"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

In this investigation, different mixtures of realgar and orpiment particles were floated in a laboratory batch flotation cell and multivariate image analysis was used to estimate the arsenic content in the froths. The analysis was based on the froth colour, as well as extraction of three groups of textural features, namely those based on grey level co-occurrence matrices (GLCMs), wavelets and local binary patterns (LBPs). Collectively, these features provided better information on the arsenic content of the froths than any one of the individual groups of features. Partial least squares models could explain approximately 78% of the variance in the arsenic by using all the features combined. The colour content, particularly the green component in the red-green-blue (RGB) features, provided almost as much information as each of the three sets of texture-base features individually.

Sapping and mining have constituted an important part of siege operations ever since powder came into general use in Europe. Before this period, famine was the main weapon in the hands of the besiegers, and the history of the middle ages is replete with accounts of sieges lasting two and three years. With the advent of explosives, a position impregnable to assault might be readily won by mining under, and blowing up, its defences. The natural offensive against this method of attack is to countermine and stop the attackers underground before their objective is reached. Thus developed the art of military mining. The French and British, in the Peninsular war, were adepts at this method of warfare, and the explosion of a mine and the formation of a breach were the usual preludes to the general assault of a position. In the Crimea, at Sebastopol, the British and Russians did a lot of underground fighting, as the old maps of their galleries, now in the museum at Chatham, will testify. In the siege of Port Arthur, in still later times, the Japanese and Russians were hard at it as well.

The increasing demand for pure aluminum and aluminum compounds of industrial quality from kaolinite ore cannot be overemphasized. Nigeria is one of the African countries endowed with abundant solid mineral resources that have not been sufficiently exploited to assist its indigenous industries. A wide array of applications of pure aluminum and its compounds are available, such as paper filling, refractories, adsorbent, catalysis, and paint additives. In this study, the upgrading of a Nigerian biotite-rich kaolinite ore by a hydrometallurgical route was investigated in oxalic acid media. During leaching studies, the effects of parameters including reaction temperature, lixiviant concentration and particle size on the extent of ore dissolution were examined. At optimal conditions (1.0 mol/L C2H2O4, 75 °C), 92.0% of the initial 10 g/L ore was reacted within 120 min. The dissolution curves from the shrinking core model were analyzed and found to conform to the assumption of surface diffusion reaction, and the calculated activation energy of 33.2 kJ/mol supported the proposed model. The unreacted product (~8.0%) analyzed by XRD was found to contain siliceous impurities and could serve as a valuable by-product for certain industries.

Most U.S. coal production is bituminous coal. Some 90% of current production comes from a geographical area enclosed in a circle with a 300 mile radius and its center at about Huntington, West Virginia. There are important underground coal production areas in the western parts of the United States as well. Most U.S. coal production comes from fairly level seams, many of which pitch rather uniformly only a few feet per mile. Usually there will be several overlying seams of varying thicknesses, separated by several tens of feet. Fig. 1. 1. Seams have minor local undulations but these? are small enough not to be a major consideration in a horizontal drilling research program.

A Reconnaissance of the manganese- and chrome-ore deposits of Cuba was made by the writer, as a representative of the U. S. Geological Survey, in company with Mr. Albert Burch of the Bureau of Mines, under instructions of the Secretary of the Interior, in the spring of 1918. The Cuban Government courteously detailed Sr. E. I. Montoulieu, a mining engineer connected with the Treasury Department, to act as escort and associate throughout the work on the island. The object of the study was to obtain authentic information for use of the United States Government as to the location, character, quantity, and availability of the manganese- and chrome-ore deposits in Cuba. Accordingly, the work comprised examination of such deposits as seemed to be of promise, without regard to stage of development, with a view to a determination of the quantity and quality of ore likely to become available for shipment during 1918 and 1919, and of the tonnage of ore in reserve. Wile it was possible to visit most of the deposits that seemed to be of promise, there was not sufficient time to visit all that were brought to our attention, some of which may have merit. Moreover, the necessity for paying particular attention to the ore deposits and the mines precluded the possibility of making more general studies of the geology of the region, and of making satisfactory collections of country rock and of fossils, and so for other details as to the geography and geology of Cuba the reader is referred to the report by Hayes, Vaughan, and Spencer.l Mr. Burch returned to the United States about 10 days in advance of the writer, who continued, in company with Sr. Montoulieu, investigations of reported deposits in Pinar del Rio, Matanzas, and Santa Clara Provinces. This paper uses a case study to demonstrate best practices techniques for predicting the contribution of gravity concentration in a gold recovery process flowsheet. The case study was selected as an example of an ore with a marginal gravity gold content, which makes the value of a gravity circuit difficult to quantify. In this instance the value of leveraging a significant knowledgebase of operating gravity circuit performance to support the decision making process was demonstrated. This case study analysis serves as a useful demonstration of best practice methodologies supporting decisions to include or exclude gravity recovery in a gold recovery process flowsheet.CITATION:Giblett, A and Afewu, K, 2015. "INTRODUCTION Frequent attempts have be,made in the electrolysis of molten magnesium chloride to regenerate the electrolyte by allowing the chlorice generated at the anwdo to read with magnesium oxide and carbon within the melt according to the following reactions: (1) MgO + C + CL2 = MgCl2 + CO,(2) 2MgO + C + 2C12 = 2MgC12. + CO2.The principal advantages of such a procedure are that (1) the direct source of magnesium is magnesium oxide rather than the more expensive dehydrated magnesium chloride and (2) it is not necessary to handle or dispose of large quantities of chlorine or hydogen chloride.Grätzll 5/ proposed the use of an MgO carbon feed in a more or,less compact form in the anode compartment, of the cell. In a patent issued to Magall Aktiengecellschaft, 6/ a compact mixture of MgO and carbon is used as the anode itself. Kato 7/ and Backer 8/ add the feed in finely divided form to the surface of the anode compartment. The Bureau of Mines 2/ recently made a pilot-plant investigation of the electrolysis of suspension of MgO and carbon from chloride melts, in which it was found that although,current efficiencies of 60 to 70 percent were obtainable, the formation of sludges containing MgO, carbon, and magnesium seriously interfered with the operation of the cell,"

This paper discusses 105 production runs at Boulder City, Nev., in which titanium metal was made in 200-pound batches. It describes the equipment and techniques used in preparing raw material, the reduction process, and the removal of the reaction mass from the reaction vessel. Also, some data on man-hour requirements are presented. The main emphasis was on producing ductile-grade titanium and developing a standard procedure for obtaining a consistently pure product. Some departures from techniques used before were found to be desirable. The most important of these was the use of a sealed room with a controlled dry atmosphere in which the reaction vessel was opened and the reaction mass removed. This minimized the moisture taken up by the magnesium chloride in the reaction mass, and it could be purified successfully by vacuum distillation. Using the dry room also made it possible to leave al-inch shell of the reaction material on the walls of the vessel, which lowered the iron contamination and eliminated the labor and time-consuming job of cleaning the vessel after each reduction. Another change was lowering of the temperature during reduction to 750° C. instead of 850° C. This was found to give good results more consistently. The first poke to break the crust during reduction was eliminated, as it was found to be unnecessary and only added more work and was another possible way of introducing air. A rotating feed arm was installed in the reaction vessel to distribute the titanium tetrachloride over a larger area and, thus, prevent local overheating. The process, conducted in a batchwise manner as described and followed by good purification and fusion, produces a metal adequate in quality for nearly all purposes.

Free-standing supports are commonly used in coal mines in the USA, particularly when longwalling. A recent trend has been the development of prop supports. Prop supports have many advantages over bulky, crib-type secondary standing supports, particularly in regard to material handling and installation. The challenge in prop design has been to develop load capacities that can compete with crib type systems and to develop reliable (stable) yielding capability. This paper will focus on the design aspects of steel prop supports, specifically, and evaluate the various concepts that have been developed to provide easy height adjustment and sustainable stable yielding under load. The design and resulting improvement provided by recent support developments called the Quik Stik and Ball Buster are discussed in detail. Performance areas that still need improvement in the design and usage of free standing supports in general are identified and possible solutions outlined.

In panel caving in underground mines, it is essential to characterise and monitor the stress and stability state of production level pillars to ensure the extraction of the mineralised ore column and the safety of workers. Pilar Norte is a sector in the El Teniente mine that forms a pillar between the undercut fronts of the adjacent Esmeralda and Reservas Norte sectors. It exists in a high-stress environment, which may induce damage to the rock mass production pillars. Generally, numerical models are calibrated using observed damage in galleries. This procedure does not discriminate between the structural condition and/or the blasting damage effects. This paper proposes a technique that uses borehole cameras to measure stress-induced damage. This has the advantage of being at the intact rock scale, meaning that the effect of geological structures is negligible and the blasting damage factor is eliminated. Comprehensive geotechnical characterisation and tomography monitoring has been undertaken at Pilar Norte to describe and determine the rock mass behaviour of the production pillars. By combining analytical, empirical and numerical models, a correlation between the measured overbreak recorded by the borehole cameras and the stress level of the pillar was developed. This manual provides basic practical instruction on the inspection of driven piles. It covers the essential components of pile driving, such as the pile driving hammer, hammer accessories, the pile and soil, and frequently encountered test methods in an uncomplicated, yet knowledgeable, and practical manner. Insight is given into the methods of installing the foundation, dealing with the cranes which do the lifting, the hammers which do the driving, the leads that guide the hammer and pile, the compressors which provide the external power when required, and finally prevalent analysis tools and testing instrumentation and procedures which enable you, the Inspector, to do your job. This DFI Inspector's Manual for Pile Foundations will help the Pile Inspector who has limited knowledge of pile foundations. Perhaps this Inspector has been pressed into service at a moments notice; a few helpful hints and suggestions to him are desperately needed and he can look here for the basics. It is also useful for the experienced Pile Inspector as a reference guide. However, this manual is not a detailed check list of every possible scenario and detail which could ever be encountered. There are other sources available which give more detailed descriptions (and should be consulted by the Inspector when appropriate; a list of this additional reference material is provided and suggestions made in appropriate chapters of this text). The current manual is more a brief "textbook overview" with some of the more important aspects given at least some emphasis so that the Inspector has some direction and hopefully helps to produce a safe, yet economical, foundation.

The principal function of the Bell limestone mine, of the American Lime & Stone Co., at Bellefonte, Pa., is to supply raw material to the company's modern rotary kiln plant at Bellefonte. Because mining costs are naturally higher than many open quarries with more favorably lying strata, it cannot compete, except to a limited extent, in the markets for road material and fluxing stone. In the broad sense, mining does have an advantage over the average open quarry when the end products are designed for and controlled by process and final inspection for the exacting demands of the industrial and chemical markets. Approximately 80 per cent of the output of the Bell mine is sold in the industrial and chemical markets. And as higher standards are set each year for construction and agricultural work, the remainder of the mine output goes to these two classes. Mining produces more fines than open quarries, and these fines are difficult to market; on the other hand, owing to absence of contaminating clay and impure rock, a greater percentage of the output of a mine is marketable. Because of the great production of fines, rotary kilns for lime burning are necessary when the stone must be mined. In 1921 the American Lime & Stone Co. operated 19 quarries and 72 lime shaft kilns spread over an area of 40 miles between Bellefonte and Altoona, Pa. At that time the lime-production capacity of the 72 kilns amounted to approximately 700 tons per day. After 15 years the company operates three rotary kilns and six shaft kilns, with a combined output of approximately 500 tons of lime per day, concentrated in its single Bellefonte plant (Fig. 1). Its only stone-producing plants are now the limestone mine at Bellefonte and a construction-stone open quarry at Union Furnace, near Tyrone, Pennsylvania. Since the middle of 1922, the management of the American Lime R. Stone Co. has been under the direction and ownership of the Warner Company, of Philadelphia. A 3-month study in 1971 of the Essen Roof Support and Rock Mechanics Research Center's program of roof and support evaluation included 25 mine trips. German miners must adopt mining methods and environmental control measures involving methane drainage and water infusion to improve productivity under severe natural conditions. Pressing problems in ground control must be solved such as cavities in front and above the supports at longwall faces and excessive convergence in gate roads. The Center developed automatic data processing methods to quantify the results of roof and support surveys and to establish criteria. Another approach to ground control is the study of mine models. The Center's laboratory has the facilities to test elements of powered roof support and whole assembled units. Face automation and roof control by powered shields are novel developments that are under close scrutiny in the field as well as in the laboratory. Though the Essen Center's roof and support evaluation techniques are tailored to the needs of the coal mining industry of the Federal Republic of Germany under conditions vastly different from those in the United States, some application may be found here, for instance, monitoring faces and roadways and preventive maintenance of hydraulic systems.

The Bureau of Mines utilizes special testing methods for the analysis of underground mine atmospheres for use during or after mine disasters and fires. These methods depend upon drawing sample gas to the surface through multiple tube bundles with the subsequent analysis of grab samples or the automatic analysis by often remotely located instruments on a continuous, 24-hour basis. During cold weather, low ambient temperature causes condensation and freezing of condensable vapors in the sample which can result in delayed response times or complete blockage of the sample line. This report covers research and resulting development conducted by the Bureau of Mines to produce a reliable method to selectively remove condensable vapors and particulate in the sample gas stream under worst-case field conditions. The effort resulted in two prototype systems which have greater field utility than originally anticipated. The innovative project resulted in two patent applications submitted through the Bureau of Mines.
