Since 1980, more than 40 miners have lost their lives by being trapped and suffocating in stock-piles, bins, silos, and similar facilities. Mine Safety and Health Administration (MSHA) is concerned about these accidents, most of which could be prevented if management and miners appreciated the hazard. In February 1986, the coal industry had a suffocation accident that drew national attention. A group of engineers were standing on a coal stockpile. It gave way under their feet. Five of the engineers were buried in the coal and suffocated. As with all fatal accidents, MSHA investigated the cause of this tragedy. I also asked my staff to look into the history of similar accidents. I found out it is rare to have multiple fatalities due to suffocation in material. But, over the years, there has been a long series of individual fatalities from this type of accident. Here are some examples. In Oct. 1985, a coal mine employee in Virginia entered a hopper to collect a coal sample. The feeders were started while he was standing in the hopper. He sank into the coal and suffocated. In Sept. 1985, an employee at a Texas sand company climbed onto a surge pile, to check a problem with sand flow. When other employees found him, the sand had swallowed him. Only his face and one hand were showing. Though the victim's face was clear of the sand, he suffocated before rescuers could free him. Many people do not realize you can suffocate in this kind of situation, though your head is free. Every time you exhale, the material has room to shift and press harder against your chest. Then you have less room to inhale. If this goes on long enough, it gets to the point where you cannot inhale at all. That is what happened to the victim in this accident. In May 1985, a plant operator at a Wisconsin sand and gravel operation purposely slid into the drawhole of a surge pile. He tried to break up large rocks that were clogging the drawpoint. Material piled on the side of the surge pile slid down and buried him. The operator was wearing a safety belt and line. But he was covered by 3 to 3.7 m (10 to 12 ft) of material and could not be pulled free. This incident illustrates another point. A safety line can help. But it is not enough. Workers should never put themselves in a position where loose material can fall on them from above. In Jan. 1985, a young Utah coal miner was waiting for a ride home from a co-worker whose job was at the tipple. A jam developed in the coal bin. The men could not clear the jam from the outside. So the young miner jumped into the bin. He started digging with a shovel at the toe of the coal pile. It caved in on him and he suffocated. The young miner had only three months of mining experience. He had not been trained for work around the coal bin. Since 1980, 16 coal miners and 26 nonmetal miners have lost their lives in loose-material suffocation accidents. Suffocation accidents occur in many areas around mines - bins, hoppers, stockpiles, surge piles, and silos, among others. One recent accident occurred at a surface coal mine in Illinois. The victim was walking over an area that had been drilled and blasted the week before. He fell into a void that had been crusted over with earth. He too suffocated. And many materials have been involved in suffocation accidentscoal, crushed stone, sand, salt, different kinds of ore, ammonium nitrate pellets used in blasting, and a powdery byproduct of iron ore milling called calcine. In addition to fatal accidents, there are near misses. We do not know how many. Most of them are not reported. A person who gets trapped in material, but is rescued, is usually unhurt. MSHA sometimes hears about these accidents by chance, or because they are reported in the local media. We do know that near misses are happening far more often

Introduction Coal has always been China's predominant energy resource. The country ranks among the three richest countries in coal reserves in the world, next to the US and the Soviet Union. According to China's Ministry of Coal Industry (MCI), coal accounted for about 72% of the total national energy consumption. Despite efforts to increase the output of oil and hydropower, coal is predicted to remain dominant on the energy scene by increasing its share to 77% by the turn of the century. To match the rapid growth in the agricultural and industrial sectors, MCI has targeted the coal industry to double coal output from 600 Mt/a (660 million stpy) in 1981 to 1.2 Gt/a (1.3 billion stpy) by the year 2000. How is this goal to be achieved? According to MCI, it will be accomplished by: • building new underground and surface mines with a total output of 400 Mt/a (440 million stpy); • increasing the output of existing state-controlled mines from the present 344 Mt/a (368 million stpy) to 400 Mt/a (440 million stpy); and • developing and increasing the output of small, local mines run by province, county, and production brigade from the present 276 Mt/a (304 million stpy) to 400 Mt/a (440 million stpy). Building new underground and surface mines New underground coal mines will be developed in areas where the coal industry has already made headway and infrastructures currently exist. This is to optimize hardpressed capital spending on investment. Ten existing coal production bases were selected in nine different provinces that met these requirements. The underground mines scheduled to be constructed in stages at he existing coal production bases will have a total output of 200 Mt/a (220 million stpy) by the end of the century. Regarding surface mines, four mines in the Inner Mongolia Autonomous Region and one in Shanxi province have been earmarked for construction during the same period. By the year 2000, these surface mines are expected to produce 200 Mt/a (220 million stpy). Foreign equipment, machinery, and surface mine stripping technology were all involved in different degrees in the development of the five new surface mines. Some have signed joint venture agreements. Occidental Petroleum Corp. is in a joint venture with China in developing Ping-shou's 15 Mt/a (16.5 million stpy) Antaibao No. 1 open-pit. The Shentou power station, situated nearby, is anticipated to consume 3.2 Mt/a (3.5 million stpy) of the coal it produces. Some is designated for export. Increasing output There are 554 state-controlled mines that have been in operation since before 1980. They produce 344 Mt/a (379 million stpy). To increase capacity, MCI has laid out some guidelines. • Coal prospecting near existing mines must be given top priority and intensified so that workable reserves can be increased. This would lengthen the mines' useful life. • Many mines and slopes that were constructed during the 1950s are very small and densely populated. Spatial and temporal temperature variations in a high-level radioactive waste repository drift can give rise to moisture transport within the drift. Knowledge of this moisture transport may be important to assessing the potential for corrosion of all metallic components in the drift. The model presented here shows one approach to simulating evaporation, transport, and condensation of water in large underground openings using a framework based on computational fluid dynamics. Simulation results that explicitly model water and latent heat transport are presented.

To understand why some companies excel in creating value, Jim Collins (2001) and a group of postgraduate business students engaged in a comprehensive, five-year research project. They examined 1,435 U.S. companies for more than 40 years, seeking those that made significant improvements in their financial performance over time. The distinctive pattern sought was: ?Fifteen-year cumulative stock returns at or below the general stock market, punctuated by a transition point, then cumulative returns at least three times the market over the next 15 years. ?The search identified 11 outperforming companies, all in different industries, that were called the ?good to great? companies. To contrast these companies with similar peers within the same industry, the search also selected a group of comparison companies that failed to make the leap from good to great. Collins then set out to examine the transition point. What distinguishing features did the good-to-great companies have that their industry counterparts did not? At the heart of the findings about these outstanding companies is what Collins refers to as the ?hedgehog concept? ? how to find the one big thing that the company must focus on. According to Collins, those who built the good-to-great companies were, to one degree or another, hedgehogs. Those who led the comparison companies tended to be foxes, never gaining the clarifying advantage of a hedgehog concept, being instead scattered, diffused and inconsistent. This insight is complemented by Peter Drucker (2004), an influential management guru, who asserts that the first question an effective executive should ask is: ?what needs to be done?? Asking this question and taking it seriously, he asserts, is crucial for managerial success; and that the answer almost always has more than one urgent task, but the effective leader only concentrates on one task. In this last respect, Drucker emphasizes: ?If they [executives] are among the sizeable minority who work best with a change of pace in the working day, they pick two tasks. I have never encountered an executive who is effective while tackling more than two tasks at a time.?

This paper describes the microbiology basics underlying heap and tank biooxidation of metal sulfide ores and concentrates. The primary organisms are identified and described along with the reactions they catalyze and their growth requirements. Also described is the interaction between ore leaching bacteria and the leaching environment in terms of solution chemistry, temperature, and mineral precipitation. Specific factors and agents that stimulate or inhibit growth and activity of ore bioleaching will be identified. Some of these factors are organism-specific and others are generic for microorganisms that grow under acidic conditions. Current knowledge of the limits of bioleaching in terms of dissolved ions in leach solutions, pH, and temperature will be summarized.

Laboratory testing was carried out to evaluate several devices for recovering clean coal from bituminous coal refuse material. The bulk of the testing focused on using a dense-medium cyclone to process -0.6-mm refuse material. A hindered-settling column and a spiral concentrator were also used to process 1.2x0.15-mm material. Product yields and ash contents were obtained for all tests, while partition curves were generated for selected tests from which the characteristic performance parameters (relative density of separation and probable error) were determined. The results indicated that the dense-medium cyclone was capable of cleaning down to 0.037 mm, while the other devices were effective to 0.15 mm.

A 1995 RETC paper of similar title presented a method for estimating rock tunnel water inflow based on semi-empirical correlations with borehole water pressure tests. Since that time the author has found the method to be very useful, and it has been used by others in the industry. This current paper is an update which is intended to: A. More clearly explain assumptions and limitations of the method. B. Describe some case history examples. C. Respond to questions which have been raised; and extend the method to other considerations including distribution of inflow along the tunnel, panning requirements, interpretation of tunnel probe hole water inflow, and estimation of potential effectiveness of pre-excavation grouting to reduce water inflow.

We performed numerical modeling studies to investigate water seepage into underground openings excavated in unsaturated fractured rock. Water seepage is a key factor affecting the performance of a nuclear waste repository such as the one proposed at Yucca Mountain, Nevada. The amount of water dripping into an underground opening depends on the connectivity and permeability of the fracture network as well as on the capillarity of individual fractures intersecting the opening. We developed a high-resolution numerical model of an unsaturated fracture network and examined the appropriateness of seepage predictions using a fracture-continuum model. Model calibrations to liquid-release tests performed at Yucca Mountain are also discussed.

Many, if not most, of today's pollution liability claims are related to events ("occurrences") that happened in the past. This fact creates an opportunity to obtain a large amount of insurance coverage at essentially no cost. Furthermore, in many cases, this insurance coverage is readily available. Scenario #1 For some 15 years, an underground gas tank has had a slow leak. The leak has now been discovered because the gas has contaminated nearby wells. As a result, an expensive $1 million cleanup is required. In addition, a liability suit has been filed for damages due to the contamination of nearby wells. Consequently, legal expenses are being incurred. Are all of these actual and potential costs insured? Obviously, if you have a pollution liability policy in place, the above costs will probably be insured. I say probably because business liability policies are not uniform. But, what if you don't have a pollution liability policy and what if your current general liability policy excludes all pollution liability (as most of them do today)? Under these circumstances, it first appears there is no insurance for this $1 million cleanup and liability suit. But this is probably not the case. You may well have insurance coverage, and you may well have much more insurance coverage than you ever expected. And, the insurance that I'm referring to is already in place. You simply need to find it and learn how to use this insurance - I'm referring to your old policies. In many cases, old liability insurance policies provide coverage for claims made today, because these policies were written on an "occurrence" basis of coverage. In fact, most still are written this way. This means that the insurance policy covers bodily injury or property damage liabilities (as defined in the policy) arising out of' "occurrences" or accidents that happen during the policy period regardless of' when the claim for damages is made. Therefore, for insurance purposes, the key question is: when did the "occurrence" take place? If someone falls and breaks a leg, that is clearly an "occurrence." The broken leg occurrence is, therefore, insured by the liability policy that was in existence at the time of the fall - regardless of when the claim or suit is filed, if ever. However, the answer to this question is seldom an easy one when you are looking at a pollution claim. In our scenario, the tank had a slow leak over 15 years, but property damage was not discovered until now. The question of when the "occurrence" (as defined in the insurance policy) took place has been litigated and continues to be litigated in many courts across the country. But, most often the answer has been that, for insurance purposes, the "occurrence" is the entire period that the tank was leaking (15 years), even though the contamination to the water was not discovered until 1987. This means that the person or company that is insured can potentially use all of its general liability policies (if they are "occurrence" policies, and most were), which were purchased during the 15 years from 1972 until 1987. And, these policies will potentially insure all of the claims and related exposures that arise out of this leaky underground gas tank. Scenario #2 In our first scenario, the claims and expenses were insured by past general liability policies because the claims arose out of events in the past, which, in turn, caused damage. Let's look at another example. In this scenario, Company XYZ has received a request to help clean up a landfill site because 10 years ago, XYZ dumped some toxic substances at the site, which was totally legal at the time. XYZ has not dumped anything at the site since this time. Nevertheless, XYZ is now being required to contribute $2 million to the cleanup of this site. In this scenario, if you had purchased "occurrence" general liability policies 10 years ago, these 1977 policies would (potentially) apply to the current $2 million cleanup claim. And, the reason is that the "occurrence" leading to the 1987 claim took place in 1977.

Australia, benefiting from a weak currency, receives 27% of the total goods export value from non-energy mining lead by aluminium, iron ore, gold and nickel. Consolidation has eliminated CRA, Comalco, North, Savage, Acacia, Ashton, Great Central, Aberfoyle, Placer Pacific, Plutonic, Normandy and RGC leaving Rio Tinto, BHP-Billiton, WMC, MIM and, increasingly, the South African companies Anglo, Gold Fields and Harmony as dominant players. Exploration stagnates with expenditure dropping 40% in Australia and 80% in PNG from peak levels. Major new regional developments include Rio’s West Angles iron ore project, Newcrest’s Ridgeway gold-copper mine and Inco’s Goro nickel-cobalt project in New Caledonia.

Introduction The South African Iron and Steel Co. Ltd., in Sishen, operates a large iron ore mine in the Cape Province of the Republic of South Africa. The mine is about 600 km (370 miles) west of Johannesburg. Its annual capacity is 100 Mt (110 million st) of ore and waste. Present production is 60 Mt/a (66 million stpy). The mine operates continuously six days a week. Primary equipment includes 55 154 t (170 st) Wabco and Unit Rig trucks and 14 P&H shovels. During 1979, a feasibility study into the application of a trolley assist system was launched. This was due to the escalation in diesel fuel costs and the appeal by the South African government to reduce the use of fossil fuels. Tests and economic evaluations proved the system technically feasible. The estimated discounted cash flow return exceeded 50%. Diesel fuel consumption would be reduced by 120 ML (31.7 million gal) in the next 10 years. The construction of 7.7 km (4.8 miles) of line and the conversion of 66 154 t (170 st) trucks were completed in March 1982. System description The conventional 154 t (170 st) diesel electric rear-dump haul-truck is equipped with two direct current wheelmotors in the rear axle. They drive the rear wheels through planetary gearboxes. A 1.2-MV (1600-hp) diesel engine and an alternator is used to power the wheelmotors. The trolley assist system consists of an overhead power line, a current collector mounted on the truck, and add-on control equipment where external electric power is routed by way of the current collector to the motorized wheels. The diesel engine is used to propel the truck on level sections. On inclines, the wheel-motors of the truck are coupled by way of a current collector to the externally generated electrical power supply for propulsion. The diesel engine, having lower output than the wheelmotors, limits the truck performance. Previously, maximum output was required from an engine on inclines. By supplying the wheelmotors from a virtually unlimited outside electric energy source, the truck performance is enhanced to the extent that speed is increased by 46% and fuel consumption reduced by 75%. System design The unique operational requirements of this mine rendered the already proven trolley pole system useless. Minimum loss in truck mobility and flexibility was required. Overhead lines and substations were required to be relatively light and portable. This enables fast repositioning of a line with no loss in material. These unique specifications set the base for the design of the Sishen system. Steel I-beams, 203 x 203 mm x 52 kg/m (8 x 8 in. x 35 lb per ft) with lengths varying from 8.3 to 11.3 m (27 to 37 ft), mounted on base plates about 4 x 2.6 m (13 x 8.6 ft) were used as masts. The masts were erected at about 50 m (165 ft) and the base plates covered with about 12 t (13 st) of iron ore to keep them absolutely stable. Results from the pilot installations indicated that using single cantilever cross arms for insulator and overhead conductor suspension was not practical. The tension in the overhead varied too much with temperature changes, causing too much sag in the lines. There had to be a positive and negative conductor. It was decided to suspend each polarity on an independent cantilever and keep the conductors under constant tension by weights acting through a two-to-one pulley tensioning arrangement. For each overhead conductor, a standard 160-mm (6.5-sq in.) grooved copper railway conductor was used. To obtain the necessary current carrying capacity, two of these conductors are used in parallel per pole. Insulators are rated at 4 kV and are of the composite type. An 11-kV three-phase line, supported by the pantograph line masts on the opposite side of the dc line, feeds the rectifier stations. The rectifier stations consist of an 11-kV to 860-V, three-phase 1.46 MVA transformer and a three-phase diode bridge rectifier. The assurance of future raw materials supply to the EU mineral industry has become, in recent years, one of the most important tasks of the EU Commission. It is focused on creation a new mineral policy within the EU zone. One of the starting points for this activity was identification of the mineral commodities that are critical for the sustainable development and technological progress. The paper presents the glance to this problem from the National Member level. Domestic mineral reserve base as a possible source of critical raw materials for the EU was shown. Poland cannot be considered as its resource base. The source of these raw materials are not only scarce in Poland, but also they are not produced, and their demand is now - and according to forecasts is going to be in the future - met by imports. The raw materials in question are not considered exactly critical for Polish economy, but they are of limited economic importance. On the other hand, their growth is a best opportunity and at once the need to the future sustainable development.

The evaluation of raw materials in terms of quality profile, mineralogical character and mechanical properties is of great importance to the cement industry. The variability of their chemistry and structure is among the decisive design parameters in the choice of any mining, handling and preblending systems. Furthermore, clinker and cement production require sound estimates of the above parameters, to help produce some appropriate and realistic raw mix configurations and achieve minimum standard deviation of the major components. This paper addresses this subject from the perspective of a cement producer and presents the principles of a methodology aimed at optimizing mine planning and process design. Finally, results from case studies on actual sedimentary deposits currently in operation are discussed.

The long-awaited COSMAR study prepared under the auspices of the National Academy of Science reaches one emphatic conclusion that has been enthusiastically seconded by the mining industry: The 1977 federal surface mining act is "poorly suited" as a vehicle to regulate noncoal mining. Though the industry greeted the report with approval, the weight it will carry and the extent of its impact are yet to be felt. The COSMAR report makes no recommendations-that's a job left to the Administration's Council on Environmental Quality-and the prospect of legislation still hovers. For a closer look at the study, MINING ENGINEERING talked to COSMAR committee chairman James Boyd, a well-known presence in the mining arena for over 50 years. After highlighting some of the study's major points, Dr. Boyd comments on the report's impact and possible repercussions. Dr. Boyd, for those readers who haven't seen the COSMAR report, would you summarize some of its major findings? The committee analyzed the surface mining act to determine if it was applicable to noncoal mining and found that there were provisions of the act that either could not be met by existing technology or would be extremely difficult and not necessarily beneficial to adapt. We found that most noncoal mineral mines, despite their obvious diversity, can be considered in two major groups: 93% of all mines move 45% of all materials mined (mostly small units mining construction materials); and the few gigantic regional operations (2%)-copper, iron, phosphate, and uranium mines-that move 50% of the material. With few exceptions, neither of the two groups is amenable to the coal mining practices addressed by the act. The strip mine law was designed for coal mining, where land is regraded, revegetated, and returned to usefulness. In contrast, construction mineral quarries supply local needs for decades, remove most of the material from the immediate vicinity, and cannot be returned to their premining uses (though they have innumerable other valuable uses). The nation's huge mineral deposits require enormous capital investments, are vital to the nation's economy, and can take centuries to exploit. Any attempt to return these locations to premining use would be illogical. Pebble phosphate in Florida and red-bed copper in Oklahoma come near to meeting the conditions contemplated in the act. This paper focuses on the separation and extraction of lead and zinc from Egyptian ore at the Eastern Desert close to Red Sea. The ore sample contains cerussite, galena, hemimorphite and other impurities. Experimental program includes flotationof galena using xanthate collector and cerussite using oleic acid as collector and sodium silicate to depress hemimorphite tailing. Galena, and hemi-morphite are converted to carbonate with sodium carbonate prior to metal extraction. Lead and zinc are extracted pyrometallurgically at 1050°C and750°C or electrochemically respectively. The overall performance of the applied method amounts to 99.2%. Extraction efficiency of lead and zinc amounts to 98% and 94.5 % respectively. A model assumed that galena and cerussite in the ore are partially oxidized and can be liberated by grinding. Oleic acid selectively reacts with cerussite in a multistep sequence. Preliminary cost estimate shows that the price of the products is competitive to their local market prices.

More than ever before, mining projects are besieged from growing regulatory, political, public, and special interest group pressure. Administrative and legal appeals to permits and environmental documents can delay project start-up and add tremendous cost to mineral development. Some mining projects have been canceled given undue external constraints. Successful managers must learn to cope with these constraints. In 1991, Placer Dome Venezuela established a 70% stake in Minera Las Cristinas, to mine the Las Cristinas gold deposits in Bolivar. This case study examines the multi-sectoral partnership arrangement that was developed to improve health services near the proposed Las Cristinas gold mine. The investment climate in Venezuela is uncertain and the socio-economic circumstances are potentially hostile. Despite that, the partnership between the mining company, local communities, government agencies and an international health nongovernmental organization (NGO) collectively leveraged nearly $2.5 million to build and operate a community health center.

Waste disposal underground is the most effective method when viewed from an environmental aspect. As costs of other methods of disposal continue to escalate, the cost effectiveness of underground disposal during the mining cycle becomes increasingly attractive. The use of concrete for sealing abandoned mines has proved to be suspect when associated with a relatively incompetent adjacent strata due to its shrinkage characteristics. Complete failure of the plug has been known to occur in some instances. A more effective, cost competitive seal can be obtained using pneumatically stowed material and incorporating a polyurethane grout, which has greater adhesion characteristics than concrete. Injection of the surrounding strata and of the stowed material helps to ensure an impervious barrier and prevent the plug from moving when subject to high pressure heads. When properly placed, impervious seals cause the old workings to flood, raise the mine pool level, and thus eventually eliminate the potential for oxidation, which is the primary cause of acidity in the water.

The paper presents a geotechnical simulation of the "Mavropigi" open pit mine (Western Macedonia Lignite Center, Greece), using the finite element method. The simulation was based on the initial planning of the pit using Mohr-Coulomb and Soil Hardening models. Results indicate high stability factors as well as the magnitude of the expected horizontal displacements and floor heaving. However, these deformations do not influence decisively the overall stability unless locally adverse tectonics are encountered. Results can be used in the ongoing update of mine planning and stability evaluation.

The theoretical analysis of tumbling mill energetics and performance has largely neglected mill dimensionality, and, in particular, the importance of the length/diameter ratio. This is in spite of the fact that practice varies substantially in this area: geographically, for semi-autogenous and autogenous mills, as between North America and South Africa/Scandinavia; historically, for overflow ball mills, for which the L/D ratio has increased significantly from the earliest small mills to/the largest mills currently. The present study is concerned primarily with the influence of the L/D ratio on the design and operation of overflow ball mills; on the occurrence of the overload phenomenon; and on the limits, if any, it may impose on mill capacities because of critical pulp axial velocity limits. It will be shown that the shape face or is of major importance in this area; and that its adjustment to the extent that this is practical should remove the diameter limitations previously postulated for this mill type.
