Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

"ABSTRACT: In the year 2010 the State of Denmark together with the Municipality of Copenhagen, launched a very important and complex Project : the complete construction of a new double Subway Line :the CITYRINGEN.Copenhagen is the capital with a metropolitan population of 1,9 million.This line is like a ring placed in the very town center which includes all the most historical and monumental buildings and churches, with a total length of 17 km, and the presence of 17 stations plus 4 service shafts.The buildings have been built in the last centuries in an area where the groundwater is near to the surface and there are lakes and channels: the techniques adopted were advanced for those times, but limited as far as regards settlements and resistance to horizontal movements.The soil consists in 2 main groups.1.Upper sediments of the glacial ages : thickness total = 5-20 m composed by clay- silt-sand-gravelcobbles- granite boulders and flint in blocs and in layers : frequency and size of these obstacles wherenot determined.2.Limestone layers : 2a. upper limestone : highly fractured ,highly permeable2b.Itermediate limestone : less fractured, medium permeability.2c. Lower limestone : almost not fractured, low permeability.2d Bryozoan limestone : composed by fossils with microscopic dimensionsIn all the limestone formations there are important layers of flint , having thickness variable up to 3 m and important extensions. Water : in a part of the areas of the Cityringen the water is drinkable and utilized in the public waternet.Pollution: there are polluted zones due to previous industrial activities. Benzene areas have been identified, and for those areas special protections against pollution have been implemented.METROSELSKABET ,the public owner of the Cityringen,has set as priority to operate for the construction of the Cityringen the full protection of the environment and the respect of all the stakeholders :Very strong limits for the exhausted gas emissions have been introduced, by far stronger than those applied from the State of Denmark, very strong limits for the acceptable noise emission. And last but not least very important safety measures and procedure to avoid any accident on site."

Microaerobic biodegradation oflow-grade manganese dioxide slurry tailings was demonstrated in enrichment cultures treated with dilute molasses at 30°C and pH 5·8. Supplementary nitrogenous nutrients, yeast extract addition and pH adjustment were not required.Continuously stirred, air-limited reactors were superior to still intermittently agitated cultures, in that, at 5 per cent (w/v) MnOz tailings and 5 per cent (w/v) molasses, the yield of 0.12 g Mn++/g molasses, the rate ofleaching of 1·06 g Mn++/l/d and a recovery of Mn++ of 58 percent from MnOz were 2·8 times, 1·7 times and 1·9 times greater, respectively. The best compromise in yield (0·45 g Mn++/g molasses) and rate ofdissolution (0·73 gMn++/l/d) was at 1 per cent (w/v) molasses in a stirred reactor.Where leaching organisms were separated from sterile ore by a bacteria impermeable membrane leaching was shown to be some 20 per cent due to diffusible metabolites and 80 per cent due to a direct contact between the bacteria and the ore particle's surfaces.

Many years ago, a relationship was established between dust inhalation and pneumoconiosis. Since then, various government agencies have regulated crystalline and amorphous forms of silica material in the workplace. In 1987, the International Agency for Research on Cancer (IARC) classified crystalline silica as a probable carcinogen. With this listing, crystalline silica became a regulated substance under the Hazard Communication Standard. The minerals and chemicals industries have responded in various ways. They have: • developed worker and user training programs and literature; • labeled products with appropriate warnings; • continued to implement dust reduction programs; and • developed meaningful epidemiological studies of workers. • begun development of reliable analytical methods for measuring crystalline silica at the 0.1 % threshold level; Background Silicosis historically has been the major health hazard resulting from exposure to crystalline silica. Silicosis is the scarring of the lung from excessive exposure to crystalline silica. It has been recognized as an occupational disease for more than 2000 years, since early Greek physicians noted the respiratory problems of miners, stonecutters, and quarrymen who were exposed to excessive quantities of mineral dust. During the industrial revolution, the incidence of the disease was greatly increased by the use of machinery that increased dust levels. Subsequently, the need for dust control was recognized and measures were instituted. Silicosis is characterized by progressive breathlessness on exertion and develops over the course of years. Solutions of acetonitrile in water dissolve water soluble copper(I) chloride. Removal of volatile acetonitrile from the resulting solutions, by distillation with low grade steam, precipitates pure copper(I) chloride. This is used to purify a crude copper chloride sublimate from the chlorination of nickel matte but is applicable to other sources of copper(I) chloride. The pure product can be converted to pure copper(I) oxide, copper(II) oxychloride or pure copper metal directly.

The use of augercast piles has been limited by the engineer's concern for the structural quality of the installed pile. The nature of the pile is such that its final structural continuity is very dependent on the skill of the personnel that do the installation. Thus, NDT methods have been used to verify integrity and when problems are indicated the available solutions are usually quite costly. If information were available during the pile installation so that immediate steps could be taken to correct perceived problems, the concerns of the engineer could be addressed and the system would gain increased reliability. Pile Dynamics, Inc. has developed a quality control system, the Pile Installation Recorder for Augered Cast-in-Place piles (PIR CFA). This system consists of the following devices as shown in Figure 1. 1) The Control Unit powers the sensors, conditions the signals from the sensors, records all measurements, takes input information from the operator, displays the results, and stores the recorded data on a flash card for later permanent storage and processing. During the use of the system all of the results obtained during a test are seen during recording and can be reviewed on the screen again after completion of the installation. The ability to review the measured information makes it unnecessary to print a copy in the field. Since printers often give difficulty in the construction site environment the ability to review the results quickly after completion of the installation is an important capability.

Laboratory-scale experiments were carried out to investigate the effect of ladle slag composition on the decreasing behaviour of aluminum content in steel, i.e., reoxidation of molten steel in contact with slag, using a vertical tube furnace. The steel composition and the slag system chosen were for ultra low carbon and CaO-Al2O3-FeO-MnO-MgO based to simulate ULC steel grades. It has been observed that the reoxidation rate shows a good relationship with (T. Fe) content in slag. In case of (T. Fe) content higher than 1.5 weight per cent, the reoxidation rate increases linearly with increasing (T. Fe) content. For a given (T. Fe) content, it decreases with the increasing (CaO)/(Al2O3) ratio of the slag. However, in the case of (T. Fe) content lower than 1.5 weight percent, no particular relationships are observed between the reoxidation rate and (T. Fe) content or the (CaO)/(Al2O3) ratio of the slag. These results were used to propose for an aim a slag composition for the ULC steel grades in a steelmaking plant. INTRODUCTION The honor of being the recipient of the Mining Society's Daniel C. Jackling Award for 1966 imposes on me the responsibility to search for the subject that I would be best qualified to discuss. It seems that my experience in the mineral industry — nearly 15 years in private and 30 years in public employment — should uniquely qualify me to comment upon the relationships between the mining industry, mineral engineering as a profession, and government, and the changing economic, technologic, political and social changes that dictate their relationships. Such conditions, the increasing knowledge of the world mineral resource base, the greater demands of an expanding population with a rising standard of living, shall be evaluated with respect to our profession — mineral engineering. In all of these forty or so years, I have been in close touch with many phases of the mineral and energy resource industries, especially those of the United States. Particularly during the past 10 years, I have had a ringside seat at the "goings on" in Washington. During all of this period I have observed and analyzed many of the problems of the industry and its relations with the government. During the past few years, I have had increased opportunity for doing research and have found a need to learn something about the mathematical techniques that are undergoing such a revival since electronic computers were developed. One of these techniques is multivariate analysis, which is a system for correlating a large number of complex variables. By applying this technique to the seemingly chaotic present-day situation, there emerges a course of steady improvement in the minerals resource industries that wars, depressions, and political strife have failed to obstruct for long. In short, I advocate a need for new concepts in mineral engineering to cope with the changes, pos- sibly adding many more specialized areas on one end of the spectrum, and broad engineer-generalist approaches at the other. The meeting place shall be where the intricate mineral resource problems at the frontier of knowledge merge into the socio-techno-logical problems of the world. HISTORICAL INVOLVEMENT OF GOVERNMENT IN MINERAL RESOURCE PROBLEMS Involvement of government in mineral resource problems undoubtedly is as old as government itself. But passage of the Mining Act of 1866, superseded by the Act of 1872, was the first major entry of our Federal Government into the problems of mining. Passage of the law was necessary to resolve many conflicting claims and disputes in mining. The General Mining Law of 1872 and the Mineral Leasing Laws of 1920 apply to millions of acres of public lands and to certain privately owned land on which the mineral rights have been reserved to the Federal Government. The extensive involvement of government in the mineral industry business today often comes as a shock to the mineral engineer who has begun to climb the ladder to the top in his company. He is often poorly prepared to cope with the situation and becomes frustrated and even hostile. Better knowledge of the role of government perhaps would ease his situation. In our political system, private industry is responsible for producing minerals. To make certain that the production is adequate for a peace-time economy, and to guarantee the Nation's security in an emergency, the Federal Government finds it expedient to develop policy and engage in regulatory, educational, and research activities. A few examples will illustrate the varied role of government in relation to mineral problems. In varying degrees many agencies of the Executive Branch of Government have responsibilities that influence the functioning of the mineral and energy industries. In addition, of course, Congress, the Judiciary, and State and local governments exercise great influence. In this report, the effects of anode-production variables, such as aggregate coke sizes, pitch binder content, and baking cycle, were investigated. A small alumina reduction cell proved effective in evaluating anodes prepared from commercial materials and from lignite coke and pitch. Compressive strength was found to be the physical property most significantly influenced, but it did not appear to affect anode consumption. Specific carbon consumption, that is, the ratio of carbon in the anodic gas to carbon theoretically oxidized at the anode, was used as an index of anode consumption. It was found that an anode made with lignite coke and commercial pitch was very low in consumption; however, anodes made completely from lignite but by a different fabrication method showed high consumption and anode deterioration. Microscopic examination of anodes after electrolysis showed evidence of higher consumption at low current densities, and was useful in judging overall anode quality. Based on the physical properties of the anodes and their consumption during electrolysis, lignite can be a suitable material for anodes, provided the proper fabrication conditions are used.

To improve mine roof bolt performance, Jennmar Corporation commenced the design and development of a new roof control sys- tem. This new system designated "Insta'l Compression", not only improves safety, but is more cost efficient due to reduced amounts of resin needed per bolt installation. In this paper the research and development of this new system will be outlined. Much of the information presented will be actual test data from various underground operations in Alabama, Kentucky, and Pennsylvania. The development of this system will also cover the design of a new expansion shell, resin compression ring, anti- friction washer, and upgrading of material specifications.

In Eastern Kentucky, the Darby seam has had an extensive history of coal bumps and pillar bursts. The combination of high overburden, strong rock and coal, subjacent and superjacent mines, and retreat mining has produced incidences of violent pillar failure. The influence of these factors on pillar stability and the potential to bump is illustrated in two case histories. In the first case history, a bump occurred unexpectedly during the retreat mining of a five heading continuous haulage panel. Alternative cut sequences and pillar dimensions were analyzed to identify a geometry that would reduce the potential for future coal bumps or bounces during retreat mining. The viability of the alternatives was constrained by the reach of the continuous haulage system, the overburden depth, and the presence of thick sandstones in the main roof. Each alternative was numerically modeled cut-by-cut to examine the mechanism of stress transfer, as a pillar row is retreat mined. This approach highlighted which cuts were being made in highly stressed portions of a pillar and therefore more prone to coal outbursts. Validation of the numerical modeling was accomplished by comparing the cuts where bumps and outbursts had occurred underground to the highly stressed cuts shown within the models. Recommendations of the barrier pillar width between adjacent panels, pillar dimensions, and changes in the retreat mining sequence were proposed based upon the overburden thickness, geology, and the result of the numerical modeling. The second case history documents a nine-entry panel 3,600-feet long that was retreat mined a distance of 2,064-feet without a significant roof fall or roof-floor convergence. Mine management became concerned about the potential for a coal bump or a large roof collapse within the gob and the resulting air blast. The stress on the pillar line and within the remnant pillars in the gob was quantified through numerical modeling. Changes in mining practices on the current panel were implemented to avoid a bump in the active panel. A barrier was established between the current and the future panels to protect the future panel against abutment pressures from the void area. A change in pillar centers was proposed in the adjacent panel to absorb the side abutment pressure from the gob of the active panel.

"The NewCrest Cadia operation boasts the largest SAG mill commissioned last June 1998. A request was made to shed some more light on possible SAG mill charge behaviour. Three different software were used to simulate SAG mill charge behaviour:Mishra/Rajamanis MillSoft DEM charge motion program, Radziszewskis agglomerated DEM charge motion program and Powell/Nuricks MillTraj outer trajectories program. The contexts simulated are: 20/30 % charge volume, 65/75/85 % critical speed using the Hi-Lo lifter configuration specified for the mill as well as a Hi-Lo 60 degre lifter face configuration. Further, lifter interval was also studied to determine the effect on power draft. Power predictions are compared with those obtained during the commissioning period.INTRODUCTIONThe NewCrest Cadia operation is located outside of Orange, New South Wales, Australia. With proven reserves of 190 million tonnes at a grade of 0.75 g/t Au, the mill was designed to process 17 Mtpa (2065 tph) of ore with an 86% copper and 73% gold recovery. The grinding circuit (Fig. 1, Table I) boasts the largest SAG mill (Fig. 2) and was commissioned last June 1998."

Horizontal stresses are caused by global plate tectonic forces. During 1995 alone, they were largely responsible for closing two U.S. longwall mines. This paper presents six case histories from Pennsylvania. West Virginia, Kentucky and Alabama. In each case history, a mine encountered roof falls or difficult ground conditions in the headgate, caused by horizontal stress concentrations. The problems are detailed and the control measures adopted are described. In most cases, nearby longwall panels without stress concentrations were trouble-free. The paper also discusses detailed measurements made at two adjacent Pennsylvania longwalls. One headgate was oriented to avoid a horizontal stress concentration, and the other was not. Eliminating the stress concentration dramatically reduced roof support loads and roof deformation. The paper concludes that proper panel orientation and sequence is the key to maintaining headgate ground control. The optimum orientation is not parallel with the maximum horizontal stress, as previously thought but rather 200 in the stress shadow of the gob. Other stress control techniques, including artificial support, are briefly discussed.

In the course of an integrated airborne survey in 1975, a uranium-molybdenum deposit was discovered by Minatome Australia Pty Limited in North Queensland, Australia, approximately 70 km by road west of Townsville.The mineralization occurs in a welded tuff unit within the Carboniferous St James Vokanics. It is structurally controlled by a complex of subvertical east-west fractures and is exposed over a strike length of 350 m. Drilling has traced a further 150 m extension underneath a barren capping consisting of a thick pile of vokano-sediments. The maximum width of the orebody is 150 m, and its vertical thickness is approximately 100 m.The primary ore minerals are coffinite, pitchblende and molybdenite, accompanied by various alteration products (particularly uranium molybdates).The vein-type mineralization is considered epigenetic.A comprehensive program of surface drilling and underground exploration is in progress to establish the uranium and molybdenum reserves.
