Each year, companies spend millions of dollars on safety training and on safe equipment, money that is generally well spent. Despite that, we still make mistakes. What happens if the mistakes are made not because of inadequate training, but because we are hard-wired to act in a certain way? If you read any of the ?Because of Human Error? text boxes in this document, situations like these may look familiar in your own workplace. ?The issues of risk, health, safety and the environment are getting more attention now than ever before, at all levels of the organization,? said Calvin Price, vice president of Global Health and Safety at LNC-Lavalin Inc., a company with 21,000 workers worldwide. Most would agree with Price that attention to safety is increasing because responsible companies care about their workers and the workers? colleagues, families and communities. However, cynics might say that the safety spotlight is powered by the corporate bottom line. Direct costs created by workplace injuries total $51.8 billion annually and include workers? compensation payments, medical expenses, civil liability damages and litigation expenses. This sum represents a staggering one quarter of each dollar of pretax corporate profits. Indirect costs may run as much as 20 times the direct costs of on-the-job accidents and illnesses. Examples of indirect costs include training replacement employees, investigating the accident and implementing corrective measures, lost productivity, repairing damaged equipment and property, and costs associated with lower employee morale and absenteeism. What?s interesting is that up to 90 percent of incidents are due to human error, not faulty equipment or other factors.

The Peñasquito deposit in Mexico is one of the largest grassroots precious metal discoveries to be developed in recent years. Goldcorp Inc. acquired the gold-silver-lead-zinc property with the acquisition of Glamis in 2006. The company has since committed US$1.5 billion to construct the mine, mill and associated facilities. The first of two 50-kt/d (55,000-stpd) mill and flotation lines is currently being commissioned while engineering and construction continues on the second line. A high-pressure grinding roll (HPGR) will ultimately take total throughput to 130 kt/d (142,000 stpd). The Peñasquito property is located in the northeast corner of Zacatecas state, Mexico. The elevation of the property averages 1,900 m (6,200 ft) above sea level and it is situated in a wide east-west valley. Except for one small outcrop (Peñasquito), the area is covered by up to 30 m (98 ft) of alluvium.

Construction for the new 2.5 MGPD raw water intake project commenced December 2002. The project, initially designed as a 12' Ø concrete lined intake shaft,145' deep, with two offset 24" lake taps, 520' and 470' long was subsequently “value engineered” running the two lake taps through a single conduit. The offset for summer and winter pools was accomplished via a second shaft located at the lakes edge. Construction involved the use of blind drilling techniques for the shaft development and drill and blast techniques for the tunneling. All works were completed through extremely hard crystallized sandstone. The project came online August 12, 2004.

The Jollyville Transmission Main will be a 10.5-km- (6.5-mile)-long, 213-cm (84-in.) diameter rock tunnel. The tunnel will pass beneath the Balcones Canyonlands Preserve, which was established to protect two endangered bird species, six endangered cave invertebrates and a threatened salamander. To address the sensitive habitat that the transmission main is to pass through, the city of Austin developed an environmental commissioning (EC) process that brought together environmental engineers and scientists, the city?s project management team and the designer team to work collaboratively to meet the city?s goals of producing a cost-effective design while also protecting the environment. This paper describes the environmental commissioning process and the benefits it brought to the project and environment through some unique environmental mitigation measures to protect this sensitive area.

Since Reserve Mining Co. began commercial operations at Silver Bay, MN, some 25 years ago, major improvements in taconite processing technology have become available. In addition, significant new government standards have been imposed on industrial operations. By implementing a $370-million modernization program, Reserve was able to accomplish two major company objectives: the quality of concentrates produced from the Peter Mitchell mine has improved, and the operation meets both present and foreseeable environmental standards. Process modifications at Reserve were designed to: •Reduce the pellet silica content from 9% to 5%; •Increase the pellet iron content from 62.5% to 65.8%; •Implement a tailings disposal system utilizing "best available technology" water quality controls; and •Reduce air emissions by installing "best available technology" air emission controls. By adding dry cobbing, fine screening, and flotation to the concentrating process, pellet silica content has been reduced; the process reclaims and reuses all process waters; all tailings are stored in a zero discharge tailings basin; and all air emission standards are being met. Initial Beneficiation at Silver Bay Reserve's Peter Mitchell mine at Babbitt, MN, is on the eastern end of the Biwabik iron formation. The open-pit mine is approximately 16 km long, 0.8 km wide, and presently has a maximum depth of about 50 m. Principal constituents of the ore body are magnetite, quartz, and iron-magnesium silicates. The company's large commercial plant began operating at Silver Bay, 75 rail km from the mine, in 1955. Facilities have historically had the capacity to process 31.5 Mt/a of taconite, containing an average 24% magnetic iron, into 11 Mt/a of iron ore pellets. Reserve's initial beneficiation process-which existed up to early 1979-consisted of two crushing stages at the Babbitt mine site, two additional crushing and screening stages at Silver Bay, grinding in a conventional rod-ball¬regrind mill circuit, size classification with cyclones, and concentration with magnetic and hydraulic separators. Concentrate produced by that circuit assayed 65% iron and 8.5% silica, with a screen analysis of 94% -44 µm, and a surface area of 9300 cm2/cm3. Pellets produced from this concentrate assayed 62.5% iron and 9% silica (dry basis). Construction began in 1977 to modify Reserve's operation. Process modifications were designed from pilot scale operations that had been operated and researched by Reserve for more than 10 years. There were two design restraints imposed on the process: economic constraints that Reserve continue to use much of the existing processes and structures in the new operation; and a substantial number of government-imposed permit constraints. In effect, the tailings process was designed first and then a concentrating process was developed that would "fit" the tailings disposal system. Operation of the modified process began in June 1980. Crushing Remains Unchanged No changes in process flow were made at either the Babbitt mine and crushing plant or at the Silver Bay fine crusher. Gyratory crushers reduce run-of-mine taconite to -90 mm-diam at the mine. Taconite is then transported to Silver Bay by intra-plant railroad. There are 10 fine crushing lines. Each consists of a double deck screen, followed by a heavy duty short head crusher, a second double deck screen, and second short head crusher. All screen undersize material bypasses the crushing lines, resulting in an open circuit crushing process. Fine crusher product is approximately 98% - 20 mm, 80% -12 mm, and 50% -10 mm in diameter. Each fine crusher line processes about 400 t/h of crude taconite. Cobbing Provides Aggregate for Tailings Dams The first major process change was the addition of dry cobbing, a dry magnetic separation step between the fine crusher and concentrator. Dry cobbing serves two major purposes in the new process. Its principal function is to provide large volumes of dry coarse aggregate for use in construction of tailings dams. This process rejects about 20% of the fine crusher product as tailings with a size analysis of 98% -20 mm and 50% - 10 mm. These tailings are free draining and have high frictional shear strengths-two key requirements for dam building aggregates. Fine crusher product is conveyed to a new dry cobbing building. Here crushed ore is discharged into one of two 30-minute capacity surge bins. This research investigates roof deformation and failure characteristics, as well as mechanical interaction between the panels of the roof strata above two adjacent working faces in a steeply inclined coal seam. Physical modeling with photogrammetry measurement method has been employed in this investigation. The movement of roof strata and section pillar, as well as the stratum morphological characteristics, was quantitatively analyzed. Results show that the roof movement direction above the two panels has zoning characteristics, especially in the lower panel. Roof migration in the lower panel of the working face is larger than that in the upper panel. The roof to floor closure perpendicular to the seam direction is larger than the sliding along the seam. The maximum roof concave and convex bending occurred at the middle and upper parts of both panels; the roof bending in the upper panel mining area indicates a larger mining disturbance induced by lower panel mining. The void ratio contour of the lower panel is an asymmetrically arched shape, which is different from that of the upper panel area. The higher roof strata above the upper panel exhibit separation and breaking, whereas the lower roof exhibits compression due to mining in lower panel. The section pillar is compressed and slides slightly, which aggravates the mechanical interaction between the panels and increases the caving height of the roof over the lower panel and the periodic weighting intensity

Unmineable coal seams present an opportunity to sequester carbon dioxide emissions from power plants, while enhancing the recovery of coalbed methane. Injected carbon dioxide is adsorbed and stored on the coal surface while releasing methane that can be commercially recovered. Carbon sequestration, therefore, allows the utilization of unexploited mineral resources while reducing “greenhouse” gas emissions. This paper presents the feasibility, efficiency and enhanced recovery potential of coalbed methane production using carbon dioxide sequestration in the Central Appalachian Basin. The focus of research is on geological characterization of unmineable coal seams in Central Appalachia and the corresponding carbon dioxide storage capacity. Virginia Tech and Marshall Miller & Associates, Inc. conducted this research under the Department of Energy’s Southeast Regional Carbon Sequestration Partnership program.

Mineral liberation data are routinely obtained for a wide variety of applications, including ore characterization and characterization of various metallurgical test products obtained in bench-scale laboratory testwork. Newmont Mining Corporation owns several properties in the Carlin Trend containing refractory gold ores. These ores are generally difficult to treat and have to undergo several processing stages for gold recovery. At Carlin?s Mill 5 operation, flotation is used to pre-concentrate some of these difficult ores to produce a sulfide concentrate to feed the roaster. It is of interest from a flotation perspective to be able to study the floatability of pyrite, arsenopyrite and arsenian pyrite, since these minerals all have different quantities of gold associated with them, and have to be recovered to ensure a successful separation has been achieved. In this paper, a detailed MLA study on several Carlin ores was undertaken on samples from flotation tests, using air or nitrogen as flotation gas, specifically to study the differences in floatability characteristics, which ultimately impacts overall gold recovery. A special type of shotcrete is used to improve the performance of open-type TBM in difficult situations. The Hakamagoshi and Johana tunnels are expressway evacuation tunnels of 3.2 km and 5.1 km in length, respectively (total 9.1 km) that were excavated by one open-type TBM. The geological conditions at the sites were estimated to be good before excavation. However, 95% of the rock turned out to be two ranking orders worse than the estimated classes. Therefore, we used a special shotcrete for bad rock conditions, and succeeded in excavating the tunnel at an average rate of 270 m/month.

The Stress Control Method of mine design provides enhanced engineering control over the behavior of underground structures in comparison to conventional mine design methods. The same Method can be applied to achieve design control of surface subsidence. The basic mechanism relating undergroued deformation and surface subsidence is disclosed by mine simulations, using the REM (Rheological Element Method) computer code, whose predictions have been validated by field measurements. The key components of the underlying mechanism of surface subsidence have been shown to be the stress envelopes formed in the process of underground excavation. It has also been demonstrated that significant control of surface subsidence can be achieved by designing the geometry and timing of underground excavation to regulate the formation of stress envelopes. A field example is given in this paper to illustrate this application of the Stress Control Method.

Passive-treatment systems that mitigate acid-rock drainage (ARD) from coal mines have operating since the mid-1980s. Large systems at metal mines are being contemplated. A 95L/sec (1500-gpm) capacity system is planned this year at an underground lead mine in Missouri. Mine, process and environmental managers need to understand how and why passive-treatment systems work. They must understand what operational limitations to expect, as well as the economics of construction and operation compared to the alternatives, like lime precipitation. Wetlands have been recognized as nature's method of improving water quality. Contaminant reductions can occur through the precipitation of metal hydroxides, sulfides and carbonates and pH adjustments. Local conditions, oxidation state and water and soil chemistries dictate whether these natural reactions will occur under oxidizing (aerobic) or reducing (anaerobic) conditions.

The respect of EPB reference pressure during TBM advance and hyperbaric activities, the complete and effective filling of the annular gap between tunnel lining extrados and the excavation section and a well organized monitoring system, to control surface settlements and ground distortion, are items of utmost importance in EPB tunnelling process. The paper describes how these concepts have been applied while excavating in the difficult underground conditions found at Sao Paulo (Brazil) during the execution of the Sao Paulo Metro Line 4-Lot 1. In this project, the annular gap has been filled by two components type, cement grouting. The back feed of the extensive monitoring campaign carried out to check the surface settlements during the excavation and hyperbaric maintenance operations will also be described in the paper.

The National Institute for Occupational Safety and Health (NIOSH) identified hearing loss as the most common job-related disease in the United States in its 1996 National Occupational Research Agenda. Previous studies by NIOSH have shown operators of surface drill rigs without cabs are commonly exposed to A-weighted sound levels exceeding 90 dB. During hammer-drilling, A-weighted sound levels exceeding 100 dB were recorded at the operator station. NIOSH collaborated with a local drilling company to design and install a partial cab between the operator and the drill steel in an effort to reduce the sound level at the operator station. Sound level measurements in the field show the partial cab reduces the A-weighted sound level by 5-9 dB at the operator’s left ear and 2-5 dB at the operator’s right ear while hammer-drilling. This paper will briefly discuss the preliminary design of a cab and the testing of materials for construction of the prototype. Explanation of the ideas behind the prototype design and fabrication will be covered. Finally, the field test method and results will be discussed in detail.

THE PROBLEM Two and a half years have gone by since the December, 1976 Civil Engineering article on subway costs caused such a furore. What has happened in this interval? Even after discounting the crushing inflation rate, costs are still increasing, and there is no sign of a turndown, even in the rate of increase. Some steps outlined in the article have been taken. In April this year, the first D.O.T.-sponsored Peer Review Group, made up of managers of other transit properties, met in Boston to review MBTA's $500 million Red Line Extension-Northwest for cost effectiveness. While the impact of a review at the end of the design phase, with substantial segments under contract, cannot be as effective as a review early in the design phase, it is nevertheless a start. This first meeting will be followed by a review of Boston's Orange Line relocation, the final design for which is just underway. In the industry at large two facts are evident: 1. We are failing to reduce underground construction costs. 2. Underground construction is in danger of pricing itself out of the market place. It has always been a greater challenge to design a Chevvy than to design a Cadillac, but it is the Chevvy we need today. Much attention has been focussed in the past on engineering design in the narrow sense: areas such as ground loading conditions and concrete design. One of the most common critical areas of longwall mining in terms of ground stability are the gateroad and bleeder entries. These critical entries provide much-needed safe access for miners and allow for adequate ventilation required for dilution of hazardous airborne contaminants and must remain open during mining of a multi-panel district. This paper focuses on the stability of the longwall entries subjected to a single abutment load such as bleeders, first tailgate, and last headgate. First tailgate and last headgate are also referred to as blind headgates and tailgates. A study of a longwall district through conditions mapping, support evaluations, and numerical modeling was conducted and evaluated by researchers from the National Institute for Occupational Safety and Health (NIOSH). The condition mapping and support evaluations were performed on entries that spanned the previous five years of mining and relied on a diverse selection of supports to maintain the functionality of the entry. Numerical modeling was also conducted to evaluate various support types with further investigation and comparison to the condition mapping. The study demonstrated the importance of the abutment load decay versus distance from the gob edge, the potential for a reduction in material handling related injuries, as well as optimal usage of secondary and standing support.

Through 1996 and 1997, CANMET and the Cape Breton Development Corporation (CBDC) conducted a project to study the Phalen Mine’s 7 East longwall ventilation network to help determine methane emission patterns that were having a negative impact on coal production. Increased methane emissions were primarily related to mining depth and were also affected by the influence of a large sandstone body located above the worked seam. The research included the installation of tube bundle systems to monitor waste (gob) methane and pressure differentials. The focus of this paper is on the novel use of tracer gas techniques that helped shed light on the performance of the ventilation system and in determining the methane migration paths in the waste.

Recent accelerated exploration efforts in southwest China have progressively quantified the vast phosphate resources in the Yunnan Province. To date, according to Chinese geologists, potential reserves in the province may be on the order of 4 Gt, which would equal the entire US reserve base. Development of this vast reserve is underway at two sites located 60-70 km southwest of Kunming, the provincial capital of Yunnan. The first, the Kunyang open-pit phosphate mine, produces an estimated 1.5 Mt/a of 22-30% P20,5. The other project located north of Kunyang is the Haikou mine, which the Chinese consider a possible 2.0-Mt/a operation. Because average ore grades run 3% less than those of Kunyang, beneficiation will be necessary. While the latter project has been initiated on a small scale, it is basically still on the drawing board. Brown and Root and the US Bureau of Mines are assisting on the project. Both operations are under the direction a regional administration which reports to the Ministry of Chemical Industries in Beijing. Combined production from both mines could represent some 70% of China's future productive capacity. Ore reserves in the region are estimated at 400 Mt by local officials. Geology of the District The phosphates are considered Cambrian in age and rest on a Precambrian dolomite. The Indian Ocean, Chinese geologists say, may have been the original source. The line of deposition stretches on a trend from Vietnam in the southwest, through Yunnan, into Sichuan Province to the northeast. Because the area is considered very stable from a geologic viewpoint, large quantities of phosphate have been deposited and large outcrops are known to occur everywhere in the province, according to Chinese sources. The Kunyang and Haikou ore bodies are thought to be two wings of an anticline with the former the southern portion, and the latter the northern portion. Each ore body strikes on an east-west axis. While there are many localized phosphate operations throughout the province, the Kunyang mine is the premium producer. Kunyang's Mining Development The Kunyang open-pit mine was opened in 1966. During the early years, operations were considered somewhat disorganized. Systematic mine planning and normal operations were not initiated until just two years ago, according to mine management. In 1979, Kunyang finally reached its design capacity of 1.5 Mt/a. The ore body consists of two phosphate beds separated by a band of shale. The top layer of phosphate varies from 4 to 8 m, averaging 6 m. The middle shale layer is 2 in. The third layer, which is not as rich as the top, is approximately 4 m. Up until ten years ago the industry had arrived at a relatively conventional, and consistent design of coke ovens. Through the years the designers and builders were not being heavily taxed to come up with revolutionary new designs. However, the stimulus for design change was ever present. These stimuli for change are quite important and they are the driving force which produce improvements. The most obvious stimulus to change a design is commercial. To "build a better mouse trap and the world will beat a path to your door" is the well used cliche which best befits this philosophy. As in all businesses, coke oven designs require that all of the considerations, which affect the cost or the desirability of the design, be optimized. One way to increase the desirability of the particular design, 1s to make the design in- expensive to operate. Or, put another way, to increase production for the same operating cost
