Introduction It is generally accepted as dogma that sampling contributes most of the error in gold fire assays. Differences in assay results on pulps from the same sample interval are frequently regarded as evidence of the presence of the so called "nugget effect" of relatively coarse gold particles. It is true that coarse gold particles can contribute to substantial sampling fluctuations. But, while the process of sampling is probably the major source of error, the analytical process cannot be completely ignored as a possible contributor to erratic assay results. To maintain a stable assay process, the analytic part of the system must also be kept in control. One method of monitoring the performance of the analytic system is to systematically assay standard materials, whose sampling characteristics are carefully controlled. Gold assay standards are not prepared, nor can they be prepared, to account for both sampling and analytical errors. It is not possible to send coarse material to a lab for both preparation (i.e., comminution and splitting) and fire assaying and then come to conclusions only about the fire-assay process. Because most gold ores are very heterogeneous, sampling errors would, in most cases, completely mask the contribution of the analytical errors. Assay-standard material is prepared only to assess the accuracy and variability in the fire assay process. Because the objective of the assay standard is to provide information about the fire assaying, it is necessary to control the sampling error of the standard material, so that it is only a minor constituent of the discrepancies observed in any assay results. To do this requires that the particle size of the standard material be reduced to a point where the relative standard deviation of the sampling error (i.e... the standard deviation of the errors divided by the average gold content of the material) is 2% or less. For all but very homogeneous mineralization, this means that the material must be reduced to 100% -150 mesh before the sampling errors are adequately controlled. However, even reducing the particle size can contribute to sampling problems. The liberation of gold may cause segregation that can cause large sampling fluctuations that are not easily controlled while maintaining the desired grade. Because, in most cases, the standard material would already be in the "pulp" state when it is submitted to a lab for assay, it is not possible to entirely conceal the nature of the sample from the lab. This is a problem inherent in using assay standard material. Because of the contribution of sampling to error generation in the assay process, the use of "coarse" material does not solve the problem of submitting a totally "blind" standard to the lab. In the sections that follow, the selection, preparation, testing and use of gold fire assay standard material is discussed. While some may dismiss the production of standard material as folly, it is possible to produce and utilize standard material to stabilize and improve the fire-assay process to produce more reliable assay results. Material selection It is desirable to use material that has as nearly the same metallurgical characteristics as the samples with which the standards will be included. However, this is usually difficult. For many reasons, including the particle size at which a significant amount of the gold mineral is liberated, the sampling characteristics of even -150-mesh material may preclude the use of geologically and metallurgically similar ore as a standard. It is usually easier to get material having desirable grade characteristics with the necessary sampling properties than it is to find geologically and metallurgically similar material with the required sampling characteristics. High-grade standards are especially difficult to find and prepare. This is because, as grade increases, the size of the gold particles usually increases. Larger gold particles are liberated and tend to segregate during comminution, and the homogeneity of the material cannot be maintained. For grades much above 3 g/t (0.088 oz/ton), it is very difficult to find material that has the proper sampling properties. Old mill tailings are likely candidates for assay standards. Some of these have sufficiently homogeneous mineral contents, so that the sampling errors can be effectively controlled. Where mill tailings are either not available or are not acceptable, mineralization that has exhibited homogeneous results in reassays of the pulp material is also a good candidate for the standard. Finally, the mineralized rock being sampled may (and should) be used if adequate homogeneity in the -150mesh material exists. "Adequate" ("acceptable") homogeneity is defined below.) INTRODUCTION The Nuclear Energy Agency plays an active role in promoting international cooperation among its member countries in the field of nuclear energy. In addition to various other functions, it plays a major role in encouraging harmonisation of government regulatory policies and practices, promoting exchange of information, and coordination of research and development in the field of radiological health and safety associated with nuclear fuel cycle activities. The work of the Agency is carried out through a number of specialised standing committees. In particular, the Committee on Radiation Protection and Public Health (CRPPH) is responsible for the Agency's activities concerned with radiological protection and related environmental problems. Its functions include review and discussion of national radiation protection policies and practices, review of developments in radiological protection, interpretation of ICRP recommendations and the study of the means of their translation into practical applications, including the establishment of radiological protection standards. Its functions also include the preparation of technical studies and reviews on specific problems requiring attention, and coordination of further research and development at the international level. Major attention is presently being given to the NEA programme of work on problems associated with radiation protection and environmental impact of nuclear fuel cycle activities, with particular attention to the front-end (uranium mining and milling) and the back-end (waste management) of the fuel cycle. In this context, the increasing attention that has been given in several countries to the problems associated with the exposure of man to radon, thoron and their daughters, and with their dosimetry and measurement, were readily appreciated by NEA, which began an active programme of work in this field in 1976. Because of the detrimental health effects,as demonstrated by epidemiological studies,caused by prolonged exposure to excessive levels of shortlived daughters of radon, particularly in poorly ventilated underground mines, the bulk of attention and needed effort has been focussed on radon and radon daughters in uranium mining. In certain countries some concern has been expressed also about the significant levels of exposure experienced by workers in non-uranium mines,and members of the public who live in particular areas or in dwellings built with particular materials which produce higher than average levels of radon and radon daughters. However, at the time of the first NEA involvement in this field it was considered that one of the most urgent problems to be solved was that of ensuring adequate personal dosimetry for uranium miners. Consequently, the NEA was urged to organise a specialist meeting on personal dosimetry and area monitoring for radon and radon daughters to provide an international forum for exchanging information and reviewing problems in this field. The meeting was held in Elliot Lake, Canada, in October 1976. A second specialist meeting, on the same subject, was held in Paris, in November 1978, to review further developments in this area. These meetings demonstrated that the overall problem associated with exposure to radon and radon daughters had many facets, each of which in recent years has been the subject of considerable attention in many countries for different reasons. It emerged from the meetings that further work was required on a variety of issues in two main areas: 1) dosimetry 2) metrology and monitoring. Conclusions and recommendations which emerged from the two NEA specialist meetings were discussed by the CRPPH. As a consequence the Committee approved, in September 1979, a detailed programme of work in the area of dosimetry and monitoring of radon, thoron and their daughters, and approved the setting up of an international Group of Experts on Radon Dosimetry and Monitoring to undertake the work. The work has been divided into two phases - phase I, on dosimetric aspects, and phase II, on metrology and monitoring aspects. The terms of reference of the work are given in Appendix 1. A list of national representatives on the Group of Experts on Radon Dosimetry and Monitoring is given in Appendix 2; many of these persons are participating here in the Conference. The Group of Experts met for the first time in April 1980 and met again in September 1981. The work of phase I is nearing completion and a report is expected to be submitted soon to the CRPPH for its consideration. Three technical papers providing interim information on the study appear elsewhere in the proceedings of this Conference. The papers cover the three principle areas examined in the study so far: 1) dosimetric aspects 2) review of the "working level" 3) review of objectives and requirements for measurement and monitoring of radon, thoron and daughters. The authors, their affiliations, and the titles of these papers are listed in Appendix 3. A brief overview follows giving the principal results.

INTRODUCTION Dynamic (seismic or microseismic) methods of determining the stability of structures in rock are based on detecting and analyzing the characteristics of seismic energy that has originated from or traveled through the rock mass. This seismic energy can be in the form of naturally occurring rock noise energy resulting from structural adjustments within the rock or can be introduced into the structure by physical means, such as by blasting or impact. In either case, the seismic energy radiating through the rock mass can be detected using standard equipment and can be analyzed by established techniques to reveal a wide variety of information concerning the condition and stability of the rock mass through which the energy has traveled. In the following sections, the basic instrumentation required for seismic and microseismic studies is described, and some of the presently used applications of these methods are discussed to exemplify the state of the art. INSTRUMENTATION Seismic disturbances in a rock structure generate two types of seismic wave radiation, body waves and sometimes surface waves, which radiate outward in all direc¬tions from the source of the disturbance. Underground mining applications are generally concerned only with discerning the characteristics of the resulting body waves, i.e., the compressional (p-wave) and the shear (s-wave) energy. As these two forms of energy travel through the rock structure, the particles of the rock mass are caused to vibrate, and the vibration character¬istics resulting from each of the two types of wave are distinct. Some important differences are: 1) Compressional and shear waves travel at different velocities through the rock structure. 2) The frequency at which each wave causes particles to vibrate is different, and may range from about 50 to 100 000 Hz. 3) The amplitude or energy level of each wave is different, with the shear energy usually being the greatest. These differences form the basis for equipment se¬lection for individual studies and for modern data analysis techniques. The following sections describe the basic equipment necessary to detect and record seismic wave energy data and show several examples of analysis procedures and how these procedures have been used. In principle, seismic equipment is very simple. It consists of a geophone (or geophones) to detect the seismic energy vibration and convert that vibration to an electric signal, an amplification system to increase the level of that signal, and a means of monitoring and/or recording the signals detected. Fig. 1 is a block diagram of a typical system. The following sections offer a very brief discussion of system components and their individual functions. A more complete discussion is given by Blake, Leighton, and Duvall (1974). Geophones The function of the geophone is to detect the vibrations caused by the passing of the seismic wave energy and to convert that vibration into an electrical signal that displays both the amplitude and frequency characteristics of the vibration. Particle motion or vibration can be quantified and measured by measuring displacement, velocity, or acceleration of the particles. Thus, there are three types of geophones: displacement gages, velocity gages, and accelerometers. The choice of gage depends on the characteristic frequencies of the seismic energy to be monitored and the sensitivities of each type of geophone. In general, displacement gages are used for low-frequency monitoring (periods to 1.0 Hz), velocity gages for medium-frequency monitoring (1.0 to 250 Hz), and accelerometers for high-frequency monitoring (250 to 10 000+ Hz). Experience has shown that in underground studies, the choice of which gage to use lies between velocity gages and accelerometers. An easy, accurate method for selection of gage type is discussed by Blake, Leighton, and Duvall (1974). Once the type of geophone has been selected for use, it must be properly installed, and in the installation procedure the most important step is insuring that the gage is firmly attached to a competent portion of the rock structure. Poorly mounted geophones may entirely fail to recognize low-level seismic signals and will distort the information from signals they do see. Amplifiers Seismic events associated with mine structures occur over a very broad range of energy which results in a broad range of geophone output levels. In general, geophone output levels occur in the microvolt to low milli-volt range, and it is necessary to amplify these signals in order to drive recording or monitoring equipment. Introduction Over the last five years, a large number of small- to medium-sized carbon-in-pulp treatment plants have been built in Australia, most designed to treat between 250,000 t/a and 1.5 Mt/a of ore. Because of the limited capital resources and tight cash-flow positions of these relatively small mining companies, the primary requirement was often to get a plant built and operating in a short period of time and at minimal capital cost. Therefore, since the inclusion of both pre-leach and tailings thickeners represents an obvious and significant capital cost, most of these plants were built without thickeners or even detailed, cost-benefit analyses on their inclusion. In some cases, the increasing use of High Rate Thickeners (HRTs) in the mineral processing industries has, however, resulted in a reassessment, because of their considerably lower cost. This reassessment was triggered primarily by the need to conserve water in arid mining areas where borefields are costly to install and water is limited. With the startup and operation of these installations, the resulting significant savings in cyanide consumption has been recognized, in many situations, as a primary justification for the installation of HRTs. Solution balances Degradation of cyanide occurs in the tailings water discharge to slime dams. The degree of degradation (cyanide loss) in the water recovered depends on a number of factors, but it is usually assumed to be about 90%. The most important mechanisms of CN loss are through HCN losses and oxidation by oxygen in the air, which also assists in the hydrolysis of CN. These mechanisms are supported by the large dam surface area and the long retention time of the tailings water in the dam. By thickening the CIP tailings at the plant and recovering as much tailings water as immediately possible, these losses are avoided. The retention time in an HRT is less than three hours, and the surface area is relatively small. Therefore, CN losses are negligible, which is not necessarily the case in conventionally-sized thickeners. Fig. I shows a block diagram of a 100-t/h gold plant without a thickener. In this example, 50% of the tailings water pumped to the tailings dam is recovered, and the CN concentration of the returned water is 10% of the tailings CN concentration of 150 ppm. Fig. 2 represents a plant with an HRT on tailings, thickening to 55% w/w solids. In this case, the thickened tailings are pumped to the dam, and 25% of the contained water is recovered. The recovered water and the mill make-up water are not sent directly to the mill; instead, they are added to the thickener feed and mixed with it prior to thickening. By doing this, the tailings are effectively washed, and the additional cyanide is recovered. Solution balances over these two circuits show cyanide recoveries of 5% and 65%, respectively. Thus, the thickener use increases cyanide recovery by 60%. Fig. 3 shows a two-stage HRT circuit in a countercurrent decantation (CCD) configuration. In this example, an additional 13% of cyanide is recovered through the use of the second-stage unit. This configuration can be justified when residual cyanide levels are high. INTRODUCTION The components of the radiological hazard in uranium mines are: 1. exposure to radon and its short-lived daughters 2. exposure to long-lived ore dust 3. external irradiation With usual ventilation rates, the dosimetry relative to the first component only requires to know each miner&apos;s exposure to a potential energy of short-lived radon daughters, (EXP[a]). Presently in France, (EXP [a]) is determined monthly from radon concentration measurements. This is an indirect method based on ambient monitoring. After technical improvements achieved during the last years it is now possible to measure (EXP [a] directly with an individual portable monitor developed by the CEA. The purpose of this study is to analyze and compare monitoring results obtained by these methods. DETERMINATION OF INDIVIDUAL MONTHLY EXPOSURES TO[ a] POTENTIAL ENERGY OF SHORT-LIVED RADON DAUGHTERS [(EXP a) amb, by ambient monitoring] In this case, it is necessary to know individual monthly exposures to radon, (EXP Rn). As mentioned by Pradel, J.,et al., 1976, (EXP Rn) is estimated for each worker from informations both on radon concentrations (CRn) measured as described by Pradel, J., 1962 and times (t) elapsed in the various workplaces. (EXP Rn) is obtained by summing up the elementary radon exposures CRn x t relative to all the workplaces where the miner was successively engaged during the month. (EXP Rn) values are then corrected by an average equilibrium factor of 0.17 deduced from many measurements and taken as representative for all the French mines. Finally: [ ] (EXP [a])ind are obtained directly with an individual portable instrument described by Chapuis, A.M., et al., 1972. Samples of the air breathed by the miners are pumped continuously and radioactive aerosols containing the short-lived radon daughters are trapped on a filter. The principle of the measurement lies on track-etch detection of the a particles emitted by the deposited material. The technical aspects have been described by Duport, P., et al., 1980a, Mouden, A., et al., 1980 and Chapuis, A.M., 1980. As mentioned by Duport, P., et al., 1980, fiability is good. Indications on measurement reproducibility are given by Pradel, J., et al., 1977. PRESENTATION OF THE STUDY The underground miner population of a French mine was followed during a period of 8 months (October 1978 - May 1979). Monthly exposure values for each miner determined either by ambient or individual monitoring, (EXP [a])amb or (EXP[ a])ind, had been previously collected by Le Gac, J., 1980, who had also defined miner categories according to the characteristics of the work performed. Four groups are considered here, the "whole mine" and three categories as described in table 1. Data on the individual monthly exposures,(EXP [a])ind and (EXP [a])amb, were taken again to analyze the individual cumulative exposures instead of the "month by month" values. The influence of the mean habits of each individual on its surveillance results is thus taken into account. Each miner has a cumulative exposure as an individual of the "whole mine" and as an individual of each category in which he has worked. The results obtained by ambient or individual monitoring were analyzed (characteristics and adjustements of the distributions) and then compared (description of the discrepancies observed). Obviously, such a study is possible only if all the exposures are cumulated on a same duration. As miners have not worked during the same number of months in the course of the data collecting period, one must considerer the individual average monthly exposures, [(EXP a)]amb and[ (EXP a)]ind, instead of the individual cumulative exposures, which is typically the same thing. To be sure that the exposure values of a given group (A, B, C or D) are representative, only those miners really present for at least 3 months in this group were taken into account. Finally, each individual of the "whole mine" and of the various work categories has a pair of average monthly exposure values : [(EXP a)amb] determined by ambient monitoring and[ (EXP a)ind] deduced from the track-etch recording of the individual portable monitor. Newmont Gold Company began full production in early 1995 at the Carlin East underground mine. The mine is currently producing at the rate of 900 tpd (1.000 stpd). for an annual production of 150.000 ounces. Approximately one hundred twenty five hourly and salary personnel are employed at the project for the estimated six-year mine life. Two mining methods are currently utilized at the mine. Longhole stoping with cemented backfill accounts for 70% of the total mine production. Conventional underhand drift and cemented fill accounts for the balance of the daily tonnage. In December 1996. Newmont purchased a new AC-Eickhoff ET-210 Roadheader. The machine went into service in mid-June of 1997 and is used in both longhole stope topcut development and cut and fill production at a rate of 225 tpd (250 stpd). The Carlin East mine is operated by Newmont Gold Company. The mine is one of four underground gold mines in production in northeastern Nevada owned by Newmont Gold.

Early identification of adverse geologic conditions and high stress concentrations during the high-speed extraction of coal using modern mechanized long wall systems is a major challenge facing the mining industry. Often, unexpected geologic anomalies that affect rock mass quality are encountered more rapidly, leaving the mine operator with insufficient time or information to deal with the changing ground conditions. The RockVision3DTM seismic tomographic system, developed by NSA Engineering, Inc., offers substantial benefits over conventional geologic/stress-detection methods and has been used successfully in coal mines worldwide. Tomographic images, produced economically and in near-real time, are easy to interpret, allowing mine operators to deal with potential hazards on a day-to-day basis. This paper focuses on the application of RockVision3DTM technology for delineating hazards caused by stress and geologic anomalies. Case studies for two bump-prone coal mines will be discussed. For example, (1) in a German deep (>1,130 m) mine, the drilling-yield method was used to verify the effectiveness of this technology in detecting stress anomalies ahead of longwalling; (2) in a Polish mine (-500 m deep), two longwall panels 4 km apart were monitored to assess structural and stress-related factors contributing to differences in mining conditions and magnitude of structural failures.

Quaternary alluvial deposits in the modern stream channel of the Agua Fria River west of Phoenix, Arizona, is mined and processed to provide aggregate to the greater Phoenix area. The alluvium occurs as three distinct, stratigraphic layers. Samples from each of the three layers were examined for gravel clast lithology, grain size distribution, and engineering tests, which revealed noteworthy differences in properties. The engineering properties of the gravels are dependent on a combination of factors including gravel clast lithology, the distance the gravel clasts have been transported from their source areas, the age of the deposits, and the degree of weathering. Knowledge of these characteristics may affect the methods that aggregate operators employ to extract and process gravel from the Agua Fria River.

"In underground mining, machine design is predominantly dictated by mine conditions and individual customer desires. In partnership with Foresight Energy, J. H. Fletcher & Company was tasked to design and manufacture a new roof bolting machine with six independent drilling apparatus on board capable of drilling and bolting the roof and ribs with material handling. This is the first Fletcher® six head machine to be designed and built for the mines in the United States. The machine is intended to work in the Illinois Coal Basin. The objective was to produce a machine capable of drilling and installing six bolts simultaneously with a limited number of operators. The goal of the mine is to decrease the time to bolt a cut to improve the safety level of the current roof bolting method, improve efficiency and to improve the bottom line cost of entry development. The customer wanted four mast feed drills at the front of the machine dedicated to installing roof bolts and then another two mast feed drills behind them dedicated to rib bolts. This dictated the requirement of latched controls, which would allow the operator to start drilling one hole and then latch the controls to be able to move on to the next. To accomplish this, we developed a unique hydraulic latch circuit to maintain operator safety and compliance. Guarding was added to prevent an operator from coming into accidental contact with a latched drill. The result of the design is a machine with a single platform and six independent masts with drillheads: four masts strictly for drilling and installing roof bolts on the front of the platform and then two masts on the back of the platform for rib bolts. The feed and rotation controls at each operator’s station include a latch control for drilling. The latch control will allow the operator to lock in the feed and rotation once the hole has been collared, allowing the operator to move on to the next hole. The rear of the machine is equipped with material handling. There are two material pods with winch controls as well as a mesh rack that includes mesh lift, mesh tilt and mesh sump. The six-head roof bolter allows fewer operators to drill and install roof and rib bolts, which in turn lowers the miners’ roof exposure per cut. This design reduces the operator’s exposure from the inherent pinch points and rotary hazards once he has engaged the latch drilling. Therefore the machine will help to decrease the time to bolt a cut, improving productivity while enhancing the ability to operate the machine safely. INTRODUCTION When mining underground it is necessary to support the roof quickly to prevent it from collapsing after an area has been mined. Since the 1950s, the primary method for supporting the roof has been the installation of roof bolts. Roof bolting is one of the most basic functions yet one of the dangerous jobs in the underground mining operation (1). The Mine Safety and Health Administration’s (MSHA) Health and Safety Accident Classification injury database showed an average of 660 roof bolter operator accidents per year over a 5-year period (1999-2003) (2). The numbers show that roof bolting is the most hazardous machine-related job in underground mining. It represents 39 percent of all machine-related accidents in underground coal mines. In 1993, the U.S. Bureau of Mines conducted a study that has shown the activity of drilling as the most hazardous job task performed by a roof bolter operator, accounting for 31 percent of all roof bolting injuries (3). Almost half (46%) of these drilling injuries were due to falling rocks or coal from the roof. All of these injuries occurred under supported roof. The majority of these injuries (about 99%) are not caused by a major roof collapse but from falls of smaller rock from the immediate roof. This type of roof failure has also been termed “skin fall.”"

TBM utilization rate is a component of machine performance prediction and critical for developing a reliable estimate of the construction completion time and cost of the tunnel. Estimation of the utilization rate involves identifying and quantifying the time dedicated to each activity in a tunneling jobsite. Ring building is a common activity in TBM tunneling and takes a large amount of tunnel excavation time when using shielded TBMs. This paper concentrates on the effect of tunnel diameter on the duration of downtimes related to ring building and examines the impact of machine diameter on downtimes. For this purpose, the dependency of segment installation time to segment configuration, and also the diameter of the tunnel was investigated. Furthermore, the ring building time for four tunnels with different sizes of diameters and a variety of ring arrangement were analyzed and the relationship between TBM diameter and segment installation time and consequently TBM utilization was evaluated.

The Pardee Chemical Feed Shaft Project involves construction of two new chemical feed shafts to improve raw water treatment capabilities at the East Bay Municipal Utility District’s (EBMUD) Pardee Reservoir in Valley Springs, California. The project includes construction of two 250-foot deep, 16-inch diameter stainless steel chemical feed shafts that penetrate an existing 8-foot diameter concrete lined conveyance tunnel. The design approach required the shafts be installed using top-down drilling methods and within a strict verticality tolerance (0.1%). Innovative precision drilling techniques ensured that the shafts penetrated the tunnel in their planned locations and the installed chemical feed lines would function as designed. Challenges included extremely limited access to the tunnel because of a requirement to maintain the tunnel in service during construction. This paper discusses the overall project’s design and construction approach, including unanticipated conditions that the project team faced and how these conditions were managed and overcome through collaboration between the contractor, designer, and owner.

INTRODUCTION This chapter outlines some general principles and procedures that a rock mechanics engineer can use to arrive at a logical design for the size, shape, and spacings of underground openings in various types of com¬petent rock formations. Rock formations can be clas¬sified into six major categories which are useful for design purposes, provided the necessary input data are available regarding the geology of the site, the physical properties of the rock, the in-situ stress field in the rock, and the general geometry of the underground openings. The US Bureau of Mines (USBM) developed a rock classification system based upon these concepts and found it to be useful for design purposes (Obert, et al., 1960; Obert and Duvall, 1967). This classification sys¬tem is given in Table 1; however, an extra column has been added to the original classification to indicate the appropriate design techniques for each rock class. In this classification system, two main classes of rock are considered: competent rock and incompetent rock. Competent Rock Competent rock is any rock which, because of its mechanical and geological characteristics, is capable of sustaining underground openings without the aid of any structural support except that provided by unmined rock in the form of pillars and side walls, etc. Roof bolts and small random props which support only local loose rock are not considered structural supports. Competent rock is further subdivided into three ad¬ditional categories-massive, laminated, and jointed¬ which are defined as follows: Massive Rock: Massive rock is a rock formation where the spacing between mechanical defects such as joints, partings, or faults is equal to or greater than the critical dimensions of openings, or where the strength of the bond across these mechanical defects is com¬parable to the rock strength. Laminated Rock: Laminated rock is a rock forma¬tion divided by approximately parallel planes of weak¬ness into lamina whose thicknesses are small compared to critical dimensions of openings and where the bond between the lamina is weak compared to the rock strength. Elastic Rock-This is rock whose physical proper¬ties will permit the use of elastic theory to predict strains and displacements from applied stresses. Inelastic Rock-This is rock whose physical proper¬ties are such that strains and displacements are a strong function of time as well as applied stress. Incompetent Rock Incompetent rock is any rock which, because of its mechanical and geologic characteristics, is not capable of sustaining underground openings without the aid of additional structural support in the form of linings, steel sets, or systems of props. This classification system for rock depends not only on the mechanical and geologic characteristics of the rock but on the size of openings and their depth be¬low the surface. For example, many competent rocks at shallow depths for small stress fields could become inelastic rock at greater depths and larger stress fields, or competent massive rock for small openings could be¬come laminated or jointed rock for larger openings. Thus, to classify rock for design purposes requires a knowledge of the geologic properties of the rock mass, the physical properties of the rock, the general size of the openings to be mined, and the average range of the in-situ stress field to be encountered. To design underground openings and structures in rock, it is necessary to use criteria of failure for rock under various stress conditions, and to use safety factors to allow for differences between laboratory and in-situ strength properties of rock and to account for errors in¬troduced by the various assumptions made in the design process. Criteria of failure based on maximum stresses have been found to give reliable results in practice. Thus, it is assumed that rock will fail in tension when the tensile stress exceeds the tensile strength of the rock as determined by a standard modulus of rupture test on samples of rock core. If the tensile stresses in rock are small, then it is assumed that the rock will fail at a value of compressive stress equal to the compressive strength of the rock as determined by standard uniaxial compres¬sive strength tests on samples of rock core. Discussion by R.W. Hutchinson Minter&apos;s excellent recent paper (1990) on the great paleoplacer gold deposits of the Witwatersrand and its subsequent discussion (Cheney, 1991) and reply (Minter, 1991) provide an informative, interesting and up-to-date review of the geology of these unique ores and some long enigmatic questions regarding their origin. Clearly summarized by Minter (1990) are the compelling sedimentologic and stratigraphic relationships indicating that the Central Rand Group conglomerates are ancient placers and documenting the fluvio-deltaic conditions of their transport and deposition. As Minter (1991) indicates, Cheney&apos;s discussion broadens Minter&apos;s 1990 coverage of the subject by considering higher and lower stratigraphic units and by adding detailed comments concerning recently published information about metamorphic and age relationships. Cheney&apos;s discussion and Minter&apos;s reply also readdress the old controversy of epigenetic hydrothermal vs. syndepositional placer origin for these deposits. This controversy has been resurrected by: • recent recognition of hydrothermally-altered granites (HAGS) in the older Archean basement rocks that lie unconformably beneath the Dominion Group rocks north and west of the Witwatersrand depository; • recognition of greenschist metamorphic assemblages in the Witwatersrand strata; and • the presence of interpreted epigenetic mineral assemblages and textures in some of the deposits. These relationships are briefly discussed by Cheney who suggests they may result from later, superimposed metamorphism and deformation and do not prove that the gold was introduced by epigenetic/metamorphogenic processes. In his paper, Minter (1990) cited the 1986 abstract of a subsequently published paper by Hutchinson and Viljoen (1988) who also considered many of these aspects and attempted to integrate them all in abroad genetic hypothesis. In addition, they emphasized the additional important problem of determining the source for the Witwatersrand gold. Although this issue was partially addressed by Reimer (1984), Mossman and Dyer (1985) and Reimer and Mossman (1990), the question has been generally under emphasized in earlier work and was, therefore, not stressed by Minter and Cheney. Reimer (1985) also emphasized the importance of weathering of the basin&apos;s Archean hinterland and its affects on Witwatersrand sedimentation. Since the epigenetic hydrothermal hypothesis was essentially discarded in favor of placerist origin, the gold source for these great deposits has generally been attributed to the weathering of lodes in greenstones or granites of the uplifted, older Archean hinterland to the north and west. However, Hutchinson & Viljoen (1988) present evidence that this explanation is inadequate, as well as data in support of an alternative explanation. They suggest that the gold and accompanying abundant, clearly detrital and auriferous pyrite in the conglomerate reefs were derived by erosional degradation of auriferous-pyritic exhalite previously deposited, by shallow marine discharge of hydrothermal fluids, along the fault-bound northwestern margins of the Witwatersrand depository. The auriferous-pyritic exhalites were generated by fluid-rock reactions in the subsurface, particularly in the highly-altered lavas of the Dominion Group but also in adjacent basement granitic rocks. The process envisaged is comparable to that observed in sea floor hydrothermal systems today, although, in this case, it occurred at the margin of a rapidly subsiding, shallow marine or continental basin. Important evidence for this explanation is the anomalous gold content of the ferruginous, shaley strata occurring throughout the Witwatersrand Supergroup and including the distinctive contorted bed that is an iron formation. These layers have been interpreted (Hutchinson and Viljoen, 1988 and 1990) to represent the more distal, finer-grained, mixed chemical/clastic sedimentary strata. Thus, they are the basinal equivalents of the proximal, auriferous-pyritic exhalites that were deposited along the basin&apos;s margin and subsequently reworked by sedimentary processes. This gold source explanation resolves many of the enigmatic questions regarding Witwatersrand geology and the reawakened controversy between proponents of hydrothermal and placerist genetic hypotheses. The theory invokes a hydrothermal origin for the gold in the basin-margin pyritic exhalites, thus explaining the visible hydrothermal characteristics (Figs. 1 and 2). It also provides compelling evidence for placer origin by encompassing all of the fluvio-deltaic processes of clastic sedimentary transport and deposition that are clearly summarized by Minter. These characteristics result from the degradational reworking of the pyritic exhalites along the depository&apos;s margins. Fluvial reworking, transport and deltaic redeposition of this detritus, along with additional detritus carried into the depository from the hinterland, then formed the conglomerate reefs. Other enigmatic aspects of Witwatersrand geology include: • the rarity of magnetite and ilmenite (the two most common heavy minerals in placer deposits) and the contrary abundance of leucoxene in the conglomerate reefs; • the presence of differing varieties of pyrite (authigenic, allogenic, compact and porous) all of which are auriferous; and • the presence of well-rounded pebbles, even cobbles, of pyrite that, in view of the brittleness of the mineral, could not have been transported great distances from a hinterland.

With the price of precious metals at an 18-year low, every ounce of metal produced is important. The theft of metals from mining and refining sites can mean the diffrence between profit and loss for many mining companies. Low metal prices do not reduce the potential for the theft of precious metals. History has shown that the price of gold has little to do with the desire for employees to steal precious or base metals. There is actually evidence that the theft of precious metals increases when the price of this commodity goes down. Several of the major precious metal thefts in the past year took place at silver mines when the price of silver was less than 16 cents/g ($5/oz). How does the lowest gold price in 18 years affect the need for security at precious metals properties? There is no short answer to this question. One reason is because the exposure to theft of precious metals is unique to each property. This makes it important that each property be evaluated individually. More than 95% of all precious metals thefts can be attributed to those working at the mine site. So preventing employee theft is the primary concern. One consideration is the location of the property. Gold selling at any price is still an attractive commodity in countries where the employees are making between US$400 and US$600 a month. It is not uncommon for employees at mines in countries where low wages are the norm to consider the value of a gram or two of gold to be a significant amount of money. A gram or two of gold a day may not seem like much. But if 15 employees steal two grams a day, that equates to a significant amount of money during a year. The type of property where the precious metals product is being recovered is also important. For example, a property with a gravity circuit is more likely to suffer from the theft of gold product than a property where all gold is finely disseminated and the only gold seen in the ore body is through a microscope. Gravity circuits increase an operation&apos;s exposure to theft because the grinding circuit that is associated with a gravity circuit often becomes a giant concentrator. Areas such as the bottom of grinding-mill pump boxes, cyclone-feed-pump clean out traps and the sumps often become locations where precious metals concentrate (Figs. 1 and 2). Muck concentrations in these locations can be as high as 25% to 40% of gold or silver. Not long ago, muck was removed from a barren-solution sump at a Merrill Crowe circuit that had concentrated to more than 40% gold. At a milling site in the Pacific Rim, residents of the community adjacent to the mine learned about the value of the concentrates in the sump under the ball mill and committed an armed rob¬bery. While several of their co-conspirators held the em¬ployees at bay with machetes, the others emptied the contents of the sump into buckets and removed it from the site. Armed robbery is not as common as employee theft. However, while this article was being written, an armed robbery occurred at a gold property in Central America. Armed perpetrators took as hostages the night shift employees at a process plant and used cutting torches that were on site to cut into the high-security and gold-storage areas. The perpetrators then stole a company vehicle to remove the stolen gold buttons and sludge from the site. Unfortunately, this type of activity goes on regularly. But managements of most mining companies are reluctant to discuss theft scenarios. So information pertaining to the theft of precious metals seldom becomes a newsworthy item. An audit conducted at a mine site with a gravity circuit recommended that the gravity recovery area be shut down until adequate protection could be provided. Although it was not connected with the audit, it was necessary to shut down the gravity area for a pro¬longed period because of problems with the gravity table. In the two months that followed, gold production at the site increased by about 31 kg/month (1,000 oz/month). It is difficult to attribute all of this increase to the theft of concentrates. But there was a good chance that at least part of the increase was due to the fact that concentrates were being stolen from the gravity area.

Daily everyone depends on the great variety of glass products, so much so that glass is often taken for granted. In fact most people do not realize how versatile glass has become. Consider the various uses and then try to imagine a day in which we are not influenced by glass. Common uses include container ware, table ware, window glass, lead crystal, automobile glass, and fiber glass. Several less common, but important, uses include laboratory ware, pharmaceutical, TV bulbs, light bulbs, glass ceramics, optical glass, fiber optics, and laser glass. Corning, Inc., a leader in specialty products, uses nearly 1 000 different compositions to manufacture about 60 000 different products (Edwards and Copley, 1977). Glass is such a complex product that-definitions vary and exceptions can be found for most definitions. Glass is an inorganic amorphous (non-crystalline) solid. Most glasses are produced by melting of a mixture of oxide raw materials, and then cooled to room temperature. Soda-lime-silica composition.s account for about 90% of all glasses melted (Anon, 1973). The properties of the glass product come mainly from its chemical composition. All of the different glasses require melting a combination of raw materials and forming the molten material into the desired shape. Both the melting and the forming processes use sophisticated technology and these technologies require experts to manage these production systems. The manufacturing process is continuous and takes place in tonnage quantities, so adjustments in the batch to achieve the desired finished product requires a great deal of expertise. Raw materials are fed to the batch mixing area in very large quantities (tons in most cases). As a result, impurities in the range of 0.1% result in addition of that impurity within the molten glass in kilogram amounts. More than twenty different industrial minerals are consumed in the manufacture of various kinds of glass (O&apos;Driscoll, 1990). This chapter describes the major and minor ingredients of the various glass batches. It discusses the roles of the various oxides in the glass batch and most importantly considers the mineral raw materials which supply the glass industry. Each of the raw materials is described in detail in other chapters so the geology and mineralogy sections are kept brief here. Container glass, by far, accounts for the most production; followed by flat glass, fiber glass, and specialty glass of which table ware accounts for the greatest tonnage. [Table 1] shows the general production data for 1987 through 1990. Statistics for many of the uses do not appear because production volumes are small compared to the major uses. The glass industry is organized in four categories: containers, flat glass, fiber glass and specialty glass. The US Department of Commerce, Bureau of Census, publishes production data about the glass industry in three different categories: 1) glass containers, 2) consumer, scientific, technical and industrial glassware, and 3) flat glass. The Bureau has very complete statistics about the glass industry in these three categories but they report production data in different units according to industry standards. Therefore, [Table 1] gives the production data in dissimilar units. The production of most glass articles follows similar steps. The raw materials are mixed and the resulting batch is fed into the furnace. In soda-lime-silica glasses melting begins between 600 and 900°C. At these temperatures CO, and other gasses are released which create bubbles in the molten glass. To remove the bubbles and insure complete melting the temperature is raised to between 1 500 and 1 600°C. This is the melting-refining stage during which the refining agents in the glass batch serve to aid in the release of gas bubbles, homogenize the melt, and prevent the formation of scum on the surface of the molten liquid. At the conclusion of the melting-refining stage the glass is too fluid for working and the melt is cooled to about 1 100°C to attain the proper viscosity for working and forming to begin. After the glass article has been made, it must undergo annealing (slowly and uniformly reheated and cooled) to remove thermal stresses that were created during the forming process.

Traditional or artisanal goldmining, also known as small scale goldmining, has a strong and probably a negative environmental impact. The processing methods applied are very frequently a source of severe pollution due to the emissions of mercury by the extraction of gold by means of amalgamation as well as the emissions of cyanide through cyanide leaching of gold bearing ores. The emissions find their way into the environment and contaminate soils, sediments, water and atmosphere. Abnormal concentrations of mercury and cyanides in waterways are known to occur year after year destroying irreplaceable regions of the world. Mercury and cyanide compounds are highly toxic and may directly create permanent damage to the whole ecosystem. Existing methods for recycling of mercury and for decontamination of mercury and cyanide contaminated tailings are not customary applied in small scale mining and are ineffective as well. Based on investigations of traditional and small size goldmining, this paper presents: -processing methods of gold and discarded tailings under consideration of environmental protection; -figures on actual situation; -recommendations for equipment; -some decontamination methods for mercury and residual cyanide. Mineral Processing methods in traditional gold mining Gold is usually existing in its ores as the metal alloyed with metallic silver and perhaps copper. The element may occur in the form of: -native gold -inclusions also of microns or submicroscopic size metal sulfides (auriferous) such as pyrite, pyrrhotite, stibnite, arsenopyrite and galena -combined as telluride or sulphotelluride. The separation process selected depends on whether the gold can be freed from its unfavorable associations (e.g. gangue) at a sufficiently coarse grain-size, or whether it is carried in a heavy sulfide which can be freed similarly. The usual practice is to concentrate the goldbearing mineral at a relatively coarse grain-size and to regrind the ore if necessary. The gold content is concentrated by secondary or tertiary gravital methods or is extracted by chemical methods (amalgamation, cyanidation etc.) Gold, even when of fine grain-size, settle readily due to its high specific gravity from pulps in which the main gangue mineral is quartz or silicates. Amalgamation is the process of separating gold and silver from their associated minerals by binding (entrapping) them into a mixture with mercury. The cyanide process is applied to separate gold or gold-bearing compounds by dissolution from the finely ground ore (CIP, CIL, RIP), or as heap leaching. The dissolved gold is separated from the solids and the metal-rich or pregnant solution is then treated to recover its gold. Gold is also recovered by flotation methods. This process is widely used in treating base metal ores and in separating various sulfide components of ores, as well as in removing the barren gangue. The gold usually associates with a specific product in a sequence of flotation operations and is recovered subsequently in the smelting of the sulfide concentrates and refining of the metallic products, or by cyanidation of the roasted concentrates. Froth-flotation can be applied to separate gold and sulfide minerals from a finely ground pulp. The Amalgamation Process Amalgamation is the main method for the recovery of gold in traditional mining and is applied for the extraction of gold from placers as well as primary ores. The mineral technology used depends on the nature of ore deposits. In winning gold from solid ore, the matrix of minerals and rocks must be crushed and ground to sufficient fineness to liberate the gold. The liberated gold could be treated similar as free gold from placers. Gold is mainly separated from the valueless gangue (barren rock) by utilizing the difference between the density of the impure native metal (density about 16-19) and the gangue (density about 2.5). In simple operations the material is carried by a stream of water down a sluice generally equipped with small transverse barriers (riffles) against which the gold collects. The riffled sluice is the principal device used by artisanal gold miners. Nowadays, spirals as well as centrifuges, such as Knelson separator or Falcon separator, are occasionally applied for gold recovery. Gold may also be recovered from the pulp, by passing it over corduroycovered tables that catch the heavier particles - a method maybe as ancient as gold mining itself. Prior to 1995, the smelting process at the Kennecott Utah Copper Corporation (KUCC) smelter consisted of Noranda reactors and Pierce-Smith converters. In the consideration of expansion opportunities, the existing process did not lend itself to a program geared towards processing the total output of KUCC mining and milling operations. During 1989 and 1990, a number of smelting options were evaluated. The final selection was the Outokumpu flash smelting process, coupled with the new Kennecott-Outokumpu flash converting technology. Facilities were designed and installed to process 1.0 million tonnes per year of copper concentrate, with the ability to expand to 1.2 million tonnes per year. Following the shutdown of the smelter facility utilizing the Noranda technology in May 1995, the flash smelting furnace commenced operation in June with the flash converting furnace startup following a few weeks later. The smelter has achieved design production targets and environmental goals. Current operating rates are at or above design rates for concentrate and matte processing. Emissions are lower than the permit levels and equate to less than 3.5 kg of S02 per tonne of copper. This paper outlines the design, construction, startup and the first two years of operation of the modernized KUCC smelter.

"Tunnel professionals should inform project stakeholders on the reality that tunnel projects are not 100 percent devoid of the need for excavations or public inconveniences during both design and construction phases. Competing stakeholder objectives to limit these disruptions must be weighed against the need to mitigate risks and to produce a constructible, operable and maintainable end product. Perhaps the approach we should take with stakeholders, also known as Authorities Having Jurisdiction (AHJs), is sharing memorable case studies that bring to life the potential risks so often buried within lengthy risk registers. This paper provides a sobering case study describing what can occur if the presence and/or location of utilities and other obstructions are not fully understood due to restrictions placed on exploration or mining operations. In this instance, the Microtunnel Tunnel Boring Machine (MTBM) encountered a buried concrete structure, followed by rows of abandoned wood piles, before finally colliding with an active high-pressure gas main under a bustling coastal highway. Multiple stakeholders imposed tight logistical and other work restrictions, compounding the difficulties experienced during the drive. However, the difficulties were effectively managed through the project team’s proactive collaboration and communication with everyone involved. INTRODUCTION Some of the most important decisions that influence the ultimate success or failure of a project may not be made by design engineers. Instead, these decisions are often made by project stakeholders and third-party agencies. Tasked with protecting the best interests of their users, these agencies are cautious of potential impacts associated with project construction and subsequent use. Unfortunately, actions required (or restricted) by stakeholders often result in unwanted ripple effects that negatively affect the very users they are trying to protect. For example, a mandate limiting a shaft site to an unreasonable size may be intended to reduce local business impacts, but may actually increase impacts as more supply and truck hauling is required."

A review of the literature reveals that methods for simulating the coupled behavior of size reduction and liberation are limited to binary mineral systems containing one composite particle class. The aim of this paper is to describe the development of a mathematical model to simulate such behavior for binary and ternary systems containing multiple classes of composite particles. A modified Levenberg-Marquardt algorithm and a finite difference Jacobian are used in conjunction with the developed model and a set of constrained equations to estimate all kinetic parameters. This estimation scheme is validated for a binary ore (sphalerite-dolomite) with five composite particle classes using experimental data.

Prior to 1971, there were several clinical trials to evaluate programs for early detection of lung cancer. Among these, the Philadelphia Pulmonary Neoplasm Research Project,(3) the Veterans Administration Study published by Lilienfeld,(7) and the controlled trial of the Kaiser Foundation Health Plan showed an overall five year survival rate of 8% for newly detected cases (the same as the national statistic for unscreened patients). In 1971, the National Cancer Institute initiated three randomized, controlled mortality studies using lung cancer screening of persons at high risk (male smokers over 45 years old). The studies are being conducted at the Johns Hopkins University Hospital, the Mayo Clinic, and the Memorial Sloan-Kettering Cancer Center. The studies have slightly different designs in the combination of sputum cytology and chest x-rays. At the Mayo Clinic the study group is offered screening with sputum cytology and chest x-rays every four months, whereas the control group is advised to have an x-ray and cytology every year. No reminders are sent, and it is believed that only about 20% of the control group is screened. At Johns Hopkins and Memorial, both experimental and control groups are offered annual chest x-rays. The experimental group is additionally offered sputum cytology every four months.(5) At present all of the programs show that screening can detect cancers that are undetectible by other means. However, at this time mortality rates in the control and experimental groups are not significantly different in any of the three studies. OUR PROGRAM Our clinic is located in Grants, New Mexico and we provide most of the pre-employment physical examinations for the mines operating in the Grants area (Kerr McGee Nuclear, Homestake Mining, United Nuclear, Western Nuclear, and Ranchers). In the examinations, we obtain the previous mining history of the worker, a chest x-ray, a sample of sputum for cytological examination, and a blood sample. We also provide routine annual physical examinations of the workers, with special interest in the detection of bronchogenic carcinoma. In the early seventies, we did not have a definite surveillance program. We did not know whether we should have a program like the one started at Memorial or like the one started at the Mayo Clinic. After long consideration, we decided to have a program that does not demand a sputum cytology and chest x-ray every four months, but that allows as many chest x-rays and sputum cytologies as needed to diagnose lung cancer as early as possible. We believe that, if a screening method for cancer is to be optimally effective, it must detect the process at stages early enough for curative therapy. We order a test depending on the age of the miner, the race, the mining history, the smoking history, the radiation exposure levels, and the results of the previous chest x-ray and sputum cytology. With the help of the computer, we have a list of all the miners who should be watched closely because of age, race, mining history, smoking history, radiation exposure, etc. Examination of the miners is performed at our clinic, where all the records are kept. The sputum is collected there but examined in Grand Junction, Colorado, by Dr. Geno Saccomanno. There are two ways to collect sputum. The best way is to collect three consecutive morning samples. For this, we need the cooperation of the miners. They have to follow these instructions and mail the bottle containing the sample to Grand Junction. "Instructions for obtaining a good cough specimen" The enclosed plastic bottle contains a preservative solution, so do not empty out the liquid in it. When you go to bed, place the plastic bottle at your bedside where it will be handy in the morning. When you first get up in the morning (before breakfast) try to cough up some "phlegm" from deep in your chest, and spit it into the liquid in the bottle. Try coughing several times. If you have difficulty coughing, try inhaling deeply the steam from a teakettle (or home-type inhalator). Keep the amount of saliva (ordinary spit) that you put into the bottle along with the cough specimen as small as possible. Do not collect the "phlegm" or mucous that comes from the back of your nose. Put the cap back on the bottle, and shake it vigorously for two minutes. If the amount of material you have coughed up is quite small, then keep the bottle at your bedside for three or four days, and each morning try to add another cough specimen. This method does not give as good a sputum sample as the previous one, but we do get a sputum sample for every miner.
