It has been almost a decade since the release of the International Council of Mining and Metals (ICMM) Health and Safety Critical Control Management: Good Practice Guide (2015a). During this time the ICMM member companies, along with many others across industry, have been busy designing, deploying and enhancing their own critical control management (CCM) programs. So, what have we learnt? This paper discusses the scope, application and learnings from the implementation of CCM within the mining industry. What’s going well? What are the common challenges? What are the opportunities to strengthen the approach? What are the threats to successful implementation? The paper draws on the author’s experience as one of the key contributors to the development of the CCM Good Practice Guide and an adviser to many mining companies, including six of the ICMM members. The paper focuses on the practical (operational) application of CCM and the cycle of identifying, defining, implementing, verifying and improving critical controls at the frontline. The paper also includes a discussion on the safety performance of the ICMM member companies over the past decade and provides a recommendation for how CCM can be incorporated into a more proactive model to measure safety performance. The paper concludes with practical recommendations that mining companies, regardless of their CCM maturity, can consider as they look ahead. They are supplemented by three additional recommendations for consideration at the industry level.

The Quaternary Age aeolian dune sands of the Sibaya Formation on the northern KwaZulu-Natal coast are well known for their heavy mineral content. Although ilmenite, rutile and zircon are the economically important members of the heavy mineral suite, many other nonsilicate minerals such as monazite, xenotime, chromite, magnetite, and spinel that are normally considered to be eitherun common or even rare are also present and these sands provide a unique opportunity to study such complex minerals. Monazite is the general term used for a complex series of rare earth phosphates dominated by lanthanum, cerium and neodymium with monazite-Ce, monazite-La and monazite-Nd recognized as distinct end members of the solid solution series. In addition to the elements indicated by the simplified formulae (LaCeNd) PO4monazite can also accept significant quantities of other elements from the lanthanide and actinide series (Pr, Pm, Sm, Eu, Gd, Tb, Dy,Ho, Er, Tm, Yb, Th, Pa, U, Np). The monazite grains in the Sibaya Formation occur as both composite grains, composed of two or more distinct mineral phases, and homogeneous grains. The net physical properties of the composite grains, and their response to magnetic and electrostatic separation methods are related to size and composition of non-monazite component. However, the homogeneous monazite grains also display significant variation in their physical properties related to their chemical composition.

Two comprehensive surveys, which focus upon backfill practices and the economics of backfilling in underground mines in Canada, have been conducted. The two surveys were designed to assess operational practices and costs associated with the wide scope of filling operations that must be met by individual mines, ranging from raw material feed to the preparation plant, through underground backfill transportation, to in-stope product placement. This paper presents the current state of backfill usage in Canada, identifies trends in minefill practices and provides mine operators with an updated database of information upon which they might compare their own practices and costs with present trends throughout the industry.

A rigorous horizontal drilling and chemical grouting program was specified beneath the Hollywood Freeway (U.S. Route 101) section of Los Angeles Metrorail Contract C-311.Twin 6.67-m (22-ft.) diameter starter tunnels were to be conventionally mined through amixed alluvial and bedrock face consisting of silty sand, gravel, and weathered sandstone and siltstone of the Topanga Group. The contact between the alluvium and bedrock dipped toward the Crossover Excavation where the horizontal drilling and grouting operations were performed. The intent of this grouting program was to stabilize the mixed face to minimize ground loss and surface settlement, and to reduce groundwater inflow into the tunnel headings. Given the highly variable mixed face conditions, horizontal drilling up to 86 m (284 ft.) resulted in upward trending trajectories which likely followed the bedrock contact. Chemical grouting was performed in a primary, secondary and tertiary, three dimensional sequence. Evaluation of the primary and secondary grouting parameters indicated where tertiary grouting was required. Grouting pressures induced a small rock failure at the tunnel heading as well as increased stresses on the tiebacks of the excavation.

An instantaneous and continuous analysis of a mineral suspension should be of great value in controlling various mineral preparation processes. Described herein is a method of analysis based on the use of high-frequency alternating current, which, under certain restrictive conditions, could be of use. The techniques are discussed briefly in the following sections. However, detailed information on the techniques is provided in the cited references. INTRODUCTION Coalbed methane, if not properly controlled in the underground mine environment, is a safety concern due to the potential risk for an explosion. This is a particular problem during longwall mining, where the high rate and volume of coal extraction can result in the release of large amounts of methane from the mined coalbed and other adjacent gas-bearing strata. The variability and potential hazards of these sometimes unexpectedly high gas flows provide the impetus to develop methods to predict methane emissions into the underground workplace. A forecast of the volume of gas that might be released during coal mining is helpful for designing ventilation systems and for implementing optimum methane drainage strategies to help mitigate expected gas emission problems. A complete assessment of the need for methane drainage prior to mine development generally requires both an empirical and a theoretical approach. If there are active mines in the general area with similar geologic conditions and coal characteristics, a review of gas problems in those mines provides an initial insight into the level of gas emissions to be expected at a new location. In addition, relatively simple methods exist to determine the in situ gas content (volume of gas per unit weight of coal) of the coalbeds in a particular mining area, as well as the gas-in-place (volume of gas in the coalbed(s) within a defined geographic area).

At the request of the U.S. Bureau of Land Management, the U.S. Bureau of Mines studied prospects, claims, and mineralized sites in a 43,053-acre portion of the 51,262-acre Desatoya Mountains Wilderness Study Area (NV-030-l10) in 1985. The study area, located 50 miles east of Fallon, NV, is underlain by alluvial deposits of Quaternary age and volcanic and volcanic-derived sedimentary rock of Tertiary age. No mines or recent exploration activity were identified within the area studied. The Gold Basin area, located 1/2 mile to the south has produced 982 ounces of gold and 10,086 ounces of silver. The Cold Springs area, located 1/2 mile to the west, has recently been explored for gold-silver. Mineralization at these two areas apparently did not extend into the study area, but future mining at these areas would likely impact it.

The Deep Underground Science and Engineering Laboratory (DUSEL) is to be constructed at the defunct Homestake Mine, in South Dakota. As a cornerstone of DUSEL?s research program, it is intended to conduct experiments into the fundamental nature of neutrino and proton particles. The experimental apparatus required to con-duct these experiments will be housed in large rock cavern(s) sited at depths of over a kilometer. As conceived, these cavern(s) are planned to have mined volumes of up to half a million cubic meters, and spans of approximately 50 meters. The paper will describe key end-user requirements, and outline the in situ rock conditions and mine site constraints that are likely to play determinant roles in identifying candidate sites and developing cost-effective design options(s).

Analyses of statistical data indicate that haulage accidents are one of the largest sources of injuries and fatalities in underground coal mines, and that the rate of accidents in transportation has been increasing during the past few years. These previous analyses were performed by correlating accident or fatality data at a rather high level of miner activity. In this present investigation, underground surveys were made by observing conditions and activities in some detail at a rather low level of activity in an attempt to understand factors contributing to hazards in the specific situations observed. The aim of the current study was to derive preventive measures that might obviate the hazardous conditions and unsafe acts observed; and, following this, to provide recommendations for additional research and development to reduce accident potential in underground transportation. The major project tasks are diagrammed together with the task outputs in figure 1-1. Initially, a preliminary study of ell underground haulage operations was performed in order to establish the kinds of data needed and to provide the rationale leading to the data requirement. At the same time, the various types of haulage equipment used for each operation were defined. A preliminary analysis of each type of haulage operation was performed in order to establish the sequence of tasks required to operate haulage equipment. These tasks were transferred directly onto observation forms, and were used in developing interview forms. In parallel with the task analysis, a review was made of accidents and hazards that might occur during each type of operation.

Paper presented at Mineralisation in the Caledonides, the Mike Gallagher memorial meeting held in Edinburgh, 27-28 June 1996. The synorogenic, mafic Ordovician intrusion comprises orthopyroxene-rich xenoliths and mafic-ultramafic cumulates with low-grade pyrrhotite, chalcopyrite and pentlandite mineralisation. Despite abundant textural, mineralogical and O isotope evidence for significant assimilation of country rocks, S isotopes indicate only a modest uptake and suggest that mineralisation is dominated by magmatic sulphur. The preferred explanantion is that the magma was close to S saturation on emplacement. Uptake of elements such as Si and O led to early crystallisation of orthopyroxene, resorption of olivine and early segregation of a sulphide liquid. Post-magmatic interaction with a high-temperature, surface-derived fluid, whose movement may have been controlled by regional shear zones, reset biotite and whole-rock O isotope values but not those of orthopyroxene. This model identifies the presence of olivine-rich rocks and evidence of extensive assimilation as guides to prospectivity

The purpose of the Inherently Safe Mining System (ISMS) project is to develop continuous and conventional coal mining equipment and systems which will reduce the incidence of fatal and nonfatal accidents in the working sections of underground bituminous coal mines by application of existing technology. The abandoned Belle Eldridge Mine near Deadwood, South Dakota, is on land controlled by the U.S. Bureau of Land Management (BLM) property. The mine produced primarily lead and zinc during the first half of the twentieth century. The BLM has expressed concern about acidic drainage as well as elevated arsenic levels in tailings and waste rock at the site. The mine site was evaluated for remediation in 1998 and 1999. Recent reclamation and remediation work included substantial regrading of waste rock, removal of material where metal loading were of concern, and re-routing of drainage from an adit. Trenching at the site shoed a greater volume than originally calculated during site characterization. Material was removed to a repository that was constructed at a nearby drainage divide. Because of physical hazards, the BLM has decided to remove some historic structures. However, a search of mine records and an inventory of archival materials were complete to preserve the history of the site.

[Study and development of sensing techniques and sensors were performed to aid shuttle car drivers in detecting, obstacles and in guiding the car along the face haulage route. Microwave, acoustic, and optical sensing techniques were considered as part of the feasibility study for obstacle detection. Various guidance techniques, including wire-following and rib-ranging were investigated. Active optical sensing was selected for obstacle detection to obtain the bear overage and sharp angular cutoff characteristics required within the packaging limitations defined by the shuttle car configuration. Rib-ranging guidance was selected to achieve completely self-contained shuttle car system which responds to the mine geometry without need for wires, stripes, or other path defining devices. Extensive hybrid computer simulation runs were performed to valuate various guidance techniques during the feasibility study. Subsequent simulation rues were performed to establish parameter values for the rib-ranging guidance system. The obstacle detection and guidance system was designed, fabricated and installed on a standard shuttle car. The system satisfied all applicable permissibility requirements of Schedule 2G. An experimental permit was issued to Bendix for operation in an underground coal mine. System evaluation tests were performed first above ground at the Bendix Research Laboratories. The simulation model was validated by these runs. Final testing wan performed underground in an operating mine with an experienced shuttle car driver. All sensors successfully demonstrated Chi predicted performance capability during underground face haulage tests. Dust did not Impair system performance and lens cleaning was not required during working shifts. Further development is required to achieve the desired obstacle detection performance Closed loop steering control with manual override is recommended o: mended for the lateral guidance e system. The guidance system should be an invaluable aid to low-coal shuttle car drivers where continuous mining machines are utilized.]

"Autoclaves have been in commercial use in the mining industry since the early 1950's the initial applications included tin, uranium, cobalt, nickel and copper extraction under both acidic and alkaline conditions. These circuits varied in batch or continuous operation. The geometry has been spherical, vertical, and horizontal type with up to six agitators, with throughput as high as 1000 stpd.Presently autoclaves are a good process selection for many refractory sulphide gold bearing ores. The first application of pressure oxidation in the gold industry was at the Homestake Mcglaughlin's Mine which was commissioned in 1985. This was an acidic pressure oxidation process treating 3000 stpd in three four compartment horizontal autoclaves 14' diameter and 53' long. Since the initiation of autoclaves for the treatment of gold bearing sulphides, there are presently 16 gold autoclaves operating worldwide varying in size up to 18'4"" inside diameter. There are seven autoclaves for the treatment of refractory sulphide gold bearing ores to be commissioned in 1997 and 1998.Autoclave technology has progressed extensively since the mid-eighties. Operational experience has made rapid progress and the result is much smoother and safer autoclave operation. There have been many advances in autoclave technology design. For example, the use of exotic materials for corrosion resistance to reduce down time for maintenance. The instrumentation involved in autoclave operation is very sophisticated which allows for many safety features. The advanced design has allowed for greater availability and as a result operating costs and maintenance costs have decreased. These technological advancements have made autoclave technology more safe and this has resulted in a greater acceptance of this type of operation. Many of the gold autoclave facilities operate up to 9 months continuously without shutdowns. The advancement of autoclave technology has inspired many new applications.Because of the outstanding operation of autoclaves in the gold industry, many other applications for autoclave technology are being considered. Experiments were conducted in 2- and 4-inch-diameter standpipes to evaluate the effectiveness of standpipes as pressure seals. During the experiments, these standpipes conveyed Alan-Wood magnetite (60 to 230 U.S. standard sieve mesh) by gravity flow from one vessel to another of relatively higher pressure. The information obtained has been helpful in a Federal Bureau of Mines investigation of how to make either synthesis gas or hydrogen by a modified steam-iron process employing a continuous recycling stream of granular iron-iron oxide at temperatures of 600° to 900° C. In this process the mixture of granular material flows by gravity from a reduction vessel at low pressures to an oxidation vessel at higher pressures. Data from the 2- and 4-inch-diameter standpipes indicate that it would be practical to use a standpipe as a pressure seal between the reductor and oxidizer. Investigators found that, by gravity flow, the standpipe could convey the Alan-Wood magnetite against pressure gradients up to 1.07 pounds per square inch per foot of column height.

This report describes the development, oil recovery, and potentialities of productive formations in the Hall-Gurney field, Russell County, Kans. It is presented as part of a general program to aid and promote increased recovery from petroleum reservoirs. The report is based on studies of well logs, drill cuttings, and oil-and water-production histories of individual wells and leases. The geological interpretations follow closely those in published literature with only slight local modifications. Oil and water analyses, made at a Bureau of Mines research center, were supplemented by many analyses obtained from oil-producing and service companies. The Hall-Gurney field, in south central Russell County, Kans., (fig. 1) was discovered in 1931 but was not developed extensively until 1941. By 1960 the rate of oil production from the field had declined to the extent that many wells were abandoned. In some areas natural productive forces have effected good recovery. However, in other areas inefficient waterflooding, both natural and accidental, has adversely affected recovery potentialities.

Reduce the danger to personnel and destruction of equipment caused by fires on surface coal augers. Approach: A reasonably priced reliable system which automatically senses a fire and activates dry chemical extinguishers, has been built and field tested. How It Works: Heat sensors mounted in the engine compartment and hydraulic hose areas detect the fire, usually before the operator does. This causes the system to automatically discharge A-B-C class dry chemical extinguisher through a fixed piping system.

Flotation reagents are critical to the optimum performance of flotation circuits. How a flotation reagent performs within the first few cells of a flotation train is a key indicator of how well it will perform throughout the entire circuit. A method to compare reagent performance was recently developed by Ashland Water Technologies. The procedure involves collecting a timed sample of concentrate from each cell and then measuring the volume of the settled solids. An estimate of the percentage of concentrate collected from each cell can then be mathematically determined, providing an understanding of the recovery changes in each cell. Data from trials at a coal flotation circuit are used to demonstrate the method's validity.

In the Coastal Plain sedimentaries of northeastern Mississippi, high-alumina clays and kaolins, which are bauxitized locally to varying degrees, are found in many places along the contact between the Midway (Paleocene) and the Wilcox (Eocene) series. The trace of the contact extends from Tippah County, on the Tennessee border, south to Winston County, thence southeast to Lauderdale County, and into Alabama. Its length within Mississippi, disregarding sinuosities, is 185 miles (fig. 1). Commercial production of bauxite from the region has amounted to about 400 tons, shipped from one outcrop in Komper County. In December 1941 and January 1942, the Bureau of Mines and the Federal Geological Survey made a reconnaissance survey of the contact zone and selected the most promising areas for further study. In 1942, four areas in Benton, Union, and Pontotoc Counties were investigated by auger and rotary core drilling, and in 194, core drilling was done on five areas in Kemper, Noxubee, and Winston Counties. The present report records the work done by the. Bureau of Mines and the factual data obtained from the investigations of the nine areas in the two projects.

[A few studies on the relationship of the fusion temperature of the ash of coke to that of the corresponding coal have been made on commercial high-temperature cokes. R. J. Rose3/ in 1921 compared the' fusion temperatures of the ashes of 10 coals and. the cokes produced from them and concluded tat they were in such close agreement that either figure could be used. More recently, Quarfort4/ found that the ash of coke fused at a somewhat lower temperature than that of the original coals As the ash of a coal and that of the coke produced therefrom have the same origin their chemical composition should be identical, provided that carbonization has not altered the inorganic, or ash-forming, constituents of the coal, and such alteration seems highly improbable. However, it has been shown5/ that coke is liable to contamination by iron during crushing and preparatory to sampling. Pieters6/ found that coke ash was cantaminated in this manner up to 0.4 percent. extraneous iron would lower the ash-fusion temperature because such iron may form ferrous silicate, which is readily fusible,7/ These results provide a basis for the prediction that the ash of coke will fuse at a lower temperature than that of the corresponding coil., especially if the fine grinding of the coke was done by rubbing surfaces of iron, as in a disk pulverizer or bucking plate.]
