This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. The study is an economic evaluation of historical, current, and projected refractory uses in the iron and steel industry of the United States. The first part of the report presents a refractories classification and identifies principal trends in the refractories industry; such as, value and volume of shipments, prices of finished refractory products, the number of manufacturers and plants for refractories production and marketing. The main part of the report treats refractory use patterns and technologic developments affecting refractories usage in the iron and steel industry. Changes in use patterns in each furnace type, refractory, and installation costs and the future outlook for refractories in ironrnaking and steelmaking are covered. The last part of the report identifies the principal raw materials used in refractories production and treats trends in refractory raw material consumption. An appendix to the report contains historical time series graphs of shipments of refractory types used primarily by the iron and steel industry and principal refractory raw materials sold or used by producers. An equation of the line of best fit was calculated from the earliest to the latest year in each time series, usually 1950 to 1965; these were extrapolated in order to derive volume estimates for the year 1975.

This report describes the work performed by Mine Research Company in developing a prototype mobile roof shield during 1971-1972 for the U. S. Bureau of Mines, along with background information leading to the design decisions adopted for the project. In addition, the report evaluates the choice of design and analysis and the results of the project in terms of future possibilities of mobile roof support systems. The report begins with the discussion of the problems of designing, building, and using mechanical and hydraulic roof support in coal mines. It then gives a historical summary of past work with discussions of the design choices made for previous roof support systems.

Burning coal-mine refuse banks were surveyed in 15 States. The following characteristics were tabulated for 495 hanks: County; name, population, and distance of nearest town; size and status of bank; status and stage of fire; and topography. Ways of controlling fires were reviewed, the compacting and sealing method proving most satisfactory. Means for preventing refuse-bank fires by proper construction of banks are described.

Seepage forces computed by a finite-element flow program are combined with gravity forces in an elastic-plastic stress-analysis program to determine the stability of mine-waste slopes. Computer output provides a cross-sectional plot of the dam or embankment showing the safety factors for each element. The combined finite-element program called SEEP-SLOPE was used to analyze several trial embankments and the 1972 Buffalo Creek Dam failure Safety factors computed by the finite-element method (FEM) were compared with those computed by Bishop limiting-equilibrium method. For individual slip circles, there was little correlation between the factors of safety computed by the two methods. Both predict failure, but FEM shows a different mode of failure plus development of potential failure zones not indicated by conventional methods. Besides having a better theoretical basis than the Bishop method, FEM requires no assumptions regarding the location or geometry of the failure surface. As a result, the program will show progressive development of a failure zone, and areas can be pinpointed where remedial measures should be taken. In addition, nonhomogeneous cross sections can be analyzed, and zones of potential piping can be defined.

Reconnaissance studies of beach sands along the eastern part of the Gulf of Alaska were conducted by the Bureau of Mines to determine if any of these coastal areas warranted investigations as possible sources of valuable minerals. Because of difficult access and limited transportation facilities only a minimum of sampling, surveying, and camping equipment is utilized to obtain spot samples from key areas as well as information indicating the location, size, and character of the deposits. Such data were obtained from beaches located along approximately 247 miles of the Gulf of Alaska coastline during parts of the 1957 and 1958 field seasons. A total of 201 3-inch-diameter auger holes, spaced roughly at 1 mile intervals, were bored by hand methods to depths ranging from 3 feet to 27 feet; also 33 shovel samples were collected from selected localities. Samples were reduced to a rough concentrate in the field by panning; the rough concentrates were shipped to the Bureau of Mines laboratory at Juneau for further concentration and for petrographic and chemical analyses. Particular attention was given to the heavy-mineral content of the concentrates, although the types and relative amounts of rock-forming minerals were also determined. The reconnaissance indicated that, generally, the valuable mineral content of the beach deposits was too low to be of further interest except in the vicinity of Yakutat and Lituya Bays where erratic but possibly significant concentrations of magnetite and/or ilmenite were found. At Yakutat, about 20 miles of beachline extending southeasterly from Ocean Cape, yielded samplas containing from 1 pound to over 300 pounds of iron and from a trace to over 100 pounds of titanium dioxide (TiO2) per cubic yard of beach material. However, the higher grade samples represented local concentrations of minor importance. The general tenor of the Yakutat beach, as indicated by the airthmetical average of 27 auger-hole samples, is about 35 pounds of iron and 20.5 pounds of titanium dioxide per cubic yard of beach material. Ilmenite is the predominant heavy mineral of the beach deposits in the vicinity of Lituya Bay, but it was found in concentrations only in limited areas near the

Barge traffic on the Nation's inland waterways over the past 15 years has been increasing at a rate much faster than intercity freight traffic by all modes, largely due to the growing consumption of mineral commodities, development of more powerful towboats and large and more specialized barges, improvements to loading and unloading facilities, and competition among bargelines for contract shipment of bulk commodities. The Bureau of Mines, in a study to determine barge traffic projections for inland waterways in the South-Central States to the year 1980, gathered data on origin-destination patterns by commodity, quantity moved, equipment used, and rates charged; the trend of each of these factors over time; and the extent of competition between water trans-port and other means of transportation. The Mississippi River and the Gulf Intracoastal Waterway, which together carryover half the Nation's barge traffic, are major carriers of coal, crude petroleum, steel, marine shells, sulfur, salt, cement, and alumina. Greater quantities of these products will be transported by barge; however, oil pipe-lines and unit trains for coal are expected to limit increased barge traffic of these two commodities. Increasing quantities of fertilizer materials will be transported upstream. Although congestion at certain locks is a current problem, the Mississippi River below St. Louis and other open-river stretches have the capacity to handle the increasing number of barges for the foreseeable future.

Achieving substantial reductions in the exposure of underground miners to diesel particulate matter in a number of metal and nonmetal mines in the U.S. depends on the ability of the industry to widely implement advanced diesel emissions control technologies, primarily diesel particulate filter (DPF) systems. Recent field studies showed that diesel particulate filter systems with Cordierite and silicon carbide ceramic filter elements are capable of reducing concentrations of diesel particulate matter and elemental carbon by more than 70 and 90 percent, respectively. But those studies and several other attempts to implement DPF systems in underground mines revealed a number of relatively unique technical and operational challenges that are limiting industry-wide implementation of this technology. This paper provides detailed analysis of some of those challenges and short overviews of several projects launched by NIOSH Pittsburgh Research Laboratory in an attempt to provide the underground mining industry with more adequate DPF systems. These systems employ advanced technologies to overcome the most pronounced challenge, the regeneration of DPF elements installed on mining engines that generate relatively low exhaust temperature. This paper presents essential findings obtained through a long-term evaluation of DPF system with diesel fuel burner technology installed on a heavy-duty load-haul dump vehicle in an underground metal mine. In addition, the essential results of the laboratory evaluation of a popular on-highway DPF system which was adapted and optimized for underground mining applications will be summarized in this paper.

The objective of this study was to prioritize the potential risks nuisance and biologically active dusts and fumes present in metal and nonmetal mines. This was accomplished by ranking numerical values derived from a matrix formula calculation that took into account dose, mine population, and sampling size. Data used in these calculations were obtained from the Mine Inspection Data Analysis System (MIDAS). The dusts determined to have the highest risk potential were quartz respirable particulates, mine dust, cristobalite respirable particulates, welding fume components, and nuisance respirable dust. Other dusts and fumes that were not ranked in this study, due to small sampling population, but which show a potential risk, include asbestos, talc, hydrogen cyanide, organic compound dusts, arsenic, metal dusts, and metal and nonmetal fumes.

To use of heavy liquids has permitted determination of the possible grades of tailings and concentrates by specific-gravity methods on any particular size, and therefore the size to which the ore must be crushed to free the valuable material. The simplicity of float-and-sink separation and the high degree of efficiency attained in the laboratory are factors that commend the method for commercial concentrations.

Bahrain's oil revenue increased from $337 million2 in 1916 to $378 million in 1977. Despite the increase in value, petroleum receipts made up only 584 of all Government income in 1877, compared with 74% in 1976 and 83% in 1975. Bahrain became an oil producer in 1932 with the discovery of the Awali oilfield, and since that time, the Awali oilfield has produced over 600 million barrels of crude oil. Awali production in 1977 was 21.2 million barrels. Bahrain shares the revenue from the Abu Safah oilfield, located in the Persian Gulf north of Bahrain, equally with Saudi Arabia. Revenue from the Abu Safah oilfield was approximately $179 million or 47% of oil revenues in 1977. Output from the Abu Safah oilfield was 48 million barrels, an increase of nearly 30% over 1976 output. The Bahrain Petroleum Company (BAPCO). the nation's only oil-producing company, was owned 40% by California Texas Oil Corp. (CALTEX and 60% by the Govern¬ment. The Government established Bahrain National Oil Co. (BANOCO) in 1976 to administer the Government's share of BAPCO, oversee exploration concessions, and manage domestic marketing of petroleum products. The Government announced its intention to acquire 100% control of crude oil and natural gas production. CALTEX was to retain complete control of the 250,000-barrel-per-day oil refinery near Sitrah.

Problems resulting from underground mines in three inactive mining districts in Colorado were investigated. The purpose was to determine how these problems could be reduced or eliminated in new closings -- essentially by benefitting from lessons of the past. Mines in Leadville, Westcliffe and the Cripple Creek-Victor districts were evaluated. Two basic types of problems were found to exist in the districts: environmental degradation and hazardous conditions. Environmental problems result largely from acid and toxic drainage from the runes and from leaching of surface waste rock piles and mill tailings impoundment. The acid drainage contributes high levels of metals to streams and deposits sediment in the stream bottoms. Erosion of waste piles is a lesser problem. Hazardous conditions from open or improperly sealed mine shafts and adits, mine subsidence pits, and from deteriorating and unsafe surface structures are common to the three districts. These problems become worse with the ravages of time and vandalism. Engineering solutions which would correct or reduce many of these problems are presented. These recommendations should also be applied to new mine closures, however, more basic changes are needed to effectively implement improved mine closures. The changes must take place in the areas of legislation, mining practices and education. Legislated regulations should realistically reflect the needs of both the mine operators, protection of the environment and the best interest of the general public. Benefits should be provided for compliance with regulations instead of penalties being assessed for noncompliance. Hazardous conditions and environmental problems associated with mines can be alleviated through improved mine design and effective cloning procedures. Education should provide an appreciation by the general public of past and present problems facing mine operators. This will result in an improved mining climate which, in turn, will insure a continuing supply of minerals to meet increasing needs.

The objective of this contract was to develop a simple, reliable, economical, remote-control roof bolting module for installation of mechanical and resin bolts. It was to be capable of placing longer than seam height bolts, up to 10 ft lengths, from a seam height of 36 to 72 in., while keeping the operator under supported roof. The resulting machine, called REM-Bolter, is a low profile unit that utilizes Foster-Miller Flexible Shaft Roof Drill, a Bendix Roof Bolt Inserter and a newly designed. Roof Bolt Lifter/Torquer. Every effort was made to strike a compromise between complete automation and complete manual control. In a controlled laboratory environment the REM-Bolter remotely installed 1000 bolts. Subsequently the machine was tested in an operating coal mine where 500 holes were drilled (and an equal number of bolts were installed) remotely, over a 4 month period. The results of all tests Live shown that the operator of a machine of this kind can quickly, and repeatedly, install rootbolts remotely from his operator compartment some 8 ft away from the drilling/bolting action. The report includes recommendations for improvements to the present REM-Bolter design that would translate the present test bed into a second generation test bed and a potential first generation production unit.

The objectives of the research were to determine whether a model health and safety program could be defined in terms which would enable its application to mines of all types, and, if such a model program could be defined to determine through evaluations in two mining companies whether it would achieve measurable improvements. This volume describes the model, how it was applied in the field, and how it was evaluated. Anecdotal data to illustrate common kinds of safety problems and their solutions are included. The literature and field research that preceded the model definition is discussed in Vol. 2, which covers the period of September, 1980 to September, 1981.

Michigan is becoming one of the important oil-producing States of the country. The amount of crude oil produced within its boundaries has increased from only 4,000 barrels in 19253/ to nearly 16 million barrels in 19935.4/ The Bureau of Mines method of crude-oil analysis and classification-indicates that the oils from the several producing fields are predominantly paraffin-intermediate-base, wax-bearing crudes. In general, they will yield 25 to 35 percent of highly paraffinic gasoline and about the t- me amount of kerosene and gas-oil combined, 7.2 to 15 percent of lubricating oil stock, and 20 percent of residuum with a carbon residue or about 6 to 7 percent. In addition, there appears to be some correlation between certain properties of the crude oils and both the geographical location and geological structure of the oil-producing areas within the State. These general conclusions are based on the analytical data presented in this report, which is one of a series of papers that record the general characteristics of crude oil from the many producing fields of the United States. This report discusses briefly the oil fields of Michigan and the characteristics of the crude oils produced from them. The analyses of 15 samples are given. The U.S. Bureau of Mines has designed and evaluated total mill ventilation systems at two different mineral processing operations. Both systems have proven very effective at reducing respirable dust levels throughout the mill in a cost-effective manner. A 25,500-cfm system installed at a clay processing mill provided approximately 10 air changes per hour. This system reduced respirable dust concentrations by approximately 40 pct throughout the mill building. The second evaluation was performed at a silica sand operation. Tests were performed with 50,000 and 100,000 cfm of ventilation to the mill building, corresponding to 17 and 34 air changes per hour. Average mill-wide respirable dust reductions were 36 and 64 pct, respectively. Not only did these systems reduce respirable dust concentrations and increase visibility throughout the mills, they were also easy to install and required minimal maintenance. A total mill ventilation system provides a general purging of the mill air; the system should be viewed as a supplemental technique to assist other dust control systems in operation.

From 1981-84, Bureau of Mines personnel examined and evaluated mines and prospects in the highly mineralized 62,400 acre Inyo Mountains U. S. Bureau of Land Management (BLM) Wilderness Study Area (WSA), located east of Lone Pine, CA. The 62,400 acres studied is the part of the 87,145 acre WSA designated as suitable prior to February 28, 1983. The WSA is underlain principally by quartz monzonite and quartz diorite that have intruded sedimentary and volcanic rocks. Three types of mineral deposits are present: 1) gold in fissure veins, 2) silver in fissure and replacement veins, and 3) talc in metasomatic lenses. Of the 84 nines, mills, and prospects examined, 54 are significant: 1) 5 in the intrusive rocks total about 4.4 million tons of inferred marginal gold reserves averaging 0.24 oz gold and 0.74 oz silver/ton; 2) three associated with intrusive-sedimentary rock contact zones total about 280,000 tons of inferred subeconomic silver resources averaging 7 oz silver/ton; 3) 27 gold and silver occurrences in the intrusive and sedimentary rocks total about 1.2 million tons of material in poorly defined bodies too small or low grade to be resources; 4) 15 gold and silver occurrences in the intrusive and sedimentary rocks are too poorly exposed to estimate tonnage and grade; and 5) four talc deposits along intrusive-sedimentary contact zones have inferred marginal reserves totaling about 640,000 tons. The marginal gold reserves at $450/oz might be profitably mined (reserve) at a gold price of about $600/oz and the subeconomic silver resources at a silver price in excess of about $27/oz. The marginal talc reserves might become minable at prices of about $50/ton and $80/ton for unprocessed and processed talc, respectively, after present supply sources are depleted. At most of the 54 significant mines and prospects, additional reserves or resources would likely be disclosed by trenching or drilling. Present in the WSA are 13 gold mills, numerous small pits and cuts, 270 underground workings totaling about 36,000 ft, and 80 mining benches totaling about 11,000 ft. Gold, silver, or talc were produced from most of the workings. A total of 1,561 lode and 18 placer samples were taken.

Pegmatites in Colorado occur for 150 miles along the Front Range (eastern-most range of the Rocky Mountains) from Larimer County on the north to Canon City on the south, and west into the Gunnison area. Many of the deposits have been classified. Minerals found in the various dikes include beryl, scrap and possibly punch mica, lithium-bearing minerals, columbite-tantalite, feldspar, and several rare minerals. Pegmatites are abundant in the Crystal Mountain district in Larimer County. About 5 miles south of the Crystal Mountain district and often identified with that district is the Hyatt pegmatite area. The Hyatt Ranch deposit has attracted particular attention because of the comparatively high beryl con-tent of the upper part exposed in opencut workings.

Injury experience at coal mines and mechanical-cleaning plants (excluding officeworkers ) in 1967 was 222 fatal and 10,115 nonfatal disabling work injuries during an exposure time of 241.8 million man-hours. These injuries occurred at a frequency rate of 42-75 per million man-hours worked; the severity rate was 7,699 days lost or charged per million man-hours. Each of these general measures of injury experience was lower than similar data recorded for 1966. Employment at 6,885 coal mines and 569 mechanical-cleaning plants, excluding officeworkers, averaged 139,312 men in 1967. The average number of days worked was 220. Coal production totaled 560.8 million short tons, at rates of 2.32 tons per man-hour and 18.32 tons per man-day. A total of 2,779 onsite officeworkers at coal mines and mechanical- cleaning plants worked 5.2 million man-hours in 1967 without a disabling work injury.
