Risk management is an established loss-control methodology that has been applied successfully in many industries. Recently, interest in this structured approach has grown in the mining industry. The main objective of this research was to develop a risk-assessment process, which is a part of risk management, that can be used by the U.S. mining industry to more thoroughly characterize risks associated with haul truck-related fatalities. The assessment is based on historical data obtained from the U.S. Mine Safety and Health Administration (MSHA) investigation reports, which includes 113 fatal incidents that occurred from 1995 through 2006. The risk-assessment process used in this research involves the following basic steps: identification of the risks, risk analysis and risk evaluation. The preliminary hazard assessment (PHA) method is used in identifying and quantifying risks. Risk levels are then developed using a pre-established risk matrix that ranks them according to probability and severity. The resulting assigned risk value can then be used to prioritize control strategies. This paper is a part of a detailed study on risk assessment for equipment-related fatalities in mining sponsored by the Western U.S. Mining Safety and Health Training and Translation Center.

The hydrocyclone problem has been a challenging task for the last 30 years. In the 80’s and 90’s Computational Fluid Dynamics solution to this problem began to appear. These were unsuccessful due to over simplification. In the past 3 dimensional solution with ?-e model and Reynolds Stress Model have been published. In this paper RSM and ?-e models are compared with velocity profiles measured in 75 mm and 250 mm diameter hydrocyclones. It is shown that results are not satisfactory with RSM or ?-e and that a model called Large Eddy Simulation is needed to describe velocity profile evolution and size classification.

During the 1990’s several copper heap leach facilities treating crushed and agglomerated oxide and sulfide ore were constructed in South America. The majority of these facilities have been in operation for at least three years, during which time the operators have been able to monitor the performance of the heaps. The basis for the paper is a compilation of these operators’ experiences, leading to a discussion of the lessons learned over the past decade. Several key geotechnical issues are examined, focusing upon the integrity of the ore, drainage material and pipe network, along with the success of any inter-lift liners.

This paper describes the Contractor’s value engineering design and the construction of the Interstate I-90 highway tunnel sections under the South Station railroad yard in Boston, MA for the Central Artery/Tunnel Project, using tunnel jacking and ground freezing techniques. Three tunnel box sections were constructed and installed, with typical cross-sectional dimensions of 11.6 m x 23.8 m, and respective lengths of 50.9 m, 78.7 m and 115.6 m. A 12-compartment concrete shield was built as part of the front of each multi-sectional tunnel box structure. The jacking systems employed consisted of rear and intermediate jacking stations and up to 87 individual jacks, providing a maximum total thrust capacity of 42,300 t. The tunnel alignments pass through a geologic profile consisting of historic fill, which contains remnants of old waterfront structures, building foundations and a forest of 19th century vintage timber piles, underlain by organic sediments and marine clay. Ground cover over the tunnels is in the range of 1.8–7.6 m. The groundwater table is at depths of 1.5–3 m below surface grade. The paper discusses the techniques developed and equipment used to successfully build and install these tunnel box structures with minimal impact on the commuter and long distance railroad operations directly above the tunnel alignments. Figure 1 shows the track area at the I-90 EB and WB tunnels.

INTRODUCTION Design of supports for underground chambers in rock has undergone considerable development. This becomes evident when comparing support dimensions of subway station caverns built in the U.S. over the past fifteen years. The Porter Square Station cavern, which is a part of the Massachusetts Bay Transportation Authority (MBTA) Red Line extension, is the latest in this series. It is approximately 168 m (550 ft) long, 21 m (70 ft) wide, and 14 m (45 ft) high, and has a permanent support of light steel ribs imbedded in a 0.4 m nominal (15 in) shotcrete lining. An extensive construction control instrumentation program made it possible to evaluate the performance of the cavern and its support. This evaluation, which is presented in this paper, was the objective of a research project sponsored by the Urban Mass Transportation Administration (UMTA) and jointly conducted by Haley and Aldrich, and MIT. The actually used design procedure was compared with other approaches, notably procedures based on Finite Element analyses and empirical rock mass classification methods. Support dimensions predicted with all these approaches were compared to the actual support and its performance.

The East Side Access (ESA) project is the largest underground rail infrastructure project under construction in New York City, USA. The Manhattan segment of the project is to be mined through rock and consists of 4.1 miles of Tunnel Boring Machine tunnels for track work and mined excavations to house rail switches, air plenums, and other rail facilities. This paper considers different methods for concrete lining, such as the use of fixed forms, adjustable forms, and pneumatically applied shotcrete. The advantages and disadvantages of these methods are discussed and the basis for considering one over the other are illustrated. Further discussions on design development and preparation of documents to facilitate quality and constructability in the field are presented.

The prediction and control of ground deformations due to underground mining are important considerations in the permitting, planning, and monitoring of coal mining operations. The prediction of mining-induced ground movements using the influence function method is a mature technology and is widely used in many coal basins around the world. Such movements mainly include subsidence, horizontal displacement, and horizontal and/or ground strain. Predictions can be accomplished using regional subsidence parameters and site-specific data.

A multiyear, comprehensive study of flotation reagents used in phosphate beneficiation was conducted. The study involved the evaluation of pure fatty acid compounds, comparison of various anionic collectors and testing of iso-acids. Various C12 to C22 fatty acids were tested as Florida phosphate collectors. The saturated fatty acids were found to be very poor collectors when using the standard conditioning procedure, whereas the unsaturated C16 to C22 fatty acids showed fair to good phosphate collecting ability. The C18 fatty acids oleic, impure oleic and linoleic and the C20 eicosenoic acid were shown to be the best phosphate collectors. The best flotation results were obtained using Liqro GA tall oil and oleic acid. Four isostearic/iso oleic acid type fatty acid collectors were compared with a commercial grade oleic acid and with Liqro GA tall oil as phosphate collectors using standard laboratory conditioning and flotation procedures. The most selective collector evaluated was Century 1108, a high isostearic acid-type reagent. However, it was concluded that this excellent-performing reagent was too expensive for commercial use. Overall, the most promising reagent was probably Century MO-5. This collector is essentially an iso-oleic acid/stearic acid mixture (not isostearic acid), and it costs much less. This program laid a sound foundation for developing single collector, all anionic flotation processes.

The rate of hydration influences many important physical properties of gypsum. Previous research examined the rate of gypsum hydration and related this to compressive strength and microstructure development using BET specific surface area analysis. The analysis of gypsum porosity in combination with BET surface area and rate of hydration remains to be done. In the present study, mesoporosity is analyzed using the numerical method of Barrett, Joyner and Halenda (BJH) to gain a greater understanding of gypsum hydration and microstructure development. The rate of hydration has a significant effect on mesoporosity of the gypsum matrix. The results reveal the effect of mesoporosity and hydration rate on some macro physical properties of gypsum.

Tunnel contracts in New York have in recent years had few bidders with the bids received being much higher than anticipated. Since 9/11, security has become a key issue in planning key infrastructure projects in New York. This is especially true in the case of large water supply tunnels. Striking the right balance between cost and security was the major factor in determining the best configurationfor the Kensico-City Tunnel. This paper outlines the efforts made to determine realistic probable bid costs for the tunnel contracts, and the innovative approaches taken to deal with security issues.

Every two years, industry leaders and practitioners of tunneling and underground construction gather at the Rapid Excavation and Tunneling Conference (RETC) to learn from their peers about the most recent advances and breakthroughs in this unique fleld. Industry experts from around the world highlight their most recent projects and share real-world experiences. They also share with their colleagues new technologies and methodologies that attendees can take home to their respective projects. The 2013 RETC was held June 23-26 in Washing-ton, D.C., itself home to several major tunneling and underground construction projects. The conference is sponsored by SME. Attendance was 1,384 professionals. The accompanying exhibit hosted 159 companies in 188 booths. A few of the topics covered in the technical sessions dealt with caverns and large spans, contracting practices, tunnel linings, design and planning, geotechnical considerations and instrumentation. The conference was preceded by four short courses, all well attended. They were ?Grouting in Underground Construction,? ?Tunnel Lining Design,? ?Microtunneling Application? and ?Risk, Uncertainty and Hard Decisions.? In addition, a sold-out field trip to D.C. Water?s Blue Plains Tunnel took place following the conference. The Blue Plains Tunnel is part of the agency?s Clean Rivers Project. Also during the meeting, three students each received $5,000 scholarships from the UCA of SME, while another three students each received $2,500 scholarships from RETC (See page 53).

INTRODUCTION Mica Dam is located on the Columbia River just downstream of the "big bend" of the Columbia, where it leaves the Rocky Mountain Trench and begins to flow southwards, approximately 85 miles north of Revelstoke, B.C. (Fig.l). The dam was built as one of three Columbia River Treaty dams in British Columbia which regulate the flow of the river and provide flood control downstream in Washington and Oregon. In addition, the three Canadian dams make possible greatly increased power generation at existing power plants in the United States and further development along the Columbia and its tributaries in British Columbia. History Mica Dam, constructed during the period 1967 to 1973, is a zoned earthfill embankment 650 feet high, which has 12,000,000 acre feet of live storage and an available head of 550 feet. Power intakes which would serve either a six-unit surface or underground plant were incorporated into the design and construction of the right abutment of the dam (Fig. 2). Prior to 1971 alternative 2500 MW capacity surface and underground powerplant arrangements were studied. Cost estimates for the two alternatives were the same within the limits of estimating procedures. However, it was believed there was a certain continuing risk in locating a surface plant at the toe of a steep canyon wall. In this study, a 3D model of the shovel motion based multi-body system dynamics in the Automatic Dynamic Analysis of Mechanical Systems (ADAMS) environment was developed. The kinematics model of the shovel was modeled as a multi-body system, which comprises the lower, upper, and the attachment mechanisms. The Euler-Lagrangian equations were employed as the computational framework to describe the dynamic behavior of the model. A virtual prototype of the shovel was built in the ADAMS software environment. This environment allows the visualization of a 3D motion of the general mechanical system and the prediction of interference of the moving components of the shovel. The numerical analysis and animation of digging, swinging and dumping motions for the shovel were performed. The superimposed display of the deployment for the shovel in three phases allows a detailed examination of the dynamic interference among assembly components. From 1993 through 1999 Cyprus Amax Minerals Company successfully completed eight major construction projects around the world. Six greenfield mines were brought into production, an ongoing operation was completely modernized, and an environmental issue was appropriately remediated. The projects stretched over three continents, from arctic Russia to the Atacama Desert, from 16,000feet to sea level, from city suburbs to remote wilderness, from a low of $130 million to over $1 billion. Open pit and underground; mill and leach; gold, copper, molybdenum, and coal were all encompassed. The breadth of diversity experience from these projects inspired Cyprus Amax to create a “best practices” project management protocol, to steer corporate expansion into the next millenium. This paper summarizes those best practices, while high-lighting project management pitfalls to avoid.

There is a growing interest among transportation planners to make more use of the subsurface to preserve the landscape and make transportation more efficient and economical. At the present time, utilization of the underground for transportation facilities is severely restrained by the adverse environmental impact and high construction costs. However, recent studies performed by the United States Department of Transportation indicate that, in spite of such problems, the volume of underground construction has significantly increased during recent years and projections point to continuation of these trends(1). It is estimated that the total volume of transportation tunnel construction will exceed $4 billion in this decade. This level of Investment justifies a significant research program. Due to the common interest in transportation tunnels and to the similarities of problems encountered during their construction, the tunneling research activities of the Federal Highway Administration (FHWA) are closely coordinated with the programs other agencies of the U. S. Department of Transportation DOT); specifically, the Federal Railroad Administration (PRA) and the Urban mass Transportation Administration (UMTA). The FHWA is responsive to the particular needs of these agencies but will benefit from the research results obtained also. In 1971, the Office of Secretary, DOT, initiated plans for an overall coordination of the DOT, FICA, UMTA, and FHWA tunneling research activities. The objectives of this effort have been to reduce tunneling costs by 30% and to increase the rate of excavation by 200% within 10 years. Conventional methods for selecting a production plan for an open pit mine is based on the assumption that ore grades throughout mine life as well as market variables are known with certainty. Another important issue that is also being ignored in the conventional procedure is the value of management flexibility to revise operating policy with time according to the arrival of new information. Based on this oversimplification of reality, an optimum mine plan that is thought to be maximizing net present value (NPV) of the project is selected. Given the multiple sources of risk affecting the estimated NPV, there is a high probability that the actual value will be different from that estimated. This could mislead decision making process regarding selecting an optimum mine plan. This paper proposes a multi-criteria ranking system for selection between alternative mine designs under both geological and market uncertainties. The system is based on integrating multiple market and geological uncertainties as well as the operating flexibility to revise the ultimate pit limits using a Monte Carlo-based real options valuation (ROV) model. To compare the methods, the article applies the proposed system, along with the conventional NPV-based method, to rank possible mine designs for a copper and a gold mine. It has been found that, considering multiple risk analysis measures while integrating uncertainty and operating flexibility results in selecting a mine design that is different from that selected by the conventional method.

The current study is part of the U.S. Department of Energy's (DOE) on-going subsidence research program. The long-term objective of the DOE program is to develop analytical methods of subsidence prediction applicable over a broad range of geologic and mining conditions encountered in the United States. The DOE currently has several contracts in progress to develop a sound data base that can be used in the development of predictive models. Dames & Moore is currently under contract to the DOE to collect and analyze detailed subsidence and hydrologic data from a coal mine employing longwall mining methods in the Eastern Coal Province. Work under Contract No. DE-AC22-80PC 30335 began in October of 1980 and will continue through September of 1984. This paper summarizes the work that has been completed through September of 1981 and presents the results that have been achieved during this period. Technical difficulties encountered during both the installation and monitoring phases are discussed, and recommendations for future work are presented.

Through the process of Riverbank Filtration (RBF), the Louisville Water Company plans to obtain a higher quality source of water for their B.E. Payne Water Treatment Plant (WTP) by means of four collector wells located in the alluvial aquifer along the bank of the Ohio River. The collector wells are connected to a 2,850 L/s (65 mgd) pumping station by a 2,362 m (7,750 ft) long, 3 m (10 ft) finished diameter tunnel located in limestone and shale bedrock, which is approximately 45 m (150 ft) below the surface. High tunnel boring machine (TBM) production rates were achieved in this moderately weak bedrock; most of the tunnel only required support in the crown. An unanticipated water-bearing fault and unanticipated ground behavior proved challenging and required quick thinking to minimize impacts to construction. Tunneling conditions are discussed in detail, together with the methods used for overcoming the groundwater and ground behavior challenges. The tunnel is expected to be completed in April 2009.

This paper describes the development of a life of mine schedule (LOM) for one of the world?s largest known sedimentary Copper-Cobalt districts located in the Katanga Province, Democratic Republic of Congo (DRC). A particular feature of the Tenke Fungurume LOM schedule is the considerable number of pits/phases for scheduling, each competing to provide the best ore available to a capacity constrained process facility. This paper discusses the main scheduling objectives and constraints, data pre-processing as a scheduling aid, the mine scheduling approach, and results and conclusions.
