This Bureau of Mines manual is oriented to the technical needs of an investigation of fire or explosion accidents and includes various practical and theoretical information that is needed in analyzing accidents or evaluating conditions that might lead to an accident. It is applicable to accidents in the chemical, mining, transportation, and other fuel-related industries. Specific procedures for conducting an accident investigation are outlined, and various guidelines are given for developing the fire or explosion evidence. Ignitability, flammability, and physicochemical data are presented for over 500 combustible gases, liquids, and solids; empirical and theoretical expressions are also given for extrapolating the behavior of combustible-oxidant-diluent systems to various environmental conditions. In addition, various data pertinent to damage analysis are summarized to facilitate the task of the investigator in developing plausible accident scenarios.

Numerous developments and improvements have been made to tap-hole designs. On the other hand, there are very specific designs in operation that have endured for many years without any modification. Nobody likes design changes because of risk - the risk of atypical wear, potential hazards, and costly repairs entailing enormous production losses. So the philosophy ‘never touch a running system’ is maintained, because among other things there is no efficient working monitoring technology for tap-holes. In the past few years a couple of new technical solutions have been investigated and developed. Not all theoretical solutions are possible in practice. However, metallurgists are inventive, and the last word regarding efficient working tap-hole monitoring technologies has not yet been spoken.

This paper describes a series of mechanical flotation cell results run with various chemical reagent schemes designed to economically improve the flotation performance associated with selected industrially available coals. The emphasis is on the identification of practical flotation methods of processing coals containing unusually coarse and/or fine feed sizes, of increasing the recovery of oxidized and/or difficult to float coal, and of increasing the selectivity of coal over pyrite in high sulfur coals.

Among underground coal miners, roof bolter operators are generally considered to have some of the highest risks for hazardous respirable dust exposure. This is because bolting requires drilling into roof strata that can often be a source of silica and silicate dust, which are associated with occupational lung diseases. However, little is known about the variability of dust characteristics (e.g., mineralogy constituents, particle size) in the vicinity of the bolter—or when specific dust controls are applied. As part of a prior NIOSH study, respirable dust samples were collected during several different events in standardized locations around an active roof bolter, and personal samples were also collected from the operator during the cleanout of the bolter’s dust collection system when it was equipped with a novel wet dust box versus a traditional dry box. Those samples were made available for follow-up analysis by scanning electron microscopy with energy-dispersive X-ray spectroscopy (SEM–EDX), as well as direct-on-filter Fourier transform infrared spectroscopy (FTIR). Results showed variability in dust constituents and particle sizes at locations around the roof bolter, and indicated some event-to-event differences in dust sources. Additionally, compared to the dry dust box, the wet dust box appeared to reduce (by 41–82%) the relative silica and silicate content in the respirable dust to which the operator was exposed during cleanout. Furthermore, an inter-laboratory comparison demonstrated the reproducibility of a standardized direct-on-filter FTIR method for estimating quartz mass (i.e., the predominant form of crystalline silica) in respirable coal mine dust samples. However, for constituent analysis by SEM–EDX, differences observed between results from two independent labs indicate that standardization of the analytical protocol is necessary to enable comparability of results.

Corrosion affects the performance of rock support and reinforcement at various underground mines, including the George Fisher Mine, where recent studies showed that this risk to mining operations is generally not well understood. Corrosion reduces the capacity and life expectancy of ground support and generally is undetected unless the traditional destructive pull out test is employed to detect defect bolts. Where it is still acknowledged that the pull out test has still an important role to play in determining critical bond lengths for static and quasi static ground support designs, it does not provide an underground operation with any reassurance regarding its bolt's integrity, which has been compromised by in situ aggressive conditions. Non-destructive rock reinforcement calibration testing on single and twin strand cable bolts, rebar bolts and friction bolts at the George Fisher Mine enabled the testing team to detect bolts with compromised integrity in one area of the mine. The non-destructive rock reinforcement integrity testing conducted uses a complex 'Stress Wave Analysis' package based on the processing of clear seismic signals imparted into the rock reinforcement element that is being tested. The seismic signals are processed by 'Fourier Transform' into various criteria which can be used to produce models of the element such as mechanical admittance, frequency spectra and velocity which are all being used in the final modelling of the rock reinforcement element under analysis. This paper highlights the enormous potential to effectively manage the geotechnical risk that corrosion presents for an underground mining operation.

ABSTRACT Nickel compounds vary significantly in their toxicological properties, so it is critical that continuing studies to accurately quantify worker exposures consider the processing that accompanies refining nickel-containing materials. The Clydach nickel refinery, which has operated for more than 100 years, was the first nickel-producing facility to show a high risk for lung and nasal-sinus cancers among workers employed since it opened in 1902. Process chemistry and equipment have changed dramatically over Clydach’s history. This paper describes the processing evolution at Clydach with a focus on the propensity for aerosol formation and the forms of nickel in airborne dusts.

Ground falls (roof and rib) have historically been responsible for nearly 50% of all fatalities in bituminous underground coal mines. In recent years, the number of some annual ground fall fatalities has approached zero, indicating that significant progress has been made. On the other hand, the twin disasters at Crandall Canyon in 2007, in which nine miners perished in violent coal bumps, provided a stark reminder that complacency is premature. One important success has been a great reduction in the number of miners killed inby roof supports. Throughout the 1990s, these accounted for nearly half of all roof fall fatalities, but there have just been two inby incidents since 2005. Progress has also been made in pillar recovery, where there has been just one fatal incident since 2005. On the other hand, more than 300 miners continue to be injured each year by rock falling from between supports and 100 more are injured by rib falls. Together, these two categories also account for a large percentage of recent ground fall fatalities available technologies such as roof screen, rib bolting and inside control roof bolters could reduce injury and fatality rates if they were used more widely. Further advances in these areas will likely be the next big advance in ground control safety.

A THOROUGHLY satisfactory crowd turned out at the fall meeting of the Industrial Minerals Division and took an active part in the entire program. On Thursday afternoon, Sept. 24, a limestone plant was inspected, technical sessions occupying the attention of the visitors all day Friday and Saturday morning. Headquarters were at the Nittany Lion Inn, an attractive hostel on the Penn State campus, at State College, right in the center of Pennsylvania. We are still in the dark, despite inquiries, as to just what breed of lions Nittany lions are. As nearly as we could find out, Nittany is not a Biblical name, as it sounded to us, but a corruption of the name of an Indian princess, or maiden, or maybe just girl that haunted those parts in bygone times and after whom a near-by mountain was named. Penn State athletes are "lions" and there you are. The meeting was supposed to be a joint one with the American Ceramic Society's Materials and Equipment Division; but it was quite evident that the Institute had a much better publicity agent than the Society, which bothered Ross. Purdy, the aggressive secretary of the ceramic people, no end. However, Secretary Purdy saw to it that everyone present fully understood that the Ceramic Society was present, so that that contingent all but stole the show, its representatives presiding at every meeting. The Pittsburgh Section of the American Ceramic Society was also supposed to be holding its fall meeting there at the same time, but made no trouble at all for anybody, as one of the speakers remarked.

In the search for the development of more "environmentally friendly" and more intensive waste disposal approaches to meet more and more stringent environmental regulations, thermal plasma-based processes? are considered one of the most promising alternatives to well-established technologies such as incineration. This paper presents an overview on the research and development status of thermal plasma-based waste remediation technologies. The properties of thermal plasma are described. Due to its unique feature of high-energy intensity and super high temperature, thermal plasma is particularly capable of destroying organic-containing wastes. Its potential and promising application to waste remediation may include: treatment of incinerator ashes; metallurgical dusts (especially, electric arc furnace dust); medical wastes; radioactive wastes; and other Resource Conservation and Recovery Act (RCRA) listed hazardous wastes. Many research institutes and vendors have been involved in the development of thermal plasma-based waste treatment-technology encouraged by the cleanup programs supported by the Environmental Protection Agency (EPA), the Department of Energy (DoE) and the Department of Defense (000). More recently, thermal plasma-based waste treatment technologies (sometimes called plasma-enhanced pyrolysis) are focused more and more on not only just destroying organic wastes but also on how to practically, and economically recover energy contained in the organic waste materials in addition to the recovery of metals in order to offset the high operating costs.

Fiber Reinforced Polymer (FRP) is an excellent material choice for a broad spectrum of corrosive fluids and, in many cases, offers the best value per dollar. FRP Flanges present the greatest design challenge of all standard piping components. The theory underlying the design of flanges is very complex. In addition, due to the nature of FRP material, proper fabrication of the flanges is as critical as the design. In this review, the challenges of making FRP flanges along with the design techniques and best practices of fabrication that will lead to a high-quality flange will be discussed.

During recent years there has been a marked increase in the use of explosives in Queensland coal mines. Delay detonators have been introduced and their use has extended with the wider application of mechanised methods of mining.The need for careful study of the characteristics of delay detonators is stressed so that the advantages of simultaneous firing, from the points of view of safety and efficiency, may be obtained. With their use new potential hazards are introduced. These hazardsare considered and means whereby the overall safety may be improved are discussed.The increase in the number of misfires since the introduction of delay detonators is shown and the examination of misfired detonators is described. Lessons to be learned! from these misfires are noted and details are given of the essential requirements for safe and efficient blasting using delay detonators.INTRonUCTIONOne of the aims of mechanization in coal mines is to reduce the physical effort applied to the winning of coal. Explosives play an important part in achieving this and, following the introduction of power borers for the drilling of shot-holes in Queensland coal mines, much of the arduous work associated with coal getting has been eased. During recent years there has been a marked increase in the consumption of explosives, as shown in Table 1, and it must be the aim of all to use these explosives safely and efficiently. One of the dewatering methods in mineral processing plant is filtration system. Disk filters are included the porous surface with pressure drop on both sides, Water in the pulp passes through the surface and solid particles remain as a cake on it. Wet concentrate production of magnetite processing plant at Golgohar Mining and Industrial Company with solid tonnage of 160 ton per hour and 49% solid amount contains 166 m3/h of water. In the present study, in addition to studying disc filter performance, the effective parameters on cake moisture were studied using Vacuum Bottom-Feed Leaf method (in the laboratory scale). Optimization was performed using the Taguchi method. The results showed that the cake moisture is reduced by increasing particle size and solid percentage (because of specific resistivity reduction), while the rate of cake formation and its thickness are increased. Also, it was found that by increasing pressure, the cake moisture is reduced and cake formation rate and its thickness are increased. In this study, the effect of fabric texture was also examined. Lowest cake moisture was obtained in condition of using compressed plain fabric with mono-multi filament fibres, pressure at 75kPa, solid percentage of 65 to 70, particle size (D80) equivalent to 123µm and disk rotation speed of 1.25 rpm.

The Bureau of Mines surveyed unit trainloading and storage installations in the midwestern, eastern, and southeastern coalfields. Nine individual operations were studied in detail as a representative cross section of unit train operations in these areas. Of the nine operations studied, three were in Illinois, two in Kentucky, and one each in West Virginia, Tennessee, Ohio, and Pennsylvania. Information was obtained from coal producers, coal sale agencies, railroads, construction and equipment companies, and power companies.

The planning, design and operation of mines must include the development of positive environmental, community and land-use benefits if it is to achieve good corporate social responsibility and minimise detrimental impacts throughout the life of the mine. Life-of-mine (LOM) planning should, therefore, consider conducting mining activities in the best socially, environmentally and economically acceptable manner, from the commencement of exploration to post-closure of the mine. Current knowledge of LOM activities in relation to public health (life-of-community, or LOC) impacts neither fully captures the full suite of human health and well-being impacts, nor comprehensively informs society on these issues. This knowledge gap is becoming more apparent as public antipathy is increasing towards mining activities within Australia, Canada and globally. The purpose of this paper is to present preliminary and informative results of a literature review to gain insight into the extent of available public health and well-being knowledge associated with mining development. LOM development must also benefit the LOC, in that it must aim to minimise detrimental impacts, and maximise beneficial (including health) impacts on all associated communities. Health planning and programing for mitigation of potential adverse health impacts of mining should extend beyond the scope of work-related injury and illness (resulting from chronic exposure to harmful substances, infectious and lifestyle illnesses), to the health and well-being of workersÆ families and extended associated communities. Accidental contact of overhead electrical power lines by mobile equipment is a leading cause of occupational fatalities in the United States, accounting for 20% of on-the-job electrocutions. Overhead electrical power line proximity warning devices (PWDs) are intended to warn personnel if mobile equipment moves within some preselected minimum distance of an energized overhead electrical power line. Two commercially available PWDs were tested at the National Institute for Occupational Safety and Health?s (NIOSH) Pittsburgh Research Laboratory (PRL). The objective of the tests was to document performance capabilities and limitations for these PWDs by identifying factors that can influence their operation. The two PWDs evaluated in this research are the SIGALARM Model 210 marketed by Allied Safety Systems, LLC, and the ASE Model 2100 from Allied Safety Engineering. Both of these devices operate by measuring the electric field present around energized power lines. The PWDs were installed on a government-owned 22-st (20-mt) rough terrain crane. A purpose-built test site used for this research at PRL allowed operation of the crane near a variety of power line configurations operating at up to 25 kV. Most of the tests involved positioning the crane adjacent to one or more overhead power lines, adjusting sensitivities of the PWDs to alarm when the crane boom was approximately 20 ft (6.1 m) from the power lines, swinging the crane boom toward the lines under a wide variety of test conditions, and finally, for each unique set of test conditions, documenting the deviation from 20 ft (6.1 m) for actual alarm activation. Test results show that several factors can adversely affect PWD performance. PWD alarm accuracy generally deteriorated when operating with a boom position significantly different than that used for the device?s last sensitivity adjustment. Another factor that can affect PWD performance is configuration of the overhead power line(s) involved. Accuracy of alarm activa­tion distances was best for simple single-circuit installations, but degraded for multiple circuits on the same poles. This degradation was slightly greater for installations with different voltage levels and/or a combination of vertical and horizontal conductor arrangements. Performance also degraded for crane operation between two intersecting power line installations, especially for intersecting lines at different voltages. An additional aspect of power line configuration shown to influence PWD accuracy was phase sequence on the power line circuit(s). Specific phase con­ductor arrangements and combinations, particularly in multiple circuit installations, resulted in either improved or degraded accuracy.

Present conventional techniques for primary steelmaking in both integrated and non-integrated plants are outlined. 'Direct reduction' alternatives are reviewed and categorized according to the nature of the product produced, with particular emphasis on the limited number designed to yield steel as the end product. A proposed new technique, referred to as the Sherwood Process, is described in some detail as a new combination of selected features from the prior technology. This process is in-tended to realize, in integrated operations, the result of continuons steelmaking directly from the ore in a single plant and, in non-integrated operations, continuons steel-making from a metallic charge of scrap, sponge iron or pig iron. Prominent features are continuons processing in elongated, horizontal, rotary furnace equipment, controlled direct heating of each process stage and, for the over-all integrated process, a size separation for removal of excess solid reductant and other waste just prior to the melting step. Material and energy balances are presented as well as exemplary cost data. Reasons for selecting this particular combination of process elements are dis-cussed.

Asmentioned more fully later, other ore reserve situationswere known, or were met during the study, in copper,gold and nickel as well, where poor realization: expectationratios had ranged from embarrassing to disastrous.These made it clear that ore reserve estimation had to belooked at afresh.The fresh look has resulted in a broadening of theconcept of ore reserve estimation from its traditionalpurely computational basis to an approach embracingall the many things that can affect expectation andrealization as essential factors. Hence this guiderepresents a gathering together of what has been learnedand what should be known to all those concerned withore reserves in any of its diverse aspects.The study has been marked"by a number of changesof thinking. At first the aspects that appeared to requireattention were two; recognition that an ore reserve statementwas an estimate, not a precise calculation; and asearch for im improved estimation procedure adapted tolow grade ores with narrow economic margins. Threeyears later the view was that the need was to incorporateinto ore reserve estimation all relevant aspects of theproposed mining operation from choice of a samplingmethod to sales contract specifications.This study is thought to be unique in being frankabout weaknesses in ore reserve estimation and intreating failures not as natural disasters (like drought orflood, which have to be endured) but as potentiallyavoidable. The opportunity to evaluate the pile design procedures used on a major project is an infrequent occurrence. It is even more infrequent that the project involves piles with significant tension and horizontal capacities. This report synopsizes over twenty years of design and construction experience on just such a major pile founded structure. The philosophy governing foundation design procedures is described and discussed. The order and extent of full scale pile testing are evaluated. Principal points in the specifications are established and their effect on construction progress and claims are noted. The report concludes with recommendations for future projects.
