The paper describes a technique that has been developed by the Health and Safety Executive (USE) to make an assessment of the effectiveness of Health and Safety (H and S) management at complex sites. The paper then describes how the technique was adapted and used for the first time at a large coal mine. The results highlighted certain deficiencies in the safety management system, allowing directed advice to he given to management.

Respect for mining in Botswana is long-standing. This article looks at Africa's most transparent mining regime and some of its ambitious plans. Botswana's mining industry value is forecast to average 5.2% growth from 2014-2018 with solid growth for the diamond sector and acceleration of coal production. Respect for the country's mining regime is maintained in the Fraser Institute 2014 annual survey where Botswana occupies the 26th position of 122 administrations. The most traum atic event within the lUK coal industry during the last 30 years was its privatisation. Who were the relatively unknown people from RJB: white knights or robber barons? This paper highlights the strong safety and progressive engineering cultures th at existed and continues within the company. A long-term view of the business coupled with proven professionalism is amply developed. Quite dearly, to revert to private ownership is not a return to Victorian values but ‘back to the future’.

The economic growth of Hong Kong and adjacent territories in Southern China is, to a considerable extent, dependant on air transportation, but the present Kai Tak Airport is incapable of expansion to meet future needs. After years of study, agreement was reached to build a new airport on the island of Chek Lap Kok, some 34 km remote from Hong Kong, with an ultimate capacity of 87M passengers and 8.9 Ml of cargo per annum. Apart from the building of the airport and its associated infrastructure (including a new town), other similar massive construction works have been necessary in the form of land reclamation from the sea and existing outer lying islands. The construction of bridges, tunnels, railways and highways are also an integral part of the project. The paper provides a broad overview of the concepts of the scheme (called The Airport Core Programme) and highlights some of the major operations and equipment being used in the projects, whose cost at 1992 prices was reckoned to he Hong Kong (HR)$l63hn.

Sulphur isotope ratios have been determined for 81 sulphides and S and O isotope ratios for 26 barites from the Alston Block in the northern Pennine ore field. In addition, both isotope ratios have been determined for 29 evaporitic anhydrites of various ages from European and Australian localities. The consistency of the sulphate isotope ratios over a wide area of the northern Pennines, and their similarity to values for Lower Carboniferous evaporites, suggests that the sulphate originated from Lower Carboniferous connate waters. It is suggested that these mingled with warm brines rising through granitic plutons beneath Tynehead and Upper Weardale, the aqueous sulphide ion in these brines having a delta34S value of approximately +10 per mille. The delta34S values for the sulphides range from +15.0 to -5.9 per mille and show an outward zoning with higher values in the central fluorite zones and mainly negative values in the outer barite zones. The wide range of delta34S values for the sulphide S is not characteristic of magmatic S and there is no adequate local source in the Carboniferous sediments. It is suggested that formation waters, probably mainly of connate origin, sank down sufficiently deep to become heated above 200°C before rising through Caledonian granite plutons into the Carboniferous sediments, where they spread laterally to mingle with cooler sulphatic connate water. The rising fluids were hypersaline brines carrying F, Ba, Pb, Zn, Cu and S. Temperatures of deposition calculated from galena-sphalerite isotope fractionation are similar to temperatures previously determined from fluid inclusion studies.

Whilst spontaneous combustion has never been considered a major problem in south Durham, certain collieries in the area have a history of heatings and precautions are essential. Three collieries are affected - Shotton, Vane Tempest and Seaham - and in the case of the last two, there are huge areas of high quality coal to work under conditions where spontaneous combustion is a real hazard. The seam being extracted has an immediate roof in goaf left by the incomplete extraction of a contiguous seam worked before the turn of the century. The presence of residual coal stocks and old roadways and cavities adds to the difficulty of preventing unwanted air movement. The main preventive measures adopted are the construction of airtight gateside packs and the use of sprayed surface coatings in the gate. Sampling systems and sampling points are described and, in particular the field of continuous monitoring from the surface is examined. The resulting possibility of a quantitative as well as qualitative assessment of the preventative measures required is discussed.

A wide variety of mining conditions exists within the South Midlands Area and across this range of conditions more than one half of the mines are highly susceptible to spontaneous combustion. Two thick seams in which the greatest risk exists are described and illustrated. The different heatings which take place are categorized by the locations at which they occur in the mines: additional special cases not easily categorized by location are discussed separately. The discussion for each category of heating follows the pattern of: (i) the reasons why it occurs; (ii) when it arises the means by which it is fought, and (iii) the preventive measures taken in an attempt to avoid its occurrence. Relatively new factors which may affect the incidence of heatings are highlighted and suggestions are made which may assist in reducing the number of incidents in the Area. A new method of continuous sampling, analysis and recording is described and mention made of an original method of interpreting carbon monoxide results with the aid of the computer. The first results from this work show that an early warning is given of an adverse change in trend several weeks earlier than with the present methods. Lastly, suggestions are made about the direction in which research might proceed to assist in reducing the special hazards created by several features inherent in the present methods of mining.

Both climate and methane control have been a prime concern in the development of the workings within the Deep Soft seam at Harworth Colliery. This paper describes the numerous methods employed over the past 15 years to effect control over these two major environmental pollutants at Harworth and describes a future method of working which will enable the output levels required to be achieved, whilst effecting the required level of both climatic and methane control. Firstly, the paper gives a broad historical overview of the steps taken since 1981, when production started in the Deep Soft seam at Harworth Colliery, to effect control over the main environmental pollutants in the seam. Secondly, the paper details the results of planning exercises undertaken to assess the various options with respect to panel layout, to achieve environmental control in future production districts. Finally, the recommended method of panel design to achieve the levels of production required is detailed.

Agre, John Newman, Gossart and Roberts, Francis Ellis, John Dillon and Mr. Wood (refrigerating lamp); 1836-1843 developments, notably of Mueseler lamps; 1844-1866, including lamps of the Davy, Clanny, Boty, Eloin and other types; 1867-1879, including the Gray lamp; 1880-1887, including the Marsaut lamp; and 1888-1913. Detailed reference is made to the findings of various relevant Royal Commissions and appendices include a list of the safety-lamps described in an appendix to the 1886 report and a publications list. 25th Julius Wernher memorial lecture given at Tunnelling '91, sixth international tunnelling symposium held in London, 14-18 April 1991. The title gives the lecturer a chance to share some of the lessons he has learnt in a lifetime of tunnelling and particularly in the last six years of his involvement with the Channel Tunnel and the International Tunnelling Association. These included: the difficulties of split contracts but the problems arising with Eurotunnel over the need to create a single owner to whom the banks could lend; the peripatetic nature of tunnellers who built London's underground in the 1960s and returned to build the Channel Tunnel from Egypt, Singapore, Hong Kong and Australia; the increasing requirement for strategic planning resulting in tunnels between European countries and the need for engineers to deal with decision-makers; EU work on harmonising regulations so that there need no longer be, for example, arguments over which regulations should be followed for temporary electrical equipment in the section of the UK tunnel that is technically in France; the huge improvements in safety standards and attitudes; the enormous increase in the scale of undertakings and individual contracts, with an accompanying change in attitudes to logistics; technological advances to cater for the high performance now required of equipment; and the rising importance of environmental issues. "(1) The radiological appearances of pneumokoniosis on the Kolar Gold Field is described in detail. The characteristic abnormality is the. presence of a diffuse, fine annular and punctate mottling. (2) The frequency of symptoms could not be determined with any degree of accuracy, but it is believed that symptoms are absent and there is no incapacity until profuse radiological mottling is present, i.e., after long exposure to dust. (3) There is an increased predisposition to tuberculosis, which, if it occurs, affects the prognosismost unfavourably.(4) Pathological examination shows that the radiological mottling is due to the presence inthe lung of numerous small dust-cdll aggregates (up to T5 mm. in dia.) deposited around the smaller bronchioles and blood-vessels.(5) The dust-cell aggregates are relatively acellular but when tuberculosis is superadded thereis an excessive cellular and fibrotic reaction.(6) The difficulties of defining the radiological and pathological appearances of early pneumokoniosisare discussed.(7) The incidence of pneumokoniosis in 7,653 workmen with 5 or more years’ undergroundservice was 43-75%, but it is suggested that in future the incidence will be appreciably lower.(8) The methods of- dust production and the chemistry of mine dust are described. It isshown that the free silica-content of air-borne mine -dust is relatively low.(9) The aetiology'of the pneumokoniotic lesion is discussed."

In the UK the development of the science of strata control has been strengthened by a resurgence of interest in the specialism of roofbolting and its associated techniques. The evolution of the theories and practices of strata bolting in roadway stabilisation is discussed. Practical experiences are considered, principally the use of Australian techniques, the relationship between bond strength and annulus and the effects of stress in the ground. Areas of new technology such as cable bolting, injection anchors and on-board bolters are also discussed. The recent advances in computer technology and rock testing equipment and methods have provided the opportunity to simulate ground behaviour under various conditions. Computer modelling developments and the use of techniques to predict ground behaviour are described.

Paper presented at the conference: Risk assessment in the extractive industries, held in Exeter, UK, 23-24 March 1994, and first published London: IMM, 1994, preprint volume, 11pp., 15 refs. Hazards such as vibration, dust emission and fragment projection can result in an area called the Danger Zone being set around a blasting site. Traditionally Danger Zones have been set solely on the basis of consequences (e.g. based on the maximum distance of rock projection or expected ground vibration plus a defined safety margin). Advances in risk assessment and its growing acceptance as a safety management tool have led to the setting of Danger Zones based on both consequences and frequency of occurrence. This allows distances to be optimised on the basis of acceptable risk levels rather than the potential consequences of infrequent events. The setting of a flyrock Danger Zone is used to demonstrate a risk-based approach compared with a traditional consequence approach

Risk management is concerned with the whole spectrum of financial, geological, engineering, legal, governmental, social and occasionally ethnic issues, concerned with mining projects. The inherent risks in mining operations are well publicised, but new issues and hazards arrive on the scene and lake precedence, but the older risks still remain, while insurance cover may not he ‘full cover’ in the future. It has long been recognised that quantitative approaches to risk analysis can play a major role, but although these methods are capable of considerable theoretical precision, the lack of sound risk input data and experienced personnel limits the usefulness of theoretical analysis. It can, however, greatly assist in comparing cases, in identifying areas of risk and help in the investment decision process. Its major contribution to a mining project is that it imposes an investigative discipline that also assists in improving the quantitative approach to risk analysis and hence risk management. Risk analysis and risk management also requires sound common sense and major inputs from experienced geologists, mining, mechanical, electrical and mineral processing engineers. The Engineering Council, in 1992, issued a ten point code of professional practice entitled ‘Engineers and Risk Issues’ (The Engineering Council. 1992), covering our professional responsibilities, the law, our conduct, the need for a systematic approach to risk issues, the use of professional judgement, communications, risk management, evaluation of risk, our professional development in this field and the encouragement of public awareness of risk issues. This is a specific area of training perhaps long neglected by our industry and academic institutions (Atkinson. 1983).
