Minutes after the Friday afternoon October 16, 2009 announcement that the 80 year old Lake Champlain Bridge was closed to traffic out of concern for public safety, emergency detour planning started at VTrans and NYSDoT. The closure severed a cross-lake link between Addison, VT and Crown Point, NY, which created a 76 mile detour and significant hardship for commuters, farmers working on both sides of Lake Champlain, and businesses relying on both traffic and employees commuting between Vermont and New York. Rehabilitating the bridge was determined to be economically and technically unfeasible and the design of a replacement bridge was accelerated for a spring 2010 construction start. In the meantime, a temporary ferry alternative was to be rushed into service beside the original bridge by February 2, 2010. This paper presents the case history of the fast-tracked ferry dock design and construction on the Vermont side. Twin 300-foot-long pile supported approach spans, a loading slip system and ice deflectors were designed, fabricated and constructed in 90 days in the fall/winter of 2009-2010. This chronicles the project from initial concepts through the extraordinary cooperation between state and federal agencies, and near round-the-clock winter construction on Lake Champlain. This project and its companion project on the New York shore succeeded by virtue of exceptional cooperation between seasoned regulatory, design, and construction participants and warrant this case-history presentation of the Vermont project to share within the industry as an example of overcoming extraordinary challenges.

A study of literature on the epidiorite dykes which transgress the Broken Hill lode and its wall rocks reveals a considerable diversity of thought regarding the time of emplacement of such dykes. In this paper it is deduced that the epidiorite dykes were formed at a late stage and were probably post-ore. Detailed mapping, sampling, and correlation of data from over 100 exposures substantiates this deduction. Physical disruption of the epidiorite dykes is indicated and some interaction of the epidiorite and ore at the time of intrusion is inferred.Further petrological study may confirm this.

South Africa (SA), a country in which all three commercially important asbestos minerals have been mined and milled, has retained proven cases of mesothelioma linked with environmental exposure to asbestos. This study illustrates the importance of fiber type in the occurrence of environmental mesothelioma. Four studies have reviewed the source of occupational or environmental asbestos exposure in 504 histologically proven cases of mesothelioma in South Africa. One hundred and eighteen cases (23%) were thought to be related to environmental exposure to asbestos. In the vast majority of these cases, exposure was linked to crocidolite mining activities in the Northern Cape Province. Two cases were thought to have occurred in relation to amosite and Transvaal crocidolite exposure in the Limpopo Province. In the balance of cases there was some uncertainty. No cases were reported with exposure to South African chrysotile. Consequently, in the vast majority of cases of mesothelioma, environmental exposure to asbestos occurred in the Northern Cape Province, in proximity to mines, mills and dumps where crocidolite was processed. Crocidolite appears to be far more mesotheliomagenic than amosite, and chrysotile has not been implicated in the disease. Session 2, Deep Foundation without Soil Excavation, has attracted no fewer than 22 papers dealing with subjects from soil improvement by grouting, injection, and deep mixing techniques through vibroflotation and special piles, including aspects pertaining to conventional driven and bored piles. In the short time available, it is not possible to both do justice to the papers and provide a critical review of the state-of-the-art. Instead, to meet the objective of making the next half hour worthwhile to the audience, I have chosen to comment on a few special points brought forward in the papers and thereafter to discuss more in detail a couple of aspects I have deemed to be of general interest. Four papers deal with jet-grouting, injection, and deep mixing techniques: DOMSKI, J. J. Reinforcement of subsoil under the foundations of historical buildings in Cracow using deep injection technique. RESTELLI, A. B, BERTERO, M., and LODIGIANI, E. The efficiency of water sprays in suppressing airborne colliery dust depends on many variable factors. A survey of previous work in this field shows that swirl spray nozzles are the best dust suppressors.Investigations carried out in an experimental dust chamber show that, in spite of limitations to the application of water sprays in underground use, promising dust suppression efficiencies in mechanised mining can be obtained with swirl spray nozzles.INTRODUCTIONA dust hazard has always been associated with coal mining, and the recent growth of mechanisation has resulted in a greater production of fine coal and dust, thus increasing the hazard. A better knowledge and appreciation of the medical aspects and dust explosion factors has led to practices reducing the seriousness of the problem. However, the dust hazard still exists and its control is basically an engineering problem.Dust, as with any atmospheric pollution, may be removed from the, mine by ventilation, which is an indirect method of control. Direct methods, in which the dust is prevented from being produced and from becoming airborne, should be employed where possible. Of these direct methods the most significant is the use of water sprays.THE DUST HAZARDAn appreciation of the physical nature of the more important dust hazards is necessary before considering the removal of the hazard. The main hazards associated with colliery dusts are the health hazard and the explosion hazard. "Dust" comprises solid particles of size minus 20 mesh Tyler (833 microns).

Two integral components of the 30 CFR 48 training requirements mandated for new miners, newly employed experienced miners, and the annual refresher training of all miners are hazard recognition and prevention of accidents. In these categories, instruction shall include the recognition and avoidance of hazards present in the mine, and a review of accidents and their causes. Unlike instruction preparation for fact-based material, program development for hazard recognition places a particular challenge before safety-training specialists because they are often limited in their ability to adequately gauge the trainees' levels awareness prior to training. Further, they must also assure that the training is relevant, and it is presented in a thought-provoking manner. Computer-based instruction is particularly well suited for assisting trainers in developing hazard- recognition topics because it can be used in pre-training and post-training modes for assessment of training effectiveness, in addition to use as a presentation tool. Therefore, this paper will present a recent approach developed at Penn State to apply this technology for assessment and instruction of accident prevention through hazard recognition, with an application prepared for surface coal mines.

The Cushion Blasting Technique applied to solve the high risk case history represented by a 10,704 TM rock locate at 120 M from the hydroelectrical plants of 350 KV each, is described, analyzed and discussed. The generation and distribution of 700 KV produced by two hydroelectrical plants must not be interrupted by any reason, because the 26 district of the province of cajatambo, of the Lima department; would not have electrical power. The search of the best alternative and methodology, the drilling and blasting pattern calculations, charge design and loading, connections and initiations of the 50 blasts are described, analyzed and discussed. Finally, the excellent results from a technical, economical, enviromental and safety point of view of the 50 primary blasts are discussed and emphasized.

Flame cutting and welding is one of the major causes of fires and explosions in underground U.S. coal mines. On May 20, 2006, a flame cutting operation at the Darby Mine No. 1 led to an explosion that resulted in five fatalities. On January 22, 2003, another explosion caused by flame cutting at the McElroy Mine resulted in three fatalities and three serious injuries. On June 22, 1999, a flame cutting and welding operation at the Loveridge Mine caused a fire that resulted in sealing of the mine for an extended period. Finally, on March 19, 1992, an explosion at the Blacksville No. 1 Mine that was caused by welding resulted in four fatalities. The National Institute for Occupational Safety and Health (NIOSH) conducted a study on fires and explosions in underground U.S. coal mines that were caused by flame cutting and welding operations to determine the root causes of these types of fires and explosions. The methodology included interviewing mining personnel who perform flame cutting or welding operations in underground U.S. coal mines and visiting mines to observe these operations, and MSHA reports of investigations and accidents statistics were analyzed. The results were used to identify and compare differences between flame cutting and welding practices and techniques in small and large mines, Eastern and Western mines, low seam and high seam mines, room and pillar and longwall mines, and between experienced miners and new miners. As a result of this study, best practices and interventions were developed to reduce the number of fires and explosions caused by flame cutting and welding. This paper provides a summary of this work. The Eurocode 7 "Geotechnical Design" is based on "Limit State Design", tackling the uncertainties as much as possible at their source through: - selection of characteristic values of variables (loads, soil properties, pile resistance,...); - partial factors applied on the characteristic values; - model factors to account explicitly for uncertainties of the calculation rule if necessary. Eurocode 7 will propose three "design approaches". The selection of one of them will be by National Determination. For pile design, the approaches are: - approach 1 is a "material factoring approach" at load side and a "resistance factoring approach" at resistance side. The structural and geotechnical design are checked for both of two separate sets of partial factors. - approach 2 is a "load and resistance factoring approach" and is in several aspects close to a deterministic approach. The design is checked for one set of partial factors. - approach 3 is a material factoring approach, at load as well as at resistance side. The design is checked for one set of factors. The aim of this paper is to introduce to the design of pile foundations based on pile load tests and on ground test results (semi-empirical and analytical methods) in the frame-work of the three design approaches. Detailed attention is devoted to: - the selection of the characteristic value of the pile resistance, accounting for spatial variability and stiffness of the structure; - the reliability of the prediction of the pile resistance using analytical or semi-empirical methods which may be accounted for through a "model factor". The results of a large test campaign on screw piles in OC Clay and a calculation example illustrate the proposed procedure when calculation rules using CPT results are used.

The U.S. Bureau of Mines investigated the relationship of bituminous coal seam parameters and the amount of airborne respirable dust generated at longwalls. Dust and coal samples were obtained from 20 longwalls operating in geographically representative coalfields throughout the United States. Statistical analyses of coal seam parameters and airborne respirable dust measurements indicate a likely causal relationship between seam type and respirable dust. Low-ash, high-volatile coal seams were associated with higher airborne respirable dust levels. A negative linear correlation (significant at the 95-pct confidence level) was observed between the seam's ash content and airborne respirable dust, explaining up to 18 pct (R2 = 0.18) of headgate dust level variation and up to 15 pct of tailgate dust level variation. Volatile matter was found to have a positive linear correlation with tailgate dust level, explaining 16 pct of the variation. Further data examination indicates that these relationships with dust are most likely nonlinear, because of improved R2 values over the linear correlations. However, a notable portion of longwall dust production is influenced by other operational parameters, so additional research under more controlled conditions is needed to determine the seam's causative functions.

Paper 1: Risk of gastrointestinal cancers from inhalation and ingestion of asbestos, John F. Gamble Selikoff suggested the hypothesis that asbestos exposure is causally associated with increased risk of gastrointestinal (GI) cancers in an early study of insulation workers. In this paper, John Gamble reports on his extensive review and reanalysis studies that address the relationship between asbestos exposure and GI cancers. The purpose of the review was to evaluate the association between asbestos exposure and risks of stomach, colorectal, colon, and rectal cancers separately. Gamble evaluated exposure?response (E?R) relationships using surrogate exposure methods ??because few individual studies of GI cancer have reported exposure? response analyses.?? The principal asbestos exposure surrogates are: (1) the rate ratio (RR) for lung cancer; and (2) the percent of mesothelioma among exposed workers. Analyzing SMRs for GI cancers from individual studies versus the risk of lung cancer and mesothelioma provides crude estimates of E?Rs for asbestos exposure and GI cancer. Asbestos fibers are highly fibrous silicate fibers that are distinguished by having a large aspect (length to diameter) ratio and are crystallized in an asbestiform habit that causes them to separate into very thin fibers or fibrils. These fibers are distinct from nonasbestiform cleavage fragments and may appear as thick, short fibers which break along cleavage planes without the high strength and flexibility of asbestiform fibers. Since cleavage fragments of respirable dimensions have generally proven nonpathogenic in animal studies, little data exists on assessing well-characterized preparations of cleavage fragments in in vitro models. The available studies show that cleavage fragments are less bioreactive and cytotoxic than asbestiform fibers. © 2007 Elsevier Inc. All rights reserved. An epithermal gold-silver deposit is presently being developed at the Golden Cross Mine in the Waitekauri Valley, 8 km northwest of Waihi, New Zealand. Highest grade mineralisation underground is mainly confined to an elongate zone of strongly banded quartz veining on the hangingwall. Footwall veining is irregular, flatter lying, and of lower grade. Hydrothermal breccias in the ore zone may have economic grades where clasts of vein quartz are present. Jigsaw breccias, shingle breccias, and layered breccias with sedimentary type structures such as layering, crossbedding, and graded bedding have been identified proximal to, and within, quartz veins. Mineralisation in the open pit occurs in and adjacent to stockwork quartz veining.   Three alteration zones are recognised: a central silicic zone enclosing Au-Ag bearing quartz veins, an argillic zone above the stockwork zone and in the footwall of the underground ore zone, and a propylitic zone which is gradational with argillic zone. Intense shearing is confined to the less competent argillic zone.   Precious metal mineralogy consists of electrum (65% Au; 35% Ag) and argentiferous tetrahedrite, polybasite, pyrargyrite, argentite, and native silver. Pyrite is the dominant sulphide mineral. Marcasite, arsenopyrite, and rare chalcopyrite and sphalerite also occur. Oxidation ranges from a few metres in the argillic western stockwork zone to 100 m in fractured rock in the footwall of the stockwork.   The open pit contains a proved and probable reserve of 2.1 million tonne grading 3.5 g/t Au and 17 g/t Ag. The underground mine will produce 2.7 million tonne grading 7.8g/t Au and 27g/t Ag.

The Reserve Mining Co., in the processing of taconite iron ore quarried from the Peter Mitchell Pit, deposited the finely crushed waste rock into Lake Superior near Silver Bay Minnesota. In the early 1970s The U.S. Environmental Protection agency filed a lawsuit against Reserve Mining Co., asking the court to force the company to stop dumping into the lake. On April 20, 1974, after a lengthy court battle, the U.S. District Court concluded that the Reserve Mining discharge into Lake Superior violated federal and state pollution laws and thus ordered Reserve Mining Co., to halt the lake disposal of taconite tailings. At the time many feared that some of the mineral particles contained in the tailings (alleged to be asbestos or asbestos-like) would cause human health problems. This Conference was thus convened to readdress the problem of the mineral content of the Biwabik Iron Formation and to whether any minerals contained in the ore could cause human disease. The four papers given in this session address the nature of the mineral particles in the taconite rock, the definition of the term ??asbestos?? and the history of the legal controversy over the possible health effects of certain mineral particles contained in the Biwabik iron range ores. The unaltered iron deposits, called taconite, contain approximately 30% iron and 50% SiO2. About 75% of the iron in taconite resides in the mineral magnetite, the remainder is largely in iron carbonate and several iron-silicate minerals.

A simulated rockburst experiment was conducted underground at a deep level gold mine. This was done by means of a large explosion detonated in solid rock close to a tunnel sidewall. The resultant shake-out damage is typical of that associated with a small seismic event in close proximity to a tunnel that is subjected to the estimated field stress, as in this case, of 50 MPa. The experiment involved: ?the design of a blast to mimic a seismic source ?seismic monitoring using a dense seismic array ?high speed video filming ?a study of rock mass conditions (fractures, joints, rock strength etc.) before and after the simulation using mapping and ground penetrating radar ?special investigations to evaluate the mechanism and the magnitude of the damage ?a study of support behaviour under excessive dynamic loading.

In the current tough mining environment with lower commodity prices, operators need to improve operational profitability. Optimised equipment asset management is a sustainable way to achieve this objective. The increased efficiency of installed process equipment positively contributes to the circuit performance, affecting metallurgical performance, plant availability and operating costs. Nui Phao Mining Company Limited (NPMC) operates a polymetallic mine located in Vietnam. The circuit produces four main products, tungsten, fluorspar, bismuth and copper. Tungsten is the major revenue producer. The operation experienced the following equipment inefficiencies in its copper and bulk sulfide flotation (BSF) circuits:• lower than expected metallurgical performance - the BSF rougher flotation tails contained coarse (+106 µm) sulfur minerals which affected the downstream tungsten gravity circuit. The unrecovered sulfur competed with tungsten minerals affecting both tungsten recovery and product grade. • structural integrity and safety - the flotation circuit structures experienced excessive vibration beyond the acceptable tolerance levels. The vibration was attributed to the flotation mechanism design and impeller tip speeds. • equipment reliability and wear life issues - the flotation mechanisms were being replaced every six months, with reported failures at three months. This resulted in high operational costs. A mineralogical evaluation of the ore indicated that the coarse sulfur minerals were well liberated and relatively rich in pyrrhotite and pyrite. It was predicted that these particles should be recoverable by flotation under optimum conditions. NPMC worked collaboratively with Outotec to investigate options to improve the metallurgical performance of both sulfur and tungsten. A review of the copper and BSF rougher circuit infrastructure was also undertaken and focused on reducing the structural vibration and improving mechanism wear life. Prior to the circuit upgrade, laboratory and pilot testing was conducted to validate the circuit improvements. The final solution included the installation of Outotec FloatForce® mechanisms in the existing copper and BSF rougher circuits, and a new flash circuit (SkimAir®) treating the rougher tails stream ahead of the tungsten gravity circuit. The flash circuit is unique, as typically these types of cells are installed in grinding circuits. By March 2016, all circuit upgrades were complete, however the initial metallurgical performance was below expectations. NPMC and Outotec worked together to investigate and improve performance through further equipment modifications. Since October 2016, the metallurgical performance has improved significantly. Coarse sulfur mineral recovery has improved, resulting in an overall increase in recovery of sulfur and tungsten of 4.7% and 2.7% respectively. The Outotec FloatForce mechanism has also delivered operational benefits, with a reduction in structural vibration to within acceptable levels (<4.0 mm/s) and an improvement in mechanism wear life by 150%. This paper discusses the BSF circuit upgrades and compares operational data before and after the upgrade to quantify the improvements in metallurgical performance, structural vibration and mechanism wear life. Failed attempts at amalgamation of a gold/osmiridium concentrate, recovered behind pebble mill liners during re-lining, led to the discovery that the gold particles were coated and that nitric acid only could remove the coatings. Following this treatment amalgamation was rapid and resulted in a complete recovery of the gold that otherwise would have been lost in plant residues. Investigations into the nature and formation of the coatings led to the realization that the coatings were impregnations of harder elements, associated with the gold during grinding and driven into the softer gold surface by high pressure, impact and attrition. It was further realized that this phenomenon could be part of gold ore milling, world-wide. With these thoughts in mind the question is asked whether concentration down to micron sizes prior to cyanidation, should not play a greater role in gold recovery.

Unfortunately from 1991 through 1995, 44 miners were fatally injured in the stone industry. Of these 12 occurred at underground mines from a total work force of less than 2,000 miners. Nine of these 12 miners were fatally injured in falls of roof or rib. A safer environment for the miners can be achieved by evaluating the nature of the hazardous ground and by developing more efficient and effective ground control strategies. Roof and rib conditions in the underground stone industry were observed and assessed at 33 mines in Illinois, Indiana, Kentucky, Maryland, Missouri, Pennsylvania, and West Virginia [(Figure 1)]. Hazard assessment indicated that those ground failures occurring under moderate to substantial overburden (> 30 m [l00 ft]) were caused by stress concentrations and geologic structures. Ground failures near the surface are caused by solution (water) processes. Selection of the mining horizon and mine layout decisions tremendously influence ground stability.

Downer’s mining division has made an operational and financial commitment to developing engineered solutions that mitigate the risks associated with operator proximity to uncontrolled development drill boom movements, and unguarded rotating drill steels.With the potential for serious injury or fatality, these identified risks are categorised as critical. Previous attempts by the industry to engineer them out have produced limited success, with numerous incidents still occurring year on year. Ultimately, the industry (as a whole) remains reliant on procedural prevention for operator safety.Downer understood the success of such an initiative would largely depend on the effective collaboration between operational experience, technology experts and automation. With so many variables requiring consideration; such as physical operator movement, machine boom movements, variable machine positions, and underground environmental impacts – challenging traditional technologies was necessary to deliver an optimal solution.For that reason, discrete detection (similar to laser curtains or beams) was considered not fit for purpose. The use of situational mapping, using rotating lasers, however, provided the optimal solution. The lasers collect data that is then processed using a custom software application.The software application is specifically programmed for tracking and classifying objects within detection zones, using advanced algorithms and filtering. Recognition and removal of non-positive disturbances are possible without losing personnel detection capability. The detection of relevant classified objects (humans) is determined by analysis of object size, age, movement, and speed etc. When the relevant object is detected, an alarm and inhibit function is applied preventing drilling functions.Any breach of the safety zone (forward of the front jacks) will automatically disable the hydraulic boom function. This can only be reset in the cabin once the person or persons have returned past the front jacks of the development drill into the safety zone.Downer, in collaboration with partner YUMARR Automation, has successfully developed this reliable boom isolation system, and is called JumboGuard.
