Pound Road Quarry is an alluvial gravel quarry owned and operated by Fulton Hogan Ltd. for the supply of aggregate in Canterbury, and is the largest quarry in the South Island in terms of production. It has evolved from three separate quarries, as a result of which extraction has occurred sporadically in a number of areas. Due to this factor, and the need to manage environmental impacts, a detailed quarry management plan was completed in 1995 which formed the basis for implementation of a certified environmental management system.   In December 1997, the quarry was the first in New Zealand to achieve ISO 14001 (TELARC) accreditation for implementation of an environmental management system. The main potential environmental impacts arising from the quarry operation include dust, noise and groundwater contamination, and visual landscape effects associated with gravel extraction, the mitigation of which is achieved by means of æbest practiceÆ methods. All quarry operations are controlled in terms of the environmental management system by means of regular (monthly and yearly) environmental reviews, appropriate auditing of environmental documents, and monitoring of environmental targets in accordance with consent and statutory requirements.

This paper describes the unusual hazardous conditions encountered in mining thick, inclined coal beds under heavy cover in Carbon County, Utah. This description includes heavy roof conditions, sloughing ribs, steep pitches, and severe bouncing experience. Emphasis is placed on the top bench system of mining and development in the top half of a 16-ft seam, under the roof rock, which is bolted, and the recovery of the bottom coal on retreat; success of bolted roof and ribs; and shuttle car haulage on pitches up to 25%. The use of protective cages for operators of face equipment, the guarding of belt conveyor systems to prevent injury associated with deliberate entry, and the positive lockout of electrical equipment is described and illustrated. Most coal mining areas of the Western United States are characterized either by thick beds, steep pitches or heavy cover. Individually, each of these may present inherent safety hazards that in- fluence mining. When all are met at one location, the combined effect is to make the safe operation of the mining property a challenge to management. These combined conditions are met in the coal mines of the Columbia-Geneva Steel Div. of the United States Steel Corp., in southeastern Utah. This coal bed measures up to 16 ft in thickness, the pitch is as much as 25% and mining is under a cover approaching 3000 ft. The Sunnyside coal bed is found in the Book Cliff Mountains, a great escarpment extending from west of Castle Gate, Utah, some 180 miles to Grand Junction, Colo. West of Green River, Utah, the Book Cliffs swing sharply to the north because of the domelike uplift of the San Rafael Swell to the southwest. It is in this northerly extension of the Book Cliffs that the Sunnyside coal in the Blackhawk Formation of the Mesa Verde Group, Upper Cretaceous Age is found. The strata are transected by numerous faults with from a few inches to as much as a hundred feet of displacement. All of these faults have had some effect on the mining operation, either in roof control or, in the case of major faulting, the pattern of mine development. Estimation techniques such as polygonal, triangular prism, trapezoid, isopach maps, and inverse distance methods are often used in ore or industrial minerals deposit evaluation. These techniques do not express the variability of the deposit and do not allow a determination of the reliability of the estimates. However, geostatistical methods can express a measure of the error associated with the estimates, by finding weighting coefficients for a given mining block, and can also help with data configuration that minimizes the error. This work addresses an application study on the quality and reserve characteristics of the cement raw materials of the Adana Cement Factory in Adana, Turkey, based on the spatial distribution and variability of the chemical components (Si02, CaO, A1203, Fe203). The study has been carried out using a geostatistical procedure that is useful for site assessment, characterization, and monitoring situations where data are collected spatially. Directional and omnidirectional experimental variograms of the cement raw material variables showed that neither strong geometric nor severe zonal anisotropy exists in the data. The most evident spatial dependence structure expressing the continuity for omnidirectional experimental variograms were characterized by exponential and spherical variogram models. These models have been used in cross¬-validation analysis, which proved that these models, their parameters, and kriging parameters are applicable for the study area. Quality contour maps of the deposits at given levels underground were estimated using a kriging interpolation technique. Anomalies such as bullseyes and drift were not observed in the maps that were generated. Kriged maps showed the spatial distribution of quality continuity and variability of the deposits. Grade-tonnage curves and total tonnage estimates in the particular grade were determined using ordinary kriging in order to improve the mining operation and planning. Consequently, local uncertainty and the probability of extreme values occurring are tools of prime importance for the mine planning, the optimum mix of raw materials coming from different quarry stopes.

The extension of Montparnasse Station in Paris required the construction of supports of very high unit capacity to support a particularly rigid super-structure. The majority of the site is characterized by the existence of old limestone quarries at a depth of approximately 20 metres. These workings have been backfilled in some places, have collapsed in some places and have given rise to local subsidences in the marne layers above them. This paper addresses the steps taken by the group of companies and their consultants to evaluate the equivalent stiffnesses of the supports necessary for the design of the superstructure, and gives details of the construction techniques chosen by the foundations specialist to adapt to the particular conditions encountered on this site.

"Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

The number of persons fatally injured in the California petroleum industry, in 1932 was 24, a decrease of 6, or 20 percent from the number killed in 1931. The decrease in the number of fatalities occurring in 1932 caused the fatality curve of the California petroleum industry to reach its lowest point in 10 years. Changing conditions in the oil industry--reduced personnel, decreased active field development, and curtailed production--have undoubtedly caused a part of this decrease, but continued accident-prevention efforts by the industry have also contributed to the reduction in the number of fatalities for 1932. The Bureau of Mines has analyzed and reported annually the fatalities in the California petroleum industry for a period of 10 years. Figure 1 shows the trend of fatal accidents during that period.

Tin deposits in Catron County, New Mexico, were explored by the Bureau of Mines under authority of the Strategic Minerals Act of August 1939. The investigation covered both lode and placer deposits and was later extended to deposits in Sierra County. Six of the known deposits were examined by E. D. Gardner, supervising engineer of the Southwest Experiment Station of the Bureau of Mines, and S. G. Lasky, a geologist of the Federal Geological Survey, in June and July 1939. It was recommended that the more promising occurrences be explored by test pitting, trenching, and sampling. Field work, under supervision of M. E. Volin, began in September and continued through December 1939. The area was mapped by Carl Fries, Jr., of the Federal Geological Survey, in 1939 and 1940. The La Cruz Development Co. acquired control of the Squaw Creek placer claims in 1942 and began active sampling and development preliminary to large-scale operations. The result of this work showed a considerably higher tin content than that found by the Bureau of Mines in 1939. Because of this report and reports from other parts of the district that large-scale operations for the production of tin from lode deposits were possible, it was recommended that the Bureau of Mines resample parts of Squaw Creek and conduct a mill test of lode material to determine whether a concentrate could be made that was high enough in tin and low enough in iron to be acceptable in American smelting practice. This work, under the supervision of Paul L. Russell, was begun in November 1942 and completed in January 1943, during which time the Sierra Metals Co. acquired control of a large area in Sawmill Canyon. At the suggestion of the Federal Geological Survey and the War Production Board, which had reason to believe that a great deal of commercial material existed on certain flatlands and gully slopes, the project was extended to include test pitting and sampling of those areas. Standard petrographic profiles for many Permian seams from New South Wales and Queensland have been drawn up. The standard profiles of seams of the same age and from one or more coalfields were used to compile an idealized or type seam sequence for those coals in which more than half the seam profiles were similar to one another. The mineralogical composition and morphology of solidified copper smelting and converting slags are of some interest from slag cleaning (by froth flotation), impurity elimination, and environmental points of view. Properties result from both the state of molten slag before solidification and processes taking place during solidification and cooling. In solidification and cooling, copper and sulfur dissolved in molten slag are .precipitating substantially from the silicate phase. Even in water quenching of slag, dissolved copper is pushed out of the solution in the form of finely divided precipitate. Elements present in the raw materials are distributed in a distinct manner between the solidified slag and non-silicate solidification products. Arsenic, antimony, and tin are strongly accumulated in copper precipitate, but not the other metallic constituents common in copper concentrates. Smelting and converting slags differ remarkably on their mineralogical composition and morphology although resulting principally of same chemical processes. Converter slags are more inhomogenous; they often contain solid particles and copper content is .as a rule higher than in matte smelting slags. The final copper content of converter slags was observed to be in a distinct manner dependent on iron oxide - silica ratio. Slag forming, solidification, and precipitation phenomena are discussed on the basis of mineralogical and morphology analysis of smelter slags and laboratory scale experiments.

The effects of hydrogen on the properties of binary Ti-A1 alloys containing 2.5, 5, and 7 pct aluminum and ternary alloys of the same compositions with added interstitial elements oxygen, nihrogen, and carbon were studied. Aluminum increases the apparent hydrogen solubility of the a-phase, resulting in a marked increase in resistance to impact embrittlement. In the Ti-5Al and Ti-7Al alloys, for example, the addition of 180 and 305 ppm hydrogen, respectively, did not cause embrittlement, while unalloyed titanium was embrittled by as little as 75 ppm. The role of aluminum in increasing hydrogen solubility of the a-phase probably accounts for the excellent hydrogen tolerance of a-ß alloys containing considerable amounts of aluminum. There is some evidence that aluminum retards the rate of hydride precipitation. If so, these alloys may be susceptible to slow-strain embrittlement. UNALLOYED a-titanium is extremely sensitive to small amounts of added hydrogen in that there is a marked decrease in notch properties. For example, as little as 100 ppm hydrogen is sufficient to lower the room-temperature notch-impact strength from over 100 to less than 20 in.-lb.' Waitemata Tectonic Event(25 Ma ago) created a subduction system from the northeast of Northland, causing: (1) the emplacement of the Northland Allochthon (24-23 Ma) which provided argillaceous limestone and serpentine to Northland, and minor copper to both there and East Cape (located off Northland then); and (2) Northland volcanism (from 22 Ma), which lacked economic mineralisation. The Kiwitahi Tectonic Event (1714 Ma) caused: (1) a realignment of volcanism - from coastal western Northland, to west of the Hauraki Plains (Kiwitahi); (2) a start to the migration southwards from Northland of (a) the Alpine Fault System towards Marlborough today, (b) subduction towards Cook Strait, and (c) volcanism towards Taranaki-to-Bay of Plenty (initially through Coromandel, Kiwitahi, and offshore west coast); and (3) a start to rhyolitic volcanism & associated hydrothermal activity. The latter caused epithermal mineralisation, providing in Coromandel the best known North Island metalliferous deposits. Andesitic volcanism has been active in both of these two contrasting, subequal belts since 22 Ma. Rhyolitic volcanism and hydrothermal activity was wholly restricted to the eastern Marshall Belt, with an abrupt limit to economic mineralisation at the Volcanic Divide (within Hauraki Plains) and a major change there to basaltic volcanism of the western Searle Belt. The Kaimai Tectonic Event (5-1.5 Ma) caused: (1) a temporary reversal in volcanic migration in the Searle Belt, and a duplication of rhyolitic & hydrothermal activity at Waihi (Marshall Belt); (2) the rotational rifting of both the Central Volcanic Region (by 30¦) and the Hauraki Rift (10¦), causing major curvature of the North Island and New Zealand, and the transfer of the offshore subduction system from Coromandel to East Coast; and (3) an effective end to Coromandel mineralisation. Post-Kaimai Volcanism (1.5-0 Ma) included: (1) andesitic volcanism in the Searle Belt, yielding the western coastal titanomagnetite deposits; and (2) andesitic, and rhyolitic & hydrothermal activity in the Marshall Belt - the latter providing geothermal energy, sulphur, and pumice/perlite deposits. Post-Subduction (Intraplate) Volcanism (10-0 Ma) developed in a steadily expanding northwest region of relative tectonic quiescence, as subduction and its volcanism migrated southwards. Intraplate volcanism was predominantly basaltic, providing excellent aggregate; but less common rhyolitic & hydrothermal activity provided geothermal potential and mercury at Ngawha and at Puhipuhi, and halloysitic clays of extreme brightness elsewhere in Northland. This volcanism has reached as far SE as Mayor Island, providing an early New Zealand mineral resource - obsidian.

Paper presented at the conference: Risk assessment in the extractive industries, held in Exeter, UK, 23-24 March 1994, and first published London: IMM, 1994, preprint volume, 10pp., 11 refs. No completely reliable method for characterising and quantifying airborne dust risk exists. A new monitoring technology has been developed whereby dust collected from four-way directional and high volume samplers is analysed for mineral content and particle size for integrated dust mapping. A dust database was set up for the area surrounding the Ffos Las open cast coal site in South Wales. It included XRD directional traces from 17 gauges over 20 months, XRD traces from window ledges in the nearby communities, the major soil types in the area, dust data from haulage roads and public roads and reference-type mineral samples. The database can be accessed relationally to compare and analyse the data using mineral, direction, time, weather and geographical relationships. Expansion of the global piling industry has been coupled with significant research efforts investigating the mechanisms controlling pile capacity. Static pile design has transitioned from total stress approaches toward effective stress design, with associated improvements in predicting pile performance. As a result, the available literature is heavily skewed toward static capacity prediction. However, when considering piles offshore, the majority of loading is not static but highly variable and cyclic in nature. This paper outlines the additional factors that require consideration for cyclic pile design, including elevated loading rates, cyclic displacements and capacity degradation. A review of the literature identified a number of cyclic pile testing programs which are collated in this paper. General trends are established through comparison of the tests within the database and the conditions where cyclic loading is most likely to be a concern are identified. In particular, loading conditions which involve full plastic shear reversals are seen to cause the most extreme cyclic damage, with the degraded capacity as low as 31% of the precyclic static capacity. The underlying mechanisms which can result in cyclic damage are explored, with particular attention given to the pore pressure response, radial stresses, shear transfer and particle reorientation.

This study was conducted by the Bureau of Mines to determine the effect of steel reheating practices in burning natural gas on the amount of silicon required to prevent the formation of copper on steels containing up to 1 wt-pct Cu. Formation of copper, which causes surface hot shortness, was readily prevented by as little as 0.3 wt-pct Si and by 2 vol-pct excess oxygen in the furnace atmosphere when reheating steels containing up to 0.8 wt-pct Cu. Up to 0.8 wt-pct Si content and careful control of reheating conditions were necessary for steels containing 1 wt-pct Cu. Best conditions for these steels were reheating to 2,200° F (1,205° C) in burning gases containing 2 vol-pct excess oxygen, and then to higher temperatures in gases of 6 vol-pct excess oxygen. Slag and mold wash were not detrimental on steels reheated for 8 hr by a soaking-pit-type furnace schedule, but they were problematical on steels heated for 2 hr by a pusher-type furnace schedule. The effect of silicon contents on scaling rates also was determined.

"At Teck-Corona Operating Corporation's David Bell Mine, a water quality management strategy is employed to produce both a chemically and biologically clean effluent prior to discharge to the environment.The strategy includes operating methods to minimize contaminants leaving the concentrator and an effluent treatment plant for cyanide, antimony, and heavy metal removal of concentrator, mine and site waste streams.Concentrator practices, the effluent treatment plant, process chemistry and the present day performance of the water quality management strategy are reviewed.INTRODUCTIONThe David Bell Mine is located in the Hernia gold camp 35 km east of Marathon, Ontario. The concentrator has a design throughput of 1,000 tonnes per day. ROM ore (-6"") is ground in a semi-autogenous mill followed by a ball mill, thickener, leaching, and carbon-in-pulp gold recovery unit operations. The concentrator flow sheet is shown in Figure 1."

A literature review on quantitative analysis of mine safety studies revealed that investigators use a wide range of statistical techniques for the analysis of mine injury data. Researchers have addressed the various issues of accident causation and prevention through analysis of mine injury data collected from mines. These studies are generally based on historical accident/injury data without considering the injured person/population at risk. In this paper, a public health approach, based on scientific methods of epidemiology, is presented to address mine safety issues. Epidemiology is the scientific study of disease and injury patterns among populations in time and space. Originally, epidemiology was concerned with epidemics of communicable diseases. Then it was extended to non-communicable diseases. Safety researchers found that epidemiologic methods are the best tools for injury research, because the occurrences of injury and the occurrences of disease are quite similar. As a result, some researchers started applying this concept to injury research. This paper presents some of the techniques used in epidemiologic studies and illustrates the application of a case-control study design to evaluate the role of some risk factors associated with injuries in underground coal mines. In this study, the injured persons were identified and, simultaneously, interviews were conducted to find the significant risk factors responsible for occupational injuries in coal mines.

"In January 1985, Equity Silver Mines Limited began operation of a cyanidation circuit to scavenge precious metals from the flotation tailings. At present, this process is treating 270,000 tonnes of tailings per month and is recovering in excess of 0.20 grams of gold per tonne. To obtain these results Equity is operating a carbon-in-leach circuit that utilizes pressure stripping, zinc precipitation acid washing and thermal reactivation. A cyanide destruction circuit is also required to minimize the deleterious effects on reclaim water in the flotation stage. This paper describes the existing scavenger plant, and details the start-up problems encountered and the modifications made to correct these problems.INTRODUCTIONEquity Silver Mines Limited , a 68% owned subsidiary of Placer Development Limited , is located 38 km southeast of Houston in north central British Columbia. The geographic location of Equity Silver Mines is shown in figure #1. In September 1980, production first began with the ore being supplied from the Southern Tail open pit. In March 1984, the Southern Tail deposit had reached its economic pit depth, and since then ore has been supplied by the Main Zone pit the gold mineralization in the Main Zone is less refractory than the Southern Tail and cyanidation of the flotation tailing appeared to be a viable alternative.An extensive test program was undertaken and the results confirmed that considerable gold and silver values were amenable to cyanide leaching. A feasibility study showed that these values could be economically recovered and the detailed engineering work began."

A drill monitoring system has been developed which measures performance characteristics of a rotary drill and relates this to the material strength and rotary specific energy. The drill monitor is computer based and is used to produce a profile of the relative in-situ material strength as a function of depth. This information has been used in determining the placement of explosive in selective loading of blastholes and estimating the amount of lump product in a particular section of an ore body.
