The implementation of robust, reliable and accurate sampling systems is the key to the successful management of any mineral processing plant. The representative sampling of a mineral processing plant is the only way to determine what is coming into the plant, what is going out as fi nal product and what is going to tails. In plants with high gravity gold components, where high recovery effort gravity circuits are installed, the use of a gravity recoverable gold (GRG) sampling system can assist in the online monitoring of the performance of the circuit. The typical installation location would be in CIP or fl otation feed (ie cyclone overfl ow), downstream of the gravity circuit. A GRG sampling system generally consists of the following equipment: true proportional, continuous drive, linear cross cut sampler, secondary vezin sampler operated on a timer, a batch centrifugal concentrator, and sample dewatering module. The operating philosophy of the GRG sampling system is that the primary sampler operates on a continuous basis, generating a constant stream of truly representative sample. Consep's H&S drive system is unique in its ability to be able to operate in a continuous mode, therefore enabling a continuous stream of representative sample to be generated. The primary sample stream reports to the secondary sampler. The secondary sampler operates on an intermittent basis (nominally ten minute interval), resulting in a relatively small sample volume per shift. Normally the sample volume would be ten to 15 L per shift. The secondary sample rejects stream reports back to the batch centrifugal concentrator. The sample generated from the secondary sampler can be dewatered using a Consep H&S dewatering module. This device produces a final sample that consists of separate filter cake and filtrate. These are suitable for taking directly to the laboratory. The continuous stream of truly representative secondary sampler rejects is passed through an appropriately sized Knelson Concentrator, typically a KCMD7, KCCD10 or KCCD12 sized unit. This unit recovers any available GRG from the stream. The Knelson Concentrator is also purged of its gravity concentrate at the end of the shift. This sample, which is nominally around 1 - 2.5 kg of concentrate, is also taken to the laboratory. At the end of the shift the client will have a measure of the gravity recoverable gold content of the target slurry stream, as well as a true representative sample of the complete stream. Through the addition of a Knelson Concentrator to an otherwise typical sample station, it is possible to generate a true measure of the performance of your gravity circuit. This will take the guess work out of the daily variation in gravity circuit gold recovery, as the site knows whether the changes are due to gravity circuit equipment performance, or due to the natural variation of ore GRG contents. Maintenance planning can also be designed around the system, as the site will have a composite sample to measure the performance of the circuit, therefore indicating where performance has dropped and maintenance is needed. New knowledge has no economic value unless it can be applied, or æcapturedÆ, as the R&D jargon has it. Australia probably spends more than $500 m per year on mineral industry R&D, distributed amongst the mining companies (IR&D Board, 1997), CSIRO, the universities and others. The investors in this research (industry, and the community via government) will expect some sort of return from their investment. The return will be poor unless the know-how developed in R&D programs can be exploited. This paper explores some of the issues involved in capturing the outcomes of mineral industry R&D in a form which can be exploited for economic benefit, and suggests ways in which it might be done better. It is based on the experience of the JKMRC in conducting applied research in mining and mineral processing and delivering it to the industry. It may seem presumptuous for a research contractor to be telling its clients how to do their job. However, the process of developing, acquiring and implementing R&D outcomes has to be a partnership between provider and receiver, in order for this process to work efficiently. This applies whether the two parties operate within a single corporation (a corporate laboratory and a business unit, for example) or are two entirely independent organisations (eg a mining company and a university). Both partners have a role to play, and the best examples of realising the benefits of R&D almost always involve the open-hearted and skilful handling of this synergy.

Most new engineers in the mining industry do not realise the importance of legal obligations. During investigations the first question you are asked is: Do you have any proof or evidence? The smallest detail in an underground notebook or a diary entry is able to be used as your proof and/or evidence. The importance of a paper trail is often undervalued and overlooked when you first enter the industry. As a new engineer, all too often your legal obligations, including your duty of care are not given enough consideration. Whose job is it to inform you of what your legal obligations are and when do they end? Everyone has legal obligations at every level of the industry, from graduate to mine manager. The level of these responsibilities varies depending on the position that you hold. When you first start in the industry you need to understand what your responsibilities are and how you are expected to discharge them, whether they are memos, databases or senior sign-offs, etc. In order to fulfil your obligations you must first discharge your own obligation, which in turn discharges those of your department and ultimately those of the mine. As a graduate engineer in the mining industry what I have learnt about my own legal obligations has come directly from my own experience, which will form the basis of this paper.

Leaders have invested heavily in traditional safety management to improve safety performance over the years. This primarily involved: • fixing the environment (eliminating the hazard) • improving the system (more/less rules, procedures etc) • making safety more conscious (more/better interactions about the importance of safety). These approaches have helped improve safety performance significantly during the last 50 years and will continue to play a role in the future. Although these approaches may be part of an overall solution, they are not enough to prevent many incidents. Today, these three approaches are used more than before, and are used better than ever before. Yet, workplace fatalities in Australia have stagnated from 2013 to 2019, as have the number of incidents, even serious ones. In the USA, workplace fatalities have increased by 15 per cent in the same period.

The effect of delay time on fragmentation has been discussed for a long period but rarely in a satisfactory and unbiased way. Despite the efforts of many researchers, delay selection for fragmentation improvement or optimisation remains a controversial issue. Large-scale tests have been plagued with scarcity and scatter of observations, while small-scale tests have been associated with boundary effects and energy losses. Theoretical work is often used to produce explanations for available measurements or tries to simplify wave propagation and wave interaction to guide observation and practical work. The present work analyses results obtained from a relatively large number of small-scale experiments in blocks made of grout, and re-analyses results of previous experiments, to conclude on the effect of delay on fragmentation. It is shown that the main contribution of the delay is found in the fragmentation of the larger fragments of the blast, while the small fractions show almost no effect. Samples, where copper liners were used to inhibit penetration of detonation products in the rock mass, showed effect of delay only when short times were used and steady but poor fragmentation when long delays were used. Based on small-scale experiments, it appears that the fragmentation is optimised when a hole fires prior to the radial cracks, produced by the previous hole, reach the free face of the blast. Gas penetration may result in the development of few more fractures that affect the fragmentation of the largest particles of the blast.CITATION:Katsabanis, P D and Omidi, O, 2015. Safety management, both personal and that associated with catastrophic hazard, helps to define the success of mining companies. Personal safety management, often categorised by slips, trips and falls, has been widely addressed, with numerous approaches successfully adopted. Catastrophic hazard can lead to a single accident (eg ground collapse, underground fires etc) causing multiple fatalities. Catastrophic hazard management in mining is an emerging discipline, and as such there are various pitfalls to demonstrate its effectiveness and success; it is much more than simply meeting regulatory requirements. Various standards, guidelines, and leading practices are available for reporting key performance indicators. While measuring and reporting safety related key performance indicators (KPIs) such as Total Recordable Injury Frequency Rate (TRIFR), Lost Time Injury Frequency Rate (LTIFR), are important, they often lack detail, and give little insight into the effectiveness of catastrophic hazard management strategies and controls. Catastrophic hazards are low likelihood, high consequence events, and as such demand a different management approach. Companies cannot use the suite of personal safety metrics to help demonstrate management of catastrophic hazards. In-field verifications have revealed that senior management commitment to safety is often clear, well organised, and resourced. However, there are often some inconsistencies between what management promotes and plan, to what the operational teams understand and implement. Communication and clear understanding of expectations were the common themes identified to help empower supervisors to reinforce requirements of catastrophic hazard management. A lack of specific, measurable, and time-sensitive goals and objectives for the short- or medium-term, making it challenging to track progress was another key observation. Development of an integrated, multi-disciplinary auditing process executed by an experienced team of technical subject matter experts, provided an in-depth understanding of the requirements of catastrophic hazard management. Areas such as underground fire prevention, explosive storage, electrical safety, mobile vehicle interactions, ground control, inrush, and shafts and winders management have substantial overlap and intertwined processes, and similar strategies for emergency responses. Auditing of catastrophic hazards and safety management systems compliance is only the first step in safety assurance. Completing operational performance audits in technical areas and benchmarking the site verification of safety management documents and verifications forms a critical part to clearly demonstrate industry leading safety practices. No longer is it the external inspection or regular hazard management plan review requirements that dominate, but rather the culture of continual hazard identification, management, controls verification, underpinned by improvement and follow-up verifications of the corrective actions.

Heavy mineral sand deposits are a source for the economically important element titanium, which is contained in minerals such as ilmenite and rutile. In heavy mineral deposits ilmenite displays various degrees of alteration. Natural weathering of ilmenite involves the removal of Fe and thus results in an increase in the Ti-content. Minerals such as pseudorutile, ferropseudobrookite, anatase and the ill-defined mineral aggregate leucoxene, which consists mainly of various amounts of remnant ilmenite, pseudorutile, ferropseudobrookite and anatase, are typical replacement products of ilmenite. Ferropseudobrookite occurs as a recrystallised, optically and chemically distinct mineral phase. Textural relationships suggest a diversity of physicochemical conditions including weathering and metamorphism that led to the formation of complex mineral assemblages and textures. In the heavy minerals industry the alteration products of ilmenite are generally referred to as æaltered ilmeniteÆ and/or leucoxene. The distinction between æaltered ilmeniteÆ and leucoxene is not consequently defined and is very much dependent on economic factors and specific mining operations (ie determined by deposit specific mineral assemblages). Generally the term leucoxene is used in the heavy minerals industry as a collective description for ilmenite alteration products with TiO2-components ranging from 65 - 95 per cent. A detailed comparative mineralogical investigation including microscopic, SEM and QEMSCAN« techniques show that it is possible to identify and quantify the replacement products of ilmenite on a micron scale. QEMSCAN« generated particle maps were graphically compared with photomicrographs taken by applying reflective light microscopy. The resulting information can be integrated over the entire sample with respect to composition, absolute and relative mineral abundances, grain sizes, volumes, densities, textural relationships and liberation potentials. This provides a powerful tool for a first appraisal of the physicochemical behaviour of bulk samples and individual particles during metallurgical processing.

Mining has played a significant role in human development and without the mining sector, society would not enjoy a large number of the benefits that it does today. However, mining has also carried with it some important lessons, which remain the source of ongoing learning. One of these lessons is the poor closure or abandonment of mines, worldwide. This has in many cases resulted in large tracts of unproductive land, representing a source of environmental pollution and a safety hazard to the community. Frequently associated with this is a legacy of mining communities having fallen into economic and social decline, beyond mine closure.Due to this legacy, governments, society and the mining sector pay far greater attention to mine closure planning today, than was done in the past. Typically closure planning has focused on the engineering aspects of a mine, namely decommissioning of the workings and plant; dismantling of unusable infrastructure; and rehabilitation of disturbed land. These are important aspects of closure but a more holistic, long-term approach is required which encompasses environmental and social aspects and aims to leave a positive legacy behind.In planning for a mine closure scenario with a lasting positive legacy, it is critical that the broader receiving environments (social, economic, physical and biological) within which a mine is located, define and drive the closure plan and the mine planning as a whole. This ensures that the broader environment influences mine closure, as opposed to looking at how a closed mine will impact on the environment. The ultimate aim is to first understand the broader environment and then to fit the closure of a mine into it. The earlier this approach is followed in the life of a mine, the greater the operational time frame available, and hence a better chance is presented for planning and managing a mine towards the desired, tested and agreed post closure scenario. Within this broad context, there is tremendous scope to achieve successful mine closure. However, it is critical to note that mine closure is not the sole responsibility of the mining companies, and neither is it possible for said mining companies to achieve mining closure alone. This concerted effort requires a multi-stakeholder approach in which partnerships are created and risks, responsibilities and opportunities are shared and managed. To achieve sustainable closure, a multi-stakeholder approach based on an initial forward-looking closure vision is required. This approach provides an early basis for consideration of alternative land uses and economic options from which specific actions to achieve the preferred option can be defined. The Anglo American plc Mine Closure Toolbox has been formulated and developed around this concept. Gold was discovered at Waihi in 1878. In 1987 the modern Martha Mine opened in the middle of the town of Waihi. Newmont Waihi Gold as the current licence holder has been operating the mine since 2000. The current open pit was scheduled to stop production in mid 2006. An underground mine is currently being developed to the east of the town.   In 2001 serious subsidence issues from historical underground mine workings outside the mining licence and the forecast closure of the open pit acted as a catalyst for the development of new communication initiatives.   In this context Newmont Waihi Gold and the Hauraki District Council initiated a community consultation process with the people of Waihi. The Waihi Community Consultation Committee (WCCC) was established in May 2003. After an initial brief to act as a forum for community participation in closure and rehabilitation of the Martha Mine, the group quickly took on a much more proactive role working towards the long-term social, environmental, cultural, economic and spiritual sustainability of Waihi.   In the course of over three years of meetings all groups represented on the committee have presented their views, developed ideas and established an organisational structure to bring those ideas to fruition. The challenge now is to work together to consolidate likely options, seek sources of funding, produce business proposals and implement the plan for the future of Waihi.   This paper backgrounds the experience to date and describes the outcomes of the process. IonicRE acknowledges within our mining industry, there is a common belief that workplace health and safety, often referred to as safety, is just a matter of ‘common sense’ – something obvious and easy. However, at IonicRE, we hold a different perspective. We understand that safety is far from easy. We recognise the need to emphasise the importance of understanding and conveying the inherent difficulties of working safely. We are committed to implementing processes that support individuals in achieving our safety objective of eliminating fatalities and permanently disabling injuries. To build resilience and sustainability into safety performance, we draw parallels with the principles underpinning sustainable and exceptional production performance. Achieving sustainable safety performance demands effective leadership, meticulous planning, adept management, genuine care and unwavering commitment. We’ve leveraged various tools and techniques, such as DISC theory, Maslow’s hierarchy, process control, Kaizen, Six Sigma and Kirkpatrick’s 4 levels, among others, to facilitate this improvement. There methods play a pivotal role in developing a Safety Blueprint for enhanced safety performance. This paper provides an overview of IonicRE’s Safety Blueprint, a strategic framework for developing our safety capability. Our capability is underpinned by three key pillars. Firstly, a comprehensive risk management approach is imperative, encompassing the understanding of hazards, the identification of threats to our people and the associated risks. Secondly, it entails providing and enforcing accountability to sustain our safety capability. Lastly, it involves constructing an effective defence system against threats and harm. Implementing these pillars requires a meticulously devised change process. Anticipating resistance and potential setbacks, we underscore the importance of having the right people and a wellcommunicated vision for safety at IonicRE. A Safety Blueprint serves as a detailed plan outlining the path toward a future goal. At IonicRE, we contend that safety in the mining industry has generally lacked a coherent and effective Safety Blueprint – a comprehensive plan to navigate the journey toward a destination. Drawing parallels with NASA’s detailed plan for landing a man on the moon safely, we question why, as an industry, we cannot similarly eliminate fatalities. While progress has been made in the last century, we must not wait another 50 years to eradicate fatalities from our industry. Examining Safe Work Australia data from 2008 reveals that 47 per cent of mining fatalities result from vehicle activity and 40 per cent stem from individuals being struck by an object. As of December 7, 2023, seven lives have been lost in the mining industry in Australia this year, indicating a stubborn trend mirroring 2022, with limited improvement over the past 25 years. These incidents are not isolated or unexpected. We understand the hazards and controls, yet fatalities persist. IonicRE considers that the mining industry stands at a critical juncture – do we accept a plateau in mining safety performance, or do we rethink our approach? At IonicRE, we are thoroughly reassessing the hazards that threaten our people, systematically redefining effective risk control measures and establishing a robust framework to hold individuals accountable for the effective implementation of the control measure.

The study commenced in October 2012, to understand the basic ecology of the spotted-tailed quoll (Dasyurus maculatus) and to confirm whether the species utilise previously mined land on the Mt Owen mine site that was rehabilitated in 1996 to 2011, as part of their home range. The spotted-tailed quoll is a widely distributed marsupial carnivore that is listed as rare or threatened in every state in which it occurs. The species is a forest-dependent marsupial carnivore that occupies a wide range of habitat types (Long and Nelson, 2004).The first part of the study involved trapping a spotted-tailed quoll and fitting the individual with a combined global positioning system / ultra high frequency (GPS/UHF) radio-transmitter collar (Telemetry Solution – Quantum 4000E). The benefit of the GPS/UHF collar is that it records locations (or fixes) at intervals programmed by the user. The UHF component allows the researcher to locate the individual at a frequent interval, and the Quantum 4000E also has a remote download function, enabling the GPS data stored in the collar to be wirelessly transmitted between the collar and researcher. The benefit of this new technology is that the animal does not need to be repeatedly captured to retrieve the GPS data.Daytime searches are also undertaken using the UHF receiver to locate the individual and locations of daytime denning and latrine sites. The location of each den site is recorded using a hand-held GPS. Data downloaded from the GPS collar is then imported and mapped using computer software program ARCGIS (geographic information system), to overlay the data points onto aerial photography.Results demonstrate that threatened species will utilise rehabilitation areas post mining. The data presented will be used to develop a greater understanding of threatened species ecology in and around rehabilitated areas, which will assist in the planning and design of native forest rehabilitation areas.CITATION:York, P, 2014. "One of the key risks to the success of a mining operation is the accuracy of the estimates of both the economic mineral and any accompanying contaminants contained within the ore reserve. Misrepresenting the mineralogical characteristics of the ore reserve can result in considerable financial investment in an operation that is in fact uneconomic and/or carries a significant environmental risk. This in turn can result in an unplanned closure of the operation with associated financial, environmental and social legacy. One way to minimise this risk is to undertake high density resource sampling to gain a better understanding of the resource characteristics. However, such an approach is expensive, time-consuming and often impractical. The risk of not having an accurate model is typically assessed during a feasibility study utilising a Monte Carlo simulation method or some form of sensitivity analysis. Importantly, the error margins used in these types of analyses are often based on subjective or very general criteria such as ‘professional judgement’ or an ‘industry standard’. This presentation introduces a conditional simulation method for developing a range of mineralogical distribution models. Based broadly on the method outlined in Journel and Kyriakidis (2004) the proposed approach develops this work further. It combines a geostatistical analysis of the available data with automated mine design software to provide a rigorous method for assessing the confidence in the mineralogical characterisation of the ore reserve. The process is based on simulating possible mine plans based on the statistical characteristics of the available resource sample data. These plans are then assessed against high resolution resource models generated using a Sequential Gaussian Simulation method. The outcome is confidence intervals for important characteristics of the ore reserve estimate relative to the actual resource. These confidence intervals are an objective and transparent measure important for assessing closure risks and represent an improvement over common practice.CITATION:Trembath, D, 2016. Last September, when I was asked to prepare this paper, I was told the subject was "mining," a broad subject indeed! Rather than choose one of the many important technical and economic details which characterize mining today, I turned to a subject which fas- cinates me: mining's image. If we are to survive and prosper into the twenty-first century, and if industry and society are to continue enjoying the material benefits which flow from mining, we must address the melancholy fact that the general public at best does not understand mining, and at worst does not believe it is needed or deserves to continue. While my point of view is largely that of an American mining executive, I would hasten to point out that Homestake has participated in Australian mining for over three decades, and our subsidiary, Homestake Gold of Australia Limited, is now a major producer in this "lucky country." Does society need a viable minerals and metals industry? The answer is obviously "yes," so the challenge is to change some minds: not just those of the amorphous "public," but on a focussed basis targeting journalists, educators, TV commentators, politicians and opinion makers of all stripes (I thought of using "rifle shot" instead of focussed but thought that might be misconstrued). Along the way we miners must also look deeply into ourselves, our attitudes towards the society in which we function, and our willing- ness to change as well as to seek change in others. How did we become such bad guys? More than a century ago mining was not just accepted, it was even romanticised. In America, we had altered the time-honored concept of "Crown" lands into public lands made easily available to settlers pushing back the frontier. Miners even preceded the farmers and stockmen in what was perceived as nation-building, although by the 1890s there was a growing interest in conservation, especially the protection and management of forests and national parks. Even though emphasis was shifting toward government stewardship and away from trans- fer of public lands to private ownership, homesteading and mining were clearly recognised as valid uses requiring not just access, but security of tenure. Indeed, conservation as it was then understood implied management of the public domain as well as its protection. Initially this was no great problem to the U.S. mining industry, but as the nation grew and competing land uses became more important, conflicts began to appear, first over water rights and later over air quality and destructive mining practices. But these were local disputes rather than national issues, and two World Wars, the Great Depression and post-war reconstruction lulled our industry into thinking that nothing had really changed. After the storm began to break in the 1960s, the mining industry found itself confronted by a growing maze of complex and costly laws and regulations. It took us longer than it should have to realise that, in facing this challenge, we seemed to have lost the understanding and support of many of our legislators and a majority of the general public. Thank you for the opportunity to speak at this important conference attended by aspiring mining professionals, some of whom will become the leaders of the mineral resources industry that forms the solid foundation of AustraliaÆs past and that will fuel the exciting growth of AustraliaÆs future.I want to briefly touch on the history of our business, then look at the opportunities that exist in the contract mining industry and finally comment on what the educators need to consider in their curriculum to meet the needs of our business. Byrnecut started out as an idea in early-1987 developed by a group of four mining people. At the time the industry was on the high end of a mining boom, the era of the Bonds, Connels, Skases and so on. Riding on this wave of euphoria we borrowed $600 000 from the National Australia Bank and, without having enough knowledge to lose sleep over the potential risks, started business in mid-1987. The euphoria quickly evaporated with the 1987 October stock market crash. The bank pulled our overdraft, equipment finance rates increased to over 20 per cent per annum and my house was mortgagedseveral times over. The business learning curve was extreme for a young mining engineer who had developed his trade in the relatively sheltered environment of a large public mining house, namely WMC. We were somewhat bitter that the banks had done a runner. We were a private company, backing the business with personal assets with our shoulders to the wheel, whilst the Bonds of the world had theirnoses in the trough ripping off mum and dad shareholders. All of this had a sobering effect, namely keep your feet on the ground, stick to the knitting, work hard and worship cash flow. Since that stressful but adrenaline pumping beginning the company now employs over 600 people and is now working for major mining houses at over a dozen locations in Australia, east Africa and WesternEurope and the National bank is again our banker. On this basis I consider myself qualified for the subject of this speech.

Titanium dioxide (TiG ) is produced 2 commercially by two methods generally referred to as the "sulfate" route and the "chloride" route. The SCM Corporation, in supplying 15% of the world's demand for TiO 2, uses both of these processes. Of the two processes the sulfate route is generally regarded as the older and more established technology. Worldwide the sulfate route accounts for some 70% of total Ti02 production. However, in the U.S.A. the chloride process accounts for 80% of production. The processes vary in chemistry and raw material requirements. In the "sulfate" process finely ground ilmenite or titanium slag is digested with sulfuric acid to produce a porous "cake" vrnich is extracted with water and recovered liquor. The solution is treated with scrap iron to reduce any ferric ions to the ferrous state which is then crystallised and removed as ferrous sulfate heptahydrate. The liquor is concentrated by evaporation and hydrolysed to form the insoluble hydroxide which is filtered and washed free of impurities. This is converted to the dioxide by calcination at 800 - 1000¦C and milled to break up any aggregates. The "chloride" process uses a higher quality feedstock of natural or synthetic rutile and/or high grade titania slag. The feedstock is mixed with ground high-purity coke and chlorinated at 950¦C in a fluidised bed. The resulting crude titanium tetrachloride is purified by distillation and chemical treatment. The purified titanium tetrachloride is burned with oxygen or oxygen enriched air to give titanium dioxide and chlorine, which is recovered and recycled to chlorination. Titanium dioxide pigment from both processing routes undergoes similar finishing techniques. It is subjected to further stages of wet and dry milling, then coated with layers of inorganic oxides in order to enhance its properties for specialist applications.

It is a common misconception that the grant of mining tenure, whether it be an Exploration Permit, Mineral Development Licence or Mining Lease, will entitle the holder to access all land within it in order to explore or mine. The ability for miners to access their tenure in order to undertake activities is critical to the industry. While significant time and energy is quite rightly dedicated to resolving native title and cultural heritage issues before the commencement of mining operations, the same cannot always be said for addressing potential impediments that arise as a result of the overlap between mining and other tenures. Miners should be aware of the overlaps that occur with other tenure and the impact of these overlaps on the minerÆs ability to explore or produce from the area. The existence of an overlap with petroleum tenure, nature refuges, national parks, wild river areas or reserves such as aboriginal land leases are all likely to have an impact on the minerÆs ability to access the area and conduct activities. In some instances, such as an overlap with petroleum tenure, Queensland legislation encourages the miner to reach agreement with the holders of the overlapping tenure. However, in other instances, such as an overlap with a national park, mining tenure is unlikely to be granted at all. This paper summarises the impacts and impediments that arise as a result of overlaps of tenure that are likely to affect mining operations. It also provides examples of how these impediments can, if possible, be overcome under Queensland legislation.

Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

Wall control programs tend to be expensive and consequently require financial justification in order to get approval for the expenditure required. The difficulty faced in getting this approval is that the outcomes of a wall control program are all subjective – a prediction of wall stability and what the wall would do with and without the program. Determining a net benefit is difficult and often justification of a wall control program is based on gut feel, history and a good selling campaign to build a perceived need.This paper outlines a simple solution to this problem by applying risk management principles to wall control. The process is to identify the risks to the operation associated with current wall conditions and then build a risk database for different potential scenarios for each risk. A financial impact is assigned to each risk scenario to complete the database. A consequence and likelihood analysis of each of the risks is undertaken and a formula applied to estimate the projected cost to the operation for the current risk profile. The process is repeated for the predicted wall control conditions after the application of the wall control program. A successful wall control program will reduce an operation’s risk profile so this projected cost should be less than that calculated for the current scenario. The difference in the two projected costs is the value that the wall control program has produced and this value can be used to determine the net financial benefit potential for the wall control program.This methodology can also be applied to other improvement programs that focus on risk reduction such as vibration control.

In 2009, Peter Munro and Peter Tilyard examined the “human capital” in the mineral processing sector in a paper entitled Back to the Future (Munro and Tilyard, 2009). One of their major concerns was a general decline in technical skills and process knowledge, exacerbated by lack of appreciation of the impact this can have on process plant performance and, ultimately, profitability or sustainability of the industry. There are many reasons for this decline, not least of which are the work schedules of today’s metallurgists which have a very different balance between technical and non-technical responsibility than those in the past. However, a shift towards a more technical focus has potential to deliver a range of benefits for industry at large and for the individuals in it This paper discusses some options for development of core metallurgical and process engineering skills that can be used by graduates, employers and training providers.Key skills are identified and mapped against available opportunities for development, such as existing graduate programs, on-the job training and external courses. The authors also review a number of past and present high-quality graduate development programs, including the graduate development program in place at Mount Isa Mines in the 1980s and the Anglo Platinum Graduate Development Program (AGDP) currently operating in South Africa.
