The underground industry is challenged by many complex technical and non-technical issues and risks. The most obvious of these are the traditional technical issues that we usually try to address with risk management techniques. However, there are many other non-technical risks that our projects face on a daily basis which we often fail to fully recognize or appreciate. This paper addresses some of the less-technical broad-based risks in order to put them into perspective with the technical risks we face day to day in underground planning, design and construction. The good news is that Risk Management techniques are not always about bad things. For obvious reasons Risk Management is always thought of in terms of the ?downside? of risk (i.e., the identification and mitigation of things that can go wrong). On the contrary, there is an ?upside? of things that might go better than expected. Fortunately, if a deliberate effort is made, the same principles and techniques that our industry uses in Risk Management can be applied toward identifying and quantifying the Opportunities and Benefits that might occur. In this way, the process of conducting risk management programs can not only identify the downside risk but can be used to identify and direct projects to take advantage of the upside of uncertainty (i.e., Opportunities as they arise). This beneficial aspect of risk management programs will be highlighted in this paper together with the importance of the non-technical issues.

INTRODUCTION The Bunker Hill mine, located at Kellogg, ID, in the Coeur d'Alene mining district, has been a major pro¬ducer of lead-zinc-silver ore since 1887. Since its dis¬covery, it has produced over 25 401 171 t (28 million st) of ore from 20 to 25 veins and ore zones. The mine now covers an area of approximately 1.609 km (1 mile) in length and 0.8 km (1/2 mile) in width and extends from 1097 m (3600 ft) above sea level to 488 m (1600 ft) below sea level. Production is presently at a rate of 1542 t/d (1700 stpd). During 1965, a new stoping method, the Bunker Hill pillar system, was developed to enable the profitable mining of large, low-grade zinc ore bodies. The system was specifically designed to in¬corporate the Atlas Copco T26GH rubber-tired loaders. High productivities were achieved, and the method was adapted for mining the higher grade stopes where large timber square sets are utilized. This results in nearly 100% extraction. GEOLOGY The area encompassing the Bunker Hill mine lies immediately south of the Osburn fault, a major east-west striking, deep-seated strike slip fault which is the dominant structural feature in the Coeur d'Alene district. The Bunker Hill forms a series of veins and mineralized zones which occur as link structures, tension fractures, and breccia zones in folded, interbedded argillaceous quartzites of the Revett and St. Regis formations of the Precambrian belt series. The Rosco ore body is predominantly a zinc ore body above the Cate fault, which is presently known to occur from the 13 level. [elevation 488 m (1600 ft)] to the 1600 level [elevation 304 m (1000 ft) ]. These ore bodies are stringer zones with at least one dominant 1.8 to 4.5 m (6 to 15 ft) wide band of ore. They dip 0.09 to 0.26 rad (5° to 15°) steeper than the local 0.61 rad (35°) dip of the Cate fault, and also deviate 0.26 to 0.52 rad (15° to 30°) in strike from the Cate. Because of the flat dip and fractured weak hanging wall, normal cut-and-fill mining has not proven practical in this ore body. The mechanized timber methods described in the next section and the Bunker Hill pillar system are the mining methods successfully used. The Devil?s Slide portion of Highway 1 between Pacifica and Montara is often closed due to the instability of the adjacent slopes. Following many years of public input and careful evaluation of alternatives, Devil?s Slide will be by-passed via two inland tunnels, providing a safe, dependable highway between Pacifica and Montara. For the twin tunnels, each 9m diameter tube will accommodate only one traffic lane. Each tunnel tube will accommodate a 3.6 meter roadway, 0.6 meter and 2.4 meter inside and outside shoulders respectively, and 1.2 meter walkways on each side. Cross passages will connect the bores at about 120 meter intervals and jet fans will provide tunnel ventilation. The twin tunnels are approximately 1,250 meter long and 9 meter wide. Excavation will progress from south to north using tunneling techniques that rely on inherent rock strength for support known as the New Austrian Tunneling Method (NATM). Both tunnels will be excavated at the same time, one tunnel face approximately 40 meters ahead of the other. Staggering the tunnels reduces the likelihood of damage from blasting in alternate tunnels. The work schedule is 24 hours a day, 7 days a week resulting in a 24-month timetable to break through to the north portal.

Federal Regulations require that coal producers conduct a written, in-house investigation with every lost time injury accident. Besides their obvious value in accident prevention, competently conducted investigations can provide a number of additional benefits to include positive supervisor involvement, the control of rising worker's compensation costs and a measure of protection from civil/criminal proceedings. The paper describes such topics as MSHA minimum requirements, investigator selection and training, accident site procedures, witness interview techniques and the management review process. The reader is also presented with a number of information resources available through MSHA, the National Safety Council and other sources.

Coal waste materials inhibit direct vegetation establishment due to adverse physical and chemical properties, particularly low water retention and high potential acidity. The Moss # I coal refuse pile is located in Dickenson County, Virginia, and was idled in the late 1980&apos;s with little topsoil resource available for final closure. The refuse was acidic (Total-S = .. 38%; pH = 3.6), black, high (70%) in coarse fragments, and had a low water holding capacity (4.5% in < 2.0 mm fraction). A small plot experiment was established on the refuse pile to evaluate the influence of liming rates (50% and 100% of lime req.) and topsoil thickness (15, 30 and 60 cm) on vegetative growth and leachate quality. Liming and topsoil amendment increased the surface soil pH from < 4.0 to > 6.0 over a two-year period, which resulted in greater vegetative cover and biomass than the control plots. All topsoil treatments resulted in greater vegetative cover and biomass than plots treated with lime only due to improved surface soil physical and chemical properties. A topsoil treatment of 60 cm gave the thickest vegetative cover and biomass yield. Such a treatment, however, would be cost-prohibitive at this location. Application of 27 Mg ha-1 of lime to the refuse surface along with 15 cm of topsoil produced acceptable two-year vegetative cover and biomass, and appeared to be the optimal treatment for this particular situation. Both liming and topsoil had no affect on leachate pH and the electrical conductivity in leachates collected below the plots. This suggests that surface revegetation will have little effect on the quality of water draining through the pile, so long term water treatment requirements may not be reduced by successfully revegetating the pile surface.

Illinois No. 6 coals of IBC 101 and IBC 112 were treated in solution with sodium hypochlorite to remove their sulfur contents, especially organic sulfur. The lab experiments were conducted in two steps of leaching at room temperature and hydrolysis at 90oC. The removal of organic sulfur from IBC 101 was by 37.8% while that from IBC 112 was only 13.7% under their respective optimum conditions. This large difference in organic sulfur removal is considered to be due to difference in concentration of surface total acid group, 3.14 vs. 0.88 mmol/g of coal.

Underground mining of limestone is becoming increasingly attractive in Illinois. Most quarries are near rivers on the western and southern borders where competition for land use is intensive. Increasing demands for stone can be met by expanded mining with access by edits in existing quarries and by inclines and shafts at downdip sites. Geologic, hydrologic, and engineering conditions indicate that deep mines in the Ste. Genevieve, St. Louis, Salem, Ullin, and basal Burlington Limestones of Mississippian age and in the Moredock Limestone Member of the Dunleith Formation of Ordovician age are possible, but development will require prudent exploration and planning to ensure that the product will be of good quality.

Research on the low-potential hydrophobic state of pyrite with potassium amyl xanthate (PAX) as collector, is reported to help explain and understand the fundamental aspects of the N2TEC flotation process, which was originally developed for the improved recovery of gold from refractory auriferous sulfide ores. Typically in the N2TEC process, air is replaced by nitrogen as the flotation gas, and potassium amyl xanthate is used as collector for the reactive auriferous pyrite particles. The first N2TEC flotation process was commissioned at Lone Tree in Nevada in March 1997 and has been in operation ever since. On the basis of electrochemically controlled contact angle measurements, the low-potential, low-pH hydrophobic state of pyrite has been examined as a function of flotation variables such as pH, gas-phase composition, collector concentration and pyrite potential.

The U.S. cement industry experienced its best year ever during 2004 in terms of production and shipments of cement. Clinker, portland and masonry cements production and shipments were up considerably. Clinker production as reported to the U.S. Geological Survey (USGS) was 87.75 Mt (96.73 million st), an increase of 5.4 percent. Gray and white portland cement production from domestic and imported clinker was estimated to be 91.6 Mt (101 million st), up 5 Mt (5.5 million st) from 2003.

The market for titanium feedstocks (ilmenite, rutile,synthetic rutile and slag) is driven primarily by offtake from the titanium dioxide (TiO2) pigment industry. The pigment sector depends on the performance of the global economy. Titanium sponge is the first stage of titanium metal manufacture. It constitutes a minor component (3 percent) of overall demand for titanium feed-stocks. Production and consumption TiO2 pigment consumption in 2004 was estimated to be 4.63 Mt (5.1 million st), up 7.6 percent from 2003. It was driven by strong demand in most regions. In the United States, apparent TiO2 pigment consumption rebounded by 5.4 percent (highest growth rate since 1997) to an estimated 1.12 Mt (1.23 million st), following the decrease in demand during 2003. Chinese TiO2 pigment consumption continued its strong upward trend, increasing by 10 percent to 598 kt (659,000 st) in 2004.

The Basalt Waste Isolation Project of the U.S. Department of Energy is a part of the National Was re Terminal Storage Program. Its mission is to assess the feasibility of siting a nuclear waste repository in the basalt formations which underlie the Hanford Site of the U.S. Department of Energy. The progress in all program areas is summarized in this paper. Activities conducted to date have led to the identification of possible repository candidate sites within the Hanford Site.

Adverse water-quality impacts of uranium mining and milling activities on both surface- and ground-water resources in the Grants Mineral Belt have recently been documented.1 In some cases, the impacts resulted in violation of existing regulations and license conditions and have endangered public health. Unfortunately, industry-sponsored ground-water monitoring programs do not adequately record the actual impacts of mining and milling on ground-water quality.

A Method to Calculate Hydrodynamic Loads on Underground Bulkheads Bulkheads are structures built in openings in underground mines to impound water, preventing the water from entering the active mine workings. These structures not only have to be designed to withstand the static, hydraulic pressure, but also the dynamic or seismic pressure, which would be generated by an earthquake. Bulkheads can be considered underground dams. Unlike dams on the surface, bulkheads are often subjected to hydraulic heads that exceed the heights of the bulkheads. Theodor von Karman in 1933, and Westergaard earlier, developed the equation by which the hydrodynamic pressures on surface dams could be calculated. Civil engineering handbooks of today contain this equation. In this paper, using the methodology developed by von Karman and Westergaard, the authors have developed an equation by which the hydrodynamic forces on bulkheads could be calculated when the hydraulic head exceeds the height of the bulkhead.

Under a DOE-funded project, four advanced flotation technologies (the Jameson cell, the Outokumpu HG Tank Cell, an open column and a packed column) were evaluated at the benchscale to identify the technology or combination of technologies that best improved fine coal recovery and reduced processing costs. The results indicated that two-stage application of the Jameson and packed column cells provided the optimal performance. Subsequently, proof-of-concept scale testing was conducted at the Cyprus Emerald Coal Preparation Plant. The results confirmed that levelized production costs were lower using two-stage cleaning with the Jameson cell followed by the packed column.

The support of the roof in coal mines remains a critical safety issue with 3 fatalities from roof falls in 2007 and 430 injuries from roof falls in 2006. A snapshot survey by the authors of room design and support details as well as unplanned falls of ground in the industry revealed some useful data concerning support use, seam details, fall dimensions, bolt lengths and spacing and the stand-up time.

The European climate and energy framework for 2030 is highlighting the reduction of man-made CO2 by increasing the use of renewable energy to mitigate climate change. In this spirit, Germany has decided to erase the coal use as an energy source by 2038 completely. Inevitably, this strategy promises a feasible, responsible, and sustainable post-mining management, because 200 years of coal mining have created numerous challenges necessary to tend to once the mining ends. Thus, the Research Institute for Post- Mining (FZN) at the TH Georg Agricola University in Bochum, Germany, has developed a holistic approach to meet these postmining challenges including further expertise present at our university. Here, the four strategic research pillars are (1) technical geoecology and hydrogeology to avoid and mitigate risks underground, (2) geomonitoring to evaluate and protect the surface, (3) industrial heritage preservation and engineering to secure and reuse brownfields such as mine sites, and (4) economic transformation planning of mining regions to enable a prosper future for generations to come. In all efforts, we aim to achieve the fundamental goals of the UN to deal with the resources of our planet responsibly and sustainably. Furthermore, we are synergizing increasingly with national and international networks in the interest of social, economic, and environmental stakeholders.

An 8.5-km (5-mile) conveyor has gone into operation in western Turkey to eco¬nomically transport crushed lignite from the mine area to a power plant. This conveyor has replaced trucks as the principal mode of transporting the lignite. One principal element of Turkish govern¬ment policy is the country&apos;s commitment to increase electricity supplies for mining and other industries. Lignite is an important source of electric power supply in Turkey. Production was about 48 Mt (53 million st) in 1987, amounting to more than 20% of the total power supply in Turkey. In the western Anatolia supply network, it plays a major role for power generation. Deposits are located in the Soma region of western Turkey. The lignite is of low quality, deposits are scattered, and the terrain is dif¬ficult and hilly. These conditions demand an economical and technically feasible method of transporting the raw material from mining areas to the central thermal power plant. The Soma power plant is located about 120 km (75 miles) northeast of Izmir, near the coast of Aegean Sea. An overland conveyor of a unique design has gone into commercial operation at the Soma power plant facilities of the Turkish state electricity organization, Türkiye El¬ektrik Kurumu (TEK). The conveyor re¬places truck transport to ensure economical use of the lignite deposits. Computer-aided design (CAD) added more precision, qual¬ity, and flexibility of engineering in design activities. It also helped quick realization of the project that was executed in cooperation with the German company Krupp Industri¬etechnik and the Turkish firm Gama Endüstri AS. Unique features The conveyor includes several major features rarely combined in a single flight: Overland long distance conveyor. • Horizontal curves. -Steep vertical concave and convex curves to match the difficult terrain. Down-hill conveying. Frequency converter drives with vari¬ able belt speed. Generating operation mode to backfeed the energy to the network. Layout The 8.5 km (5 mile) long conveyor takes the -200 mm (-8 in.) lignite from the primary crushing plant located near the mine to the central, final crushing plant located in the stockyard area near the power station. The drive and belt tensioning assemblies are situated near the tail end of the flight. The high voltage switchgear is housed in a room in the primary crushing plant build¬ing upstream from the conveyor flight. The frequency converters are installed in the low voltage switchgear room located below the control room within the primary crushing plant building. Smooth roll crushers are widely used as secondary crushers, operating most effectively when set for size reductions in a ratio between 3 or 4 to I. The feed supplied to these units varies from 13 to 102 mm in size and the product obtained ranges from 13 mm to about 20 mesh. Smooth roll crushers produce very few fines and absolutely no oversize. The diameter otfhe rolls varies with the clearance between them, feed size, and the coefficient of internal friction ofmaterial to be crushed, Detailed calculations of this relationship may be found in Unit operations of Chemical Engineering, (McCabe, W. The objective was to determine operating protocols for a lead and silver column flotation cleaner, being considered for installation at Sunshine Mining & Refining Co.’s Kellogg mill in Northern Idaho. In this column, tetrahedrite and galena were floated, and pyrite and other gangue minerals, including siderite and silica, were depressed. The procedure was to develop a dynamic simulation using a linear transfer function approach. The transfer function related cause and effect variables such as the influence of collector and depressant dosages on recovery and grades. Bench-scale flotation tests followed by pilot column testing provided the data used to develop a linear transfer function. The transfer function was derived using a spreadsheet. This model was applied to simulate how the flotation column could be made to respond to disturbances in feedgrade. The results were that xanthate, zinc sulfate, and sodium cyanide were found to be suitable controls for grade and recovery. Simulations showed the best sequence of manipulations of these reagents, and these sequences were useful for future operator training. The conclusion was that linear transfer functions were able to simulate lead and silver recovery, develop scenarios for operator training, and provide a foundation for future on-line control.

As part of ongoing deep-ocean mining research, hydraulic transport systems as well as status of deep-ocean mining systems are reviewed in the aspects of manganese nodules and crusts from 8006,000-m depth. The emphasis of the investigation of hydraulic transport is placed on the shape effect of particles on the pressure gradients and flowrates. Series of tests are being conducted to study the lift characteristics of a 2-phasevertically upward hydraulic transport of 2 types of small solid particles in a PVC pipe of 1-in (2.54 cm) diameter: irregular-shaped silica sands and equivalent spherical glass beads. Pressure drops and mass as well asvolumetric flow rates were measured by electromagnetic(EM) flowmeter and mass flowmeter. The particle sizes used are 8-10 mesh. The results in turbulent flow range of 101045Re show that pressure gradients or friction factors are smaller for the spherical beads than the sands. The larger the concentration of the particles are, the larger the pressure gradients become. The larger the particle concentration is, the less the effect of particle shape on pressure gradients becomes. The shape effect is greater for the minimum particle transport velocities than for the settling velocities.
