast report. While some blasters use paper forms, others use electronic means to streamline the process. The authors have served as attorneys and expert witnesses in representing companies and individual blasters in numerous administrative and legal matters in which documentation of the blaster’s work was both critical and detrimental to the effectiveness of defending their work. This paper discusses in a case-study manner the authors' observations concerning good blast report format, specific observations around electronic and paper report completion, record-keeping, seismograph setup and recordkeeping, video and photo documentation, internal auditing, and other topics related to documenting the blast. The paper aims to serve as a discussion guide for blasters to improve their blast record-keeping process, develop better practices that effectively document their work, and allow them to defend their work should a claim arise concerning their operations.

IN reviewing mineral industry education a year ago, occasion was taken to congratulate the Institute in general and to felicitate the Education Di- vision in particular on "the most gratifying growth which is taking place in Institute consciousness in regard to its responsibility for the recruitment and adequate training of student personnel and for the successful bridging of the difficult gap between graduation and achievement of professional status." Continuation of this growth of interest throughout 1939 is a recognition of the validity of the splendid statement of George Otis Smith that the mining students of today are the ore reserve of the Institute for the future. In this connection one recalls the equally apt expression of another distinguished geologist, George M. Fowler, who, speaking of exploration for ore deposits, says, "The place to look for elephants is where elephants have been found."

The realignment of Interstate 880 replaces the double-decked (Cypress) viaduct collapsed in Oakland, California during the 1989 Loma Prieta Earthquake. The California Department of Transportation (CALTRANS) evaluated several pile foundation schemes for supporting the proposed structures. Structural support demands led to the selection of steel pipe piles (16-, 24-, and 42-inch-(406-, 610- and 1067-mm-)diameter), filled with concrete and steel reinforcement. Deep penetrations required through layers of very dense/cemented sands in some locations, caused serious drivability and pile integrity concerns. The need for large pile driving hammers and large diameter steel pipe piles pushed the engineers and prospective contractors well beyond the limits of local experience. Such questions as the feasibility of driving the pipes closed-ended through dense upper strata, and quantifying the effects of installation on the long-term capacities of the piles, led CALTRANS to develop a comprehensive pile test program to address the design and construction issues effectively. The results were furnished to prospective bidders on the production stages of the ensuing contracts.

Main ore pass systems should last the life of a mine, and too often expensive and hazardous remedial measures are required to repair a damaged ore pass in an attempt to extend its life. In order to maximize the life expectancy of all ore passes at Moab Khotsong, ?wearing blocks? and ?softer support? systems, combined with basic rock engineering principles, have been implemented. Failure of an ore pass can have severe financial implications, both indirectly because the ore pass cannot deliver the broken rock either from level to level or directly in the shaft ore pass system to the required level for tramming to take place, and directly due to the cost of the repair work that is required. Several factors need to betaken into consideration so that production interruptions are eliminated or minimized. Investigations of damaged ore passes at Moab Khotsong and surrounding mines revealed some interesting mechanisms of damage, and some innovative methods of reducing or eliminating the damage have been implemented at Moab Khotsong. The system design was based on 30 years, experience in main shaft rock passes. The design was to counteract scaling, hang-ups and dust problems associated with main rock passes, and the objective was to increase the life of rockpasses. These problems, including the initial design aspects to try and eliminate the repair measures, are discussed in detail.

A potentiometric model study has been made of the effect of vertical fractures existing in the matrix of the reservoir on the flooding or cycling performance. Fractures can have unusual flow characteristics. Fluid entering one side of a fracture can emerge on either the same or opposite side of the fracture, depending on the particular streamline. Fracture orientation has a great influence on the sweep efficiency. However, sustained fluid injection may still permit large areas to be swept. INTRODUCTION One can find considerable information available in the literature on the effect of fractures originating at the wellbore on sweep efficiencies of water-flooding or gas cycling programs.1-5 However, there arc few, if any, quantitative data reported in the literature on the effect of vertical or horizontal fractures existing out in the reservoir matrix on secondary recovery performance. Several papers have been presented on the effect of vertical and horizontal fractures on the productivity or conductivity of waterflood and gas injection patterns, but in all cases the fractures initiated at the well and simulated commercial fractures.6,7 It is believed that natural fractures may exist out in the reservoir matrix and would effect the displacement performance. The purpose of this report was to study the effect of a few isolated vertical fractures existing out in the reservoir matrix on the performance of waterflood or gas injection patterns. DESCRIPTION AND EQUIPMENT Since the potentiometric model was described by Lee8 it has been widely used to study numerous types of fluid displacement problems. The potentio- metric model can be used for fluid displacement studies when the following assumptions are valid: (a) steady-state conditions exist; (b) the mobilicy ratio is one; and (c) the capillary and gravitational effects can be neglected. Five-spot and direct line drive square patterns were studied using a copper strip of the desired length and orientation to simulate a vertical fracture in the reservoir matrix. The 20 x 20-in. model was considered to represent only one element of an infinite array of similar patterns. DISCUSSION AND RESULTS FIVE-SPOT PATTERN — ONE FRACTURE BETWEEN INJECTION AND PRODUCING WELLS Fig. 1 shows a quadrant of a five-spot pattem with the vertical fracture existing out in the matrix along a line connecting the injection and producing wells. Length of the fracture was equal to 35.4 per cent of the distance between injection and producing wells. In this particular pattern it is noted that fluid breaks through from the injection well to the producing well when the dimensionless time is equivalent to 26.9. Available statistics show that the demands of the American chemical industry for sodium sulphate have been increasing slightly in recent years. The supply from domestic sources, particularly in the form of byproduct salt cake from the acid manufacturers, has been tending to decrease this to the growing manufacture of synthetic acid. Both hydrochloric and nitric acids are being synthesized from their elements. The natural result has been that a smaller amount of salt cake is available. Simultaneously, the expansion of the kraft-paper industry, especially in the South, has brought about greater demand for sodium sulphate in that section. This demand is being met by domestic byproduct salt cake, by imported German sodium sulphate made from potash waste liquors, and by favorably situated domestic deposits of Glauber salts and the anhydrous salt, thenardite. No shortage of natural sodium sulphate exists in the United States, but usually such deposits occur in semiarid regions far removed from transportation facilities and from markets. Moreover, the major portion of the natural deposits is Glauber salt or mirabilite (Na2 504 • 10H20), which must be dried or dehydrated in order to meet consumer requirements. The water of hydration represents, roughly, 55 percent of the total weight of the crude salts and therefore has a very important influence on transportation costs, in many instances the cost of getting even the dehydrated product to market is the deciding economic factor. Therefore, the recovery of production of anhydrous sodium sulphate from plant-waste liquors or from natural brines of suitable composition might be economically feasible.

"IntroductionLaboratory and industrial grinding tests have shown that the process of size reduction of metal containing ores can be significantly influenced by chemicals added to the slurry being ground.,The terms grinding aid or grinding additive refer to a substance which, when mixed into the mill contents, causes an increase in the rate of size reduction. The increased rate can be used to grind a higher feed rate to the desired product size or it can be used to produce a finer product size at a fixed rate. Whether the use of a grinding aid is justified in any given situation depends on the cost of the substance versus the improvement of output or product quality obtained with its use.A wide variety of grinding additive mechanisms have been suggested in the general mining literature (1,2,3,4,5,6,7). The prevention of particle agglomeration, slurry cushioning of grinding media impact, grinding media coating, changing the strength of macro porous rocks, and chemical adsorption on flaw surfaces to change bonding forces and surface energy have all been suggested. More recently, some grinding additive work has been carried out (8,9,10,11) which suggests that the use of efficient chemicals to control the fluidity of dense slurries is a viable economic approach to achieve improved grinding rates. These increased grinding rates are achieved with no increase in power to the grinding mills and apparently with no increase in media consumption costs. Unlike much of the previous work with grinding additives, this recent work is accompanied by a rather complete engineering characterization and prediction of performance capability. The chemicals involved were chosen so as to efficiently control the viscosities of slurries under conditions of high shear yet not adversely affecting flotation, flocculation, and other downstream surface chemistry operations."

Roof warring signals from eleven underground coal mines (principally in the Pittsburgh seam) were studied and analyzed. The character of individual acoustic warning signals is shown to be dependent on spectral distribution of energy. Reverberation tires (pulse decay rates) were found to be usually between 0.1 and 0.5 seconds. Listening tests were conducted to assess the miner's ability to discriminate speech and roof talk signals in noise and in quiet, with and without ear protectors and with various levels of hearing loss. The ability to detect roof talk is shown to be generally degraded while wearing car protection in quiet, although, if car protectors are worn only when required by high noise levels, the ability to detect roof talk will be preserved at its maximum.

In this paper, the surrounding rocks and the supports in a longwall are taken as an integral, i.e. a system. The structural characteristics and behavior of the system are discussed based on in-situ observations. A set of equations, which describe the interaction among the support, immediate roof and main roof, are developed. These equations can be used to design the longwall powered supports more reasonably and economically.

"The Pulp Chemistry Monitor (PCM) has been installed and operated in a number of concentrators in Australasia, and has produced some very interesting results. PCM measures the pH, Eh, dissolved oxygen, temperature and oxygen demand of process streams within a concentrator continuously and in real time.In the example presented in the paper the cleaner feed pulp chemistry is measured, and related back to changes in the mineralogy as well as variations in metallurgical response. For example, a decrease in Eh and dissolved oxygen as well as an increase in the oxygen demand. These changes in the pulp chemistry appear to coincide with increases in the feed grades, probably higher pyrite grades. Further, these variations in pulp chemistry and feed mineralogy have a negative impact on the concentrate grade and recovery.The implication is that if the pulp chemistry is monitored it is possible to predict changes in the mineralogy of the system, and from this the metallurgical response. Further, the changes in pulp chemistry can be used in a control system to counter the changes in mineralogy and maintain the metallurgy.IntroductionIt has long been recognized at a fundamental level that the flotation behaviour of sulphide minerals is dependent on the pH and Eh of the system (for example: Tolun and Kitchener, 1964; Woods, 1976). However, measuring pulp chemical parameters within an operating concentrator has had limited attention. Woodcock and Jones (1970a and b) were among the first to actually collect data from operating plants. Jones and Woodcock (1984) identified some of the chemical complexities of flotation pulps, and noted that most concentrators already operate at high levels of metallurgical performance but there was still considerable scope for improvement. These gains would, in part come from more effective chemical control. In the same volume, Healy (1984) discussed metal ion hydrolysis and precipitation, redox chemistry and how the surface chemistry of sulphide minerals can be influence flotation behaviour. The impact of grinding environment on the pulp chemistry and flotation response of a complex polymetallic sulphide ore is discussed by Forssberg and Subrahmanyam (1993). Their work essentially contrasts the pulp chemical and flotation performance of pulps ground with conventional ferrous grinding media to a fully autogenous system. The data suggested that the reduction or removal of iron hydrolysis contamination from the system could have a positive influence on flotation behaviour, and perhaps the first stage of “controlling” the chemistry of the system should be in the grinding circuit. Grano et al (1995) demonstrated that the collection of pulp, solution and surface chemical data could identify weaknesses within a concentrator, and from these data develop, test and implement solutions that improved the process. Greet et al (2006) showed that the pulp chemical trends observed through primary grinding and rougher flotation circuits of most sulphide flotation concentrators are approximately the same. There are differences in the magnitude of various pulp chemical parameters (pH, Eh, dissolved oxygen, temperature and oxygen demand) as a result of variations in the feed mineralogy and processes employed. Further, it was noted the Eh shifted to more reducing values of the ten days measurements were collected, and this change could be traced to an increase in the iron sulphide content of the feed. Metallurgically, this resulted in an increase in nickel recovery through the recovery of nickeliferous iron sulphides, and a decrease in nickel concentrate grade."

The NASA Lewis Research Center, under U.S. Bureau of Mines program direction, performed an assessment of alternative mobile power sources for mining applications. A wide variety of heat engines and energy systems was initially examined to ascertain which could be considered as potential alternatives to presently used power systems. The present mobile power systems are electrical trailing cable, electrical battery, and diesel - with diesel being largely limited in the United States to noncoal mines. Each candidate power source was evaluated in terms, of the following requirements: (1) Ability to achieve the duty cycle (2) Ability to meet Government regulations (MSHA certification) (3) Availability (production readiness) (4) Market availability (5) Packaging capability An initial screening reduced the list of candidates to the following: (1) Diesel (2) Stirling (3) Gas turbine (4) Rankine (steam) (5) Advanced electric (batteries) (6) Mechanical energy storage (flywheel) (7) Use of hydrogen evolved from metal hydrides This list of candidates was divided into two classes of alternative power sources for mining applications - heat engines and energy storage systems. Anhydrous neodymium trichlorids (NdCl3), containing ~1 weight percent neodymium oxychloride (NdOCI) was prepared by the dehydration of neodymium trichioride hexahydrate (NdC13.6H2O). Dehydration of the hexahydrate was conducted in three steps. First, the hexahydrate was slowly heated in air to about 250 °C until all the low temperature water of crystallization was removed. In the second stop, the neodymium hydroxychloride (Nd(OH)2C1) fanned in the first step was decomposed by heating under argon to 430 ° C. Finally the salt was ground and then treated with HCl gas at 330 °C until practically all of the NdOCl was converted to NdCl3.

Mining and metallurgical processing of gold and base metal ores often results in solid wastes and effluents containing high concentrations of arsenic. In addition, arsenic can be released to the atmosphere from gold roasters and base metal smelters. Speciation of arsenic in roaster products, in a stack sample from a copper smelter, in organic soils impacted by smelter emissions, and in an anaerobic effluent treatment system at a smelter site was determined in order to broaden our understanding of the nature and occurrence of arsenic in a wider range of metallurgical wastes. Micro-XANES spectra obtained from iron oxide particles forming in a gold roaster indicate preferential enrichment of As3+ species in maghemite-rich domains and microlayers. In comparison, haematite-rich iron oxide particles are dominated by As5+ species. It appears that maghemite is retarding oxidation of arsenic and its volatilisation during roasting. Arsenic occurs as both As3+ and As5+ species in a stack sample emitted from a Cu smelter, confined to fine-grained secondary product layers accumulated on the surfaces of spherical Cu particles. This is probably resulting from condensation of As species upon cooling following their volatilisation during the combustion process. Soil samples collected at various distances from the Cu smelter are dominated by As5+ species including monomethylarsonic acid and tetramethylarsonium iodide as the organic arsenic species. The presence of reduced As3+ species highlights the importance of organic material influencing the speciation of arsenic and mineralogical transformations taking place within the soil profile. The XANES spectra indicate that arsenic occurs predominantly as aqueous arsenite species in the anaerobic treatment system, contrary to the conventional thinking of As retention by the formation of secondary sulfides.

A series of laboratory blasting and grinding tests on three types of hard and competent rock (coarse-grained monzonite, fine-grained granite and very fine-grained gabbro) showed that a higher level of applied explosive energy enhanced grindability and produced a larger amount of fine (-0.106 mm) material. This confirmed reported industrial experience and the results of previous investigations. However, the results did not provide any information on the quantitative relationship between the energy applied in primary blasting and that required in subsequent comminution. Only about 1% of energy consumed in conventional grinding is used to create new surfaces, while power efficencies in cone crushers have been estimated at 70-80%. In blasting, up to 20% of explosive energy may be transmitted to the surrounding rock as elastic strain energy, leading to both primary rock breakage and the generation of microcracks

"The raw ores which are processed in mineral processing plants exhibit very strong variations of their physical and chemical properties. As a consequence, the performances of the comminution and separation processes vary greatly from time to time. Model-based control strategies improve the performance of the plant; however, they require some form of adaptation to cope with the variations of the are properties.This paper deals with the use of adaptive techniques for the control of mineral processes. It begins with a survey on adaptive control algorithms including parameters estimation aspects. Then several examples dealing with crushing, rod, ball, autogenous and semi-autogenous grinding, flotation , drying and calcination processes are reviewed.IntroductionThe operation of the various processes of mineral processing plants (crushing, grinding, flotation, dewatering, etc.) is generally controlled using empirically tuned regulators of constant structure, such as PID controllers. To improve the operation performance, mineral processors try to implement specifically designed controllers, i.e. controllers based on plant dynamic models. Most of the time, these techniques improve the control of the mineral processing installations. However, modelling is a complex procedure because of the problems of measurement and data management, specific to this industry, and because of the variations of the model parameters. In this context model-based control strategies do not always bring the expected improvements or are sometimes discarded and replaced by more conventional techniques .The sources of variation of the dynamic behaviour of mineral processing systems can be classified into three categories: (I) the inherent changes of the raw ore properties (composition, grindability, mineralogical texture, etc.); (2) the process tuning modifications (grinding media loads, number of hydrocyclones, slurry level in flotation cells, agitation speeds, etc.); (3) the feedrate variation of the ore, water, chemical reagents, etc. All these factors change the fundamental properties of the process dynamics such as the grinding or flotation rates and the residence time in the equipment. The simplified linear models which are used to describe the process dynamics are not able to track the complex behaviour of the mineral processing systems when the amplitude of the sources of variation are too large. The mismatch between the model and the plant behaviour is then frequently a cause of model-based controller failures."

Talvivaara deposits, Kuusilampi and Kolmisoppi, comprise one of the largest sulfide nickel resources in the world with 642 Mt in Measured and Indicated Resource categories. TalvivaaraÆs preferred extraction technology is bioheapleaching, which is already widely used for other metals, notably copper and gold. The company has demonstrated the viability of bioheapleaching technology for the extraction of nickel using the Talvivaara ore. The leaching process is heat generating and therefore suitable for the subarctic climatic conditions. The planned annual metal production is 33 000 t of nickel, 60 000 t of zinc, 10 000 t of copper and 1200 t of cobalt. Talvivaara deposits are well-suited for open pit mining due to a thin overburden, favourable resource geometry and a low waste to ore ratio. The ore is relatively low-grade, but well suited to bioheapleaching due to its high sulfide content. Bioheapleaching is a process whereby metals are leached from ore as a result of bacterial action. The bacteria used in the Talvivaara bioheapleaching process are endemic to the area and therefore well adjusted to the prevailing environmental conditions. The solution is collected at the bottom of the heaps and either re-circulated through the heap or fed to metal recovery. In metal recovery, nickel, copper, zinc and cobalt are precipitated from the pregnant leach solution and filtered to produce saleable metal products. After the metals are removed, the solution is further purified and returned to irrigate the heaps. Mining, bioheapleaching and metal recovery techniques for the Talvivaara mine have been tested in a 17 000 t on-site demonstration trial during 2005 and 2006. Subsequent construction of the mine started in 2007 and commercial production in the fourth quarter of 2008.
