The authors previously had developed a methodology that combines injury and citation measures into an overall mine safety performance evaluation tool, called the Safe Performance Index (SPI). It can be easily adopted by mine operators to monitor trends in their injury and citation experiences as a risk assessment method. In this paper, six normalized safety measures and the SPI are used to evaluate the risk that existed at mines in the two years preceding the occurrence of a fatality. The percentage of mines is given for which the risk was high or where trends of some of the safety measures indicated moderate risk. In 50% of the mines, this mine safety performance tracking method could have been helpful to the companies, state agency, or MSHA in recognizing and addressing emerging problems with actions that may have been able to prevent high-risk conditions, the fatality, and/or other serious injuries. The approach would have given scrutiny to the risk for mines that encompassed 74% of the fatalities during 2007-2010.

Ventiflex is a flexible ventilation duct system with a global market within mine and tunnel ventilation. The system is distributed in a wide range of qualities and dimensions. It is very important that ventilation systems are designed correctly in mines and tunnels. An incorrectly dimensioned system might result in bad air quality and too high costs. The objective of this study has been to assess some important design parameters for ventiflex comprising the friction loss factor for the duct wall itself, as well as the shock loss factors and the leakage factors for the different couplings systems. This has been obtained by carrying out a large scale experimental program on ducts with all the three different couplings: Velcro, zip and steel-ring. Pressure profiles and air flow rates have been measured, and based on these data the different factors have been calculated. The following results have been obtained: A friction factor l = 0,012 (K = 10) for the duct wall, a shock loss factor z = 0,0125 for velcro- and zip couplings, a shock loss factor z = 0,005 for steel-ring couplings, and a leakage factor f* = 0 mm2/m2 for all three coupling types. The latter result proves that no leakage is possible for a correctly installed duct with proper alignment. The results also confirm that ventiflex complies with class S, or better, according to the SIA-norm. However, whether ventiflex should be designed as class S, A or B according to the SIA-norm, depends on the particular application.

INTRODUCTION The level of analysis for the selection of extraction practices and equipment for surface mining differs significantly from that found in underground mining. In underground mines, manufacturers establish the operating ranges of the equipment, such as continuous miners, loaders, and boring machines, based upon required clearances. Even in conventional mining, the drilling and blasting operation is usually based upon experience. The selection of extraction methods and equipment for surface mining is more complex. Since the equipment is not as confined as in deep mining, and the topography tends to be highly variable over the limits of the mining property, primary extraction equipment must be sized according to reach and capacity. Further, due to the depositional characteristics of the over- burden, drilling and blasting patterns must be well- designed to provide proper fragmentation. The following sections provide a general outline for the selection of surface excavation practices and equipment. The topics covered include drilling and blasting and the selection of both primary extraction equipment (draglines and stripping shovels) and secondary extraction equipment (bulldozers, scrapers, and front-end loaders). EXCAVATION FUNDAMENTALS Before discussing surface excavation, a few concepts need to be defined. A bank cubic yard of material is equal to one cubic yard as it lies in its natural or undisturbed state. A loose cubic yard is that portion of a bank yard that after being disturbed has expanded to measure one cubic yard. Swell is the volume increase of a material when it is removed from the natural state. Finally, load factor is the percentage decrease in the density of a material from its natural state to a loose state. Table 1 summarizes these concepts for various materials. When the amount of overburden to be removed to meet a predetermined production rate is established, a suitable bucket size for the primary stripping equipment must be calculated. The following equation can be used: NBSIZE = TBC/[(OF) (BF)] (1) where NBSIZE is the actual bucket capacity after considering the swell percentage, the bucket fill factor, and the operating efficiency; TBC is the theoretical bucket capacity (TBC = COB/NTCPM); OF is the operating efficiency; BF is the bucket fill factor; COB is the cubic yards of overburden to be removed; NTCPM is the number of theoretical cycles per month [NTCPM = (SMH) (3600)/(CT)]; SMH is the scheduled monthly hours; and CT is the cycle time in seconds. FRAGMENTATION PROCEDURES Drilling Where the strength of overburden material is such that blasting is required prior to excavation, drilling is conducted before using explosives. Two methods of drilling are found in mining-percussion and rotary. Penetration of overburden by percussion drilling is carried out by successive impacts of the bit into the rock. Emphasis in this section will be placed on rotary drilling, because for surface mining it is the most versatile form of overburden penetration. Rotary drilling is conducted by rotating a rigid string of tubular rods to which a rock-cutting bit is attached. The drilling energy is supplied by rotation as well as thrust and the cutting action consists of abrasion, scraping, spalling, or chipping. The size range for rotary-drilled blastholes is from 4 in. to 15 in., with 9 in. and 12 in. being common sizes. There are two types of bits used for rotary drilling-drag and rolling cutter. Drag bits are applicable in soft to medium-soft shales or their equivalents. Since their effectiveness is dependent upon a successful scraping action, drilling thrust is lower for drag bits than for rolling cutter bits; the opposite is true for torque requirements. The bits are usually fitted with replace- able tungsten-carbide cutters. Rolling cutter bits, though available in a two-cone configuration, are usually of the three-cone style (Table 2). The rolling cones have either large steel teeth for drilling through soft formations or small tungsten-car- bide inserts for drilling through hard formations. Table 3 relates bit size and weight to the strength of the rock. The major advantage of rotary drilling is its rapid drilling rate. This is due to the fact that, unlike percussion drilling, rock penetration is uniform with depth. Unmineable coal seams present an opportunity to sequester carbon dioxide emissions from power plants, while enhancing the recovery of coalbed methane. Injected carbon dioxide is adsorbed and stored on the coal surface while releasing methane that can be commercially recovered. Carbon sequestration, therefore, allows the utilization of unexploited mineral resources while reducing “greenhouse” gas emissions. This paper presents the feasibility, efficiency and enhanced recovery potential of coalbed methane production using carbon dioxide sequestration in the Central Appalachian Basin. The focus of research is on geological characterization of unmineable coal seams in Central Appalachia and the corresponding carbon dioxide storage capacity. Virginia Tech and Marshall Miller & Associates, Inc. conducted this research under the Department of Energy’s Southeast Regional Carbon Sequestration Partnership program.

The recently completed Lower Northwest Interceptor Program (LNWI) in Sacramento, California comprises, ten pairs of micro tunnels totaling 4,369 m (14,417 ft),three horizontal directionally drilled bores totaling 956 m (3,154 ft), and two large diameter tunnels under the Sacramento River totaling 1,198 m (3,953 ft). This paper briefly summarizes surface and subsurface conditions, and planning and design considerations. It then goes on to describe problems encounterd with each trenchless crossing, as well as lessons learned with respect to shaft construction, ground control at shafts during tunnel break-in and break-out, frac-out and heave, settlement, obstructions, coordination with permitting agencies, and application of geotechnical baseline reports.

An assessment of U.S. western hardrock miners? noise exposures was conducted as part of a multi-year National Institute for Occupational Safety and Health (NIOSH) survey of noise exposures in each sector of the mining industry. Noise from selected mining equipment and operator noise exposures were measured, analyzed and tabulated for dissemination to the participating sites and are being used to direct NIOSH research and interventions to address the greatest noise hazards. Eighty-two noise dosimeter measurements were obtained, along with time-motion studies as the miners operated hardrock mining machines. Of the operators, 96% had daily noise doses that exceeded the U.S. Mine Safety and Health Administration?s (MSHA) permissible exposure level. The average gold miner dosages while operating the following equipment were: haul trucks - 261%, load-haul-dumps (LHDs) - 235%, single boom drills - 221%, bolters - 214% and dual boom drills - 163%. The worst exposure level was a silver miner with a daily dose of 873%. Time-motion data showed that this miner?s exposure accumulated most rapidly while operating a jackleg drill. These results will be used to help prioritize noise control development by NIOSH and other partners.

The past several decades have seen the rapid expansion of multinational mining companies? exploration and production activities around the globe. This trend has resulted from a number of factors, including increased demand for resources as a result of worldwide economic growth, the diminishment of known and proven reserves, rising commodity prices, technology improvements, and the liberalization of investment regimes in developing countries with significant natural resources. With those opportunities, however, come associated risks and significantly increased legal compliance obligations that may be imposed by various bodies. That includes domestic regulations, as well as foreign civil and criminal laws with extraterritorial reach. A failure to adhere to those laws can have dire consequences, including substantial monetary fines and penalties, severe damage to a company?s reputation, and even jail time for individual company employees.

Explosion-resistant seals in underground coal mines isolate the environment within the sealed area from the active workings of the mine. The current United States Title 30 Code of Federal Regulations (CFR) requires these seals to be constructed of solid concrete blocks. The CFR also allows for alternative methods or materials to be used to create a seal if they can withstand a static horizontal pressure of 138 kpa (20 psi), provided the method of installation and the material used is approved in the ventilation plan. The purpose of this report is to identify those explosion- resistant seals that have been tested and meet or exceed all the requirements of the CFR.

High titania slag is produced from the smelting of ilmenite. These slags contain approximately 85 per cent TiO2 and consist mainly of a M3O5 solid-solution phase. During cooling the slag can decrepitate to a fine particulate material, which decreases the value of the product. Based on X-ray diffraction analyses of the decrepitated slag after cooling, it was found that the M3O5 phase had almost disappeared. The disappearance of the M3O5 phase was linked to oxidation of the slag. A M6O11 phase formed in the decrepitated slag. It is postulated that decrepitation of high titania slag is caused by volume changes due to the oxidation of the M3O5 phase in the slag.

The San Vicente Pipeline Tunnel Project involves the construction of an 8.5-foot-diameter water pipeline in an 11 mile tunnel in San Diego, California. The tunnel is located in extremely variable geologic conditions of strong granitic and volcanic rocks, weak sedimentary rocks, abrasive conglomerates, and mixed face conditions. One reach of the tunnel is a 5,200-foot mixed face reach that includes Friars Formation Conglomerate overlying granitic rock. Due to the variable ground conditions in this reach, the use of a TBM was deemed too risky and the contract documents required drill-and-blast and hand mining methods. Although it was not required, the contractor elected to construct this reach using NATM methods. This paper describes the geology of the 5,200-foot mixed-face reach, the design evaluation, and the construction challenges faced on this project.

Mining is one of the industries with the highest risk of occupational accidents in Turkey and globally. The most important feature that distinguishes the mining industry from other sectors is the constantly changing working conditions. The risk of accidents is very high, especially in underground mining, and can happen for several reasons. Therefore, it is essential to carry out all necessary measures in coordination with other disciplines to adapt to changing workplace conditions. The dangers that might occur underground must be determined in advance, controlled at their source, or eliminated since it is critical to take the necessary precautions before the accident. Accident prevention will protect miners’ living conditions by contributing to the country’s economy through reduced labour and production losses. In this research, the data from 13,653 accidents reported in the Karadon Hard Coal Enterprise between 2000 and 2011 were analysed and compared using the Life Table and the Kaplan-Meier methods. Accident records were classified and analysed into four categories: environmentalrelated accidents, equipment-related accidents, person-related accidents, and other accidents. Findings in terms of survival time indicate that environment-related and person-related accidents had the highest risk in Karadon, with equipment-related accidents having the lowest. This study established the reliability of using survival analysis methods to investigate occupational accidents in the mining sector. Given that environment-related and person-related accidents occur in a relatively short period, such as 1 year after starting work, it has been recommended that research be done to improve workplace and occupational safety measures and broaden the training given to miners. Additionally, it has been determined that increasing the use of automation and mechanisation in underground coal mines, combined with developing a thorough equipment safety training programme to raise awareness of the danger, can help prevent person-related occupational accidents caused by the interaction between equipment.

INTRODUCTION Design of supports for underground chambers in rock has undergone considerable development. This becomes evident when comparing support dimensions of subway station caverns built in the U.S. over the past fifteen years. The Porter Square Station cavern, which is a part of the Massachusetts Bay Transportation Authority (MBTA) Red Line extension, is the latest in this series. It is approximately 168 m (550 ft) long, 21 m (70 ft) wide, and 14 m (45 ft) high, and has a permanent support of light steel ribs imbedded in a 0.4 m nominal (15 in) shotcrete lining. An extensive construction control instrumentation program made it possible to evaluate the performance of the cavern and its support. This evaluation, which is presented in this paper, was the objective of a research project sponsored by the Urban Mass Transportation Administration (UMTA) and jointly conducted by Haley and Aldrich, and MIT. The actually used design procedure was compared with other approaches, notably procedures based on Finite Element analyses and empirical rock mass classification methods. Support dimensions predicted with all these approaches were compared to the actual support and its performance.

"One of the most important environmental issues associated with contemporary openpit mining is the closure of mine pit lakes, and the perpetual legacy these lakes can potentially generate. Ideally, successful pit lake closure includes the following:• The water quality within the pit lake, plus surface water and ground water discharging from the pit lake, meets premining or other acceptable water quality conditions of the receiving environment. • The termination of all active treatment processes.• The return of the pit lake to the government for long-term care and maintenance.• The release of the mining company from all future liabilities associated with the pit lake.• The return of the environmental bond to the mining company.• The reuse of the pit lake for some post-closure, beneficial purpose.The achievement of these goals is known as walk away pit lake closure.Peer-reviewed publications of successful walk away, pit lake closure provide mining companies with the ability to address public concerns regarding the safety of pit lakes which commonly occur during the permitting, operations, and closure phases of mining. To date, most published work on walk away, pit lake closure work has occurred in the coal mining sector in Germany and Australia (McCullough et al., 2009; Jones and McCullough, 2011; Kumar et al., 2013; Schultz et al., 2013). In North America, examples of walk away, pit lake closure are not widely discussed in the literature despite there being excellent examples from the coal mining sector in Alberta, Canada (e.g. Pleasure Island, Black Diamond, East Pit, Sphynx, Robb, Lovette, Siltstone and Sterling), and from the iron mining sector in Minnesota in the United States (e.g. Portsmouth Lake, see MNDNR, 2018). Although seldom included in discussions of pit lakes from the metal and coal mining sectors, the aggregate and fabricated-stone mining sectors also produce pit lakes, and there are numerous examples throughout North America where these pit lakes have been successfully closed and transformed into post-mining resources (Vandenberg, 2018; Crossman, 2019). The difficulty of finding these good examples in the literature makes it challenging for companies to respond to public and regulatory questions regarding pit lake closure and reuse. As such, there is a pressing need to document the motivations and approaches used in examples of successful, walk away, pit lake closure in North America so that the mining industry, at large, can learn from these examples and can better address public concerns."

Presupposition for realizing the vision of SANDVIK for zero harm for working with SANDVIK equipment is the installation of the appropriate processes. Into the product engineering and product development processes of Sandvik Mining and Construction, a specific "Product Safety Process" has been integrated, where all safety related activities like identification of all life cycle hazards, risk management and safety verification / validation are planned, performed and documented. Product safety shall be „designed into" the products. Therefore, specific research activities have been started, to address the safety and health related topics for the development and for the engineering of SANDVIK mining machines."

The Sacramento Regional County Sanitation District (SRCSD) is improving the sewer system in the Natomas area of Sacramento, CA. The Upper Northwest Interceptor (UNWI) system is an ongoing series of tunnel pipeline projects that will provide sewer conveyance for all residents of northeast Sacramento County to the New Natomas Pumping Station, located North of downtown Sacramento, also in the city of Sacramento limits. The New Natomas Pumping Station then carries wastewater flowsto the Sacramento Regional Wastewater Treatment Plant in Elk Grove via the existing Lower Northwest Interceptor. The Joint Venture of Traylor Brothers, inc. and J.F. Shea (TSJV) were awarded the UNWI 1&2 project in late September, 2007. The planned 5.8 km (3.65 mile) construction route for UNWI 1&2 begins at the New Natomas Pump Station and runs north along the East Drainage Canal to the begin-ning of UNWI 3&4 at Bridgecross Drive. UNWI 1&2 includes approximately 5.8 km (19,240 feet) of 3.65 m (144 inch) inside diameter pipe using Earth Pressure Balance Tunneling Methods. The project also includes 16.76 m (55 feet) of 3.04 m (120-inch) diameter interceptor pipeline, twenty three access manholes spaced approximately 305 m (1,000 feet) apart along the alignment, a 2.13 m (84-inch) by 3.65 m (144-inch) transition structure, a 3.65 m (144-inch) by 3.04 m (120-inch) transition structure, and the connection of three existing sanitary sewer lines to the interceptor.

Although many SAG operators may agree that on-line measurement of SAG feed-size is an attractive notion, applying this information in a control strategy that recovers the investment, engineering time, and maintenance is not necessarily straightforward. The P. T. Freeport Split-Online system was justified based on the assumption that SAG feed-size could be used in a feed-forward control strategy either at the regulatory or expert system control level. Something like “as the feed size changes adjust the throughput to avoid under or overloading the mill” was envisioned. However, following installation and calibration, Cluster Analysis showed that such an approach would not be successful. However, the sensor has been successfully integrated into Freeport’s grinding control strategy other ways. It provides an input to the Kalman filter parameter estimator used in a model-based control strategy that optimizes throughput and grind. The Split sensor also provides a characteristic feed size parameter for use in an on-line work index estimator. This paper describes the calibration and installation of the Split SAG feed-size analyzer, and reviews the evolution of its use in the Freeport C3 grinding circuit control strategy.

The Yankee Gulch sodium minerals project (YGP) is a solution mining project operated by American Soda. The mine operates on a federal lease administered by the U.S. Bureau of Land Management (BLM). Commercial operations began in November2000. In 1964, Irv Nielsen discovered nahcolite mineralization where the YGP pilot plant is now located near the Piceance Creek in Rio Blanco County, Colorado (Nielsen, 1969).Development of the project accelerated after Williams Corp., of Tulsa, OK, joined with American Alkali in1996 to form American Soda. Pilot testing by American Soda led to commercial permitting, plant design, construction and startup. Figure 1 shows the location of the Yankee Gulch leases and the location of the upper and lower plant sites.
