The mining industry faces many workplace hazards that are a threat to human health and safety. The Bureau of Labor Statistics has found that employees in coal mining are more likely to be killed or sustain an injury or illness than workers in private industry. These injuries are also likely to be more severe (Bureau of Labor Statistics, 2007). In recent years, coal mining injuries have attained national attention due to highly publicized disasters. Despite these threats to worker safety and health, the U.S. relies on the mining of coal to meet its need for electrical power, with42% of electricity in the U.S. generated through the burning of coal (U.S. Energy Information Administration, 2012b; United Mine Workers of America, n.d.). Furthermore, the production of coal continues to increase and reach record levels every year(National Mining Association, 2012; U.S. Energy Information Administration, 2012a). For this reason, the coal mining industry must continue to find ways to protect its workers while maintaining productivity. One potential approach to protecting workers while maintaining productivity is through improving the safety culture at coal mines. In order to achieve this culture, operators, employees, the inspectorate, etc. must share a fundamental commitment to safety as a value. This type of culture is known in other industries as a ?safety culture? and can be defined as the characteristics of the work environment, such as the norms, rules, and common understandings that influence facility personnel?s perceptions of the importance that the organization places on safety. Fundamental to improving safety culture within an organization is understanding the existing safety culture; improvements cannot be made without first elucidating areas that require improvement. This manuscript details the process undertaken by researchers at the National Institute for Occupational Safety and Health (NIOSH) Office of Mine Safety and Health Research (OMSHR) to assess the safety culture at five underground coal mines across the United States. The purpose of this manuscript is to introduce the mining industry to a multi-methodological approach to safety culture assessments. Further, it is the goal of this manuscript to highlight the value of including both quantitative and qualitative methods into safety culture assessments in order to obtain the most comprehensive view of an organization?s safety culture. Because this manuscript focuses solely on the methodology implemented, data and results obtained during the five safety culture assessments will not be presented in this manuscript, but will appear in a future publication.

The US Geological Survey (USGS) is currently conducting a coal resource assessment of the coalbeds and zones that are projected to provide the bulk of the nation’s resources for the next few decades. The Pittsburgh and Upper Freeport coals are the first two beds in the northern and central Appalachian basin region to undergo fully digital coal assessments. The bed-specific assessments are being carried out in partnership with the state geologic surveys of West Virginia, Ohio, Pennsylvania and Maryland. Comprehensive stratigraphic and geochemical databases have been developed for both of the beds. The extent of the bed with mined areas, structure contour, isopach, and overburden thickness maps for the Pittsburgh coal bed have been released as USGS open-file reports.

Lost foam casting patterns are foamed polystyrene shapes glued together into the shape of the part to be cast and are coated with a refractory material consisting of a mineral mixture and binders. Prior to casting, a significant number of the patterns are damaged and inevitably land-filled. In this study, a process is developed for the recovery of foamed polystyrene from the coating and glue contaminants. The process consists of shredding, impact comminution, size classification and density separation. It is successful in separating up to 97% of the contaminants, while recovering 95% of the polystyrene.

Flame retardant materials can be incorporated or applied to products, including plastics and textiles, to increase their resistance to fire. Incorporating a flame retardant in combustible materials and plastics is a common procedure to reduce the risk of fires. Flame retardant applications include interior and exterior woods, structural steel columns, fabrics, open and closed cell foam, pressure treated wood, Formica and all types of fabrics for use in commercial applications as well as residential applications where their fire protection products can save lives and valuables. Huntite and hydromagnesite are amongst the most important flame retardant additives materials. Although various areas of the world have potential huntite and hydromagnesite deposits, their place in industry is not well known, and a detailed field search for their deposits has not yet been conducted. They are active during the starting phase of a fire, in that they prevent the spread of an inner or exterior source of ignition to the flame-retarded material, thereby, preventing, delaying or inhibiting the spread of a fire. Flame-retardants are, therefore, just one element among many in a concept for preventive fire safety. To obtain huntite and hydromagnesite concentrates that are more than 95% pure, which are used in flame retardant industry, flotation tests were conducted on low-quality huntite and hydromagnesite ores.

The Washington State Department of Transportation (WSDOT) is constructing a new tunneled route under Seattle to replace the existing State Route 99, Alaskan Way Viaduct (AWV), which follows the historic waterfront for several miles. The viaduct separates the businesses that rely upon the tourist trade from the down-town area. The reconnection of the city to the waterfront areas will occur following the completion of the SR99 Re-placement program that replaces the aging structure that was severely impacted by the 2001 Nisqually earthquake, with a state-of-the-art, twin-deck highway with a world record-breaking 17.4-m (57.3-ft) bored tunnel. Constructed in the 1950s, the double-tiered viaduct is nearly 3.2 km (2 miles) long and runs parallel to city?s waterfront. The viaduct, which is partially supported by the seawall, is a vital local and regional transportation link and carries about 110,000 vehicles each day. The seawall, built from concrete and timber in the 1930s, extends along Seattle?s waterfront and supports the soil behind it. Studies in the 1990s showed that the viaduct was nearing the end of its useful life, apparent by its exposed rebar and weakened columns. The 2001 Nisqually earth-quake further damaged the viaduct, forcing WSDOT to temporarily close it for inspection and limited repairs. The viaduct and nearby seawall are vulnerable to an-other earthquake and continue to show signs of age and deterioration. Following a collaborative process, WSDOT developed the following guiding principles to the viaduct replace-ment:

"The Ninetyeast Ridge (NER) extends roughly N-S as a topographic feature for 5000 km, from 31° south to 10° north (Fig. 1) where it becomes buried beneath the Bengal Fan at its northern end. These two geographic characteristics make the NER an ideal natural laboratory for determining the influence of latitude and terrestrial input to ferromanganese (Fe-Mn) crust formation and composition. In addition to study of geographic controls on crust composition, the NER is the largest feature in the Indian Ocean that may host crusts of economic interest.Crusts were collected from 9 of 33 dredge sites from 1.4° north to 26° south latitudes during the 2007 R.V. Roger Revelle Knox06 cruise. Nineteen samples were analyzed for 69 elements each. Nine bulk crusts and 10 layers from four crusts were analyzed. Three of the crusts sub-sampled for layers did not have the bulk crust analyzed and the weighted means for those layers were calculated to obtain bulk compositions. One sample was left out of the statistical analyses because it is phosphatized and chemically quite different from the other samples (Table 1).In order to determine changes in composition with latitude, Pearson product-moment correlation coefficients were calculated for the chemical dataset and latitude of dredge sites, as well as water depth. At the 95-99% confidence level, the following elements increase in crusts moving north along the NER: Mg, Ti, S, Cd, Ni, Tl, Mn, Ba, Co, Cu, In, Li, and Zn; and the following elements decrease northward: Fe, Fe/Mn, P, As, Sc, the rare earth elements plus yttrium (REY), Bi, Ge, and Pb. These correlations indicate that the iron oxyhydroxide component and its associated elements are more prevalent to the south, and the manganese-oxide phase and associated elements increase toward the north end of the NER. This change occurs between dredges D20 and D14 (Fig. 1), with crusts from D20 and south having Fe/Mn !1.0 and those north of D20 ""1.2, with the exception of the phosphatized crust. Crust composition also changes with water depth: Th, Ge, Pb, and the middle rare earth elements (REEs) increase with increasing water depth. Thorium commonly increases in deeper water crusts, which explains the higher Th concentrations in Atlantic and Indian ocean crusts in general compared with Pacific crusts (Fig. 2 and Hein et al., 2012), including the samples analyzed here with a mean water depth of 3584 m. However, water depth does not explain the latitudinal changes because the mean water depths of samples along the northern and southern segments of the NER are roughly comparable. Surprisingly, Si and Al, representing the aluminosilicate/detrital phase, do not correlate with latitude; that is as the ridge approaches and is buried by the Bengal Fan the Si and Al contents do not increase. However, overall, Si and Al have higher concentrations in NER crusts than in central Pacific crusts. Ba can be used as a proxy for primary productivity and statistically does increase to the north along the ridge in the direction of the equatorial zone of high biological productivity. Correlations of Mg, Li, and Zn with latitude may also reflect increased primary productivity at lower latitudes."

San Diego’s Stadium Conglomerate consists of poorly cemented cobbles, gravel, sand, silt and clay. Conventional laboratory and in-situ testing is not suitable for characterizing its engineering properties. Although the formation is often considered a “rock” its behavior is more readily modeled using soil mechanics. Static and seismic design of a tunnel for the Mission Valley East light rail extension required that the stress-strain characteristics of the ground be identified over a broad strain range. Geophysical methods and field plate load testing were used to develop a full non-linear stress-strain model of the expected ground behavior. Quantification of the coarse material distribution was also required for selection of tunneling methods and construction considerations.

In April 1997, Kiewit/Parsons Brinckerhoff (K/PB) completed the 7,600-m (25,000-ft) primary access tunnel for the Yucca Mountain, NV proposed high-level nuclear waste site. K/PB then began boring an exploratory east-west cross drift tunnel at the site. The 2,681-m (8,796-linear ft)East-West Cross Drift Tunnel was excavated through volcanic tuffs having zones of incompetent rock using a rebuilt Robbins 5-m (16.4-ft)hard-rock tunnel boring machine(TBM) with a special arrangement of supporting equipment. This portion of the Yucca Mountain project was completed in eight months.

This paper presents analysis of damages and rehabilitation design for three major French rail-way tunnels built in mid-19th century. The La Nerthe tunnel underwent ovalization deformation with excessive displacement at the springline, and pinching and bursting of the liner bricks at the crown due to tectonic residual stresses with very high lateral earth pressure. The Saint-Louis les Aygalades tunnel invert was subjected to undulatory deformation resulting in tracks misalignment, which was caused by non-homogeneity of mechanical and hydraulic properties of the ground. The Roches de Condrieu tunnel required widening to comply with the increase in dynamic envelope of trains due to their larger cross section and higher speed. The applied rehabilitation methods and constraints of work in these busy mainline railway tunnels have been discussed in detail.

Asbestos fibers are highly fibrous silicate fibers that are distinguished by having a large aspect (length to diameter) ratio and are crystallized in an asbestiform habit that causes them to separate into very thin fibers or fibrils. These fibers are distinct from nonasbestiform cleavage fragments and may appear as thick, short fibers which break along cleavage planes without the high strength and flexibility of asbestiform fibers. Since cleavage fragments of respirable dimensions have generally proven nonpathogenic in animal studies, little data exists on assessing well-characterized preparations of cleavage fragments in in vitro models. The available studies show that cleavage fragments are less bioreactive and cytotoxic than asbestiform fibers. © 2007 Elsevier Inc. All rights reserved. Geotechnical conditions exposed by the phased exploration programs have had a major impact on the selection of excavation and support methods, dewatering requirements, size of openings, vertical alignment, and estimated cost of construction for the Beacon Hill Tunnel and Station section of the “Link” Light Rail. Beacon Hill is comprised of a very complex glacial geology, including numerous slickensided fractures, injected sand dikes, and multiple perched groundwater levels as revealed by a phased boring program and exposed in a test shaft. The Geotechnical Baseline Report (GBR) presents this complex geology along with a “tool box” of alternative additional support systems to accommodate the varying ground conditions during construction of the station excavation. Additional explorations will also be accomplished during construction to further refine the geotechnical interpretations and provide an opportunity to use pro-active ground improvement techniques to optimize the station excavation and support requirements.

The Allen Park Sanitary Sewer Overflow (SSO) Tunnel and relief sewer project, located in the city of Allen Park, MI, is a long-term corrective action designed to bring Sanitary District One?s sanitary system into compliance with its 2005 consent order and service contract with the Detroit Water and Sewerage Department (DWSD). The $20 million project is intended to reduce Allen Park?s wet weather discharges to DWSD, reduce bypass pumping to the Ecorse Creek and limit the future risk of basement flooding by providing storage during wet weather events and eliminating hydraulic bottlenecks in the sanitary sewer system.

A decanter centrifuge of novel design was developed for the dewatering of fine-solids slurries having micron-range particles that form paste-like, flowable, hard-to-dewater cakes. The design combines a compound beach with a cake-flow control structure that allows expressed liquid from the cake in the dry beach region to be returned back to the liquid pool while maintaining high solids throughput. In order to provide optimal performance with various operating parameters (feed rate, centrifugal gravity, pool, differential speed, concentration of feed solids, and different slurry types), the cake-flow control is adjustable. The second part of the beach is preferably at zero angle. Tests showed that the new design allows increases in both solids dryness and solids throughput.

A tunnel boring machine (TBM) was designed and built at the Cold Regions Research and Engineering Laboratory in 1992 for use in Antarctica. The TBM system was tested at the South Pole where a 120-m long tunnel was bored to prove the concept. Modifications were made to the system resulting from this work. In 1999,boring of a 600-mutilidor began as part of the new South Pole Station. Tunneler hydraulics reliability problems prevented the completion of the tunnel before the end of the short construction season. This paper discusses the design and operation of the TBM, proposed solutions to the problems encountered, and current operations of the system.

The 1989 Annual Meeting and Exhibit of SME in Las Vegas, NV, February 27 - March 2, 1989, was the largest ever, drawing more than 4000 regis¬trants and 240 exhibit booths. Technical sessions included presentation of nearly 500 papers, including symposia on Biotechnology in Minerals and Metals Processing, Surplus Capacity in the International Metals Industry, and APCOM `89 - the 21st International Symposium on Application of Computers and Operations Research in the Mineral Industry. Technical sessions covered the gamut of minerals industry interests; however, the proximity of the meeting to recently surging gold development in Nevada drew special attention to gold operating practices. Several of the papers on gold processing are reviewed below. To keep pace with the strong world- wide interest in gold development, SME has scheduled three additional upcoming programs that will focus specifically on gold: World Gold '89 - Gold Forum on Technology and Practices will be held November 5 - 8, 1989 in Reno, NV, as a joint meeting and exhibit between SME and the Australasian Institute of Mining and Metallurgy. Technical sessions will encompass a full program of papers covering geology and exploration, mining, and mineral and metallurgical processing. Cooperating sponsors for this meeting include the Canadian Mineral Processors Division of CIM, The Institute of Metals, The Institution of Mining and Metallurgy, The Mining, Geological & Metallurgical Institute of India, and The South African Institute of Mining and Metallurgy. Gold '90 - Salt Lake City, UT, February 26 - March 1, 1990, will be a symposium held in conjunction with the 1990 SME Annual Meeting. Current research on the processing of refractory gold ores will generate innovative papers on process mineralogy at this symposium, with special emphasis on gold with pyrite and gold with carbon. Other sessions are planned for gold exploration and mining, gold metallurgy, gold plant design and practice, and gold in scams. Advances in Gold and Silver Processing Conference, October 1 - 3, 1990 will be held in Reno, NV. Preliminary planning calls for technical sessions on heap leaching practice, refractory ore processing, non-traditional lixiviants, bioleaching, metal recovery from solution, and the environmental aspects of processing operations. PLANT DESIGN, OPERATION Technical sessions organized by the Minerals and Metallurgical Processing Division of SME at this year's meeting in Las Vegas covered a wide range of gold operating practices. Eight papers relating to gold plant design, operation, and equipment are summarized here. Preprints of these papers are available by writing to Publications Sales, PO Box 625002, Littleton, CO 80162-5002 ($2.50 per paper for AIME members, $4 for nonmembers). Continuous elution process A. M. Stone, of Bechtel Australia Pty. Ltd., described the "Development of the Continuous Elution Process for Extraction of Gold from Carbon" (pre-print 89-75). Developed by Bechtel Pacific Corp. for Ok Tedi Mining Ltd., the Continuous Elution Process uses the same reagents as batch processes for elution of gold from carbon, while yielding significant savings in stripping vessel size and energy consumed in comparison with equivalent batch processes. The process is also simpler to operate than batch processes. Other advantages include reduced maintenance costs, higher average eluate grade, and near steady final eluate grade. Pilot testing for the Continuous Elution Process was conducted at Western Mining's Kambalda gold plant beginning in December 1982. During the past 30 years, the research arena has changed significantly in all fields, mining included. The relative representation of the topics of research, how the research is performed and who has performed the research have all changed. Although there are a lot of subjective ideas and anecdotal "analyses" concerning these changes, there has been no actual study of these trends to verify the facts. This study was divided into six five-¬year periods for the purposes of comparison. Some of the trends analyzed are co-authorship, research areas, research sponsors and cooperative efforts. In today's research arena, 30 years is a long time. Technology has advanced more rapidly than in any time previously and that pace continues to increase. There are more researchers, more tools for research and an increasing number of companies, agencies and institutions engaged in research. The pace of change and its impact is often discussed and debated at professional meetings and in institutions that engage in research. These conversations will often influence the research decisions of the participants in directions to take or subjects to pursue.

The New Croton Aqueduct (NCA) was the first major water supply tunnel for New York City. The 50-km (31-mi) long tunnel has 33 open shafts 6 m (20 ft) to 121 m (397 ft) deep. The tunnel and shafts are lined with brick and mortar, with cast iron lining and pressure covers in six pressurized shafts. Details of the 1895 construction are provided and compared to modern construction techniques. The paper provides rehabilitation techniques for the shafts and pressure covers, including grouting, epoxy lining, temporary bulkhead design and installation and placement of tremie concrete 98 m (320ft) below the water level in a 122 m (400ft)deep shaft using a Remote Operated Vehicle (ROV) to monitor the placement.

India produces about 70 Mt/a (77million stpy) of coal ash (fly ash and bottom ash) from burning about 200 Mt/a (220 million stpy) of coal for electric power generation. The average ash content is 35%. In contrast, the United States produces about 70 Mt/a (77 million stpy) of coal ash from burning 800 Mt/a (880million stpy) of coal for power generation. Its average ash content is less than 10%. In 1998, GAI Consultants Inc.(GAI) embarked on a federally funded, multiyear project to assist the government of India in solving its growing coal-ash management problem.

Since the promulgation of the first Federal Asbestos Standard by the Occupational Safety and Health Administration in 1972, other federal agencies have modified the standard to better carry on their own unique missions. The instruments used to identify and measure asbestos, the sampling protocol, and the criteria used to define asbestos, have been modified to some degree. The Mine Safety and Health Administration regulates and controls asbestos dust in the mining and mineral commodity industries. However, crushed stone and processed ores contain mineral fragments that are frequently difficult to distinguish from asbestos. Mineral nomenclature, instruments for particle analysis, and sampling strategy must be accommodated to some degree to make asbestos control workable and meaningful. Precedent in other agencies has made consideration of these changes possible. Newly identified amphibole asbestos minerals have further complicated the agency?s regulatory charge. Changes in its Asbestos Standard are now being considered. Crushed taconite ore in the Eastern Mesabi highlights many of these issues. ©2008 Published by Elsevier Inc.
