Most accidents are the result of risk taking. They can happen anywhere. Our coal industry categorizes accidents by type, occupation, commodity, age, every way you can imagine. We now know, how many were injured and when, for all the different types of accidents. We have ask ourselves many times, why are we having accidents, how can we stop the accidents and fatals in our industry? Present numbers and projections do not show that this will happen. Serious accidents can result in fatalities. Fatal accident can be prevented, and the coal industry can achieve zero fatalities. How soon this happens, depends on a great number of factors. I will discuss accidents and reasons for us having accidents in an attempt to get to the root causes. The historical perspective will be touched on, and the risk and analysis of accidents will be probed.

Cadmium ingot and nickel powder were prepared by high-temperature distillation, solvent extraction and liquid phase reducing method with spent Cd-Ni batteries as raw material. The impacts of the temperature in the distilling stage, the concentration of H2SO4 in the leaching stage, the pH value in the extracting stage and the dosage of hydrazine in the reducing stage were discussed respectively. The as- prepared samples were characterized by the means of X-ray diffraction (XRD), scanning electron microscopy (SEM), atomic absorption spectrometry (AAS) and inductively coupled plasma-atomic emission spectrometry (ICP-AES). The results showed that the recovering efficiencies of Cd and Ni reach 99.5% and 99.3%, and the purities of Cd ingot and nickel powder were higher than 99.90% and 99.80%, respectively.

Effective strata control is a prerequisite for maintaining an efficient mining operation. If the strata in the vicinity of an opening are not controlled effectively, it is impossible to perform a safe, efficient and economical operation. In this paper, the reasons and outcome of instability problems observed in roadways opened in clay and marl at the Bolu Colliery of Yeni Çeltek Establishment are investigated. Roadways under examination were located in a syncline causing high residual tectonic stresses. Factors affecting the instability have been studied thoroughly by examining geology and tectonics of the area, establishing convergence measuring stations to understand in situ behaviour of supports and laboratory tests to determine engineering properties of samples taken from nearby convergence measurement stations. As a conclusion, a new production strategy and a new support system are recommended to maintain stability.

In 1992, a paper was compiled highlighting a common-sense approach to electric smelting at Impala Platinum. Since then, the smelter complex has undergone considerable changes. Mineral economics, UG2 exploitation, and changing environmental legislation have enforced numerous process upgrades, yet a common-sense approach has always been adopted. This document details the major changes in the smelter complex at Mineral Processes (Minpro) over the past 15 years, and provides the rationale for the changes. The Mineral & Metallurgical Processing Division (MPD) of the Society for Mining, Metallurgy & Exploration held the 12th MPD Student Poster Contest on March 2, 2022, during the MINEXCHANGE 2022 SME Annual Conference & Expo. The event showcased the work of 32 graduate and undergraduate students from U.S. and international institutions. The contest began in January when the students submitted one‐page abstracts for judging. In February, they prepared and presented posters that explained their research, and each student had five minutes to explain their research to a panel of judges during the conference. Recognizing the students’ efforts in working hard on their research topics and setting aside time to complete the necessary steps to enter the poster contest, the MPD provides monetary rewards to all participants. For graduate students, first place receives $1,000, second place $500, and third through seventh places $200 each, and for undergraduate students, first place receives $1,000 and second place $500. The main authors of nonplacing entries receive $125 each. Participation in the contest includes a free ticket to the MPD Luncheon, where the winners are announced.

The strength characteristics of hydraulic sandfill modified by varying quantities of type III portland cement and two flocculants were investigated by the Bureau of Mines. Three different sand-cement ratios were examined using different percentages of flocculant. Two other experimental variables were curing time and weight-percent solids for hydraulic mixture. Results of unconfined compression tests with strain are discussed. Maximum strength was achieved with 0.5 weight-percent of ferric sulfate per ton of solids when the slurry mixture was 75 weight-percent solids. The bearing capacity of hydraulic sandfill is reduced by increasing the water content and decreasing the amount of cement mixed with sandfill.

Palabora Mining Company operates a low grade underground copper mine situated in the Limpopo Province, South Africa. Full production was first reached in May 2005 after a successful transition from an open pit operation to an underground block cave operation. The operation faces a number of technical and operational challenges, not the least being fragmentation of a competent rock mass and external dilution. The cave is subdivided into three main sectors being the east, the west and the central sectors. Caving was initiated from the weaker central sector, which has the majority of its drawpoints located directly under the open pit bottom currently filled with about 130 million tons of waste material from the pit wall failure incurred in 2004. About 48% of the ore reserve has to be extracted from the western sector, which has a much courser fragmentation compared to the other two sectors and only 25% from the central sector with the finest fragmentation. The main constraint during the ramp-up stage has been secondary breaking of drawpoint blockages. Ground-breaking secondary breaking and cave management initiatives and practices have made it possible for Palabora to realize and sustain high production rates.

Noise in general and drilling noise in particular have received increasing attention within the South African mining industry during the last 10?15 years. The Mine Health and Safety Committee1, during their 2003 annual summit, established two significant milestones to be met by December 2008 and December 2013. The 2008 milestone target is that of no more than ten per cent deterioration in hearing among occupationally exposed individuals, whereas the 2013 milestone target is that of less than 110 dB sound pressure levels at any place in the workplace. While these targets relate to general noise within the mining industry, it is widely believed that the most significant source of noise contributing to noise induced hearing loss (NIHL) among the underground workforce, is that of pneumatic rock drills. Data pertaining to the noise emission level of any machine is generally reported as either a sound pressure level or a sound power level of a single machine measured under free field conditions as defined in Appendix 3 of SABS-ISO 37442. However, rock drills are seldom used in isolation and underground conditions cannot be considered as free field. A methodology for the selection of rock drills on the basis of total operating cost has been developed. However, this paper focuses on the noise elements of the generic methodology and provides a model for the prediction of the composite sound pressure level and the equivalent noise exposure levels of groups of individuals from machines, used singly and in combination in an underground environment based on the sound pressure level determined according to SABS-ISO 3744 under free field conditions. The methodology includes provision for the consideration of the effect of hearing protection devices and the use of multiple machines in combination and provides an indication of the potential costs of hearing compensation.

"This paper revisits data from previous studies (Hernandez-Aguilar, 2010, 2011) to better understand the link between metallurgical performance and pulp-zone bubble size (Db) in full-scale flotation systems. To obtain conclusive evidence exclusively related to the pulp (collection) zone, the experiments were designed in those studies to control both the bubble generation method and the influence of the froth zone on the overall metallurgical performance in industrial and pilot-scale flotation columns. The results showed that Db had a significantly larger effect on the recovery of coarse (> 75 µm) particles compared to the effect on fine (< 16 µm) particles. It was observed that, under certain practical conditions, the recovery of coarse particles was close to zero, but dramatically increased by reducing Db. The evidence revealed an inconsistency of the so-called “k–PSb” model: at constant froth recovery, increasing Sb (i.e., the rate of production of bubble surface area) did not always result in higher recovery, as the model predicts. Furthermore, the evidence showed that it was not the Sb but the bubble size that proved to be a more reliable link to metallurgical performance. Only in the presence of suboptimal-sized bubbles (Db > 2 mm) did the froth zone seem to play a role, possibly reflecting an effect of pulp-zone bubble size on froth stability. However, the role of the froth was negligible for Db < 1 mm.High-resolution mineralogical measurements performed on process samples revealed that, regardless of the degree of liberation, Db had a profound effect on the shape of the recovery-by-size curve, particularly the coarse particle size fraction. This observation is key as it implies that the loss in efficiency associated with floating the coarse particles was not the result of neither particle hydrophobicity nor particle detachment issues: even in the absence of mechanical agitation, liberated and highly hydrophobic coarse particles did not seem to attach to the bubbles in the first place, an inefficiency that was corrected by reducing bubble size. The data analysis suggests that poor attachment efficiency can practically affect both fine and coarse particles but via different mechanisms which are discussed here."

The ongoing Overpeck Valley Parallel Sewer Project of Bergen County Utilities Authority (BCUA), New Jersey, is a US $65 million CSO improvement program to meet a State DEP consent order by 2010. One major program component includes a total of 1.8 km (1.2 mile) long, 1,800 mm (72 in) diameter reinforced concrete pipe (RCP) installed by a Herrenknecht AVND1800AB MTBM. The alignment includes drives through 850 m (2,788 ft) of extremely soft glaciolacustrine varved clay, 980 m (3,214 ft) of loose to medium dense mixed soils containing silty sands, gravels, and boulders, and crosses under the New Jersey Turnpike, US Highway Route 46, and a CSX Railroad Intermodal Yard and a branch mainline. Two types of MTBM cutterheads are being used, one configured for the weak varved clay, and one for the mixed soil conditions. The MTBM is also equipped with a double-steering system consisting of a standard front steering joint and a second trailing steering joint to assist grade control should adequate steering not be achieved by the front primary steering in extremely weak soil conditions. This paper discusses the significant engineering aspects and challenges related to site settings, ground characterization, key MTBM features and performance, and required ground treatment.

When designing excavation support systems in urban environments, it is often necessary to consider the effect of vertical surcharge loads from adjacent structures. Several methods are used in practice to establish the horizontal surcharge-induce pressure on the excavation support. According to the experience of the authors, these methods often seem to overestimate the effect of the surcharge and may have significant impact on the construction costs. This paper explores several of these procedures and provides a discussion as to the reasons for the differences in surcharge effect predictions between different procedures. It also provides practical recommendations for designers dealing with excavation support design.

The National Institute for Occupational Safety and Health (NIOSH), in partnership with the Mine Safety and Health Administration (MSHA), is conducting research to test, evaluate, improve or modify coal mine fire-fighting strategies and methodologies through large-scale tests. Since wood is the second most abundant fuel available during a coal mine fire, it was decided that series of large-scale wood crib fire tests were needed to measure the products of combustion and to observe the capability of commonly available direct application fire suppression equipment, namely, fire extinguishers, water and gas-enhanced foam. This paper discusses the wood crib fire tests, provides insight into the products of combustion and describes observations made during the application of the fire suppression agents.

According to the 2010–2019 Mine Safety and Health Administration (MSHA) accident report database, 91% of reported ground control accidents in US longwall mines were caused by roof instability. Gateroads are subjected to significant changes in loading conditions from the development to the longwall abutment loading phases. When combined with thinly bedded shale roof, found in many US longwall coal mines, the design of efficient roof support becomes challenging. In previous work, the bonded block modeling (BBM) of roof by UDEC was validated against field extensometer measurements in a longwall entry roof at a 180-m depth of cover. The BBM was shown capable of capturing delamination and buckling of shale roof, one of the main roof instability mechanisms in longwall mines. This paper presents the recent findings on the roof-support interaction using BBM models of the same longwall entry. The effects of cable bolts, roof bolt density, and strap support on potential roof instability are studied. Results demonstrate the potential for BBM numerical models to help understand the complex roof and support system interactions and to assist with optimizing gateroad support systems.

Procedures for detecting and locating seismic sources are adapted to the case of entrapped miners. During an emergency the entrapped miner is likely to generate seismic signals, by knocking against the walls of the shaft. With a seismic array station deployed near the disaster area, the seismic signals can be recorded. Two algorithms are presented for the location of the source from the arrival times of a seismic phase: The first algorithm produces one, most probable location, under the assumption of constant velocity of propagation of seismic waves in the medium. The second algorithm is similarly applicable, however without any restriction on the seismic velocities in the medium between the source and the array. While the first algorithm uses an analytic approach, the second one is based on a trial and error procedure. The second algorithm gives locations with arbitrarily high accuracies, depending only on the accuracy of arrival time measurements and on the compatibility of the true velocity distribution and the assumed velocity model. The algorithm is shown not to require under certain conditions direct .arrival time measurements, thus eliminating a potential source of errors in the location of entrapped miner.

Outotec has performed numerous research and development projects and various surveys in the study of reverse iron ore flotation. Over the past four years their TankCell-70 and TankCell-160 flotation cells at the Germano concentrator of Samarco in Brazil were extensively surveyed. These surveys have revealed some extreme operating conditions in reverse iron ore flotation and are the subject of this paper. Some of the findings include large variations in the content of solids in the various flotation stages. At the bottom of the cleaner cells, the content of solids by weight reached over 60 per cent, whereas at the bottom of the scavenger cell this was less than 50 per cent. Froth carry rates were also significantly different in the various flotation stages. In the case of coarse material, the scavenger cells operated at a froth carry rate of over 40 t/h.m2. With fine material, however, froth carry rates of less than 10 t/h.m2 were measured. High froth carry rates caused significant changes in the residence times at Samarco. In the rougher cells, the fifth cell had more than double the residence time of the first cell. This was even more pronounced with the scavenger cells, where the second cell had more than triple the residence time of the first cell. High froth carry rates also caused losses in iron recovery. As a consequence of such findings, it appears that different rules apply in the sizing of reverse flotation circuits compared to direct flotation circuits. Different operating parameters (mixing intensities and air flow rates) must also be used in the rougher, cleaner and scavenger cells in order to optimise the metallurgical performance. Sheryl Sandberg, the COO of Facebook, published a book titled 'Lean In', where she invites women to sit at the table. In summary, to 'sit at the table' entails not only embracing opportunity, but to actively seek out opportunities in leadership positions. Sandberg points out that, unfortunately, apart from institutional obstacles, a woman's own perceived limitations have a negative impact on her leadership ambitions. To create an environment where diversity will flourish, where everyone has a seat at the table and everyone has a voice, safety must be established first. To establish safety within a diverse team, there is power in relationships and connections. This study will first and foremost review theories in literature as to why diversity and inclusion are so important and the value that diversity brings to our industry. The study also recommends a few focus areas for improving diversity and inclusion in the industry.

Reagents are added to the flotation process of Merensky Reef ores to achieve specific functions that enable the separation of valuable minerals from gangue minerals on the basis of the difference in their surface properties. However, due to the complexity of the process, the various interactions that occur as well as the incomplete liberation of the ore and range of minerals present, it is difficult to quantify the behaviour of each of the reagents. More specifically, changes made to achieve a particular outcome may have secondary effects that can override the desired effect. If all the effects and interactions can be accounted for, it is possible to manipulate reagent suites and obtain better overall metallurgical performance. This paper discusses the effect of the addition of copper sulphate at different operating points of the recovery of the sulphide minerals. Batch flotation tests indicated that the recovery of chalcopyrite was not affected when operating conditions were varied, whereas pentlandite recovery was strongly affected by the point of reagent addition, and pyrrhotite recovery was enhanced by copper sulphate addition, irrespective of the point of addition, with the extent being dependent on the ore source. Floatable gangue was activated by copper sulphate in the presence of the dithiophosphate collector. Froth stability was also affected by copper sulphate addition.

The 1½-day workshop was held on October 15-16, 2015 at the Oakland Marriott City Center in Oakland, CA.Post-grouting, in the context of this workshop and report, refers to the alternative design and construction of base-grouted drilled shafts wherein a grout distribution system is installed during construction and used to grout the base under pressure after the drilled shaft concrete has gained adequate strength, resulting in a stiffer axial-compression load-displacement relationship and additional possible ground improvement effects.The primary goals of the workshop were to•?Identify and summarize the state of the practice for design, construction, inspection, verification, acceptance criteria, reliability and performance of post-grouting.•?Identify gaps and needs in quality assurance that should be addressed for successful use of post-grouting.This report is structured to follow the workshop program: workshop presentations outlined the state of practice and views from owners, engineers and contractors on acceptance of post grouting methods for drilled shafts; and subsequent workshop discussions attended by a small invited group of participants focused on identifying areas of agreement and disagreement on post-grouting best practices, acceptance criteria, design and verification procedures, and inspection requirements.
