Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage individuals within the mining companies to work together to identify such indicators, executing risk assessments comes with challenges. Specifically, varying or inaccurate perceptions of risk, in addition to trust and buy-in of a risk management system, contribute to inconsistent levels of participation in risk programs. This paper focuses on one trona mine’s experience in the development and implementation of a field-level risk assessment program to help its organization understand and manage risk to an acceptable level. Through a transformational process of ongoing leadership development, support and communication, Solvay Green River fostered a culture grounded in risk assessment, safety interactions and hazard correction. The application of consistent risk assessment tools was critical to create a participatory workforce that not only talks about safety but actively identifies factors that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s previous process of risk-assessment implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks, as well as a variety of mini case examples that showcase how the organization worked through these barriers to facilitate the identification of leading indicators to ultimately reduce incidents.

A case history of design, prediction, probe pile driving, and construction of a pile foundation for Building 3 of Waterside at Bay Beach Condominiums located in Fort Myers Beach, southwest Florida, is presented. Site geometry constraints, the presence of a 12.0 ft (3.6 m) deep lake occupying the southern one-third portion of the proposed building footprint, and marginal subsoils provided specific challenges for design and construction of the foundation. The subsoil conditions were explored by performing Standard Penetration Test (SPT) borings and a Piezocone Penetration Test (PCPT) Sounding to depths of 100 ft (30.5 m) within the building footprint. Additionally, borings were performed to envelope the building. Deep foundation alternatives included driven concrete, monotube, and steel pipe pile as well as drilled shaft. Following an evaluation of high structural loads, site-specific constraints, foundation test data from buildings 1 and 2, and preference expressed by the project owner, a pre-cast prestressed concrete (PPC) pile-supported foundation system was selected as an economical and effective foundation approach. Use of an augered cast-in-place (ACIP) pile foundation system was precluded by the project structural engineer. This paper deals with the design aspects and methodologies utilized in choosing and economizing the foundation system. The building foundation was designed for an allowable pile capacity of 40 tons (353 kN) in compression, 1.5 tons (12 kN) in lateral capacity for a deflection of 0.25-in (0.64 cm) and 15 tons (133 kN) in uplift capacity. For the single story extended garage structure, a pile capacity of 27 tons (238 kN) was indicated in compression. As part of the design process, an extensive probe pile driving program was carried out in order to evaluate the pile driving equipment, determine and verify pile capacities, and optimize the production pile lengths. A total of 33, 14-in (356 mm) square, PPC concrete piles were driven within the entire building limits, and dynamically load tested utilizing Pile Driving Analyzer (PDA) as part of the probe pile driving program. Utilization of PDA during probe pile driving helped in predicting pile set-up (i.e., increase in pile resistance with time) as well as reducing the pile lengths by as much as 20 percent. It resulted in variable pile lengths during the production phase. This information was effectively utilized as part of the overall quality control program to install a total of 559 PPC piles (including 33 probe piles) within the Building 3 footprint, including the reclaimed lake area, to depths ranging from 45 to 58 ft (14 to 18 m) below existing ground elevation as part of the foundation support system. Prior to installation of probe or production piles, a termination criterion was established using 1-dimensional wave equation analyses in addition to the design pile embedment requirement. The predicted pile capacities compared reasonably well with the capacities from the CAPWAP analyses for probe piles. This engineered, monitored and tested foundation solution provided a cost effective and satisfactory basis for completion and approval of the foundation work for the building resulted in cost savings over other foundation alternatives for the owner. The building has since been occupied and a recent reconnaissance has indicated that the building foundations are performing satisfactorily.

"An overview of various physical and interfacial characterization methods for use in mineral processing is presented in this paper. The availability of a large number of characterization techniques and commercial instruments requires that the reliability and limitation of each be carefully examined. The choice of a method may depend upon the manner in which the information is to be used, for example in development of new processes, plant and process design, plant control, problem solving, etc. IntroductionThe behavior of particulate systems, in mineral processing and other applications, is primarily affected by the physical characteristics (size, shape, surface area, pore structure) of the particles and the nature of the different solid-fluid interfaces present. Mineral processing operations in general are either designed to modify and/or control physical and interfacial properties (grinding, classification, agglomeration, etc.) or are strongly influenced by them (flotation, flocculation, solid/liquid separation, etc.). Characterization of physical and interfacial properties should be an integral part of the development of new processes, plant and process design, and routine plant operation.Our purpose in preparing this article has been to present an overview of the current status of characterization, to point out areas of weakness, and to discuss the reliability and limitations of the various techniques, procedures, and instrumentation available. For more detailed discussion of methods, etc., the reader is referred to standard references.Sampling particulate solids.Sampling is a critical factor in any procedure for characterizing particulate materials — the reliability of characterization data is ultimately limited by the extent to which the sample used was truly represen¬tative of the population from which it was taken.Sampling from particulate solids involves two basic problems: (1) the selection of a random', unbiased sample and (2) choice of the appropriate sample size.The problem of sample selection involves the design of a sample collection procedure that ensures that every kind of particle in the population has the proper chance of being included in the sample without bias. "The tungsten (WO3)-rich ores from King Island Scheelite (KIS) Mine, Australia, containing scheelite(CaWO4) were previously recovered by gravity separation and flotation of fine scheelite (1978-92) with an overall recovery of 76% when both circuits were operating. Recently extensive test work has been conducted both in China and in Australia on KIS ores, assaying from 0.50%-1.34%WO3, and tailings assaying 0.2%WO3(GZRINM 2009; 2011a; 2011b). A new approach, which aimed at improving both recovery and grade of concentrate by means of an innovative all flotation route WOF (Whole Ore Flotation), achieved excellent results: 90%-92% recovery to concentrate grade of 66%-70%WO3for the ores. The process is characterized by the use of selective reagents in flotation, including a newly developed collector for scheelite, GYWA. The WOF process also embraces the "Improved Petrov Method" in the cleaning of scheelite rougher concentrate. Earlier investigations found that the removal of sulfides by flotation prior to scheelite flotation was not necessary. It was also found that recycling of process water did not influenceresults. Two main ore types were tested as well as tailings from 2009to 2001(0.20%WO3). In addition a flow sheet using wet high gradient magnetic separation (WHGMS) in tandem with flotation was also investigated for the tailings."

The Liaoning Dahuofang Reservoir Water Transfer Project is an important development for the Department of Water Resources of Liaoning Province in northeastern China. The 85.3 km water conduit will transport water from the Hun River downstream of the Huanren Reservoir to the outlet on the Suzi River leading to the Dahuofang Reservoir. The additional water is required to sustain the growing cities of Fushun, Shen-yang, Liaoyang, Anshan, Panjin and Yingkou. The project is divided into seven contracts. Three TBM contracts comprise almost 75% of the work and the others are drill and shoot. The focus of this paper is about the creditable TBM performances achieved and some of the problems encountered by Chinese contractors; notably two having had no previous TBM experience.

"The Aurora North mine has been operating for more than ten years, and is the source of over half of Syncrude’s bitumen production. The hydrotransport systems at Aurora experience high rates of wear, and mission lengths on current slurry pumps are nominally 1,500-2,000 hours. Recent work has enabled a better understanding of the significant adverse effect of large particle impingement damage to impeller vane leading edges - which is the known life determining issue for Aurora hydrotransport pumps. A new pump design has been developed and installed at Aurora, incorporating significantly increased impeller diameter and pumping vane lengths. The new pumps are expected to triple Aurora’s previous mission length for hydrotransport pumps. Part II of this paper will share actual hydraulic and wear performance observed in the first several months of operation, and draw comparisons relative to what was expected for these pumps in design / development.INTRODUCTION Slurry Pumps are Critical Equipment at Syncrude Slurry pumps are a key component in Syncrude’s operations. In a typical Aurora application, each 5000 HP pump stage helps move over 8,000 dry tonnes per hour of oil sand, plus an equal volume of process water that has been added to create the slurry. The average impeller life in oil sand hydrotransport service at Aurora has been nominally 1,500 to 2,000 hours, or 12 to 16 million tonnes of oil sand pumped. Generally, the slurry pump application at Aurora has proven very difficult to improve due to the unique combination of high sand abrasivity, 125 mm (5”) large particle top size (lumps), and high flow rate relative to basic pump size; a factor that results in unduly high impeller vane leading edge speeds for the service conditions."

The Precambrian Eastern Creek Volcanics comprises spilitic basalts with interbedded sediments and tuffs which were extruded and deposited during Middle Proterozoic time. The rocks have undergone low-grade regional metamorphism.The Eastern Creek Volcanics are approximately 17,000 ft thick and have been divided into four units.The rocks were laid down in a geosynclinal basin elongate to the north. The main structural features are the northerly trending Mount Isa and Gorge Creek-Quilalar fault systems.Major east-west faults cause repetition of the sequence.Copper and uranium mineralization is known within the formation. Copper mineralization is confined to quartz-filled shears anp basalts. Nothing of economic importance has been found in the area though ore deposits have been found elsewhere in this type of environment.

IN the study of the structure or the properties of a series of alloys or nonmetallic compounds, it is often advantageous to express the com-position not as percentage by weight, according to which the material is always made up and analyzed, but as the atomic percentage of the two constituents. The conversion from the one to the other is repre-sented by the formulas: 100A 100x Y x 100 ? x/A A [1] A+ B x + A/B(100-x) X _ 100yA 100y [2] yA+(100-y)B or y+A(100-y) where x and y are the weight and atomic percentages respectively of the element of atomic weight A, and (100 - x) and (100 - y) are the per-centages by weight and by atoms respectively of the element of atomic weight B. While calculations according to these formulas are simple, they are' time consuming if several conversions have to be made. In 1931 Water-man' constructed a nomograph for performing the conversion, but this did not permit an accuracy greater than about 1 per cent to be obtained in the neighborhood of 50 per cent. Olander2 has proposed a graphic method simpler than Waterman's and one that gives a uniform precision throughout the entire scale. The possible accuracy is about 0.5 per cent of the whole, depending on the size and accuracy of the coordinate paper used. Recently, Hermann3 has described a slide rule for making the calculations. For many years line-of-sight (LOS) issues for underground mobile equipment is a growing focus of research. This research is a result of the numerous fatalities and injuries which occur in the mining industry and which are related to poor operator LOS. Three assessment methods are currently used to assess operator LOS of underground mobile equipment. The light filament (LF) method is a hands-on assessment method that is performed in the field. This method is not evaluated in this paper. The laser scan (LS) method of assessing equipment is a quick and reliable method that comes from the need to evaluate vehicles already located in the field. The computer simulation (CS) method can assess LOS issues using a computer aided drawing (CAD) model, which is useful for assessing current or prototype models. The purpose of the current research is to compare and validate different operator LOS assessment methods. Comparison of the results of the LS and CS visibility plots yielded similar quantifiable results. A visual comparison of the results further illustrate that the LS and CS methods are acceptable assessment tools for mobile equipment operator LOS evaluation. The visibility assessment methods are now being prepared as guidelines for the mining industry to assess current and potential designs.

Detailed structural studies in southeast Arizona have successively revealed (1) the local attitudes of individual fractures (with lateral and/or vertical displacements), (2) the patterns exhibited by groups of fractures, and (3) the trends and directions shown by fracture groups or zones on a regional scale. The results of the studies have been twofold: First, domes were locally determined from these fracture arrangements, even where other criteria failed; and, second, fundamental breaks brought out the breadth of the Texas lineament in southeast Arizona as well as major northwest- and northeast-trending fracture zones. Coincidence of one or several domes with the major intersections of fundamental breaks can be centers of important mineralization, as in the instances of Bisbee, Ajo and Morenci. THE OVER-ALL STRUCTURE OF ARIZONA A northwest-southeast trending anticlinorium has been suggested as the structural setting of Arizona. Starting in Triassic and culminating in late Cretaceous times, this deformation resulted from stresses of considerable magnitude with varying ratios of com pres sional and uplifting forces. However, the series of parallel flexures could just as well have been caused by isostatic movements involving dilation under tension with only incidental compression, as later emphasized by dike filling and block faulting (Fig. 1). The anticlinorial deformation, which is also suggested by the fact that the Pre-cambrian rocks to the southwest are all concealed by sediments, could be somewhat reminiscent of the conditions that affected the classical swarm of parallel dikes of wager.' ANALYSlS OF DATA As a first step in this analysis, all structural information obtained on such features as fractures and their relative displacements, faults, dikes, anticlines, synclines, and Precambrian and Laramide-Tertiary intrusives were plotted on maps along with structural trends inferred from aerial photographic mosaics. Next, the faults in southwest Arizona were classified according to time of occurrence and relative importance. The fault assemblages were then examined as to geometric or spatial arrangement and correlated with the occurrences of domes and existing Finally, the conjunction of fracture intersections and domes was analyzed and compared with locations of known ore deposits. Differentiation of Faults According to Time and Relative Importance: The faults encountered were grouped as follows: (1) Basin and Range faults, latest in time; (2) secondary faults; (3) primary faults; and (4) lineaments, earliest in time by inference. Quality control is essential for augered cast-in-place (ACIP) piles (also known as continuous flight auger (CFA) piles outside the United States) and drilled displacement (DD) piles since the pile is constructed below the ground surface out of the direct sight of the personnel constructing and inspecting the pile. ACIP piles are being successfully constructed to support high-rise structures, hospitals, arenas and stadiums, mass transit systems, and power and industrial facilities. The use of DD pile technology is also gaining ground in the United States, typically utilizing proprietary systems such as Omega (currently utilized by Richard Goettle Inc., and LG Barcus and Sons, Inc.), Berkel, and DeWal (currently utilized by Morris-Shea). Different types of ACIP/DD pile installation equipment are shown in Figures 1 and 2. ACIP piles are installed into granular and cohesive soils, and soft rock to lengths currently approaching 130 ft (39.5 m), with diameters up to 24 to 36 inches (610 to 915 mm), and sustaining design loads approaching 900 kips (4000 kN). Current DD pile installation equipment in the United States is capable of installing up to 79-ft-long (24 m) piles with diameters ranging from 12 to 18 inches (305 mm to 460 mm).

Britain's first pressurised water reactor (PWR) Nuclear Power Station is now under construction at Sizewell on the Suffolk coast. A linear cut-off wall was proposed to ensure that the drawdown of water to some 14m below sea level in the overlying sands during construction of the new power station would not affect the existing adjacent nuclear power station. The low permeability cut-off wall was constructed by diaphragm walling techniques by excavating a continuous deep trench supported by bentonite mud and then dis­placing the bentonite by a plastic concrete to form the "impermeable" membrane. The paper describes the automated digging apparatus ("Hydrofraise") with the extensive support facilities required to achieve completion within programme, control methods used to obtain the high vertical tolerances and the properties of the plastic concrete and the production plant used for its production.

An important problem in the efficient conduct of industry is that of helping the individual employee to formulate an attitude toward his work which will result in the reduction of accidents and the promotion of safety. Perhaps nowhere is the need greater for this type of Psychological training of carefully selected men than at oil refineries whore both the raw materials and the finished products are highly inflammable and explosive in nature. However, the refinery organization usually is well adapted to educational safety activities - much more so than in the producing, pipe line, and marketing departments where the personnel are more or less scattered; but the same general principles of conduct hold true in any industrial organization. This problem of directing the attention of the employees to their specific tasks or helping them to control and frequently eliminate hazards engendered by adverse mental attitude or emotion is met most satisfactorily by carefully selecting applicants and placing them at jobs for which their ability, best fits them. The subsequent training of these carefully selected employees to assure their most effective work in accident prevention mu be accomplished in a variety of ways, such as by personal supervision and instruction, inspections, drills, and by the studied use of well thought out safety pamphlets and posters. This procedure is simple in outline and not original in scope, yet the importance of selecting and training the personnel has, not been generally accepted by the petroleum industry until recent years. In 1926, the Bureau of Mines, while making a study of accidents in the petroleum industry, found that medical examinations of prospective employees were not receiving unanimous endorsement by all companies, and the point was stressed that the selection and allocation of employees was a problem requiring serious consideration in the petroleum industry.

Electric utility response to certain amendments of the Clean Air Act has resulted in a production of several types of alkaline coal combustion byproducts. Alkaline combustion byproducts are gaining increased usage for acid mine drainage mitigation and abatement as research leads to a better understanding of their beneficial applications. Since January of 1997, Mettiki Coal Corporation has been injecting alkaline flue gas desulphurization byproducts from Virginia Electric?s Mt. Storm Unit #3 wet limestone scrubber into abandoned portions of the active Mettiki mine. This paper provides an overview of the key design, transportation, regulatory, and environmental issues faced in the project.

"Current interest in the distribution of mercury in the natural environment comes from quite different, though not unrelated, sources. Mercury has long been an important metal in many industries and has, therefore, been much sought after by mining interests. The presence of mercury traces in the various media of the natural environment, namely soil, rock, air, water and vegetation, is exploited by geochemists not only to find mercury and polymetallic ore deposits, but also to find most other types of economic metal mineralization. More recently, however, mercury has caught the attention of health authorities and environmental scientists who are becoming increasingly concerned over the hazards of mercury wastes and the danger these present to the environment and to man. It is now apparent that the trace quantities of mercury, considered so important and helpful to exploration geologists, can be concentrated in the biosphere to such levels as to present considerable problems for ecologists. This article summarizes most of the available data on mercury contents in materials of geological interest and presents average normal values for rocks, ores, minerals, soils, sediments, waters and plants. The geoch.emical cycle for mercury in nature is presented and the mechanisms of mercury migration around the cycle are discussed."

In 1801, Hatchett discovered the oxide of a new element while he was examining a heavy black mineral from Haddam, America, which had been presented to the British Museum. He named the mineral Columbite and the metal Columbium. A year later a Swedish investigator, Eckeberg, while working with a new mineral resembling columbite-yttrotantalite discovered a new element, to which he gave the name of tantalum.In 1844, Rose announced the existence of two new elements in a sample of columbite from Bodenmais. One was similar to Eckeberg's tantalum and the other was called Niobium. Wollaston, in 1909, attempted to show experimentally that columbium and the tantalum were identical, but it was soon proved that they were two separate elements. Subsequently a number of investigatiors found other "new elements" in tantalum and columbium bearing ores, but these were shown to be mixtures of the twin metals.Since Hatchett's Columbium and Rose's Niobium are identical, it is more fitting to use the name assigned to this element by the earlier investigator. However, the term niobium is still retained in Germany and England.The separation of tantalum and columbium has been one of the major problems for analytical chemists since the discovery of the two elements and it is only in recent years that tantalum of a high degree of purity has been produced. High purity columbium metal is probably still to be obtained.Until 1903 tantalum was merely a laboratory curiosity, but about this time its use in incandescent electric light bulb filaments was discovered. Eight years later it was replaced by tungsten, which was better and cheaper. For a time the electrolytic valve action of tantalum was used in the charging of storage batteries, but the chief uses of the metals' to-day are for corrosion resistant apparatus, in vacuum tubes and as carbides in the manufacture of hard cutting and shaping tools.Most of the recent work on the metals has been done by C. W. Balke, in America. Tantalum has been studied more than columbium. OCCURRENCE OF TANTALUM AND COLUMBIUMI. "The Yukon's numerous (185) placer gold mines are still a major social and economic force in this developing economy. Gold recovery at each of the various sluiceboxes is not optimized due to extremely difficult sampling logistics and a shortage of technical expertise and industrial research.At each of six operating placer mines, numerous sluicebox tailings samples were collected with hand held sample cutters and combined into large (2-7 cubic yards) duplicate samples. The entire sample volumes were screened and processed on a shaking table to determine gold losses. "Comminition process costs, which typically represent an important contribution to mining operating costs, can themselves be divided roughly between energy and liner /charge wear. As such, developing the ability of predicting mill wear will allow a more accurate estimation of wear costs and therefore their control. In this paper and based on the possibility of estimating energy dissipated in impact and abrasion by using a discrete element charge motion model for mill charge motion simulation, media wear will be investigated principally as a function of three wear mechanisms : impact, abrasion and corrosion. Experimental results will be used to identify a total media wear model. Model results will be compared with real mill data from three operating Australian mines. These preliminary results show for the cases tested that the overall average error, as compared with the observed cases, is 0 % with a standard deviation of 66.1 %. Results from the Bond abrasion index tests, for the same cases investigated, give an overall error of 73 % and a standard deviation of 192.5 %. In Greenfield situations where the estimation of operating costs is critical in determining project feasibility, the level of confidence in such estimates becomes quite important. With regards to media wear, the Bond abrasion test currently serves as the standard media wear prediction device. However, as shown [l, 21 test results once compared with actual wear rates can vary greatly. Figure 1 shows test results as compared with 15 cases. Average error over this data is 73 % with a standard deviation of 192.5 %. As such, developing the ability of more accurately predicting mill wear will allow greater confidence in wear cost estimation and therefore their control."
