"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

There is a strong focus in the mining industry to limit its impact on water quality and usage. This is even more evident in developing countries, where mining companies operate within local communities and have to be diligent not to affect them negatively. In mineral processing operations, the vast majority of water reclamation occurs at the tailings thickener area and tailings storage dam. It is often more effective to maximise water recovery at the tailings thickener before material transport to the tailings dam. Tailing dams are relatively inefficient, and experience a high degree of water loss due to evaporation, seepage and water retention. In addition, tailings dams present the biggest environmental liability for mineral resource projects, so it is beneficial to limit their size and loading.In the past, it was common for tailings thickener areas to have a significant footprint with several large diameter conventional thickeners, producing underflows of between 45-55% solids (w/w). At times, these multiple thickeners were difficult to operate, leading to inefficiencies in water recovery and quality. The development of High-Rate Thickeners (HRT) has resulted in improved settling properties and HRTs usually operate at significantly higher solids loadings (= 1.2 t/m2.h) when compared to conventional thickeners (= 0.1 t/m2.h). Subsequently, thickener diameters have reduced in size and HRT technology has become the preferred option for thickener circuit design in both new and existing operations. HRTs also produce higher water recoveries with increased underflow densities = 55% solids (w/w).The Carmen Copper Corporation (“Carmen Copper”) circuit upgrade is a recent example of these HRT benefits, where one, 39 m diameter, Outotec HRT treats the solids flow rate of five 76 m diameter conventional thickeners. This has allowed operations to increase dry plant tonnage by 50%, maintain key performance targets and provide a framework for further plant expansion. "Numerical modelling is becoming universal in rock engineering practice. Most applications focus on the prediction of failure loads of 2D problems, where the numerical models are useful extensions to analytical methods and experience-based design criteria. For 3D problems and problems where the assessment of serviceability indicators – displacements, ground-structure response, etc. – are of importance, the selection of the constitutive models themselves and the implications of the selection of material parameters may have a very heavy impact on the reliability of the model outcome and its predictive capability.This paper overviews basic common aspects of constitutive equations used in practical rock engineering and summarizes some challenges that are frequently observed when reviewing numerical models performed for routine problems in rock engineering. INTRODUCTIONNumerical methodsFrom a philosophical perspective models, either analytical or numerical, are idealizations of a perceived reality. For us engineers, models are just tools we use to guide our judgement and understanding of the problem in hand. When dealing with problems in rock engineering, we resort to a wide range of techniques:1. Closed form solutions like confinement-convergence methods for circular tunnels: these methods satisfy equilibrium and compatibility requirements at the cost of a crude simplification of the material behavior and geometry;2. Limit analysis methods, like bearing capacity formulas: lower bound methods satisfy equilibrium but sacrifice compatibility, and upper bound methods do the opposite. Material behavior must be simple, geometry admits limited freedom;3. Limit equilibrium methods, like method of slices for slope stability: they satisfy restricted forms of equilibrium, do not satisfy compatibility requirements, require simple material behavior but have a much greater flexibility on geometry;4. Numerical methods, where a mesh is employed to discretize the problem geometry: when properly formulated, these methods satisfy equilibrium and compatibility requirements and give reasonable approximations to the behavior of very complex geometries and construction sequences. Very little restrictions remain on geometry and material behavior, but the cost of calibrating the material parameters can be very high."

It has previously been observed that industrial high-titania slags display a compositional invariance, remaining close to stoichio-metric M3O5. The present study focuses on this invariance and the phenomena which may cause it. The invariance was studied using thermochemical calculations and a mechanism is proposed for the observed behaviour. The mechanism suggests that a combination of thermochemical equilibrium and transient slag solidification and melting results in the observed compositional invariance. The proposed mechanism provides new insights and prompts questions about other mechanisms in the smelting process.

Interest in mergers in the nonfuel mineral industry developed when 5 were listed among the 50 highest valued mergers in the United States in 1981. This report presents a Bureau of Mines study of trends in merger activity from 1960 to 1979 and explores motives for mergers in the nonfuel mining and mineral industry. The objective was to determine precedent for or a trend toward the 1981 acceleration in merger activity and to examine certain financial motives for non fuel mineral mergers. Data indicate that the relative value of non fuel mineral mergers has grown since 1972 although the number of mergers has remained proportionate to historical trends for all mergers. Since 1974, the assets acquired in "large," nonfuel mineral firm mergers, those of $10 million or more, have comprised at least 93 pct of assets acquired. More nonfuel mineral firm mergers involved mineral companies as the acquiring firm than vice versa but mining and minerals firms acquired were of higher value than nonmining firms. A major motivation for mergers involving large mineral firms was the acquisition of assets at less than replacement costs. Univariate statistical testing, a methodology chosen for its applicability to the data, confirms this hypothesis while rejecting other financial motives. The remainder of the work is devoted to a discussion of suggested areas for future study.

As a part of the strategic minerals program of the Bureau of Mines, nine chromite-ore samples were obtained during the period 1949-56 from mines in the vicinity of Red Mountain, near Seldovia, Kenai Peninsula, Alaska. This report summarizes the results of beneficiation studies of the samples at the Bureau's Juneau, Alaska, and Albany, Oreg., laboratories. In general, the ores were found amenable to concentration by gravity methods for producing concentrates that assayed over 48 percent chromic oxide. Intimate association of chromite and gangue minerals, however, necessitated fine grinding and limited the amount of chromite recoverable. Recovery of chromium in concentrate that assayed 48 percent chromic oxide or higher ranged from approximately 20 percent for low-grade ores (10 to 12 percent Cr203) to more than 95 percent for 1 high-grade sample (42 percent Cr203). The iron content of the chrome-bearing mineral -hence, the chrome/iron ratio of finished concentrate -appears to vary with the grade of the ore. Concentrates made from high-grade ores had chrome/iron ratios as high as 3.1:1; the concentrates from low-grade ores, however, had chrome/iron ratios as low as 1.8:1.

The importance of goethite to Australian iron ore production is steadily increasing as the proportion of goethite-rich, martite-goethite bedded and channel iron deposits increases at the expense of declining production from high-grade microplaty hematite deposits. For this reason, an improved understanding of goethite in iron deposits, its genesis, goethite types and their physical and metallurgical properties is vital to maintaining optimum sintering and blast furnace performance in Asian steel mills.The terminology used to describe goethite in Australian iron deposits is currently highly variable. The discredited mineral name ‘limonite’ is still in widespread use as a field term for goethite and there are many misconceptions concerning goethite chemical grade and downstream physical and metallurgical properties. The goethite classification proposed here is based on geological and physical properties from which three types are recognised including brown goethite, dark brown-black vitreous goethite and highly microporous, friable yellow ochreous goethite.In practice, typical vitreous goethite with highly reflective surfaces and conchoidal fracturing, characteristic of the hard cap zone appears from X-ray fluorescence (XRF) and electron microprobe analysis to be significantly higher in Si (up to 18.7 wt per cent), Al (up to 3.5 wt per cent) and S (up to 0.14 wt per cent) compared to the generally very low (<1 wt per cent for Si and Al) levels of these elements in ochreous goethite and brown goethite. The data dispels the common misconception that friable earthy or yellow ochreous goethite is high in Al and Si and other chemical impurities. However, P was found to be consistently higher (mean of 0.112 wt per cent and up to 0.296 wt per cent) in ochreous goethite than brown goethite (mean of 0.011 wt per cent) or vitreous goethite (mean of 0.031 wt per cent). The highly porous microstructure of ochreous goethite is thought to have a much higher surface area, which may aid in preferential adsorption of P onto the ochreous goethite mineral surface.CITATION:Manuel, J R and Clout, J M F, 2017. Although many investigations of the deformation behaviour of rocks have been carried out, most of them are carried out in compression. In all rock engineering practice it is usually assumed that the deformation of rocks in tension is the same as that in compression. However, data from numerous researchers have shown that this assumption is not correct for some rocks. A new loading frame for testing rocks in compression and direct tension has recently been developed by the authors, with the capacity to conduct both compression and tensile tests on the same rock sample. The results obtained show that the ratios between the average Young?s modulus in compression (ECA) and in direct tension (ETA) are 1, 1.16, 2 and 6 respectively in four rock types tested. For Poisson?s ratios, analogous relations were obtained. It was observed that, in all cases, the tangent Young?s modulus intension reduces with an increase in the applied tensile stress, while the behaviour of Poisson?s ratio is similar, but shows greater variability. The difference between the Young?s modulus of rocks in tension and that in compression could lead, large errors in calculating induced stresses around underground openings, as well as in determining the Brazilian tensile strength of rocks. This may have a significant influence in rock engineering and therefore more investigation is necessary into the tensile deformation behaviour of rocks and its influence.

The United States Bureau of Mines, in cooperation with the Natural Gas Department of the American Gas Association, has been studying actively for the past-several years the flow of natural bas through high--pressure pipe lines.4 This report deals with that part of the study involving the design of pipe-line, systems consisting of lines laid parallel to each other. The lines are deferred to as the original line and the parallel line. The paralleling line may or may not extend the entire length of the original line; however, the parallel line is, connected at its two ends to the original line. As a result of the large expansion of the natural-gas industry almost all markets within economic distance from the developed gas fields have at least one pipe line supplying them with natural gas. Small increases in the consumers' demand for natural gas can be met usually by increasing operating pressures, but any marked increase in demand necessitates paralleling sections of the existing systems. As part of JAGUARS (JApanese-German Underground Acoustic emission Research in South Africa), high-resolution tiltmeters were installed during 2007 at Mponeng gold mine, South Africa, in an attempt to study and understand the risks posed by in-mine seismic events and to subsequently use the knowledge gained to mitigate these risks. In December 2007, an M2.2 event occurred at Mponeng and was detected by the tiltmeters installed. The tilt expected for the M2.2 event is modelled through the use of numerical and analytical tools and utilizing, as input parameters, characteristics of the event such as the rupture area, the amount of slip, elastic properties of the rock, the state of stress before the event occurred, and frictional parameters of the rupture. The calculated tilt values are correlated and compared with the recorded data, and are found to be of the same order. Certain input parameters, such as the initial rupture point, are further constrained by varying them, during modelling, until the calculated and observed tilt values are approximately equal. From modelling, it can be concluded that the rupture point had to be closer to the location of the tiltmeters. In addition, possible locations acting as source areas for the observed aftertilt are determined. Further modelling still needs to be done to quantify the effects of the tunnel close to the tiltmeters and plastic deformation of the rock mass. Although near-term prospects are poor for recycling most present wastes, urban refuse is an important exception. Such wastes comprise a mine above ground, and much less processing energy is normally required to recycle the constituents of urban ore than to obtain them in equal amounts from natural raw materials.

The beneficiation of heavy metal oxides, such as iron ore, is usually performed with heavy medium separation in cyclones, static baths and lately with Larcodems. Atomized ferrosilicon is most suitable as a heavy medium at densities above 3600 kg/m3 whereas a mixture of milled and atomized ferrosilicon is suitable at densities below 3600 kg/m3. When operating a beneficiation plant at densities higher than 3600 kg/m3 the quality of the ferrosilicon particles is an important factor that influences the control of the beneficiation process. Therefore, it is important to know how the rheology would be affected by the ferrosilicon quality as it also influences the control of the process at these elevated densities. Background The Kumba Resources Sishen Iron Ore Mine in South Africa produces 26.5 Mt iron ore per annum from its beneficiation plant. The ore body consists mainly of laminated and massive type hematite ore that is crushed down to ?90mm before being beneficiated by means of a combination of Wemco drums, Larcodem and dense medium cyclone separators. The need for higher densities When the percentage of high-density gangue2 is higher than normal in the run-of-mine (ROM), a higher separation density is necessary to produce a product to specification. Certain ore types of this quality in the Sishen reserves require a separating density higher than 4000 kg/m3, whereas densities would normally average 3600 kg/m3 when beneficiating ores of normal high-density gangue content. Sink and float tests conducted on the ROM material at Sishen revealed some interesting characteristics. In the waste fraction, (density < 3600 kg/m3) it was found that the %Fe was <21% (Fig. 1). Similarly, for the product fraction (density >3000 kg/m3), the percentage Fe varied between 35% and 67.2%. The bulk of the material (88%) actually had a density greater than 4000 kg/m3 and a Fe content that varied between 63% and 67.2%. At a density interval of 3600 to 4000 kg/m3, the Fe content varied from 37% to 60% with an average of 45%.

The Liaoning Dahuofang Reservoir Water Transfer Project is an important development for the Department of Water Resources of Liaoning Province in northeastern China. The 85.3 km water conduit will transport water from the Hun River downstream of the Huanren Reservoir to the outlet on the Suzi River leading to the Dahuofang Reservoir. The additional water is required to sustain the growing cities of Fushun, Shen-yang, Liaoyang, Anshan, Panjin and Yingkou. The project is divided into seven contracts. Three TBM contracts comprise almost 75% of the work and the others are drill and shoot. The focus of this paper is about the creditable TBM performances achieved and some of the problems encountered by Chinese contractors; notably two having had no previous TBM experience.

The methods of reinforcement to be discussed in this paper are all insitu methods, ones that can be used on the soil structure as it exists in the ground, and do not rely on excavation and replacement of the soil. Current practice in the United States, as well as new ideas and methods, will be discussed as they relate to slope stability, retaining walls, and to other uses such as control of mine or cavity related subsidence and to liquefaction control under existing structures. The method is based on the concept of "nailing" or reinforcing the soil in place, and thereby causing a mass of soil to act as a coherent unit. When performed properly, a true gravity structure can be constructed, one that will remain intact under both horizontal and vertical loadings and that will also resist vibrational and seismic loadings.

Keywords: Solvent extraction, dispersion, phase separation, CFD The liquid-liquid dispersion phases are separated by gravity in a solvent extraction settler. This separation step is made more effective by picket fences. The picket fence geometry and its physical placement have a significant effect on the separation process, because the purpose of a fence is to smoothen and control organic-aqueous dispersion flow in the first part of the settler and achieve a deep and dense dispersion layer in the front end of the settler resulting in a clean phase separation. In this study, physical phenomena in the solvent extraction process and phase separation in a solvent extraction settler with and without picket fences have been examined using a commercial CFX software package. It can be concluded that the used CFD-approach proved to be a complementary tool for optimizing and designing the solvent extraction settler.

Modern electrolysis technologies can provide possibilities for developing sustainable processes in terms of energy and environment. Laboratory experiments were carried out to study electrodeposition of iron from molten salts and aqueous solutions using an inert oxygen evolving anode to eliminate the emissions of CO2. The possibilities of molten salt electrorefining of metallurgical grade silicon to produce solar grade silicon were also investigated. Electrochemical techniques were used to study the electrochemical behaviour of dissolved iron, silicon, and titanium species. Bulk electrolysis was carried out to deposit pure iron, silicon, and titanium. The quality of the deposits was studied by SEM/EDS and XRD analyses. It had been more than a decade since this many booths were occupied at an SME Annual Meeting, but at the 2010 meeting in Phoenix, AZ 630 booths were sold to 470 vendors from the entire spectrum of the mining and minerals industry. Exhibitors such as Metso, FLSmidth, West-Tech, ABB, NIOSH, Eriez and Resource West took their usual prominent places on the exhibit floor and were joined by newcomers such as tunnel boring machine manufacturer Herrenknecht AG, Frontier Kemper, the International Society of Explosive Engineers and more than 100 other first-time exhibitors. On the floor, one could find everything from a new pair of boots to a new career in the recruitment pavilion where some of the biggest companies in mining (Barrick Gold, Freeport-McMoRan, Newmont Mining, etc..) set up shop. The following is a look at some of the companies and products that were on display at the 2010 meeting. Thirty mixed cultures of microorganisms from soil, water and mineral samples from the Groote Eylandt Mining Company mine site were approximately three times more effective in biodegrading manganese dioxide (34&#177; 18 mg Mn++/1/day) than sixteen purified strains (11 &#177; 10 mg Mn++/1/day) isolated from the primary enrichment cultures. Two pure bacterial genera identified as a Pseudomonad and a Bacillus, with an excess MnOz and nutrient, were able to dissolve 106 mg Mn++/litre/day in mixed culture compared with 29 mg Mn++/litre/day when grown separately.

Researchers at the National Institute for Occupational Safety and Health partnered with the water well industry to develop engi­neering control technologies and training products to address noise exposure at the job site. Noise­induced hearing loss continues to be a problem in the well drilling industry. Research con­ducted by NIOSH found that air rotary drill rigs produce sound levels more than 90 dB(A) over the course of an eight­hour workday. Sounds levels above 90 dB(A), identified during hammer drilling and the hammering of casing, exposes workers to levels that can damage hearing. A typical air rotary drill rig provides no barrier, for noise and dust pro­tection, between the drill’s control panel and the drill stem. The development of a partial cab is the clearest example of NIOSH’s research partnership with the water well industry. NIOSH researchers determined that an engineering noise control to protect the oper­ator at the rig’s control panel would be effective, based on typical work site conditions. Many surface rigs do not have full cabs as a result of the original equipment manufacturer design or the high cost of aftermarket products. Developed as a partial cab, the NIOSH engineer­ing noise control design addressed stakeholders’ needs associated with cost, transport, and worker usage (Figure 1). Field tests found the partial cab effective in reducing sound levels at the rig’s control panel, a typical location for a drill rig operator. Sound levels at the control panel were reduced from 104 to 96 dB(A) (Figure 2). Coupled with continued use of personal protective equipment, such as hear­ing protection devices, the partial cab can help to reduce noise exposure. NIOSH researchers created a CD with informa­tion about noise exposure and the partial cab. Most important to well drilling companies is information on how to construct and attach the partial cab to drill rigs. The CD can be viewed on a standard computer and includes a video of the partial cab in use on a work site. Computer­aided design drawings are included on the CD and provide detail of the cab design as well as information on construction mate­rials and design details. Additionally, the CD provides background infor­mation on noise­induced hearing loss and hearing protection devices to increase knowledge about this occupational health concern. This information can be viewed by all company employees to help promote the partial cab’s importance to workers using the cab. More information on the partial cab noise control technology can be downloaded from www.cdc.gov/ niosh/mining NIOSH researchers also examined the health and safety training needs of workers in the ground water industry. It was quickly realized that most workers learn health and safety on the job and do not attend any sort of formal training. In addition, there is not a lot of time for daily “tailgate talks” or “shop talks” on health and safety issues. As a small business industry, the owner is often operating a drill rig along with his other employees. In general, new employees learn the job in an apprentice style sys­tem. As a result, workers and owners have few opportunities to access uniform health and safety information based upon the job — an outdoor work environment with daily change in work sites. With this information, NIOSH researchers devel­oped two products that deliver health and safety information to fit into the daily work of well drillers. This report describes the performance of 17 selected water-injection projects producing from Pennsylvanian-and Permian-age reservoirs. Illustrations depict reservoir structure and thickness, and production and injection history. A selected bibliography of publications pertaining to area oil recovery and geology is included. Tabulations of field and reservoir data reveal marked differences in the projects. Increased oil recovery resulted from injecting water in most cases; however, seven projects recovered less oil than the operators anticipated. Oil recovery by water injection ranged from 0.2 to 262 bbl per acre ft, or 0.1 to 22.5 pct of the initial stock-tank oil in place. Major reasons suggested for poor performance include: (1) Adverse directional horizontal permeability, (2) channeling of injected water through fractures or zones of high permeability, and (3) lack of containment of injected water. One 330-foot-deep project is among the first successful shallow water-floods in north Texas. Another project is the first successful Canyon sand-stone waterflood reported in the area. Previous gas injection in two Cisco sandstone projects did not prevent the reservoirs from responding favorably to waterflooding.
