Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

The CANALSIDE commercial development in Buffalo, New York serves to revitalize the former site of the Buffalo Memorial Auditorium, adjacent to Lake Erie. The Auditorium was constructed in 1940 and founded on steel H-piles driven to limestone bedrock. After decades of professional sport and entertainment events, the “Aud,” as it was locally known, was closed in 1996. It sat vacant until it was demolished in 2009, at which time plans were made to revitalize the area with shops, restaurants, and a historic replica canal. The only portion of the Aud that was left intact was a subbasement boiler room. The subbasement was renovated in 2014 and has since been used to house water and ice making equipment for the historic replica canals, which boast recreational ice skating in the winter. The subbasement floor was about 10 feet below groundwater and the Lake Erie water level. The floor consisted of an approximately 4-foot-thick layered inverted waffle slab system, with reinforced concrete grade beams and concrete infill between the grade beams. A waterproofing membrane was located near the base of the slab. There were no major seepage issues in the subbasement, so the existing waterproofing membrane was assumed to be intact and performing adequately. A four-story building housing a children’s museum was proposed to be constructed directly above the subbasement. The loads imparted by the new building would exceed the capacity of the existing piles, so it was determined that drilling micropiles through the subbasement floor would be the most practical means of supporting the new building. Drilling through the existing slab and waterproofing system was a concern due to the challenges of controlling water during construction and the importance of maintaining the integrity of the permanent waterproofing system.

The end of the war in Europe and the Pacific failed to curb the upward trend of production in Venezuela,- which easily surpassed that of any previous year. The total production for 1945 amounted- to 323,369, 430 bbl., which was 26 per cent over the total for 1944, the previous peak in the history of the oil industry of the country. This increase was caused mainly by the greater demand for petroleum in the last stages of the war; the fact that the danger from enemy submarines had been overcome, which allowed all oil companies to use increased shipping facilities, and the fact that more materials were available for drilling operations. However, the materials supplied were by no means adequate and considerable credit is reflected on the Venezuelan petroleum industry for the increased production obtained by utilizing all available equipment to its fullest extent.

INTRODUCTION This paper summarizes the result of a two and a half year research program. The objective of the research was to develop a fundamental understanding of the Rib-in-Roc method. Through the development of a new model technology, namely three-dimensional physical models which powerfully exploit the results of dimensional analysis Stillborg, Stephansson and Swan (1), it has been possible to establish the contribution to both permanent and immediate stabilization of a large underground opening pre-reinforced by the Rib-in-Roc method. Using the results from these physical models and earlier work by Stillborg and Stephansson (2) and Åkesson (3), a conceptual synthesis is given in this paper so demonstrating the Rib-in-Roc method to be a practical working system of pre-reinforcement. DESCRIPTION OF THE RIB-IN-ROC METHOD To give the reader an idea of how this method of pre-reinforcement is performed, a short description will be given, Figure 1. Before any full-scale excavation takes place an entry is made in the host rock and rib raises are driven around the proposed opening. These raises are made at a spacing that is dependent upon rock quality; an opening in poor rock needs more ribs per length of opening. As the ribs are driven they are used as a site from which very competent rock bolting can be done. As a rib is excavated it is filled with reinforced concrete. Finally after having in this way reinforced the zone around the proposed opening, (a reinforced arch is formed) the displacements are automatically controlled as the opening is being excavated. Using this method openings with a span of 50-60 meters (160-200 ft) and a height of 60-80 meters (200-260 ft) are likely to be realised.

This Bureau of Mines report presents a computer code that has been developed for describing and testing the statistical distribution of general continuous data sets. The statistical distributions used include the normal, lognormal, exponential, chi square, gamma, and uniform. Histograms for observed and expected values are plotted for each distribution. An example using geologic data obtained from a study of the geometric properties of natural fractures in oil shale rock illustrates the use of the computer program, GDIST.

This research investigates roof deformation and failure characteristics, as well as mechanical interaction between the panels of the roof strata above two adjacent working faces in a steeply inclined coal seam. Physical modeling with photogrammetry measurement method has been employed in this investigation. The movement of roof strata and section pillar, as well as the stratum morphological characteristics, was quantitatively analyzed. Results show that the roof movement direction above the two panels has zoning characteristics, especially in the lower panel. Roof migration in the lower panel of the working face is larger than that in the upper panel. The roof to floor closure perpendicular to the seam direction is larger than the sliding along the seam. The maximum roof concave and convex bending occurred at the middle and upper parts of both panels; the roof bending in the upper panel mining area indicates a larger mining disturbance induced by lower panel mining. The void ratio contour of the lower panel is an asymmetrically arched shape, which is different from that of the upper panel area. The higher roof strata above the upper panel exhibit separation and breaking, whereas the lower roof exhibits compression due to mining in lower panel. "Explosives used in blasting operations, natural and anthropogenic sources such as septic systems,fertilizers, and decomposing vegetation can potentially contaminate groundwater with nitrate in thevicinity of construction sites and make identification of blasting impacts difficult. Blasting operationsfor a private construction project in Windham, New Hampshire were indicated as the source of water quality impacts to private drinking water wells prompting the New Hampshire Department ofTransportation (NHDOT) to implement a proactive approach to limit the potential for impacts fromblasting for ongoing NHDOT projects. NHDOT has developed a baseline drinking water monitoringprogram designed to detect potential impacts and to ensure alternative drinking water is providedthroughout the construction phase of projects. In 2013, the U.S. Geological Survey (USGS) andNHDOT began a study to determine the source and fate of nitrogen compounds near blasting sites using a combination of isotopic, time series, geochemical, hydrologic, and geologic data. Approximately 1.75 million cubic yards (1.33 cubic meters) of rock were removed by blasting in several locations for roadway construction at a major highway construction site in southern New Hampshire."

Direct transfer, or community translocation, comprises salvage and replacement of intact æsodsÆ of vegetation together with underlying soil. It has been applied to New Zealand mine sites restoration of native grasslands, wetlands, scrub lands, and forest. It is an equivalent technique to æinstant lawn or tree gardenÆ, commonly used in urban landscaping. Two to six m2 scoops of vegetation, with up to 0.7 m of soil attached, and including root plates of trees and shrubs, are relocated from stripped areas ahead of mining to sites prepared for restoration. Direct transfer uses hydraulic excavators or face shovels and dump trucks, possibly with the aid of bulldozers and other earthmoving machinery. The main advantages of direct transfer, compared with sowing or planting systems for mine site restoration, are the recycling of plant and soil materials which were often wasted by burying under overburden materials, a significantly faster revegetation process, and restoration of the whole ecosystem (plants, animals, birds, invertebrates, and microbes). The main disadvantages are higher costs of stripping and dumping, a greater degree of machine operator skill required, and scheduling becomes critically important. To illustrate the key principles, this paper gives examples of successes and failures of the technique for a variety of New Zealand mining situations. Direct transfer has been successful for a range of communities, from fern wetland and sub-alpine tussock grasslands to 5 m-tall beech forest under storey and seral shrub land, and from temperate to sub-alpine climates. Successful examples show both rapid recovery of indigenous vegetative cover and conservation of habitat for birds and invertebrates. Failures have resulted from excessive death of plants because of inadequate soil salvage depths, acidic leachates, inappropriate handling of stripped sods by inexperienced machinery operators, or prolonged drought conditions immediately following relocation. Change in plant species composition has occurred where clumps of vegetation are too far apart, or shifted plants were too small - in both cases invasive weeds have smothered the original vegetation. Changes in hydrology can also have significant impacts. Our experience indicates that the advantages of direct transfer far outweigh the disadvantages and is now used as the preferred restoration method, where practicable, at mines where it has been trialled.

The lead-acid battery recycling is a major industrial activity (more than four million tonnes recycled lead per year) that generates large amounts of toxic wastes such as lead slag, mattes and acidic sludge, besides airborne emissions. This is a waste-producer industry that needs a definitive solution to avoid its negative environmental impact and getting a sustainable lead production. That is the principal target of the CLEANLEAD technology, which properly combines both hydrometallurgical and pyrometallurgical routes aiming to get zero-waste production, reducing the use of energy and resources in all phases of battery reprocessing, and consequently, greatly decreasing the operating cost. There will be no toxic emissions to land, water or air, and obtained products are pure lead metal or pure lead oxide, and commercial gypsum instead of waste sludge.

"INTRODUCTION The injection of water into partly depleted-oil-bearing formations as a means of supplying additional energy to flow oil wells now is recognized by the petroleum industry as an effective method of increasing the recovery of oil. The benefits to be derived from water-flooding oil sands were discovered accidentally more than 4o years ago, when oil producers in the Pennsylvania and western New York fields observed suustantial increases, in oil-production rates when water leaked past corroded casings of old wells into producing sands. This observation prompted some operators purposely to perforate or slit the casings opposite the producing formation in some of their wells to permit water to enter the formation. However, as many operators viewed the practice with skepticism and considered it to be in violation of oil-conservation principles,- systematic water-flooding was not initiated until the Pennsylvania legislature, in 1921, enacted a law authorizing water-flooding of oil sands in that State.Although water-flooding has been practiced in a haphazard manner for more than 25 years in Chautauqua and Montgomery Counties, Kansas, planned or systematic water-flooding in that State had its inception in 1935, when the legislature, following the example of Pennsylvania, also passed a lava/ authorizing water-flooding. The early operations and, to some extent, those still, practiced in the Chautauqua-Montgomery area consisted in raising or perforating the casing to allow water from upper strata to gravitate into underlying oil-producing formations. The extent to which this type of water-flooding affected petroleum production in the old stripper area of eastern Kansas is not known, but it is known to have increased oil production ap¬preciably from many leases.Following enactment of the law permitting water-flooding in Kansas, three operators inaugurated, in that same year, three organized flooding projects in three different counties of the southeastern part of the State. The first organized project on record was that of the York State Oil Co. in the Seeley pool, Greenwood County. On this project, the injection was started on May 15, 1935, into the Bartlesville sandstone at a top depth of 1940 feet."

There are several methods (empirical, statistical, and machine learning tools) to calculate blast-induced ground vibration. Still, due to complications in the blasting procedure, under varying blasting loads and strata conditions, it is not easy to predict and simulate blast vibration precisely. Therefore, a study was conducted to simulate the field blast data in Ansys software using explicit dynamics. For this study, monitoring of ground vibration for 20 single hole blasts was carried out. The density of emulsion explosives was in the range of 0.9 to 1.15 g/cc. In the laboratory, rock properties were determined from the rock sample collected from the mine site before blasting. The in-hole velocity of detonation (VoD) and the density of the emulsion explosive were also determined. These parameters were used for numerical analysis to know the ground vibrations at specified points. The results of ground vibration predicted by the model are validated using field data.

This paper discusses how elemental analysis of conveyed bulk flows in real time using PGNAA (Prompt Gamma Neutron Activation Analysis) technology has resulted in significant improvements in advancing plant control in mineral processing operations. Basis of operation of the PGNAA through belt, full stream, continuous, multi-elemental, real time analysis technology is explained. Capabilities and limitations in plant operating environments are discussed including comparison with other real time, online technologies through review of published information available for on-conveyor minerals applications. Case studies demonstrate benefits provided by this technology in two operating mines. An iron ore company had prior experience with the PGNAA analyser technology, implementing an early model in a minerals application in 2003 to develop confidence in its measurement performance and usefulness of the data. A number of analysers were subsequently included throughout the new plant design for extensive measurement and control of conveyed bulk flows, from conveying after mining through to rail load-out. A copper-gold company installed an analyser to address copper ore blending issues where an on-belt system was the only viable option for the degree of control required. Analyser suitability was evaluated through test work on site samples to determine customised multi-element precision guarantees for each application. Analyser results from normal operation are compared against laboratory analyses of multiple cross stream samples collected from conveyed flows to assess performance. Both projects discussed were able to realise benefits previously unattainable for plant quality control, surpassing capabilities of traditional technologies which were either unsuitable for measuring conveyed flows with adequate accuracy or in real time. Equipment payback within a few months was achieved for both sites. Through proven performance in these applications, the companies consider the technology an essential component in advanced plant control necessary in new process flow sheet designs, rather than an optimisation tool.

Yielding rock bolt support systems have been developed to accommodate ground movement in shifting ground such as in coal operations; in creeping ground such as salt, trona, and potash; and in swelling ground associated with some clays. These systems, designed to remain intact despite ground movement, should enhance mine safety and help contain costs in areas where rebolting of rigid non-yielding systems is typically required. Four such systems were tested in straight tensile pulls in the laboratory. They include the Slip Nut System from Dywidag Systems International USA, Inc., Ischebeck's bolt mounted Titan Load Indicator, Rocky Mountain Bolt Company's Yielding Cable Bolt, and a rock bolt installed variation of the yielding steel post developed by RE/SPEC Inc. The first two systems are currently marketed products and the latter two are prototype systems. Each system responds to load and displacement by yielding in a unique manner. All are designed to yield at predetermined loads. A description of each system and its yield function is provided. Each system was tested over its prescribed yield range in a test machine. At least five tests were performed on each system. Each system yielded and continued to provide support according to its design. Each shows promise for ground control use in shifting or creeping rock. This work helps to illustrate the comparative differences in performance between these specialized systems and the applications where they may be most useful.

Cyanide leaching of two West African gold ores was studied in multiple laboratories to provide process-engineering data for plant design purposes. Unusually slow leach extraction kinetics were observed using routine cyanide leach test procedures with pregnant solution monitoring. Carbon-in-leach testing, however, showed normal rapid kinetic behavior, with the same ultimate gold extractions being reached in much shorter elapsed times. Kinetic curves based on washed solids assays also exhibited more typical leaching characteristics. The results bore all the hallmarks of some equilibrium inhibition of the leaching. It was surmised that ore gangue components were responsible for weakly adsorbing gold-cyanide complex reaction product, thereby holding up soluble gold in the solid phase. The term ?preg-borrowing? was coined to describe the reversible phenomenon and distinguish it from irreversible preg-robbing. The two ores investigated may represent an extreme form of a more general effect that in most instances might pass unnoticed. The effect can lead to misinterpretation of kinetic leach curve data and underestimation of calculated ore head grades.

Asbestos crystallizes within rock formations undergoing intense deformation characterized by folding, faulting, shearing, and dilation. Some of these conditions have prevailed during formation of the taconite iron ore deposits in the eastern Mesabi Iron Range of Minnesota. This range includes the Peter Mitchell Taconite Mine at Babbitt, Minnesota. The mine pit is over 8 miles long, up to 1 mile wide. Fifty three samples were collected from 30 sites within areas of the pit where faulting, shearing and folding occur and where fibrous minerals might occur. Eight samples from seven collecting sites contain significant amounts of ferroactinolite amphibole that is partially to completely altered to fibrous ferroactinolite. Two samples from two other sites contain ferroactinolite degraded to ropy masses of fibers consisting mostly of ferrian sepiolite as defined by X-ray diffraction and TEM and SEM X-ray spectral analysis. Samples from five other sites contain unaltered amphiboles, however some of these samples also contain a very small number of fiber bundles composed of mixtures of grunerite, ferroactinolite, and ferrian sepiolite. It is proposed that the alteration of the amphiboles was caused by reaction with water-rich acidic fluids that moved through the mine faults and shear zones. The fibrous amphiboles and ferrian sepiolite collected at the Peter Mitchell Mine composes a tiny fraction of one percent of the total rock mass of this taconite deposit; an even a smaller amount of these mineral fragments enter the ambient air during mining and milling. These fibrous minerals thus do not present a significant health hazard to the miners nor to those non-occupationally exposed. No asbestos of any type was found in the mine pit. © 2007 Elsevier Inc. Experimental studies for quantification of force and brittleness indices relevant to rock cutting performance are presented. A simple interpretation method is developed, which quantifies the force-penetration response obtained from indentation tests. Basic parameters of the quantification are force increment and decrementrates, force increment and decrement periods, and number of increment and decrement data points. Blocks of eleven different rock samples were collected from different operating mines in Turkey and subjected to indentation tests to calculate the indices. The samples included harzburgite, serpentinite, trona, limestone, claystone, two different sandstone samples, siltstone and three different chromite ore samples. The concept was validated by a set of full-scale linear cutting tests using a conical cutter for measuring the cutting performance in relieved and unrelieved conditions, i.e. cutter forces, specific energy, size distribution of rock cuttings. The rock cuttings were sieved to determine coarseness index, indicates coarse chip generation characteristics, and size distribution. The force index is moderately correlated with the cutting performance and mechanical properties of the rocks, and poorly correlated with the coarseness index and size distribution of cuttings. A general and moderate trend is observed between the brittleness index and the coarseness index, cutting performance for unrelieved cutting conditions and mechanical properties of the rocks. However, additional data is required to fill the data gaps and to potentially establish relationship between the brittleness index and all other parameters.
