The most frequent causes of accidental injury and fatality are carelessness and ignorance; and the most common accidents are haulage accidents, fall, drowning, and fire. However, in this modern day and age, severe injury has become the dire consequence of an ever-increasing number of industrial and mining accidents as well as sport-, military- , etc. Mine accidents have declined dramatically in number and severity through decades of research, technology, and preventive programs. However, preventing recurrence of disasters like those of the past remains a top priority requiring constant vigilance by management, labor, and government. The goal of the present research is to investigate and identify root causes associated with types of mining accidents (victim being caught by an equipment, bodily motion, over exertion, etc) and nature of injuries such as (sprain and strains, fracture, dislocation, amputation, etc). Further, we hope to discover the impetus of the root causes occurring more and more often in this modern day of technology and quality awareness. With the different dimensions of accidents and injuries brought into the fold, there is the need for researchers and mining operators to comprehend and better still visualize significant patterns of associations among these variables of interest. We assume that, there is a high correlation between many dimensions of nature of injuries and types of accidents. This suggested a principal component analysis to reduce the dimensions for easy and amenable interpretation.

Introduction Elevated zinc concentrations are observed in groundwater within alluvium adjacent to Silver Creek east of Park City, Utah. The study area is located in a small valley one and one-half miles northeast of Park City, Utah in Summit County, Utah (Figure 1). Although surface water concentrations in Silver Creek are typically less than 1 mg/l, groundwater concentrations have been measured as high as 85 mg/l in a near neutral pH environment. The elevated zinc concentrations in groundwater result from several factors including: (1) dissolution of zinc from tailings exposed at the surface and within the alluvium, (2) oxidizing conditions created by surface/ground-water exchange, and (3) ideal geochemical conditions for the dissolution of zinc-containing minerals. These ideal conditions include readily soluble minerals, available anions for complexation, and lack of available surface adsorption sites.

Increasing pressure from State and Federal agencies to mitigate mining-induced subsidence damage to overlying structures has presented a unique problem to the coal industry in the western United States. Because sandstone escarpments are an environmental issue, millions of tons of coal reserves that underlie these escarpments risk being classified as unminable by regulatory agencies. At this time, the effect of subsidence on escarpments has not been well documented or characterized. The Bureau of Mines is using numerical modeling techniques to analyze escarpment response to longwall mining. Two- and three-dimensional finite element models have been constructed for a study area near Price, Utah, where longwall panels were mined near an escarpment. This status report includes preliminary results showing that the pattern of subsidence surrounding the escarpment can be simulated through numerical modeling. Models that include such structural details as joint and fracture patterns will better simulate subsidence patterns .and magnitude. By locating longwall panels strategically in relation to the escarpment, stability may be preserved throughout the mining process. Numerical modeling provides a method to analyze relatively quickly the effects of longwall panel location and resulting subsidence on escarpment stability. The goal is to provide a relatively accurate predictive tool for mine planning to determine if escarpment stability can be maintained while maximizing extraction of the coal reserves.

The first version of the longwall strata control and maintenance system (LSCMS) has been successfully developed by the Strata Control Re¬search Team at the University of Alabama. The system includes a software package designed to monitor shield leg pressure, interpret the data and predict the forthcoming roof loads and the existence of geological anomalies. It can also detect maintenance problems of the face equipment. The LSCMS was installed underground and connected to the surface terminals at Drummond's Shoal Creek Mine in Alabama. The system has proven to be a useful and practical system for improving safety and efficiency in coal-mining operation. The Shoal Creek Mine is a new, large coal mine operated by Drummond with two longwall panels. The Blue Creek seam is the primary source of coal production. It is located at a depth of 275 to 440 m (900 to 1,450 ft) below the surface with an approximate 2% southwest dip. Typical strata conditions are represented by two stratigraphic columns (Figs. la and b). A fireclay/dark shale parting (called Middleman) 0.2- 0.7-m- (1- to 2.5-ft-) thick exists between the Mary Lee and Blue Creek seams. Mining conditions vary from area to area. This mainly depends on the Blue Creek seam thickness that ranges from 0.9 to 2.9 m (3 to 9.5 ft). The thickness of the Mary Lee and the parting is relatively consistent with a to¬tal of 0.9 m (3 ft). To mitigate and control the combined sewer over-flows from 17 outfalls in Detroit, MI, three outfalls in Dearborn Heights, MI and eight outfalls in Redford Township MI, the Detroit Water and Sewerage Department (DWSD) completed the preliminary design of a storage tunnel in 2005. In 2006, a final design revised the alignments and the inside diameter (ID) of the tunnel. The revised tunnel system includes an 11.28-km (7-mile) 9.14-m (30-ft) ID tunnel, a 24.4-m (80-ft) ID pump station, two work shafts about 15.2-m (50-ft) ID each and 14 drop shafts with various IDs. A small portion of combined sanitary and storm sewers serve the majority of the Rouge Valley water-shed. This includes Detroit, MI and most of the western suburbs. The Northwest Interceptor (NWI), that extends between Eight Mile Road and Ford Road and roughly parallel to the Rouge River, intercepts sanitary sewage and some wet weather flows from trunk sewers in the tributary areas for transport to the wastewater treatment plant (WWTP) for treatment. During extreme storm events, when the capacity of the NWI is exceeded, the excess combined sewer and overflow (CSO) over-flows into the Rouge River through 17 outfalls to local rivers. The purpose of the Upper Rouge Tunnel system (URT) is to capture and significantly reduce the uncontrolled CSO from these 17 outfalls in Detroit, three outfalls in Dearborn Heights and eight outfalls in Redford Township. Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

East Side Access (ESA) will extend Long Island Rail Road into midtown Manhattan via 11 km of tunnels, a new terminal station and associated rail support facilities. The hard-rock tunneling in Manhattan will traverse under Park Avenue and some of the most historically and commercially significant real estate in the world. The soft-ground tunneling in Queens will traverse beneath Amtrak’s Sunnyside Yard and the Harold Inter locking, the busiest rail interlocking in North America. This paper details ESA’s program-wide geo-instrumentation monitoring system for obtaining, processing and presenting all geotechnical monitoring data. Data streams will be real-time, such as from automated total stations, and manually read, such as from optical survey instruments and handheld data loggers. All data will be input into a web-based Geo-Instrumentation Data Management System, which will be accessible to project stake holders via ESA’s intranet or via the Internet using remote access controls.

No commodity agreements now exist for sulfur and a lack of reference to sulfur commodity agreements exists in the literature. Law (1975) made a passing reference to sulfur as one of a group of commodities where "there have been additional control efforts of various types." Clearly, sulfur was a candidate, at least conceptually in the past, for some form of commodity agreement and the following discussion centers on the world sulfur market of the early 1980's in this context. GENERAL THEORY OF COMMODITY AGREEMENTS The upsurge in demands for commodity agreements seen in the last decade or so stems largely from agitation by the less developed countries (LDC) for better economic treatment by more developed countries (MDC). Several economic factors, real or imagined, adversely affect the LDC's as follows (MacAvoy, 1977): 1) variable demand in MDC's causes wide fluctuations in commodity prices which hurt the economies of commodity producers, who are often LDC's although consumer economies may also benefit from more predictable prices; 2) a general decline of commodity prices relative to the price of manufactured goods which hurts commodity producers in their balance of trade while their stocks and overcapacity rise; 3) instability of LDC economies enhanced by the above factors lowers internal stability, inhibits foreign investment, and retards economic growth; and 4) a lack of self respect when dealing with developed countries o, following the OPEC lead, LDC producers want the stature related to cartel market power.

Pennsylvania Power & Light Company is an investor-owned public utility which was incorporated under the laws of the Commonwealth of Pennsylvania on June 4, 1920, with general offices at 901 Hamilton Street, Allentown, Pennsylvania. At the present time PP&L is operating five coal-fired steam electric stations, two hydroelectric stations and one steam heat plant which is being converted from anthracite to oil fired. In addition, PP&L is a partner in both the Keystone and Conemaugh mine mouth bituminous coal-fired steam electric stations in Central Pennsylvania and also part owner of a company which operates another hydroelectric station. PP&L plants, other than the two mine mouth stations, are located in Central Eastern Pennsylvania. (Figure 1) The company derives about 99% of its operating revenues from supplying electric service, while the balance is from the one steam heat plant serving the city of Harrisburg. The entire service area covering 10,000 square miles in 29 counties, has a population of approximately 2,300,000 people. The area served is mostly suburban. It includes 109 communities each having a population in excess of 5,000 people. The largest cities served are Allentown, Bethlehem, Harrisburg, Hazleton, Lancaster, Scranton, Wilkes-Barre, and Williamsport which range from 50,000 - 120,000 in population. So you see our service area does not include any major cities. The area has industrial, commercial, residential, agricultural and recreational activities that are linked with the Eastern Megalopolis by an extensive interstate limited-access highway system and railroad networks. We consider this Central Eastern Pennsylvania area we serve as the "heart of the market" -- an area with great growth potential.

"Mining makes civilization possible. It also creates socio-economic problems surrounded by sustainability controversies yet continues extraction without considering the balance and finitude of nature. Nature requires restoration of equilibrium if mining and life on earth are to continue. Mining is still practiced according to Agricola’s views and the consequences it has wrought on our planet demand we critically reexamine it from its very core, to its metrics, to its design, to its technology, to how it is taught, and to reconsider its raison d'être in 2017. This paper offers tools to new mining engineers that are required to transform and solve new challenges. The task before us is to formulate an updated core curriculum that meets the demands and realities of the 21st Century. Mining professionals must rethink and reteach how to redesign mines before shovel hits dirt, how to mine with restoration of nature figured in to the mining design. Mining needs a new extractive technology in order to transform the results of its well-known liabilities. Mining has evolved and its future must be different than its past. Our industry is capable of doing this but it needs to be shown the way. INTRODUCTION The statement that ‘mining makes civilization possible’ is of the utmost importance and has the most value to us. We can examine the past and present appropriately to see the future that we are building. Mining must be addressed in a different way that makes a positive difference and improves upon rather than continues to get the negative impacts of the past. See Fig 1. Mining creates disequilibrium in Nature by producing ore, waste, tailing, dilution of resources, toxic, and pollution to its ecosystems. What is next for mining on a limited planet? Delaying the transformation of mining is not an option to developing an updated and modern mining business-as-usual paradigm. It is hurting the essentials of living in a civilization; our livelihood is in danger to the point of unrest in nature as we generate a lot of eco-print by present mining practices. So, we are urged to eliminate this bottleneck in life by employing a different design. This way, we will have a vision on how to sustain our civilization. [1]. This new design, with input from current and future generations, will be our contribution to posterity."

Waste phosphatic clay presents a difficult disposal problem and a significant loss of P2O5. Recent developments make this study on the use of phosphatic clay as a feed material for H3PO4 production worthwhile. (a) New data from Florida Industrial and Phosphate Research Institute (FIPR) suggest that the Bclay^ waste can be effectively treated by improved solid separation coupled with flotation to produce feed material for recovering additional phosphate. (b) The use of BOLI Flowsheet^ provides capability to predict performance of aqueous-based processes for a range of possible operating conditions. Using the new FIPR data and BOLI Flowsheet^ enabled the design of a new process that can potentially extend the usefulness of current P2O5 ore deposits. Predictions using OLI Flowsheet were able to identify process conditions that effectively overcome the difficulty of high Mg content of phosphatic clay. Estimated manufacturing cost in the current study lead to valuation of the P2O5 content of the phosphatic clay recovered/utilized at 60 to 70% of that of the phosphate rock feed material currently used. Phosphatic clay should be considered as a valuable material that is currently given zero value. The industrial vision system provides on-line 2D particle size analysis for aggregate slurries in both the overflow and underflow of hydrocyclones. This paper covers required vision system features allowing CCD cameras to function in the harsh environment of a mine milling area. The study conducted demonstrates the system’s capacity to obtain particle size distributions for controlling hydrocyclonic separators. The particle size distribution is obtained within seconds and immediately updated to allow for process adjustments. Results are then provided as a graphical computer generated display or output as a 4-20mA current loop for process control. The primary use of the vision system is in classifying particles in cyclone overflows which are often a few microns in size. Typical size information for actual mine slurry samples are presented and possible control schemes discussed.

As part of a $2.2 Billion bridge project that will link Indiana and Kentucky, an interchange project northeast of Louisville will require the mining of a 6-lane tunnel system about 2,000 ft long in order to cross beneath the historic Drumanard Estate. This paper will discuss preliminary engineering that has been undertaken to compare two-bore and three-bore layouts, the planning of an exploratory tunnel facility, and the potential for including a test chamber to investigate the ability of high capacity roadheaders to mine the 22,000 psi limestones.

Surface subsidence induced by underground coal mining activities, especially longwall mining operations, can cause various problems to interstate highways ranging from structural integrity, drivability to safety. Accurate subsidence prediction, correct influence assessments, and effective mitigation measures are the keys to ensure the safety of highway traffic. The surface subsidence prediction program, called comprehensive and integrated subsidence prediction model (CISPM), was developed based on the principle of influence function method and has been validated with a large number of subsidence measurements. In this paper, the techniques to use the predicted final surface movements and deformations for assessing their influence on the integrity and functionality of highway structures (e.g., road surface pavements, concrete slabs, transverse joints, etc.), vehicle dynamics, comfort level, and drivability are presented. A number of mitigation measures to protect the highway structures and to ensure traffic safety are proposed along with an application case.

In order to better understand the effects of longwall mining on the methane reservoir overlying the panel, the National Institute for Occupational Safety and Health/Pittsburgh Research Laboratory (NIOSH/PRL) designed and implemented a vertical borehole monitoring experiment (BME) on an active longwall panel. The mine operates in the Pittsburgh Coalbed in Greene County, Pennsylvania. The BME site was located away from either end of the panel to avoid mechanical influences caused by the draping of overburden in this region and was positioned at the same distance from the tailgate gateroad as the mine?s gob gas venthole sites. The borehole drilling depths and monitoring intervals were chosen to address a different stratigraphic zone in each of the three planned holes. The monitoring boreholes were positioned perpendicular to face advance using a close spacing pattern of about 15 m (50 ft) between collars. Boreholes were instrumented downhole and on the surface for various measurements. It was observed from the field data that the gob gas venthole fracture network formed 24-46 m (80-150 ft) ahead of the mining face. Overburden permeability?s within the same test zones increased 100 to 500 times following undermining. The borehole test interval within the fractured rock responded to overburden gas pressure while the test interval in the caved zone responded to mine ventilation conditions. Eight months after the boreholes were undermined, slug testing indicated permeabilities had increased further in the fractured rock zone. Measurements of surface movement indicate that the permeability development observed in the boreholes is a product of vertical subsidence without lateral movement within the tensional rock mechanics zone on a supercritical longwall panel. Results from this study suggest that maximum permeability in the longwall panel overburden is achieved 58 to 190 m (190 to 620 ft) behind the longwall following maximum compaction of the overburden.

For many years, underground workings in the Bou Azzer mine have been challenged by geotechnical problems related to the serpentinite instability. The stage of evaluation of the compression potential is crucial to develop an appropriate drilling method and the choice of non-standard support. This research work presents a retro-analysis of serpentinite behavior in underground excavations at different depths, using a multi-approach method, based on several empirical and semi-empirical and theoretical-analytical methods, calibrated by the observational method. The analysis performed in this work reveals that underground excavations in Bouazzer East and Ait Hmane area would encounter compression problems that were recognized as the most serious from a depth of 300 m because of the effects of lithology and tectonics. Therefore, the Aghbar region has a slightly negligible compression potential. The compressive failure mechanism in this mine, depending on the mechanical strength, is started at induced stress of 9 MPa.

EXECUTIVE SUMMARY An approach borrowed from the petroleum industry was applied to the gas development evaluation for the Bulga underground coal mine in New South Wales, Australia. The results enabled the coal mine operator, Xstrata Coal, to systematically plan the extraction of gas in harmony with mine production planning of multiple coal seams. This has allowed for a more proactive approach in preparing the resources and infrastructure needed to capture the gas and accelerate coal mining while maintaining their high level of commitment to safety. Norwest has for decades used gas recovery simulation models to assist the petroleum industry and the coal bed methane (CBM) industry in understanding and forecasting fluid saturations and flow behavior. Norwest developed a similar process with Xstrata to migrate existing mine data at Bulga into reservoir simulation models to study various drainage alternatives. An interesting feature to the evaluation was also developing an understanding of test procedures and nomenclature from the mining industry to properly translate the data for input in simulators developed for the petroleum and CBM industries. The model was then calibrated to actual field data collected from drainage pilot tests that Norwest helped design and monitor at Bulga. The resulting calibrated predictions were then used to develop budgets, schedules, resource requirements, and alternatives for capturing the gas and delivering it to market ahead of mining. Increasing revenue, reducing emissions, improving schedules and cutting costs for the mining operations have all contributed to justifying this advanced approach at Bulga. It has also allowed Xstrata to expand their focus to the sizable gas reserves in seams not currently planned for mining. Several of these seams would be economic gas development targets once the gas recovery infrastructure is installed for the mine. The evaluation process was refined throughout this project and the resulting approach will benefit Xstrata with their broader strategic development of resources in the Hunter Valley and beyond to their worldwide operations.

Blast-induced rock mass displacement can have a significant impact on ore loss & dilution. The mischaracterisation of the grade boundaries both prior to, as well as a result of blasting can lead to significant financial losses on account of ore loss and dilution. Ore dilution occurs when waste material is miscategorised as ore and sent for processing or stockpiling, diluting the run of mine head grade and recovery. Ore loss takes place when valuable mineral is miscategorised as waste and sent to the waste dumps, thus never being processed and recovered. Extensive research has been conducted at the University of Queensland over the last fifteen years to understand the impact of blast movement on ore loss and dilution. Recent outcomes of this research have developed innovative tools and techniques to measure and model blast movement. These techniques have been implemented at Ahafo gold mine in Ghana to understand blast movement dynamics and to implement solutions to reduce blast-induced ore loss and dilution. These solutions are validated through controlled trials and have been incorporated into site standard operating procedures to sustain the benefits.
