Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

This report will differ slightly from the traditional scientific and technical discussions given at this type of conference. It will focus on the personal experiences and choices that I undertook when deciding on my graduate position and location after completing university studies. Throughout the duration of my course I had experience in both fly-in, fly-out operations in the Pilbara and residential operations in the West Australian Goldfields. These experiences, though limited, allowed me to explore the two different methods and their pros and cons, and consequently to make my choice for graduate employment. Throughout the duration of my degree I believed it was important to gain a basic understanding of the two methods, focusing on lifestyle and personal interactions. The decision that a residential site suited my personality and lifestyle then allowed me to refine my job applications to a more specific list of mine sites and companies, resulting in a residential position with BHP Billiton at their Iron Ore operations in the Pilbara, Western Australia.

This paper presents the evolution of the planning, scheduling and mining practices at the Yilgarn Star underground. The paper will concentrate on the principals of æjust in timeÆ and the impact of such practices on mine planning and scheduling and subsequent development and production. The principals of æjust in timeÆ were developed to minimise the impact of creep and time dependent failure of the ground in the ore and hangingwall zones. These zones are characterised by altered schistose material with inherent weak rock strengths compounded, at depth, by the maximum principal stress of the virgin stress field approaching the UCS of the rock mass. Yilgarn Star underground has and continues to develop the æjust in timeÆ methodology to ensure the continued viability of the mine. The mine continues to use longhole open stoping mining methods to ensure high levels of productivity with minimal dilution and at an acceptable cost of production.

A material aim when feeding ore blends into a processing facility is to reduce the variability of the properties of the feed material and to maximise the output, or throughput. At the Mining Geology conference of 2019, we presented a paper on optimal blending of coking coal given geological variability. The paper introduced integer-programming techniques to both maximise value and minimise feed or product variability. We discussed an application where the geological uncertainty is characterised using a normal distribution and exact solutions are readily obtainable using second order conic constraints in the mathematical model. Geological uncertainty is not always normally distributed, many properties will be characterised as log normal or some other skewed distribution. For example, copper can be some form of lognormal and Fe in Iron ore has a left skewed distribution. In this paper, we discuss some distribution free techniques to optimise the blends of plant feed or output given a quantified geological uncertainty. We discuss the sample average approximation method and how to set-up and apply the method to an ore blending problem. Evaluating a case study demonstrates that the explicit use of non-normally distributed geological uncertainty can better optimise feed into an ore processing facility and potentially add significant value. The approach is equally applicable to all mining and processing operations that blend material into a processing facility or directly into a saleable product.

Most of the copper deposits in the Lachlan Geosyncline fall within areas having either a common host rock, mineral assemblage, form of ore body or time and method of emplace- ment. On this basis a Cobar-type ore has been recognised in the deposits of the Cobar -- Mt. Hope - Melrose region. The copper ores in the Tottenham-Girilambone area to the east have a different mineral assemblage. In the more rugged Central Highlands nearer the coast copper deposits are generally concentrated either in one of several Ordovician andesite belts, particularly between Wellington and Woodstock, or within Silurian acid volcanics developed in irregular fashion over a considerable portion of this region. In only a few instances is copper known in the granite batholiths which adjoin these volcanic belts. Native copper and magnetite are characteristic of the deposits of the andesite province and it is possible that future mineragraphic studies will indicate significant differences in the base metal sulphides of both the andesitic and the more acid volcanic belts. Ores having similar mineralogy and form to the Captain's Flat deposit are known at various localities in the eastern segment of the latter belt. In the Gundagai area several small copper deposits with associated nickel minerals have been developed in the Coolac Serpentinite belt.

Bulk Ore Sorting (BOS) has the potential to add significant value to mining operations by manipulating the feed grade and throughput to the processing plant. However, assessing the performance of BOS is complex, as it is influenced by geology, equipment, spatial and time domains. This increases the risk (real and perceived) of the technology. To manage risk, brownfield operations have the option to: (a) first test the variability in feed grade using a detector on the feed conveyor and subsequently construct the diverter and associated materials handling equipment when the value is established; or (b) conduct site trials with standalone sorting plants. The approaches described for brownfield projects are not available for greenfield projects where the in-situ heterogeneity needs to be quantified and translated into an economic evaluation to test the viability of BOS. Ausenco has developed a methodology that allows assessing the in-situ ore heterogeneity and uses an in-house technoeconomic model to determine the value of BOS for greenfield projects and assess the effects of sorting performance on project economics. The in-situ grade heterogeneity can be measured on a length of drill core and fitted to a mathematical model to assign a heterogeneity index. Given the spatial location, this heterogeneity index can be mapped across a deposit in combination with the average grade. Linking this information with relevant mixing models can determine the likely technical performance of BOS at an operation. However, the real value is only realised when this technical understanding is linked with an economic model and used to optimise the mine plan. Three case studies will be presented that show how Ausenco applies this methodology to assess the value of bulk ore sorting for greenfield studies.

Geophysical signatures of the Taranaki and Tongariro volcanoes reflect similarities in their age, composition and hydrothermal histories, but also reflect differences in host rock settings and edifice evolution. The Taranaki volcanoes have strong positive gravity anomalies showing that their edifices largely consist of dense andesite which reflects substantial lava production or dyke injection during construction and that large sub-edifice dykelstock complexes occur. In contrast, the Tongariro volcanoes have strong negative gravity anomalies indicating low density edifices of volcaniclastic material; no sub-edifice intrusions can be identified, probably because of the lack of density contrast with host basement rocks. Strong positive magnetic anomalies over the younger cones can mainly be attributed to near-surface andesite lavas or dykes whereas subdued magnetic effects are associated with hydrothermally altered zones within both edifices and sub-edifice intrusions.

The gold-scheelite-quartz lodes of the Wakamarina valley were the most productive of the mesothermal gold-bear in the Marlborough Schist. Most of the production came from the Empire City and Golden Bar mines which worked tug, Golden Bar lode. Only 134 m of its 1800 m strike length are accessible, in the No. 2 level of the Golden Bar Mine. A fau I has downthrown the Golden Bar Mine section by 2 47 m and displaced it left-laterally by 18 m. The lode is a typical ribbon-banded mesothermal vein, averaging c. 1.75 m wide, and enclosing narrow horses of schist country rock. It i composed of bands of milky "buck" quartz with thin laminae of schist. Scheelite occurs in short strings of grains withi quartz on the footwall side of the lode and also in small quartz-carbonate veinlets in the foot wall and hanging wall schist The lode quartz contains only localised traces of pyrite and arsenopyrite. They are more abundant in the wall-rock schist.

The allocation of boggers and trucks at Telfer’s sublevel cave mine is currently assigned by manual techniques, with decision-making primarily dependant on the judgement of the operators and mine control. Field observations indicate that allocation decisions could be improved by establishing a systematic approach to dispatch. An investigation into the current state of technology determined that no currently available solution would provide a tool to make optimal decisions for the Telfer dispatch challenge.In an underground sublevel cave (SLC) environment, the destination decision is fixed and the source is variable, so open pit and block cave solutions are not relevant. Newcrest underwent a development venture with an external vendor to develop a novel production dispatch algorithm and user-friendly software program to make allocation decisions in a systematic way with consideration given for the true complexity of the operation.This new production software, nicknamed Dingo (for Dispatch ‘n’ Go), is a smart way to assist forepersons and mine control operators in their day-to-day roles of planning shifts and allocating trucks and loaders to optimal locations during shifts.CITATION:S Graskoski, 2017. As part of global systems of mineral production and consumption, the Australian minerals sector is facing sustainability challenges across technological, social, ecological, economic and governance domains, as well as between local, national and global scales. To ensure that the Australian minerals sector progresses towards sustainability, it is imperative to understand the possible ways in which AustraliaÆs mineral resources could support sustainable futures. A significant research gap exists between the complex nature of questions concerning minerals sustainability and the reductionist methods available to deal with them. This paper argues the need for broader, more integrated approaches to questions concerning minerals sustainability, which can address multiple human perspectives, complex and æmessyÆ patterns and processes across multiple organisational, temporal and geographical scales and whole systems of mineral production and consumption. To inform the development of a new approach to minerals sustainability, this work reviews the contemporary understanding of the Australian minerals sustainability problematic, from the perspective of the Australian minerals sector (Minerals Council of Australia), the Australian research sector (Mudd 2007a, 2007b; Mudd and Ward 2008) and a multi-scale international project (MMSD 2002). This review shows the focus of current responses to the minerals sustainability problematic, identifies the need for an integrated approach to questions of minerals sustainability and addresses how different approaches are informed by underlying and unarticulated assumptions about the tolerability of tradeoffs between different societal goals, the treatment of uncertainty and the application of different conceptual geographical, organisational, temporal and life cycle scales to define the minerals sustainability problematic. The Mineral Resources Landscape proposed, offers an expanded conceptualisation of minerals sustainability, to link minerals production and consumption in an integrated assessment across the entire minerals supply chain, connecting social, ecological, technological, economic and governance domains across local, national and global scales. The key leverage points governing change in the Mineral Resources Landscape are identified as: the material source, extraction and production technologies, level of service and value, and consumption patterns. Mapping the key challenges facing the minerals sector, as identified in the review herein, indicates that the boundaries defining traditional conceptualisations of minerals sustainability focus on the material source and technology and ignore two very key drivers of the Mineral Resources Landscape û the æservicesÆ minerals offer to society and the æconsumption trendsÆ which assimilate these services into society. Understanding these overlooked aspects of the Mineral Resources Landscape, along with the conventional areas of focus, is essential for identifying the Australian minerals sector as a provider of sustainable mineral services. This insight prompts a reconsideration of the role of minerals services and consumption trends, together with the role of the material source and technology in shaping change and the emergence of sustainable systems of Australian minerals production and consumption, across multiple scales and domains.

A distinct zonation occurs in the nature of submarine volcanic activity associated with westwards propagation of the Woodlark spreading system into continental crust. Olivine tholeiites (Type I MORB) flooring one spreading segment in the Eastern Sector of the Western Woodlark Basin are accompanied by more evolved "ferrobasalts". Basaltic andesites and Mg-poor andesite-dacites occur on axial and flanking seamounts respectively. In the Western Sector, Mg- and K-rich olivine andesites occur in a rift valley and on an axial seamount near the tip of the propMr/8 tig rift. Observed correlations between 6Sr ratios (0.7028-0.7041), REE patterns and K contents imply a complex petro- genesis with the relative importance of mantle heterogeneity and crustal contamination difficult to resolve.

Self-potential anomalies are generated by fluid pressure gradient, by temperature gradient or by chemical gradient. On the basis of this principle, a new technique is proposed to locate the fire source of spontaneous combustion of coal by the self-potential method. In order to investigate whether this method can be applied to locate the spontaneous combustion of coal in practice, surveys of self-potential distributions were undertaken in a coal waste pile undergoing spontaneous combustion. The results showed that there is a negative centre of the electrical potential distribution in the measurement area. The potential in this centre is about 140 mV lower than that in the surrounding area. Moreover, the negative centre of electrical potential distribution almost overlaps with the high temperature zone. Therefore, it is concluded that it is possible to locate the heat source by the measurement of self-potential distribution.

Disastrous earthquakes are a reality on the Australiancontinent. Unfortunately it has taken the fatal 1989 New-castle earthquake to demonstrate to all Australians that we do not reside on a "stable continent". While the world accepts the potential of plate tectonics and its associated plate margin earthquakes, particularly around the active Pacific Rim, the realisation of devastating intra-plate, or continental, earthquakes is only now beginning to take effect. The 1989 Newcastle earthquake provides many lessons on which to further fundamental earthquake research and the applications to the future welfare of human civilisation. To mitigate the earthquake hazard in relation to the devastation to life, property and industry, research must be undertaken with the resulting informa-tion to be included in the nation's disaster management programs. The earthquake hazard is everpresent in east-ern Australia and the major step in its quantification is a more realistic determination of earthquake risk estimates through an integrated study of seismology, engineering, insurance and sociology.

Lake Kopiago is located some 71km northwest of Tari in the Southern Highlands Province and lies on the northern edge of the Papuan Fold Belt. The area is underlain by middle Miocene carbonate and fine grained clastic sediments intruded by late Miocene hornblende gabbros.Two mineral deposits occur to the south of Lake Kopiago. These are the Bisamu gold prospect and the Wunaki zinc-lead gossan.The Bisamu deposit consists of a calcic skarn assemblage which is geochemically anomalous in gold, silver, copper, zinc, cobalt, bismuth and tellurium. The prograde mineral assemblage includes garnet, clino-pyroxene and epidote whereas the retrograde assemblage includes epidote, quartz, actinolite and chlorite. Pyrrhotite is the dominant sulphide with minor pyrite and marcasite and trace chalcopyrite and sphalerite. Minor magnetite is also present. The Bisamu skarn shows many of the features typical of gold-skarn type mineral-isation.

Mining operations must become more predictable to overcome the volatility that has characterised the mining industry. Comparisons of production against predictions will become more consistent as mining operations merge and expand, as personnel move from site to site, and as public reporting of reconciliations becomes more prevalent. More consistent approaches are already being seen in the industry using the terminology used in this paper and elsewhere.Examples are provided that show how use of the waterfall chart as a reconciliation management tool can provide objective analysis to improve practices.Geometallurgical modelling is becoming more useful and will continue to do so as we move from a few bulk samples to better interpretation of material properties from geological mapping and drill sampling. Reconciliation has evolved to the stage where predictions of geometallurgical properties to optimise mill performance, and of geotechnical properties derived from better 3D geological models, should also be reconciled.CITATION:Shaw, W J, 2014. The resurgence of Silicosis and Coal Worker’s Pneumoconiosis (CWP) in coalmines has placed the spotlight back on the management of worker exposure to particulate matter across all mining sectors (Coal Workers Pneumoconiosis Select Committee, 2017). There already exists a substantial body of knowledge on particulate matter and its impacts upon human health (National Academies of Sciences, Engineering and Medicine, 2018). The contribution of silica to respiratory disease is well known. However, it is also clear that there are major gaps in our understanding. In part, those gaps relate to the contribution to adverse health effects of mineralogical constituents in the dust (LaBranche et al, 2021a). It is also becoming evident that the size and shape of the particulate matter can affect its potential impact on human health. The University of Queensland has developed a methodology for characterising respirable and inhalable dust samples using scanning electron microscopy. Multiple samples from numerous mining operations have already been analysed using the UQ methodology. The process has been successful in showing the variety of mineralogical components and particle size distributions present in various areas of the mines.

The approach to the disposal and storage of mine tailings is slowly evolving. Tailings disposal has been based on minimising short-term capital and operating costs, with rehabilitation costs discounted by the application of a high discount factor in a net present value approach to accounting. This has led to the widespread adoption of surface tailings storage facilities (TSFs) to store slurried tailings delivered by robust and cheap centrifugal pumps and pipelines. Depending on the site climate, rate of rise of the tailings, nature of the tailings and the extent to which supernatant water is removed from the surface TSF, the tailings may or may not dewater, densify and gain strength. High rates of rise of dilute tailings to a facility used to store both tailings and supernatant water results in a wet, low-density, soft tailings deposit that not only occupies a large storage volume but also presents rehabilitation difficulties. A low tailings density results in high containment wall costs, very high rehabilitation costs and a low potential for future land use or ecological function. Overall, the cost of slurried tailings disposal, thought to be low, is actually high. However, low rates of rise, even of dilute tailings, in a dry climate with efficient supernatant water removal can produce a dewatered, high-density, stiff tailings deposit that may occupy a large footprint but to shallow depth that is relatively easy to rehabilitate. There are advantages in dewatering tailings in the processing plant to recover process water and chemicals, facilitate tailings disposal in a more solid form and facilitate rehabilitation. However, the cost of dewatering in-plant can be high, and the process water may not lend itself to reuse. There are also advantages in removing supernatant water to facilitate solar and wind drying of exposed tailings, although water is lost to evaporation and the dried tailings may best be harvested prior to inundation by fresh tailings or rainfall run-off. "Surveying, and as a result, surveyors, have been around since the start of time, and it has been said that so has their methodology!The theodolite and tape have been the backbone of a time-tested practice that has led to ground breaking advancements such as electronic distance measurement (EDM), global positioning system (GPS) and positional laser scanning. However, it was the introduction of the unmanned aerial system (UAS) that has allowed mine sites such as St Ives to take the next step towards truly turning survey data into metadata.This paper will explain how the unmanned aerial vehicle (UAV) was adapted to meet the requirements of a mine site supporting a major open pit operation, and how the open pit mining team adjusted their planning and production processes to maximise use of the extra data and information available to them.The advantages of the system include point cloud surveys with corresponding large-scale up-to-date photography and computer generated high detail contouring of mined areas that are then incorporated into the mine plan. All the while, the use of the UAV must be tailored to each specific problem encountered and a solution then matched.Citation:Steven, R, 2016. Conditions encountered at a recently completed brownfields mining project provided valuable insight into, and development of, techniques for working in and around known underground mining voids in an open cut environment. The careful application of underground blasting principles to a pit environment and successful excavation whilst maintaining appropriate stand-off distances were two primary areas of innovation enabling the success of the project.Mary Mac Hill is a gold deposit situated less than 2 km south-west of the town of Laverton, Western Australia. It is owned and mined by Crescent Gold Limited. Previous underground mining by various other mining companies has left an extensive network of underground horizontal development, subvertical stopes and shafts, some of which daylighted at surface. This was identified as potentially posing substantial risks, which would prevent the application of conventional mining methods. Two major stopes were located under a hard banded iron ridge topped by a peak of cap, connected variously by narrow drives and vertical development. Due to the proximity of the first of these stopes to the surface, its dimensions and governed by procedures and advice provided by externally engaged engineers, Crescent Gold could not access the area of the ridge immediately overlying the stope using conventional open cut mining methods. This was the first of many hurdles surmounted in mining this deposit.The following report outlines how the challenges this geometrically complex deposit presented. Solving these problems enabled the team to mine the contained gold ore to a completed design, without subjecting personnel or mining equipment to harm due to a void breach incident. This paper outlines the numerous complications and difficulties Mary Mac Hill posed and explains the strategies developed to overcome them. Maps of mineral grade used to assess mineral reserves are most commonly generated using one of the many estimation methods available such as inverse distance weighting or ordinary kriging. Most such maps incorporate a degree of spatial smoothing which is an integral part of the averaging process. In deposits with highly variable grade which include many gold deposits, this smoothing can mask the detailed geometry of the mineralization at the economic cutoff grades being considered. In underground mining, a knowledge of this geometry is important in the choice of mining method and thus in the assessment of the minable reserves.The method of conditional simulation in geostatistics can be used to generate maps of grade which reflect the actual spatial variability of grade as measured through the variograms of the drill hole data. Simulated maps can provide a number of equiprobable images of the mineralization geometry from which a more appropriate mining model of the mineralization can be developed. Multiple simulations can also be used to assess the uncertainty existing in the current knowledge of the reserves.
