In response to the mandates in the MINER Act of 2006, NIOSH conducted refuge alternatives research that included characterizing the utility, practicality and survivability of refuge chambers and outby safe havens. NIOSH also prepared and delivered a report to Congress in late December 2007 that summarized the findings of the research, included recommendations concerning the design and performance specifications for refuge alternatives, and focused on specific information that could inform the regulatory process on refuge alternatives. This document highlights NIOSH's research and recommendations concerning refuge alternatives, survivability evaluations of refuge chambers, and presents a brief review of the current deployment of refuge chambers in underground coal mines in the U.S. The research has lead to the conclusion that refuge alternatives have the potential for saving the lives of mine workers if they are part of a comprehensive escape and rescue plan, and if appropriate training is provided. "Bi- and multilateral banks as well as export credit agencies in the OECD countries, but also numerous commercial banks, last year committed themselves to compliance with the World Bank’s standards. These environmental and social guidelines, also known as the Safeguard Policies, are the strictest international standards in the area of financing of major investment projects. The Safeguard Policies are procedural and quality guidelines by means of which socially and environmentally relevant impacts can be taken into account in the planning and implementation phase of a project. Objectives of the Bank Safeguard Policies are to ensure that environmental and social issues are evaluated in decision-making, to reduce and manage risk of projects and programs, and to provide a mechanism for consultation and disclosure of information. They are often referred to as the "10+1 Safeguard Policies," since ten of the guidelines address environmentally and socially relevant issues, while one guideline is exclusively concerned with public participation ("Public Disclosure"). In the presentation, the transparency of risk management measures and emergency response plans in pipeline projects will be discussed as an indicator of sustainability. The discussion is to be carried out on the basis of the results of an evaluation of several major pipeline projects, which have been financed at least in part by multilateral banks over the past few years. The controversies surrounding some of these projects due environmentally and socially relevant impairments connected with the investments have also been addressed by the Extractive Industry Report of the World Bank. Pipeline construction was the investment type chosen for the examination to be presented here, since pipelines frequently start in remote, unspoiled regions, and their routes often cross various natural zones and areas with various social structures. The question of safety during construction and operation of the line is always central to these projects; not only for the responsible authorities but also for other societal groups involved, such as environmental associations."

This section deals with one of the most fascinating aspects of mining the evaluation of mine investment opportunities. Be-fore proceeding further, it is important to clarify the meaning of some fundamental terms used frequently in the text. 6.0.1.1 Mine Valuation vs. Evaluation The words "valuation" and "evaluation" in a mining context are often used interchangeably. However, for the purposes of this section, a distinction is made between the two terms in accordance with commonly accepted practice. The word valuation in a mining project context has the rather narrow meaning of placing a dollar or other currency value on the worth of the project as a whole. In other words, the value of a mining project refers to a measure of the desirability of ownership of that property. As such, the major item of interest is, "what is the mine worth?," or `what is the value of the mine?" The value of a mining project may actually be determined in the marketplace at any specific point in time, or it may be estimated by one of several methods. In the last four years, 2007~2010, 9 out of 24 metal/nonmetal underground mine fatalities were caused by ground fall across United States of America (Mine Safety & Health Administration, MSHA). To reduce or eliminate incidents/accidents caused by ground fall, this paper clarified how to design and select ground support based on different ground conditions, how to use mine ground support standards, what geotechnical engineers and underground operators should do to keep mine production free of ground failures.

The seepage forces generated around pressurized water tunnels in rock are investigated using the analysis tools of geomechanics. The theoretical background of the two-phase material behavior is explained and the equations of motion and continuity are presented. Applications on two case studies typically occurring in practice deal with the required minimum rock cover for a pressurized tunnel and the separation between two tunnels carrying different heads. The rock mass behavior as described by the Mohr-Coulomb and Hoek-Brown material models is investigated by numerical analyses and the results are compared. The effect of key material parameters on the safety factors and the associated modes of failure is discussed.

The current state of knowledge concerning mesothelioma risk estimates is reviewed. Estimates of the risk of mesothelioma exist for the commercial asbestos fiber types chrysotile, amosite and crocidolite. Data also exist on which to assess risks for winchite (sodic tremolite) and anthophyllite asbestos. Uncertainty in estimates is primarily related to limitations in measurements of exposure. Differences in the dimensions of the various fiber types and of the same fiber types at different stages of processing add a further complication. Nevertheless, in practical terms, crocidolite presents the highest asbestos related mesothelioma risk. The risk associated with sodic tremolite (winchite) appears to be similar. In chrysotile miners and millers, the mesothelioma risk has been linked with exposure to asbestiform tremolite. Exposure to chrysotile in a pure form seems likely to present a very low if any risk of mesothelioma. While the majority of mesothelial tumors result from exposure to the asbestos minerals, there are other well established and suspected etiological agents. While a practical threshold seems to exist for exposure to chrysotile, it is unlikely to exist for the amphibole asbestos minerals, especially for crocidolite. To date there is no indication of an increased risk of mesothelioma resulting from non-commercial fiber exposure in the taconite industry. © 2007 Elsevier Inc. All rights reserved. Exposure?response relationships between the relative risk of lung cancer and quantitative measures of exposure to asbestos are available from a number of epidemiological studies. Meta-analyses of these relationships have been published by Lash et al. (1997) [Lash, T.L., Crouch, E.A.C., Green, L.C., 1997. A meta-analysis of the relation between cumulative exposure to asbestos and relative risk of lung cancer. Occup. Environ. Med. 54, 254?263] and Hodgson and Darnton (2000) [Hodgson, J.T., Darnton, A., 2000. The quantitative risks of mesothelioma and lung cancer in relation to asbestos exposure. Ann. Occup. Hyg. 44, 565?601]. In this paper, the risks derived in these meta-analyses have been compared. Lash et al., concentrated on process and found that the risk of lung cancer increased as the asbestos is refined by processing. Hodgson and Darnton concentrated on fibre type and found that the risk was highest for exposure to amphibole asbestos (crocidolite and amosite), lowest for chrysotile and intermediate for mixed exposure. Some of the differences between the conclusions from the two meta-analyses are a consequence of the choice of studies included. The range of asbestos types included in the studies in the analysis of Hodgson and Darnton was wider than that in Lash et al., enabling differences between fibre types to be analyzed more readily. There are situations where occupational exposure to chrysotile asbestos has shown no detectable increase in risk of lung cancer. In the early 1970s, it became a concern that exposure to the mineral fibers associated taconite ore processed in Silver Bay, Minnesota would cause asbestos-related disease including gastrointestinal cancer. At that time data gaps existed which have now been significantly reduced by further research. To further our understanding of the types of airborne fibers in Silver Bay we undertook a geological survey of their source the Peter Mitchell Pit, and found that there are no primary asbestos minerals at a detectable level. However we identified two non-asbestos types of fibrous minerals in very limited geological locales. Air sampling useful for risk assessment was done to determine the type, concentrations and size distribution of the population of airborne fibers around Silver Bay. Approximately 80% of the airborne fibers have elemental compositions consistent with cummingtonite-grunerite and the remaining 20% have elemental compositions in the tremolite-actinolite series. The mean airborne concentration of both fiber types is less than 0.00014 fibers per milliliter that is within the background level reported by the World Health Organization. We calculate the risk of asbestos-related mesothelioma and lung cancer using a variety of different pessimistic assumptions. (i) that all the non-asbestos fibers are as potent as asbestos fibers used in the EPA-IRIS listing for asbestos; with a calculated risk of asbestos-related cancer for environmental exposure at Silver Bay of 1 excess cancer in 28,500 lifetimes (or 35 excess cancers per 1,000,000 lifetimes) and secondly that taconite associated fibers are as potent as chrysotile the least potent form of asbestos. The calculated risk is less than 0.77 excess cancer case in 1,000,000 lifetimes. Finally, we briefly review the epidemiology studies of grunerite asbestos (amosite) focusing on the exposure conditions associated with increased risk of human mesothelioma. © 2008 Elsevier Inc. "A fire design innovation of the concrete lining of the Ohio River Bridges East End Crossing Tunnel has been developed. As a result from a fire suppression and tunnel ventilation system, the project design Heat Release Rate (HRR) was able to be constrained from 300 MW to about 50 MW. With this reduced HRR, concrete lining without passive fire protection may be acceptable, if the tunnel concrete lining will not result in explosive spalling during the designed fire event. Concrete explosive spalling test was proposed to verify this assumption. Since no standard concrete fire explosive spalling testing procedure exists, a project specific testing procedure was developed and fire tests conducted. From fire test results, it is concluded that the proposed tunnel concrete lining, in conjunction with the proposed fire suppressing and ventilation system, will not experience explosive spalling and does not require a passive fire protection system. INTRODUCTION The Ohio River Bridge East End Crossing Tunnel is part of a Public Private Partnership project, which connects transportation network between Louisville, Kentucky and southern Indiana. Located in Louisville, Kentucky, the twin-tube highway tunnel provides northbound and southbound lanes of the Kentucky approach to a new bridge across the Ohio River. Tunnel lengths are approximately 1,675-ft and 1,685 for the northbound tube and the southbound tube, respectively. Connected by two cross-passages, both tubes have a roadway width of 40-ft, for 3-lane traffic, and a vertical clearance of 26.5-ft.The Tunnel was constructed by drill and blast method with shotcrete and rock dowels as initial ground supports. The final tunnel lining consists of 16 inches cast-in-place concrete lining and 20 inches invert slab, with a waterproofing system around the tunnel perimeter. Two (2) tunnel side drains and one (1) invert drain were provided for each tube. This paper describes the rational in assessing fire design requirements of the tunnel final concrete lining, to answer the question whether a passive fire protection system is needed or not."

Texas is a major producer of industrial mineral resources required by the state’s ever-growing population that now exceeds 28 million. Texas typically ranks in the top three states for the value of non-fuel mineral production with a total value of $5.2 billion in 2017, accounting for 6.9% of the total US mineral value. Almost 90% of current Texas industrial mineral value comes from the production of cement, crushed stone, industrial sand, and construction sand and gravel. Construction material consumption is focused in the 25 metropolitan areas of the state with populations of > 100,000 residents. Industrial mineral consumption is dominated by the four most populous regions of the state—Houston, Dallas-Fort Worth, San Antonio-New Braunfels, and Austin-Round Rock—with an aggregate population of more than 18 million. More than 11,000 employees work in Texas mining and processing operations. Texas produces more crushed stone than any other state from more than 200 quarries; a similar number of operations produce sand and gravel. Specialty industrial sand production for use as proppants in hydraulic fracturing for enhanced petroleum production serves as an excellent example of how developments in one industry sector can markedly affect another industry; the production of industrial sand in Texas has markedly increased over the past decade, reaching $1.3 billion in 2014. With Texas’ population projected to reach 50 million by 2050, continuing in-state development and production of industrial minerals will be required to supply its residential, commercial, and industrial users. Favorable geology and economic factors provide opportunities for identification and development of new industrial mineral production sites.

In the last few years major industrial companies have begun to realize the importance of Reliability as an enterprise strategy that, properly implemented, will improve financial performance. Typical results of an effective Enterprise Reliability Strategy include a 20% -50% reduction in maintenance cost accompanied by a 5% - 10%increase in real production capacity, with no capital investment in production equipment. One factor fueling the race to acquire and implement enterprise reliability is the pressure for companies to be increasingly cost-effective and competitive in global markets. In a recent study, Asset Productivity: The Next Wave, the Boston Consulting Group (BCG)stated that, “In order to compete for funds, companies must offer investor returns that are competitive with other opportunities. Alternative investments such as dot-com and others with non-traditional business models have raised the bar.” (APDF version of the study can be downloaded at www.bcg.com). BCG views asset productivity as the most powerful mechanism for increasing shareholder return. The author believes that the most effective way to increase Fixed Asset Productivity is to maximize asset utilization by increasing the Reliability of the Assets and the associated processes. Developing and implementing an ERS requires significant effort and knowledge. Most companies have difficulty finding a sufficient supply of these valuable commodities to implement an ERS Program internally. Many companies also struggle with figuring out how to get started and what direction to proceed.

The reduction process of pre-oxidized ilmenite concentrate powder in 30%CO–70%N2 atmosphere was characterized by thermogravimetric and differential thermogravimetric (TG-DTG) analysis system at the temperature from 1073 to1223 K. The isothermal reduction results show that the reduction process is divided into three stages and the corresponding apparent activation energies are obtained by iso-conversional method and model-fitting method, respectively. For the first stage, the conversion degree increases sharply with increasing time, and the reduction process is controlled by the random nucleation and subsequent growth model (A2 model). For the second stage, the conversion degree increases slowly with increasing time, and the reduction process is controlled by the diffusion of carbon monoxide in the reduced layer (D1 model). For the third stage, the reduction processes at different temperatures are controlled by the diffusion of carbon monoxide in the reduced layer (1073 K and D1 model), phase boundary chemical reaction (1123 K and R2 model), random nucleation and subsequent growth (1173 to 1223 K, A2, and A3 models), respectively. For the whole reduction process, the apparent activation energies obtained by iso-conversional method and model-fitting method are 71.72 and 31.92 kJ mol−1 , respectively

Researchers from the Office of Mine Safety and Health Research (OMSHR) of the National Institute for Occupational Safety and Health (NIOSH) completed field trials during coal mine rescue contests using simulated gas detectors in place of placards. The gas monitor simulator (GMS) devices replaced static, paper placards to deliver gas concentration data. Eleven teams were observed during the field contest, which was approved by the Mine Safety and Health Administration (MSHA). GMS devices were given to three team members, the captain, and both gasmen. Observation of teams revealed that team members changed behavior by acknowledging and sharing information more often while using the GMS. As a result of this research, it has been demonstrated that under the existing contest format, the newly developed GMS device may be used with no adverse impact on team contest performance. Furthermore, when GMS technology advantages are used, contest realism is improved. In the future, competency assessment may be incorporated.

Exposure to respirable crystalline silica (RCS) can cause serious and irreparable negative health effects, including silicosis and lung cancer. Workers in coal mines have the potential of being exposed to RCS found in dust generated by various mining processes. The silica content of respirable dust in one single mine can vary substantially over both time and location. The current monitoring approach for RCS relies on the use of traditional air sampling followed by laboratory analysis. Results generated using this approach are generally not available for several days to several weeks after sampling, and this delay prevents timely and effective intervention if needed. An alternate analytical method is needed to reduce the time required to quantify the RCS exposure of mine workers. The National Institute for Occupational Safety and Health (NIOSH) has developed a new method using commercially available portable infrared spectrometers for measuring RCS at the end of the sampling shift. This paper will describe the application of the new field-based RCS analytical process for coal mines, including the use of the new method with the existing Coal Mine Dust Personal Sampler Unit. In a case study conducted by NIOSH with a coal mine operator in West Virginia, field-based RCS analysis was completed at a mine site to evaluate the new technique. The RCS analysis results obtained by the field-based method in this case study showed sufficiently strong correlation with results obtained by the MSHA standard laboratory analysis method to allow the mine operator to use the field-based method for evaluating process improvements.

Among the impurities present in phosphate ore, dolomite is the most difficult to remove. This is because dolomite and phosphate have similar chemical compositions and because the dissolved species of dolomite is adsorbed on the surface of the phosphate mineral. A new amphoteric surfactant was found to be a suitable collector for the separation of dolomite and phosphate. Flotation results showed that the new amphoteric collector had a high flotation selectivity for dolomite over a wide pH range. studies on the flotation of francolite and dolomite in each other's supernatant demonstrated that the dissolved ions had little effect on the selectivity of the new collector. It was found, however, that the dissolved ions were detrimental to the flotation separation using sodium oleate as collector. From a mixture offrancolite and dolomite containing 26.2% P2O5, a phosphate concentrate containing 35.2% P205 with a 90% recovery was obtained using 290 g/t of the amphoteric collector. The rejection of MgO was 81.41 %.

This international symposium was convened to assess the current state of knowledge concerning the health hazards that might be associated with the ingestion and inhalation of fibrous particles produced from the processing of taconite ore. Concern about fibrous particles in taconite arose from their similarities to amphibole asbestos minerals that are known to be human carcinogens. The mineralogical characteristics of asbestos fibers are reviewed and compared with the fibrous particles known to be present in taconite ore from the eastern end of the Iron Range. The epidemiological and experimental animal studies for asbestos-related disease (including gastrointestinal cancer from asbestos in dinking water)were critically evaluated andcompared to those of the fibrous minerals in the taconite. The focus was on the amphiboles in the cummingtonite?grunerite and tremolite?ferroactinolite series and other fibrous particles in both the Iron Range and the Duluth Complex. This included examination of the reported occurrence of any asbestiform ferroactinolite, crocidolite asbestos, chrysotile asbestos, etc. In addition, an effort was made to provide a scientific framework to address fibrous minerals, which may be associated with nonferrous mineral deposits in the Duluth Complex. State-of-the-art risk assessment models for asbestos were presented. Risk assessment scenarios were developed for taconite-derived fibers at environmental exposure levels related to the operation of the Silver Bay taconite processing facility. These calculated risks were found to be trivial even after it was assumed that all mineral fibers were as carcinogenic as amphibole asbestos.

Grunerite asbestos (amosite) has been shown in epidemiological and experimental animal studies to cause lung cancer, mesothelioma and pulmonary fibrosis commonly referred to as asbestosis. An overview of the human and experimental animal studies describing the health hazards of grunerite asbestos (amosite) is presented. Of the many human studies describing the health hazards of asbestos, only three factories using mainly, if not exclusively, grunerite asbestos (amosite) have been studied. The first is a series of reports on a cohort of 820 workers from a plant located in Paterson, NJ. Among this cohort, 18.7% died of lung cancer and 17 mesotheliomas occurred. The Paterson factory closed in 1954 and moved to Tyler, Texas where it operated until 1972. Among the 1130 former workers in the Tyler plant 6 mesotheliomas were reported with 15.8% lung cancer mortality. The third grunerite asbestos (amosite) exposed cohort was an insulation board manufacturing facility in Uxbridge, United Kingdom. Here 17.1% of the workers died of lung cancer and 5 mesotheliomas occurred. The lung content from 48 Uxbridge workers was analyzed by analytical transmission electron microscopy for mineral fibers. The relationship between grunerite asbestos (amosite) concentrations in the lung correlated with grades of fibrosis and asbestos bodies and was lower than the concentration found in the cases with malignant tumors. The lung cancer cases contained more grunerite asbestos (amosite) than mesothelioma cases, and in the cases of non-alignant disease the concentrations were still lower. In both types of malignancies the concentration of grunerite asbestos (amosite) was very high-over a billion fibers per gram of dried lung tissue. Occupational exposure to airborne concentrations of between 14 and 100 fibers of grunerite asbestos (amosite) per milliliter after 20 year latency causes marked increases in lung cancer, mesothelioma and pulmonary fibrosis (asbestosis). © 2007 Elsevier Inc. Occupying a refuge alternative (RA) is an accepted practice in mine fires and explosions when escape is not feasible. MSHA has promulgated a final rule on refuge alternatives that includes a recognition that thermal environment is an important consideration. To this end, an environmental index was specified. The question remains about how to test a refuge shelter to demonstrate that it meets the final rule. The analysis followed five activities: A. Provide an estimate of heat generated by an individual refuge alternative occupant; B. Evaluate the Steadman Apparent Temperature criterion of 95°F; C. Discuss simulations based on body temperature or skin temperature limits; D. Suggest method of simulating human heat generation during refuge occupancy; and E. Suggest alternative methodologies/criteria for evaluating heat and humidity survivability inside a refuge alternative.

Machine-related injuries are one of the major safety concerns in underground coal mines. Severe injuries and fatalities occur when a miner is struck, crushed, or pinned by mining equipment such as a continuous mining machine (CMM), shuttle car, or a scoop. Proximity detection systems (PDSs) have been applied in mining to reduce these types of injuries and fatalities. All of the PDSs that are currently approved by the Mine Safety and Health Administration (MSHA) for use in underground coal mines are magnetic-field based and could be affected by metallic objects such as trailing cables. Researchers from the National Institute for Occupational Safety and Health (NIOSH) investigated the influence of trailing cables on the performance of PDSs. In particular, the magnetic field coupled from proximity system generators to a de-energized trailing cable was characterized. The results show that significant energy can be coupled from the proximity system generators to a trailing cable when there is a closed loop in the cable. The effect on PDS performance from the magnetic field radiated around an energized trailing cable was also quantified for different current amplitudes in the cable. It is shown that the magnetic field caused by the electric current in the trailing cable mainly consists of a 60-Hz signal and its harmonics which causes little interference to the PDS. The results presented in this paper can help PDS manufacturers to design better systems that are more immune to these effects.
