This paper presents an analysis of fracture depth and intensity due to subsidence over the longwall panel. Sonic reflection techniques were utilized to determine fracture depth, and a variation of p-wave velocity was used as an indication of the fracture intensity. Instrumentation has been tested for consistency and accuracy first in the laboratory using small scale models, then applied in the field in two mine sites with different mining geometry and geologic conditions. A new sonic viewer was utilized for sonic velocity measurements. Regular hammering method was on acoustic source. The results were concurrent with monitored horizontal strain profiles and measured open fractures over the longwall panels.

In 2007, a total of 2,030 coal mining operations reported employment to the Mine Safety and Health Administration (MSHA).1 Coal mines comprised 13.7% of all mining operations. ? Bituminous mines comprised 92.4% (n=1,875) and anthracite mines 7.6% (n=155) of coal mining operations. ? Approximately 72% of all coal mines were located in three states: Kentucky (28.4%), West Virginia (22.6%), and Pennsylvania (20.5%).

The Metropolitan Transportation Authority (MTA) East Side Access project (ESA) will, when it is complete, provide a new link to the east side of Manhattan from Long Island, NY that, linked with the Second Ave. Subway currently under construction, will assist in relieving overcrowding at Penn Station as well as the Lexington Ave. subway. ESA will bring Long Island Railroad (LIRR) directly from Long Island into a new station located 37 m (120 ft) beneath the existing Grand Central Terminal (GCT) located in the heart of Manhattan. It will connect the LIRR?s mainline tracks in Queens, via the existing 63rd Street Tunnel under the East River, to a new terminal constructed within, and beneath, the historic Grand Central Terminal.

Xstrata Copper at Mount Isa in Australia has operated one copper Isasmelt furnace and four P-S converters. The phenomena of foaming occur in above 10% of converter operations. A thermodynamic model of copper P-S converter and a viscosity model of converter slag have been developed to simulate the slag blow and copper blow. The model predicts the chemistry and viscosity of copper skims during copper blow. The effects of oxygen potential, charge of dirty materials, slag carry-over, under-blowing, over-blowing, and temperature on the foaming and viscosity of copper skims have been discussed. Some improvements of the industrial operations have been presented.

For the last 150 years, subsidence damage to the surface has been an unavoidable phenomenon associated with underground coal mining. In particular, the mechanization of mining methods in multiple seam mining and the concentration of mining activities in densely populated European metropolitan areas (e.g., Ruhr, Upper Silesian industrial region) have led to mining having a very strong influence on the earth?s surface. In both these regions, surface subsidence of more than 20m has been reported up to now in comparison to the original state. Underground coal mining in these regions strongly impairs all kinds of surface use, such as urban and industrial development, infrastructure (roads, waterways, and railways) and pipelines?without restricting the functionality of those objects to a greater extent. Demanding and, in some cases, globally unique engineering solutions have been developed and implemented for this purpose. The previously mentioned mining and industrial regions are still very strongly characterized by the (increasingly declining) coal industry and will remain dependent to a large extent and for an indefinite period on artificial drainage measures because subsidence that has already occurred is irreversible. This paper will primarily discuss the prediction of subsidence that is caused by intensive multiple seam mining and its influence on surface objects (e.g., underground pipelines). However, an approach for predicting ground uplifts due to rising mine water as a consequence of mine closures will also be described.

The need for anti-corrosion coatings has existed for many years in the ground support industry, but the application requirements have made coating selection limited. Varying groundwater and chemical conditions continually challenge long term coating performance. Traditionally the industry coating preference has been hot-dip galvanizing due to its performance characteristics in highly corrosive environments and low cost nature. However, the galvanized coating has application limitations. This paper highlights some of these limitations and provides selection criteria for a new organic coating developed for one such difficult application, DSI?s Omega BoltPTM Pwhich is an expandable ground support. "The issue of mine worker fatigue is as complex as it is understudied. Generally, worker fatigue lies somewhere on a continuum between psychology and physiology, which makes the measurement and management of it difficult with no easy solutions for operators or workers. Answers to even basic questions remain elusive—such as what is fatigue, how is it measured, and how can organizational resources reduce fatigue—and answers seem to present no single, clear path to action for the health and safety of the worker. This manuscript supplements the growing body of interest in the problems of fatigue in the mining industry by reviewing some of what is known about worker fatigue in general and in the mining industry in particular. The material covered here includes basic topics of the hows and whys of worker fatigue; an overview of fatigue models, findings, and evidence-based solutions with a focus on mining; and potential future directions aimed at mitigating miner fatigue. INTRODUCTION “It is recommended that the whole fatigue test problem be stated in a form the nature of which may be indicated by the following suggestions: that the term fatigue be absolutely banished from precise scientific discussion, and consequently that attempts to obtain a fatigue test be abandoned [emphasis added].” – Bernard Muscio, Pioneer Australian Philosopher and Industrial Psychologist, in a 1921 Report to the Cambridge Industrial Fatigue Research Board Fatigue presents several challenges for the mining industry. Depending on the specific occupation, daily work, or operational setup on any given mine site, mining jobs can have a fair amount of labor-intensive tasks mixed with monotonous and repetative duties. Combined with the long working hours and shift-work schedules of mining work, the prevalence of fatigue in mine workers may seem rather unsurprising. On the one hand, mining is certainly not alone in facing the challenge of addressing worker fatigue. Indeed, many of the characteristics above mirror the similarities of fatigue in other industries, such as health care, aviation, and security. To the extent that fatigue in mining acts like fatigue in any other industry, then any fatigue management applications, trainings, or interventions in existence can be borrowed from other industries and applied to mining in a “cookie-cutter” approach. On the other hand, some have argued that mining in particular is especially susceptible to increases in the prevalence of fatigue beyond the characteristics listed above due to the multifaceted combination of factors in mining environments associated with fatigue: dim lighting; limited visual acuity; hot temperatures; loud noise; highly repetitive, sustained, and monotonous tasks; shiftwork; long work hours; early morning awakenings; and generally poor sleep habits (Canadian Centre for Occupational Health and Safety, 2012; Legault, 2011). Legault (2011) in particular argues that it is the combination of these factors simultaneously that can make mineworkers particularly susceptible to sleep deprivation and fatigue in comparison to other industries where these factors are often not present alltogether. If fatigue looks and acts different in mining, as others have argued, more research is needed to determine if, how, and why worker fatigue might need to be managed differently in mining."

One of the valuation approaches applied to Advanced Exploration Areas and Pre-Development Projects is the so-called ?in situ Mineralization Method? or the ?Yardstick Method?. This method is based on the concept of a known undeveloped Mineral Resource being worth a certain fraction (the ?Yardstick?) of the contained commodity value (?metal in ground?), using commodity prices ruling at the date of valuation. The Yardstick value is derived usually from an average market-based figure obtained from an analysis of recent transactions and is expressed as a monetary value per ounce or per tonne. It maybe modified to accommodate the quality (e.g. grade) and category (e.g. confidence level) of the Mineral Resource/Reserve involved and the time, effort and expenditure expected to be involved in bringing the project to fruition. Some eighteen recent and relevant transactions were analysed, allowing the following general conclusions to be reached: ? a Yardstick with which to value undeveloped PGE mineral resources is likely to range between 0.5% and 2.5% of the in-ground contained metal value ? the expected method of mining (especially with regard to mining costs) and the grade of the mineralization both have a strong influence on the Yardstick ? the status (classification) of the Mineral Resource has some influence on the Yardstick and ? the ruling price of platinum and the regional location of the deposit appear to have insignificant influences on the Yardstick.

"Abstract: BAW and BAM have performed a large scale comparison and calibration test on static and dynamic load capacity evaluation of bored piles in glacial sandy soil. The test was performed using eight piles at the BAM test site for technical safety at Horstwalde 50 km south of Berlin. The test area has been prepared and investigated in great detail using boreholes, cone penetration tests, pore pressure sensors and geophysical methods to assure controlled conditions for all piles and tests. The piles (10 m length, 0.9 m diameter) are mainly friction piles (low toe resistance) and have been checked by integrity testing. Five piles have been tested by five contractors using the dynamic method in a blind experiment, the other ones piles by static load and/or later on by the dynamic method. Some piles have been equipped with additional fibre optic instrumentation which proved to be robust and helpful in interpreting the results of static, dynamic and integrity tests. We have experienced a deviation of the dynamic load test results gathered in the blind experiment from the static values of up to 20% in most cases, sometimes even up to 30%. This can be related to the known soil inhomogeneities, interpretation and modelling in CAPWAP and method inherent uncertainties. In cases where the static values were known by the testers for calibration, the deviations were significantly smaller. It has to be taken into account, that the two static load tests showed different results as well. Due to the low toe resistance, use of a big drop weight (11 tons) and large drop heights most piles suffered from cracking, which was clearly seen in follow up integrity tests and confirmed by excavation. The piles are available for further research.IntroductionBored piles are used in many construction projects of the German waterways authorities, e. g. bridges, piers or locks. Their load carrying capacity varies with pile type and geometry as well as soil type, parameters and structure and is thus not easy to calculate in advance. Dynamic pile testing as a cost effective addition or replacement for expensive load tests is accepted in many places (Beim and Likins 2008) but still doubted in others. Inspired by the international prediction event for piles in residual soil carried out in the frame of ISC’2 (Viana de Fonseca and Santos 2008), governmental research institutes have designed an experiment in a geological setting typical for glacial and post-glacial areas in northern Germany. Some results have already been presented by Niederleithinger et al. (2012), Herten et al. (2013) and Baeßler et al. (2013). It was intended to perform a blind test for load capacity estimation using dynamic pile testing on six drilled shafts involving five test crews. Two other piles would have to be tested thereafter using static and dynamic methods."

The operation is now amongst the industry leaders in plant availability.The integrated maintenance service has delivered: An increase in plant availability from 93%, in the first year, to 96.5% today, generating approximately $50 million additional income per year, Quality work procedures, Process and equipment improvements, Major shutdown planning involving more than 700 personnel, 6000 parts and over 2 000 discrete activities.The ongoing success of the partnership requires a high level of understanding between both companies. A commitment to maintain a long term sustainable relationship, based on a ""win - win"" culture, is the key to this success. "Safety performance in many companies and even industries has stalled in the few years. Accidents rates are at a "plateau" and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas disaster, organizations that have "exemplary" safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Other classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters and even the Chernobyl nuclear reactor. Looking into the root causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there specific cultural features that can be delineated? The research and review presented in this paper covers a period since 1994, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions, based on extensiveresearch since 1997 of organizations that suffered unexpected disasters.The Piper Alpha Oil Rig disaster in the North Sea, on the 6 July 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry.In short, an explosion and fire occurred when a pipe started leaking gas and ignited. A temporary flange, with no safety valve, was used to block off this pipe during a maintenance operation the previous shift. The permit to advise operators not to start the pumps on this line was misplaced and lost. Several deficiencies, problems and system failures coincided. A key factor was that the water deluge system was inoperable at the time and failed to extinguish the large fire that erupted, followed by an even larger gas explosion. The accommodation unit, situated on top of the oil rig, was the main "killing field" - most men gathered here to await instructions (which never came) and where they died from smoke inhalation. Later, a large explosion of the gas pipeline from an adjoined rig exploded and killed more people.The line of events and the associated system failures are all very relevant in the analysis of the accident, and provide the only insight into what caused the accident. But much "further back", inside the organization’s culture, lurks more dark matter. The first inclination is to ask what was "deficient" in the culture. Did they not care enough about safety, or about their people? Did they have a culture where production decisions were more important than safety considerations? Did they ‘coerce’ supervisors and employees to ignore safety precautions or did they just "fly by the seat of their pants"?Not surprisingly, it is very difficult to identify the true nature of the culture in that business because in such an aftermath of death and destruction, no one dare say that the safety culture was a ‘positive one, focused and caring’ or that the company was doing well in safety and that this is only a freak accident. Indeed, there were many deficiencies in the organization’s systems and procedures, such as the permit to work system.But there are some niggling questions: How is then that the Piper Alpha Oil Rig was actually considered the company’s most productive and safest oil rig in the North Sea? Why would this rig win a safety competition, 6 months before the disaster, with the deficient permit as its most outstanding successful system?"

In general, overall experience has shown that the approach to the appointment of a project manager is given much attention, but little specific attention is given first to the risk associated with the project conceptual design and its overall executability within either the new legislative framework or the sustainability of the outcome. The risk in project executability and sustainability within the ?new age? legislative framework of the Mining Charter, etc, is itself however, of far greater importance. The degree of importance being defined by the intended projects impact on the business going forward. This is particularly the case where the intention of the implementation is effectively to turn around or even enhance a loss-making strategic investment like a mine or process plant operation in an ?old-order rights? or Brownfield?s operation. This paper attempts to highlight the importance of a ?pre-project project? (PPP) to mitigate the risk in the ?new age?, with specific reference to operational due diligence (ODD) and project conceptual design.

THE slide-rule, as a quick and convenient instrument for performing approximate calculations, is steadily growing more and more into favour with the engineer, and results obtained by it are considered close enough to enable him to satisfactorily dealwith a considerable portion of his work. To the surveyor, however, whose ordinary computations must be as precise as his measurements, the present makes of rule are insufficient, except for a very narrow margin of the scales and for rough work. Few will carry the rule when they can clearly see that it is unreliable for general routine, and the careful surveyor is inclined to look askance at those who do use it, recognizing the temptation there undtmbtedly must be to push it into work that the log book should be called upon to treat. From the fact that a surveyor's duties take him on the surface in all weathers, and underground in many wet and uncomfortable places, where the manipulation of a book of mathematical tables becomes both awkward and patience-trying, an efficient slide-rule must offer the mbst transparent advantages. Again, a surveyor being specially trained to read divided scales quickly and accurately, the slide-rule must commend itself as an apt addition to his outfit.

Crush pillars have been incorporated into a mechanized, low-profile trackless system at South Deep Gold Mine. These pillars had to be designed to fail near the face and to ensure that pillar failure is contained within the pillar, to avoid bursting and the risk of high loads being generated during a seismic event, respectively. PoweRite backfill bags were recommended to maintain the integrity of the pillars; except in the main access drives, where the sidewalls were to be supported on 5.6 mm diameter weldmesh and yielding anchors. The results of a trial site investigation exceeded expectations, showing a residual pillar strength of about 37 MPa for a newly formed pillar and 8 MPa for a pillar subjected to seismicity and a closure of more than 300 mm. The introduction of these pillars has improved the rock mass conditions because of the active nature of the support, compared to the previous passive backfill method. Importantly, the pillars have increased mining efficiencies and improved face availability. A potential cost saving to the mine of R140.9 million could be realized over a period of 10 years.

A simulated rockburst experiment was conducted underground at a deep level gold mine. This was done by means of a large explosion detonated in solid rock close to a tunnel sidewall. The resultant shake-out damage is typical of that associated with a small seismic event in close proximity to a tunnel that is subjected to the estimated field stress, as in this case, of 50 MPa. The experiment involved: ?the design of a blast to mimic a seismic source ?seismic monitoring using a dense seismic array ?high speed video filming ?a study of rock mass conditions (fractures, joints, rock strength etc.) before and after the simulation using mapping and ground penetrating radar ?special investigations to evaluate the mechanism and the magnitude of the damage ?a study of support behaviour under excessive dynamic loading.

Significant movement to an existing slurry wall which serves as a retaining wall on 'O' Street in Southeast Washington, D.C. initiated the Department of Public Works for the District of Columbia to invite specialty contractors to propose a rehabilitation and stabilization for the failed slurry wall. Nicholson Construction Company proposed and was awarded a contract for the construction of an anchored precast lagging and soldier pile wall to be located approximately 4 feet in front of the existing structure with granular fill placed between the new and existing walls. Stabilization of the existing slurry wall also included drilling horizontal weep holes and demolishing the failed panels down to the existing ground surface. A new and improved drainage system was also constructed to increase the amount of water directed toward the existing storm sewer system running along 'O' Street. This paper provides a detailed account of these construction activities.
