The Mine Safety and Health Administration (MSHA) defines an independent contractor as "any person, partnership, corporation, subsidiary of a corporation, firm, association or other organization that contracts to perform services or construction at a mine." Contractors that perform specific types of work are required to report the number of employees who perform work at coal mines and at noncoal mines.

NIOSH researchers have been examining underground coal mining activities in order to evaluate work crew hazards. In 1994 a continuous mining machine operator was killed by falling roof during extended cut mining. Many aspects of the incident were used by NIOSH researchers to develop a scenario interview. The goal was to provide a realistic framework for acquiring frank and detailed insights. The interview consists of two sections. The first describes the underground mining conditions. The second recounts the fatal incident. Each section is supplemented by a diagram and a set of questions addressing relevant safety issues. The interview was administered at three mines that actively take extended cuts. Researchers found the scenario approach to be an effective interview tool as well as an effective hazard awareness and safe work practices training platform.

The Bureau of Mines conducted a series of field tests to improve the understanding of the support interaction mechanics between fully grouted bolts and coal mine roofs and to help lay the foundation for improved design and evaluation techniques. Strain gauges were installed on 14 fully grouted bolts placed in shale roof rock at four mines in Colorado, Illinois, and Pennsylvania to determine how load was transferred between the bolts and the rock. The results of field tests on elastic bolt behavior compared well with previous laboratory work and numerical models. The field tests showed that the anchorage length of grouted bolts installed in shale was slightly longer than the anchorage length determined in laboratory tests conducted in concrete blocks. The field results produced more variability because of geological variations. Tests run past the yield point of the steel bolt indicate that the yield zone varies significantly and translates down the length of the bolt anywhere from 4 to 22 in.

Metal mines were the smallest mining sector, comprising 1.8% of all mining operations.1 • Gold mines comprised 44.2% (n = 114) of all metal mining operations. Other common types of metal mines were iron ore (n = 29; 11.2%), copper ore (n = 28; 10.9%), and lead and/or zinc ore (n = 21; 8.1%). • Nevada had the largest number of metal mines (n = 48; 18.6%) followed by Alaska (n = 29; 11.2%).

The Bureau of Mines believes that all coal mines are potentially gassy; for the purpose of administration in the prevention of explosions and fires, the Bureau recommends: That any coal mine wherein methane or other combustible gas can be detected in amounts as much as 0.27 of 1 percent or more by frequent systematic searches shall be classified as a gassy mine. Permissible methane-indicating detectors such as are given in I.C. 7110 or subsequent lists of Bureau approved methane-indicating detectors, or conventional methods of sampling and analysis of required accuracy may be used. The concentration referred to in the decision is percent by volume.

During the past quarter century, coal mine geologists have evolved from providing a qualitative description of drill core and coal reserves into quantitative geotechnical specialists who bridge the gap between geology and engineering. In the late 1970’s and early 1980’s, hazard prediction mapping was in its infancy. At best, hazard maps identified rudimentary structures and conditions such as stream valley locations, sandstone/shale transition zones, and lineaments. A “technical disconnect” had existed between geologists and mining engineers as each profession operated largely independent of each other. Engineers did not readily integrate geology and geologic conditions, with the exception of coal thickness, into mine planning. Geologists were relied upon for reserve estimates and did not participate in mine planning or design. The employment and market slump of the 1980’s and 1990’s combined with mining more challenging reserves mandated that this gap be bridged and forced geologists to learn to speak more “numerically” to engineers so they could be understood. The purpose of this paper is to describe the procedures and thought processes used by coal mine geologists when determining various mineability and reserve extraction feasibility issues. The paper will also address underground geologic hazard mapping and prediction techniques. To achieve these objectives, the opinions and experiences of leading coal mine geologists, geotechnical and strata control specialists across the United States were solicited.

This U.S. Bureau of Mines study examined spontaneous heating episodes in coal mine pillars in an active underground coal mine. The information obtained from these incidents was then analyzed to learn which sampling methods provided the earliest indication of pillar heating. The objective of this study was to discover if the location of future events of pillar spontaneous heating could be inferred from the available information. The spontaneous heating-prone area in this evaluation involved pillars located just in by the mine portals. Several detection methods were used to determine gas levels outside as well as inside the affected pillars. It was hoped that, by incorporating external and internal sampling methods into an organized program, locations undergoing spontaneous heating could be determined more readily. This study found that by drilling small-diameter boreholes into the pillars, then obtaining gas samples from the affected pillars, the ability to locate early spontaneous heating episodes was improved. However, the ability to accurately predict future spontaneous heating events remains in question.

Forty-eight workers who bad escaped large underground coal-mine fires were interviewed using an open-ended protocol. This information, and the actual experiences of one eight-person mine-section crew who escaped from a Fire, were used to construct an 18-frame (page) table-top simulation exercise, which was then field tested with 134 miners. The exercise is a research toot that measures miners' proficiency in the information-gathering and decision-making skills related to escaping from a mine fire. It also helps miners to learn and practice these cognitive skills. The exercise was found to be valid and reliable. All miners reported that the exercise was authentic and would help them to remember important information. The exercise total score, and all but two item scores, significantly discriminated among miners with different levels of training. The two items that did not discriminate dealt with whether or not to abandon a helpless fellow miner. When the exercise total score data were pooled, only 13.6% of the miners achieved a desirable score of 90% mastery or greater. The simulation is important because it teaches and assesses critical judgment and derision-making skills that are rarely addressed in miner training. General A review of the literature has revealed important areas which should be investigated during the field work. Of the utmost importance is the safety of mine personnel. Should a phase-to-ground fault occur on a shovel or other piece of equipment, current may return to the substation through the machine's contact surface with the mine floor, thereby raising the frame above ground potential and posing a shock hazard to anyone who touches the equipment. If the frame-to-earth resistance is very low, then no dangerous potentials will be developed, and a basis for elimination of the ground conductors from trailing cables could be postulated. However, should the contact resistance be high, then lethal voltages could be present on the machine frame. In this instance, low-resistance ground conductors in the trailing cable would be a necessity. Therefore, field tests should include measurements of the earth resistance of electrically operated portable or mobile mining equipment such as shovels, draglines, and drills. When the electrical distribution system is subject to lightning discharges, the grounding grid must safely carry to earth and dissipate enormous current surges. It is essential that the grounding grid have a low value of earth resistance, because any potential developed across the grounding grid as a result of current flow will appear on the frames of all machinery connected to the grid. This surveillance report summarizes data on work-related fatal and nonfatal injuries, illnesses, and hazardous exposures in the mining industry for the 10-year period 1986-1995. The term "surveillance," as used in public health, may be new to many readers of this report. With regard to occupational safety and health, the goal of surveillance is to describe the occurrence of work-related injuries, illnesses, and known hazardous exposures; to identify new hazards that may occur due to the introduction of new technology or other factors; and to assess the effects of preventive measures designed to improve worker safety and health. Surveillance provides an overall picture, which can be used to focus resources on areas most in need of prevention programs or further research. Surveillance generally requires the use of multiple sources of data, all of which have their own strengths and limitations. Surveillance in the U. S. mining industry is complicated by the fact that U.S. national data systems that provide information on mining are based on two different definitions of the mining industry. One is established by the Mine Safety and Health Administration (MSHA), the other by the Standard Industrial Classification (SIC) System [Office of Management and Budget 1987] and the 1980 U. S. Bureau of the Census (BOC) Classification System [U.S. Bureau of the Census 1982]. The mining industry as defined by MSHA is based on the regulatory jurisdiction of that agency. Reporting requirements for injuries, illnesses, and workplace exposures are stipulated under the Federal Coal Mine Health and Safety Act of 1969 and the Federal Mine Safety and Health Amendments Act of 1977. Of note is that the oil and gas extraction industry falls outside of MSHA jurisdiction. The Occupational Safety and Health Administration (OSHA) was charged with enforcement in the oil and gas industry under the Occupational Safety and Health Act of 1970. Under the SIC, the definition of mining covers the extraction of naturally occurring minerals, including liquids (petroleum), gases (natural gas) and solids (coal, metal, and nonmetallic minerals). Exploration and development of mineral properties are also included in the SIC definition of the mining industry. The SIC was developed by the Office of Management and Budget for use in classifying establishments based on the type of economic activity in which they are engaged and serves as the industry classification standard for all establishment based Federal economic statistics. This classification is important for occupational safety and health surveillance because it also serves as the industry classification system for all data on work-related injuries and illnesses collected by the U.S. Bureau of Labor Statistics (BLS). The U.S. Bureau of the Census uses an industry coding scheme similar to that used by the SIC. For the mining industry, the categorization is identical in both systems. Death certificate data made available by the National Center for Health Statistics (NCHS) are categorized under this system. Death certificate data are particularly important for surveillance of mortality from work-related illnesses. Key differences between the SIC/BOC categorization system and the MSHA system can be summarized as follows: • MSHA excludes the oil and gas industry, which is classified as part of the mining industry under SIC/BOC. • MSHA excludes work that is done off of mine property; work off of mine property is included under SIC/BOC if it is performed by an establishment falling into one of the SIC/BOC mining industry codes. • MSHA includes mine-associated mills and processing plants; these are classified under the manufacturing industry, rather than mining, according to the SIC/BOC coding structure. Since the data collected by MSHA and the data collected by BLS and NCHS using the SIC/BOC classification system are both important sources of information on the mining industry and in many cases serve different purposes, data from both systems are presented in this report. In general, data using the SIC/BOC system are comparable across U.S. industries and should be used when comparing mining to other U.S. industries. Much of the data in this report are presented in the form of frequencies or rates. A frequency provides only a count of the number of cases without providing any information about the risk of occurrence. Rates provide an estimate of the risk of injury or illness. The reader is cautioned, however, that rates based on small numbers can be unstable; thus, any inferences should be drawn with care. In this report, no rates are presented for groups in which there were fewer than three cases. The incidence of most injury and illness conditions differs by age, sex, and race. For this reason, data in many surveillance reports are adjusted for these demographic factors. It was not possible to do this for most of the rates in this report because information on demographics was not available. With the exception of the proportionate mortality ratios (PMRs), all data presented are unadjusted. A detailed account of the sources of data is in appendix A. The methods used in data selection and analysis are described in appendix B.

Many coal companies have a system for checking men into and out of the mine , and generally such systems appear to be perfectly sound and adequate . However , when the real test comes at the time of a mine disaster , too often the system is found to have failed , and hours and sometimes days pass before it is known how many men are in the mine and who they are . Most of the systems in use , when properly maintained , are adequate for the routine work of the mine , and some of them may prove adequate at the time of a mine disaster . All systems , however , depend upon proper supervision and strict adherence to the system .

The rather sudden increase in the use of methanol as an antifreeze for automobile radiators has caused considerable comment , with some anxiety to persons interested in industrial and public health , regarding the possible health hazard in dispensing and using the product . In view of this , the chemical industry interested in the manufacture and marketing of methanol requested the Bureau of Mines to make an impartial and extensive investigation of the hazard involved . With the assistance of the industry this investigation is now in progress and will extend throughout the present winter . When these observations are finished a complete report will be made . At present , however , the acute interest in the problem and the many inquiries that have been received from manufacturers of methanol , garage owners , the automotive industry , public health officials , and private persons demands a brief statement relative to the reasons for the recent availability of methanol for antifreeze purposes , the plan of the bureau's investigation , and the trend of the results of the investigation obtained to date .
